Abstract Machine learning tools have demonstrated viability in visualizing pain accurately using vital sign data; however, it remains uncertain whether incorporating individual patient baselines could enhance accuracy. This study aimed to investigate improving the accuracy by incorporating deviations from baseline patient vital signs and the concurrence of the predicted artificial intelligence values with the probability of critical care pain observation tool (CPOT) ≥ 3 after fentanyl administration. The study included adult patients in intensive care who underwent multiple pain-related assessments. We employed a random forest model, utilizing arterial pressure, heart rate, respiratory rate, gender, age, and Richmond Agitation–Sedation Scale score as explanatory variables. Pain was measured as the probability of CPOT scores of ≥ 3, and subsequently adjusted based on each patient's baseline. The study included 10,299 patients with 117,190 CPOT assessments. Of these, 3.3% had CPOT scores of ≥ 3. The random forest model demonstrated strong accuracy with an area under the receiver operating characteristic curve of 0.903. Patients treated with fentanyl were grouped based on CPOT score improvement. Those with ≥ 1-h of improvement after fentanyl administration had a significantly lower pain index (P = 0.020). Therefore, incorporating deviations from baseline patient vital signs improved the accuracy of pain visualization using machine learning techniques.

<title>Abstract</title>Monitoring the pain intensity in critically ill patients is crucial because intense pain can cause adverse events, including poor survival rates; however, continuous pain evaluation is difficult. Vital signs have traditionally been considered ineffective in pain assessment; nevertheless, the use of machine learning may automate pain assessment using vital signs. This retrospective observational study was performed at a university hospital in Sendai, Japan. Objective pain assessments were performed in eligible patients using the Critical-Care Pain Observation Tool (CPOT). Three machine-learning methods—random forest (RF), support vector machine (SVM), and logistic regression (LR)—were employed to predict pain using parameters, such as vital signs, age group, and sedation levels. Prediction accuracy was calculated as the harmonic mean of sensitivity, specificity, and area under the receiver operating characteristic curve (AUROC). Furthermore, 117,190 CPOT assessments were performed in 11,507 eligible patients (median age: 65 years; 58.0% males). We found that pain prediction was possible with all three machine-learning methods. RF demonstrated the highest AUROC for the test data (RF: 0.853, SVM: 0.823, and LR: 0.787). With this method, pain can be objectively, continuously, and semi-automatically evaluated in critically ill patients.

OBJECTIVE: To identify the outcome of patients with sepsis using high-frequency blood pressure data. MATERIALS AND METHODS: This retrospective observational study was conducted at a university hospital ICU (derivation study) and at two urban hospitals (validation study) with data from adult sepsis patients who visited the centers during the same period. The area under the curve (AUC) of blood pressure falling below threshold was calculated. The predictive 90-day mortality (primary endpoint) area under threshold (AUT) and critical blood pressure were calculated as the maximum area under the curve of the receiver operating characteristic curve (AUCROC) and the threshold minus average AUT (derivation study), respectively. For the validation study, the derived 90-day mortality AUCROC (using critical blood pressure) was compared with Sequential Organ Failure Assessment (SOFA), Simplified Acute Physiology Score (SAPS) II, Acute Physiology and Chronic Health Evaluation (APACHE) II, and APACHE III. RESULTS: Derivation cohort (N = 137): the drop area from the mean blood pressure of 70 mmHg at 24-48 hours most accurately predicted 90-day mortality [critical blood pressure, 67.8 mmHg; AUCROC, 0.763; 95% confidence interval (CI), 0.653-0.890]. Validation cohort (N = 141): the 90-day mortality AUCROC (0.776) compared with the AUCROC for SOFA (0.711), SAPSII (0.771), APACHE II (0.745), and APACHE III (0.710) was not significantly different from the critical blood pressure 67.8 mmHg (P = 0.420). CONCLUSION: High-frequency arterial blood pressure data of the period and extent of blood pressure depression can be useful in predicting the clinical outcomes of patients with sepsis.

BACKGROUND: Veno-venous extracorporeal membrane oxygenation (V-V ECMO) is essential for managing severe respiratory failure; however, hemorrhagic and thrombotic complications remain major challenges. Conventional activated partial thromboplastin time (APTT) monitoring may inadequately reflect the complex coagulation changes during ECMO. Although viscoelastic hemostatic assays (VHAs) provide a comprehensive assessment, discrepancy patterns between VHAs and conventional tests in V-V ECMO patients remain unclear. OBJECTIVES: This study aimed to identify distinct coagulation phenotypes in patients receiving V-V ECMO using hierarchical clustering of VHA and conventional coagulation parameters. METHODS: This retrospective observational study was conducted at a tertiary university hospital in Japan. Patients receiving V-V ECMO between November 2020 and June 2023 who underwent VHA and conventional coagulation testing were included. Coagulation profiles were clustered using hierarchical analysis of five variables. Cluster validity was assessed, and results were visualized using principal component analysis and generalized additive models. RESULTS: A total of 19 patients and 110 VHA measurements were analyzed. Three distinct coagulation phenotypes were identified. Cluster 1 demonstrated concordant prolongation of APTT and citrated kaolin reaction time (CK-R). Clusters 2 and 3 exhibited disproportionately prolonged CK-R relative to APTT, with Cluster 3 further characterized by elevated fibrinogen and platelet counts. Anticoagulant use, laboratory markers, and clinical outcomes, including thrombotic and bleeding events, were evaluated. Complication rates did not differ significantly among clusters. CONCLUSIONS: Hierarchical clustering identified three distinct coagulation profiles among V-V ECMO patients, highlighting the heterogeneity of ECMO-related coagulopathy. Thromboelastography-derived CK-R may complement APTT monitoring to inform more tailored anticoagulation strategies in this population.

Objectives: The spillover impact of the COVID-19 pandemic on patients without COVID-19 in ICUs should be assessed. We aimed to assess the association of ICUs’ medical burden and capacity changes with clinical outcomes in such patients during the COVID-19 pandemic. Design: Retrospective cohort study. Setting: Twenty-three ICUs in Japan. Patients: Patients without COVID-19 in ICUs from January 2019 to February 2023. Interventions: None. Measurements and Main Results: These ICUs completed a web-based questionnaire on medical burden and capacity limitations in November 2023, and they were classified as having a limited capacity if their scores exceeded the median; otherwise, they were categorized as having a maintained capacity. The primary outcome was the standardized mortality ratio (SMR), calculated with the Acute Physiology and Chronic Health Evaluation III-j model, compared with the pre-pandemic level. Using individual-level patient data, a generalized linear Poisson mixed-effects model including an offset-time term was employed to assess the association of capacity limitation, the number of patients with COVID-19, and ICU bed occupancy on the day of admission, with hazard ratios for in-hospital death. Nine and fourteen ICUs had a limited capacity (25,568 patients) and a maintained capacity (45,068 patients), respectively. SMRs increased in four epidemic waves in the ICUs with a limited capacity but in only one wave in those with a maintained capacity. After adjustment, capacity limitation (hazard ratio, 1.19; 95% CI, 1.01–1.41; p = 0.04) and the number of patients with severe COVID-19 (per five patients; hazard ratio, 1.09; 95% CI, 1.03–1.16; p = 0.002) were associated with in-hospital mortality, but ICU bed occupancy was not. Conclusions: SMRs increased more frequently in ICUs with a limited capacity during the pandemic. Our findings emphasize the need for proactive strategies to mitigate medical burden and capacity limitations for future preparedness.

Transcatheter aortic valve implantation (TAVI) is an effective treatment for severe aortic stenosis (AS); however, postoperative delirium (POD) can worsen patient outcomes. This study aimed to examine the risk factors for POD after TAVI, including possible intervening factors. We included 87 patients (mean age: 83) who underwent TAVI between May 2014 and September 2018. POD was defined by the presence or absence of delirium on ICU admission, assessed using the Confusion Assessment Method for the ICU. Factors that showed significant differences in the univariate analysis were analyzed using a multiple logistic regression analysis. In total, 31 patients (36%) had POD after ICU admission, and 56 (64%) did not. The preoperative frailty score and aortic valve opening area (AVA) were significant risk factors for POD. The multivariate analysis also showed that both factors were independent risk factors for POD (area under the receiver operating characteristic curve: 0.805). There were no significant differences in the number of ICU days. However, postoperative hospitalization was significantly longer in the POD group (19 (17–31) days vs. 16 (13–22) days; p = 0.002). POD was associated with a narrow AVA and frailty; this suggests that frailty prevention interventions according to the AVA may be important.

Introduction: Supra-laryngeal mask airway (LMA) is widely accepted as an alternative to the tracheal tube. However, compared to the use of a tracheal tube, it may take longer to identify the many different causes of sudden respiratory distress. In particular, heat and moisture exchange filters are one of the most overlooked causes. Case presentation: The case was that of a 76-year-old male Japanese patient (161.9 cm, 66.5 kg) who underwent an open renal biopsy. He presented with chronic obstructive pulmonary disease, with a Hugh–Jones dyspnea score of 2. The patient did not discontinue smoking prior to the operation. Anesthesia was induced using propofol (100 mg), fentanyl (100 ?g), and remifentanil (0.3 ?g/kg/min). I-gel™ #4 was inserted following neuromuscular blockade with rocuronium (40 mg). Anesthesia was maintained with 3–6% desflurane under positive pressure ventilation. After induction in the left lateral and jackknife positions, the following ventilator settings were used: volume-controlled ventilation with tidal volumes of 450 mL, respiratory rate of 12 breaths per minute, an inspiratory: expiratory ratio of 1:2, and a positive end expiratory pressure of 5 cmH2O. With these settings, the peak inspiratory pressure was 16 cmH2O. Five minutes after initiating the operation, the peak inspiratory pressure steadily increased to 30 cmH2O. Although we administered rocuronium, the peak inspiratory pressure and end-tidal carbon dioxide concentration increased over time. When we disconnected the heat and moisture exchange filter and LMA, we noticed a large quantity of sputa. A suction catheter was passed down the LMA and the sputa was removed, but the LMA was not obstructed. The peak inspiratory pressure continued to increase with tidal volumes of only 20–30 mL. Despite a normal external appearance of the heat and moisture exchange filter, we replaced it with a new one. The ability to ventilate improved immediately and the SpO2 recovered from 92% to 100%. Conclusions

As a proposition for predicting the outcome of sepsis, we pointed out the need for both high sensitivity and specificity and the need to reduce the complexity of collecting predictors due to the large number of tests and inputs associated with the increase in the number of predictors, and proposed the use of big data to solve the proposition. Of the three methods, high-frequency blood pressure data, text data from nursing records, and machine learning, only high-frequency blood pressure data led to the construction of the framework, suggesting the possibility of solving both the problems of high sensitivity and specificity and reducing the complexity of collecting predictors.

I constructed integrated big data in the intensive care unit (ICU) of Tohoku University Hospital, analyzed the relationship between the severity and the cost of each patient in the ICU. I planned to develop the original algorithm to derive the optimal treatments that would achieve optimal medical resource distribution and maximum medical benefits, depending on the changes in each patient's condition of severity. I set up a server to construct panel database. In regards to the patient’s severity, I gathered the ICU system data, and bed-side monitoring data of each patients admitted to our ICU for the past five years. In regards to the cost, I gathered material cost, labor cost, expense data, medical record and DPC/PDPS data with the cooperation of the university hospital medical affairs department. I cleaned and integrated the all above data, transferred to my server, and constructed the panel database. Finally, I calculated departmental costing and cost forecasting with this database.
