    BACKGROUND: An increasing number of women are undergoing breast implantation for cosmetic purposes and for reconstructive purposes after breast excision. The surface morphology of the breast implant is one of the key factors associated with the induction of capsule contraction. The effect of surface morphology on the inflammatory response following implant insertion remains unclear, however. This study conducted comparative analyses to determine the effect of the textured and smooth surface morphology of silicone sheets. METHODS: Each type of silicone sheet was inserted into the subcutaneous pocket below the panniculus carnosus in C57BL/6 mice and mice with genetic disruption of CARD9, Dectin-1, Dectin-2, or Mincle. We also analyzed the collagen fiber capsule thickness, histological findings, and macrophage inflammatory response, including TGF-β synthesis. RESULTS: We found that textured surface morphology contributed to the formation of collagen fiber capsules and the accumulation of fibroblasts and myofibroblasts, and was accompanied by the accumulation of TGF-β-expressing macrophages and foreign-body giant cells. CARD9 deficiency attenuated collagen fiber capsule formation, macrophage responses, and TGF-β synthesis, although the responsible C-type lectin receptors (CLRs) remain to be clarified. CONCLUSIONS: These results suggest that CARD9 may have a strong impact on silicone sheet insertion through the regulation of macrophage responses.

    Optimal conditions for wound healing require a smooth transition from the early stage of inflammation to proliferation, and during this time alternatively activated (M2) macrophages play a central role. Recently, heat-killed lactic acid bacteria (LAB), such as Lactobacillus plantarum (L. plantarum) have been reported as possible modulators affecting the immune responses in wound healing. However, how signaling molecules regulate this process after the administration of heat-killed LAB remains unclear. In this study, we examined the effect of heat-killed L. plantarum KB131 (KB131) administration on wound healing and the contribution of CARD9, which is an essential signaling adaptor molecule for NF-kB activation upon triggering through C-type lectin receptors, in the effects of this bacterium. We analyzed wound closure, histological findings, and inflammatory responses. We found that administration of KB131 accelerated wound closure, re-epithelialization, granulation area, CD31-positive vessels, and α-SMA-positive myofibroblast accumulated area, as well as the local infiltration of leukocytes. In particular, M2 macrophages were increased, in parallel with CCL5 synthesis. The acceleration of wound healing responses by KB131 was canceled in CARD9-knockout mice. These results indicate that the topical administration of KB131 accelerates wound healing, accompanying increased M2 macrophages, which suggests that CARD9 may be involved in these responses.

    C-type lectin receptors (CLRs) recognize microbial polysaccharides. The CLRs Dectin-1 and Dectin-2, which are triggered by β-glucan and α-mannan, respectively, contribute to up-regulation of the inflammatory response. Recently, we demonstrated that activation of the Dectin-2 signal delayed wound healing; in previous studies, triggering the Dectin-1 signal promoted this response. However, the precise roles of these CLRs in skin wound healing remain unclear. This study was conducted to determine the roles of Dectin-1 and Dectin-2 in skin wound healing, with a particular focus on the kinetics of neutrophilic inflammatory response. Full-thickness wounds were created on the backs of C57BL/6 mice, and the effects of Dectin-1 or Dectin-2 deficiency and those of β-glucan or α-mannan administration were examined. We also analyzed wound closure, histological findings, and neutrophilic inflammatory response including neutrophil extracellular trap (NET) formation at the wound sites. We found that Dectin-1 contributed to the acceleration of wound healing by inducing early-phase neutrophil accumulation, whereas Dectin-2 was involved in prolonged neutrophilic responses and NET formation, leading to delayed wound healing. Dectin-2 deficiency also improved collagen deposition and TGF-β1 expression. These results suggest that Dectin-1 and Dectin-2 have different roles in wound healing through their different effects on the neutrophilic response.

    BACKGROUND: In the process of burn wound healing, promoting healing and suppressing wound progression by enhancing blood flow is considered essential. Various agents that increase blood flow are currently being assessed. Batroxobin (DF-521; Defibrase®) is a denitrogenating agent extracted from Bothrops moojeni, used as a thrombin-like serine protease to improve ischemic conditions. However, it remains unclear how this agent affects the burn wound healing process. In this study, we conducted analyses to define the effects of batroxobin administration on burn wound healing. METHODS: Full-thickness burn wounds were created on the dorsal skin of C57BL/6 mice by applying a 90℃, 5-mm-diameter soldering iron for 10 s. Immediately after wounding, batroxobin (30 batroxobin units/kg/mouse) was administered intraperitoneally daily. As a vehicle control, the same volume of saline was administered. We analyzed wound area, histological findings, blood flow, growth factor and chemokine synthesis, and ischemia-reperfusion related factor expression. RESULTS: We found that the systemic administration of batroxobin prevented burn wound progression, which was accompanied by decreased expression of TNF-α and NOX2 and reduced synthesis of Hif-1α. In addition, this agent promoted wound healing by enhancing blood flow, increasing S100A4-positive fibroblast accumulation, and stimulating the production of growth factors (bFGF, EGF, and PlGF). CONCLUSIONS: These results indicate that the systemic administration of batroxobin prevented burn wound progression and accelerated the healing process by enhancing blood flow. (Word count: 222).

    INTRODUCTION: Lactic acid bacteria (LAB) are well known for their beneficial effects on the regulation of immune responses and host protection against microbial infections. We previously reported that heat-killed Enterococcus faecalis strain KH2 (heat-killed KH2), a species of LAB, enhances inflammatory responses at wound sites and accelerates the skin wound healing process. In this study, we aimed to clarify the pathway underlying the wound-healing effects of heat-killed KH2. We focused on CARD9, a common adaptor molecule for C-type lectin receptors and Dectin-2, the upstream receptor for this adaptor molecule. METHODS: Four full-thickness dermal wounds were created on the backs of wild-type (WT) mice, CARD9 KO mice, and Dectin-2 KO mice, and the effects of heat-killed KH2 administration were examined. We analyzed the percent wound closure, re-epithelialization, granulation tissue formation, and the production of inflammatory cytokines and chemokines. RESULTS: Heat-killed KH2 administration enhanced wound closure, granulation tissue formation, and re-epithelialization in WT mice. However, these effects were absent in heat-killed KH2-treated CARD9 KO mice. Similar results were observed in the migration of neutrophils and the production of TNF-α, IL-6, KC, and MIP-2 in heat-killed KH2-treated CARD9 KO mice. Furthermore, heat-killed KH-2 induced activation of reporter cells expressing Dectin-2. Finally, heat-killed KH-2 treatment in Dectin-2 KO mice did not promote skin wound healing. CONCLUSION: These results suggest that recognition of heat-killed KH2 by Dectin-2 may activate CARD9-mediated signaling, which may contribute to the promotion of skin wound healing through KH2 treatment.

    Cryptococcus deneoformans is a yeast-type fungus that causes fatal meningoencephalitis in immunocompromised patients and evades phagocytic cell elimination through an escape mechanism. Memory T (Tm) cells play a central role in preventing the reactivation of this fungal pathogen. Among these cells, tissue-resident memory T (TRM) cells quickly respond to locally invaded pathogens. This study analyzes the kinetics of effector T (Teff) cells and Tm cells in the lungs after cryptococcal infection. Emphasis is placed on the kinetics and cytokine expression of TRM cells in the early phase of infection. CD4+ Tm cells exhibited a rapid increase by day 3, peaked at day 7, and then either maintained their levels or exhibited a slight decrease until day 56. In contrast, CD8+ Tm cells reached their peak on day 3 and thereafter decreased up to day 56 post-infection. These Tm cells were predominantly composed of CD69+ TRM cells and CD69+ CD103+ TRM cells. Disruption of the CARD9 gene resulted in reduced accumulation of these TRM cells and diminished interferon (IFN) -γ expression in TRM cells. TRM cells were derived from T cells with T cell receptors non-specific to ovalbumin in OT-II mice during cryptococcal infection. In addition, TRM cells exhibited varied behavior in different tissues. These results underscore the importance of T cells, which produce IFN-γ in the lungs during the early stage of infection, in providing early protection against cryptococcal infection through CARD9 signaling.

    Insulin is used as a therapeutic agent in patients with diabetes, and cutaneous lipohypertrophy (LH) and localized insulin-derived amyloidosis (LIDA) are well-known adverse effects associated with insulin injections. The clinical implications, management, assessment methods, and pathological differentiation of LH and LIDA have been recently updated. This review was to update our knowledge of the pathological differentiation, effects of insulin absorption, hypoglycemic events, and recent assessment methods for LH and LIDA. A scoping review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta Analyses extension for Scoping Reviews guidelines. Original studies and case reports in English were also included. PubMed and Scopus databases were searched for keywords to identify papers published up to January 2022. A total of 113 studies were identified through a database search, and 31 were eligible for inclusion in this scoping review. In the 31 studies included in this review, patients with type 2 diabetes had high frequencies of LH and LIDA. LH outcome parameters were assessed using pathological findings and imaging. LIDA is mainly determined by pathological methods, such as hematoxylin and eosin and Congo red staining. Several in vitro and in vivo LIDA models of LIDA have been developed. These results suggest that pathological analysis is required to identify LH and LIDA. It is important to consider LIDA, as it likely influences insulin adsorption and glycemic control. Although several studies have evaluated the LIDA process, little is known about the mechanisms underlying the development of adverse effects associated with insulin injections.

    Streptococcus pneumoniae is a major causative bacterium of community-acquired pneumonia. Dendritic cell-associated C-type lectin-2 (dectin-2), one of the C-type lectin receptors (CLRs), was previously reported to play a pivotal role in host defense against pneumococcal infection through regulating phagocytosis by neutrophils while not being involved in neutrophil accumulation. In the present study, to elucidate the possible contribution of other CLRs to neutrophil accumulation, we examined the role of caspase recruitment domain-containing protein 9 (CARD9), a common adaptor molecule for signal transduction triggered by CLRs, in neutrophilic inflammatory response against pneumococcal infection. Wild-type (WT), CARD9 knockout (KO), and dectin-2 KO mice were infected intratracheally with pneumococcus, and the infected lungs were histopathologically analyzed to assess neutrophil accumulation at 24 h postinfection. Bronchoalveolar lavage fluids (BALFs) were collected at the same time point to count the neutrophils and assess the production of inflammatory cytokines and chemokines. Neutrophil accumulation was significantly decreased in CARD9 KO mice, but not in dectin-2 KO mice. Tumor necrosis factor alpha (TNF-α), keratinocyte-derived chemokine (KC), and macrophage inflammatory protein-2 (MIP-2) production in BALFs were also attenuated in CARD9 KO mice, but not in dectin-2 KO mice. Production of TNF-α and KC by alveolar macrophages stimulated with pneumococcal culture supernatants was significantly attenuated in CARD9 KO mice, but not in dectin-2 KO mice, compared to that in each group's respective control mice. In addition, pneumococcus-infected CARD9 KO mice showed larger bacterial burdens in the lungs than did WT mice. These data indicate that CARD9 is required for neutrophil migration after pneumococcal infection, as well as inflammatory cytokine and chemokine production by alveolar macrophages, and suggest that a CLR distinct from dectin-2 may be involved in this response.

    Cryptococcus deneoformans is an opportunistic fungal pathogen that frequently causes fatal meningoencephalitis in patients with impaired cell-mediated immune responses such as AIDS. Caspase-associated recruitment domain 9 (CARD9) plays a critical role in the host defense against cryptococcal infection, suggesting the involvement of one or more C-type lectin receptors (CLRs). In the present study, we analyzed the role of macrophage-inducible C-type lectin (Mincle), one of the CLRs, in the host defense against C. deneoformans infection. Mincle expression in the lungs of wild-type (WT) mice was increased in the early stage of cryptococcal infection in a CARD9-dependent manner. In Mincle gene-disrupted (Mincle KO) mice, the clearance of this fungus, pathological findings, Th1/Th2 response, and antimicrobial peptide production in the infected lungs were nearly comparable to those in WT mice. However, the production of interleukin-22 (IL-22), tumor necrosis factor alpha (TNF-α), and IL-6 and the expression of AhR were significantly decreased in the lungs of Mincle KO mice compared to those of WT mice. In in vitro experiments, TNF-α production by bone marrow-derived dendritic cells was significantly decreased in Mincle KO mice. In addition, the disrupted lysates of C. deneoformans, but not those of whole yeast cells, activated Mincle-triggered signaling in an assay with a nuclear factor of activated T cells (NFAT)-green fluorescent protein (GFP) reporter cells expressing this receptor. These results suggest that Mincle may be involved in the production of Th22-related cytokines at the early stage of cryptococcal infection, although its role may be limited in the host defense against infection with C. deneoformans.

    IL-17A is a proinflammatory cytokine produced by many types of innate immune cells and Th17 cells and is involved in the elimination of extracellularly growing microorganisms, yet the role of this cytokine in the host defense against intracellularly growing microorganisms is not well known. Cryptococcus deneoformans is an opportunistic intracellular growth fungal pathogen that frequently causes fatal meningoencephalitis in patients with impaired immune responses. In the current study, we analyzed the role of IL-17A in the host defense against C. deneoformans infection. IL-17A was quickly produced by γδT cells at an innate immune phase in infected lungs. In IL-17A gene-disrupted mice, clearance of this fungal pathogen and the host immune response mediated by Th1 cells were significantly accelerated in infected lungs compared with wild-type mice. Similarly, killing of this fungus and production of inducible NO synthase and TNF-α were significantly enhanced in IL-17A gene-disrupted mice. In addition, elimination of this fungal pathogen, Th1 response, and expression of IL-12Rβ2 and IFN-γ in NK and NKT cells were significantly suppressed by treatment with rIL-17A. The production of IL-12p40 and TNF-α from bone marrow-derived dendritic cells stimulated with C. deneoformans was significantly suppressed by rIL-17A. In addition, rIL-17A attenuated Th1 cell differentiation in splenocytes from transgenic mice highly expressing TCR for mannoprotein 98, a cryptococcal Ag, upon stimulation with recombinant mannoprotein 98. These data suggest that IL-17A may be involved in the negative regulation of the local host defense against C. deneoformans infection through suppression of the Th1 response.

    INTRODUCTION: The enhanced type 2 helper (Th2) immune response is responsible for the pathogenesis of allergic asthma. To suppress the enhanced Th2 immune response, activation of the Th1 immune response has been an alternative strategy for anti-asthma therapy. In this context, effective Th1-inducing adjuvants that inhibit the development of allergic asthma but do not flare the side effects of the primary agent are required in clinical treatment and preventive medicine. OBJECTIVE: In this study, we aimed to determine the regulation of the Th2 type immune response in asthma by a novel immunostimulatory oligodeoxynucleotide (ODN) derived from Cryptococcus neoformans, termed ODN112, which contains a cytosine-guanine (CG) sequence but not canonical CpG motifs. METHODS: Using an ovalbumin-induced asthma mouse model, we assessed the effect of ODN112 on prototypical asthma-related features in the lung and on the Th1/Th2 profile in the lymph nodes and lung of mice treated with ODN112 during sensitization. RESULTS AND CONCLUSION: ODN112 treatment attenuated asthma features in mice. In the bronchial lymph nodes of the lungs and in the spleen, ODN112 increased interferon-γ production and attenuated Th2 recall responses. In dendritic cells (DCs) after allergen sensitization, ODN112 enhanced cluster of differentiation (CD) 40 and CD80 expression but did not alter CD86 expression. Interleukin-12p40 production from DCs was also increased in a Th2-polarizing condition. Our results suggest that ODN112 is a potential Th1-inducing adjuvant during Th2 cell differentiation in the sensitization phase.

    Interferon (IFN)-γ is mainly secreted by CD4+ T helper 1 (Th1), natural killer (NK) and NKT cells after skin injury. Although IFN-γ is well known regarding its inhibitory effects on collagen synthesis by fibroblasts in vitro, information is limited regarding its role in wound healing in vivo. In the present study, we analyzed how the defect of IFN-γ affects wound healing. Full-thickness wounds were created on the backs of wild type (WT) C57BL/6 and IFN-γ-deficient (KO) mice. We analyzed the percent wound closure, wound breaking strength, accumulation of leukocytes, and expression levels of COL1A1, COL3A1, and matrix metalloproteinases (MMPs). IFN-γKO mice exhibited significant attenuation in wound closure on Day 10 and wound breaking strength on Day 14 after wound creation, characteristics that are associated with prolonged neutrophil accumulation. Expression levels of COL1A1 and COL3A1 mRNA were lower in IFN-γKO than in WT mice, whereas expression levels of MMP-2 (gelatinase) mRNA were significantly greater in IFN-γKO than in WT mice. Moreover, under neutropenic conditions created with anti-Gr-1 monoclonal antibodies, wound closure in IFN-γKO mice was recovered through low MMP-2 expression levels. These results suggest that IFN-γ may be involved in the proliferation and maturation stages of wound healing through the regulation of neutrophilic inflammatory responses.

    The physiological functions of TNF receptor-associated factor 5 (TRAF5) in the skin inflammation and wound healing process are not well characterized. We found that Traf5-/- mice exhibited an accelerated skin wound healing as compared with wild-type counterparts. The augmented wound closure in Traf5-/- mice was associated with a massive accumulation of plasmacytoid dendritic cells (pDCs) into skin wounds and an enhanced expression of genes related to wound repair at skin sites. In accordance with this result, adoptive transfer of Traf5-/- pDCs, but not wild-type pDCs, into the injured skin area in wild-type recipient mice significantly promoted skin wound healing. The expression of skin-tropic chemokine receptor CXCR3 was significantly upregulated in Traf5-/- pDCs, and treatment with a CXCR3 inhibitor cancelled the promoted wound healing in Traf5-/- mice, suggesting a pivotal role of CXCR3 in pDC-dependent wound healing. Traf5-/- pDCs displayed significantly higher expression of IFN regulatory factor 5 (IRF5), which correlated with greater induction of proinflammatory cytokine genes and CXCR3 protein after stimulation with TLR ligands. Consistently, transduction of exogeneous TRAF5 in Traf5-/- pDCs normalized the levels of abnormally elevated proinflammatory molecules, including IRF5 and CXCR3. Furthermore, knockdown of IRF5 also rescued the abnormal phenotypes of Traf5-/- pDCs. Therefore, the higher expression and induction of IRF5 in Traf5-/- pDCs causes proinflammatory and skin-tropic characteristics of the pDCs, which may accelerate skin wound healing responses. Collectively, our results uncover a novel role of TRAF5 in skin wound healing that is mediated by IRF5-dependent function of pDCs.

    Acute respiratory distress syndrome (ARDS) is a pathological condition that involves diffuse lung injury and severe hypoxemia caused by pulmonary and systemic diseases. We have established a mouse model of severe ARDS, developed by intratracheal injection of α-galactosylceramide (α-GalCer), an activator of natural killer T (NKT) cells, followed by LPS. In the present study, we used this model to investigate the regulatory mechanism in the early inflammatory response during acute lung injury. In α-GalCer/LPS-treated mice, the number of CD4+ CD25+ Foxp3+ regulatory T (Treg) cells and the expression of a Treg cell-tropic chemokine, secondary lymphoid-tissue chemokine (SLC), in the lungs was significantly lower than in mice treated with LPS alone. Giving recombinant (r)SLC increased the number of Treg cells in α-GalCer/LPS-treated mice. Treatment with anti-IFN-γ mAb enhanced the expression of SLC and the accumulation of Treg cells in the lungs of α-GalCer/LPS-treated mice, whereas giving recombinant (r)IFN-γ reduced the number of Treg cells in mice treated with LPS alone. IL-10 production was significantly lower in α-GalCer/LPS-treated mice than in mice treated with LPS alone. Giving rIL-10 prolonged survival and attenuated lung injury as a result of reduced production of inflammatory cytokines (such as IL-1β, IL-6, TNF-α, and IFN-γ) and chemokines (including MCP-1, RANTES, IP-10, Mig, MIP-2, and KC) in α-GalCer/LPS-treated mice. Treatment with anti-IFN-γ mAb enhanced IL-10 production in α-GalCer/LPS-treated mice. These results suggest that the attenuated accumulation of Treg cells may be involved in the development of severe ARDS through a reduction in the synthesis of IL-10.

    Dectin-2 is a C-type lectin receptor that recognizes high mannose polysaccharides. Cryptococcus neoformans, a yeast-form fungal pathogen, is rich in polysaccharides in its cell wall and capsule. In the present study, we analyzed the role of Dectin-2 in the host defense against C. neoformans infection. In Dectin-2 gene-disrupted (knockout) (Dectin-2KO) mice, the clearance of this fungus and the inflammatory response, as shown by histological analysis and accumulation of leukocytes in infected lungs, were comparable to those in wild-type (WT) mice. The production of type 2 helper T (Th2) cytokines in lungs was higher in Dectin-2KO mice than in WT mice after infection, whereas there was no difference in the levels of production of Th1, Th17, and proinflammatory cytokines between these mice. Mucin production was significantly increased in Dectin-2KO mice, and this increase was reversed by administration of anti-interleukin 4 (IL-4) monoclonal antibody (MAb). The levels of expression of β1-defensin, cathelicidin, surfactant protein A (Sp-A), and Sp-D in infected lungs were comparable between these mice. In in vitro experiments, IL-12p40 and tumor necrosis factor alpha (TNF-α) production and expression of CD86 and major histocompatibility complex (MHC) class II by bone marrow-derived dendritic cells and alveolar macrophages were completely abrogated in Dectin-2KO mice. Finally, the disrupted lysates of C. neoformans, but not of whole yeast cells, activated Dectin-2-triggered signaling in an assay with nuclear factor of activated T cells (NFAT)-green fluorescent protein (GFP) reporter cells expressing this receptor. These results suggest that Dectin-2 may oppose the Th2 response and IL-4-dependent mucin production in the lungs after infection with C. neoformans, and it may not be required for the production of Th1, Th17, and proinflammatory cytokines or for clearance of this fungal pathogen.

    Caspase recruitment domain-containing protein 9 (CARD9) is an adaptor molecule signal that is critical for NF-κB activation and is triggered through C-type lectin receptors (CLRs), which are pattern recognition receptors that recognize carbohydrate structures. Previous studies have reported that Cryptococcus neoformans, a fungal pathogen that causes meningoencephalitis in AIDS patients, is recognized through some CLRs, such as mannose receptors or DC-SIGN. However, the role of CARD9 in the host defense against cryptococcal infection remains to be elucidated. In the present study, we analyzed the role of CARD9 in the host defense against pulmonary infection with C. neoformans. CARD9 gene-disrupted (knockout [KO]) mice were highly susceptible to this infection, as shown by the reduced fungal clearance in the infected lungs of CARD9 KO mice, compared to that in wild-type (WT) mice. Gamma interferon (IFN-γ) production was strongly reduced in CARD9 KO mice during the innate-immunity phase of infection. Reduced IFN-γ synthesis was due to impaired accumulation of NK and memory phenotype T cells, which are major sources of IFN-γ innate-immunity-phase production; a reduction in the accumulation of these cells was correlated with reduced CCL4, CCL5, CXCL9, and CXCL10 synthesis. However, differentiation of Th17 cells, but not of Th1 cells, was impaired at the adaptive-immunity phase in CARD9 KO mice compared to WT mice, although there was no significant difference in the infection susceptibility between interleukin 17A (IL-17A) KO and WT mice. These results suggest that CARD9 KO mice are susceptible to C. neoformans infection probably due to the reduced accumulation of IFN-γ-expressing NK and memory phenotype T cells at the early stage of infection.

    Cryptococcus neoformans is an opportunistic fungal pathogen that causes meningoencephalitis in immunocompromised patients. Recently, we reported that Toll-like receptor 9 (TLR9) is involved in host defense against C. neoformans: specifically, it detects the pathogen's DNA. In the present study, we aimed to elucidate the mechanisms underlying TLR9-mediated activation of innate immune responses by using the URA5 gene, which encodes a virulent component of this fungal pathogen. A PCR-amplified 345-bp URA5 gene fragment induced interleukin-12 p40 (IL-12p40) production by bone marrow-derived dendritic cells (BM-DCs) in a TLR9-dependent manner. Similar activity was detected in the 5' 129-bp DNA fragment of URA5 and in a synthesized oligodeoxynucleotide (ODN) with the same sequence. Shorter ODN fragments, which contained GTCGGT or GACGAT but had only 24 or 21 bases, induced IL-12p40 production and CD40 expression by BM-DCs, but this activity vanished when the CG sequence was replaced by GC or when a phosphorothioate modification was introduced. IL-12p40 production caused by active ODN was strikingly enhanced by treatment with DOTAP, a cationic lipid that increases the uptake of DNA by BM-DCs, though DOTAP failed to induce IL-12p40 production by inactive ODN and did not affect the activity of an ODN-containing canonical CpG motif. There was no apparent difference in intracellular trafficking between active and inactive ODNs. Finally, an extremely high dose of inactive ODN suppressed IL-12p40 production by BM-DCs that had been stimulated with active ODN. These results suggest that the C. neoformans URA5 gene activates BM-DCs through a TLR9-mediated signaling pathway, using a mechanism possibly independent of the canonical CpG motif.

    本年度は急性創傷におけるナノ型乳酸菌の効果の検証と熱傷創モデルの確立を中心に研究を進めた。 急性創傷におけるナノ型乳酸菌の効果に関しては、直接投与の濃度を検証し、さらに初回単回投与での創閉鎖率の改善を認めた。加えて、病理学的解析においても、CD31陽性血管数やαSMA陽性筋繊維芽細胞数の増加を認め、創傷治癒に関わる肉芽の増生を認めた。上皮化に関しても、上皮の進展率の上昇を認め、増生した肉芽上に皮膚が進展し、治癒が促進していることを確認できた。 急性期において炎症の調節に必要なサイトカインに関してELISA法での解析を行ったところ、IL-6やTNF-αの早期での上昇を認めた。加えて、好中球の遊走に関与するCXCL2やCXCL1なども著明な上昇を認めた。これらは、乳酸菌を急性創傷に投与することで創部における炎症が惹起され、創傷治癒過程における炎症期のメカニズムと合致する。炎症を惹起することで創の清浄化が促進する可能性が示唆された。さらに、抗炎症に作用するI L-10も後期にて上昇を認めた。炎症期から増殖期への移行を適切にかつ早期に誘導することで、創傷治癒を早めている可能性がある。今後、フローサイトメトリーで創部に遊走される白血球の分画を解明し、創傷治癒過程の変化を観察する予定である。 熱傷創モデルに関しては、マウス背部に電気ごてで作成した熱傷創の治癒過程及び病理学的変化を解析した。熱傷創が治癒する過程と、温度による変化、保護剤による違いなどを検討している。今後、標準的な治癒過程を明らかにし、ナノ型乳酸菌投与による変化を比較検討していく予定である。

    本研究はサケ白子由来抽出物プロタミンを使用した創傷治癒に関する解析である。プロタミンは抗凝固薬であるヘパリンの拮抗薬やインスリンの作用持続薬として実臨床で使用されており、今回さらなる新規治療薬としての効果を解析することが目的である。in vitroの解析ではマウスから抽出した骨髄由来樹状細胞を培養し、プロタミンで刺激する実験を行った。その結果、IL-6やTNF-α、IL-12p40などの炎症性サイトカインの上昇を認めた。創傷治癒において早期の炎症性サイトカインの上昇は白血球の遊走を促進し、創の清浄化を早め治癒を促進すると言われている。今後は、より後期のフェーズで炎症が低下し、創傷治癒における増殖期へのスムーズな移行がなされているかをフローサイトメトリーなどで解析する必要がある。また、in vivoの解析において、マウスへのプロタミン投与と至適濃度の解析を行った。非固着性ガーゼに塩酸プロタミンを浸漬させ乾燥させた疑似的な創傷被覆材を作成し、マウス背部に3mmパンチで作成した全層皮膚欠損に貼付、連日交換のうえDay3で創閉鎖率を解析した。結果、1.25mg/mlの溶液で作成した擬似的被覆材において創閉鎖率の改善を認めた。in vitroの解析で高濃度での細胞毒性の可能性が指摘されていたため、本実験で示された濃度を至適濃度として今後解析を進めていく必要がある。今後、さらに被覆材に準じた形で交換の頻度や解析日、さらに創サイズの変更や感染創、糖尿病マウスでの解析などを検討している。

    乳癌術後の乳房再建術において、組織拡張期(TE)や人工乳房(SBI)を用いた手術が広く行われているが、これらのシリコン性の人工物周囲には、長期的な被膜拘縮を高頻度で生じ、さらに近年ブレストインプラント関連未分化大細胞型リンパ腫(BIA-ALCL)の発声が報告されている。また、被膜拘縮は表面が平滑なスムースタイプのシリコンで多いのに対し、BIA-ALCLは表面に微細な凹凸のあるテクスチャードタイプのシリコンで多いとされており、シリコンの表面形状による反応の違いが指摘されているが、これらの原因は全く解明されておらず、深刻な合併症に対する対策は立てようがない。 申請者らはこれまで乳房再建において43臨床検体のテクスチャードタイプのシリコン周囲被膜を採取し、驚くべきことに51%の検体でバイオフィルムが検出されるという結果を示した。 本研究では、これまで本邦で主流であったテクスチャードタイプのシリコンから、BIA-ALCLを契機に使用が開始されたスムースタイプのシリコン周囲の被膜検体を同様に検討することで、炎症遷延化の原因の一つと考えられているバイオフィルム形成がシリコンの表面形状の違いにどのように関与するかを解析することを目的としている。 以上よりバイオフィルムによるシリコンインプラントへの影響について基盤情報を獲得し、さらに被膜の肥厚化・拘縮に対してはどのような生体反応が関与しているのかを明らかにする。

  13. 慢性創傷に対する「好中球細胞外トラップ」阻害によるユニバーサル創傷ケアの開発

    慢性創傷は、バイオフィルム形成、血流障害などが複合的に関与し、難治化する。しかし、複数の原因に対し、同時にアプローチできる創傷ケアは存在しない。難治性の原因によらず、全ての創傷を治癒に導く「ユニバーサル創傷ケア」が求められている。 本研究では、既に突発性難聴や慢性閉塞症などの治療薬として用いられ、血流改善効果や血栓を形成する好中球細胞外トラップ阻害効果が確認されているデフィブラーゼが創傷治癒に与える影響を解明することにより、ユニバーサル創傷ケアの確立を目指す。 急性創傷として、マウス背側皮膚に直径6 mmの全層欠損層を4個作成し、生理食塩水(生食)またはデフィブラーゼ 30 BU/kgを腹腔内に投与した後、創閉鎖率、再上皮化率、肉芽面積、血管新生の解析を行った。さらに、慢性創傷モデルとして、我々が確立したα-mannan投与モデルを用い、創作成後、生食またはデフィブラーゼ30 BU/kgを腹腔内に投与し、創閉鎖率の解析を行った。 急性創傷モデルでは、デフィブラーゼ投与により、創作成後3, 5, 7日目の創閉鎖率が有意に増加した。デフィブラーゼ投与による再上皮化率には差は認められなかったが、肉芽面積は創作成後7日目に有意に増加した。さらに、デフィブラーゼ投与により創作成後5日目の血管数が有意に増加した。 慢性創傷モデルもデフィブラーゼ投与により、創作成後5日目に創閉鎖率が生食投与群と比較し、有意に増加した。

    本課題において申請者らは、褥瘡や糖尿病性下腿潰瘍など難治性皮膚潰瘍におけるBiofilm形成への免疫と細菌間クロストークの関与について解明し、宿主由来のBiofilm誘導因子をターゲットとした新規治療法の確立を目指している。特に、好中球がBiofilm誘導に関与する可能性に注目している。創傷治癒において、好中球は最も早く創部に到達し、初期の感染防御や炎症反応に重要な役割を担う。その一方でBiofilmを伴う皮膚潰瘍では、好中球による組織障害が報告されており、好中球の暴走が慢性的感染症のトリガーではないかと指摘されている。 我々は昨年度、SDラットを用いて皮膚潰瘍緑膿菌Biofilm形成モデルを確立し、①皮膚潰瘍への緑膿菌接種により、24時間をピークに好中球集積数が高まり、その後低下すること、②好中球集積には、緑膿菌クオラムセンシング分子が関与することを明らかにした。 今年度は、緑膿菌クオラムセンシング分子の合成に関わるlasI遺伝子を欠損した緑膿菌とその親株であるPAO1を用いて、lasI遺伝子が好中球集積に関わるか否かについて検証を行った。PAO1株と比較してlasI遺伝子を欠損した緑膿菌株では、創部への好中球集積が約半数に減少した。また、好中球の誘導に関わるサイトカインであるTNF-αは約3分の1の産生量に減少した。次にlasI遺伝子が産生に関わるクオラムセンシング分子である3-oxo-C12-HSLを創傷部位に投与したところ、TNF-α産生を伴い、創傷部位に大量の好中球集積を認めた。 以上のことから、緑膿菌クオラムセンシング分子3-oxo-C12-HSLは、好中球の遊走、TNF-α産生の増加に関与することが明らかになった。今後、好中球と緑膿菌クオラムセンシング分子のクロストークについて解析を進める予定である。

    C-type lectin receptors (CLRs) recognize microbial polysaccharides. The precise roles of these CLRs in wound healing remain unclear. This study was conducted to determine the roles of Dectin-1 and Dectin-2 in wound healing, with a particular focus on the kinetics of neutrophilic inflammatory response. Wounds were created on the backs of C57BL/6 mice, and the effects of Dectin-1 or Dectin-2 deficiency. We also analyzed wound closure, histological findings, and neutrophilic inflammatory response, including neutrophil extracellular trap formation. We found that Dectin-1 contributed to the acceleration of wound healing by inducing early-phase neutrophil accumulation, whereas Dectin-2 was involved in prolonged neutrophilic responses and neutrophil extracellular trap formation, leading to delayed wound healing. These results suggest that Dectin-1 and Dectin-2 have different roles in wound healing through their different effects on the neutrophilic response.

    The purpose of our study is to develop an “innovative skin wound care using nano-type lactic acid bacteria (heat-killed KH2).” We analyzed percent wound closure and granulation area. We found that heat-killed KH2 contributed to the acceleration of the formation of granulation tissue by inducing tumor necrosis factor-α, interleukin-6, basic fibroblast growth factor, transforming growth factor (TGF)-β1 production. In addition, heat-killed KH2 also improved wound closure, which was accompanied by the increased production of TGF-β1 in diabetic mice. Topical administration of heat-killed KH2 might have therapeutic potential for the treatment of chronic skin wounds in diabetes mellitus. In the present study, we concluded that heat-killed KH2 promoted skin wound-healing through the formation of granulation tissues and the production of inflammatory cytokines and growth factors.

    Interferon (IFN)-γ is mainly secreted by CD4+ T helper 1 (Th1), natural killer (NK) and NKT cells after skin injury. Although IFN-γ is well known regarding its inhibitory effects on collagen synthesis by fibroblasts in vitro,information is limited regarding its role in wound healing in vivo.In the present study, we analyzed how the defect of IFN-γ affects wound healing. Full-thickness wounds were created on the backs of wild type (WT) C57BL/6 and IFN-γ-deficient (KO) mice. We analyzed the percent wound closure, accumulation of leukocytes, and matrix metalloproteinases (MMPs). IFN-γKO mice exhibited significant attenuation in wound closure on Day 10, characteristics that are associated with prolonged neutrophil accumulation. Expression levels of MMP-2 mRNA were significantly greater in IFN-γKO than in WT. Moreover, under neutropenic conditions created with anti-Gr-1 monoclonal antibodies, wound closure in IFN-γKO mice was recovered through low MMP-2 expression levels.

    本研究は、深部損傷褥瘡(Deep Tissue Pressure Injury: DTPI)の炎症誘導機序の解明とDTPI発生初期およびDTPIの治癒調整・促進の適切な看護ケア技術の開発を行うことを目的としている。これまでの好中球の枯渇実験により、好中球の適正な時期の活性がない場合は、ケモカイン(KC)が持続的に増加し、炎症性サイトカイン(MCP-1,IL-1β、TNF)などの増加活性の遅延をもたらしており、炎症期が遷延することが明らかになった。そこで、今年度は好中球の適正な時期の活性を促進する刺激として、臨床で一般的に用いられる冷・温罨法を実験群に施した。その結果、冷・温罨法を施していない対照の圧迫創は、3日目以降に、IL-1β、TNF-α、IL-17Aが増加を示した。冷罨法はIL-1β、TNF-α、IL-17A、IL-10いずれも創傷作製後1日目にピークを迎えその後数値が低下するのに対し、温罨法は3日目、7日目に数値が上昇することが明らかになった。好中球遊走を促進するIL-17Aの増加は、炎症を抑制するIL-10と連動しており、マクロファージを活性化するIL-1βも促進されていることが示された。冷罨法は、比較的炎症性サイトカインの上昇はなく、温罨法は炎症性サイトカインの上昇が認められたものの、肉眼的には冷罨法の創は痂疲が顕著で収縮が認められ、温罨法は表層の創傷の程度は悪化せずに経過した。圧迫創において、温罨法は炎症系サイトカイン、抗炎症系のサイトカイン双方の上昇を認め、好中球の遊走刺激になることが考えられた。サイトカインは上昇したものの、拮抗するサイトカインの上昇が認められたことから炎症のバランスを保っているとも考えられ、肉眼的な創傷の程度が軽傷であったことから、温罨法は圧迫創の表層部における創傷治癒調整に効果をもたらす可能性が示唆された。

    This study was to evaluate the effect of the spine, left lateral decubitus, and right lateral decubitus position on autonomic nervous activity elderly by using spectral analysis heart rate variability (HRV). Forty-five elderly adults were enrolled. The lowest heart rate continued for 10 min, when participants were left lateral decubitus position on compared with the other two positions, The low frequency HRV to high-frequency HRV ratio (LF/HF) for the right lateral decubitus position was significantly lower than that for the other position. The right lateral decubitus position may attenuate sympathetic nerve activity in elder adults.

    It is well known about immunocompromised state and neutrophil dysfunction in diabetic foot ulcers. But the detailed mechanism of neutrophil dysfunction remains unclear. In the current study, to identify the possible contribution of NETosis the co-localization between Ly6G (marker of neutrophils) and citrullinated histon (an indicator of NETosis) and elastase activity after α-mannan administration were examined. We discovered that the expression of citrullinated histone was accerated at the wound sites. In addition, elastase activity level was significantly higher in α-mannan-administrated mice than in vehicle-administered mice. We next investigated the effect of sivelestat sodium, a neutrophil elastase inhibitor, on wound closure in WT mice administered α-mannan, and the administration of sivelestat sodium was found to significantly improve the delayed wound closure caused by α-mannan.

    The involvement of biofilms in chronic wounds is becoming widely known, but all chronic wounds don't have biofilms.In this study, biofilm formation in chronic wounds was 33%. In addition, it was found that biofilm formation in ischemic conditions was inhibited, and that biofilm formation and neutrophils were involved. The possibility of inhibiting the biofilm formation by negative pressure closure therapy (NPWT-id) among the therapies for various biofilms was indicated.

    In chronic wounds, the inflammatory responses are prolonged with persistent neutrophil infiltration. Recently, we showed that skin wound healing was delayed and the healing process was impaired under conditions lacking invariant natural killer T (iNKT) cells, an innate immune lymphocyte with potent immuno-regulatory activity. In the present study, to identify the possible contribution of NKT cells activation, the effects of α-Galactosylceramide (α-GalCer) administration on wound healing, specific activator of iNKT cells, were examined. Administration of a-GalCer at the wound sites significantly promoted wound closure, granulation tissue formation, angiogenesis compared with vehicle treatment. In addition, the inflammatory cells were rapidly decreased and granulation tissue were enhanced in α-GalCer administered mice.

    The role of endogenous damage-related molecular patterns (DAMPs) in skin wound healing remains unclear. In the present study, to identify the possible contribution of CLRs, such as Dectin-2, the expression of Dectin-2 mRNA was examined. Dectin-2 mRNA expression reached peak levels at 12 hours in WT mice, and expression levels declined thereafter to the basal level. we found that both neutrophils and macrophages expressed Dectin-2, and the proportions of these cells expressing Dectin-2 were 40.1±5.0% and 32.1±2.9%, respectively. In immunohistochemical analysis, DAMPs recognized by Dectin-2 was increased in infiltrating cells at the wound site.

    The aim of this study was to evaluate the effect of the spine, left lateral decubitus, and right decubitus position on autonomic nerve activity in young adults and elderly by using spectral analysis of heart rate variability(HRV). After laying in spine position, all participants in a random order and maintained the position for 10 min, while ECG data were recorded to measure HRV. The right lateral position may attenuate sympathetic nerve activity in elderly adults.

    Macrophage inducible C-type lectin (Mincle) is an activating receptor, expressed in macrophages and dendritic cells, which senses PAMPs and DAMPs. However, the role of Mincle at the wound sites has not been elucidated. In the present study, we addressed a question how defect of Mincle affected to the healing process of wounds. Wounds were created on the backs of wild type (WT) C57BL/6 and Mincle-deficient (KO) mice. We analyzed percent wound closure, height of granulation tissue, expression of Mincle by real-time PCR and immunohistochemistry. The percent wound closure was significantly increased in Mincle KO mice compared to WT mice, which was associated with the increased height of granulation tissues. Mincle was detected at the early phase after wound creation. These results suggest that Mincle may be involved in the regulation of granulation at the wounded tissues, which may lead to delayed wound healing.

    In particular, parasympathetic nerve activity as well as arterial baroreflex sensitivity reduces with aging process. In elderly patients with coronary artery disease, high parasympathetic nerve activity and low sympathetic nerve activity have been observed in right lateral decubitus position compared with supine and left lateral decubitus positions. However, little research has been conducted to determine sex and age differences in autonomic nervous activity in different positions, i.e., supine, left recumbent, and right recumbent positions. The aim of this study was to evaluate the effect of the left and right recumbent positions on heart rate variability in elderly was the lowest among the three positions regardless of sex. In elderly men, the LF/HF ratio in right recumbent position was significantly lower than that in supine position. Right recumbent position may attenuate cardiac sympathetic nerve activity in elderly adult men.

    Recently, the interaction between multiple organs and tissues have been clarified. However skin-liver network have not been reported. In the present study, we analyzed the network between skin and liver after wound creation. Hepatic NKT cells and neutrophils were increased after wounding. In addition, MIP-2 expression was also increased after wounding. These results suggest that skin-liver network may be existed.

    Type I interferons (IFNs) are secreted by many cells upon stimulation via pattern recognition receptors like TLR3 and TLR7, and deliver the activation signal through a specific cell surface IFN-α/β receptor (IFNAR) that consists of IFNAR1 and IFNAR2 chains. Although type I IFNs are well known in their antiviral activity, limited information is available in their role during wound healing. In the present study, we analyzed how defect of IFNAR-mediated signaling affected the wound healing . Wounds were created on the backs of C57BL/6 mice, we examinesd the percent wound closure and accumulation of leukocytes were compared between IFNAR1 gene-disrupted (KO) mice and wild type mice. The percent wound closure was significantly delyed in IFNAR1KO mice compared to WT mice on day 5, which was associated with decreased macrophage accumulation. These results suggest that type I IFNs may be involved in the regulation of the wounds healing.

    Interleukin-17 (IL-17) is a proinflammatory cytokine. The role of this cytokine in the wound healing process is not fully clarified. In this study, IL-17A knockout mice showed promoted wound healing and decreased neutrophil accumulation compared with wild-type mice, and the administration of recombinant IL-17A led to delayed wound closure, accelerated neutrophilic accumulation and improved wound repair with treatment of a neutrophil elastase inhibitor. Neutrophils, macrophages and gamma-delta T cels showed IL-17A expression after wounding in wild-type mice. These results indicated that IL-17A hampared the wound healing process and neutrophilic inflammation caused by IL-17 led to impaired wound healing in skin.

    Low birth weight(LBW) conformed as a risk of high blood pressure in later life.However, a number of studies have shown that Mongolian people have higher prevalence of hypertension compared to other ethnic groups. including the Japanese. The purpose of this study was to identify whether there are differences in the relationship of LBW and the risk of hypertension between Japanese and Mongolian.We measured BP and heart rate variability at resting condition and postal change for both Japanese and Mongolian volunteers with LBW and normal birth weight,respectively. We conclude that Mongolian young adults a higher risk of hypertension compared to the Japanese counterpart.

    We addressed the possible involvement of quorum sensing (QS) molecules, 3-oxo-C12-HSL and C4-HSL, from Pseudomonas aeruginosa to inhibition of methicillin-resistant Staphylococcus aureus (MRSA). We used Vancomycin (VCM) as a positive control. More than 10% Dimethyl sulfoxide (DMSO) had the effect of inhibition of MRSA, but on the other hand QS molecules had not that effect. In addition, the MRSA growth did not affect by the stimulation of 3-oxo-C12-HSL and C4-HSL at the same time. Minimum inhibitory concentration of VCM showed 1 μg/ml. These results suggest that QS molecules, 3-oxo-C12-HSL and C4-HSL, have not effect to inhibition of MRSA.

    In the present study, we evaluated the effects that cool and hot packs as an initial therapy has contributed wound healing on post-pressure induced deep tissue injury model mouse. Wound healing is similar to that of secondary intention on the model mouse. The cool group had significant tissue necrosis in the epidermis/dermis, but l though the deep skeletal muscle had been damaged, muscle cell regeneration was noted on days 5 to 7. The hot group exhibited reverse findings in the respective tissue layers, and had delayed regeneration of skeletal muscle. Applied cool or hot packs to the pressure wound were indicated different effects on each layer of the skin tissue including muscles.

These results suggest that DAMPs may be involved in the inflammatory response.

    Little research has been conducted on age-related differences in autonomic nervous activity and circulation between the various recumbent positions. The purpose of the present study was to evaluate elderly subjects show differences in the autonomic nervous activity and circulation between the 3 positions (supine, right lateral decubitus, and left lateral decubitus position). We performed electrocardiographic measurements for 50 elderly subjects (63-85 years). No significant differences were observed in mean BP, low frequency HRV, high frequency HRV, and their ratio. However, in the left lateral decubitus position, HR was significantly lower than in the supine position. In contrast, the LF/HF ratio in the right lateral decubitus position was significantly lesser than that in the supine position the elderly .

    Natural Killer T (NKT) cells, an unique lymphocyte with a potent regulatory ability in various inflammatory responses, produce IFN-γ and IL-4 upon stimulation with α-galactosylceramide (α-GalCer). However, it is not understood how this lymphocyte is involved in skin wound healing. In this study, we elucidated the role of NKT cells in the skin wound healing process. In Jα18KO mice lacking NKT cells, wound healing was impaired compared with wild-type (WT) mice. The adoptive transfer of liver mononuclear cells (LMNCs) from WT, but not from Jα18KO mice, resulted in the reversal of this impaired wound healing in Jα18KO mice. Administration of α-GalCer resulted in the acceleration of wound healing. These results indicate that NKT cells play important roles in wound healing.

    There is the country where low power outside the body shock wave treatment is really used for for a diabetic foot ulcer and a chronic leg ulcer in the Western European countries, but the authorization in this country is not done. The fundamental law can expect the role as the ace in the hole in the case that revascularizing it has difficult for diabetes, the arterial ischemia-related ulcer caused by the seriously ill ischemic limb (occlusive arteriosclerosis). The vascularization action was clarified through animal experiment. I pushed forward preparations for clinical application in parallel.

    Several bacteria, including Pseudomonas aeruginosa exist in chronic wounds with prolonged inflammatory reaction. In the present study, using a rat model with full-thickness dermal wounds, we examined the effect of P. aeruginosa and quorum sensing (QS) molecules, N-(3-oxododecanoyl)-_L-homoserine lactone (3-oxo-C_<12>-HSL), which is produced by P. aeruginosa. Within 3 days, inoculation with P. aeruginosa or3-oxo-C_<12>-HSL had accelerated local infiltration of neutrophils and tumor necrosis factor-α. These results indicate that 3-oxo-C_<12>-HSL may be involved in induction of inflammatory responses.

    The screening for detecting genes which involved in breast cancer development revealed 25 related ones. The five of them were unknown as DNA repair factors. Also, we characterized a novel repair gene, MDT1, which we isolated previously, and the experiment showed that MDT1 has a role for the protein metabolism.

    Quorum-sensing(QS)systems regulate virulence factors in Pseudomonas aeruginosa, which produces autoinducers. Accerelations of wound healing were not observed when inoculated with ΔlasI or ΔrhlI in place of PAO1 and cancelled when co-treated with these anti HSL antisera, respectively, but not with control rabbit serum. By contrast, administration of HSL significantly accelerated the re-epithelialization. These results suggest that HSL may be deeply involved in the skin wound healing during P. aeruginosa infection. Low-energy extracorporeal shockwave treatment(LE-ESWT)has been demonstrated to accelerate wound repair. E-NOS is induced at the wound tissues by LE-ESWT and plays a critical role in the therapeutic effects of this treatment accelerating the wound healin g through promoting VEGF expression and neo-vascularization.

    In the present study, we examined the activity of the silver-containing dressing on wound infection was studied in vivo model. Full- thickness wounds were created on the backs of SD rats. P. aeruginosa strain PAO1 was inoculated at the wound surface. Wounds were harvested at 3 days postwounding, processed for histology and bacterial counts in the wound tissue. The silver-containing dressing did not demonstrate an antibacterial effect in the minimal volume of wound fluid. In conclusion, it is necessary for appropriate measures of the silver-containing dressing to need an accurate acessment of wound environment.

    ラット皮膚潰瘍創実験モデルを用いた細菌と創傷治癒に関する検討 我々が昨年度確立したラット感染性皮膚潰瘍創実験モデル(人工創を作製し、緑膿菌PAO1を接種後、閉鎖環境においた状態)を用い、創傷におけるバイオフィルム形成や細菌と創傷治癒との関連性、創傷における洗浄の効果について検討した。 細菌と創傷治癒との関連性 創作製、細菌接種7日目に組織を摘出し、光学顕微鏡的に観察した結果、緑膿菌を接種した創傷では、好中球浸潤に伴って顕著な表皮の肥厚や表皮化の促進が観察された。このことから細菌のcolonizationにより、通常とは異なる創傷治癒機転が生じ、その際生体側の要因として白血球や表皮細胞の作用が推定された。 創傷における洗浄の効果 潰瘍創作成2日目より、創部を生食にて連日1回/日洗浄した結果、洗浄7日目では洗浄なし群と比較し洗浄群ではバイオフィルムの発達を認めなかった。この結果より、洗浄はバイオフィルム抑制に有効な手段であることを確認した。 創部の細菌感染は創傷治癒遅延原因の一つであるため、創面の細菌数コントロールは重要だと考えられている。本研究により、洗浄が細菌数コントロールに有効であり、潰瘍創に対する洗浄の重要性が確認された。 研究成果の発表 平成19年度に得られた結果のうち、皮膚潰瘍創における緑膿菌バイオフィルム形成や潰瘍創に対する洗浄について、国内外の専門学会にて発表を行うとともに、論文にて公表した。
