INTRODUCTION: Transcutaneous fluorescein isothiocyanate (FITC)-sinistrin fluorescence enables convenient, minimally invasive estimation of the glomerular filtration rate (GFR) in rodents. However, the commonly used fixed-factor conversions (i.e., GFR/BW = conversion factor (21.33)/HL, where HL is sinistrin half-life and BW is body weight) assume the volume of distribution scales proportionally with BW. This assumption can bias GFR estimates when body composition shifts with growth or when extracellular fluid volume expands under high-salt intake. Here, we quantified these errors and developed improved HL-to-GFR conversion methods. METHODS: Male and female Dahl salt-sensitive rats with chronically implanted venous catheters underwent simultaneous measurements of serum and transcutaneous FITC-sinistrin fluorescence. Serum clearance provided reference GFR and serum-derived HL, and the transcutaneous signal provided transcutaneous HL. In separate cohorts, body composition and extracellular fluid were assessed by Nuclear Magnetic Resonance-Bioimpedance Spectroscopy. A regression of GFR×HL was performed using BW, age, sex, and salt intake. RESULTS: In low-salt fed rats, sex-specific BW-based models explained much of the variability in GFR×HL (females: 97.81×BW0.385; males: 203.0×BW0.855; BW in kg). Separately, using data from all rats across salt conditions, we derived a salt-dependent conversion to allow for temporal salt effects. The corrected model detected the transient increase in GFR on high-salt day 3 that was missed by a fixed factor. GFR scaled sub-linearly with BW (exponent 0.744). CONCLUSIONS: Allowing the sinistrin HL-to-GFR conversion to vary with BW and salt condition moderately improves the accuracy of HL-based FITC-sinistrin GFR estimates compared with a single fixed conversion factor and captures the transient GFR increase after salt loading. Sub-linear scaling of GFR supports allometric size adjustment (BW0.744) to reduce size-dependent bias and better reflect true GFR in growing rats.

Studies have found that blood flow to the renal medulla is an important determinant of pressure-natriuresis and the long-term regulation of arterial pressure. First, a brief review of methods developed enabling the study of the medullary circulation is presented. Second, studies performed in rats are presented showing medullary blood flow plays a vital role in the pressure-natriuresis relationship and thereby in hypertension. Third, it is shown that chronic reduction of medullary blood flow results in hypertension and that enhancement of medullary blood flow reduces hypertension hereditary models of both salt-sensitive rats and salt-resistant forms of hypertension. The key role that medullary nitric oxide production plays in protecting this region from ischemic injury associated with circulating vasoconstrictor agents and reactive oxygen species is presented. The studies cited are largely the work of my students, research fellows, and colleagues with whom I have performed these studies dating from the late 1980s to more recent years.

Objective: Increased central venous pressure in congestive heart failure is responsible for renal dysfunction, which is mediated by renal venous congestion. Pericyte detachment from capillaries after renal congestion might trigger renal fibrogenesis via pericyte-myofibroblast transition (PMT). Platelet-derived growth factor receptors (PDGFRs), which are PMT indicators, were upregulated in our recently established renal congestion model. This study was designed to determine whether inhibition of the PDGFR pathway could suppress tubulointerstitial injury after renal congestion. Methods: The inferior vena cava between the renal veins was ligated in male Sprague-Dawley rats, inducing congestion only in the left kidney. Imatinib mesylate or vehicle were injected intraperitoneally daily from 1 day before the operation. Three days after the surgery, the effect of imatinib was assessed by physiological, morphological and molecular methods. The inhibition of PDGFRs against transforming growth factor-β1 (TGFB1)-induced fibrosis was also tested in human pericyte cell culture. Results: Increased kidney weight and renal fibrosis were observed in the congested kidneys. Upstream inferior vena cava (IVC) pressure immediately increased to around 20 mmHg after IVC ligation in both the imatinib and saline groups. Although vasa recta dilatation and pericyte detachment under renal congestion were maintained, imatinib ameliorated the increased kidney weight and suppressed renal fibrosis around the vasa recta. TGFB1-induced elevation of fibrosis markers in human pericytes was suppressed by PDGFR inhibitors at the transcriptional level. Conclusion: The activation of the PDGFR pathway after renal congestion was responsible for renal congestion-induced fibrosis. This mechanism could be a candidate therapeutic target for renoprotection against renal congestion-induced tubulointerstitial injury.

Abstract Diabetic kidney disease is a major cause of renal failure that urgently necessitates a breakthrough in disease management. Here we show using untargeted metabolomics that levels of phenyl sulfate, a gut microbiota-derived metabolite, increase with the progression of diabetes in rats overexpressing human uremic toxin transporter SLCO4C1 in the kidney, and are decreased in rats with limited proteinuria. In experimental models of diabetes, phenyl sulfate administration induces albuminuria and podocyte damage. In a diabetic patient cohort, phenyl sulfate levels significantly correlate with basal and predicted 2-year progression of albuminuria in patients with microalbuminuria. Inhibition of tyrosine phenol-lyase, a bacterial enzyme responsible for the synthesis of phenol from dietary tyrosine before it is metabolized into phenyl sulfate in the liver, reduces albuminuria in diabetic mice. Together, our results suggest that phenyl sulfate contributes to albuminuria and could be used as a disease marker and future therapeutic target in diabetic kidney disease.

    More details Close

We created a rat renal congestion model and investigated the effect of renal congestion on hemodynamics and molecular mechanisms. Tubulointerstitial injury and medullary thick ascending limb hypoxia were observed only in the congestive kidneys. Using this rat congestion model and cell culture system, we revealed that vacuolar ATPase system in acidic organelles and pericyte injury were involved in the development of renal interstitial fibrosis due to renal congestion.
