    Measurement of nitric oxide (NO) concentration in exhaled breath (FeNO) is a quantitative, non-invasive, simple, and safe method for assessing airway inflammation. It serves as a complementary tool to other methods for evaluating airway diseases. However, little is known about the typical NO levels in healthy individuals, including individual differences and the influence of measurement timing. Therefore, this study classified measurement times into four periods and statistically analyzed NO levels in healthy individuals. The mean values among groups were compared using repeated measures ANOVA on six participants. The analysis showed large individual variations in NO levels, resulting in no significant difference (p = 0.29). Notably, greater fluctuations were observed in the morning. These findings align with previous studies suggesting the influence of circadian rhythms and the redundancy of repeated measurements. This study highlights the need to consider timing and individual variability when using FeNO as a physiological marker in healthy populations.

    We previously demonstrated that sleep apnea (SA) can be detected using acceleration and gyroscope signals from smartwatches. This study investigated whether an inertial measurement unit (IMU) embedded in non-wristwatch devices, such as smartphones, can also detect SA when worn during sleep. During polysomnography (PSG), subjects wore an IMU-embedded GPS device (Amue Link®) and/or smartphones (Xperia® or iPhone®) on their abdomen. Triaxial acceleration and gyroscope signals were recorded overnight. Data were split into training and test groups (2:1) for each device. An algorithm was developed in the training groups to extract respiratory movements (0.13-0.70 Hz) and detect respiratory events, which were validated in the test groups. IMU-derived respiratory events showed breath-by-breath concordance with PSG apnea-hypopnea events, yielding F1 scores of 0.786, 0.821, and 0.796, respectively. Regression model derived from IMU signals correlated with PSG AHI in the test groups (r = 0.90, 0.93, and 0.96), with limits of agreement of -16.7 to 25.9, -17.4 to 22.5, and - 18.4 to 20.5. Using cutoff values from the training groups, moderate-to-severe SA (AHI ≥ 15) was identified in the test groups with AUCs of 0.95, 0.98, and 0.94 and F1 scores of 0.89, 0.96, and 0.92, respectively. IMUs embedded in non-wristwatch devices, including smartphones, can quantitatively detect SA when worn during sleep.

    PURPOSE: Despite increased awareness of sleep hygiene, over 80% of sleep apnea cases remain undiagnosed, underscoring the need for accessible screening methods. This study presents a method for detecting sleep apnea using data from the Apple Watch's inertial measurement unit (IMU). METHODS: An algorithm was developed to extract seismocardiographic and respiratory signals from IMU data, analyzing features such as breathing and heart rate variability, respiratory dips, and body movements. In a cohort of 61 adults undergoing polysomnography, we analyzed 52,337 30-second epochs, with 12,373 (23.6%) identified as apnea/hypopnea episodes. Machine learning models using five classifiers (Logistic Regression, Random Forest, Gradient Boosting, k-Nearest Neighbors, and Multi-layer Perceptron) were trained on data from 41 subjects and validated on 20 subjects. RESULTS: The Random Forest classifier performed best in per-epoch respiratory event detection, achieving an AUC of 0.827 and an F1 score of 0.572 in the training group, and an AUC of 0.831 and an F1 score of 0.602 in the test group. The model's per-subject predictions strongly correlated with the apnea-hypopnea index (AHI) from polysomnography (r = 0.93) and identified subjects with AHI ≥ 15 with 100% sensitivity and 90% specificity. CONCLUSION: Utilizing the widespread availability of the Apple Watch and the low power requirements of the IMU, this approach has the potential to significantly improve sleep apnea screening accessibility.

    OBJECTIVE: Pain is subjective, and self-reporting pain might be challenging. Studies conducted to detect pain using biological signals and real-time self-reports pain are limited. We evaluated the feasibility of collecting pain data on healthy females' menstrual pain and conducted preliminary analysis. RESULTS: Five healthy adult females participated. They wore two wristwatch devices (Silmee and Fitbit) and a Holter ECG (electrocardiogram) during menstruation to record the pain intensity and timing. Subsequently, we analyzed the correlation between heart and pulse rates and assessed pre- and post-pain biometric differences. We collected sixty pain records from five participants. The correlation coefficients between heart rate and pulse rate ranged from 0.79 to 0.95 with Holter ECG vs. Fitbit and 0.32 to 0.74 with Holter ECG vs. Silmee. Analysis revealed significant changes in motion frequency post-pain (p = 0.04). For abdominal pain with a numerical rating scale score of ≥ 4 (n = 13), motion frequency (p < 0.001) and pulse rate (p = 0.02) showed significant differences post-pain compared to baseline values. Healthy females could wear the wristwatch device in daily life and report pain in real time. Wristwatch devices can effectively collect biological data to detect moderate pain by focusing on acceleration and pulse rate.

    Pain assessment becomes challenging for patients unable to self-report, given the subjective nature of pain. This study introduces an automatic pain detection model utilizing biological signals from wristwatch wearables and time series data from patients with cancer experiencing pain. Biological signals and pain data were obtained from 10 patients with cancer pain for 7 days during their hospitalization. A total of 73,154 minutes of data and 407 pain reports were obtained. We developed automatic classifiers to detect moderate or severe pain and pain above the personalized pain goal by several machine learning algorithms using per-patient and mixed data sets. The best-performing algorithm achieved an F1 score of 0.87, with enhanced performance using the personalized pain goal as the cutoff. While the generalized model requires improvement, the study demonstrates the feasibility of automatic pain detection using extended real-world patient data.

    This study focused on the evaluation of driver excitability and examined the utility of continuous deep body temperature variability analysis. It was suggested that the combination of ear temperature and heart rate could be used to objectively assess a driver's biological state to a certain degree. This study may contribute to improving traffic safety and promoting the psychological health of drivers, and future research and practical applications are encouraged.

    In this study, a recommendation system was constructed based on exercise questionnaires of 100 subjects (20-80 yo) using the ForecastFlow system (GRI Incorporated, Japan). The questionnaire was created using Google Forms, and a simulation model and prediction algorithm were developed to predict exercise withdrawal and recommend optimal timing. Results showed that the recommendation system is applicable to various health conditions and age groups, but on the other hand, revealed concerns about the extensive search space before sufficient data has been accumulated

    This study focused on the use and analysis of 6-DOF accelerations measured from humans. Data obtained mainly from smartphones were visualized to visualize the movement and body acceleration changes of several individuals. In our experiments, we visualized 6-DOF acceleration data of a healthy individual obtained from a smartphone and compared it to 3-axis acceleration data used in conventional wearable sensors and medical devices. As a result, we were able to visualize the swaying that cannot be captured by 3-axis acceleration while riding in a car. This information could provide useful insights in health monitoring and motion assessment.

    Focused on the visualization of fatigue detection using a sitting pressure sensor, especially through the variation of human ischium in different age groups. The subjects were 10 healthy people. Sitting pressure data was acquired at 5Hz, and a heat map was created and visualized. Comparison of sitting pressure patterns by age revealed age dependence. This study contributes to the understanding of and countermeasures against fatigue in different age groups.

    In this paper, we used ALLSTAR big data to investigate changes in Heart rate variability(HRV) and body acceleration(BA) in younger (20s) and elderly groups (70s) during the COVID-19 pandemic. Compared to before (2019) and during (2020, 2021) the COVID-19 pandemic, significant decrease in HRV was observed in both the younger and the elderly groups (P<0.01). BA in the younger groups decreased significantly in 2020 compared to 2019, and no significant difference was observed in the elderly group. Mental stress due to the COVID-19 pandemic is thought to be one of the factors contributing to the decrease in HRV. In the elderly group, respiratory frequency was faster in 2020 than in 2019 (P<0.01) and slower in 2021 than in 2020 (P<0.01). It is thought that the respiratory frequency is changing due to the mask life.

    In recent years, animal therapy has been adopted in various fields such as medicine and welfare because it is expected to improve symptoms of dementia and depression as well as relieve stress. However, repeated exposure to a certain stimulus may cause a gradual loss of response to that stimulus (habituation), and previous studies have shown that habituation tends to occur with neutral stimuli that are neither rewarding nor harmful. In this study, 5 animal caretakers (2 female) and 5 healthy subjects (4 female) who were exposed to nature on a regular basis were fitted with wristwatch-type wearable sensors, and their biological parameters were compared during pony riding. The results showed that the caretakers did not show significant biometric changes in response to animal and were considered to be accustomed to it.

    In recent years, wearable sensors have been used to non-invasively obtain a variety of biometric information such as heart rate, pulse wave, acceleration, etc. What is the degree of personal identifiability of biometric information obtained from such wearable sensors? In Japan, the revised Personal Information Protection Law, which was fully enforced in 2017, clarified that personal identification codes such as DNA, facial features, and iris are included in personal information. And in recent years, consideration of human rights and privacy for technologies that monitor and track people has been called for. In this study, 14 parameters were extracted from wearable sensor data of 8 subjects over a 4-day period, and their personal identifiability was verified using machine learning. The results suggest that subjects whose biometric values are close to each other may be misidentified, and that subjects with large intra-individual variation in biometric information are significantly less discriminable.

    Human body temperature consists of skin temperature, which is easily affected by the external environment, and core body temperature, which is maintained constant to protect the functions of the body's center. Core body temperature is less susceptible to the external environment and is maintained constant in order to protect the functions of the internal organs. The deeper the sleep, the greater the decrease in body temperature. Upon entering sleep, the body temperature threshold is lowered and heat dissipation through the skin becomes more active. When sleep is deep slow-wave sleep, heat dissipation becomes very active, accompanied by sweating and a large drop in body temperature. Recently, it has become possible to continuously measure human core body temperature using infrared sensors. Therefore, we attempted to actually measure the phenomenon in which core body temperature reaches a minimum value around 4:00 a.m. using continuous measurement of human core body temperature.

    Our lives have changed dramatically since the spring of 2020 due to the new coronavirus infection COVID-19. And in an environment where we are required to coexist with the virus for the foreseeable future, teleworking to work remotely and social distancing to maintain intervals between people are required. Thorough infection prevention measures" and "ensuring the availability of healthcare delivery systems" remain critical. Recent studies have shown that patients with moderate or severe coronary infections have reduced pulmonary diffusion function as a sequela. Thus, changes in the environment, in the human organism itself, naturally affect heart rate variability indices. To investigate the changes in heart rate variability and body acceleration between the young and the elderly groups, we collected data from the Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR) database on 1342 cases (mean age 22 ± 1 year) in the young group and 12,000 cases (mean age 22 ± 1 year) in the elderly group from 2019 to 2021.) and 12,994 cases (mean age 72 ± 1 years) in the older group from 2019 to 2021 were extracted from the ALLSTAR database and analyzed. Results showed that the younger group showed a decreasing trend in activity due to the spread of coronavirus (P<0.01). The elderly group showed no significant difference in activity in 2020, but the proportion of time spent in the supine position tended to increase (P<0.01).

    In order to realize simple and easy health management at home or workplaces, our research group has been developing a cloud service system “The Mirror-Magical” that performs remote and contactless sensing of biological information without wearing any sensors, which was named “wear-less sensing.” The system can measure, accumulate, and analyze health-related information such as blood perfusion, autonomic nervous system indices, and blood pressure-related values. As well as on the cloud service system, this paper has reported on the “The Sphere-Magical”: a palm-based pulse wave measurement system that is never affected by body motion and lighting fluctuations, and the “The Eye-Magical”: a video pulse wave measurement system that can simultaneously analyze multiple faces, and discuss their effectiveness and limitations.

    To examine the possibility of non-contact unconstrained continuous pulse wave measurement during driving, we installed a microwave Doppler element pulse wave sensor on the dashboard of a large truck. The sensor detects the pulsatile movements of the heart and blood vessels from the Doppler shift of the reflected waves of microwaves emitted toward a person at a distance. We analyzed pulse rate variability from the signal during a long-distance operation and compared it with heart rate variability obtained from ambulatory electrocardiogram recorded simultaneously.

    The purpose of this study is to evaluate physiological and psychological stress in truck driver towing self-driving trucks. The truck A with which two self-driving truck B and C are coupled in series is run along a guide line expressway for running for about 30 minutes, 3times in a day. This experiment last 3 days, with 3 subjects derived the lead truck while towing 2 self-driving truck. For safety, driver was on the self-driving trucks B and C. In addition, each subject towing self-driving operation 3 times, solo operation (not towing self-driving) was 1 time. The bio-signal data (ECG, body temperature, skin temperature) of the subject were continuously measured at each driving phase. Subject's data phase "solo" compares with the towing data compared to detect the difference. As a result of those data, we have significant increase in skin temperature and heart rate between solo and towing operations.

    Although many attempts have been made to detect driver’s drowsiness by ECG and pulse wave signals, it is not easy to record stable bio-signals while driving. In this study, we examined whether the driver's drowsiness can be detected from the respiration signal that can be acquired relatively easily with wearable clothing sensor. In 7 healthy subjects (five males and two females; age, 45 ± 9 y), respiration, ECG, and acceleration signals were recoded for a total of 2,359 min (137-468 min per subject) of driving with a smart shirt biometric sensor (Hexoskin). Minute-to-minute respiration amplitude and frequency and their variability were analyzed by complex demodulation between 0.05 and 0.45 Hz. The changes in the respiration parameters were analyzed in relation to the Dip & Waves, which are known to be a characteristic ECG R-R interval pattern associated with driver’s drowsiness. Neither respiratory amplitude nor frequency showed significant changes with Dip & Wave, but respiratory frequency variability increased progressively from 4 minutes before, peaked at Dip & Waves, and then decreased shortly thereafter. Our observations suggest the possibility that respiration signal obtained by wearable garment sensor may be used to detect driver’s drowsiness.

    Usefulness of subjective sleep quality assessment by a questionnaire (OSA-MA sleep inventory) was examined in ten track drivers (age, 23-62 yr) in reference to the objective measure by cyclic variation of heart rate (CVHR) in electrocardiogram (ECG) during sleep. Total CVHR suggesting moderate-to-severe sleep apnea (average >15 cycles/h) was observed only in one subject and frequent CVHR in a limited time was detected in the same subject and two other subjects. Subjective sleep quality assessed less sleepiness on rising, good initiation and maintenance of sleep, less frequency of dreaming, refreshing feeling, and subjective sleep length as factors 1-5, respectively. The subject with high total CVHR showed factor scores <-1 SD for factors 1, 2, and 3 and reported subjective sleepiness during driving. In the two subjects with frequent CVHR in limited time, one showed factor score <-1 SD for factors 3 and 5, while the other subject did not show score <-1 SD for any of the factors. Although this is preliminary study in a small sample size, it suggests the possible associations between the subjective assessment of sleep quality and the objective measure of CVHR.

    <p>In this paper, we analyzed coordination of chewing and heart rate in two healthy subjects to examine if chewing rhythm affects autonomic outflow. Results revealed no significant relationship between chewing rhythm and change of heart rate modulation. Future studies should investigate heart rate modulation by control of chewing rhythm.</p>

    時代の流れのともに経済状況や社会的情勢の変化によってヒトの行動習慣が変化していく。それに伴い自律神経活動も変化していくことが考えられる。本研究では日本全国で記録された約70万例規模のホルター心電図ビッグデータから生成された時系列データベース(Allostatic State Mapping by Ambulatory ECG Repository, ALLSTAR)のうち,2015年から2021年迄のデータを解析対象として,20代から80代の年齢層別に心拍変動指標(RRI,SDRR,ULF,VLF,LF,HF,LF/HF,HFpeak freq.)の推移を調査した.その結果,各年齢層ともSDRRとHFの減少とLF/HFの増加が有意に認められた.従って,2010年代中盤から後半にかけて心拍変動の減少と交感神経活動の促進が示唆される。

    近年,生体計測技術の発展によってウェアブルセンサから人の心拍状態を推定し,疾患を早期検出できるようになった.身体から発せられるさまざまな情報を取り出す生体信号処理技術は,人の健康を守る救世主となっている. そのほかにも,生体情報のリアルタイム取得は,眠気や疲労など生体の異常検出が可能であり,生理指標や感性指標として人間を定量的に可視化することも可能である.本稿は,心拍に反映される呼吸性変動成分(呼吸性洞性不整脈,Respiratory Sinus Arrhythmia, RSA)の抽出に成功したHayanoらによる画期的な論文を紹介するものである.

    先行研究で高齢者のVR酔いにおける生体反応を評価した。今回は,若年者と高齢者のVR酔いにおける主観評価(SSQ),心拍変動,鼓膜温度を比較した。被験者は,若年者10名(男5名,女5名,27±6歳),高齢者8名(男4名,女4名,75±7歳)である。実験プロトコルは,心電図電極と鼓膜温センサを装着して安静5分を保ち,遊園地アトラクションVR(ジェットコースター,回転ブランコ,回転カート)を3分30秒実施した。その後,安静5分,主観評価(SSQ)を行った。安静時とVR使用中の平均鼓膜温度と心拍変動指標(HR,SDRR,LF,HF,LF/HF)を算出し,安静とVR使用中で比較した。SSQの結果,若年者は回転カートよりジェットコースターの方が有意に目の疲れ(P=0.017)とふらつき感(P=0.014)が強かった。また,若年者と高齢者のSSQスコアーを比べると若年者の方が約2~4倍くらい有意に高い値を示した。若年者のジェットコースターにおける鼓膜温度は有意に増加した(P=0.032)。VRを使用した場合,高齢者に比べて若年者の方が主観的に酔いが強くなり,アップダウンの激しいジェットコースターにおいて若年者の鼓膜温度が有意に増加した。高齢者より若年者の方が,VR酔いを強く引き起こすことが示唆される。

    スマートウォッチやスマートリングなどのウェアラブルセンサは,心拍数や身体加速度などの生体情報を計測・収集して管理する健康管理機能を有し,運動や睡眠の客観的評価に役立つデバイスとして普及しつつある.そして,我が国が目指すべき未来の目標として,超早期の疾患予測・予防の実現が提唱されており,未病状態から健康な状態に引き戻すための方法の確立と疾患の発症自体の抑制・予防の推進が重要視されている. 非侵襲手法で生体信号時系列データ得られるウェアラブルデバイスは,人間の健康状態の推定や改善に大きく寄与することが期待され,得られたビッグデータを解析する研究が世界中で進展している.米国ではウェアラブルセンサから得られた皮膚体温,心拍数および関連指標と,COVID-19の特徴である発熱,咳,疲労などの症状発現との関連を観察するビッグデータプロジェクトが発足し,生理学的反応検出と罹患予測のアルゴリズム開発を目的として生体情報の収集が進んでいる(TemPredict Study).体内ウイルスの早期検出は感染症の封じ込めにも重要であり,とりわけ医療従事者は,早期予測によって蔓延危険性を低減することができる. このように,パンデミックとの戦いに貢献しうる容易計測手法が模索され,生体信号処理を用いた生体ビッグデータ研究も発展しつつある.報告者らは,これまでにウェアラブルセンサから得られた生体信号に機械学習技術を応用して疾患の予兆を捉える複数の研究に着手しており,心疾患や精神疾患,睡眠障害のスクリーニングを行うアルゴリズムの研究開発を行ってきた. 本発表では,COVID-19の推定に関する最新研究について具体的に紹介した上で,生体医工学分野における展開可能性について述べる.

    In this study, we evaluated the eardrum temperature, heart rate variability, and reaction time of hand and foot in elderly people using VR of amusement park attractions.As a result of PVT, the reaction time of the hand was significantly faster after the swing ride (P = 0.023). In addition, the reaction time of the foot was significantly faster after the rotating cart (P = 0.034).As a result of SSQ, roller coaster, swing ride, and rotating cart tended to be Nausea in that order (P = 0.084).There was a significant difference in the time phase change in eardrum temperature (P = 0.008).The eardrum temperature of the roller coaster changed drastically, and the eardrum temperature of the swing ride gradually increased. No significant changes were observed in the rotating cart.

    [Aims] Atrial fibrillation (AF) is known as a major cause of acute cerebral infarction. Cerebral infarction with AF could be prevented by taking oral anticoagulants. Thus, it is critically important to detect and diagnose the presence of AF. Machine learning of Lorenz plot (LP) images is recognized as a promising method for the detection of AF in long-term ECG monitoring, however, the optimal segment length of LP image is unknown. We examined the performance of AF detection by differing LP segment length using convolutional neural network (CNN). [Methods] The datasets of 32x32-low-resolution LP images of R-R interval segments between 20 and 600 beats were created from 24-h ECG data in 52 patients with AF and 58 non-AF controls as teacher data and in 53 patients with paroxysmal AF and 52 non-AF controls as verification data. [Results] In the verification, the positive likelihood ratio (LR) for detecting AF showed a convex parabolic curve with a peak positive LR at 100 beats, while negative LR decreased monotonously as the segment length decreased. [Conclusions] The optimal segment length using CNN models should be 100 beats for discriminating between AF and non-AF.

    [Background] The mortality rate varies depending on the temperature, and the risk increases in both heat and cold. However, the detailed mechanism is unknown. [Purpose] The purpose of this study was to investigate the effects of temperature on heart rate and heart rate variability, including regional differences. The cases of Tokyo and Hokkaido were compared after extraction from the Holter ECG database. [Results] MeanNN showed M-shaped change in both regions. In Hokkaido, the curve shifted toward lower temperatures compared to Tokyo. The standard deviation of NN interval (SDNN) and the power of high frequency range (HF) showed the same changes as MeanNN. [Conclusions] The 24-hour mean heart rate was fluctuated depending on the temperature. Regional variations in heart rate variability were recognized.

    寝たきり状態などの身体活動の著しい低下が、心拍数の概日リズムの位相に与える影響を調べるために、1日の中の臥位で過ごした時間の割合(%lying time、%LT)と、基底心拍数(basal heart rate、BHR、24時間の最低心拍数)の生起時刻(clock time of BHR、CTBHR)との関連をビッグデータから分析した。男性12,530人 (20-99歳)と女性17,529人(20-101歳)において、CTBHRの中央値[IQR]は男性で04:07 [01:35-05:35]、女性で03:50 [01:35-05:53]であった。年齢と%LTの間の交絡を調整した分析で、CTBHRは、男女とも加齢と共に僅かに前進し(加齢10歳ごとに-11分および-8分)、%LTの増加によって僅かに遅延した(10%の%LT増加あたり+ 6分および+ 9分)。%LT≧70%の寝たきりに近い状態の人のCTBHRは、70%未満の人に比べて、男性で13分、女性で45分遅れていた。心拍概日リズムの起点としてのCTBHRは、加齢や身体活動の低下によっても容易に失われない堅牢な生物学的特性である。

    <p>心拍変動解析は、短時間解析と長時間解析に分けられる。前者は5分を、後者は24時間を基準とするが、両者のより重要な違いは前者が対象者の状態と測定環境のコントロール下で測定されるのに対し、後者は自由行動下で測定されることである。心拍変動は自律神経機能評価に用いられ低周波数成分や高周波数成分などの指標が使われるが、それらは短時間解析で確立されたものである。長時間心拍変動には、自律神経機能の前に、心拍数に対する心身の活動や環境因子の影響を考えるべきである。例えば24時間のSDNNは主に昼夜の心拍数の差を反映し、昼間の身体活動レベルによって規定される部分が大きい。SDNNの低下と心筋梗塞後の死亡リスクとの関連は副交感神経機能障害よりも身体活動の制限を強いられる状態によるものかもしれない。心拍変動の周波数と交感・副交感神経区分との関係も、長時間心拍変動においては呼吸数の高周波数帯(0.15-0.4 Hz)からの逸脱によって失われているかもしれない。一方、長時間心拍変動のメリットとして、記録中の睡眠時無呼吸などの偶発的事象に対する心拍数応答から自律神経の反応性の評価などの新しい試みも注目したい。</p>

    <p>心拍数フラグメンテーション(HRF)は、心電図が洞調律を示すにも関わらず、R-R間隔時系列に頻繁(多くの1拍毎)に極値が出現することを特徴とする洞機能障害の一つである。 心拍変動解析による迷走神経機能の評価は、心拍変動の高周波(HF、0.15-0.4Hz)成分が迷走神経によってのみ媒介されることを前提として仮定しているが、HRFが存在するとそれによるR-R間隔変動もHF成分の一部として測定され得るために、HF成分による迷走神経機能の評価を混乱させる要因となる。 本研究では、24時間心電図ビッグデータ、Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR)の心電図より、HRFの指標であるpercentage of inflection points(PIP)を用いて、年齢によるHRF発生状況とHF成分の評価に対する影響を調べた.その結果、HRFは小児期(0~20歳)および80歳以降に増加し,PIPの増加とともにHF成分パワーにしめるHRFの割合が15%から30%と増加していることが推定された。PIPが60%を超える時には.SDNN.LF,VLFのパワーにもHRFによる増加分が5~30%含まれていると推定された.</p>

    In order to obtain basic information on the weekly fluctuation of Japanese physical activity(PA) for gender and age, 24-hour activities of 33,840 males and 42,108 females recorded by Holter ECG recorders with built-in 3-dimensional acceleration sensor were statistically determined by sex, age, and day of the week in Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR).A significant weekly fluctuation was observed in both gender, the average of PA in 24 hours was the highest on Monday and then Thursday (men 99.0% ,females 99.4% for Monday 100%) was high. Tuesday(97.8%,98.9%), Wednesday(95.7%,98.2%) and Friday(96.1%,97.7%)were intermediate, Sunday (93.5%, 97%) was low, Saturday(90.4%, 91.2%) was the lowest .It is suggested that there is a weekly fluctuation of about 10% in Japanese PA.In addition, we will also consider weekly fluctuation of PA for regional differences in healthy life expectancy.

    There is a criticism against big data analyses that they would seldom provide new findings that are not recognized by statistical analysis of small samples because statistical analysis has been developed to estimate population features from a small number of samples. We examined this by analyzing randomly sampled 100 to 100,000 data from 24-h ECG big data. The results indicated that statistical significance provides an indicator of the representability of the samples for population features and that once statistical significance is reached, further increases in sample size do not result in substantial differences in outcome.

    Heart Rate Variability has various indices and many of them correlate with each other, suggesting the presence of common underlying factors determining them. We therefore extracted common factors among indices using big data and analyzed the characteristics of each factor. We performed factor analysis on mean NN, SDNN, rMSSD, VLF, LF, HF, LF / HF, scaling exponent alpha 1, DC, the amplitude of cyclic variation of heart rate, and non-Gaussianity index of lambda 25s obtained from 254,357 subjects with sinus rhythm in ALLSTAR 24-h electrocardiogram big data. Two factors that reflect cardiac vagal function and complexity/irregularity of fluctuation, respectively, were extracted. They showed different relationships from each other with aging and daily physical activities.

    Heart rate variability of Holter electrocardiogram (ECG) that incidentally recorded the onset process of heatstroke during golf play was analyzed in comparison to control ECGs in the same subject. During playing golf normally, the amplitude of high-frequency component (HFA), known as an index of cardiac parasympathetic function, was suppressed (4.3 ± 2.2 ms). Whereas, 135 min before the onset of syncope, spiky HF bursts (amplitude ~10 ms and width <60 s) appeared at an interval of 10-15 min and the burst amplitude increased to 20-30 ms 50 min before the onset. No such phenomenon was observed in control ECGs even during walking exercise. Spiky HF burst may be used as a prodromal sign for predicting heatstroke from heartbeat information.

    <p>In order to obtain basic information on the weekly fluctuation of Japanese physical activity(PA) for gender and age, 24-hour activities of 33,840 males and 42,108 females recorded by Holter ECG recorders with built-in 3-dimensional acceleration sensor were statistically determined by sex, age, and day of the week in Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR).A significant weekly fluctuation was observed in both gender, the average of PA in 24 hours was the highest on Monday and then Thursday (men 99.0% ,females 99.4% for Monday 100%) was high. Tuesday(97.8%,98.9%), Wednesday(95.7%,98.2%) and Friday(96.1%,97.7%)were intermediate, Sunday (93.5%, 97%) was low, Saturday(90.4%, 91.2%) was the lowest .It is suggested that there is a weekly fluctuation of about 10% in Japanese PA.In addition, we will also consider weekly fluctuation of PA for regional differences in healthy life expectancy.</p>

    <p>There is a criticism against big data analyses that they would seldom provide new findings that are not recognized by statistical analysis of small samples because statistical analysis has been developed to estimate population features from a small number of samples. We examined this by analyzing randomly sampled 100 to 100,000 data from 24-h ECG big data. The results indicated that statistical significance provides an indicator of the representability of the samples for population features and that once statistical significance is reached, further increases in sample size do not result in substantial differences in outcome.</p>

    <p>Heart Rate Variability has various indices and many of them correlate with each other, suggesting the presence of common underlying factors determining them. We therefore extracted common factors among indices using big data and analyzed the characteristics of each factor. We performed factor analysis on mean NN, SDNN, rMSSD, VLF, LF, HF, LF / HF, scaling exponent alpha 1, DC, the amplitude of cyclic variation of heart rate, and non-Gaussianity index of lambda 25s obtained from 254,357 subjects with sinus rhythm in ALLSTAR 24-h electrocardiogram big data. Two factors that reflect cardiac vagal function and complexity/irregularity of fluctuation, respectively, were extracted. They showed different relationships from each other with aging and daily physical activities.</p>

    Analysis of heart rate (HR) variability has demonstrated that decreased HR variability is increased risk for adverse clinical events. This is apparently inconsistent with the concept of critical slowing down that indicates an increase in slow fluctuation is an early warning signal of adverse events. In this paper, we propose a method for quantifying random component in HR fluctuation using autoregressive (AR) model analysis. Analysis of simulated data consisting of sinusoidal fluctuations and random noise revealed that the relative variance of AR residual to total variance reflects the relative amount of random component. Applying this method to actual HR time series data in 3604 subjects from 0 to 100 yr of age, we found that the ratio of random component in HR fluctuation increases at the beginning and end of life. We also found that the ratio of random component was markedly increased in heart failure patients with sinus rhythm.

    24時間心電図より得られる心拍変動指標は循環器疾患の死亡率の予測指標とされるが,多くの研究者が様々な指標を提案しており,それらが互いに独立した指標なのか,あるいは同じ特性を別の面から捉えた冗長性を孕む指標の集合なのかは不明である.そこで,洞調律を示す24時間心電図24万件のbig dataを用い,死亡リスク予測指標である,R-R間隔標準偏差,超低周波数成分,scaling exponent α1,deceleration capacity,非ガウス性指標λ25sの5指標の異常値出現率を調べ,指標間の関連が無い場合に複数の指標が重なる期待確率を求めて実際の観測値と比較した.一つも異常値のない人の期待確率は50.2%に対し観測値は60.5%で1.2倍,異常値を1つだけ持つ人は38.9%対24.8%で0.6倍,2つの人は9,9%対9.1%で0.9倍,3つの人は1.0%対3.9%で4倍,4つの人は0.04%対1.4%で34倍,5つ全て持つ人は0.0006%対0.3%で434倍であった.多くの異常値を持つ人の存在率が期待値よりも高いことから指標間の強い冗長性が示唆される.

    <p>Analysis of heart rate (HR) variability has demonstrated that decreased HR variability is increased risk for adverse clinical events. This is apparently inconsistent with the concept of critical slowing down that indicates an increase in slow fluctuation is an early warning signal of adverse events. In this paper, we propose a method for quantifying random component in HR fluctuation using autoregressive (AR) model analysis. Analysis of simulated data consisting of sinusoidal fluctuations and random noise revealed that the relative variance of AR residual to total variance reflects the relative amount of random component. Applying this method to actual HR time series data in 3604 subjects from 0 to 100 yr of age, we found that the ratio of random component in HR fluctuation increases at the beginning and end of life. We also found that the ratio of random component was markedly increased in heart failure patients with sinus rhythm.</p>

    To improve our quality of life, we need extend our healthy life expectancy (HALE) close to our average life span. Although reduction in daily physical activity has been proposed as a major cause that shortens HALE, convincing evidence for their association is lacking. In a physiological big data project named Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR), we developed a large database of 24-hr physical activities in 19,345 men (age, 65 ± 17 yr) and 23,985 women (67 ± 17 yr) who underwent Holter accelerometer and electrocardiogram monitoring in all over Japan. We examined if regional difference in physical activity is associated with the inter-prefecture ranking of HALE reported by the Japanese Ministry of Health, Labour and Welfare in 2013. Analysis of covariance adjusted for the effect of age revealed that the level of physical activity decreased progressively in the order of prefectures divided into upper, middle, and lower tertiles of HALE in both men and women (P <0.001 for both). When the subjects were divided into every 10s yr of age, the correlation between the rankings of physical activity and HALE was observed in 30s to 60s for men and in 50s to 60s for women. Our observations are consistent with the putative association between the level of daily physical activity and the length of HALE.

    Allostatic State Mapping by Ambulatory ECG Repository (ALLSTAR) is a physiological big data project in which 24-hr electrocardiogram and physical activities measured by three-dimensional accelerometer have been collected from all over Japan since 2008. In the present study, we analyzed the diurnal distributions of body postures in 19,345 men (age, 65 ± 17 yr) and 23,985 women (67 ± 17 yr) in ALLSTAR database and compared them with the inter-prefecture ranking of healthy life expectancy (HALE) reported by the Japanese Ministry of Health, Labour and Welfare in 2013. When the subjects were divided into upper, middle, and lower tertiles according to the HALE ranking of the prefecture they lived, the distribution of time in the lying position differed among HALE tertiles so that both go-to-bed and get-up times started earlier in prefectures in the upper tertile and delayed in the lower tertile. Our observations suggest that nocturnal life style may be associated with shorter HALE.

    本研究では,映像から得られる脈動情報であるイメージプレチスモグラム(IPG)の生成パターンが皮下の動脈走行状態によって決まるという仮説のもと,身体の各部位でみられるIPG生成パターンに基づいて個人認証を行う「動脈紋」認証の手法を確立することを目指している.本研究課題では,この「動脈紋」抽出に関する基礎的技術を確立すると共に,その認証精度について検証を行う.従来の生体認証が主に静止画像から捉えられる身体的特徴を用いるのに対し,本提案手法では経時的変化を伴うIPGの生成パターンから得られる身体的特徴を用いており,偽造が極めて困難であると考えられる. 初年度にあたる今年度は,先行研究で報告されている,手掌部において得られるIPGで生じる特異的な脈動領域である「ホットスポット」の存在を確認するため,白色光と近赤外光を照射光として用いた際の手掌部マルチスペクトル画像から緑色光画素と近赤外光画素によるIPGデータをそれぞれ取得し,これに周波数スペクトル解析を行って脈動成分を抽出した後,手掌領域内における脈動強度の空間分布を計算した.3人の被験者に対して分析を行った結果,先行研究と同様に,脈動が強く現れる箇所とそうでない箇所が存在し,特に緑色光画素における脈動強度分布において個人毎に異なるパターンが生じることを確認した. 次に,このIPG脈動強度分布パターンに認証として活用できる情報が含まれているか否かを調査するため,生体認証の基本要件である「普遍性」に着目して検証実験を実施した.実験では,複数の被験者から日付や時間帯を変えた場合のIPGを取得し,前述の脈動強度分布を求めた.その結果,測定回によらず,同一被験者の脈動強度分布では特に強い相関がみられることが確認された.
