    Background/Objectives: Malocclusion in primary dentition affects permanent dentition. However, the factors contributing to malocclusion in the oral cavities of children have not been fully elucidated. We hypothesized that environmental factors affect malocclusion in the primary dentition of the Japanese population and aimed to identify factors associated with anterior occlusion in primary dentition. Methods: The study involved 3793 parent-child pairs from the Miyagi Regional Centre as part of a supplementary survey to the Japan Environment and Children's Study, a cohort study. A questionnaire assessing oral development and environmental factors was administered to parent-child pairs who consented to participate. Parents assessed anterior occlusion when their children were 3.5 years old. Results: The maxillary primary central incisors tended to erupt earlier in the open bite group. Significantly more children in this group were breastfed until 1 year and drank ionic beverages at 1.5 years. In addition, thumb sucking or pacifier use was significantly common at 2 years of age. A correlation was observed between the mother's body mass index (BMI) before and after pregnancy and anterior occlusion. Conclusions: In the open bite group, the occlusion status of the anterior teeth at 3.5 years of age showed distinctive results influenced by the eruption period of the primary anterior teeth, oral habits, intake of sweetened beverages, and maternal BMI. These findings suggest that parental observation could be useful for screening children for malocclusion until the eruption of permanent dentition.

    BACKGROUND: The timing of primary teeth eruption is a visible indicator of infant physical growth other than body weight or height. It also reflects neurological integrity and development as well as nutrition, socioeconomic state, or underlying diseases. Therefore, the timing of primary teeth eruption is one of the major concerns for parents in health checkups for infants and children. However, the detailed developmental timing of teeth eruption differs depending on the survey methodology, country, or generation. We hypothesized that the timing of primary teeth eruption differs between the medical checkup by dentists and the daily records by parents. METHODS: We conducted a questionnaire survey on the date of eruption of primary teeth as an adjunct study among Miyagi Regional Center participants in the Japan Environment and Children's Study (JECS), a large-scale birth cohort study. A total of 1695 responses (3793 participants) were analyzed. RESULTS: The median ages of eruption were 7.1 months (male) and 7.6 months (female) for mandibular primary central incisors, 8.7 months (male) and 9.2 months (female) for maxillary primary central incisors, 10.0 months (male) and 10.3 months (female) for maxillary primary lateral incisors, and 10.4 months (male) and 10.8 months (female) for mandibular primary lateral incisors, which were earlier than the reported timings based on dental check-ups. Comparing the eruption time of preterm and term infants, the eruption time was earlier in preterm infants in the corrected ages. CONCLUSIONS: The eruption timing observed and described by the parents is earlier than that examined by dentists at regular check-ups. In addition to examining the primary teeth eruption of full-term birth children, we also examined that of preterm birth children because of the increasing number of premature births. To the best of our knowledge, this is the first report from a large cohort study to clarify the eruption time of primary teeth monitored by parents.

    Background Bone metastases in breast cancer patients are a common concern for medical doctors and dentists. Bone-modifying agents, which are necessary to prevent skeletal-related events (SREs), are associated with osteonecrosis of the jaw as an adverse side effect. Hypersensitivity to alcohol is an unfavorable response caused by deficiency of aldehyde dehydrogenase-2 (ALDH2) activity. Inactive ALDH2 is associated with osteoporosis, but its influence on bone metastases is unclear. The aim of our study was to evaluate the effects of alcohol sensitivity on bone metastases and SREs in primary operable breast cancer patients. Methods We retrospectively analyzed patients who were administered docetaxel, an anti-tumor agent, for histologically diagnosed breast cancer between April 2004 and September 2015. Alcohol sensitivity was assessed based on medical records of hypersensitivity to alcohol. The primary endpoint was time to bone metastases and the secondary endpoint was time to first SRE from the initial docetaxel administration. Data were stratified by alcohol sensitivity and tumor stages, and differences were estimated by the Kaplan-Meier method. Prognostic risk factors were analyzed by the multivariate Cox proportional hazards model. Results The median follow-up period of patients with high sensitivity to alcohol (n = 45) was 54 months and that for those with low sensitivity (n = 287) was 64 months. Stratification by alcohol sensitivity revealed that tumor stage exhibited significant correlations with the cumulative incidence of bone metastases in low-sensitivity patients; however, no differences were found in high-sensitivity patients. In multivariate analysis, alcohol sensitivity was a significant prognostic risk factor for bone metastases (HR 2.721, 95% CI 1.268–5.841, P = 0.010). Conclusion Alcohol sensitivity may be a prognostic risk factor for bone metastases. More detailed genetic investigations and metabolic analyses are needed.

    OBJECTIVES: Oral bacteria may contribute to postoperative infectious complications including postoperative pneumonia or surgical site infection. The aim of this study was to investigate the impact of preoperative dental care on postoperative outcomes among surgical patients under general anesthesia. DESIGN: Retrospective cohort study. SETTING AND PARTICIPANTS: We analyzed clinical records of major surgical patients at a university hospital between 2016 and 2018. Subjects were categorized into either the preoperative dental care group, those being referred to dentists by their surgeons based on an individual surgeon's judgment for dental care before surgery, or the control group. METHODS: The primary outcome was postoperative infectious complications. Secondary outcomes were postoperative inflammation markers (C-reactive protein and fever), and economic outcomes (postoperative length of hospital stay and medical expenses). As the main analysis, the average treatment effects of the preoperative dental care were obtained from the augmented inverse-probability weighting (AIPW) method with consideration of demographics and perioperative risk factors to estimate causal effect of the intervention from the observational data. Then, stratified analyses by age and surgical sites were conducted with the inverse-probability weighting and linear regression methods, respectively. RESULTS: In the AIPW estimation, compared with the control group, the care group saw a significantly lower rate of postoperative infection (average treatment effect -3.02) and shorter fever duration (-2.79 days). The stratified analysis by age revealed significant positive impact of dental care in all age groups, including the highest treatment effects observed among patients younger than 60. Also, treatment effect was observed in wider surgical sites than previously known. CONCLUSION/IMPLICATIONS: This study indicates a significant impact of preoperative dental care on preventing postoperative infection and inflammation. Along with old age or certain types of surgeries in which advantages of dental referral have been already known, preoperative dental referral could be beneficial for broader types of patients.

    PATIENT: A 33-year-old man diagnosed with extranodal natural killer cell/T-cell lymphoma, nasal type (ENKTCL-NT) inducing palatal perforation was referred to the perioperative oral care support center of Tohoku University Hospital for dental care to support cancer treatment including chemotherapy and hematopoietic stem cell transplantation (HSCT). Dental review during chemotherapy revealed mucositis suspected to be caused by mucosal trauma from altered jaw function (chewing and speech) due to palatal perforation. Although the patient was already in the cleanroom, an oral appliance as well as conservative care as recommended in oral management guidelines for HSCT were used to prevent worsening of oral mucositis at subsequent HSCT including High-dose chemotherapy and total body irradiation. After HSCT, a prosthodontist fitted a palatal obturator made by a dental technician and an oral surgeon reviewed the necrotic bone and removed the sequestra according to the changes in the palate. This approach involving a multidisciplinary team including a hematologist improved the impaired oral function and minimized oral complications . DISCUSSION: ENKTCL-NT and its treatment have a significant impact on patients' oral status. Hence, it is important to provide customized dental care based on previously endorsed guidelines according to the type of disease, treatment requirements, and oral and systemic status. CONCLUSIONS: This report indicated the importance of dental care with a customized plan before, during, and after HSCT for ENKTCL-NT with multidisciplinary supportive care for cancer patients to improve the impaired oral function and to minimize oral complications.

    Acetaldehyde is a potential carcinogen for esophageal cancer, and some oral microorganisms produce acetaldehyde from ethanol or glucose. In this prospective study, we examined the influence of professional oral care on acetaldehyde levels in mouth air of esophageal cancer patients. Acetaldehyde concentrations in mouth air and breath were measured by a portable gas chromatograph, and acetaldehyde production from oral microbiota was also evaluated. Samples were taken from 21 esophageal cancer patients (median age 68 years) and 20 age-matched healthy volunteers (control group) before and after oral care. Post-operative samples were also taken from 17 patients who had undergone surgery. All samples (mouth air, breath, and saliva) were collected 2 to 3 hours after lunch. Oral microbial samples were prepared from saliva. Genotype analysis of alcohol dehydrogenase 1B (ADH1B) and aldehyde dehydrogenase-2 (ALDH2) genes revealed no significant differences in the genotypes between the two groups. In the control group, acetaldehyde levels in mouth air showed no significant changes after oral care, while the amount of microbial acetaldehyde production from ethanol was significantly decreased. By contrast, among the patients, acetaldehyde levels in mouth air were significantly decreased after oral care and after operation, while the amount of microbial acetaldehyde production from ethanol showed no significant changes. Moreover, microbial acetaldehyde production from glucose was significantly decreased after operation. Overall, oral health was poorer in the patient group. In conclusion, professional oral care for esophageal cancer patients is effective for reducing acetaldehyde levels in mouth air due to the reduction of microbial count.

    Metabolites are sensitive indicators of moment-to-moment cellular status and activity. Expecting that tissue-specific metabolic signatures unveil a unique function of the tissue, we examined metabolomes of mouse liver and testis and found that an unusual metabolite, 2-hydroxyglutarate (2-HG), was abundantly accumulated in the testis. 2-HG can exist as D- or L-enantiomer, and both enantiomers interfere with the activities of 2-oxoglutarate (2-OG)-dependent dioxygenases, such as the Jumonji family of histone demethylases. Whereas D-2-HG is produced by oncogenic mutants of isocitrate dehydrogenases (IDH) and known as an oncometabolite, L-2-HG was the major enantiomer detected in the testis, suggesting that a distinct mechanism underlies the testicular production of this metabolite. We clarified that lactate dehydrogenase C (LDHC), a testis-specific lactate dehydrogenase, is responsible for L-2-HG accumulation by generating and analysing Ldhc-deficient mice. Although the inhibitory effects of 2-HG on 2-OG-dependent dioxygenases were barely observed in the testis, the LDHA protein level was remarkably decreased in Ldhc-deficient sperm, indicating that LDHC is required for LDHA expression in the sperm. This unique functional interaction between LDH family members supports lactate dehydrogenase activity in the sperm. The severely impaired motility of Ldhc-deficient sperm suggests a substantial contribution of glycolysis to energy production for sperm motility.

    BACKGROUND: Sports-related dental injuries, such as tooth fracture, loosening, and avulsion, are a major concern among young athletes because they directly impair oral function. Although the preventive efficacy of mouthguards has been well established, the prevalence of sports-related dental injuries remains high among young athletes. The aim of this study is to identify the variables contributing to the risk of sports-related dental injuries by conducting a survey on large population of young athletes in Miyagi prefecture. METHODS: A cross-sectional study was conducted with school-aged athletes (aged 6-15 years, n = 5735) using a self-reported questionnaire. The questionnaire examined general variables, including sex, age, and body mass index; sports-related variables, including sports-type, team level, activity schedule, break time, and verbal/physical abuse by coaches; and lifestyle variables related to free time, including screen-time and sleep duration. Their associations with sports-related dental injuries were examined using multivariate logistic regression models. RESULTS: The prevalence of sports-related dental injuries was 13.3% (763 of 5735 young athletes) and was higher in males (14.3%, 592 of 4132) than in females (10.7%, 171 of 1603; adjusted odds ratios [ORs] and 95% confidence intervals [CIs]: 1.48 [1.22-1.79], p < 0.001). After stratification according to sex, significant associations with the prevalence of sports-related dental injuries were evident for three variables-insufficient break time, verbal abuse, and physical punishment-in males (adjusted ORs [95% CI]: 1.35 [1.03-1.77], p = 0.032; 1.31 [1.05-1.62], p = 0.015; and 1.36 [1.06-1.75], p = 0.016, respectively) but not in females (adjusted ORs [95% CI]: 0.88 [0.53-1.47], p = 0.623; 1.29 [0.87-1.91], p = 0.206; and 0.97 [0.57-1.63], p = 0.894, respectively). CONCLUSIONS: Although our results might be based on the individual athlete's self-perception to the sports-related variables, our results suggest that insufficient break time, verbal abuse, and physical punishment from coaches are positively associated with the prevalence of sports-related dental injuries in young male athletes.

    We constructed a new dental examination using standardized examination criteria and data analysis systems in order to assess the status of caries experiences of preschool children in S city. In 2007, clinical dental examinations on the new system were carried out at total of 118 nursery schools and 106 kindergartens in S city. A total of 23,222 childrens data (10,432 nursery school children and 12,790 kindergarten children) was obtained, and caries prevalence and status of treatment were compared between nursery school children and kindergarten children. The following results were obtained. 1. Percentage of children with caries was 29.6% for 3-year-olds, 44.1% for 4-year-olds, 56.1% for 5- year-olds in nursery schools, whereas it was 24.3% for 3-year-olds, 39.8% for 4-year-olds, 52.0% for 5-year-olds in kindergartens. Those were significantly higher in nursery schools than in kindergartens. 2. Mean dft values per child at 3 to 5-years-old in nursery school children were also higher than those in kindergarten children. 3. Trend toward decrease in caries prevalence was showed in both nursery school children and kindergarten children, but it was 2 to 3 years behind the national trend. 4. Percentage of children with caries experience and without decayed teeth and percentage of treated teeth at each age of 3 to 5-years-old in nursery school children was significantly lower than those in kindergarten children. 5. Percentage of treated teeth for all age children in both nursery schools and kindergartens except 5- year-old children in kindergartens was far lower than the percentage of decayed teeth. These results demonstrated that both nursery school children and kindergarten children in S city still showed high caries experiences and nursery school children had higher caries prevalence and more untreated teeth than kindergarten children.

    In this study, the use of fluoride toothpaste was surveyed by questionnaire in 2,188 subjects aged between 1 and 56 years in Sendai City, Miyagi Prefecture, for the purpose of the promotion of its use. The subjects were categorized as 1-3 year-old children, 4-5 year-old children, primary school children, their parents and vocational school students. The survey was carried out from April to June 2000 using a questionnaire prepared by the Committee for the Promotion of Fluoride Use of the Japanese Society for Dental Health, and the results were as follows : 1. Tooth brushing was performed once a day or more in 88.8% of the subjects. The percentages of the daily use of fluoride during brushing in 1-3 year-old children, 4-5 year-old children, primary school children, vocational school students and the parents were 13.4, 34.0, 52.3, 55.3 and 66.5 (%), respectively. 2. The parents and vocational school students used a larger amount of toothpaste and rinsed more frequently than younger subjects. There was a significant difference in the frequency of mouth rinsing after brushing among the groups, dependent on the amount of toothpaste used. 3. The first reason for toothpaste selection was the preventive effect on dental caries and the second was inclusion of fluoride. For infants, most of the parents did not use toothpaste because of the anxiety of swallowing toothpaste. The vocational school students selected toothpaste primarily because it was available at home. Our findings suggested that dental professionals should prepare guidelines for appropriate use of toothpaste and should recommend the use of fluoride-containing toothpaste to increase exposure to small amounts of fluoride.

    We previously identified an IS200-like sequence (ISAa1) in the genome of Actinobacillus actinomycetemcomitans FDC Y4. One or more hybridizing bands to the ISAa1 probe were detected in each of several reference strains, representing three of the serotypes (a through c) of A. actinomycetemcomitans. In this study, we examined whether a restriction fragment-length polymorphism (RFLP) with ISAa1 as a probe could differentiate clinical isolates. One or more hybridizing bands were detected in each of the 27 strains examined, which could be divided into seven groups according to restriction fragment-length polymorphism pattern. Several strains were observed with identical restriction fragment-length polymorphism types but with different serotypes. Conversely, strains were also observed with differing restriction fragment-length polymorphism types and identical serotypes.

    The purpose of this study was to investigate the effect of periodontal treatment and maintenance therapy of 10 years duration. The study population consisted of 1 juvenile, 2 rapidly progressive and 43 adult periodontitis patients. The average age of the patients was 42.3 years at the initial visit and the average duration of observation was 14.1 years. At the initial and most recent maintenance visits, the number of remaining teeth and the interproximal bone levels on X-ray films were measured. The average number of remaining teeth was 27.4 at baseline and 24.7 at final follow up. The total number of lost teeth during observation was 124, that is 2.7 per individual. Four patients had lost more than 0.6 teeth per year, 5 patients had lost 0.3 to 0.6 teeth and 37 patients had lost less than 0.3. Twenty maxillary second molars and 18 mandibular third molars were extracted. The extractions were commonly located in the molar region and bilaterally symmetrical patterns of extraction were observed. Bone loss was observed in some regions of the mouth at the most recent visit. However, our data suggest that, despite slight bone loss, periodontal therapy was effective if maintenance therapy was correctly performed and that such treatment modalities facilitate reaching the goal of the "8020" movement.

    The effect of paraformaldehyde-fixed murine macrophage P388D1 cells stimulated with Actinobacillus actinomycetemcomitans Y4 lipopolysaccharide (membrane-associated interleukin-1) on osteoclastic bone resorption was investigated. Both the number of osteoclasts and the bone resorption surfaces increased considerably along the inside of the marrow space of mouse calvaria when membrane-associated interleukin-1 mediated by A. actinomycetemcomitans Y4 lipopolysaccharide were injected into the subcutaneous tissues overlying the central calvaria of 4-week-old C3H/HeJ mice. Membrane-associated interleukin-1 caused hypercalcaemia when injected into C3H/HeJ mice twice a day for 2 days. The calvaria of C3H-HeJ mice injected with membrane-associated interleukin-1 contained less mineral than those of the control mice. These results suggest that membrane-associated interleukin-1 on macrophages stimulated with A. actinomycetemcomitans Y4 lipopolysaccharide plays an important role in inflammatory osteoclastic bone resorption in periodontal diseases.

    ? Springer 2012. Difference of breath acetone between type 1 and type 2 diabetes mellitus (DM) is examined under informed consent. We measure acetone in mouth air and in deep breath by Gas Analyzer? XG-100V (New Cosmos Electric Co., Ltd., Osaka, Japan) and volatile sulfide compound in mouth air by Breathtron? (New Cosmos Electric Co., Ltd., Osaka, Japan), after meal. The acetone ratio of deep exhaled breath to mouth air and concentration of volatile sulfide compound (VSC) are com- pared between type 1 and type 2 DM patients. The acetone ratio of deep exhaled breath to mouth air is significantly higher in type 1 DM than type 2 DM patients. The concentration of VSC shows no significant difference. Though more cases should be studied, there is a possibility that the acetone ratio might reflect metabolic disorder of type 1 DM patients.

    抗体修飾マイクロバブルと超音波の併用による抗癌剤導入法の有効性の解析:抗体修飾マイクロバブルの作製は、既にプロトコール化されている。同法によるブレオマイシンの導入後、培養48時間後にWST-8による生細胞数の評価を行ったところ、BLM単独群やマイクロバブルを使用したBLM導入群、またIgG修飾マイクロバブルを使用したBLM導入群では、そこまで細胞数の減少は認められなかったが、抗EGFR抗体修飾マイクロバブルを併用したBLM導入群では有意に生細胞数が減少した。 また,一般的な2次元培養下の細胞は組織環境が生体内と大きく乖離している。口腔癌細胞においても薬剤に対する感受性や増殖様式は生体内の挙動が異なると考えられる。細胞同士が凝集した球状の集合体(スフェロイド)を大量に作製可能なデバイスを用いて、口腔癌組織の環境を模倣した3次元培養を行い、同法による癌治療の開発に生かせないか検討した。HSC-3とCa9-22をそれぞれ培養し、非接着処理を施したマイクロウェルチップ上に播種した。形成されたスフェロイドの形態観察と直径の計測およびPCRによる遺伝子発現の解析を行った。デバイス上に播種したHSC-3とCa9-22は、培養5日目にかけて徐々に凝集し、直径も短縮化した。また、幹細胞マーカーのCD44・Oct4・Nanog・Sox2と低酸素性マーカーVEGFの発現量は、スフェロイド群では2次元培養群と比較していずれも亢進していることが確認された。LIVE/DEAD染色では培養5日目においても、スフェロイド内部は生細胞を主体に構成されていることが示された。回収したスフェロイドは2次元培養下で定着・増殖し、幹細胞マーカーの発現は維持されていた。専用デバイスを用いた口腔癌スフェロイドは生態環境に近い培養法であることが示唆された。

    In the physicochemical function analysis of saliva, cross-sectional analysis of stimulated saliva collected at the time of adult oral health examination and component analysis of resting saliva with different viscosity in the same person were performed. In salivary components, various glycoproteins play an important role in determining the oral environment. Our study indicated that several salivary proteins in the high and low molecular weight might be associated with general health condition in stimulated saliva. In addition, in a longitudinal survey of saliva during radiotherapy of head and neck cancer, changes in the physical properties of saliva and the concentrations of polysaccharides and glycoproteins were observed. From the obtained results, we tried to prepare artificial saliva that approximated the physical and biochemical properties of saliva, and considered the factors necessary for the reconstruction of the oral environment.

    Bone is the most frequent metastatic site of breast cancer, and metastases cause a poor quality of life due to bone pain, pathological fracture, hypercalcemia, and spinal cord compression. Bone metastases in breast cancer patients are a common concern for medical doctors and dentists. Bone-modifying agents, which are necessary to prevent skeletal-related events, are associated with osteonecrosis of the jaw as an adverse side effect. Prognostic risk factors of bone metastases are important for predictive oral supportive system for breast cancer patients. Univariate analysis and multivariate analysis were performed using the Cox proportional hazards model to investigate associations between prognostic variables and time to bone metastasis in primary operable breast cancer patients. In multivariate analysis, tumor stage and alcohol sensitivity were significant prognostic risk factors for bone metastases.

    About two-thousand participates of large-scale birth cohort survey were analyzed about the health-related factors, such as environmental, hereditary, social and lifestyle factors. In the same time, we analyzed the behavior and the attitude of parents for oral health care and promotion. Furthermore, we developed the new method to estimate the risk of dental caries of deciduous tooth, which was suitable for mass examination. From these results, we established the sustainable research base and fundamental data of long-term cohort survey continuing more than ten years.

    It is very important to prevent or reduce adverse events caused by the radiotherapy and chemotherapy for completing the treatment plan. Especially, the adverse events of head and neck cancer caused by radiotherapy are very severe. For instance, patients complain dry mouth and oral mucositis so on. Oral mucositis cause severe pain and reduce patient’s quality of life. It is very hard for patients to eat or talk due to the pain caused by oral mucositis. So it is requested to establish the methods inhibit oral mucositis during radiotherapy. In this study, we proposed the possibility to reduce the severity of oral mucositis using Hangeshasinto, which is one of traditional medicine in Japan in clinical study. Also we provide the mechanism how Hangeshasinto prevent oral mucositis by using cell culture experiments.

    Acetaldehyde is a powerful carcinogen in thoracic esophageal cancer. The aim of this study is to evaluate perioperative change of mouth acetaldehyde and its causal oral bacteria in thoracic esophageal cancer patients under intensive oral care. Analysis was done compared to control cases. Drinking and smoking histories were longer and oral condition was worse in patients group than in control. Acetaldehyde in mouth air of thoracic esophageal cancer patients decreased after oral intensive care before operation. It also decreased after operation. It is suggested that intensive oral care for perioperative thoracic esophageal cancer patients decreases acetaldehyde produced in oral microbiome.

    NaF Positron Emission Tomography (PET) can identify subtle functional variation prior to the major structural change detectable by X-ray. NaF-PET/CT scan was performed to examine the change of bone metabolism in the residual ridge underneath denture before and after the RPD treatment. The resultant mechanobiological response was analyzed using a 3D finite element (FE) model derived from the computerized tomography (CT) images of a corresponding patient under the occlusal loading. Mechanobiological stimulus determined from the FEA are quantified and compared with the standardized uptake values (SUV) of PET. The increased application of occlusal force after RPD insertion induced higher mechanical stimuli in the residual bone, and SUV increased in the region of residual ridge with higher mechanical stimuli. SUV had clear correlation with mechanical stimulus in the cancellous bone, especially after RPD insertion (R2 > 0.8, p < 0.01).

    To prevent the oral mucosal problem and to enhance the promotion of oral-related QOL, a preventive method of oral mucositis and mucosal injure was built for the elderly suffering xerostomia and for the patients who were undergoing perioperative medical treatments. The updated technology of visual examination of mucosa, so-called Narrow Band Imaging, was employed in this system to visualize the abnormality of oral mucosa. The oral mucosa mapping system which covered whole oral cavity was also developed. By using these two systems, new diagnostic method of oral mucosa was proposed to prevent oral mucositis. Finally, the comprehensive guideline of diagnosis and treatment of oral mucositis were discussed.

    This study aimed to clarify the whole picture of metabolic systems of oral biofilm through the metabolome analysis and metabolic activity measurement, and to consider an assessment of oral health and disease using oral microbiome metabolism. The CE-MS metabolome system enabled to analyze the metabolites associated with carbohydrates and amino acids comprehensively and revealed the whole picture of metabolic systems of carbohydrates and amino acids in the oral biofilm. In addition, the metabolic activity of a tiny amount of oral biofilm was successfully measured using the alamarblue system. Furthermore, the metabolic activity was diverse among sample sites and metabolic substrates, suggesting that these metabolic characteristics of oral microbiome can be used as an indicator of oral health and disease.

    口腔・頭頚部がんに対する放射線治療では、粘膜や皮膚の炎症、唾液分泌障害、嚥下障害、味覚障害など様々な副作用を生じ、一旦出現すると放射線療法中は悪化の一途をたどる。特に唾液腺は放射線感受性が高い臓器あり、照射線量が多くなるに従って唾液分泌量が低下し、40Gy 以上の線量では唾液腺が不可逆に障害され、照射終了後も重度の口腔乾燥を生じるといわれている。重度の口腔乾燥により唾液による自浄作用を失った口腔内では、歯牙自体が乾燥により脆くなることも相まって齲蝕が多発する。がんに対する放射線治療を受けた患者に対するフッ化物応用を含めた齲蝕予防法は確立しておらず、口腔乾燥に対しては保湿剤を使用し、スポンジブラシ等も併用して試行錯誤しながらできる限り口腔清掃を試みているのが現状である。一方、歯質強化にはフッ化物応用が有効であると考えられるが、重度の口腔乾燥では従来のフッ素の応用法では齲蝕予防のコントロールが難しく、より積極的な用法が必要と思われた。 本研究は、ナノ・マイクロバブルと超音波を用いた歯質へのフッ素導入法の開発を目的としている。平成27年度においては、ヒト抜去歯を用いるため東北大学大学院歯学研究科研究倫理委員会に倫理申請書を提出した。倫理委員会からの承認を得る間、作製予定のフッ化ナトリウム封入ナノ・マイクロバブルと同じナノ・マイクロバブルに蛍光色素を封入し、蛍光色素封入ナノ・マイクロバブルを作製、同ナノ・マイクロバブルの生体内での物理学的性状を検討し、フッ化ナトリウム封入ナノ・マイクロバブルの作製法の基礎的技術を確立した。

    To evaluate the recovery level of the craniofacial and oral functions after the treatment of the head and neck cancer, new estimation approach for healing of soft tissues, which had created by the surgical operation, had been studied. The index of oral functional rehabilitation was composed with the measurement values of oral functions and surgical wound healing.     This study was designed to elucidate the effect of periodontal bacteria on the progression of aortic aneurysm by inspecting the specimens of aortic aneurysm surgery. So far, several aortic samples were examined of long-term culturing, followed by qPCR of 16sRNA for the identification of the bacterium.     Saliva has important roles in maintaining the environment of the healthy mouth and in supporting the smooth activities of many oral functions in daily lives. When the secretion of saliva was reduced, the quality of life (QOL) related to the mouth would be heavily decreased, because of the increasing the risks of oral diseases and the dysfunctions of nutrition intake and difficulty in speaking.     Novel diagnostic method and preventive treatment system of oral mucosa had been developed based upon the evaluation of the degree of injury and atrophy, to prevent oral problems of the elderly and side effects of cancer treatment. For this purpose, we established the haptic diagnostic devices of oral mucosa with the technology of ultrasound sensing. Using this device and conventional evaluation systems of oral environment, we analyzed the symptoms of injured oral mucosa, and objective estimation of damage on oral mucosa had advanced to utilize for the clinical applications.     Metabolome analysis for oral biofilm and saliva using CE-TOFMS was established. This method made it possible to analyze not only sugar metabolism but also amino acid metabolism in oral biofilm and even in saliva, where only small amount of metabolites (about 1/100 of oral biofilm) was contained. In addition, the analyses of the effect of sugar alcohols and fluorides on the biofilm metabolism suggested close coordination between sugar and amino acid metabolisms. Metabolome profiles of sugar and amino acid metabolisms can be used for risk indicators of dental caries and periodontitis/oral malodor, respectively.

    We analyzed gasses from exhaled and mouth air and saliva of perioperative patients of lung cancer at bedside before oral care, after oral care before operation, and after operation. Protective reflexes (cough and swallowing reflex) were also measured before and after operation. As for patients without postoperative pneumonia, oral care before operation increased ratio of salivary streptococci, usually known as healthy-associated bacteria, in anaerobic condition. Oral care also decreased total number of bacteria in saliva and decreased malodorous gasses. Acetone concentration increased after operation with rapid increase of CRP. This study suggests possibility that non-invasive measuring volatile sulfide compounds and volatile organic compounds at bedside indicate oral environments and general inflammatory conditions of perioperative lung cancer.

    The evaluation method of the effectiveness of biofilm removal was innovated and fundamental bases of researches and applications of novel functional strings were developed. Clinical application of novel functional strings, such as dental floss or swab sticks, revealed the high performance of biofilm removal and high biological compatibility with human oral mucosa. These results indicated the possibility of wide ranges of clinical application of novel functional strings, not only for oral hygiene but also for general medical use. The debridement method by using novel functional strings might be one of the best cleaning methods for every surface of human bodies, including soft and hard tissues.

    Clinical diagnosis of initial regions of dental caries is important for oral health promotion and prevention of dental caries. To development of diagnostic devices of tooth surfaces condition, including the initial caries regions, we utilized the ultrasonic mineralization measurement system of hard tissues with ultrasonic technology of haptic sensor. In the clinical use of the ultrasonic mineralization measurement system, the thermal protective equipment was needed for the stabilization of readout, which was sensitively influent to the thermal shift of the sensing oscillator. Furthermore, for the data management of mass oral health check-up, the dental chart input method system was developed. Thus, the evaluation system of caries risk assessment was established.

    According to the modification of the Industrial Safety and Health Law and Ordinance on Prevention of Hazards due to Specified Chemical Substances , the administrative level of the formaldehyde vapor concentration in working space was changed from 0. 25 ppm to 0. 1 ppm on administrative action to the evaluation of the risk of occupational diseases. In endodontic treatment, formaldehyde is commonly used for the intracanal dressing as a compound. The authors measured the formaldehyde vapor concentration in the nearby air of the simulated intracanal dressing of Formalin Guaiacol (FG). The results indicated that the root canal dressing of FG produced the formaldehyde vapor at the acceptable concentration designated by the law. There was a difference between the vapor concentration of the air collected in the summer and that in the winter. The use of dental vacuum was advocated during the root canal dressing of FG in order to decrease the vapor concentration .

    We studied acetone in exhaled air, volatile sulfur compounds and acetone in mouth air, to analyze relation between general condition and oral environment of patients with diabetes mellitus, with healthy volunteers as control. Acetone in exhaled and mouth air of type 2 diabetes patients decreased with improvement of plasma glucose during hospitalization. Study of healthy volunteers with good oral condition showed significantly strong correlation between acetone in exhaled air and acetoacetic acid or 3-hydroxybutyric acid in plasma after fasting overnight. There was no significant correlation between acetone and volatile sulfur compounds. This study suggests possibility that non-invasive acetone measuring indicates ketone bodies in plasma, which reflect pathologic condition of diabetes mellitus.

    Ultrasonic measurement system for mineral density of tooth surfaces had been developed. This system actualized significantly sensitive, non-destructive, and quick measurement. Using this system, we analyzed the relationship between mineral density of tooth and oral environment, especially function of saliva. The result revealed that acid resistivity was correlated with ultrasonic density of tooth mineral. This indicated the possibility of developing new preventive program against tooth decay according to the ultrasonic density of tooth mineral, buffering capacity of saliva, and living practice of daily life.

    パウダージェットデポジション(PJD)法を応用して歯質上に歯の主成分であるハイドロキシアパタイト(HAp)膜の生成し,歯質-生体材料間に全く新しいインターフェイスの創成を実施した。エナメル質の成膜に関しては、PJD装置、HAp微粒子,噴射条件等を検討してきた結果、小窩裂溝内への成膜が確認された。特にPJD装置では、実験用装置ではなく、ハンドピース型装置での成膜が可能になっており、実際の治療に準じたハンドピース先端の噴射ノズルの速度、角度等の条件を自由に設定することにより、より良い成膜条件の検討を可能とした。様々な粒子半径、焼成温度のHAp微粒子を使用し、成膜に適した条件(粒子半径3-4μm,焼成温度1200℃)を明らかにした。さらに、HAp膜の象牙細管透過性へ与える影響を中心に評価を行ったところ、HAp粒子が象牙細管に入り込み、象牙細管開口部を閉鎖し、象牙細管透過性が低下することが判明した。また、条件を変化させることで、PJD装置をそのまま利用して、歯質を切削するアブレイシブジェット加工も行えることが判明した。これらの結果から、今回申請した方法で、軟化象牙質と脱灰エナメル質の選択的除去し清掃された硬組織の実質欠損部分に、PJD法にてハイドロキシアパタイトの歯質と一体化したハイドロキシアパタイト膜を形成させて象牙細管の封鎖と歯髄の保護を行う裏層を施し、その上で充填術を施す全く新しい治療法の可能性が示唆された。

    Metabolome analysis for the representative plaque bacteria and human oral biofilm using a capillary electrophoresis equipped with a time-of-flight mass spectrometer was established. By this method, a new finding was obtained about the pentose-phosphate pathway and the Kreb's tricarboxylic acid cycle, as well as the Embden-Meyerhof-Parnas pathway. In addition, the metabolome analysis was applicable to a very small amount of oral biofilm. These results suggest that the metabolome data can be used as an indicator of oral health status.

    歯肉炎の診査は、発赤・腫脹等の視診による方法、歯周ポケット検査、プロービング時の出血等による方法を使用されることが多い。特に歯肉炎の診査では、若年者が対象であることが多く、侵襲性の少ない診査方法が好ましく、かつ客観性の高い診査方法であることが重要である。 本研究は、トノメーター(i-care^<TM>)改造し、侵襲性の少ない歯肉トノメーター(以下トノメーター)として用い、歯肉炎診査の有用性を検証した。 対象は、健康と思われる成人男性8名(平均年齢24.5歳、ragne:23-27)の可及的健康と思われる前歯部歯肉である。測定対象となる患者からは、前もってインフォームドコンセントを書面で得た。測定装置としてトノメーター(i-care^<TM>)改造し、歯肉トノメーターとして用い、対象者の上下顎前歯部頬側歯間部歯肉の硬度を測定した。対象部位の歯肉溝の深さおよびBOPも同時に測定した。 健常歯肉の硬さは、平均40.8±15.6mmHgで、上顎が下顎より硬いことが示された。歯肉硬さと歯肉溝深さとの相関も示された。 しかし改良型のトノメーターの測定条件からさらなる改良が必要となったことで、再度改良を施した。また、これにより再度測定方法を解析する必要があると考えられ、さらなる研究の必要性が示唆された。

    本年度は、昨年度に引き続き、口臭原因菌の一つと捉えられる口腔Veillonella属の硫化水素産生活性について検討し、(1)環境因子、とくに口腔内に多く存在する乳酸等の有機酸が硫化水素産生能に与える影響、及び、(2)硫化水素産生に関わる代謝酵素の検索を行った。培養したVeillonella atypica、 V. dispar、 V. parvulaの各標準株(ATCC株)の培養菌にシステインを加えて3時間インキュベートし、産生した硫化水素をメチレンブルー法で定量した。さらにこの反応系に乳酸等の各種有機酸を加え、硫化水素産生活性への影響を検討した。また、培養菌を超音波処理して無細胞抽出液を調整し、システインからの硫化水素産生に関わる酵素活性を検索した。Veillonellaのシステインからの硫化水素産生活性は、乳酸が存在すると著しく増加した。Veillonellaの無細胞抽出液は、システインから硫化水素を産生する酵素活性を示したが、無細胞抽出液に乳酸を添加しても活性の増加は認められなかった。以上のことから、Veillonellaは乳酸の存在下、システインから効率よく硫化水素を産生することが明らかになった。乳酸による促進作用は、酵素の活性化作用ではなく、システインの取込み促進であるものと予測された。口臭源の一つである硫化水素は口腔細菌によって産生されるが、その産生活性はpH(昨年度成果)や有機酸(今年度成果)という口腔環境によって影響を受けるものと考えられ、口腔環境のコントロールによって口臭を抑制し得るものと考えられる。

    The literature is replete with evidence supporting that human dental caries, an infectious disease of bacterial origin, can be preventable by mucosal immunization. Recently, significant genetic contribution to caries progression and severity was reported in a twin study. However, it has not been reported to confirm the association of certain immune related genotypes with caries susceptibility. The purpose of the present study was to confirm the association of certain immune related genotypes with caries susceptibility in Japanese individuals. Human leukocyte antigens (HLAs) play an important role in regulating the immune system. A total of 104 children aged 10 yrs (48 children with caries and 56 children without caries) were included to examine the susceptibility to early onset caries. The HLA・DQB1*03/*05 genotypes were significantly associated with dental caries susceptibility (P=0.005, Pc=0.004). The DRB1*04/*15 genotypes were significantly associated with dental caries susceptibility in a total of 135 children aged 13 yrs (75 children with caries and 60 children without caries) (P=0.0045, Pc=0.04). The HLA-DQB1*06/*06 genotypes were significantly associated with dental caries susceptibility in a total of 164 subjects, aged 26 yrs or younger (mean age, 20.4 ±2.2, 98 adolescents with caries and 66 adolescents without caries) (P=O.0027, Pc=0.027). In conclusion, this study using a cohort of a 10-year old, 13-year-old children, and adolescents with and without caries demonstrated the possible effect of specific HLA genotype on dental caries progression.

    平成18年度に製作したレーザー光による歯の表面性状解析装置は、読み取り装置に小型CCDエリアセンサと特殊プリズムを応用することにより、臨床応用可能なレベルに小型化したが、実際には歯の表面の曲率に反射光の光軸が大きく影響を受けて、受光面の面出しが容易でなく、システム全体の再考を迫られた。これを解消する目的で反射光のスペックルをスクリーン投影法にて検出すると共に焦点距離を計測する仕組みを加えた装置を組み立て直した。情報処理装置は開発したプログラムを走らせる小型の可搬型コンピュータを用い、歯科健診の会場や日常臨床で使用できる形態に近づけた。さらに、臨床で使用する場合のキャリブレーションを行う標準切片として、各種の研磨法を用いたエナメル質切片を用意した。 う蝕の進行リスクの診断として用いるための、う蝕モデル歯は、倫理規定に基づいて収集した人抜去歯を用い、他のう蝕診断機器を併用してう蝕の進行度を計測しながら化学的実験的平滑面う蝕を製作し、エナメル質表層の性状の変化を計測した。モデル歯に関しては、臨床から得られた無加工のエナメル質は鏡面に近いスペックル値を示すが、実験室で研磨したエナメル質は研磨によってできた傷によるスペックルの乱れが観察され、これは脱灰課程の各切片において変動が大きかった。さらにこの変化は脱灰の方法でも大きく影響が現れた。これらの変化を臨床に応用する場合は、実際の臨床的データとその症例の転帰をあつめた更なる長期的な追跡データベースを構築する必要性が示された。

    There are no standardized diagnostic criteria of root caries, which occur on the exposed root surface of tooth suffered with periodontitis, especially in elderly people. This absence of evaluation method of decalcification level and mass of soften root surfaces hamper the epidemiological and preventive research of root caries developments. In this study, we evaluated optical measurement with laser and ultrasonic measurement for detecting the mineral loss of root surfaces. Ultrasonic sensor system developed in this study was non-destructive, and small enough to handle in oral cavity. The round shape of front edge of sensor prevents to damage the tooth surfaces during ultrasonic measurements. Furthermore, ultrasonic sensor was placed onto the constant contact-force generator. Then repeatable ultrasonic measurement of decalcification by hand manipulation was achieved. This decalcification detection system of root surface showed reproducible results in considerable clinical conditions, when calibrated with the standards specimen of decalicification. On the other hand, the laser determination method of decalcification was not efficient because root caries and calculus could not be distinguished by using laser fluorescence. These results indicated the diagnostic system of decalcification level of root surfaces will be available by using newly developed non-destructive ultrasonic sensor system. By using this system, the researches of root caries development and preventive method will be proceeded and help to maintain high quality of oral health in elderly people.

    Periapical bone resorption occurs following infection of the dental pulp. Bone destruction has been linked to excessive expression of inflammatory cytokines, particularly interleukin (IL)-1. IL-10 is an anti-inflammatory cytokine, produced by various immune cells including macrophages and T2 cells, that we previously found was a key endogeneous inhibitor of infection-stimulated peripheral bone resorption in vivo, likely acting via IL-1 inhibition. More recently, we have shown that IL-10^<-/-> mice are also extremely susceptible Porphyromonas gingivalis (Pg)-stimulated periodontal bone loss, without an increase in gingival IL-1 levels. In contrast, no significant bone loss is induced by the Pg infection in corresponding wild-type (WT) mice. In the present study, we will employ the IL-10^<+/+> mice model, a ‘reverse‘ model of IL-10^<-/-> mice, to test the hypothesis that induction and progression of apical periodontitis can be ameliorated by modulating IL-10, IL-10 receptors their signaling, and cytokine network that IL-10 regulates. The dental pulps of the first molars were exposed and infected with a mixture of four common endodontic pathogens, and bone destruction was determined by micro-computed tomography at sacrifice on day 21. The results demonstrate that IL-10^<+/+> mice model had no significantly greater infection-stimulated bone resorption in vivo compared with wild-type mice, although IL-10 in alveolar bone of them was over-expressed compared with that of wild-type mice.

    本年度は、1.前年度設計した口腔細菌特異的プローブを作成し、DNAチップへの搭載の可否(実用性)を検討し、2.DNAチップへの搭載が可能と考えられた24種のプローブを用いて口腔微生物叢解析用DNAチップを試作し、口腔微生物種特異性、使用法などを検討した。 1.DNAチップ搭載用口腔細菌特異的プローブの作成: プローブの特異性についてはGeneBankなどの既存のDNAデータベースから、プローブのDNAチップへの搭載の可否についてはDNAチップ作成手順などの実用性から検討し、その結果、24種の口腔微生物に対する特異的プローブが完成した。さらに、この1年間で新たに入手した口腔微生物16S ribosomal RNA遺伝子配列データに基づき、新たなプローブを設計・作成中である。 2.DNAチップ搭載用の24種の口腔微生物特異的プローブを用いた口腔微生物叢解析用DNAチップの試作: DNAチップ作成共同研究企業にて、24種の口腔微生物特異的プローブを搭載したDNAチップを試作した。各プローブに該当する標準菌株由来DNAを用いて検討したところ、試作したDNAチップは24種の標準菌体由来DNAを特異的に検出することができることがわかった。さらに、DNAチップ上のスポットは肉眼で明瞭に判別でき、市販の可視光用透過型スキャナーで計測・記録することが十分可能であることが分かった。 現在、実際の臨床サンプルを用いて検討中であり、臨床サンプルに含まれる24種の口腔微生物の従来法による検出(通常のnested PCR法での検出)を終え、DNAチップによる検出との照合を進めつつある。

    The onset of dental caries is characterized by white-spots or staining on tooth surfaces. However, estimation of caries activities of these lesions is difficult because we have no tool for recognition which lesion is demineralizing, arresting or re-mineralizing. For detection of the initial changes of the tooth surface affected by dental caries on the enamel and root, we developed a noninvasive, nondestructive dental explorer employing ultrasonic sensor technology. The ultrasonic sensor detection system consists of an oscillating piezoelectric element for sensing, a piezoelectric detective element to obtain frequency characteristics of the tooth surface, and a lightweight arm for positioning the sensor on the tooth surface. Extracted teeth were collected with informed consent and demineralized enamel specimens were prepared with lactate-CMC gel (6%, pH 4.5). The ultrasonic sensor was loaded on these tooth surfaces and scanned for drawing a map of the frequency characteristics. The surface characteristics were represented by the frequency shifts of oscillation in the resonance method. The detecting element of the developed ultrasonic sensor was sufficiently small in the clinic. By measurements using four sets of enamel specimens, early lesions of enamel caries were dearly represented by low frequency shifts of oscillation in a demineralized time-dependent manner. By scanning surfaces of ten extracted teeth, this detection system was able to distinguish sound or caries lesions on the enamel and roots, calculus depositions, staining, white spots and cracks on the enamel surface. However, additional information was need when exploring complex surfaces with overlapping lesions. Our new sensory explorer employing the ultrasonic resonance method was found to be a powerful tool for estimating initial change and degree of progression of dental caries.

    In the process of developing dental caries on tooth surfaces, characteristic patterns of mineral loss of enamel were observed ahead of collapsing the tooth surface and forming the caries cavity Although this process is affected by microenvironments around the tooth surface, it is difficult to analyze these in detail because of its size and complexity These microenvironments are considered to be determined by several factors, including the presence of microbes and saliva throughout the oral cavity. To understand the microbial factors, we analyzed the interrelationship among the microbes covering the tooth surfaces and host factors which affect the ecology of dental biofilm. Furthermore, to understand the environmental factors, we analyzed biological and physical characteristics of saliva, which play an important role in the microenvironment of the tooth surface. The secretion rates and physical properties of saliva are key factors in the nature of oral circumstances. Thus, physiological properties of saliva, such as viscosity and spinnbarkeit, were analyzed as to their relationship with the pathogenesis and physiological nature of oral cavities. Furthermore, we developed a minute device to obtain data on the microenvironment on tooth surfaces, this device being equipped with semiconductor microprobes for pH and temperature measurement. The memory of this device is sufficiently large to log environmental data for several days. It also includes a highly integrated microcontroller enabling continuous observation of the dynamism of mineral loss. Clinical application of this device to elucidate the nature of the pathogenesis of microbial biofilm will be undertaken to prevent mineral loss due to early dental caries lesions.

    Actinobacillus actinomycetemcomitans is a Gram-negative, capnophilic coccobacillus which has been considered to be an etiologic agent especially in the severe form of juvenile and adult periodontitis. Previous studies suggested that A.actinomycetemcomitans can invade into periodontal tissues of advanced periodontitis and then survive inside the host cells. We previously showed that apoptotic cell death of the marine macrophage-like cell line was induced by A. actinomycetemcomitans infection. During the course of this apoptosis, activation of several caspases was observed. In general medicine, the facts that severl important pathogenic bacteria can infect and multiple inside the host cells and cause the severe life-threaten infection has been reported. Intracellular invasion mechanisms of some pathogenic bacteria were analyzed in detail, and several caspases has been reported to activate during the course of invasion. We hypothesized that Legionella pneumophila, which causes legion disease, and periodontopathic bacteria, A.actinomycetemcomitans, might invade inside the host cells in similar manner, because both target macrophaees to invade and activate similar set of caspases in host cells. We first adapted the invasion assay designed for A. actinomycetemcomitans to that for L. pneumphila. Mouse macrophage cell line J744.1 and mouse monocyte cell line RAW264.7 are both showed to induce cell death by infection of these bacteria. However, the cell death cased by L. pneumophila was necrosis because the content of cell is filled with multipled bacterial cells just before cell explosion. The difference of the cell death caused by each bacterium might depended on the different signal pathway which are activated by infection even if the similar set of caspases was activated. The RAW264.7 cells which constitutively expressed anti-apopotic Bcl-2 or Bcl-x_L did not rescue cell death by the infection of A. actinomycetemcomitans. Considering all, the mechanisms of invasion and infection of A. actinomycetemcomitans is unique and we need further analysis of these mechanisms for preventing periodontal diseases.

    Dental plaque is the most important etiologic agent of two major dental diseases, dental caries and periodontal diseases. This consists of bacterial cells and their extracellular products and forms the microbial biofilm which adheres sticky to the tooth surfaces. Dental plaque is divided to two parts according to the location to the gingival margin, supragingival plaque and subgingival plaque. Supragingival plaque is easy to remove and to control by toothbrushes expect which is located in interdental area. Subgingival plaque is located inside the periodontal pockets and is difficult to remove completely without visual observation. In the clinics, dental calculus is also major target to eliminate from the tooth surfaces because their rough surfaces are possible retainers of a significant amount of dental plaque. To destroy and to remove dental biofilm and calculus from tooth surfaces completely, we analyzed the physiological resistance of microbial viability of the dental biofilm against the chemical reagents and physiological stresses. We established the assessment method of dental biofilm model to analyze the bactericidal efficiency of various debridement methods. Next, we analyzed the detection method of subgingival calculus by using autofluorescence properties of dental hard tissues. We found subgingival calculus and dentine caries showed characteristic autofluorescence excited by laser irritation, but sound dentine and enamel did not. This method provides us the new diagnosis device for diseased root surfaces without direct visualization. Understanding the physiological properties of dental plaque and calculus, we would established the easy, safe and effective method for tooth cleaning and debridement, to prevent the oral diseases, especially for elderly people who need to care for daily life.

    若年性歯周炎の病原菌であるActinobacillus actinomycetemcomitansをマクロファージに感染させるとアポトーシスが誘導され、さらにこの菌の産生する新規毒素もマクロファージにアポトーシスを誘導することを我々は報告した。ここで歯周炎発症機序を解析するにあたり、この歯周病原菌によるアポトーシスの細胞内メカニズムの解析が大きな課題となった。一方、細胞のアポトーシスの実行分子は、カスパーゼと呼ばれる蛋白質分解酵素ファミリーであり、細菌の細胞内感染によるアポトーシス発症機序の場合のカスパーゼの関与は、サルモネラ菌および赤痢菌がCaspase-1を活性化してIL-1βの分泌を促進すること、およびレジオネラ菌がCaspase-3を活性化してアポトーシスを誘導することが報告されている。そこで本研究では、下位カスパーゼであるCaspase-1及びCaspase-3を活性化する上位カスパーゼであるCaspase-8及びCaspase-9の両者を検索した。J774.1細胞では、A.actinomycetemcomitansの感染マクロファージのアポトーシスの過程で両者が活性化することが示された。しかしながらどちらのCaspaseの活性化経路が重要であるかは、さらなる検証が必要であることも示された。そこで、A.actinomycetemcomitansの感染マクロファージのアポトーシスの過程をこれまでの実験に使用しているJ774.1細胞に加えて、レジオネラ菌の実験に使用されるマクロファージ細胞株RAW264.7細胞へのA.actinomycetemcomitansの感染実験系を確立した。さらにこのRAW264.7細胞にてアポトーシス抑制効果のあるBcl-2及びBcl-X_Lの形質転換株を作製した。この株を使用して上位のCaspaseの活性化経路を検索し、詳細を見当した。

    Infection of murine macrophages in vitro with periodontopathic bacterium Actinobacillus actinomycetemcomitans induces apoptotic cell death. In this study, we investigated the involvement of caspases in apoptotic cell death of A.actinomycetemcomitans-infected macrophages. Two peptide inhibitors of caspases, benzyloxycarbonyl-Val-Ala-Asp(OMe)-fluoromethyl ketone(Z-VAD-FMK)and benzyloxycarbonyl-Asp-Glu-Val-Asp(OMe)-fluoromethyl katone(Z-DEVD-FMK), inhibited apoptotic cell death of mouse macrophage cell line J774.1 infected with A.actinomycetemcomitans. During the process of apoptosis, interleukin-1β(IL-1β) was detected in the culture supernatants of J774.1 cells. IL-1β secretion was blocked by the caspase-1 inhibitor, Z-VAD-FMK, indicating that caspase-1 is involved in not only the induction of apoptosis but also the IL-1β secretion from A.actinomycetemcomintas-infected J774.1 cells. Immunoblot analysis revealed that the infection of A.actinomycetemcomitans to J774.1 cells induced the cleavage of retinoblastoma protein(Rb), suggesting that caspase-3 was activated by A.actinomycetemcomitans-infection. The cytosol from A.actinomycetemcomitans-infected J774.1 cells induced Rb proteollysis in vitro, which was inhibited by the caspase-3 inhibitor, Z-DEVD-FMK.Furthermore, caspase-3-like activity was markedly increased in J774.1 cells infected with A.actinomycetemcomitans between 12h and 24 h, which was subsequently inhibited by the addition of caspase-3 inhibitor, Z-DEVD-FMK.These findings indings indicate that caspase-3 induces apoptosis in J774.1 cells infected with A.actinomycetemcomitans. Taken together, these results suggest that caspase-1 and caspase-3 are involved in the induction of apoptosis in A.actinomycetemcomitans-infected macrophages.
