    Background and Objectives Although tooth loss and periodontitis have been considered risk factors of Alzheimer disease, recent longitudinal researches have not found a significant association with hippocampal atrophy. Therefore, this study aimed to clarify a longitudinal association between the number of teeth present (NTP) and hippocampal atrophy dependent on the severity of periodontitis in a late middle-aged and older adult population. Methods This study included community-dwelling individuals aged 55 years or older who had no cognitive decline and had undergone brain MRI and oral and systemic data collection twice at 4-year intervals. Hippocampal volumes were obtained from MRIs by automated region-of-interest analysis. The mean periodontal probing depth (PD) was used as a measure of periodontitis. Multiple regression analysis was performed with the annual symmetric percentage change (SPC) of the hippocampal volume as the dependent variable and including an interaction term between NTP and mean PD as the independent variable. The interaction details were examined using the Johnson-Neyman technique and simple slope analysis. The 3-way interaction of NTP, mean PD, and time on hippocampal volume was analyzed using a linear mixed-effects model, and the interaction of NTP and time was examined in subgroups divided by the median mean PD. In all models, dropout bias was adjusted by inverse probability weighting. Results Data of 172 participants were analyzed. The qualitative interaction between NTP and the mean PD was significant for the annual SPC in the left hippocampus. The regression coefficient of the NTP on the annual SPC in the left hippocampus was positive (B= 0.038,p= 0.026) at the low-level mean PD (mean −1 SD) and negative (B= −0.054,p= 0.001) at the high-level mean PD (mean +1 SD). Similar results were obtained in the linear mixed-effects model; the interaction of NTP and time was significant in the higher mean PD group. Discussion In a late middle-aged and older cohort, fewer teeth were associated with a faster rate of left hippocampal atrophy in patients with mild periodontitis, whereas having more teeth was associated with a faster rate of atrophy in those with severe periodontitis. The importance of keeping teeth healthy is suggested.

    OBJECTIVE: The present study was designed to investigate the whole transcriptome of periodontal tissues of both young and aged mice to identify the characteristic up-regulation of protease genes with aging and to localize their translated protein products in the periodontal tissues. BACKGROUND: The metzincin protease superfamily is composed of matrix metalloproteinases (MMPs), a disintegrin and metalloproteinases, and a disintegrin and metalloproteinases with thrombospondin motifs. Up-regulation of these extracellular matrix-degrading proteases has been implicated in senescence of tissues and organs, including the skin. However, few studies have investigated the expression profiles of these proteases and potential involvement in aging of periodontal tissues. METHODS: Periodontal tissues with the surrounding mandibular bones were collected from 50- and 10-week-old mice. Total RNA was extracted from the periodontal tissue and analyzed by cap analysis of gene expression (CAGE) to identify differentially expressed genes encoding the metzincin proteases. Furthermore, quantitative real-time polymerase chain reaction (qRT-PCR) was performed to validate the CAGE results, and the phenotypic expression of proteases involved in aging was localized via immunohistochemical analysis. RESULTS: The CAGE results showed that the expression levels of MMP-3, -10, and -12 were up-regulated at 50 weeks. Subsequent qRT-PCR analysis showed that the gene expression levels of MMP-3 and -10 were significantly increased with age. MMP-10 immunoreactivity was localized exclusively in the cementum and alveolar bone adjacent to the periodontal ligament and was stronger and broader in aged mice than young mice. MMP-3 immunoreactivity was localized in the periodontal ligaments at both 10 and 50 weeks. CONCLUSION: In the present study, we demonstrated that the expression of MMP-3 and -10 increased with aging and identified their characteristic localizations in aged periodontal tissues.

    Analysis of the internal local activity distribution in human skeletal muscles is important for managing muscle fatigue/pain and dysfunction. However, no method is established for three-dimensional (3D) statistical analysis of features of activity regions common to multiple subjects during voluntary motor tasks. We investigated the characteristics of muscle activity distribution from the data of ten healthy subjects (29 ± 1 year old, 2 women) during voluntary teeth clenching under two different occlusal conditions by applying spatial normalization and statistical parametric mapping (SPM) to analysis of muscle functional magnetic resonance imaging (mfMRI) using increase in transverse relaxation time (T2) of the skeletal muscle induced by exercise. The expansion of areas with significant T2 increase was observed in the masticatory muscles after clenching with molar loss comparing with intact dentition. The muscle activity distribution characteristics common to a group of subjects, i.e., the active region in the temporal muscle ipsilateral to the side with the molar loss and medial pterygoid muscle contralateral to the side with the molar loss, were clarified in 3D by applying spatial normalization and SPM to mfMRI analysis. This method might elucidate the functional distribution within the muscles and the localized muscular activity related to skeletal muscle disorders.

    OBJECTIVE: To assess caregiver-reported dementia as a risk factor for retained roots, an indicator of poor oral hygiene, among patients receiving home-visit dental treatment in Japan. METHODS: The medical records of 231 dentate patients who received home-visit dental treatment (covered by public medical insurance) for more than 2 years were retrospectively analyzed. The number of teeth and retained roots at the initial and final examinations were obtained from the dental charts, and the "change in the number of retained roots from initial to final examination" was determined. The presence or absence of caregiver-reported dementia, diabetes, and osteoporosis, as well as the level of long-term care needed, were used as indicators of general health condition at the initial interview. Multiple regression analyses were conducted in five models that tested the association of independent variables (age, gender, observation period, general health, presence or absence of caregiver-reported dementia at the initial interview) with changes in the number of retained roots. RESULTS: In all models, the presence of caregiver-reported dementia at the initial interview was significantly associated with the change in the number of retained roots (p < .05). The adjusted coefficient of determination (R2 ) of model 5, which included all the predetermined independent factors, was .168. CONCLUSIONS: Caregiver-reported dementia may be a risk factor for an increase in the number of retained roots among patients who receive home-visit dental treatment and may serve as an indicator of the need for regular and proactive oral hygiene management.

    新型コロナウィルス感染症の流行に伴い、感染リスクの高い口腔内への咬合力測定装置の挿入が困難となり、解析対象となるmfMRIの撮像をほとんど行うことが出来なかった。そのため、過去に別の研究課題で撮像した頭頚部MRIを利用して、3次元画像の空間的標準化を応用した咀嚼筋の体積および脂肪化率の自動計測パイプライン構築の検討を行った。 まず、医用画像Registrationの分野で精度に定評のある「Advanced Normalization Tools(ANTs)」の脳MRIテンプレート作成スクリプトである「antsMultivariateTemplateConstruction2.sh」を利用して、30名の頭頚部MRIから頭頚部全体のテンプレート画像を生成した。テンプレート作成に必要なパラメータは、試行錯誤的に最適な数値を検索した。完成したテンプレート画像上で各咀嚼筋の外形をトレースすることで、各咀嚼筋の領域に対応するマスク画像を作成した。 次に、ANTsを利用して、各被験者の頭頚部MRIのテンプレート画像に対する空間的標準化を実施した。その際に出力される変換パラメータ画像を利用して、咀嚼筋マスク画像の逆変換を行い、各被験者の咀嚼筋領域を反映するマスク画像を出力した。マスク画像のマスク領域の体積を算出し、これを各咀嚼筋の体積とした。また、逆変換したマスク画像を利用して各被験者の咀嚼筋内における信号強度の分布を求めることで、脂肪化率の指標とした。これらの処理はスクリプトによる自動化が可能であり、バイアスを排除した自動解析を実施できることが確認された。

    The purpose of this study was to visualize the distribution of muscle activity inside the tongue during tongue pressure training in three dimensions by applying anatomical standardization and statistical parametric mapping to analysis of muscle functional magnetic resonance imaging (mfMRI). First, the local activity distribution in the masticatory muscles, which have a more distinct morphology than the tongue, was analyzed to establish the image processing method needed for this study, and then the muscle activity distribution inside the tongue in one subject was analyzed. The results of this study show that the distribution of muscle activity inside the tongue can be visualized.

    The incidence of c-fos deficient (-/-) mice was examined by genotyping with PCR using DNA extracted from embryos obtained from mating between c-fos heterozygous (+/-) mice. The results showed that heterozygous (+/-) mice accounted for more than half of the total number of mice (61.2%), while the incidence of c-fos deficient (-/-) mice was low at 14.3%. Micro-CT analysis was performed at 4, 8, and 12 weeks after creation of a standardized defect in the parietal bone of wild-type mice to examine the process of bone defect repair. The results showed that the amount of repaired bone increased over time, and that repaired bone was not only formed from the margins of the defect, but also sporadically formed within the defect. Furthermore, our histological analysis with hematoxylin and eosin staining showed consistent findings with the results of micro-CT analysis.

    This study was started with the aim of three-dimensional visualizing masticatory muscle internal activity by applying spatial normalization and statistical imageing when analyzing the masticatory muscle activity with mfMRI. First, the characteristics of T2 shift associated with exercise within the masticatory muscles were clarified by comparison with electromyographic measurements. Second, a technique for spatial normalization of the masticatory muscles was established and its accuracy was verified. The results of this study suggested the possibility of visualizing the masticatory muscle local activity common to the subject group, and the basis of the method was established.

    This study was planned to visualize and quantitatively analyze the functional load of masticatory muscles and temporomandibular joint. However, the masticatory activities were analyzed under the various scclusal support conditions with the total muscle T2 shift as a new index using a novel bite force gauge, due to the problem in the MR imaging of temporomandibular joint. The novel bite force gauge provided reliable real-time measurements of the bite force in the strong magnetic field of an MRI scanner. The index of total muscle activity was found to be a stronger correlation than the traditional index to the real bite force in the inter-subject study.

    Bruxism and/or clenching, resulting in fatigue or dysfunction of masseter muscles (MM), may cause temporomandibular disorders (TMD). Functional support of the microcirculation is critical for prolonged muscle activity. Histamine is a regulator of the microcirculation and is supplied by release from its stores and/or by de novo production via the induction of histidine decarboxylase (HDC). In the present study, we examined the roles of histamine and HDC in MM activity. Experiments were conducted using our R+G+ model. In addition that fexofenadine (a histamine H1 receptor antagonist) reduced MM activity, both H1 receptor-deficient and HDC-deficient mice exhibited low MM activity. Prolonged R+G+ induced HDC activity in MM. These results suggest that: (i) peripheral histamine supports strenuous MM activity; (ii) strenuous MM activity stimulates mast cells to release histamine and to induce HDC; and (iii) peripheral histamine H1 receptor antagonists may be effective in treating TMD.

    The aim of the present study was to investigate the effects of different mastication on the dopaminergic system in the rat striatum using positron emission tomography (PET)and brain microdialysis (MD). Male F344 rats were fed solid diets (group-S)or powder diets (group-P). These rats were bred reach to 8 and 60 weeks old under the above conditions. As the result of PET study, the accumulation of 11C-RAC at 60 weeks old of group-S was lower than that of group-P. As the result of MD study, dopamine release at 8 and 60 weeks old of group-P was higher than that of group-S. These results suggest that the different mastication influenced on dopaminergic system at the molecular level.

    The new features about the masticatory activities during unilateral clenching were suggested in the present study, which differed from the previous electromyographic studies. Especially, the present results suggested that the superior head of the lateral pterygoid muscle was controlled independently from other masticatory muscles, and the activity of superior head of ipsilateral lateral pterygoid muscle increased in association with the changes in the occlusal point from the first molar to the first premolar during unilateral clenching.

    Nine subjects (mean age 28.7) underwent fMRI during chewing gum on the left side of jaw, with and without a resin cap on the lingual region of the maxillary left canine. fMRI acquisition protocol used in the present study has been optimised in our pilot study. fMRI was performed before, immediately after and 30min after applying resin cap. Six out of nine subjects were asked to masticate a wax cube on the left side of jaw in each condition to determine masticatory efficiency. fMRI data were analyzed with SPM2 (Statistical Parametric Mapping 2) to determine differences of brain activity (p=0.001 uncorrected, extent threshold=20voxels). During left side gum chewing, bilateral cerebellar posterior lobe and the proximal part of right supramarginal gyrus were more activated immediately after applying resin cap than before applying it. The activation of cerebellum might show the activity of cerebellum during "motor learning". During left side gum chewing, right cerebellar posterior lobe were less activated 30min after applying resin cap than immediately after applying it. This result might show that the motor learning of left masticatory muscles and the adaptation to left gum chewing with regin cap were achievement. The brain regions showing negative correlation to masticatory efficiency were bilateral postcentral gyrus, left precentral gyrus, right cingulate gyrus, right superior temporal gyrus, right cerebellar posterior lobe and right cerebellar anterior lobe. These findings suggest that right cerebellar posterior and anterior lobe were activated to achieve motor learning of left masticatory muscles to remedy reduced masticatory efficiency immediately after applying resin cap, and were less activated with the improvement of masticatory efficiency and achievement of motor learning 30min after applying resin cap.

    骨格筋機能的磁気共鳴画像による咀嚼筋活動の定量的測定法を確立し、緊張型頭痛患者に応用することを目的として、健常な若年成人被験者に1Hzの下顎側方運動を一定時間(2.5分、5分、7.5分、10分)行わせてMR画像を撮像し、下顎側方運動を行う時間の増加に伴う各咀嚼筋のT2値の変化を確認した。各課題運動の間には40分間の安静を確保した。各被験者のMR画像の全スライス上で、両側咬筋、内側翼突筋、外側翼突筋の外形をトレースすることにより関心体積を設定し、各関心体積における平均T2値を算出した。その結果、左側方運動を「安静→2.5分→5分→7.5分→10分」の順番で行わせた場合(実験1)には、下顎運動時間の増加に伴うT2値の増加が観察され、特に右側(平衡側)の外側翼突筋で著明な増加が確認された。一方、右側方運動を「10分→7.5分→5分→2.5分→安静」の順番で行わせた場合(実験2)には、下顎運動時間の増加に伴うT2値の増加は左側方運動の場合と比較して明確でなく、筋間の差もあまり観察されなかった。実験1の結果ではT2値と運動量は相関しており、10分咀嚼後の各筋のT2値の増加量を運動量と考えると筋電図による過去の報告と一致していた。しかし、ガム咀嚼時と比較すると各筋のT2値の変化量は小さく、さらに実験2の結果では各筋におけるT2値の増加量は運動量と相関せず、各筋間の差も不明確であった。ガム咀嚼と比較して下顎側方運動は負荷が小さいためにT2値の変化が安定せず、ノイズや測定順序の影響を受けやすいと考えられる。下顎運動時の咀嚼筋活動測定にT2値の変化を利用するためには、負荷を適切に増大させる工夫が必要であると考えられる。今後も緊張型頭痛を含めた臨床的研究への応用に向けた更なる検証を行う予定である。

    We evaluated brain function using functional magnetic resonance image (fMRI) to demonstrate brain activity change of mastication. fMRI acquisition protocol used in the present study has been optimised in our pilot study. 6 adult male subjects (mean age 23.2) were told to use occlusal splint for 7 days. fMRI was performed on day 1, 3, 7 of occlusal splint use and day 7 after occlusal splint removal. After fMRI acquisition, subjective sensations of every subject were recorded with "Visual Analog Scale". fMRI data were analyzed with SPM2 (Statistical Parametric Mapping 2) to determine differences of brain activity between with and without occlusal Splint (p=0.001 uncorrected, extent threshold=20voxels). On day 1 of occlusal splint use, cerebellum and left middle frontal gyrus (BA10) were more activated with occlusal splint than without it. These results showed the activation of cerebellum and frontal lobe during "motor learning". On day 3 and 7 of occlusal splint use, no difference of activity was detected in cerebellum and frontal lobe. On day 7 after occlusal splint removal, left premotor cortex was more activated with occlusal splint than without it. These findings suggest that brain neural network corresponding to new pattern of masticatory movement was constructed and maintained after occlusal splint removal. Although we analyzed association between subjective sensation score by VAS and brain activity using SPM2, no significant correlation was observed.
