    Abstract Aim Little is known about the mental health status of children in Japan whose roots are in foreign countries. The differences in language that are used every day may be a factor that makes adaptation difficult for these children. The aim of the present study, therefore, was to examine the mental health status of children who use foreign languages at home via a cross‐sectional survey in a large cohort. Methods The survey was conducted among children who attended public elementary and junior high schools in a large city in Japan. Data were received from 20,596 elementary school‐aged (above 4th grade) and 19,464 junior high school‐aged children. We compared mental health status evaluated by the Patient Health Questionnaire‐4 in the group based on language usage at home (only Japanese, only foreign languages, and both languages). Results We found that children who used foreign languages at home exhibited worse mental health status than children who used only Japanese at home. In addition, mental health status was slightly better among junior high school‐aged children who used only foreign languages at home than among elementary school‐aged children. This tendency was not observed in the group of children who used both languages at home. Conclusion Our results suggest that children in Japanese society who use foreign languages at home have worse mental health, therefore there is a need for support for these children living in Japan.

    PURPOSE: Pathological Internet use has been predominantly studied in junior high/middle school-aged or older children; data from elementary/primary school-aged children, however, are scarce. The current study aimed to examine the prevalence of problematic Internet use, including pathological and maladaptive Internet use, in elementary and junior high school-aged children and the relationships between problematic Internet use and mental health problems and health-related quality of life. METHODS: The survey was conducted among children who attend national and public elementary and junior high schools in a medium-sized city in Japan; data were received from 3845 elementary school-aged and 4364 junior high school-aged children. RESULTS: Based on the Young's Diagnostic Questionnaire score, the prevalence of pathological and maladaptive Internet use was 3.6% and 9.4% and 7.1% and 15.8% in elementary and junior high school-aged children, respectively. The prevalence of problematic Internet use, including pathological and maladaptive Internet use, consistently increased from the 4th grade to the 8th grade. In addition, the prevalence sharply increased between the 7th grade and the 8th grade. Our study revealed that children with pathological and maladaptive Internet use exhibited more severe depression and decreased health-related quality of life than those with adaptive Internet use. CONCLUSIONS: Our results demonstrated that pathological Internet use is not uncommon even in elementary school-aged children and that those with pathological and maladaptive Internet use have severe mental health problems and decreased health-related quality of life, supporting the importance of providing these children with educational and preventive interventions against problematic Internet use and associated risk factors.

    OBJECTIVE: Our recent single-photon emission computed tomography (SPECT) study of patients with late-onset Alzheimer's disease (AD) revealed that regional cerebral blood flow (rCBF) was reduced in the frontal, temporal, and limbic lobes, and to a lesser degree in the parietal and occipital lobes. Moreover, these patients' scores on the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-cog) were significantly correlated with rCBF in some gyri of the frontal, parietal, and limbic lobes. Our present study aimed to understand how vascular factors and metabolic disease influenced the relationship between rCBF and ADAS-cog scores. METHODS: We divided late-onset AD patients into two groups according to their Hachinski Ischemic Score (HIS), low vascular risk patients had values of ≤4 (n=25) and high vascular risk patients had scores ≥5 (n=15). We examined rCBF using brain perfusion SPECT data. RESULTS: The degrees and patterns of reduced rCBF were largely similar between late-onset AD patients in both groups, regardless of HIS values. Cognitive function was significantly associated with rCBF among late-onset AD patients with low vascular risk (HIS≤4), but not among those with high vascular risk (HIS≥5). Furthermore, metabolic diseases, such as hypertension and diabetes mellitus, disrupted the relationships between hypoperfusion and cognitive impairments in late-onset AD patients. CONCLUSION: Factors other than hypoperfusion, such as hypertension and diabetes mellitus, could be involved in the cognitive dysfunction of late-onset AD patients with high vascular risk.

    OBJECTIVE: This study investigated the combined influence of autism spectrum disorder (ASD) traits, attention-deficit/hyperactivity disorder (ADHD) traits, and positive childhood experiences (PCEs) on suicide-related behaviors in a large, representative sample of Japanese adolescents and young adults. Additionally, it explored the role of PCEs in mitigating the risks associated with neurodivergent traits. METHODS: Data were collected from 5,000 individuals aged 16-25 years using validated scales to measure ASD traits, ADHD traits, PCEs, and suicide-related behaviors, including suicidal ideation and attempts. Hierarchical regression analysis was conducted in multiple steps to assess the influence of these variables. Interaction effects between PCEs and neurodivergent traits were examined to determine potential moderating effects. RESULTS: ASD traits and ADHD traits were positively associated with suicidal ideation, with the highest risks observed among individuals with elevated levels of both traits. The inclusion of PCEs demonstrated a significant negative association with suicidal ideation, indicating that individuals with more PCEs reported lower levels of suicidal ideation. PCEs also reduced the strength of the associations of ASD traits (from β = 0.180 to β = 0.092) and ADHD traits (from β = 0.216 to β = 0.185) with suicidal ideation. Interaction analyses showed that the protective effect of PCEs on suicidal ideation was particularly pronounced among individuals with high levels of ADHD traits. Simple slope analyses demonstrated that higher levels of PCEs were significantly associated with reduced suicidal ideation for those with both low (β = -0.339, z = -18.61, p < 0.001) and high levels of ADHD traits (β = -0.475, z = -21.84, p < 0.001), with a stronger effect for the latter group. CONCLUSION: These findings highlight the cumulative and potentially compounding effects of ASD and ADHD traits on suicide risk while underscoring the critical protective role of PCEs. PCEs can mitigate emotional dysregulation and impulsivity, particularly in individuals with high levels of ADHD traits, thus reducing suicide-related behaviors. This study underscores the importance of fostering PCEs as part of targeted interventions to promote resilience and mental health in vulnerable populations.

    Abstract Background Long-term exercise is increasingly considered an effective strategy to counteract cognitive decline associated with aging. Previous studies have indicated that circuit training exercises integrating aerobic and resistance modalities positively affect cognitive function. Furthermore, a growing body of evidence suggests that long-term exercise alters the gut microbiota, leading to an optimal environment for cognitive enhancement. Recent empirical evidence suggests that exercise plays a significant role in modulating aging-control factors at the protein level. Although the interaction between exercise and cognitive function is multifaceted, most studies have only examined a direct pathway from exercise to cognitive function. Therefore, this study aims to elucidate the effects of long-term circuit training on cognitive function through a comprehensive analysis of factors such as gut microbiota and proteins related to aging control. Methods A total of fifty-one participants will be randomly assigned to either the circuit training or waitlist control group. The intervention group will participate in a circuit training program developed by Curves Japan Co., Ltd. two to three times weekly for 16 weeks. The control group will continue their usual daily routines without participating in any new active lifestyle program. The participants will undergo cognitive assessments at baseline and after the intervention. Fecal and blood samples for protein analysis will be collected before and after the intervention. The effect of exercise on cognition will be analyzed by comparing the measured outcomes before and after the intervention. The associations among these outcomes will be assessed using a linear mixed model and structural equation modeling approaches. Discussion This study aims to provide the first insights into the comprehensive effects of exercise on cognitive function from the perspectives of gut microbiota and aging control. The findings are expected to contribute to improving brain health and combating age-related cognitive decline. Furthermore, the findings may help establish new guidelines for future studies on the relationship between exercise and cognitive function.

    BACKGROUND AND AIMS: Internet addiction (IA) and depression commonly co-occur in adolescents, yet the mechanisms underlying their comorbidity remain unclear. This study aims to elucidate the comorbidity mechanism through network analysis, identifying bridge symptoms linking IA and depression, and exploring sex differences. Additionally, the study examines the association between effortful control (EC) and bridge symptoms, providing insights for interventions. METHODS: A school-based survey was conducted among 7th to 9th-grade students in Japan. Participants completed questionnaires assessing IA (measured by the Young Diagnostic Questionnaire), depression (measured by the Patient Health Questionnaire for Adolescents), and EC (measured by the Early Adolescent Temperament Questionnaire). Network analysis was employed to identify bridge symptoms and examine their association with EC. Bootstrapping for network analysis was conducted to assess network accuracy and stability as well as sex differences in the network structures. RESULTS: Among the 4,111 students approached, 3,909 (1,904 male and 2,005 female) students filled out the survey. Bridge symptoms such as "Escape" (from the IA cluster) and "Concentration" (from the depression cluster) were found important in both male and female students. Our analysis also revealed differences in the importance of the bridge symptoms across males and females with "Psychomotor" symptoms (from the depression cluster) predominantly in males and "Feeling Guilty" (from the depression cluster) and "Functional impairment" (from the IA cluster) predominantly in females. EC showed a notable negative association with "Concentration", suggesting important relationships between the transdiagnostic factor and bridge symptoms in understanding the comorbid conditions. The network comparison test did not reveal significant differences in the network structures across sexes. DISCUSSION AND CONCLUSIONS: The study revealed differences in bridge symptoms linking IA and depression between male and female students. Our findings provide valuable insights for understanding the comorbidity mechanisms of IA and depression in adolescents. Further research using a longitudinal study design is warranted to identify the directionality between EC and bridge symptoms.

    INTRODUCTION: With the rapid aging of the population worldwide and the prevalence of dementia and mental health problems among older adults, it is important to extend healthy life expectancy by maintaining brain and mental health. Playing musical instruments, which requires the integration of auditory, visual, and somatosensory functions, is considered an effective way to prevent the development of dementia. However, the effectiveness of group (band) music sessions in healthy older adults has not been investigated. Our purpose, therefore, was to investigate the effects of group music sessions on cognitive and psychological functions among healthy older adults. METHODS: In this open-label randomized controlled trial, participants aged 65-74, who had no musical experience, were randomly assigned to either the intervention or control group. The intervention group received in weekly 90-minute sessions with the instrument for 16 weeks. The control group received no intervention. RESULTS: The results showed that the Mini-Mental State Examination (MMSE) total score and the Wechsler Memory Scale Logical Memory Ⅱ (WMS-LM Ⅱ) score improved significantly, and the Vigor-Activity subscale score of the Profile of Mood States 2nd Edition (POMS 2) tended to improve. DISCUSSION: These findings indicated that group music sessions have a potentially beneficial effect for maintaining and improving cognitive and psychological functions in healthy older adults.

  18. Early detection of students’ mental health issues from a traditional daily health observation scheme in Japanese schools and its digitalization Peer-reviewed

    Objective The implementation of school-based mental health screening offers promise for early detection of mental health issues in children; however, various barriers hinder its widespread adoption. This study aimed to investigate the predictive value of digital data obtained from an established daily health observation scheme in Japanese schools to identify later mental health issues in children. Methods Data for the analysis were obtained from 2,433 students enrolled in five public schools. The data acquisition period spanned 76 school days, from September 1, 2022, to December 23, 2022, and student absences were recorded during this period. Depressive and anxiety symptoms were assessed in January 2023. The students’ daily physical and emotional health status was recorded as “daily health issue” scores and group-based trajectory modeling was employed to classify the long-term trends in these scores. Additionally, rolling z-scores were utilized to capture variability in daily health issue scores, with z-scores above +1 considered unusual responses. Results After 4 months of daily health observations, students’ response trends were classified into five trajectory groups. The group experiencing the highest number of daily health issues (Group 5; 5.4% of the sample) exhibited more subsequent depressive and anxiety symptoms compared to the group with fewer issues (Group 1; 47.5%) (incident rate ratio [IRR] = 5.17; 95% confidence interval [CI]: 3.82, 6.99). Group 5 also demonstrated significantly more days of absence than Group 1 (IRR = 2.14, 95% CI: 1.19, 3.85). The average daily health issue scores for the entire period were associated with both depressive/anxiety symptoms and the number of days absent from school (IRR = 1.59, 95% CI: 1.45, 1.73; IRR = 1.18, 95% CI: 1.04, 1.35, respectively). Furthermore, a higher number of unusual responses during the entire period was also associated with more depressive/anxiety symptoms (IRR = 1.10, 95% CI: 1.07, 1.12). Conclusion The current study is the first to demonstrate the predictive capability of a traditional daily health observation scheme to identify mental health issues in children. This study highlights the scheme’s potential to screen and safeguard children’s mental health, emphasizing the importance of digitalization and collaboration with various stakeholders.

    Background Adverse childhood experiences (ACEs) have been found to negatively impact adult mental health outcomes. Numerous studies have highlighted on ACEs in family and community settings. However, few have examined the impact of ACEs in school settings, despite the potential influence on social participation. Hikikomori, characterized by severe social withdrawal, was first studied in Japan and has gained recognition in recent years. The present study aims to present the concept of ACEs specific to schools and investigate the impact of both school ACEs and traditional ACEs on adult mental health and Hikikomori. Methods A total of 4,000 Japanese adults, aged 20–34, were recruited through an Internet survey form. All data were obtained in October 2021. Participants answered questions regarding their ACEs in the family (10 items), school ACEs (five teacher-related items and two bullying-related items), depressive/anxiety symptoms, and Hikikomori (remaining at home for more than 6 months). Results A significant association with depressive/anxiety symptoms was shown in both ACEs and school ACEs. An increase of one point in the ACE scores was associated with a 24% increase in the risk of depressive/anxiety symptoms. School ACE scores also demonstrated a significant association with depressive/anxiety symptoms, with an increase of one point associated with a 44% increase in the risk of these symptoms. As for Hikikomori, a significant association was shown in the school ACEs only: a 29% increased risk of Hikikomori for every one-point increase in school ACE scores. Both school ACE scores for teacher-related and bullying-related factors revealed a significant association with Hikikomori; the rates of increased risk were 23 and 37%, respectively. Conclusion These results suggest that school ACEs, rather than ACEs in the family, are associated with the risk of Hikikomori. School ACEs are important for social adaptation, and reducing traumatic experiences in school settings may have the potential to prevent problems in later life, specifically in terms of social participation.

    Though autism spectrum disorder (ASD) traits are associated with depression, it is unclear if school social capital mediates their association. We examined whether school social capital mediates the association between ASD traits and depression, and moderation effect of sex on the mediation effect among adolescents in a general population sample (1750 males, 1779 females; equivalent 12-15 years old). The results of this study indicate that ASD traits are associated with depression among adolescents, and that this association is partly mediated by school social capital. Furthermore, the results of the moderated mediation analysis suggest that lower level of school social capital can lead to more increase level of depression for females than for males.

    Little is known about the trajectory patterns and sex differences in adaptive behaviors in the general population. We examined the trajectory classes of adaptive behaviors using a representative sample and examined whether the class structure and trajectory patterns differed between females and males. We further explored sex differences in neurodevelopmental traits in each latent class. Participants (n = 994) were children in the Hamamatsu Birth Cohort for Mothers and Children (HBC Study)-a prospective birth cohort study. Adaptive behaviors in each domain of communication, daily living skills, and socialization were evaluated at five time points when participants were 2.7, 3.5, 4.5, 6, and 9 years old using the Vineland Adaptive Behavior Scales-Second Edition. Parallel process multigroup latent class growth analysis extracted sex-specific trajectory classes. Neurodevelopmental traits of children at age 9, autistic traits, attention deficit hyperactivity disorder (ADHD) traits, and cognitive ability were examined for females and males in each identified class. A 4-class model demonstrated the best fit. Moreover, a 4-class model that allowed for differences in class probabilities and means of growth parameters between females and males provided a better fit than a model assuming no sex differences. In the communication domain, females scored higher than their male counterparts in all four classes. In the daily living skills and socialization domains, the two higher adaptive classes (Class 1: females, 18.6%; males, 17.8%; Class 2: females, 48.8%; males, 49.8%) had similar trajectories for males and females, whereas in the two lower adaptive behavior classes (Class 3: females, 27.5%; males, 29.4%; Class 4: females, 5.1%; males, 3.0%), females had higher adaptive scores than their male counterparts. In Class 4, females were more likely to have autistic and ADHD traits exceeding the cutoffs, while males were more likely to have below-average IQ. Different trajectories in females and males suggest that adaptive skills may require adjustment based on the sex of the child, when standardizing scores, in order to achieve better early detection of skill impairment.

    Under the COVID-19 pandemic, concerns regarding prolonged screen time and mental health effects in children have increased. We examined the association of depression with smartphone ownership in school children at four time points: September 2019, July 2020, December 2020, and March 2021. The analysis revealed an interaction between group and time, indicating that depressive symptoms among smartphone owners were significantly more severe than in the other group. These results were clearer for fourth-year students, pointing that smartphone possession at younger ages may be a risk factor for mental health in the new lifestyle caused by the COVID-19 pandemic.

    Social capital is an important factor that affects mental health. The purpose of this study was to examine the relationship between social capital and depression and between social capital and quality of life (QoL) in children in elementary and junior high school and to examine how this relationship differs in relevant patterns at both the individual- and school-level. The study was conducted in all elementary and junior high schools in a single municipality; the subjects consisted of 3,722 elementary school and 3,987 junior high school students (aged from 9 to 15). A multilevel linear mixed effect model analysis revealed that all three subscales of social capital were associated with depression and QoL at the individual-level: The school social capital at the individual-level showed the strongest association with depression and QoL. We also found that some of social capital at the school-level was associated with depression and QoL. An interactive effect was observed between educational stage (elementary and junior high) and some of social capital subscales. Specifically, the inverse association between school social capital and depression was stronger among the junior high students, while the positive association between school and neighborhood social capital and QoL was stronger among the elementary students. These interactions suggest that social capital impacts depression and QoL differently in elementary and junior high students. These findings suggest that the degree of association of social capital domains differs in mental health among the educational stage.

    OBJECTIVE: Although autism spectrum disorder (ASD) occurs worldwide, most genomic studies on ASD were performed on those of Western ancestry. We hypothesized ASD-related copy number variations (CNVs) of Japanese individuals might be different from those of Western individuals. METHODS: Subjects were recruited from the Hirosaki 5-year-old children's developmental health check-up (HFC) between 2013 and 2016 (ASD group; n = 68, control group; n = 124). This study conducted CNV analysis using genomic DNA from peripheral blood of 5-year-old Japanese children. Fisher's exact test was applied for profiling subjects and CNV loci. RESULTS: Four ASD-related CNVs: deletion at 12p11.1, duplications at 4q13.2, 8p23.1 and 18q12.3 were detected (P = 0.015, 0.024, 0.009, 0.004, respectively). Specifically, the odds ratio of duplication at 18q12.3 was highest among the 4 CNVs (odds ratio, 8.13). CONCLUSIONS: Four CNVs: microdeletion at 12p11.1, microduplications at 4q13.2, 8p23.1 and 18q12.3 were detected as ASD-related CNVs in Japanese children in this study. Although these CNVs were consistent with several reports by Western countries at cytoband levels, these did not consistent at detailed genomic positions and sizes. Our data indicate the possibility that these CNVs are characteristic of Japanese children with ASD. We conclude that Japanese individuals with ASD may harbor CNVs different from those of Western individuals with ASD.

    There is relatively little information about prospectively reported developmental milestones from caregivers of children who go on to be diagnosed with neurodevelopmental disorders (NDDs), including autism spectrum disorder (ASD). The current study examined rates of early skill attainment in 5-year-old children who participated in a comprehensive in-person assessment for NDDs in Hirosaki in Japan. Developmental milestone data were extracted from their Maternal and Child Health Handbook (MCHH), a booklet distributed to all pregnant women as part of universal health care. Seven hundred and twenty children underwent the assessment, among whom 455 received one or more NDD diagnoses (ASD: n = 124, non-ASD NDD: n = 331). Developmental skills were organized into four domains (motor, social interaction, communication, self-help), and the cumulative number of potential delays in each domain was calculated for each participant within three different age ranges (by 12 months, by 24 months, and by 36 months). Even by age 12 months, children with ASD/NDDs showed more potential delays across domains compared to those who received no NDD diagnosis. However, differences between those with ASD and those with non-ASD NDDs were not apparent until 24 months for social interaction and communication, and 36 months for self-help. These findings provide insights into specific behaviors that could be used to screen for ASD and other NDDs. In addition, the present study indicates the potential utility of the MCHH as a broadband screening tool to educate parents about what to look for in charting their child's early development. LAY SUMMARY: The present study examined prospectively charted developmental milestones from home-based records used as part of universal health care in 720 5-year-old children from Hirosaki, Japan. All children participated in a comprehensive evaluation to determine if they met criteria for a neurodevelopmental disorder (NDD), including autism spectrum disorder (ASD). Compared to those who received no NDD diagnosis, children with NDDs exhibited higher rates of potential delays across developmental domains, including social interaction, communication, and self-help. For some children, these delays were apparent before the age of 12 months. Differences between diagnostic groups became even more pronounced by 24 and 36 months, well before the average age of diagnosis. This suggests that home-based records can be useful tools to educate caregivers about what to look for in charting their child's early development and could assist with early screening efforts.

    Through this meta-analysis, we sought to examine the prevalence of, risks for, and factors associated with bullying involvement (victimization, perpetration, perpetration-victimization) among students with autism spectrum disorder (ASD). Additionally, we attempted to examine sources of variance in the prevalence and effect sizes of bullying in students with ASD across studies. Systematic database and literature review identified 34 relevant studies (31 for Western countries, three for Eastern countries). Pooled prevalence estimates for victimization, perpetration, and perpetration-victimization in general were 67%, 29%, and 14%, respectively. The risk of victimization in students with ASD was significantly higher than that in typically developing students and students with other disabilities. Further, deficits in social interaction and communication, externalizing symptoms, internalizing symptoms, and integrated inclusive school settings were related to higher victimization, and externalizing symptoms were related to higher perpetration. Finally, moderation analyses revealed significant variations in the pooled prevalences thereof depending on culture, age, school settings, and methodological quality and in the pooled effect sizes according to publication year and methodological quality. Our results highlight needs for bullying intervention for students with ASD, especially those who are younger, are in an inclusive school setting, and have higher social difficulties and externalizing/internalizing symptoms; for intensive research of bullying experiences among students with ASD in Eastern countries; and for efforts to improve the methodological quality of such research.

    Despite increasing attention to internet addiction (IA) in both clinical practice and research, our understanding of longitudinal changes of IA status is limited. In the present study, we employed latent transition analysis to investigate patterns of transitions and the stability of IA status among 5483 students (aged 9-12 years) over the two-year study periods. Additionally, we examined whether neurodevelopmental traits predicted certain transition patterns. The stability rate of IA class membership and the conversion rate from non-IA to IA status across the 2 years were 47% and 11%, respectively. The regression model revealed that autistic traits predicted the persisting IA pattern and that inattention traits predicted both the persisting and converting (from non-IA to IA status) patterns.

    The aims of this study were to identify sensory processing profiles specific to preschoolers with DCD in a community sample and examine the association of sensory processing problems with motor coordination difficulties in these children. Sixty-three 5-year-old children with DCD and without other neurodevelopmental disorders and 106 age-matched typically developing children participated in this study. Sensory processing problems were assessed using the Sensory Profile. Our results demonstrated problems in wide sensory processing patterns (low registration, sensitivity and avoiding) and areas (auditory, vestibular, touch and oral) in children with DCD compared with typically developing children. Additionally, the association of problems in sensory processing patterns (sensitivity and avoiding) and areas (touch and auditory) with motor coordination difficulties were identified in children with DCD alone. Our findings indicate that sensory processing abnormalities may contribute to the pathophysiology of DCD, suggesting the importance of assessing sensory processing functions in children with DCD.

    BACKGROUNDS: Whether there is a true increase in autism spectrum disorder (ASD) frequency or not remains unclear. Additionally, the rates of co-existing neurodevelopmental disorders (NDD) in a total population sample has not been fully examined before. Therefore, using a total population sample in Japan, we aimed to estimate the prevalence and cumulative incidence of autism spectrum disorder (ASD) annually, to determine whether there is a true increase in ASD prevalence by estimating the cumulative incidence of ASD annually, and to examine the rates of co-existing neurodevelopmental disorders (NDD). METHOD: In this cross-sectional sequential design study, all 5-year-old children in the catchment area underwent the screening annually from the year 2013-2016. Screen-positive children were invited to participate in a comprehensive assessment, including child and parent interview, behavioral observation, and cognitive and motor function testing. All cases were reviewed by a multidisciplinary research team. RESULTS: Caregivers of 3954 children returned the screening, among which 559 children underwent the assessment with 87 children receiving an ASD diagnosis. Adjusted ASD prevalence was 3.22% (95% confidence interval (CI) 2.66-3.76%). The male to female ratio of the crude prevalence was 2.2:1. The cumulative incidence of ASD up to 5 years of age for the total study years was 1.31% (95% CI 1.00-1.62%). A generalized linear model revealed no significant linear trends in 5-year cumulative incidence over the study years. Only 11.5% of children had ASD alone; the remaining 88.5% were found to have at least one co-existing NDD. LIMITATIONS: Modest sample size for a total population study. CONCLUSIONS: Our findings demonstrate the stability of the 5-year cumulative incidence of ASD, implying no true rise in ASD incident cases over the 4-year study period in the study catchment area. High rates of co-existing NDDs reflect the importance of investigating broad developmental challenges in children with ASD.

    BACKGROUND: Despite their importance in population health among children and adolescents, our understanding of how individual items mutually interact within and between pediatric health-related quality of life (HRQOL) and school social capital is limited. METHODS: We employed network analysis in a general population sample of 7759 children aged 9-15 years to explore the network structure of relations among pediatric HRQOL and school social capital items measured using validated scales. Furthermore, network centrality was examined to identify central items that had stronger and more direct connections with other items in the network than others. Network structure and overall strength of connectivity among items were compared between groups (by sex and age). RESULTS: Our analysis revealed that the item related to school/academic functioning and the item related to shared enjoyment among students had the highest strength centrality in the network of HRQOL and school social capital, respectively, underpinning their critical roles in pediatric HRQOL and school social capital. Additionally, the edge connecting "I trust my friends at school" and "trouble getting along with peers" had the strongest negative edge weight among ones connecting school social capital and pediatric HRQOL constructs. Network comparison test revealed stronger overall network connectivity in middle schoolers compared to elementary schoolers but no differences between male and female students. CONCLUSION: The network approach elucidated the complex relationship of mutually influencing items within and between pediatric HRQOL and school social capital. Addressing central items may promote children's perceived health and school social capital.

    自閉スペクトラム症(ASD)特性が自殺リスクを増大させるメカニズムの一部を明らかにするために,ASD 特性と自殺関連行動の関連性におけるエフォートフル・コントロールの媒介効果を検討した。日本全国から比例抽出された 5,000 名の若者に対し,インターネットを介した質問紙調査を実施した。相関分析,階層的重回帰分析の結果,ASD 特性の高さは自殺関連行動の高さと関連があることが示された。エフォートフル・コントロールの下位尺度である注意制御およびアクティベーション制御の低さは,自殺関連行動の高さと関連があることが示されたが,インヒビションとの関連は示されなかった。媒介分析では,注意制御とアクティベーション制御が ASD特性と自殺関連行動の関連を部分的に媒介することが確認された。以上の結果から,ASD 特性が自殺リスクを増大させる上で注意制御およびアクティベーション制御が重要な媒介要因となることを指摘した。 To examine the mediating factors through which autistic spectrum disorder(ASD)traits increase suicide risk, we examined the mediating effect of effortful control on the relationship between ASD traits and suicide-related behaviors. We conducted an internet-based questionnaire survey of 5,000 proportionally sampled adolescents from across Japan. Correlation analysis, hierarchical multiple regression analysis showed that high levels of ASD traits were associated with high levels of suicide-related behaviors. Low levels of the effortful control subscales, namely attentional control and activation control, were associated with high levels of suicide-related behaviors, but there was no association with inhibitory control. The mediation analysis confirmed that attentional control and activation control partially mediated the relationship between ASD traits and suicide-related behaviors. Based on these results, we suggested that attention control and activation control are important mediating factors in the increased risk of suicide due to ASD traits. Article 【原著/Original Articles】

    BACKGROUND: Early detection and intervention for neurodevelopmental disorders are effective. Several types of paper questionnaires have been developed to assess these conditions in early childhood; however, the psychometric equivalence between the web-based and the paper versions of these questionnaires is unknown. OBJECTIVE: This study examined the interformat reliability of the web-based parent-rated version of the Autism Spectrum Screening Questionnaire (ASSQ), Attention-Deficit/Hyperactivity Disorder Rating Scale (ADHD-RS), Developmental Coordination Disorder Questionnaire 2007 (DCDQ), and Strengths and Difficulties Questionnaire (SDQ) among Japanese preschoolers in a community developmental health check-up setting. METHODS: A set of paper-based questionnaires were distributed for voluntary completion to parents of children aged 5 years. The package of the paper format questionnaires included the ASSQ, ADHD-RS, DCDQ, parent-reported SDQ (P-SDQ), and several additional demographic questions. Responses were received from 508 parents of children who agreed to participate in the study. After 3 months, 300 parents, who were among the initial responders, were randomly selected and asked to complete the web-based versions of these questionnaires. A total of 140 parents replied to the web-based format and were included as a final sample in this study. RESULTS: We obtained the McDonald ω coefficients for both the web-based and paper formats of the ASSQ (web-based: ω=.90; paper: ω=.86), ADHD-RS total and subscales (web-based: ω=.88-.94; paper: ω=.87-.93), DCDQ total and subscales (web-based: ω=.82-.94; paper: ω=.74-.92), and P-SDQ total and subscales (web-based: ω=.55-.81; paper: ω=.52-.80). The intraclass correlation coefficients between the web-based and paper formats were all significant at the 99.9% confidence level: ASSQ (r=0.66, P<.001); ADHD-RS total and subscales (r=0.66-0.74, P<.001); DCDQ total and subscales (r=0.66-0.71, P<.001); P-SDQ Total Difficulties and subscales (r=0.55-0.73, P<.001). There were no significant differences between the web-based and paper formats for total mean score of the ASSQ (P=.76), total (P=.12) and subscale (P=.11-.47) mean scores of DCDQ, and the P-SDQ Total Difficulties mean score (P=.20) and mean subscale scores (P=.28-.79). Although significant differences were found between the web-based and paper formats for mean ADHD-RS scores (total: t132=2.83, P=.005; Inattention subscale: t133=2.15, P=.03; Hyperactivity/Impulsivity subscale: t133=3.21, P=.002), the effect sizes were small (Cohen d=0.18-0.22). CONCLUSIONS: These results suggest that the web-based versions of the ASSQ, ADHD-RS, DCDQ, and P-SDQ were equivalent, with the same level of internal consistency and intrarater reliability as the paper versions, indicating the applicability of the web-based versions of these questionnaires for assessing neurodevelopmental disorders.

    目的:Early Start Denver Model (ESDM)は生後12ヶ月から48~60ヶ月までの発達早期の幼児を対象とし、対人相互の結びつきに焦点を当てることを特徴とし、効果が検証されている自閉スペクトラム症(ASD)の超早期介入法であるが、一人の児に要する個人セッションの時間及び頻度の多さから、支援や介入を行える機関や療育者数が限られる日本の現状では同様の枠組みを提供することが難しい現状があり、多くのASD児にESDMによる介入を提供できていないのが現状である。このことから本研究は日本の現状に合わせて提供可能なプロトタイプを作成し効果検証を行うこと、ESDM治療者の地域レベルでの人材育成を促進することを目的とするものである。 研究実績:①ESDMの人材育成-2019年3月に行ったESDMの紹介講演で本介入法に興味を示した臨床心理士1名を、7月に札幌市で開催されたESDMイントロダクションワークショップに派遣した。②ESDMの実施とプロトタイプの作成-ESDMへのリクルートを行うために、2019年度中に弘前大学医学部附属病院を受診した4歳以下の乳幼児71名に対して自閉症評価としてAutism Diagnostic Observation Schedule 第2版(ADOS-2)、SRS-2対人応答性尺度(SRS-2)、適応行動尺度としてVineland-II適応行動尺度(Vineland-II)、知的レベル評価としてWPPSI-IIIまたは田中ビネーVを実施し、観察・診断を行った。ESDMの実施は、2018年度からの継続1名、2019年度7月から1名、2019年度9月から1名の計3名の幼児に対して週1回1時間の介入を行っている。

    本研究の目的は、1歳半及び3歳児健診にて運動発達検査、眼球運動検査及び在胎週数・出生体重・睡眠習慣・栄養状態・感覚特性などの調査を行い、内部モデル障害が顕在化する時期及び発達の軌跡を検証するとともに、内部モデルに影響を及ぼす因子を検証し、病態の解明及び予後の改善及に役立てることである。2019年度計画は、乳幼児健診から就学後における認知・感覚・運動発達の調査として、以下の内容を実施した。 ①1歳6か月健診:2020年2月~3月に160名の健診参加者に発達調査としてM-CHAT、マッカーサー言語発達調査、CBCL、睡眠習慣(JSQP、CSHQ)、ICT調査、吃音とチックの調査を配布した。結果は郵送で回収を行う。 ②3歳児健診:2019年4月~2020年3月に約1000名にSRS-2によるスクリーニングを施行。カットオフを43点とし、精査対象として知能・運動・社会性について評価を継続している。また2018年に健診参加した1090名のうち、精検対象となった129名中当院で精査を受けた63名について解析を行った。併存障害を含め、発達性協調運動障害の診断を受けた児は17名であった。3歳児におけるDCD有病率は推定で1.67%となった。 ③5歳児健診:2019年5月及び11月施行。5歳に特徴的な協調運動障害の5症状を抽出した。吃音やチックの症状と協調運動障害の症状に有意な関連性が見られた。1歳半健診で言語発達と吃音やチック障害との関連について調査を追加した。 ④DCDの診断を受けた児(年間30名)に就学後、発達外来で順次協調運動・学習・認知機能を評価した。現在結果を集計中である。 また、DCDに関する学会発表、総説の執筆を行った。

    本研究の目的は、地図情報から取得した学校区毎の構造的地域ソーシャルキャピタルのデータを企業から取得し、学校区毎の構造的地域ソーシャルキャピタルが子どもの情緒及び行動に対して与える影響を明らかにすることである。そのために、本研究では、地域ベースの大規模な子どもの精神的健康に関する調査を行い、それらの指標と、環境因子である学校区毎の構造的地域ソーシャルキャピタルとの関連を検討する。 本年度は計画通り、対象となる市内国公立小中学校に通う全児童生徒(約8,000人)とその保護者を対象に調査を行い、データの整理を進めている。地域に関する構造的ソーシャルキャピタルに関するデータの提供元である企業とはデータの売買契約について、その内容などの詳細を詰めており、年度が替わり次第すぐにデータが使用可能になるよう、準備を進めているところである。 これまでの成果としては、質問紙によって測定した、ソーシャルキャピタルと抑うつおよび生活の質(QOL)の明らかにしたことが挙げられる。本成果では、豊富なソーシャルキャピタルが小中学生の抑うつや生活の質に良い影響を及ぼすことを明らかにした。また、年齢によって、影響を受けやすいソーシャルキャピタルの種類が異なることも明らかになった。具体的には、中学生の抑うつに対しては学校ソーシャルキャピタルが相対的に強い影響を持っていた。また、QOLについては、中学生と比較して、小学生のQOLに対する学校ソーシャルキャピタルと近隣ソーシャルキャピタルの影響が強いことが明らかになった。これらの結果は、ソーシャルキャピタルの精神的健康やQOLに対する影響の強さが年齢によって異なることを示唆している。

    対象は2次発達健診で神経発達障害群の診断がついた5歳児約650名の血漿である。その中から自閉症(ASD)54人(男児36人、女児18人)、注意欠如・多動性障害(ADHD)50人(男児31人、女児19人)、ASDとADHDの合併例64人(男児44人、女児20人)、健常児(TD)66人(男児41人、女児25人)を選択し、質量分析器 (SCIEX社、QTRAP6500)を用い、血漿中のメタボローム解析を行った。AbsoluteIDQ p180Kit メタボライト測定キット (Biocrates)を使用した。このキットは重要な代謝物の定量的な解析が可能である。5つの化合物クラスに属する186種類の代謝物を定量化した。1.アシルカルニチン(37個);生物学上のパスウェイはエネルギー代謝、酸化ストレスなど、2.アミノ酸(21個)、3.生体アミン(21個);神経傷害など、4.各種ヘキソース;炭水化物の代謝、5.リン脂質及びスフィンゴ脂質(ホスファチジルコリン:76個;脂肪酸プロファイルなど、リゾホスファチジルコリン:13個;脂肪酸プロファイルなど、スフィンゴミエリン:15個;シグナルカスケード)。結果は、ASD対TDでカットオフ値を2.0にしたところ、Aアミノ酸が有意差を示した。Bアミノ酸はROC curveから感度は65.2%、特異度は77.8%であった。ADHD対TDではC生体アミンがカットオフ値が4.0で有意差を示した。D脂肪酸はROC curveから感度は72.7%、特異度は74.0%であった。ASDとADHDの合併対TDでは4種類の因子でカットオフ2.0で有意差を示した。その中で、Eアミノ酸がROC curveから感度は80.3%、特異度は78.1%であった。このようにASD,ADHDの診断マーカーもしくは病態解明につながる因子がいくつか明らかになった。

    本年度は、研究分担者とともに、思春期における精神科的・心理的問題を調査するための質問紙の作成に取り組んだ。具体的には、思春期における精神疾患や心理的問題を調査した国内外の疫学の先行研究を系統的に検索し、該当する文献を批判吟味することから開始した。その後、先行研究より得られた知識をもとに、研究分担者と議論を重ねることで、質問紙に含める質問項目を検討・決定した。具体的には、子供の行動チェックリスト(Child Behavior Checklist)を使用し、こどもの内在化問題(不安や抑うつ気分)と外在化問題(行動問題や衝動性)を測定することに決定した。また、質問項目には、精神疾患や発達障害の既往歴、治療歴(薬物治療、心理療法を含む)、保護者の心理的ストレスなどを含めることと決定した。これは、研究目的である、思春期における一般人口レベルでの精神疾患、心理的問題の罹患率を調査すること、を達成するための重要な過程である。 <BR> さらに、研究計画書の作成、同意書、同意撤回書の作成を行い、倫理委員会に提出した。 同意書には、思春期のこころの調査への参加同意と、これまでに実施された弘前市5歳児健診と学校調査に参加した場合は、それらのデータを思春期調査のデータと連結することへの同意、の2か所を含めた。これは、本研究の第二の研究目的である、幼児期から思春期への社会性・感情・行動問題の発達軌跡曲線パターンの同定を行うために必要な準備である。

    We think that adult ADHD would be diagnosed with ADHD due to problems in their lives and work in an environment with many stress factors. The subjects were 41 people who were suspected of having ADHD by using an adult ADHD Self Report Scale-Screener with a total score of 4 points or more. They were diagnosed based on DSM-5 using the Japanese version of CAADID. Adult ADHD who did not meet the CAADID criteria were cases in which symptoms were not noticeable as a child, social maladaptation occurred in adulthood, and ADHD symptoms became apparent. They were not in the new concept adult ADHD group, within the diagnostic criteria of ADHD by DSM-5.

    The aim of the present study was to clarify the neural mechanisms of executive dysfunction in autism spectrum disorder (ASD). Therefore, we obtained the data of executive function tests and structural brain imaging from the individual with ASD or typical development. As a result of comparing the performance of executive function tests between ASD and typical development, it was found that the lower score of the visuospatial working memory and the later performance speed of planning task and cognitive regulation task in ASD compared to typical development. In addition, the later performance speed of the planning task was related to the differences of white matter structure in the right superior frontal gyrus and left insular between ASD and typical development. These results suggest that structural differences in white matter in these regions are involved in the impairment of executive function in ASD.

    A new screening tool and a risk child extraction algorithm were created using data accumulated in the 5-year-old developmental health checkup in Hirosaki City. Verification of screening tools was conducted during the research period, and it was confirmed that developmental disorders could be extracted at a rate of around 15%, and a patent application was filed. According to epidemiological studies, the prevalence of autism spectrum disorder at age 5 years is 3.22%, the incidence is 1.3%, and the incidence is slightly increasing. We analyzed the characteristics of developmental coordination movement disorder, blood markers of developmental disorder children and gaze point biomarkers and obtained useful results. Research results have been published and published in academic meetings in Japan and abroad.

    We conducted verification of the validity of the screening tool to extract children has possibility of neurodevelopmental disorders, including autism spectrum disorder (ASD) and the cut-off value, based on the preliminary survey at the 3-year old health checkup. As a result, it was suggested that the combination of the SRS-2 Human Response Scale and the SDQ Children's Strength and Difficulty Questionnaire is appropriate as a measure of use for extracting ASD. Furthermore it found that the statistically reasonable value of SRS-2 was above 43.5 points. We have started the extraction in a 3-year-old health checkup with a child indicates a score of 44 or more on SRS-2 as the subject for precision screening from April 2018. In addition, a total of more than 150 data such as the Autism Diagnostic Observation Test (ADOS) were accumulated to extract directly observable items.

    In this project, we examined the applicability of the ASSQ in preschool children. We examined the short version, which has been verified in Japan for psychological measurement accuracy, and the full scale, which is used internationally. In the short version, 1,919 children, including 59 children diagnosed with ASD, were examined to verify the measurement accuracy of the ASSQ. As a result of ROC analysis, AUC was 0.92 for boys and 0.91 for girls, showing excellent discrimination accuracy. From the viewpoint of sensitivity and specificity, the cut-off in the community were proposed 3 points for boys and 4 points for girls. In the full version, we tested parent-rating ASSQ for preschool children in clinical (N = 154) and community settings (N = 1,390). The results showed, the ASSQ had reliability and validity as a screening instrument for preschool children. A cut-off of 7 with sensitivity of 0.93 and specificity of 0.84 is recommended for community screening.

    The aim of this study is to elucidate the developmental trajectory from infancy to early adulthood of white matter in inferior frontal gyrus (IFG), which is critically related to language processing, using diffusion tensor image DTI. We calculated fractional anisotropy value (FA) and radial diffusivity value (RD) of white matter using MRIcloud in IFG and investigated the change of these value as growth and laterality. As a result, we demonstrated that FA in bilateral IFG rapidly increased by 50 months after birth and, then, the increase became gentle. We also showed the laterality with a higher FA in left IFG. This laterality in IFG suggests there is the superiority of left hemisphere, which may be related to language processing of left hemisphere, in the developmental period as well as adulthood.
