PURPOSE: To identify the factors associated with employment status among mothers of childhood cancer survivors (CCSs). METHODS: We conducted a questionnaire survey on mothers of survivors of childhood cancer to clarify practical factors such as care demands, psychological factors such as motivation to work, and support. After calculating descriptive statistics for all variables, binary logistic regression analysis was performed. RESULTS: Of 171 mothers, 129 (75.4%) were employed. The most common form of employment was non-regular (n = 83; 48.5%), including part-time, dispatched, and fixed-term workers. At the time of the survey, compared with nonworking mothers, working mothers tended to be more motivated to work and have lower scores for "Long-term Uncertainty" on the Parent Experience of Child Illness Scale. The results of the binary logistic regression analysis indicated that employment was related to higher motivation to work, the continuation of employment during treatment, more outpatient visits, and a higher amount of support. CONCLUSION: As employment of CCSs' mothers is associated with psychological factors such as motivation to work and long-term uncertainty, psychological support for CCSs' mothers might promote employment. In addition, because the continuation of employment during treatment affects the employment of mothers after the end of cancer treatment, a leave system that covers the treatment period for childhood cancer needs to be established.

Objective: To report on the teaching design and educational practice of an online pediatric nursing practicum and present a strategy for effective online practices. Methods: From May to July 2020, the University of A offered online nursing practice courses to 30 fourth-year nursing students in three classes of ten students per class. At the end of the course, the students completed a questionnaire survey evaluating the course design and educational practice quality. Results: The average overall class rating was 4.72±0.46. The students’ opinions of the class included such positive comments as these: “I was able to learn through good communication with the instructors,” “I was able to think carefully,” “I had many opportunities to come into contact with other people’s ideas,” and “It was a learning experience similar to the actual clinical practice.” There were also some negative comments, such as “There were few opportunities to consult with the faculty individually” and “The time was too tight.” Conclusions: To ensure that online nursing courses effectively deliver the same high-quality education as in-person classes, the faculty designing the courses must consider the needs of the students and the technology’s limitations.

Background: Assessing the quality of life (QoL) of children receiving end-of-life (EoL) care through evaluations by the children and their bereaved families is challenging; presently, there is no QoL assessment measure that is appropriate for use in pediatric EoL and/or palliative care. Objective: To develop and test a proxy rating scale (the "Good Death Inventory for Pediatrics," GDI-P) for the QoL of pediatric cancer patients receiving EoL care, evaluated from the nurse's perspective, as well as a short version of the scale. Design, Setting, and Measurements: The GDI-P was developed based on previous studies. After initial testing, it was distributed to hospitals across Japan, where nurses in charge of patients with childhood cancer receiving EoL care used the scale to evaluate a patient retrospectively. To examine inter-rater reliability, we encouraged two nurses to evaluate one patient. The GDI-P was modified on the basis of the responses, and the validity and reliability were measured. Results: In total, 85 questionnaires were completed, including 32 pairs of responses from two nurses evaluating one patient. In addition, 47 retest questionnaires were returned. The final, modified GDI-P comprised eight factors with 22 items and showed high convergent and discriminant validity, scaling success rates for each item and factor, and Cronbach's α values. A short version of GDI-P was prepared, comprising eight representative items. Conclusions: The final GDI-P was confirmed to have adequate reliability and validity. The QoL scale developed in this study should provide useful outcome evaluation criteria for assessing the EoL care of pediatric cancer patients.

The purpose of this study was to examine the features of role acceptance and parental role behaviors of the parents of children with neuroses, and the influence of role acceptance and parental role behavior on family function and the mental health of children. The clinical group comprised 14 sets of parents and children with neuroses from grades 5 to 9. The control group comprised 140 sets of parents and normal children from grades 5 to 9. The following results were obtained. There were no differences in role acceptance at work and at home between the two groups. The clinical group reported lower levels of paternal confidence in their nurturing and maternal acceptance of their child than the control group. In the clinical group, paternal role acceptance at work and at home had significantly positive associations with the social functions of children. In contrast, maternal role acceptance at work and at home was not significantly associated with the children's variables. Paternal acceptance of the children, confidence in their nurturing, and facilitation of independence had significantly positive associations with the mental health of the children. Maternal confidence in their nurturing and facilitation of independence also had a significantly positive association with the mental health of the children. However, acceptance of the child had a significantly negative association with the mental health of the children. There was no difference in the child-evaluated family function between the two groups. In the control group, parental acceptance of the children had a significantly positive association with the child-evaluated family function. On the other hand, parents' variables were not associated with the child-evaluated family function in the clinical group.

The main purpose of this research is to clarify the process in which a mother recognizes that the leukemia of her child is in the low risk group. The low risk leukemia has a high frequency in children but has the lowest risk of relapse. Interviews were conducted with seven mothers whose children had low risk leukemia, and the interviews were analyzed with the Grounded theory approach. The major results were as follows: Five categories were generated; (1)the inquiry of the cause of the disease, (2)the sense of the existence of other mothers and children in the similar situation, (3)the realization of the prognosis, (4)keeping the sense of the child being far from death, and (5)goal setting in the daily living of the child. The categories may occur in any order in different individuals, but "goal setting" is always the last step of the process. The core category is "stability in the child' s daily living" . It is suggested that an appropriate nursing intervention for a mother corresponding to each category is important when other children die, when other children relapses, and after her child is discharged from the hospital.

The case patient was a boy first examined at the age of 13. All tests showed no abnormality so the boy was examined by a paediatric nurse counsellor on the advice of the initial doctor. From the initial meeting, existing psychology criteria were used to measure the patient's mental state and the effectiveness of the nursing intervention's input was examined. From December, 1999, the counsellor has continued sessions with the patient for approximately 50 minutes, and with the mother of the patient for about 15 minutes, once every week or two weeks over a period of three and a half years (until August, 2003) . There have been a total of 65 sessions. Since the patient entered senior high school in April, 2002, due to the large change in environment and in respect to the patient's wishes, visits now continue to occur once every one or two months. The patient has been able to enter school and keep up attendance by releasing the patient from mental restraints and controls by supporting the patient without restricting speech or behaviour. A nursing intervention program was also implemented for the parents, and stimulating the restoration and development of the patient's sense of oneself, and the ability to mentally act one's age was also put into practice. In the future, study will focus on independent input of counselling as well as ongoing practices presently used.

The purpose of this study is to investigate how play, as a nursing intervention, changes child inpatients feeling of uneasiness and self-efficacy, or health locus of control. The object is ten children hospitalized in the pediatric ward. Approving of the purpose of this study, they can cooperate in this study by answering the questions. The method is to play with children for approximately 30 minutes a day, which is conducted for 4 days. Childrens feeling of uneasiness and self-efficacy, or health locus of control are measured by the existing scale.The result of this study is as follows : Childrens uneasiness, though not so strong before nursing intervention, becomes weaker after nursing intervention : Health locus of control, powerful others externality was admitted before, becomes weaker for a while, but a week later it returns to the previous level : Self-efficacy tended to be high, and after nursing intervention self-efficacies itself and in learning become much higher : Self-efficacies in friendship and physical exercises become lower.

小児期発症の慢性疾患の子供の多くが思春期，成人期を迎えるようになっているが，これらの患者は原疾患や合併用の病態生理が年齢とともに変化し，さらに新たな合併症も加わって成人の病態の比重が増していくため小児診療科のみで診ていくことは難しく，小児医療から成人医療への移行支援がスムーズに行われる体制が求められている。小児専門病院や総合病院では子供と保護者が主体的に成人医療へ移行できる組織的な体制作りに取り組んでいることから，これらに勤務する小児医療にかかわる看護師は移行期支援についての知識をもっている。しかし，成人領域や地域のクリニックに勤務する看護師は移行期支援についての認識や理解が少ないのが現状である。在院日数の短縮や原疾患以外の疾患（感染症など）は小児科クリニックで治療することになるため，原疾患を踏まえたケアが必要となり，小児科クリニックの看護師には，これまで以上に高度な知識や看護の提供が求められている。子供や保護者の看護に際し，クリニックの看護師には最新の治療法と看護の知識，病院と同程度の水準の看護の提供が求められており，当然ながらこの中には移行期支援も含まれると考える。今年度はCOVID-19の影響で調査協力者（施設）との事前打合せや，調査依頼が難航し，文献レビューと2022年度以降の計画の検討がメインとなった。文献レビューからは移行期支援の多くは事例や症例報告であり，施設間で横断的な取り組みの報告はほぼなかった。また，支援の実際も総合病院で行われているものが多く，地域のクリニックでの報告は見当たらず，総合病院とクリニックで協同した取り組みはほとんどなかった。クリニックにおいても，COVID-19によりオンライン診察が普及しており，オンライン会議ができる環境も整備されてきつつあるため，研究実施においてもオンラインシステムを活用しながら計画的に実施できるように再検討していく。

1.データベース登録項目の加除修正：前年度に引き続きデータベース登録情報の項目の見直しを繰り返して行った。収集する情報に合わせて，情報項目の加除修正を検討し実施した。情報源の再分化，適格化を検討し，データベースに反映する作業を継続して実施した。 2.データユーザー側の活用の実態の把握：データベースを公開した際に，発達障害を持つ子供の保護者の活用を想定した場合，検索し活用する情報をどのように取捨選択するかを把握して，その特性やニーズに合わせたデータベースを構築した方がよいと考え調査を実施した。調査目的は，幼少期の自閉スペクトラム症をもつ子どもの母親が，子どもの育ちと暮らしを支えるために生活の中でどのような行動をとっているのか，その背景として母親が集めた情報をどう活用しているのかを明らかにすることとした。子供の母親7名にインタビューを行い，個々の情報活用の実態をSCAT(Step for Cording and Theorization)を用いて分析した。その結果，(1)ほとんどの母親は初めにネット上の情報を検索して試行するが，自分の子供に合わないと判断すると，その情報の活用を破棄し，以後はネット上の情報を活用しなくなる，(2)母親は子供の発達障害特性に合わせたかかわりを行う目的で情報を活用している，(3)母親は試行錯誤を繰り返しながら自分の子供に合う情報を選択するとともに，子供を支える自分に合う情報源を求めて情報源の取捨選択も行っていることが把握できた。 3.データベースの登録情報，検索方法の再検討：調査結果から，保護者が初めにネット上で検索し入手する情報に，子供の障害特性に関連するキーワードを多重的に付加するなどして，個別性が高い検索結果を提供できるよう登録項目を再検討することにした。

研究目的を達成するために，倫理的調整の実践能力の高い小児専門看護師を対象にインタビュー調査を行い，子供の意思決定能力の構造を明確化することを試みた。研究目的は小児看護専門看護師（小児CNS）が子供の意思決定を支援するプロセスを明らかにすることである。方法は小児CNS ８名を対象に修正版グラウンデッド・セオリー・アプローチにより調査・分析を行った。結果、【子供に合った説明・方法への調整の循環】と【支援の基盤となる小児CNSの認識】の２カテゴリと７のサブカテゴリ＜＞が生成された。小児CNSは＜支援の必要性の認知＞から＜子供の受け止めの理解＞を行い，＜子供の状況を踏まえた説明・提案＞を行っており，子供が［自分で決めるのを待つ］という【子供に合った説明・方法への調整の循環】が生じていた。【支援の基盤となる小児CNSの認識】カテゴリは＜支援の判断の基準となる倫理観＞と＜支援方法を決めるための子供の見極め＞のサブカテゴリからなり，これらが支援の基盤となっていた。結論として子供の意思決定の支援のためには【支援の基盤となる小児CNSの認識】が必要であることが示唆された。この調査により、子どもの意思決定能力の構造を明らかにすることができた。また、＜支援方法を決めるための子供の見極め＞のサブカテゴリには、子供の意思決定能力を評価するための指標に関連する項目が網羅的に含まれており、今後子供の意思決定能力を評価する方法の開発に有益であると考えられた。これらの結果は、令和4年の日本小児看護学会学術集会で公表予定である。

A survey of mothers with severe physical and mental disabilities who suffered the Great East Japan Earthquake at home found that, before the disaster, “disaster preparedness awareness was thin” and “the school had provided disaster preparedness guidance.” One respondent said, “Immediately after the disaster, I felt it was necessary to prepare for disasters,” but stayed in “to prepare for the matter while actually in trouble.” The statements, “The mind does not remain conscious” and “The thought of doing something is not acted upon” were made. Moreover, “insufficient guidance from the hospital about preparation for a disaster” became clear. Even in the survey of nurses, despite the increase in awareness immediately after the disaster, and while teaching to prepare for action is practiced, the current situation is such that awareness and content both become diluted over time.

Through field studies including interviews with professionals in UK, Italy, USA & Australia, the characteristics of total care program for adolescents and young adults (AYA) with cancer and cancer survivors were identified. Two main purposes of them were improving survival rate and quality of life. Each country made great efforts to build systems and infrastructure of multi-institutional clinical trials such as cancer registry specifically for AYA, referral system to specialized institution for detecting cancer and starting treatment in early stages, as well as for increasing access to new trials. Support programs has philosophy of respecting to patients' rights dignity for developing seamless and long-term support programs by using multidisciplinary team approach. Three care delivery models were identified in 4 countries. Fertility, sexuality, and developing models of care delivery will be continuous issues in each country.

This research was carried out with the objective of implementing general overall support and making clear those support needs after finding out the current situation of the health (physical and mental) of infant children, nursery school children, their parents/guardians and childcare providers after the Great East Japan Earthquake. Many parents/guardians and childcare providers in coastal areas affected by damage from the tsunami and earthquake have a high risk of PTSD symptoms. There are also links between a parent/guardian’s state of health and the appearance of psychosomatic symptoms in their children. On top of this, parents/guardians are struggling with how to deal with the physical and mental changes in their children/charges. Results from the study suggested that even though general overall support was found to be necessary for the target groups, a more individualized needs study into mental and physical health of those in the target groups must be implemented first.

The purpose of this study was to examine associations between psychosomatic symptoms and emotion regulation of children, parents’ emotional expression. And, Development of Support Model using the information of EE for the Family, who have Neuroses or Psychosomatic Disorders Children. First, we developed a children’s emotion regulation scale. Second, we examined associations between psychosomatic symptoms and emotion regulation of children. Third, we examined the hypothetical model that parents’ emotional expression (EE) affects children’s emotion regulation levels and psychosomatic symptoms. Parents’ state anxiety were positively associated children’s psychosomatic symptom level. And, fathers’ EE raised girls’ psychosomatic symptom level. Finally, we derived six pattern of parent’s anxiety and EE. Then, we considered the way to support the family who have Neuroses or Psychosomatic Disorders Children.

Nursing care for children with cancer and their families presents various problems in Japan. Many nurses have limited experience in the care for children with cancer , and there are no benchmarks for providing such care. Purpose : This study aimed to investigate the factors associated with ineffective coping behaviors exhibited by children while undergoing venipunctures. Materials and Method : Forty-nine children aged 3-6 years were observed while undergoing venipuncture after their parents responded to a questionnaire. Children were assigned to ineffective (n=12) and effective (n=35) coping behaviors groups based on the Manifest Upset and Cooperation Scales. Results : The children in the ineffective coping behaviors group were younger than those in the effective coping behaviors group (P=0.013). Parent's prediction and children's tendency toward ineffective coping behaviors before venipuncture was higher in the ineffective coping behavior group as compared to the effective coping behavior group (P=0.000). Conclusion : This study strongly recommends assessment of children's age, parents' prediction, and childrens'behavior before venipuncture to help ensure a positive outcome to venipunctures.

It was proven that the tendency of alexithymia of the parent was greatly concerned as the factor which is related to the neurotic child. The tendency of alexithymia of the parent decreases the empathy experience of themselves. The poor of empathy experience of the parent inhibits the emotional interaction with the other of the child. It can be considered that these become risk factor in the neurosis of the child. Then, these relations may be the intergenerational transmission.

小児の白血病や固形腫瘍などの悪性疾患は近年治療成績が向上してきたが,それらの病気はなお「不治の病」というイメージが強いため,我が子が「小児がん」という告知を受けた親の衝撃は大きく計り知れない。小児看護では患児の看護のみならずその家族への看護も必要であるが,悪性疾患患児の母親への援助はまだ十分ではない。そこで一番身近で患児を支える母親への看護を検討する際の基礎とするため,悪性疾患の子どもを持った母親の病気の受容過程を,攻撃性の変化から調査している。調査は東北大学医学部附属病院小児病棟に入院した子どもの母親のうち同意を得られた母親に対して,Buss ＆ Durkee(1957)の攻撃性尺度を中心に,Rosenberg(1965)の自尊感情尺度,大日向(1988)の母性意識尺度,田研式親子関係診断テストを加えて行っている。これまで収集できたデータでは,入院し病名告知を受けて2〜3日後のBuss ＆ Durkee(1957)の攻撃性尺度のうち,身体的攻撃尺度(10項目),言語的攻撃尺度(13項目),短気尺度(11項目)では平均点が低いが,恨み尺度(8項目)では平均点が高かった。また治療の進行に従って,攻撃性が低くなっていく傾向にあった。今後はさらにデータを収集し,攻撃性の変化と病気の受容との関連性および自尊感情,母性意識,養育態度との関連性,その他の関連因子を明らかにしていき,悪性疾患患児への母親への看護を検討していく予定である。
神経症患児における父性の世代間伝播の検討

1.調査・解析の実施と神経症の水準の群別化対照群については,平成12年9〜10月に東北地方の県庁所在地にある小学校2校の協力を得て,質問紙調査を実施した。対象者は小学5,6年生の児童およびその父親213組であり,有効回答は198組から得られた。神経症群については,平成11〜12年に大学附属病院小児科外来で治療を受けた神経症患児とその父親10組に調査を依頼して8組から有効回答を得た。対照群の子どもをGHQ28日本版によって神経症水準群と非神経症群に分類したところ,前者は64名(37.0%)であり,後者は109名(63.0%)であった。 2.非神経症群,神経症水準群および神経症群の比較検討 (1)父親自身が父親から受けた養護的養育態度は,非神経症群,神経症水準群,神経症群の順に弱くなっており,非神経症群,神経症水準群間では差が有意であったが,神経症水準群,神経症群間では有意ではなかった。これらから,神経症患児では患児の父親が自身の父親から受けた養護的養育態度が弱いことが特徴であることが明らかになった。 (2)父親の子どもに対する養育態度の自己評価,および子どもが受けている父親による養育態度の特徴と父親自身が父親から受けた養育態度の関連を3群間で比較した。非神経症群では自身の父親から受けた養育態度が養護的で非干渉的である父親は,子どもへの養育態度がより望ましいパターンを示しており,父性にかかわる養育態度の世代間伝播が明らかであった。神経症水準群および神経症群では,自身の父親から受けた養護的養育態度が弱いにもかかわらず子どもへの養育態度が比較的良いという矛盾したパターンを示しており,これが神経症水準群および神経症群の特異的な養育態度の世代間伝播であることが明確になった。

The aims of the present study are to develop a screening method of developmental disorders in a community and to clarify what kind of support is necessary for families of children with developmental delay. The subjects of the first survey were children and parents who had come to the health checkup for children of a year and six months in Yonezawa City from January 1996 to September 1997. A questionnaire about child development had been mailed to the parents. The questionnaire is consisted of 15 items selected from Early Language Milestone Scale (ELMS) and 11 items selected from Enjoji's Checklist for Child Development. Informed consent was given at the places of health checkup. On the basis of the results of the questionnaire, children were screened. One hundred children were suspected of developmental delay. The subjects of the follow-up study were these 100 children (suspected delay group) and their parents and 100 children who were not suspected of developmental delay (control group) and their parents. The follow-up study was conducted at the age of three years. Forty-five children of suspected delay group and 47 children of control group were responded to the follow-up survey, In the follow-up survey, Suzuki Binet Intelligence Scale was administered and child psychiatrists made Developmental Assessment. Furthermore, investigators assessed needs for supports of the parents. Average IQ of the suspected delay group wad 102 (n=45) and that of the control group was 117 (n=47), According to the results of the developmental assessment at the age of three, three of the suspected delay group were moderately retarded, 14 mildly retarded, 12 on the borderline and 14 normally developed. Seven of them were judged hyperactive. Forty-six of the control group were normally developed and only one of them was mildly retarded. Two of them were judged hyperactive, The results suggested that the screening method of the present study might be useful in a community. The results also indicated the community based educational and psychological support system for families of children with developmental delay should be established.
