Costly Causes of Funeral Dissatisfaction and Satisfaction—Responses to an All-Japan Survey

Background: Previous research shows that grief causes medical and psychological problems for a substantial minority of the bereaved, which places stress on modern medical and social welfare systems. Other research demonstrates that funeral dissatisfaction correlates with medical and psychological problems, but does not address what aspects of funerals cause greatest satisfaction or dissatisfaction. We examined anonymized survey responses describing aspects of funerals causing the greatest anxiety or relief to bereaved Japanese individuals—and in turn affecting medical/welfare costs for the entire society. Method: A research team centered at Kyoto University collected over 1400 questionnaires from recently bereaved Japanese, of whom more than 300 volunteered anonymous comments about their funeral experiences. This article categorizes and analyses these qualitative data. Results: We classified their comments into 10 themes: Hospital Interaction; Pre-arrangement; Timing good-byes; Attendance; Friends/relations; Rituals; Crematorium; Ceremonial meals; Costs; Follow-up. Conclusions: The greatest disappointment appeared in attendance, connection to friends and relations and rituals. When lacking, insufficient, or ill-handled, these leave long-standing bad memories and dissatisfaction that aggravate mourners’ psychological and physical ailments. Our research highlights the psychological effects of cremation, and the value of itemization and explanation of funeral costs. Funeral directors can significantly reduce dissatisfaction by awareness of and sensitivity to these issues, thus contributing to national health and welfare.

Our nationwide postal survey analyzing 190 responses from Japanese bereaved who had responded a year earlier found that funeral dissatisfaction rose during the second year after the funeral. Controverting previous research advocating participation in funeral planning, elderly bereaved spouses forced to decide about and/or pay for the funeral showed elevated grief as much as 2 years later. While not reaching levels diagnosed as prolonged, traumatic, or complicated grief, nevertheless one out of three of our bereaved sample showed continuing daily symptoms of grief from 14 to 24 months after their bereavement, with continued or increasing use of tranquillizers or antidepressants. The medical and pharmaceutical costs incurred by mourners more than a year after bereavement warrant further research into what factors improve or exacerbate the health of grieving bereaved, not limited to a single year after the bereavement.

Objective Severe grief adversely affects the health of bereaved families, potentially burdening medical and community health services. Interventions for effective community health maintenance must identify the people likely to face severe effects of bereavement. The present study identified characteristics of mourners who experience severe grief within a year of bereavement to confirm whether this grief increased their reliance on Japanese medical and social services. Design We conducted a nationwide postal survey of Japanese bereaved within the previous year, to compare those reporting daily or overwhelming ‘heavy’ grief to those with less heavy grief, in terms of demographic and socioeconomic details, daily work and non-work activity, frequency of medical and social service use. Setting/participants In 2019, with the support of the Ministry of Education and the All Japan Funeral Co-Operation, we distributed approximately 5500 questionnaires to Japanese who had presided at funerals within the past year for anonymous return. By January of 2020, we received 1078 complete voluntary responses from bereaved Japanese. Results Half of the ‘heavy grief’ group (n=143) reported adverse effects on health and daily life, including needs for pharmacological, medical or welfare support. Losses of husbands or children were particularly connected to severe grief; ‘unexpected’ death from cancer caused the greatest shock. Employment (even part-time) buffered against severe grief; grief was greater for the unemployed and substantially worse for those who lost significant income at the same time as they lost loved ones. Conclusion These findings suggest that prior counselling should reduce the shock of bereavement and economic loss, which increases subsequent medical dependence. Medical professionals and community health workers can use the above factors to target in advance the family members in greatest danger of heavy grief, to intervene lest grief adversely affect their physical and psychological health after bereavement.

Japan’s super-aged mortality rate bereaves millions of people annually, threatening the mental health of the bereaved population. Previous research suggests that participation in satisfying funeral rituals can protect or improve the health of a bereaved population—but pandemic restrictions threaten traditional funeral assemblies. To determine how bereaved mourners’ mental health—and consequent dependence upon medical, pharmaceutical, or social services—are affected by funerals and the aspects of funerals most likely to cause satisfaction or dissatisfaction, we conducted an anonymous nationwide survey across Japan. In total, 1078 bereaved Japanese responded; we analyzed their responses by comparing the 106 citing funeral dissatisfaction with the 972 citing no dissatisfaction. The cohort showing greatest satisfaction with funerals tended to be older widows or parents who lost children; they showed greater grief but spent less on medical, pharmaceutical, or social services thereafter than the dissatisfied. Conversely, mourners with the greatest dissatisfaction toward their interactions with funeral directors and Buddhist priests tended to spend more on medical, pharmaceutical, or social services after bereavement. We conclude that training or education to improve priests’ and funeral directors’ interactions may reduce dissatisfaction with funerals, potentially reducing subsequent costs of medical, pharmaceutical, or social services for the rapidly growing population of bereaved Japanese.

Bereaved families may experience psychological and physical problems increasing their reliance on medical, pharmaceutical, and financial/legal services. Our Japan-wide survey (n = 1078) researched bereaved who showed increased reliance on medical, pharmaceutical, and financial/legal services. Increased use was most evident in the ‘50’s age bracket, and for unemployed widows; it corresponded less with low annual income than with high income declining significantly after bereavement. Increased users showed higher psychological and physical symptoms of grief, and reported their decline in physical health seriously influencing their work and lives, suggesting “presenteeism”—reduced productivity for those continuing to work. Increased users spent 2.7 times more for medical and pharmaceutical services than those reporting continual use, portending 4 to 10 times more Japanese government expense for this group, half of whom considered their own out-of-pocket expenses a financial burden. These findings warrant further research on cost-effectiveness of interventions to reduce declining health of the bereaved.

Buddhist Chaplains chanting sutras after the Great East Japan Earthquake in 2011 often encountered survivors who felt that hearing sutra chanting itself ameliorated their bereavement grief. This research is the first experimental examination of the effects of sutra chanting on listeners' bereavement stress. Prior research demonstrates that sudden pet loss causes bereavement stress in students and that physiological stress can be noninvasively measured by salivary alpha-amylase. We asked Japanese college students to raise pet goldfish until they developed an attachment to them, then confiscated the fish, and told the students that they had to be killed. To compare the bereavement stress of groups listening and not listening to sutra chanting, we used psychological and salivary analyses. Perceived Stress Scale (PSS), Multidimensional Empathy Scale (MES), and State half of the State Trait Anxiety Inventory (STAI) psychological scales showed no statistically significant differences between sutra and control groups, but salivary analyses indicated measurable stress reduction in the sutra-listening group only. This pilot study tentatively confirmed the hypothesis that listening to Buddhist sutra chanting reduces Japanese bereavement stress. Further research is needed both to verify these stress-reduction effects and to determine whether such effects are primarily musical or cultural/spiritual.

Grief has been shown to weaken bereaved persons' health, but measurements of their lost time and medical expense remain rare. Funerals traditionally managed and assuaged grief through ritual expression, approval, and social support. Research suggests that satisfying funeral participation reduces grief, while poverty exacerbates it. We hypothesized that: (1) psycho-physical symptoms of grief, (2) abbreviation/dissatisfaction in the funeral, and (3) poverty, correlate with decreased productivity and increased medical and social services use. We collected data from 165 mourning families about their grief, funerals, and subsequent medical conditions. (1) Deeper grief after bereavement in Japan correlated with more physical problems, more down time, and more medical dependency. (2) Low satisfaction with funerals correlated with higher hospital, pharmacy, and counseling costs. (3) Low income families lost more time, while declining incomes showed increased pharmaceutical costs. This suggests that satisfying funerals and income safeguards may reduce costs of low productivity and increased public services dependency.

Soon after being hit by a disaster, the local clergies in the West spring to action as a disaster chaplain as a matter of course. They support survivors through spiritual and religious care while maintaining good relations with secular healthcare teams and government. After the Great East Japan Earthquake and Tsunami, many clergies in Japan supported survivors in many ways and people still need their help. They have provided activities around death, grief care, prayer and counseling to console people for their grieving and anxiety. Four cases are shown in this paper to study about how we can establish a system for disaster chaplain in Japan. The clergies in the cases have interfaith relations, do not aim to proselytize, and provide religious care. Before the next disaster happens, we need to have 1) interfaith organizations and common standards for chaplains, 2) relationships of mutual trust with healthcare teams and government, 3) an examination on the quality and effect of religious care, and 4) a system that takes into account the diversity of each community.

The purpose of this study is to show that a person of religion should provide spiritual care prior to religious care when caring for a dying person or family members. The facts that dying persons have questions on death, and that religion can provide answers on death, have encouraged the clergy to participate in hospice care. These two facts, however, do not necessarily grant them entry in Japan. It is necessary to discuss the difference between spiritual care and religious care from the clinical point of view. Five cases, including a failure, are shown and analyzed, through my clinical experience as a Buddhist chaplain at the Vihara Ward (Buddhist Hospice), Nagaoka-Nishi Hospital in Niigata Prefecture. These cases and other studies point out that it is important for a care provider to follow the client's beliefs and not force them to follow one's own doctrine. Thus a person of religion as a care provider should give first priority to spiritual care, and then specific religious care if needed.

In the fields of medicine, social welfare, and education 'spiritual care' is a topic of today. The purpose of this study is to show a Buddhist understanding. According to Christian-based practitioners and my own clinical experience as a Buddhist chaplain in Vihara, it is necessary for a spiritual care provider to accept the patient's existential value unconditionally. The concluding part of the Ratnagotravibhaga could explain this attitude: a spiritual care provider should believe (adhimukti) in the Tathagatagarbha of himself and the patient, the possiblity to resolve the problem, and the meritorious end result.

This study examined the religious care for non-believers by religious people at hospitals and considered how it could be "care" for nonreligious people. First, we conducted a questionnaire survey of 470 medical facilities in Japan where religious people were assumed to be active. In addition, we interviewed religious people working in six hospitals, two Christian hospitals, two Buddhist hospitals, and two new-religious hospitals and asked them what kind of religious care they provide to non-believers. It was found that each hospital has its own distinctive activities, and that there are differences in how they demonstrate religiosity to patients and how they show considerations for other religion. This was not only due to religious differences, but also to the different ways of thinking of different hospitals.

We researched life worth living for seniors from three perspectives: Continuing care retirement communities (CCRC), Continuing bonds, and Intergenerational exchange. Although the American CCRC system seems ideal in some respects, its high cost renders it unpractical for most Japanese elders. The few attempts at CCRC in Japan are all transition types that may pose issues about past relationships. Regarding Continuing bonds, we found that listening to priests’ sutra chanting was useful for Japanese survivors’ grief care. As regards intergenerational exchange, analysis of writing ending notes showed that the young and old generation exhibited distinctly different contents. Therefore, we propose that young and old write ending notes together as activities of intergenerational exchange, because it should prompt young people to understand the elderly better, contribute to elderly happiness, and help to preserve and transmit cultural inheritance.

The following three points were shown in this study. (1) Religious care in the German-speaking world has a rich history under the name of Seelsorge (the care of souls), and there is an established awareness of Seelsorgers (people who care for souls) as professionals responsible for spiritual care in medical settings. (2) Due to an increase in empirical research on the contribution of religion and spirituality to prevent diseases and boost recovery, more and more hospitals employ Seelsorgers, paying labor expenses to churches. (3) It is extremely difficult to create a spiritual care system similar to that of Seelsorge in modern Japan’s setting of clinical death, but programs for interfaith chaplaincy are beginning to be offered at several universities.

We have researched on ‘interfaith chaplain’, who is a professional of religious and spiritual care, and on their way and process to provide care. We organized various ways of care, including ‘the Application of Religious Resources,’ that can show the feature of interfaith chaplaincy. This research revealed the best ethical process of care in public places. Furthermore, the result of our first experiment on the effect of religious care shows that listening to sutra chanting can reduce bereavement stress. These fruits are made use of several training programs. The chaplains are helping the survivors of Tohoku and Kumamoto Earthquakes; as well as patients, families and staff of health care, especially at palliative care settings.

In the disaster area of the Great East Japan Earthquake and tsunami in 2011, bereaved people have various representations of the dead, which are sometimes expressed as ghost phenomena. Religious specialists, such as Buddhist priests, treat them, referring to the context of local religious custom or tradition of the sect they belong to. According to our survey, four common characteristics of how priests treat their clients can be pointed out: (1) Acceptance and listening, (2) Performing rituals, (3) Providing moral instruction, and (4) Promoting self-care for the afflicted. These could be important lessons when considering the nature of “kokoro no kea” by religious specialists among different professionals.

The Dainenbutsu-ji community services administration started from the dormitory for fatherless families founded in Hirano-ku, Osaka-city in June, 1951 gaining support of Yuzu-nenbutsu sect head temple of Dainenbutsu-ji. This research tried examination from three viewpoints about The Dainenbutsu-ji community services administration which it has against the background of the Buddhism idea established in response to a local resident's request.In the first, the historical-records document possessed while the corporate organization had been unsorted was arranged, and the list was created.Together with the measure of Yuzu-nenbutsu sect head temple of Dainenbutsu-ji., it was able to reconfirm about a spread of the Buddhism system social work after World War II by utilizing this data.As the last viewpoint, illustrative examination is tried about the state of the Buddhism social welfare work in the inside of the Japanese society after the Great East Japan Earthquake.

最終目的である国内のスピリチュアルケア専門職の養成については、本研究によって既にある程度の貢献をしている。『仏教福祉』10(2007年3月)などで発表した「スピリチュアルケアの構造」の図は、特定の宗教言語・表現に依拠せずにスピリチュアリティ表現を可能にした。これを8月の8th International World-Congress of International Council on Pastoral Care and Counselling (ICPCC) 2007 (Krzyzowa, Poland)で発表したところ、欧米の臨床家たちから世俗化した現代社会において有益との評価を受けた。市立堺病院・和泉市立病院・府中病院で活動するスピリチュアルケア専門職のために共同開発した、「臨床スピリチュアルケア・サマリーシート」にもこの図が活かされている。次の3点の成果は出稿中で2008年7月までに出揃う予定である。サマリーシート開発についての報告(『ホスピスと在宅ケア』16-1)、共著書『続スピリチュアルケアを語る』(関西学院大学出版会)、共著書『仏教とスピリチュアルケア』(東方出版)。 2007年8月にはドイツにおけるビハーラ活動を調査したが実際にはあまり活発ではなかったため、相応の発表の場を模索している。また、仏教思想に基づいたスピリチュアルケア・アセスメントについての研究を進めており、2年以内に成果を発表するつもりである。ビハーラ関連の理論研究については、前年度までに必要な分析を終えている。文献調査について、仏典の原典調査についてはさらに数年が必要であり、今後も地道に取り組みたい。
