Chronic obstructive pulmonary disease (COPD) remains a leading cause of mortality yet understanding of disease initiation and progression is incomplete. Current therapeutic agents provide symptomatic relief but fail to halt disease progression, highlighting the urgent need to elucidate underlying pathogenic mechanisms, particularly in the preclinical and early stages before irreversible lung damage becomes established. COPD exhibits profound heterogeneity in clinical presentation and disease trajectories, even among patients with similar spirometric measures, indicating that divergent pathogenic mechanisms arise from distinct interactions among genetic, environmental, and temporal factors throughout the lifespan. This review integrates three mechanistically distinct yet biologically interconnected pathogenic processes underlying the progression of COPD. First, we synthesize evidence that genetic susceptibility and developmental trajectories shaped by cumulative early-life exposures determine individual disease risk and phenotypes. Genome-wide association studies have identified more than 80 COPD-associated loci, with polygenic risk scores capturing a substantially greater proportion of genetic liability and enabling identification of high-risk individuals amenable to early intervention. Second, we establish the terminal bronchioles as the primary anatomical site of obstruction, characterized by loss of radial alveolar attachments and aberrant extracellular matrix remodeling that precedes emphysematous tissue destruction. Third, we synthesize emerging single-cell transcriptomic and proteomic evidence revealing that interferon-γ-mediated epithelial-stromal-lymphocyte crosstalk perpetuates tissue destruction through complementary mechanisms: augmented matrix metalloproteinase production overwhelming anti-protease defenses, and direct reprogramming of alveolar epithelial and stromal cells toward apoptosis and senescence. Together, these mechanistic insights provide a unified framework for understanding disease heterogeneity and identifying stage-specific therapeutic targets before irreversible damage becomes established.

<p lang="en">Aim: Despite the revolutionary impact of biologics (Bx) on severe asthma management, predicting individual treatment responses remains challenging. We aimed to characterize the heterogeneous nature of clinical status and disease activity in patients with severe asthma after biologic therapies through a comprehensive evaluation of real-world clinical outcomes. Methods: In this retrospective, multicenter study of 53 patients with severe asthma who received biologic therapies, hierarchical clustering analysis was performed based on three key parameters during treatment: exacerbation, maintenance oral corticosteroid (mOCS) dose, and lung function. Canonical correlation analysis and multinomial logistic regression were used to identify predictors of response patterns. Results: Clustering analysis revealed three distinct control groups: well-controlled (n = 23), moderately controlled (n = 22), and poorly controlled (n = 8). Well-controlled patients exhibited minimal exacerbations, no oral corticosteroid (OCS) use, and optimal or stabilized lung function. Moderately controlled patients showed minimal exacerbations and no mOCS use but variable lung function improvements. Poorly controlled patients exhibited persistent exacerbations, mOCS dependence, or both with limited lung function improvement. Baseline forced expiratory volume in 1 second (FEV1) %predicted (percent predicted FEV1) values and blood eosinophil counts independently differentiated well-controlled from moderately controlled patients, whereas baseline mOCS use distinguished moderately controlled from poorly controlled patients. Conclusions: Our findings reveal distinct patterns of disease control following biologic therapy in severe asthma, with baseline lung function, eosinophilic inflammation, and OCS use as key predictive factors. These results support the need for personalized treatment approaches in severe asthma management.</p>

Immune checkpoint inhibitor (ICI) therapy represents a significant advancement in cancer treatment through immune system activation. However, ICI presents paradoxical immunotherapy infections due to dysregulated immunity (ITI-DI). Here, we report varicella-zoster virus (VZV) meningitis in a patient with malignant pleural mesothelioma undergoing ICI therapy, characterized by disturbed consciousness and cerebrospinal fluid abnormalities. Although antiviral therapy with acyclovir effectively reduced VZV DNA to undetectable levels, persistent cerebrospinal fluid mononuclear pleocytosis was observed, which is akin to immune reconstitution inflammatory syndrome (IRIS). This case highlights the complex immunological interplay during ICI therapy, where immune activation paradoxically creates vulnerability to certain infections through mechanisms distinct from classic immunosuppression. Our findings underscore the importance of recognizing IRIS and ITI-DI as distinct entities requiring specific clinical consideration. We propose that comprehensive monitoring for atypical infections and implementation of pathogen-specific management strategies are essential components of care for patients receiving ICI therapy, particularly given the unique immunological milieu these treatments create.
