I specialize in clinical nutrition and energy metabolism, with a particular focus on older adults and patients with chronic conditions, including heart failure and dysphagia. By integrating clinical practice with advanced measurement techniques such as indirect calorimetry and the doubly labeled water (DLW) method, I aim to generate evidence that enhances the quality of personalized nutrition care in clinical settings.

My research also extends to nutritional epidemiology, home care nutrition, large-scale data analysis, disaster nutrition, and sports nutrition. Ultimately, I seek to contribute to the development of evidence that supports the extension of healthy life expectancy and the improvement of patient-centered outcomes.

I have approximately 10 years of clinical experience and hold multiple professional certifications related to clinical nutrition, nutrition support teams (NST), heart failure, dysphagia management, home care nutrition, and disaster nutrition, including international activities.

I actively welcome collaborative opportunities with researchers and clinicians from diverse disciplines.

    BACKGROUND & AIMS: Malnutrition is highly prevalent in patients admitted to the intensive care unit (ICU), and it is associated with poor outcomes. The Global Leadership Initiative on Malnutrition (GLIM) criteria provide internationally standardised diagnostic criteria; however, their prognostic value for ICU populations has not been investigated using nationwide real-world data. METHODS: We conducted a retrospective cohort study using a nationwide administrative claims database including inpatient and outpatient data between April 2018 and November 2024. To ensure uniform application of the GLIM weight-loss criterion, patients aged 18-99 years who were admitted to ICU and had available information regarding weight changes within the preceding 6 months were included. Nutritional status was classified as no malnutrition, moderate malnutrition, or severe malnutrition according to the GLIM criteria. Primary outcomes were in-hospital mortality, overall mortality, length of hospital stay, and length of ICU stay. RESULTS: A total of 6065 ICU patients were analysed (mean age: 74.4 ± 12.7 years; 63.6% male). The prevalence of GLIM-defined malnutrition was 85.4% (n = 5181). Compared with no malnutrition, severe malnutrition was independently associated with higher in-hospital mortality (subdistribution hazard ratio 1.58 [1.16-2.15], p = 0.004), higher overall mortality (hazard ratio 1.62 [1.32-1.99], p < 0.001), and longer length of hospital stay (+4.09 [2.72-5.46] days, p < 0.001). There was no significant difference in the length of ICU stay (+0.04 [-0.17-0.25] days, p = 0.724). CONCLUSIONS: Severe GLIM-defined malnutrition may be associated with higher mortality and longer hospitalisation among ICU patients. This finding emphasises the importance of early standardised nutritional assessment.

    BACKGROUND AND AIMS: Hip fractures are common among older adults and are associated with functional decline and increased mortality. Body mass index (BMI) is a known prognostic factor for adverse outcomes. However, evidence on nonlinear associations across the entire BMI spectrum remains limited. This study aimed to examine the association between BMI at admission and in-hospital mortality, complications, and changes in functional status as measured by the Barthel Index. METHODS: This retrospective observational study used data from a nationwide inpatient database in Japan between 2014 and 2024. This study included patients aged 65-99 years who had undergone surgery for hip fractures. BMI at admission was categorized according to the World Health Organization Asian criteria: underweight (<18.5 kg/m2), normal (18.5-22.9 kg/m2), overweight (23.0-27.4 kg/m2), and obese (≥27.5 kg/m2). The primary outcome was in-hospital mortality, and the secondary outcomes were in-hospital complications and changes in the Barthel Index. Cox proportional hazards and linear regression models were applied and nonlinear associations were assessed using restricted cubic splines. RESULTS: In total, 146,699 patients were included in the analysis (mean age 84.5 ± 7.5 years; 78.4% female). In-hospital mortality occurred in 2,554 patients (1.7%). The associations of BMI at admission with in-hospital mortality and complications showed L-shaped patterns in the spline curves. The hazard ratio for in-hospital mortality at a BMI of 18.5 kg/m2 was 1.83 (95% CI: 1.69-1.99), compared with a BMI of 23.0 kg/m2 as the reference. A similar L-shaped pattern was observed for complications (BMI 18.5 kg/m2: HR = 1.06, 95% CI: 1.04-1.09). No substantial changes were observed for BMI ≥23.0 kg/m2. Changes in Barthel Index showed a reverse J-shaped pattern, decreasing progressively below 23.0 kg/m2 (18.5 kg/m2: β = -4.44, 95% CI: -4.92 to -3.96) and increasing above this threshold (27.5 kg/m2: β = 1.45, 95% CI: 0.78-2.12). CONCLUSION: BMI at admission is an important prognostic factor of clinical outcomes in patients with hip fractures. Maintaining an adequate BMI through nutritional management before injury may improve survival and functional recovery. However, the exclusion of patients with missing BMI data and the absence of information on surgical delay and other potential confounders should be considered when interpreting these results.

    OBJECTIVE: Selenium, an essential mineral with antioxidant properties, can potentially prevent atherosclerosis and maintain cardiovascular health. However, the association between selenium and ischemic heart disease (IHD) remains unclear. This study aimed to determine the impact of selenium on global IHD incidence (IHDi) and mortality (IHDd) over a 28-year period from 1990 to 2018, using open data for global comparisons. METHOD: IHDi and IHDd per 100,000 people were obtained from the Global Burden of Disease Study (GBD) 2019 database and estimated selenium intake from the Global Dietary Database. Covariates were obtained from the World Bank and GBD databases. The associations of selenium intake with IHDi and IHDd in the 28 years from 1990 onward were analyzed for 149 countries with populations >1 million, using a Bayesian generalized additive mixed model, controlling for covariates. RESULTS: A nonlinear relationship existed between selenium intake and IHDi and IHDd. The selenium intake levels with the lowest risk for IHDi and IHDd were 93.3 and 78.5 µg/d, respectively. The risk ratios (RRs) for IHDi were 2.30 (95% CI, 1.82-2.84) and 1.40 (95% CI, 1.13-1.73) for selenium intakes of 10.0 µg/d and 200.0 µg/d, respectively. For IHDd, the RRs were 3.40 (95% CI, 2.62-4.40) for intakes of 10.0 µg/d and 1.72 (95% CI, 1.31-2.20) for 200.0 µg/d. The risk was higher for selenium underintake than for overintake. CONCLUSIONS: This study revealed a nonlinear relationship between selenium intake and IHDi and IHDd, aiding in establishing a selenium target intake for the primary prevention of IHD and addressing public health problems.

    BACKGROUND: As the global population ages, extending not only the life expectancy (LE) but also the healthy life expectancy (HALE) has become a critical public health objective. Dietary choices, especially the intake of various fatty acids, may play a role in achieving good health. OBJECTIVE: The aim of this study was to describe the intake of fatty acid (FA) fractions across different countries and explore its associations with HALE and LE from 2010 to 2019 according to publicly available international data. METHODS: Data on FA intake in 2010 were obtained from the Global Dietary Database. HALE and LE data were sourced from the Global Burden of Disease Study 2019. A total of 151 countries with populations exceeding 1 million people were included in the analysis. Associations of the intake of each FA with HALE and LE were assessed using linear mixed models adjusted for various covariates. RESULTS: High national omega-3 fatty acid (n-3 PUFA) intake was positively associated with both HALE (β: 2.63, SE: 0.73, p < 0.001) and LE (β: 2.32, SE: 0.63, p < 0.001). These associations remained unchanged when n-3 PUFAs were separated into plant-derived and seafood-derived n-3 PUFAs. No significant associations were observed for the other FAs. CONCLUSION: Higher intake of n-3 PUFAs is positively associated with longer HALE and LE at the national level. These findings underscore the importance of dietary fat composition in promoting healthy aging. Further research is needed to develop public health strategies adapted to each country.

    OBJECTIVE: Prevention and improvement of hypertension is one of the most important health issues worldwide. The traditional Japanese diet contributes to the prevention of some non-communicable diseases associated with hypertension. However, whether traditional Japanese dietary patterns are associated with hypertension prevalence and systolic blood pressure (SBP) worldwide remains unknown. In this study, we aimed to investigate whether the traditional Japanese diet score (TJDS) is associated with hypertension prevalence and SBP. METHODS: This cross-sectional and longitudinal ecological study from 2009 to 2019 included 141 countries with a population of at least 1 million. All data used for analysis were obtained from internationally available databases. The TJDS was calculated using country-specific supplies of five foods commonly eaten in the traditional Japanese diet (rice, fish, soybean, vegetables, and egg) and three less commonly eaten foods (wheat, milk, and red meat). Hypertension was defined by SBP ≥140 mmHg, diastolic blood pressure ≥90 mmHg, or use of antihypertensive medications. The mean SBP was >25 age-standardized data. A general linear model was used for the cross-sectional model in 2009, and a linear mixed model was used for the longitudinal analysis from 2009 to 2019. The covariates included gross domestic product, percentage of population aged >65 years, years of education, smoking rate, average body mass index, physical activity, and energy supply. RESULTS: The TJDS was negatively associated with hypertension prevalence (β ± standard error: -0.861 ± 0.202, p < 0.001) and SBP (β ± standard error: -0.364 ± 0.154, p < 0.05) in the cross-sectional analysis. The TJDS was significantly negatively associated with hypertension prevalence (β ± standard error: -0.845 ± 0.200, p < 0.001) and SBP (β ± standard error: -0.312 ± 0.149, p < 0.05) in the 10-year longitudinal analysis controlled for full covariables. CONCLUSIONS: Traditional Japanese dietary patterns are associated with lower hypertension prevalence and SBP worldwide.

    BACKGROUND: Reducing the environmental impact of the food supply is important for achieving Sustainable Development Goals (SDGs) worldwide. Previously, we developed the Traditional Japanese Diet Score (TJDS) and reported in a global ecological study that the Japanese diet is associated with reducing obesity and extending healthy life expectancy etc. We then examined the relationship between the TJDS and environmental indicators. METHODS: The average food (g/day/capita) and energy supplies (kcal/day/capita) by country were obtained from the Food and Agriculture Organization of the United Nations Statistics Division database. The TJDS was calculated from eight food groups (beneficial food components in the Japanese diet: rice, fish, soybeans, vegetables, and eggs; food components that are relatively unused in the traditional Japanese diet: wheat, milk, and red meat) by country using tertiles, and calculated the total score from - 8 to 8, with higher scores meaning greater adherence to the TJDS. We used Land Use (m2), Greenhouse gas (GHG) emissions 2007/2013 (kg CO2eq), Acidifying emissions (g SO2eq), Eutrophying emissions (g PO43- eq), Freshwater (L), and water use (L) per food weight by Poore et al. as the environmental indicators and multiplied these indicators by each country's average food supply. We evaluated the cross-sectional and longitudinal associations between the TJDS and environmental indicators from 2010 to 2020. This study included 151 countries with populations ≥ 1 million. RESULTS: Land use (β ± standard error; -0.623 ± 0.161, p < 0.001), GHG 2007 (-0.149 ± 0.057, p < 0.05), GHG 2013 (-0.183 ± 0.066, p < 0.01), Acidifying (-1.111 ± 0.369, p < 0.01), and Water use (-405.903 ± 101.416, p < 0.001) were negatively associated with TJDS, and Freshwater (45.116 ± 7.866, p < 0.001) was positively associated with TJDS after controlling for energy supply and latitude in 2010. In the longitudinal analysis, Land Use (β ± standard error; -0.116 ± 0.027, p < 0.001), GHG 2007 (-0.040 ± 0.010, p < 0.001), GHG 2013 (-0.048 ± 0.011, p < 0.001), Acidifying (-0.280 ± 0.064, p < 0.001), Eutrophying (-0.132 ± 0.062, p < 0.05), and Water use (-118.246 ± 22.826, p < 0.001) were negatively associated with TJDS after controlling for confounders. CONCLUSIONS: This ecological study suggests that the traditional Japanese dietary pattern might improve SDGs except Fresh water.

    BACKGROUND/OBJECTIVES: Resting energy expenditure (REE) constitutes the largest component of total energy expenditure and undergoes an age-related decline that is unexplained by decreased fat-free mass. Phase angle (PhA) is a cellular health indicator that is possibly associated with REE. We investigated the association of REE and PhA in hospitalized older adults. SUBJECTS/METHODS: This single-center, cross-sectional analysis utilized the baseline data from a prospective longitudinal study and included 131 eligible patients aged ≥70 years. The REE was measured using indirect calorimetry, and PhA and body composition were assessed using bioelectrical impedance. The association between REE, PhA, and body composition was examined, and REE was compared using previously reported PhA cutoff values. RESULTS: In this cohort with a mean (±standard deviation) age of 87.4 (±7.0) years, 34.4% of the participants were men. REE and PhA correlated strongly (r: 0.562, p < 0.001) and significantly after adjusting for age and sex (r: 0.433, p < 0.001). Multivariate analysis showed a significant independent association between REE and PhA and skeletal muscle mass (standardized β [95% CI]; 28.072 [2.188-53.956], p = 0.035) without any significant interaction between PhA and age on REE. The low PhA group had a significantly lower REE (kcal/day; 890 [856-925] vs. 1077 [1033-1122], p < 0.001), and this remained significant after adjusting for age, sex, and skeletal muscle mass index. CONCLUSIONS: PhA is associated with REE in older adults. Adjusting REE calculation algorithms based on PhA values and correcting predicted REE according to PhA may aid in determining more accurate energy requirements.

    OBJECTIVES: Accurate resting energy expenditure (REE) prediction is needed to prevent over- or underfeeding in older hospitalized patients. However, few validated REE prediction Equations are known for such patients. Therefore, this study aimed to develop new REE prediction Equations and evaluate their validity. METHODS: This single-center, cross-sectional study enrolled 134 patients ages ≥70 y. For holdout validation, patients were randomized in a 3:1 ratio; for the development data set, a new Equation was developed according to the measured REE using indirect calorimetry. The new and existing Equations were compared using the validation data set. RESULTS: Mean patient age was 87.4 ± 6.9 y, and 34.3% were male. Two Equations were developed in multivariable regression models: Equation 1: REE (kcal/day) = 313.582 + Height (cm) × 3.973 + Body weight (kg) × 5.332 - Age (y) × 5.474 - (0 if male; 1 if female) × 20.012 + Calf circumference (cm) × 12.174; and Equation 2: REE (kcal/day) = 594.819 + Height (cm) × 3.760 + Body weight (kg) × 8.888 - Age (y) × 6.298 - (0 if male; 1 if female) × 16.396. The mean relative bias (95% CI) with measured REE as a reference had a small bias for Equations 1 and 2 (-0.1 [-4.1 to 3.9]% and -0.2 [-4.4 to 4.1]%, respectively); however, the Harris-Benedict, Food and Agriculture Organization of the United Nations/World Health Organization/United Nations University, Ganpule, and body weight × 20 Equations had larger biases (-6.2 [-10.3 to -2.0]%; 5.3 [1.3 to 9.3]%; -13.9 [-18.6 to -9.3]%; and -11.6 [-16.1 to -7.1]%, respectively). CONCLUSIONS: New prediction Equations using height, body weight, age, sex, and calf circumference improve REE prediction accuracy in older hospitalized patients.

    Interventions for stroke-related sarcopenia in patients with stroke are needed, but the details of the target population are unclear. This systematic review aimed to identify trajectories of the prevalence of sarcopenia in the pre- and post-stroke periods and to determine the diagnostic criteria used in patients with stroke. We searched for literature in six databases: MEDLINE, EMBASE, Web of Science, Cochrane Central Register of Controlled Trials, CINAHL, and Ichushi-web (in Japanese). We included 1627 studies in the primary screening, and 35 studies were finally included. Of the 35 studies, 32 (91.4%) included Asian patients, and the criteria of the Asian Working Group for Sarcopenia was mainly used as the diagnostic criteria. Nineteen studies used muscle strength and muscle mass to diagnose sarcopenia, whereas a full assessment, including physical performance, was performed in five studies. The estimated prevalences of sarcopenia in pre-stroke, within 10 days of stroke, and from 10 days to 1 month after stroke were 15.8%, 29.5%, and 51.6%, respectively. Sarcopenia increased by approximately 15% from pre-stroke to 10 days, and increased by approximately 20% from 10 days to 1 month. Healthcare providers should note that the prevalence of sarcopenia increases during the acute phase in patients with stroke.

    OBJECTIVE: Studies conducted on Japanese people have suggested that a traditional Japanese diet contributes to good health, longevity, and protection against several non-communicable diseases. However, it is unknown whether traditional Japanese dietary patterns are associated with all-cause mortality, cardiovascular disease, and cancer mortality globally. The purpose of this cross-sectional and longitudinal ecological study is to clarify the global association between the traditional Japanese diet score (TJDS) and all-cause, cardiovascular disease, and total cancer mortality. METHODS: Data on food supply and all-cause mortality, cardiovascular disease mortality, total cancer mortality, and covariables by country were obtained from a relevant internationally available database. TJDS by country was calculated from eight food groups and the total score ranged from -8 to 8, with higher scores indicating greater adherence to a traditional Japanese diet. We evaluated the cross-sectional and 10-year longitudinal association between TJDS and all-cause, cardiovascular disease, and total cancer mortality using 2009 as the baseline in 142 countries with populations of more than one million. A cross-sectional analysis and a longitudinal analysis were performed using three general linear models or three linear mixed models with different covariables. RESULTS: In cross-sectional models controlled for fully-adjusted covariables, TJDS was negatively associated with all-cause mortality (β ± standard error; -43.819 ± 11.741, p < 0.001), cardiovascular disease mortality (-22.395 ± 4.638, p < 0.001), and total cancer mortality (-3.893 ± 1.048, p < 0.001). In 10-year longitudinal models controlled for fully-adjusted covariables, TJDS was significantly negatively associated with all-cause mortality (-31.563 ± 7.695, p < 0.001), cardiovascular disease mortality (-16.249 ± 4.054, p < 0.001), and total cancer mortality (-3.499 ± 0.867, p < 0.001). CONCLUSIONS: This cross-sectional and longitudinal ecological study suggests that the traditional Japanese diet is associated with lower all-cause mortality, cardiovascular disease mortality, and total cancer mortality, worldwide.

    The association of Japanese and Mediterranean dietary patterns with muscle weakness in middle-aged and older Japanese individuals is unclear. This cross-sectional study investigated the association between Japanese and Mediterranean dietary patterns and muscle weakness in community-dwelling, middle-aged, and older Japanese individuals (enrolled from 2007 to 2011). Based on the dietary consumption information obtained from the brief self-administered diet history questionnaire, we assessed adherence to the Japanese (12-component revised Japanese diet index (rJDI12)) and Mediterranean (alternate Mediterranean diet (aMed) score) dietary patterns. Muscle weakness was defined as handgrip strength &lt;28 and &lt;18 kg for men and women, respectively. Logistic regression was used to ascertain the relationship between dietary pattern and muscle weakness. In our study, with 6031 participants, the Japanese, but not Mediterranean, dietary pattern was inversely associated with muscle weakness (p for trend = 0.031 and 0.242, respectively). In the model adjusted for confounders, including energy intake, the highest quartile of rJDI12 scores (9-12 points), and the rJDI12 scores, entered as continuous variables, showed an independent association (odds ratio (95% CI), 0.703 (0.507-0.974), and 0.933 (0.891-0.977), respectively). Our findings showed that adherence to the Japanese dietary pattern is associated with a low prevalence of muscle weakness.

    In previous observational studies, the association between coffee intake and risk of cardiovascular disease has reversed from positive to negative over time. This long-term international ecological study examined whether the association between coffee intake and mortality and incidence rates of ischemic heart disease (IHD) changed between 1990 and 2018 using multiple coherent data. We obtained data on coffee intake per capita, IHD mortality and incidence rates per 100,000 population, and socioeconomic and lifestyle indicators for each country from various publicly available databases. We integrated and analyzed data from 147 countries with populations of ≥ 1 million. We employed a linear mixed model analysis to assess the association between coffee intake and IHD mortality and incidence rates by year. The mean global coffee intake increased (p < 0.001), whereas IHD mortality (p < 0.001) and incidence (p = 0.073) decreased. In all models, the interaction between coffee intake and year showed a significant inverse association for IHD mortality and incidence rates (p < 0.001 for all). The country-level association between coffee intake and IHD mortality and incidence rates between 1990 and 2018 was stronger in the negative direction.

    BACKGROUND: This study examined the global association between the Traditional Japanese Diet Score and suicide rate. METHODS: Average food supply and energy supply by country were obtained from the Food and Agriculture Organization of the United Nations Statistics Division database (FAOSTAT). Traditional Japanese Diet Score (TJDS) was calculated from food group classifications. Age-standardized suicide rates per 100,000 people by country were obtained from the Global Burden of Disease 2017 database. The 26-year associations between TJDS and suicide rates were examined controlling for covariates using a mixed-effects model in 138 countries with populations of 1 million or greater. RESULTS: A significant negative association was found in the analysis of the relationship between TJDS and the prevalence of suicide [β (se) = -0.126 (0.031), p < 0.001]. The interaction term between TJDS and year was also significant [β (se) = -0.011 (0.003), p < 0.001]. The change in the effect of TJDS on suicide rates by year was examined. The effect of TJDS was not significant from 1991 to 1999 but became significant after 2000; the slope of the TJDS to suicide rate in 2017 was -0.270 (95% confidence interval -0.382, -0.169, p < 0.001). LIMITATIONS: This was a national ecological study and did not consider individual differences in sex, age, and lifestyle. CONCLUSIONS: TJDS may be is a protective factor for depression, which may prevent suicide. The association between TJDS and suicide rates became stronger over time and was inversely associated with suicide rates after 2000.

    BACKGROUND: The traditional Japanese diet is considered one of the important factors of health and longevity in Japanese people. Breast cancer is the most common cancer among women in the world. However, the association between the traditional Japanese diet and breast cancer is unclear. The purpose of this study was to investigate the longitudinal association between the traditional Japanese diet score (TJDS) with the incidence and mortality of breast cancer in an ecological study. METHODS: Food supply and breast cancer incidence and mortality by country were obtained from an international database. TJDS by country was calculated from nine food groups and the total score ranged from -9 to 9, with higher scores indicating greater adherence to a traditional Japanese diet. Longitudinal associations of interaction between TJDS and fiscal year on breast cancer incidence and mortality were investigated in 139 countries with populations of 1 million or greater. The longitudinal analysis was evaluated using four linear mixed-effect models with different adjustment covariables. RESULTS: Many countries with high scores on TJDS had lower distributions of breast cancer incidence and mortality in 1990-2017. Longitudinal analysis using a linear mixed-effect model controlled for socio-economic and lifestyle covariables showed that the interaction between TJDS and fiscal year was significantly associated with incidence of breast cancer (-0.453 ± 0.138, p < 0.01) and mortality of breast cancer (-0.455 ± 0.135, p < 0.001). CONCLUSIONS: This longitudinal analysis suggested that a traditional Japanese diet has been associated with lower breast cancer incidence and mortality worldwide in recent years.

    BACKGROUND: The purpose of this study was to clarify the global relationship between the Mediterranean diet score (MDS) and country-wise incidence and mortality of ischaemic heart disease (IHD) using an international database. METHODS: We used population data from a global longitudinal database covering 137 countries with a population of over one million. MDS were evaluated based on the total score of the nine foods that comprise the Mediterranean diet. The incidence and mortality of IHD by country was derived from the Global Burden of Disease (GBD) database. Average food (g/day/capita) and energy supply (kcal/day/capita) by country, excluding loss between production and household, were obtained from the Food and Agriculture Organization of the United Nations Statistics Division database. Data from the GBD database were used for body mass index, current smoking rates, physical activity, years of education and percentage of the Muslim population. We identified the percentage of the population over 65 years of age (aging rate) and gross domestic product per capita (US$/capita) using the World Bank database. A linear mixed-effect model was used for evaluating the effects of MDS on incidence and mortality of IHD controlled for socioeconomic and lifestyle variables. RESULTS: Analysis showed that MDS was significantly associated with IHD incidence after controlling for covariates (-1.01 ± 0.27, P < 0.001). Similarly, there was a significant association between MDS and IHD-related mortality after controlling for covariates (-0.73 ± 0.34, P < 0.05).     Background: There is a need for global public health strategies to effectively curb the ever-growing global diabetes population. This longitudinal ecological study was conducted to elucidate the country-specific trends of the age-standardised prevalence of type 2 diabetes mellitus (T2DM) and their association with socioeconomic, lifestyle and health indicators. Methods: Data pertaining to the age-standardised prevalence of T2DM (rates per 100 000) and socioeconomic, lifestyle and health indicators were obtained from several international databases. Data available from 1990 to 2017 for 139 countries with populations of 1 million or greater were analysed, followed by estimation of slopes for T2DM prevalence in each country. The longitudinal association between T2DM and the standardised variables of socioeconomic, lifestyle and health indicators was fitted using a generalised linear mixed-effects model with random intercept for each country and random slope for year. Results: The country-specific age-standardised prevalence of T2DM decreased significantly in 9 countries, remained unchanged in 11 countries and increased significantly in 119 countries. The estimated standardised effect of age-standardised education for T2DM was the largest at -524.5 (SE; 109.9), followed by -514.8 (SE; 95.6) for physical activity and 398.8 (SE; 45.8) for body mass index (BMI) (P < 0.0001 for all). Conclusions: The factors largely associated with global T2DM prevalence and trends were years of education, followed by physical activity and BMI. This study also provides basic resources for examining public health approaches to curb the increase in global T2DM prevalence.

    【目的】全世界で3 億人以上の人たちが抑鬱状態であり、さらに80万人に近い人たちが自殺によって死亡している。抑鬱は身体障がいの最大の要因である。近年、食事と鬱病との関連が注目を集めており、特に抗酸化物質と抗炎症成分が豊富な野菜類は、鬱病の発症予防に有益な効果を持つ可能性が指摘されている。本研究の目的は、野菜類・果実類の供給量と鬱病有病率との22年間の縦断的関連を国際比較研究で明らかにすることである。【方法】生産から家計までのすべての段階における減耗を除く、各国の食品供給量と総エネルギー供給量を国連食糧農業機関データベース(FAOSTAT)から入手し、食品群分類から野菜類の供給量を求めた。鬱病については、Global Burden of Disease( GBD) 2017データベースから各国の10万人当たりの年齢標準化鬱病有病率を入手した。調整変数として、世界銀行データベースから人口、国民一人当たりの国内総生産(GDP)、高齢化率、失業率、GBD から平均BMI、喫煙率、教育年数、国別の中心経度緯度を入手した。データの得られた100万人以上の人口を持つ137カ国を対象とし、共変量を調整した線形混合モデルを用いて、野菜類、果実類の供給量と鬱病有病率との1991年から2013年までの22年間の縦断的関連について解析を行った。解析にはR 3.5.3を用いた。【結果】野菜類供給量と鬱病有病率との関連の縦断的解析では、すべての共変量を調整したモデルで有意な負の関連が認められた(β = -0.058±0.028、p <0.05)。また、果実類の供給量と鬱病有病率についても同様にすべての共変量を調整したモデルにおいて、有意な負の関連が認められた(β=-0.097±0.036、p <0.01)。【結論】野菜類および果実類の供給量は鬱病有病率と有意な負の関連を示した。豊富な野菜類や果実類を摂取する食生活は鬱病有病率を低下させる可能性が示された。 Background and objective: More than 300 million people worldwide are depressed and nearly 800,000 people have died from suicide. Depression is the biggest cause of disability. The association between diet and depression has drawn attention in recent years. Among the dietary factors, vegetables and fruits, which are rich in antioxidants and anti-inflammatory components, were hypothesized to play an important role in depression development. The aim of this research is to clarify the longitudinal association of vegetables and fruits with depression rate using 22-year worldwide statistics.Methods: Average food supply (g/day/capita) and energy supply (kcal/day/capita) by country, excluding loss between production and household, were obtained from the Food and Agriculture Organization of the United Nations Statistics Division database (FAOSTAT). Each food was sorted, and supplies of vegetables and fruits were obtained. Data of age-standardized prevalence of major depression per 100,000 people by country were derived from the Global Burden of Disease (GBD) 2017 database. As control variables, population, gross domestic product (GDP) per capita, aging rate, and unemployment rate by country from the World Bank database, and BMI, smoking rate, expected years of education, and central longitude and latitude by GBD database. The 22-year longitudinal associations of fruits and vegetables with major depression were examined in the 137 countries with populations of 1 million or greater controlling for covariates by the mixed effect model.Results: A significant negative association was found by the longitudinal analysis of the relationship between the vegetables supply and the prevalence of major depression in the model controlled for all covariates (β = –0.058 ± 0.028, p<0.05). In addition, a significant negative association between the supply of fruits and the prevalence of major depression was also found in the model controlled for all covariates (β = –0.097 ± 0.036, p<0.01).Conclusions: Vegetables and fruits supply were significantly negatively associated the rate of major depression. Vegetables and fruits may reduce the prevalence of depression.

    【目的】乳癌は女性にとって主要な癌の一つである。これまでに、乳癌と乳製品摂取との関連がメタアナリシスによって検討されているが、一貫した結論が得られていない。その理由として、食事調査の手法や乳製品摂取量の評価が論文間で異なること、調査地域が限られていることが挙げられる。そのため、乳製品と乳癌の関連をさらに明らかにしていくためには、地球規模で同一手法を用いた乳製品摂取量の調査による研究が必要と考えられる。そこで本研究では、現在公表されている国際データを用い、乳癌と乳製品の関連を地球規模による国際比較研究で縦断的に明らかにすることを目的とした。【方法】乳癌発症率および乳癌死亡率は、Global Burden of Disease( GBD) 2017データベースから各国の10万人当たりの年齢標準化された値を入手した。生産から家計までのすべての段階における減耗を除く、各国の食品供給量と総エネルギー供給量は国連食糧農業機関データベース(FAOSTAT)から入手し、乳製品の供給量を求めた。調整変数として、人口、国民一人当たりの国内総生産(GDP)、高齢化率(以上世界銀行データベース)、平均BMI、喫煙率、教育年数、身体活動量(以上GBD データベース)を入手した。全てのデータが得られた100万人以上の人口を持つ139カ国を対象とし、共変量を調整した線形混合モデルを用いて、乳製品供給量と乳癌発症率および乳癌死亡率との1990年から2013年までの23年間の縦断的関連について解析を行った。解析にはR 3.6.1を用いた。【結果】すべての共変量を調整したモデルにおいて、乳製品供給量と乳癌発症率の間に有意な正の関連があった(β= 7.393、標準誤差1.553、p<0.001)。同様に、すべての共変量を調整したモデルにおいて、乳製品供給量と乳癌死亡率の間にも有意な正の関連があった(β= 2.123、標準誤差0.613、p<0.001)。【結論】乳製品供給量と乳癌発症率および乳癌死亡率が正の関連を示すことを、比較的近年のデータを用いて縦断的に明らかにした。このことから、乳製品を多く摂取する食生活は、地球規模において乳癌発症率や乳癌死亡率を上昇させる可能性が示された。今後さらに、乳製品を低脂肪と高脂肪に分けて解析を行うことが必要である。 Background and objective: Breast cancer is one of the major cancers for women. The relationship between breast cancer and dairy products has been studied, however, the results differ among the previous studies. The reasons seem to be that dietary survey methods of previous studies are inconsistent, and the survey countries are limited. The 23-years longitudinal associations of dairy products supply with breast cancer were examined in the 139 countries with populations of 1 million or greater controlling for covariates by the mixed effect model.Results: The model controlled for all covariates was a significant positive association between dairy products supply and breast cancer incidence (β = 7.393±1.553, p <0.001). Similarly, the model controlled for all covariates was also a significant positive association between dairy products supply and breast cancer mortality (β = 2.123±0.613, p <0.001).Conclusions: Dairy products supply was significantly positively associated the breast cancer incidence and mortality. These results suggest that breast cancer incidence and mortality may increase with increasing dairy consumption worldwide.

    Background and objective: Healthy life expectancy (HALE) is the number of years from age 0 that can be expected to maintain good health. The purpose of this study was to clarify the difference between HALE basedon the Ministry of Health, Labor and Welfare's (MHLW) "Comprehensive Survey of Living Conditions" and HALE obtained by an international method based on the Global Burden of Disease Study (GBD) in Japan on the change over time and difference by prefecture.Methods: The large-scale survey of "Comprehensive Survey of Living Conditions" is conducted once every three years, and the results are used to calculate HALE (MHLW-HALE). The HALE by sex from 2001 to 2016, and the HALE by prefecture in 2016 were used for analysis. The annual HALE in Japan based on the GBD 2017 database from 1990 to 2017 were also used. The life expectancy based on abridged life table and complete life table by MHLW from 1990 to 2017 was used and the difference between the life expectancy and the HALE was defined as the "unhealthy years". The changing rates of MHLW-HALE and GBD-HALE were compared. The changing rate of the "unhealthy years" were also compared. For the HALE by prefecture, the correlation between MHLW-HALE and GBD-HALE was assessed. The difference between MHLW-HALE and GBD-HALE by prefecture was also assessed. R 3.5.3 was used for analysisResults: The annual changing rate for men was 0.180 ± 0.023 / year (p = 0.001) for MHLW-HALE and 0.135 ± 0.003 / year (p < 0.001) for GBD-HALE, and the former was slightly larger. For women, the changing rate of MHLW-HALE was 0.152 ± 0.012 / year (p < 0.001), and that of the GBD-HALE was 0.130 ± 0.006 / year (p <0.001), again, the changing rate was larger in the former. The difference between MHLW-HALE and GBDHALE was up to 1.0 years, and the difference was not so large, but the secular change was very different. In particular, the "unhealthy years" by MHLW-HALE seemed to tend to be shortened recently, but the "unhealthy years" by GBD-HALE was getting consistently longer. In the HALE by prefecture, no correlation was found between MHLW-HALE and GBD-HALE, and the former was significantly longer.Conclusions: MHLW-HALE and GBD-HALE showed a difference in secular changes and prefecture data. MHLW-HALE seems to have a certain role as a health index in Japan. However, GBD-HALE is estimated over the long term using the same objective method worldwide, which is useful in international comparisons.

    OBJECTIVE: The aim of the present study was to clarify the global relationship between Mediterranean diet score (MDS) and the incidence of IHD by country using international statistics. DESIGN: The incidence of IHD by country was derived from the Global Burden of Disease (GBD) database. Average supplies of food (g/d per capita) and energy (kcal/d per capita) by country, excluding loss between production and household, were obtained from the FAOSTAT database. MDS was evaluated based on the total score of nine food items that characterize the Mediterranean diet. The association between MDS and the incidence of IHD was examined in countries with a population of 1 million or greater using a general linear model controlled for socio-economic and lifestyle variables. SETTING: Population data from global international databases.ParticipantsOne hundred and thirty-two countries with a population of over 1 million. RESULTS: MDS was inversely correlated with obesity rate, ageing rate, years of education and IHD incidence; however, no associations were found with gross domestic product, life expectancy, smoking rate, energy supply or health expenditure. In the general linear model of IHD incidence by MDS controlled for socio-economic and lifestyle variables, the β of the MDS was -26·4 (se 8·6; P&lt;0·01). CONCLUSIONS: The results of this global international comparative study confirmed that the Mediterranean diet is inversely associated with the incidence of IHD.

    BACKGROUND/OBJECTIVES: The prolongation of healthy life expectancy (HALE) is a core issue of health policy in many countries. The purpose of this study is to clarify the relationship between dietary diversity and HALE using international databases. MATERIALS/METHODS: HALE data by country were derived from the Global Burden of Disease (GBD) 2015 database. Average food supply (g/day/capita) and energy supply (kcal/day/capita) by country, excluding loss between production and household, were obtained from the Food and Agriculture Organization of the United Nations Statistics Division database. Each food was sorted across 12 food groups, and dietary diversity was obtained from food groups using the Quantitative Index for Dietary Diversity (QUANTIDD). The cross-sectional and longitudinal associations between QUANTIDD and HALE were examined in the countries with populations of one million or greater. RESULTS: Cross-sectional analysis showed that HALE was significantly associated with QUANTIDD (β = 99.9 ± 11.4, p < 0.001) in the single regression model and in the multiple regression model controlled for covariates (β = 36.4 ± 11.3, p = 0.002). Longitudinal analysis showed that HALE increased with QUANTIDD during the 15-year study period (β = 46.4 ± 5.1, p < 0.001), and this association was also significant when controlled for covariates (β = 39.7 ± 5.1, p < 0.001). Longitudinal association of QUANTIDD with the percentage difference between life expectancy and HALE controlled for covariates was significantly negative (β = - 1.3 ± 0.5, p = 0.011). CONCLUSIONS: After controlling for socioeconomic indicators, longer healthy life is enjoyed by populations of countries with greater dietary diversity.

    OBJECTIVES: We created a Traditional Japanese Diet Score (TJDS), and to clarify the relationship between TJDS and obesity, ischemic heart disease (IHD), and healthy life expectancy (HALE). DESIGN: Ecological study. SETTING: Food (g/day/capita) and energy (kcal/day/capita) supply was determined using the Food and Agriculture Organization of the United Nations Statistics Division database. The sum of characteristic traditional Japanese foods (beneficial food components in the Japanese diet: rice, fish, soybeans, vegetables, eggs, and seaweeds; food components rarely used in the Japanese diet: wheat, milk, and red meat) was divided as tertiles (beneficial food components: -1, 0, 1; rarely used food components: 1, 0, -1). Obesity rate was determined using the World Health Organization database. Incidence of IHD, HALE and smoking rate were determined using the Global Burden of Diseases, Injuries, and Risk Factors Study 2015 database. Gross domestic product per capita, percentage of population > 65 years old, and health expenditure were determined using the World Bank database. Education years were obtained from the United Nations Educational, Scientific and Cultural Organization Institute for Statistics. Associations between TJDS and obesity, IHD and HALE were examined in 132 countries with a population of 1 million or greater using a general linear model controlled for co-variables. RESULTS: TJDS was distributed from -6 to 7. TJDS was inversely correlated to obesity (β±SE; -0.70±0.19, p<0.001), IHD (-19.4±4.3, p<0.001), and positively correlated to HALE (0.40±0.14, p<0.01). CONCLUSIONS: TJDS is a good indicator of a healthy diet, and applies to preventing obesity, IHD and extending HALE.

    Purpose: Healthy life expectancy (HLE) is defined as the probable period of life where people are expected to live on their own without the aid of nursing and without getting so sick that their daily activities are interrupted. The difference between life expectancy (LE) and HLE is "unhealthy period" which is restricted in daily life. To make this "unhealthy period" as short as possible is the biggest health policy issue in many countries around the world, including Japan. In this study, we aimed to clarify the difference between LE and HLE from the international comparative study and the factors influencing this difference.Methods: Data on LE and HLE by country were obtained from the database of Global Burden of Disease (GBD) 2015. We obtained energy supply, energy ratio of lipids and proteins, education years, obesity rate, smoking rate, total population, aging rate, global domestic product (GDP), and health expenditure by country from the international database. We analyzed 131 countries with data of over 1 million population to investigate the factors affecting the difference between LE and HLE by stepwise multiple regression.Results: In the data of 2015, both Japan's LE and HLE were the world's best. The difference between LE and HLE was larger as LE was longer. However, the difference in Japan was 9.27 years, which was 60th in 131 countries. In the stepwise multiple regression including socioeconomic and lifestyle factors, the obesity rate, GDP per capita, and energy supply amount were significant increasing factors of the difference between LE and HLE. The obesity rate and health expenditure per capita were factors that increase the ratio of the difference between LE and HLE, and the education years and the aging rate were the factors that reduced the ratio of the difference.Conclusion: Japan has the longest LE and HLE in the world. The difference between LE and HLE and the rate of the difference were most strongly related to obesity. The rate of obesity in Japan is lowest in developed countries, and it would be the reason why the difference between LE and HLE was relatively short in Japan.
