Postoperative reduction of anti‐seizure medication is associated with cognitive improvement in seizure‐free patients after anterior temporal lobectomy

Abstract Objective To determine whether postoperative reduction of anti‐seizure medication (ASM) burden is associated with cognitive improvement after anterior temporal lobectomy (ATL) in seizure‐free patients. Methods We retrospectively analyzed data for adult patients with drug‐resistant temporal lobe epilepsy who underwent unilateral ATL between March 2013 and January 2025. Analyses were restricted to patients achieving complete freedom from seizures (Engel Class I) at 1 year. ASM burden was quantified using the prescribed daily dose/defined daily dose ratio, and the reduction rate was calculated. Preoperative and 1‐year postoperative neuropsychological outcomes, including full‐scale intelligence quotient (FSIQ) and domain‐specific indices, were assessed. The effect of ASM reduction on cognitive change was evaluated using a doubly robust estimation framework integrating inverse probability of treatment weighting with multivariable outcome regression. Robustness and potential dose–response relationships were assessed via sensitivity analyses, analysis of covariance (ANCOVA), and categorical modeling. Results Fifty‐seven seizure‐free patients were included in the primary analysis (median age at surgery, 32 years; 33 female). Greater ASM reduction was significantly associated with improved working memory (WM) ( β = .066, p = .001). A positive but non‐significant trend was observed for FSIQ ( β = .021, p = .062). A dose–response relationship was identified, with greater ASM reduction associated with progressively greater improvement in WM ( p = .013). Findings were consistent across sensitivity and ANCOVA analyses. Significance Postoperative ASM reduction was associated with domain‐specific cognitive improvement following ATL, particularly in WM. These findings support ASM tapering as a potentially modifiable factor influencing postoperative cognitive outcomes.

PURPOSE: Functional/dissociative seizures (FDS) are challenging to diagnose. Video-electroencephalography monitoring, the gold standard for the diagnosis, is impractical for all patients. To improve FDS diagnosis, a scale for suspicion of psychogenic non-epileptic seizures (SS-PNES) was developed in Brazil, consisting of 15 items, on a scale of 0-3 for each item, combining epileptological questions, current and past psychiatric characteristics. A total score of 20 or more is considered as FDS-like. Here, the reliability and validity of SS-PNES were evaluated in a Japanese cohort. METHODS: The subjects were 250 of 326 patients who received a comprehensive epilepsy evaluation at our institution. Factor analyses were applied to the SS-PNES data. RESULTS: A two-factor model with six items was found to be optimal for FDS diagnosis, demonstrating acceptable reliability coefficients and validity to identify FDS. The two factors were named "emotional stress" and "psychiatric characteristics." Most patients with FDS and comorbid epilepsy showed high scores for both factors. However, nearly 40 % of epileptic seizure-only patients were misdiagnosed as FDS-like. CONCLUSION: In our Japanese cohort, the original 15-item scale did not show reproducibility, so a shortened version with a two-factor structure was developed and evaluated. The new SS-PNES-short had high sensitivity but low specificity for identifying FDS. The addition of epilepsy-specific factors may improve the specificity of SS-PNES.

Adult‐onset epilepsy with startle‐induced seizure after febrile infection‐related epilepsy syndrome: A case report 査読有り

Abstract Startle‐induced seizure is a rare type of reflex seizure triggered by unexpected sensory stimuli that often occurs in children with early acquired cerebral lesions or brain malformations. We report a unique case of adult‐onset epilepsy with startle‐induced seizures. A 24‐year‐old woman had suffered high fever and focal to bilateral tonic–clonic seizures. A diagnosis of febrile infection‐related epilepsy syndrome (FIRES) was made based on the febrile infection occurring 7 days to 24 h before the onset of status epilepticus, which met all criteria for cryptogenic new‐onset refractory status epilepticus (NORSE) according to the cryptogenic NORSE score. Immunotherapy and several antiseizure medications resulted in transient resolution of the seizures. Four months later, she experienced startle‐induced seizures triggered by unexpected stimuli, such as auditory, visual, or unexpected events, and manifesting as initial tachycardia followed by right ear deafness, right hemifacial dysesthesia, eye deviation to the right, and tonic–clonic convulsions. Ictal electroencephalography revealed left temporal initial rhythmic delta activity, followed by rhythmic theta activity. The patient was diagnosed with startle epilepsy associated with FIRES and continued to receive anti‐seizure medications. Claustrum‐insular‐operculum lesions may have been the epileptic focus in this case, in contrast to previous cases of epilepsy with startle‐induced seizures originating in a frontoparietal network. This case indicates a new category of adult‐onset post‐FIRES epilepsy with startle‐induced seizures.

BACKGROUND: Identifying important types of social support for patients with epilepsy is valuable to construct an effective system for support in daily life. However, previous studies have been inconsistent in identifying the most important types of social support for better quality of life (QOL) due to the high correlations between the social support factors. The present study employed network visualization analysis to identify the relationships between QOL and types of social support. METHODS: Two hundred and eighty-three patients with epilepsy (age range: 18 to 75 years) completed questionnaires of the Medical Outcomes Study Social Support Survey (MOS-SSS) and the Quality of Life in Epilepsy Inventory-31-Problems in the epilepsy monitoring unit at Tohoku University. The MOS-SSS was established to measure the four types of social support including emotional/informational support, tangible support, affectionate support, and positive social interaction. Our network visualization analysis represented the entire structure of the interrelationships between the four functions of social support and QOL. In addition, the relative importance of each function of social support were estimated by the centrality indices using three commonly used centrality indices: strength, betweenness, and closeness. RESULTS: The visualized network showed that positive social interaction and emotional/informational support were directly associated with QOL, whereas tangible support and affectionate support were not. Positive social interaction had the highest value for all three centrality indices and affectionate support had the second highest. Therefore, positive social interaction had the strongest connection to the other functions of support. DISCUSSION: Our network analysis and centrality indices findings showed that positive social interaction is the most important within the network of four types of social support and QOL. The emotional informational function is also important for directly improving QOL but is less related to the other functions. The affectionate function might be an indicator of whether a patient has a foundational relationship that leads to other functions of support. CONCLUSION: These results showed the importance of increasing positive social interaction in the social environment of patients with epilepsy. Therefore, practitioners need to encourage patients with epilepsy to increase their positive social interactions such as doing something enjoyable with others or someone to associate for relaxation to ensure high QOL. Connections outside the epilepsy support are important, such as having fun regardless of epilepsy, rather than only providing emotional or tangible support for epilepsy.

BACKGROUND: Acceptance of disability (AOD) is a key concept in rehabilitation psychology that enhances psychosocial adjustment of individuals with disability. However, the impact of AOD on well-being has never been examined for patients with epilepsy. The present study investigated whether AOD affects quality of life (QOL) in patients with epilepsy in the presence of other multiple aspects of epilepsy based on the biopsychosocial model. METHODS: We retrospectively reviewed 151 consecutive patients with epilepsy (77 men, aged 18-74 years) who underwent comprehensive assessment including long-term video-EEG monitoring, neuroimaging studies, and neuropsychological and psychosocial assessment in our epilepsy monitoring unit. Data were obtained from medical records and self-reported questionnaires. The outcome variable was QOL. Predictive variables included demographic characteristics, seizure-related variables (i.e., duration of epilepsy, seizure frequency, and number of antiepileptic drugs), psychological factors (i.e., AOD, depression, and self-stigma), and social factors (i.e., social support and education level). Acceptance of disability was measured by the Adaptation of Disability Scale-Revised (ADS-R), which we translated into Japanese with the original author's approval, and examined its internal consistency reliability. Data were analyzed using four hierarchical multiple regression analysis models. RESULTS: The mean ADS-R score was 80 (range 45-115). The predictors accounted for 42% of the variance in QOL (R2 = 0.45, ΔR2 = 0.42, F[8, 141] = 14.47, p = 0.00). Higher AOD (p < 0.01), higher social support (p < 0.01), and lower depression scores (p = 0.02) were found to contribute significantly to higher overall QOL. CONCLUSION: The present study revealed AOD as an important psychological concept, in addition to social support and depression as previously reported, to improve the QOL of patients with epilepsy. Acceptance of disability should be incorporated in the intervention to increase QOL of patients with epilepsy.
