  1. Initial jaw-preserving management of large ameloblastomas: treatment trajectories and outcomes of repetitive dredging. 国際誌

    OBJECTIVE: To evaluate whether selected large ameloblastomas (maximum diameter ≥50 mm) could be initially managed with a jaw-preserving strategy and to describe treatment trajectories during repetitive dredging. METHODS: We retrospectively reviewed consecutive ameloblastoma patients treated at a single institution between August 2011 and September 2022. Patients with a maximum tumor diameter of ≥50 mm on preoperative CT who underwent initial jaw-preserving management (enucleation alone or repetitive dredging) were included. In the repetitive dredging cohort, the primary endpoint was 2-year event-free status (EFS2), with events defined as histopathologically confirmed recurrence or conversion to definitive resection. Secondary endpoints included dredging burden and achievement of pathological and confirmed clearance. RESULTS: Thirteen patients met the eligibility criteria, of whom four underwent repetitive dredging. EFS2 was 75% (3/4). One patient underwent hemimandibulectomy 12 months after the first dredging session because of persistent histopathological positivity. No recurrence occurred within 2 years. During longer follow-up, one patient developed histopathologically confirmed recurrence after confirmed clearance, approximately 3.5 years after the first dredging session. The number of dredging sessions ranged from 3 to 5, and confirmed clearance was achieved in three patients. CONCLUSIONS: In this small retrospective descriptive study, jaw-preserving management was observed in highly selected large ameloblastoma cases. Within this strategy, persistent histopathological positivity supported timely conversion to definitive resection, whereas recurrence could still occur after confirmed clearance. These findings should be interpreted as hypothesis-generating and support continued long-term surveillance.

    OBJECTIVE: To characterise oral lichen planus-associated oral microbiome alterations separately in saliva, supragingival dental plaque, and tongue coating. METHODS: In this cross-sectional study, samples from patients with oral lichen planus and healthy controls were analysed using bacterial amplicon sequencing. Microbial diversity, taxonomic composition, and genus-level differential abundance were compared within each sample type. RESULTS: Analyses included 47 control and 35 oral lichen planus saliva samples, 10 and 30 dental plaque samples, and 47 and 32 tongue coating samples, respectively. Significant alpha-diversity differences were observed for all four indices in saliva, three in tongue coating, and one in dental plaque. Beta-diversity differed between groups within all three sample types. Unweighted phylogenetic and presence-absence distances most consistently detected group differences, whereas abundance-weighted phylogenetic distances were not significant after correction. Unweighted phylogenetic differences remained significant after adjustment for Plaque Control Record. Descriptive taxonomic and differential-abundance patterns varied among sample types, with saliva yielding the largest number of significant taxa. CONCLUSION: Oral lichen planus was associated with microbiome alterations in all three sample types, mainly reflecting differences in microbial community membership. These sample types may provide complementary information for characterising oral lichen planus-associated dysbiosis.

    Radiation-related dental caries (RRC) is a rapidly progressing and treatment-resistant condition commonly observed after head and neck radiotherapy. It has traditionally been attributed to radiation-induced salivary gland dysfunction; however, RRC often develops even when appropriate oral care is provided, including fluoride application and regular hygiene practices. This clinical inconsistency suggests that salivary dysfunction alone cannot fully explain the onset and severity of RRC. The direct physicochemical effects of radiation on the organic and inorganic components of dental hard tissues remain poorly understood. In this study, extracted human third molars were subjected to a clinical IMRT protocol, using either a single high dose (20 Gy × 1) or a fractionated regimen (2 Gy/day × 35, total 70 Gy). We then evaluated radiation-induced changes in tooth structure by measuring Vickers microhardness, acid resistance (calcium elution), collagen degradation (autofluorescence), and internal dentin pH. Radiation exposure significantly altered enamel hardness: it increased after 20 Gy but decreased following 70 Gy. Calcium release increased in crown enamel (70 Gy) and root dentin (20 Gy), while decreasing in crown dentin (20 Gy). Autofluorescence imaging showed a significant reduction in dentinal collagen after fractionated exposure. Both groups exhibited an alkaline shift in dentin pH, more pronounced in the single-dose group. These findings provide physicochemical evidence for a non-salivary mechanism contributing to RRC. To effectively prevent RRC, additional strategies to reduce direct radiation damage to dental tissues may be necessary alongside conventional salivary-focused approaches.

  6. Differences in the Oral Microbiome Between Patients With and Without Oral Squamous Cell Carcinoma. 国際誌

    BACKGROUND: Although studies have demonstrated a relationship between pathogenic microorganisms and oral cancer, no study has demonstrated a relationship between changes in bacterial flora and oral squamous cell carcinoma (OSCC). Therefore, we investigated the association between oral microbiota and oral squamous cell carcinoma using metagenomic analysis. METHODS: Saliva samples from 64 patients with OSCC and 50 healthy controls who visited the Department of Oral Surgery, Tohoku University Hospital, were collected, and bacterial genomic DNA was extracted using polymerase chain reaction amplification. Single-end sequencing was performed using the Illumina MiSeq platform, and sequence data were analyzed using the Quantitative Insights Into Microbial Ecology 2 platform. The Steel-Dwass test was used for between-group comparisons, and Analysis of Compositions of Microbiomes with Bias Correction was used to detect significant differences in microbiome composition. RESULTS: Significant differences were observed in alpha-diversity indices of bacterial flora (richness, Faith- phylogenetic diversity, Shannon index) in the OSCC group compared to those in the control group. Among the OSCC group, patients with larger tumor diameters and lymph node metastases (T3/T4, N1 or greater) formed independent clusters in the beta diversity analysis of the bacterial flora. Bacteria of the Actinomycetia phylum, such as Actinomyces and Rothia, were significantly reduced in patients with higher stage and pathological grade. Conversely, bacteria of the phylum Spirochaetia and Proteobacteria, particularly those of the genus Treponema, were significantly elevated in advanced cancer cases. CONCLUSIONS: Our results suggest that changes in the oral microbiota may play a role in OSCC development and progression.

    The oral cavity contains several microbial niches, including saliva, dental plaque and tongue coating, each shaped by distinct local environments and host factors. This study compared the ecological and functional characteristics of the microbiomes of these three oral sites within the same individuals and examined host conditions associated with their variation. Saliva, supragingival plaque and tongue coating samples were collected simultaneously from 31 adults without clinical oral lesions. The bacterial 16S rRNA gene (V3-V4 region) was sequenced using the Illumina MiSeq platform, and analyses included α and β diversity, Mantel correlations, differential abundance tests, network analysis and functional prediction. The three sites displayed a clear ecological gradient. Saliva and tongue coating were taxonomically similar but were influenced by different host factors, whereas plaque maintained a distinct, biofilm-like structure with limited systemic influence. Functional divergence was most pronounced on the tongue coating despite its taxonomic similarity to saliva, whereas functional differences between saliva and plaque were modest despite larger taxonomic separation. These findings indicate that microbial composition and function vary independently across oral niches and support the need for multi-site sampling to more accurately characterize oral microbial ecology.

  8. Bonegraft Using Octacalcium Phosphate Collagen Composite with Teriparatide and Polylactic Acid Cage. 国際誌

    Octacalcium phosphate (OCP) is a bone grafting material known for its biocompatibility, osteoconductive, and osteogenic properties. Current treatments for extensive jaw defects often involve vascularized bone grafts or titanium mesh-based osteogenesis, which have limitations such as graft resorption, infections, and reoperation needs. In this study, a new bone regeneration therapy was explored, in which OCP combined with collagen (Col), treated with teriparatide (TPTD), was encased in a polylactic acid (PLA) cage to enhance structural stability and promote controlled bone formation. The therapeutic effects of this approach were evaluated using a rat model for calvarial regeneration, employing immunohistochemical staining. TPTD-treated OCP/Col composites were encased in cylindrical PLA cages, which were created using a 3D printer, and implanted into rat skulls. Three cage designs were tested: no holes, one large hole, and several small holes. Following implantation, the specimens underwent microcomputed tomography (micro-CT), histological, and immunohistochemical analyses to assess bone regeneration. In the micro-CT analysis, radiopacity at the OCP/Col graft site was higher in the "no hole" cage group than in the other groups from 4 to 12 weeks after implantation, particularly in the marginal area and region adjacent to the bone. Histological analysis revealed that, in all groups, new bone formation was observed along the surface of the skull 12 weeks postimplantation. In the "no hole" cage group, bone formation extended to the upper middle section, and bone matrix was present in areas where mature bone formation was lacking. In the other two groups, fibrous tissue filled the holes in the PLA cage, and no bone formation was observed directly beneath the holes. Immunohistochemical analysis revealed the expression of osteopontin, osteocalcin, runt-related transcription factor 2, vascular endothelial growth factor, and collagen I in all groups. The "no hole" cage group exhibited uniform and successful bone formation, with these cell markers consistently observed throughout all regions. These results suggest that using PLA cages to cover TPTD-treated OCP/Col discs effectively promotes bone regeneration. This approach provides a promising alternative to conventional bone grafting techniques and may help overcome the limitations associated with free or autologous bone grafts in oral and maxillofacial reconstruction. Impact Statement This study demonstrates that encasing teriparatide-treated octacalcium phosphate/collagen composites in polylactic acid (PLA) cages enhances bone regeneration. Using a rat model, microcomputed tomography, histological, and immunohistochemical analyses revealed that the "no hole" PLA cage design promoted uniform and successful bone formation, whereas perforated cages led to fibrous tissue infiltration. These findings highlight the potential of PLA cages in optimizing bone grafting strategies, offering a promising approach for tailored bone regeneration therapies.

    OBJECTIVE: To clarify the relationship between oral squamous cell carcinoma (OSCC), potentially malignant oral disease (OPMD), and bacterial flora using metagenomic analysis. METHODS: This cross-sectional observational study included 50 patients in the control group and 77 patients with OPMDs, 41 with early OSCCs, and 20 with advanced OSCCs. Patient saliva samples were subjected to high-throughput sequencing of 16S rRNA gene amplicons to evaluate the composition and diversity of the oral microbiome. RESULTS: No significant differences were observed in patient backgrounds, other than sex. Patients with advanced OSCCs had greater oral bacterial diversity than those with early OSCC or OPMD. The advanced OSCC group formed a distinct cluster separate from the other groups. Sixteen and 275 species were identified at the phylum and genus levels, respectively. Compared with the control group, Actinomycetia and Streptococcus were significantly elevated in the early OSCC and OPMD groups. Peptostreptococcus and Fusobacterium were significantly higher in the advanced OSCC group than in the control, OPMD, and early OSCC groups. CONCLUSIONS: The composition and diversity of oral microbiota may be associated with OPMD development and progression to OSCC. Consequently, the salivary microbiome may serve as a biomarker for oral cancer and help predict cancer progression.

    BACKGROUND: The development of synchronous multiple primary cancers is one of the major causes of death in patients with head and neck cancer. Herein, we report a case of synchronous intraductal papillary mucinous carcinoma (IPMC), invasive in a patient with maxillary gingival carcinoma. CASE PRESENTATION: A 73-year-old female visited our hospital complaining of a mass on the left side of the maxillary gingiva. Intraorally, an exophytic tumor, 50 × 25 mm in size, was found on the gingiva of the left maxillary posterior, and a diagnosis of squamous cell carcinoma was revealed by cytology. Emission tomography/ computed tomography with 18 Fluorodeoxyglucose-Positron (18FDG- PET/ CT) showed increased accumulation in the left maxillary gingiva, the left side of cervical lymph nodes, and the main pancreatic duct. The pancreatic ductal tumor was performed the biopsy at esophagogastroduodenoscopy (EGD) and resulted in a pathological diagnosis of IPMC, invasive. The patient was diagnosed as synchronous double primary cancers consisting of maxillary gingival carcinoma cT4aN2bM0 and IPMC, invasive cT3N0M0. She refused radical treatment, and died 11 months later. CONCLUSION: 18FDG- PET/ CT, EGD and multidisciplinary approach is required for the detection and determining the treatment strategy of synchronous double primary cancers.

    症例は74歳女性で、13年前より高血圧症、糖尿病を発症し、2年前より骨粗鬆症、慢性関節リウマチを発症し薬物治療を受けていた。下顎右側犬歯および側切歯を抜歯し、翌月より同部に骨露出と排膿を認め洗浄を継続したが改善しないため紹介受診となった。体格は中等度、栄養状態は良好であった。下顎右側犬歯、側切歯の抜歯窩相当部に腐骨の露出を認めた。腐骨露出部周囲の歯肉は発赤を伴うび漫性腫脹をきたし、同部から排膿を認めた。パノラマX線で下顎右側犬歯および側切歯部の抜歯窩を含めた骨体部にX線透過性亢進を認めた。慢性下顎骨骨髄炎、BRONJ疑いと診断し、アレンドロン酸ナトリウムを休薬し、ラロキシフェン塩酸塩、カルシトリオールを開始した。同時期より抗菌薬(セフカペンピボキシル(CFPN-PI))内服を開始し、保存的に経過観察を行ったが骨露出が拡大したため、CTを撮影し、腐骨の分離を認めた。入院し、全身麻酔下に右下顎骨腐骨除去術を施行した。不良肉芽を掻爬し、除去の上、創部を縫合し、閉鎖相と診断した。抗菌薬アモキシシリン(AMPC)内服を開始し、生理食塩水による洗浄で保存的に経過観察した。術創から排膿は消失し創部は肉芽化したが上顎左側前歯部にび漫性の腫脹・発赤、排膿を認めた。上顎左側中切歯、側切歯、犬歯に動揺をきたし、同部から持続的な排膿、一部に骨露出を認めた。パノラマX線で上顎左側前歯部に境界明瞭な骨溶解像を認め上顎前歯3本の抜歯とともに18×10mm大の腐骨を摘出した。

    69歳男。2ヵ月前に口底部の腫脹を自覚した。その後、腫脹の増大とともに会話や摂食が困難となった。前口底部に腫瘤を認め、触診では弾性硬であった。舌運動は制限され、会話はやや不明瞭であった。造影CTでは、前口底部に右側を主座に正中を超える43×41×35mmの腫瘤性病変が認められた。下顎舌側の皮質骨に吸収像を認め、下顎骨浸潤を疑った。両側の頸部には転移と思われる腫大したリンパ節が認められた。造影MRIでも同様な腫瘤性病変を認め、オトガイ舌筋、オトガイ舌骨筋等への浸潤と思われる像が確認された。口底部の腫瘍は病理組織学的検査と画像所見から、口底扁平上皮癌と診断された。上部消化管CTでは、胃幽門前庭部に10×5cm大の内部不均一な腫瘤が認められた。肝臓には多発性の嚢胞様病変を認め、肝嚢胞あるいは肝臓への転移が疑われた。胃切除術を施行し、病理組織学的にGISTと診断された。その後、呼吸不全のため死亡した。

  56. 4歳児の下顎骨に生じた巨細胞腫の1例

    症例は4歳4ヵ月男児で、右側下顎第二乳臼歯が自然脱落し、近歯科にてパノラマX線で右側下顎骨にX線透過像を認め紹介受診した。口腔内で右側下顎乳臼歯部にび漫性の無痛性腫脹を認めた。パノラマX線で右側下顎骨内に境界明瞭な類円形のX線透過像を認め、CTでは右側下顎骨体部に17×12×10mmの境界明瞭な透過像、下顎骨の膨隆と周囲皮質骨の菲薄化を認めた。右側下顎腫瘍と診断し、全身麻酔下に病変摘出術を行った。右側下顎第一乳臼歯抜歯後に骨面から病変を剥離し、病変に接していた第二小乳臼歯胚と共に摘出した。摘出腔と第一乳臼歯抜歯窩との交通を認めた。病理所見では、充実性に増殖する短紡錘形細胞中に均一に分布する多数の破骨型多核巨細胞の混在を認め、酒石酸抵抗性酸性ホスファターゼは多核巨細胞にのみ強陽性反応を示し、巨細胞腫と診断した。術後1年6ヵ月経過で再発は認めず、骨欠損部の骨再生、腫瘍周囲の永久歯胚成長を認めた。

  57. 上顎に発生した類腱型エナメル上皮腫の1例

    症例は66歳男性で、右側頬部の腫脹に気づいたが自覚症状がないため放置した。半年前から右側上顎前歯の動揺を自覚し受診した。顔貌は左右非対称、右側頬部に瀰漫性腫脹を認め、右側鼻翼部は挙上されていた。同部に発赤、疼痛、知覚麻痺はなく、触診で骨様硬で圧痛を認めた。所属リンパ節腫脹は触知しなかった。右側上顎中切歯から第二小臼歯の頬側歯肉に膨隆を認めた。触診では骨様硬で圧痛を認めなかった。右側上顎中切歯から第一小臼歯の動揺を認め、側切歯は唇側へ、犬歯は口蓋側への転位がみられた。パノラマX線で右側上顎中切歯から第一大臼歯根尖部に境界不明瞭なX線透過像がみられた。右側上顎側切歯は近心へ、犬歯は遠心へ傾斜し、側切歯と犬歯間は歯根離開を認めた。CTで右側上顎部に45×33×35mmの外方性に膨隆した充実性腫瘤を認めたが、頬側皮膚骨の連続性は保たれていた。病変は造影効果を示し、右側上顎中切歯から第一大臼歯根尖が病変内に突出し、内部に一部石灰化物を含有していた。上顎骨良性腫瘍と臨床診断し、局所麻酔下に生検を行った。病理組織学的所見は線維性結合組織と歯原性が考えられる上皮集塊が散在性にみられ、上皮に角化がみられた。病理組織診断は歯原性腫瘍の疑いで、全身麻酔下に腫瘍摘出掻爬術を行った。術後経過は良好で、顔貌の変形も改善され、第8病日に退院した。

  60. 口腔外科手術における吸収性ポリグリコール酸シートとフィブリン糊スプレー被覆法による創部の治癒に関する臨床統計的検討

    口腔外科手術後にポリグリコール酸(PGA)シートとフィブリン糊スプレーによる創部被覆法を行った20例(男性12例、女性8例、平均67.2歳)を対象に、本法の予後因子について検討した。本法施行部位は上顎歯肉4部位、下顎歯肉6部位、口蓋3部位、舌5部位、頬粘膜2部位であった。術後の経口摂取は術翌日から可能が11例であった。A群(上・下顎歯肉、口蓋の13部位)とB群(舌、頬粘膜の7部位)に分けた比較では、両群とも術後の非経口摂取日数とPGAシートの付着日数との間、切除面積とPGAシート付着日数との間に有意な相関は認めず、A群に比べB群で有意にPGAシート付着日数が短かった。PGAシートは上皮化に伴い辺縁から剥離することが多く、剥離部を鋏刀で除去した。A群では全例でraw surfaceに骨面が認められたが、骨面にも良好な付着を認め、PGAシート脱落時に全例が「治癒」と判定され、「非治癒」の判定は全てB群であり、早期脱落例であった。

  62. 急性骨髄性白血病に伴い歯肉と皮膚に生じた顆粒球肉腫の1例

    32歳男。1ヵ月前より経口摂取不良となり、体重減少、全身倦怠感を認め呼吸器内科を受診したところ、白血球数20650/μL、CRP5.30mg/dL、頸部リンパ節腫脹と開口障害を認めたため紹介となった。口腔外には両側頸部に多発性、有痛性のリンパ節腫脹と開口障害、右側頸部と右側下腿内側部に結節状の皮疹を認め、口腔内には左右上顎臼歯部の歯間乳頭部を中心に有痛性の歯肉腫脹を認めた。造影CTでは両側頸部にリンパ節腫脹を認め、MRIでは腫脹リンパ節の内部がほぼ均一であることを認めた。入院後、抗菌薬投与を開始したものの第3病日にも血液検査で異常値を認めたため、血液・腫瘍内科を受診したところ骨髄検査で急性骨髄性白血病と診断された。また、染色体検査で46、XY、t(11;17)、(q23;q25)の異常が100%の細胞で検出され、同科へ転科し寛解導入療法を開始した。完全寛解が得られた後地固め療法、さらに同胞間での骨髄移植を施行し、治療後1年以上で再燃の兆候はなく寛解を維持している。治療後にGSと頸部リンパ節が消失したため、歯肉と皮膚病変はGSの原発あるいは歯肉からの転移巣と考えられた。

    症例は33歳男性で、左頬部に軽度の鈍痛を伴う腫脹を自覚した。左上7番の動揺を自覚したため受診し、左上3-7番の羊皮紙様感を伴う骨様硬の膨隆を指摘された。パノラマX線写真で、左上顎洞前壁近傍に石灰化物を示唆する不透過像を含む単房性のX線透過像と、それに連続して左上顎洞上壁近傍に埋状歯を認めた。左上5-7番根尖の吸収像を認めた。CT画像では、左上顎骨の膨隆を認め、皮質骨は菲薄化していたが著明な骨破壊は認めなかった。初診時の口腔内所見とパノラマX線写真所見、およびCT画像所見より歯原性良性腫瘍を疑い生検を行ったが、歯原性良性腫瘍の診断には至らなかった。画像所見と生検時の所見から、病変は単嚢胞型エナメル上皮腫や石灰化嚢胞性歯原性腫瘍などの嚢胞状の腫瘍性病変と判断し、開窓による病変の縮小を待ったうえで、二次治療として摘出・掻爬術を行うことを治療方針とした。術翌週に保護床を目的とした部分床義歯を作製し装着した。術後4年6ヵ月が経過した現在、臨床症状は認めていない。病理組織学的に歯牙エナメル上皮腫と診断した。

    自己フィブリン糊は，同種フィブリン糊と比較し凝固に時間がかかるが，より強い接着が得られることが報告されている．また種々の感染症・アレルギーのリスクを回避でき，安価であり，使用領域が拡大している． 当大学輸血細胞治療部の協力のもと，自己血フィブリン糊の生成に関してはクリオシールシステム®を使用した．貯血した血漿からクリオプレシピテートとトロンビン液を滅菌閉鎖回路内で自動的に調製する．抽出が難しいとされるトロンビン調整に関しては，陰性荷電刺激ビーズを用い抽出する．完全自己血フィブリン糊生成および使用に関しては，先行研究にて確立した 臨床試用では期待されていた安全性，簡便性などのほか，疼痛や瘢痕形成軽減および創部治癒促進などの効果も認められ，自己血フィブリン糊の優れた上皮形成能を示唆する成果であった． また自己血フィブリン糊の上皮形成能について上皮欠損マウスモデルを用いて評価した．頭蓋骨露出・上皮欠損ラットを用いた．上皮欠損部で骨露出面に吸収性ポリグリコール酸シートおよびクリオシールシステムにて作成した自己血フィブリン糊にて被覆した．上皮形成能をHE染色および 1)と同様の免疫染色にて評価した．免疫組織学的には、Laminin,VEGF,Cytokeratinにて上皮形成および血管新生が確認できた． またMacro評価では上皮の迅速な再生が認められ、自己血フィブリン糊による創部処理は，上皮形成能に優れ，創傷治癒効果を期待できる結果となった．
