Gender Differences in the Impact of Autism Spectrum Traits and Camouflaging on Mental Health and Work Functioning: A Structural Equation Modeling Approach 査読有り

In white-collar workplaces, individuals with autism spectrum disorder (ASD) traits may experience psychological strain and reduced productivity. This study examined structural relationships among ASD traits, social camouflaging, psychological distress, and work functioning impairment, with a focus on gender differences using a secondary analysis of data from an online survey of 543 Japanese white-collar workers (284 men, 259 women). Validated instruments were used to assess ASD traits, camouflaging, psychological distress, and work functioning impairment. Multi-group structural equation modeling by gender was applied using a NIOSH-inspired model. Men scored higher on the Imagination subscale of ASD traits, whereas women scored higher on Attention Switching and Assimilation. ASD traits were indirectly associated with work impairment through psychological distress, while the direct path between ASD traits and work impairment became negative when distress was controlled, indicating a statistical suppression pattern that was more pronounced among women. Assimilation was significantly associated with psychological distress in women but not in men, although the gender difference was at the trend level. The findings indicate a cross-sectional, context-dependent association between ASD traits and work functioning and highlight the importance of considering both gender and workplace context in non-clinical working populations.

Elucidating nurses’ work values will contribute to creating a work environment consistent with individual nurses’ work values and help them overcome challenging situations. This study aimed to create a short version of the Nurses’ Work Values Scale and verify its psychometric properties and usability among nurses, public health nurses, midwives, and licensed practical nurses. The short version was developed in three steps. In Step 1, a 12‐item scale was created from the original 30‐item Nurses’ Work Values Scale via a multidimensional item response theory based on data obtained from 1587 nurses. In Step 2, we modified the wording of the items and verified the scale’s content validity. In Step 3, we obtained new data from 1220 nurses, public health nurses, midwives, and licensed practical nurses who worked in various settings. Subsequently, we verified the psychometric properties of the 12‐item scale. The developed scale made it possible to measure work values among nurses working in hospitals, nursing care facilities, visiting nursing settings, and public health nurses. Simultaneously, the burden of responding to surveys was reduced. In the future, it is hoped that this scale will be used on a large longitudinal cohort of nurses working in various workplaces to track life events and changes in family background, thereby clarifying changes in work values and the factors that influence them. In addition, by knowing nurses’ values, nursing managers can take measures such as adjusting work to match each nurse’s work values. This could help promote alignment between the values of individuals and organizations and reduce turnover rates.

OBJECTIVES: To clarify the conceptual structure of "cultural competence (CC)" among Japanese public health nurses (PHNs), to enhance culturally appropriate support. METHODS: A modified grounded theory approach (M-GTA) was used. A total of 11 municipal PHNs participated in this study. Data were collected through semistructured interviews. Interviews were audiotaped and transcribed. A comparative analysis was performed using M-GTA. RESULT: Five categories were identified. Japanese PHNs supported foreign residents while (1) maintaining a sincere attitude with curiosity and humility toward others regarding CC; (2) deepening their realization of issues arising from awareness of one's own and other cultures; (3) developing their knowledge about clients' cultures and the surrounding environments; and (4) mastering the skills of building a relationship with clients while creating supportive systems surrounding them. As they gained more experience in supporting foreign residents, as indicated in the above categories, their cultural competence grew, allowing them to (5) gain experience while encountering individuals without holding stereotypes and prejudice. CONCLUSIONS: The concepts of CC that emerged are based on cultural humility and are cultivated through supportive activities. The concepts identified in this study can serve as educational guidelines for health nurses and other care providers in Japan.

本研究の目的はポジティブ・デビエンス(以下PD)・アプローチを用いて、1)住民の健康増進に寄与した、Community Health Worker（以下CHW；のグループ・仕組み）の好事例を抽出し、2) CHW（のグループ・仕組み）の活動活性化要素を抽出することである。第１段階として、CHW制度に関する資料・データから、PDと位置付ける項目を定める。そのためにまず、アラスカ州のCHW２名にインタビュー（オンライン）を実施し、これまでの各人のCHWとしての歩みや地域保健活動において印象に残る好事例・課題等を聞き出した。更にPDと位置付ける項目を決定する前段階として、米国先住民の文化的背景を取り入れた治療や予防的な取り組み・効果についての文献検討を行い、歴史的・文化的背景が相互的に影響し、依存症や慢性疾患等の高い罹患率となっている米国先住民社会全体の現状や健康課題についての理解を深めた。方法は、Web of scienceを用いて、cultur* とtreatment 、prevention、substance、alcohol、suicideのキーワードを含む英文文献を検索し、文献中の文化的取り組みについての意味内容を抽出し、類型化した。更に、文化的取り組みの効果についての記載部分を全て抽出しコード化・カテゴリ化したのち、テーマ分析を行った。結果、米国の先住民の伝統や習慣から発した15種と、西洋医学と融合した7種の取り組みに分類された。取り組みの効果については<個人のエンパワメントを促進する>、<人とのつながりを取り戻す>、<先人や大地とのつながりを得る>の３テーマを抽出した。本研究により、文化的取り組みは米国の先住民の伝統や習慣の要素だけでなく、西洋医学的アプローチと重なる治療方法が多く組み込まれ、先住民がより受容しやすい治療・予防となっている可能性が考えられた。

【課題意識】地域社会の人間関係の希薄化・縮小化により育児や介護、災害や転居、独居や死別など、いつでもどこでも誰にでも社会的排除のリスクが存在する。コロナ禍を経て社会的排除のリスクがさらに高まり、社会的包摂の推進は国策の喫緊の課題である。住民のよりよく生きる権利を守るために社会の包容力を志向することは保健師の責務である。【研究目的】現代社会における弱者を包容する地域づくりに取り組む保健師の実践知から社会的包摂を志向する公衆衛生看護の概念構造（原理）を解明し、これを公衆衛生看護の原理と位置づけることができるロジックモデルを開発する。このモデルを用い、社会的包摂を志向する公衆衛生看護を実践現場に根付かせる実装を試みる。【研究実績】これまで国内外の社会的包摂に関する文献検討、および Social Inclusionの概念分析を行い、社会的包摂の概念の構成要素を抽出し、ロジックモデル原案を作成した。次いで社会的包摂を志向した公衆衛生看護の活動事例について中堅期保健師を対象にヒアリングを行い、仮説検証型multiple case studyアプローチを用いてモデル原案に追試を重ね、日本の公衆衛生看護の実践に適用可能なロジックモデルに改良した。さらに管理期保健師を対象にグループインタビューを行い、ロジックモデルを洗練した。次いで、モデルの信頼性と妥当性を担保すると同時に実践現場への適応可能性を高めるため、関連学会にてワークショップを行い、意見聴取した。聴取した意見に基づきモデルを改良し、実践現場への適応を図るために必要な教材（モデルの解説、事例集、実践の振り返りシート、社会的包摂志向性の変化の評価指標・項目など）を開発した。
