BACKGROUND: Accurate estimation of the retinal age, defined as the age predicted from fundus photographs by a deep-learning model trained on chronological age, provides a non-invasive biomarker of biological ageing and disease risk. METHODS: In this study, we trained an ensemble multitask learning model that integrates fundus photographs with glycated haemoglobin using 50,595 quality-controlled fundus photographs from 27,214 disease-free adults and validated it on an independent set of 7288 additional images from disease-free adults. Model performance was evaluated using mean absolute error. Prediction uncertainty was quantified by calculating the standard deviation across ensemble predictions for each eye, and eyes were stratified based on this standard deviation. RESULTS: Here we show that the model achieves mean absolute errors of 2.78 years in internal validation and 3.39 years and 8.63 years in two external cohorts comprising 135 and 4992 eyes, respectively. Eyes with ensemble standard deviations below the median demonstrate improved age-prediction accuracy (mean absolute error: 2.46 years internally; 2.87 years in the primary external cohort). In a systemic disease cohort of 8467 individuals, the retinal age gap (predicted minus chronological age) is significantly higher in participants with diabetes, cardiac disease, or stroke after adjustment for age and sex, indicating older-appearing retinas and supporting the biological relevance of retinal age. CONCLUSIONS: Retinal age derived from a single fundus photograph provides a scalable and clinically deployed biomarker of biological ageing. This approach may enable opportunistic screening for cardiometabolic and other ageing-related diseases in other routine ocular imaging workflows.

Objectives: Glaucoma is an age-related neurodegenerative disease, characterized by retinal ganglion cell loss and progressive visual field deterioration. Beyond intraocular pressure (IOP), vascular and metabolic dysregulation contributes to optic nerve head (ONH) ischemia and neuronal vulnerability. Nutritional factors with antioxidative and vasodilatory properties may help preserve ocular perfusion. This study investigated the acute and subacute effects of a single dose of a dietary supplement containing red ginger extract (Zingiber officinale var. rubra), lutein, and vitamin B6 on ONH blood flow in patients with open-angle glaucoma (OAG). Methods: A retrospective self-controlled study was conducted at Tohoku University Hospital between August 2023 and March 2025. ONH blood flow was quantified using a laser speckle flowgraphy (LSFG) baseline one hour after and one month after continuous oral supplementation in patients with OAG. Systemic parameters, ocular biometry, and concomitant glaucoma medications were recorded in medical charts. Relative mean blur rate (MBR) changes were analyzed using a linear mixed-effects model, accounting for repeated measures and inter-eye correlations. Results: Nineteen glaucoma patients (38 eyes) were included in the acute phase and 13 patients (26 eyes) completed the one-month follow-up. After adjusting for age and sex, a single oral dose of red ginger extract significantly increased the relative MBR at 1 h (106.9 ± 3.1%; p < 0.05), and this enhancement increased after 1 month of continuous intake (115.4 ± 6.7%; p < 0.05). Greater ONH perfusion was particularly prominent in eyes with shorter axial length. Conclusions: Oral supplementation was associated with acute and short-term increases in ONH blood flow in glaucomatous eyes. Although this study was a retrospective study without a placebo-controlled comparison group, our findings offer hypothesis-generating evidence that nutritional interventions may support ocular perfusion alongside conventional glaucoma management. Future prospective randomized controlled trials are required to confirm these associations.

Glaucoma is a major ocular neurodegenerative disease and a leading cause of irreversible blindness worldwide, with prevalence projected to exceed 110 million by 2040. Although lowering intraocular pressure (IOP) remains the only proven treatment, glaucoma arises from a complex interplay of genetic, local, and systemic factors-including oxidative stress, vascular dysregulation, mitochondrial dysfunction, and neuroinflammation. Emerging evidence suggests that modifiable lifestyle factors may influence these pathogenic pathways. In this review, higher dietary nitrate from leafy greens is consistently associated with lower primary open-angle glaucoma risk, aligning with nitric-oxide-mediated endothelial support and more stable ocular perfusion pressure. Flavonoids (anthocyanins and flavanols), carotenoids (lutein/zeaxanthin), and B vitamins have strong biological rationale for glaucoma prevention but have limited support from long-term, large population-based studies. The effect of polyunsaturated fats on glaucoma remains inconsistent and warrants source-(plant vs. animal) and substitution-based analyses. Consistent protective effects of aerobic exercise and high-quality sleep may be associated with favorable metabolic profiles and ocular perfusion, potentially mitigating retinal ganglion cell loss. Conversely, smoking and alcohol use are frequently coupled with poorer diet quality (e.g., lower vegetable intake) and heightened oxidative stress, which may exacerbate glaucomatous neurodegeneration. However, much of the current literature is constrained by cross-sectional designs, reliance on self-reported food frequency questionnaires, and insufficient use of structural endpoints such as retinal nerve fiber layer imaging. This review focuses on the potential of lifestyle modification and future directions in prevention and treatment strategies for glaucoma, highlighting the need for large-scale, multi-ethnic, genotype-stratified longitudinal studies and randomized controlled trials to establish causality and define optimal intervention strategies.

PURPOSE: We explored the role of oxytocin in glaucoma by measuring the blood levels of oxytocin in glaucoma patients, comparing them to normal control subjects, and examining its association with clinical parameters. STUDY DESIGN: Retrospective cross-sectional study. MATERIAL AND METHODS: After obtaining informed consent from 181 glaucoma patients and 44 age-matched control participants, we collected blood samples in ethylenediaminetetraacetic acid (EDTA) tubes and centrifuged them at 2000 g for 25 minutes at 4 °C. The resulting plasma was assayed for oxytocin concentration with an Enzyme Linked Immunosorbent Assay (ELISA) kit. We compared oxytocin concentrations in the control and glaucoma groups, and within the glaucoma group, we determined whether the oxytocin levels were correlated with mean deviation (MD) and sectoral total deviation (TD). Furthermore, in 33 patients who underwent at least five visual field tests over a two-year period following oxytocin measurements and received no surgical interventions during that time, we determined whether the oxytocin levels were correlated with MD slope and sectoral TD slopes. RESULTS: Oxytocin levels in glaucoma patients were significantly lower than in age- and sex-matched normal controls (723.34 ± 303.44 vs. 557.59 ± 296.04 pg/ml, p=0.002). In glaucoma patients, oxytocin levels were significantly correlated with MD and inferior TD after adjustment for age and sex (β=0.149, p=0.041; β=0.156, p=0.034, respectively). There was a weak negative correlation between oxytocin concentration and MD slope (β=-0.334, p=0.084) and a weak negative correlation with central TD slope (β=-0.405, p=0.039), adjusted for age, sex, and history of additional eye drops. CONCLUSION: Oxytocin concentrations in glaucoma patients were significantly lower than in normal subjects and associated with the severity and progression of visual field defects. Given the wide variety of the pharmacological actions of oxytocin, it may be involved in the pathogenesis of glaucoma. Our results suggest that plasma oxytocin measurements may open a new avenue for glaucoma care.

PURPOSE: The effects of enolase dysfunction-mediated glycolytic attenuation on oxidative stress-induced retinal ganglion cell (RGC) death were investigated. METHODS: Retinal expression of Enolase (ENO) 1 and 2 was detected using immunohistochemistry. Exogenous genes were introduced into mouse RGCs using viral vectors, and into the rat-derived retinal progenitor cell line R28 using lipofection. The effects of enolase dysfunction were evaluated using N-methyl-D-aspartate (NMDA)-induced RGC death and H2O2-induced death assays. Cell viability and gene expression were investigated using the alamarBlue, quantitative RT-PCR, and Western blotting, respectively. Reactive oxygen species (ROS) were detected using CM-H2DCFDA dye. 2-deoxyglucose and oligomycin were used to attenuate adenosine triphosphate (ATP) production in glycolysis and oxidative phosphorylation (OXPHOS), respectively. RESULTS: ENO1 and ENO2 were expressed in the RGCs. Enolase overexpression inhibited NMDA-induced mouse RGC death, whereas deficiency of ENO1 but not ENO2 enhanced cell death. Additionally, ENO1 overexpression prevented the CDKN2B-induced enhancement of RGC death. H2O2 treatment increased ENO1 expression in R28 cells and inhibited H2O2-induced cell death. ATP production was immediately enhanced in ENO1 wild-type cells treated with H2O2. 2-deoxyglucose and oligomycin prevented enhanced ATP production, but ATP levels were still higher than, or similar to, those in untreated wild-type cells; in contrast, in knockdown cells, oligomycin-mediated OXPHOS inhibition led to lower ATP production in H2O2-treatment than in untreated cells. H2O2 treatment increased ROS production in knockdown R28 cells. CONCLUSIONS: ENO1 dysfunction leads to glycolytic attenuation, resulting in an excessive dependence of ATP production on OXPHOS under oxidative stress, contributing to excitotoxicity-induced RGC death. Preventing glycolytic attenuation may represent a promising treatment for RGC degeneration.

PURPOSE: We tested the hypothesis that a questionnaire-based risk score predicts the prevalence of patients with primary open-angle glaucoma (POAG) with vascular predisposition. METHODS: The Flammer Syndrome Questionnaire (FSQ) was used to determine vascular risk scores in 823 healthy subjects and 512 patients with POAG. Next, we characterized blood flow pulsatility changes within the optic nerve head (ONH) in Flammer syndrome (FS) using laser speckle flowgraphy (LSFG) in 358 eyes of 206 patients with normal-tension glaucoma (NTG). Last, we examined the association between changes in Mean blur rate (MBRAve), an LSFG-derived ONH blood flow measurement, during cold provocation and the FSQ risk score in 56 eyes of 56 patients with NTG. RESULTS: Five FSQ-related symptoms were significantly associated in patients with POAG patients; cold hands/feet (odds ratio [OR] = 1.82), low blood pressure (BP; OR = 3.29), increased response to drugs (OR = 2.27), underweight (OR = 1.99), and tendency toward perfectionism (OR = 1.88). The vascular risk score showed the best discriminative accuracy in differentiating healthy subjects from patients with NTG (area under the curve [AUC] = 0.73). In the NTG eyes, ONH pulsatile blood flow in the FS group was characterized by greater pulsatility. Moreover, the negative correlation between the high FSQ risk score and the cold-induced ONH blood flow reduction was pronounced in eyes with NTG (correlation coefficient = -0.41). CONCLUSIONS: The FSQ risk score can be a screening tool to identify patients with POAG with increased vascular stiffness and further reduced ONH blood flow during cold stress. TRANSLATIONAL RELEVANCE: The vascular risk score may help tailor individual glaucoma care.

Glaucoma, a leading cause of blindness, requires accurate early detection. We present an AI-based Glaucoma Screening (AI-GS) network comprising six lightweight deep learning models (total size: 110 MB) that analyze fundus images to identify early structural signs such as optic disc cupping, hemorrhages, and nerve fiber layer defects. The segmentation of the optic cup and disc closely matches that of expert ophthalmologists. AI-GS achieved a sensitivity of 0.9352 (95% CI 0.9277-0.9435) at 95% specificity. In real-world testing, sensitivity dropped to 0.5652 (95% CI 0.5218-0.6058) at ~0.9376 specificity (95% CI 0.9174-0.9562) for the standalone binary glaucoma classification model, whereas the full AI-GS network maintained higher sensitivity (0.8053, 95% CI 0.7704-0.8382) with good specificity (0.9112, 95% CI 0.8887-0.9356). The sub-models in AI-GS, with enhanced capabilities in detecting early glaucoma-related structural changes, drive these improvements. With low computational demands and tunable detection parameters, AI-GS promises widespread glaucoma screening, portable device integration, and improved understanding of disease progression.

Corneal hysteresis (CH) is associated with glaucomatous structural changes. We retrospectively investigated the association between CH and the regional circumpapillary retinal nerve fibre layer thickness (cpRNFLT) in 419 eyes of 419 patients with normal-tension glaucoma (NTG) and primary open-angle glaucoma (POAG). CH was used as the explanatory variable, and cpRNFLT (total and quadrant) was used as the dependent variable. Standardized β coefficients were compared both overall and between the NTG and POAG groups. Multiple regression analysis with CH as the explanatory variable and cpRNFLT parameters as the dependent variables suggested that even after adjusting for age, sex, intraocular pressure (IOP), axial length, and central corneal thickness, all areas of the cpRNFLT were significantly associated with CH, except for the nasal quadrant. In the stratified analysis by glaucoma type, a positive relationship between CH and regional cpRNFLT tended to be maintained in the NTG group but not in the POAG group (P = 0.060 for interaction). Additionally, in the NTG group, the CH-inferior cpRNFLT relationship was particularly strong in the less-IOP fluctuation group. These results suggest that CH may be a potential predictor of anatomical vulnerability around the optic nerve, particularly enhancing the inferior regions of NTG.

PURPOSE: This study aimed to investigate differences in microvasculature dropout (MvD) between the superior and inferior hemispheres in glaucoma patients. STUDY DESIGN: Retrospective and cross-sectional. METHODS: Fifty-eight eyes of 58 open-angle glaucoma patients (age 61.12 ± 10.19 years, mean deviation - 7.32 ± 6.36 dB) were included. MvD was detected with en face images from swept-source optical coherence tomography angiography. Blood flow at the optic nerve head was measured with laser speckle flowgraphy, represented as the mean blur rate in tissue (MBRT). Logistic and linear regression models adjusted for age, intraocular pressure, axial length, and circumpapillary retinal nerve fiber layer thickness were used to investigate the relationship between various factors and MvD angle in each hemisphere. RESULTS: The presence of inferior MvD was related to peripapillary atrophy-β area (odds ratio = 14.10 [2.49-234.00], P = 0.019). Superior MvD angle was significantly related to MBRT in the superior quadrant (β = -0.31 [- 0.60 - -0.02], P = 0.037). Inferior MvD angle was significantly related to peripapillary atrophy-β area (β = 0.49 [0.21-0.77], P = 0.001). CONCLUSIONS: Only superior MvD demonstrated a significant relationship with reduced ocular blood flow. In contrast, inferior MvD was associated with mechanical stress. These findings may suggest a potential difference in pathophysiology between superior and inferior MvD.

PURPOSE: To investigate factors associated with intrachoroidal cavitation (ICC) and sinkhole formation in eyes with glaucomatous visual-field defects. METHODS: This retrospective, cross-sectional study enrolled a total of 2808 eyes of 1482 patients who were diagnosed/treated for glaucoma and underwent swept-source optical coherence tomography (OCT). We first determined the prevalence of ICCs and sinkholes and their locations. Next, we selected one eye from each patient and compared the clinical characteristics of eyes with and without ICCs. Finally, in eyes with ICCs, we compared the clinical characteristics of eyes with and without sinkholes. Blood flow (BF), represented by laser speckle flowgraphy-measured tissue-area mean blur rate (MBR), was measured in the temporal optic nerve head (ONH), temporal peripapillary chorioretinal atrophy (PPA) zone, and in the ICC zone. ICC area and angle were analyzed in OCT en-face images. Mean deviation and total deviation in the central area (TD-central) were measured with Humphrey visual-field testing. RESULTS: A total of 86 eyes (3.1%) had ICCs and 52 eyes (1.9%) had sinkholes. ICC eyes had a lower spherical equivalent and longer axial length (AL) than non-ICC eyes (P < 0.05). Patients with eyes with sinkholes were more elderly and had worse best-corrected visual acuity, worse TD-central, a larger ICC, and lower tissue-area MBR in the temporal ONH, temporal PPA zone, and ICC zone (P < 0.05). CONCLUSION: In eyes with glaucoma, AL elongation might be linked to ICC formation. Sinkhole formation might be associated with ICC enlargement, impaired ocular BF, and impaired retinal structure and function involving the central area.

BACKGROUND: Trabeculectomy (trab) is the most effective surgical procedure for lowering IOP and preventing glaucoma progression. However, decline in best-corrected visual acuity (BCVA) is one of the most serious postoperative complications of trab. Here, we investigated methods to predict decreased BCVA after trab in glaucoma patients with good preoperative BCVA. METHODS: This study included 35 eyes of 35 open-angle glaucoma (OAG) patients (male / female: 21 / 14, age: 64.0 ± 9.7 years old, preoperative intraocular pressure: 15.9 ± 5.4 mmHg, mean deviation: -18.1 ± 5.6 dB) with preoperative BCVA of 0.7 or better who underwent trab and were observed for more than 12 months. As a preoperative analysis, we measured temporal quadrant circumpapillary retinal nerve fiber layer thickness (cpRNFLT) and ganglion cell complex thickness in a central strip between the disc and fovea (csGCCT), an area that corresponds to the location of the papillomacular bundle (PMB) in swept-source optical coherence tomography (OCT). We defined BCVA decline as a loss of more than 3 lines of BCVA after 12 months. Measurement parameters were compared between the BCVA-decline group and the non-BCVA-decline group. RESULTS: BCVA decline was detected in 11 cases (31.4%) 12 months after trab. There was a statistically significant difference in axial length (P = 0.049). A single logistic analysis showed that the BCVA-decline group had significantly lower cpRNFLT than the non-BCVA-decline group (27.7 ± 8.0 μm vs. 45.1 ± 5.3 μm, P < 0.001, cut-off value: 33.4 μm), as well as lower csGCCT (72.4 ± 7.7 μm vs. 87.5 ± 5.1 μm, P = 0.002, cut-off value: 82.3 μm). Multivariable logistic analysis showed that the BCVA-decline group had significantly lower temporal quadrant cpRNFLT (P < 0.001) and lower middle csGCCT (P < 0.001) compared to the non-BCVA-decline group. CONCLUSIONS: Lower temporal quadrant cpRNFLT and middle csGCCT, OCT scan areas that correspond to the location of the PMB, might be biomarkers that predict BCVA decline after trab in OAG patients with good vision.

We aimed to investigate the impact of glutathione trisulfide (GSSSG) on lipopolysaccharide (LPS)-induced inflammation in retinal glia. Inflammatory responses in mouse-derived glial cells and Wistar rat retinas were stimulated with administration of LPS. Cell survival and proinflammatory cytokine production were examined using the Calcein-AM assay, and reverse transcription-quantitative polymerase chain reaction (RT-qPCR) and enzyme-linked immunosorbent assay (ELISA), respectively. Retinal microglia were visualized with immunohistochemistry for Iba1. Administration of LPS (10 µg/mL) or GSSSG (less than 100 µM) did not affect survival of cultured primary Müller cells and established microglial cells (BV-2). RT-qPCR and ELISA indicated that GSSSG inhibited LPS-induced gene upregulation and protein secretion of proinflammatory cytokines in these glial cells and rat retinas. GSSSG inhibited LPS-induced activation of TGF-β-activated kinase 1 (TAK1), which is an upstream kinase of NF-κB, in BV-2 cells. Finally, in vivo experiments indicated that intravitreal administration of GSSSG but not its relative glutathione disulfide (GSSG) inhibited LPS (500 ng)-induced accumulation of Iba1-immunopositive microglia in rat retinas. Taken together, GSSSG has the potential to prevent pathogenesis of inflammation-associated ocular diseases by inhibiting proinflammatory cytokine expression in retinal glial cells.

PURPOSE: To investigate the association between ocular/systemic factors and visual acuity decline in glaucoma patients with loss of ganglion cell complex thickness (GCCT). METHODS: In 515 eyes of 515 patients with open-angle glaucoma (mean age, 62.6 ± 12.8 years; mean deviation, -10.95 ± 9.07 dB), we used swept-source optical coherence tomography to measure macular GCCT in sectors classified as corresponding to circumpapillary retinal nerve fiber layer clock-hour sectors from 7 o'clock (inferotemporal) to 11 o'clock (superotemporal). We calculated Spearman's rank correlation coefficient between each sector and best-corrected visual acuity (BCVA), determined cutoff values for BCVA decline (<20/25), and used multivariable linear regression models to determine the correlation between BCVA and biological antioxidant potential (BAP), corneal hysteresis (CH), and temporal-tissue optic nerve head blood flow (represented by temporal mean blur rate, or MBR-T). RESULTS: Macular GCCT corresponding to the 9 o'clock sector had the highest correlation with BCVA (Rs = -0.454; P < 0.001) and a cutoff of 76.17 µm (area under the receiver operating characteristic curve = 0.891; P < 0.001). Subjects below this cutoff (N = 173) showed significant correlations between BCVA and age, BAP, CH, and MBR-T (β = 0.192, P = 0.033; β = -0.186, P = 0.028; β = -0.217, P = 0.011; and β = -0.222, P = 0.010, respectively). CONCLUSIONS: Multiple factors are involved in BCVA decline in patients with glaucoma with decreased macular GCCT. This suggests that evaluating BCVA may require assessing multiple factors. TRANSLATIONAL RELEVANCE: Multiple factors contribute to BCVA decline.

BACKGROUND: Glaucoma is multifactorial, but the interrelationship between risk factors and structural changes remains unclear. Here, we adjusted for confounding factors in glaucoma patients with differing risk factors, and compared differences in structure and susceptible areas in the optic disc and macula. METHODS: In 458 eyes with glaucoma, we determined confounding factors for intraocular pressure (IOP), central corneal thickness (CCT), axial length (AL), LSFG-measured ocular blood flow (OBF), which was assessed with laser speckle flowgraphy-measured mean blur rate in the tissue area (MT) of the optic nerve head, biological antioxidant potential (BAP), and systemic abnormalities in diastolic blood pressure (dBP). To compensate for measurement bias, we also analyzed corrected IOP (cIOP; corrected for CCT) and corrected MT (cMT; corrected for age, weighted retinal ganglion cell count, and AL). Then, we determined the distribution of these parameters in low-, middle-, and high-value subgroups and compared them with the Kruskal-Wallis test. Pairwise comparisons used the Steel-Dwass test. RESULTS: The high-cIOP subgroup had significantly worse mean deviation (MD), temporal, superior, and inferior loss of circumpapillary retinal nerve fiber layer thickness (cpRNFLT), and large cupping. The low-CCT subgroup had temporal cpRNFLT loss; the high-CCT subgroup had low cup volume. The high-AL subgroup had macular ganglion cell complex thickness (GCCT) loss; the low-AL subgroup had temporal cpRNFLT loss. The high-systemic-dBP subgroup had worse MD, total, superior, and inferior cpRNFLT loss and macular GCCT loss. The low-BAP subgroup had more male patients, higher dBP, and cpRNFLT loss in the 10 o'clock area. The high-OBF subgroup had higher total, superior and temporal cpRNFLT and macular GCCT. CONCLUSIONS: Structural changes and local susceptibility to glaucomatous damage show unique variations in patients with different risk factors, which might suggest that specific risk factors induce specific types of pathogenesis and corresponding glaucoma phenotypes. Our study may open new avenues for the development of precision medicine for glaucoma.

PURPOSE: Retinal nerve fiber layer defects (RNFLDs) become enlarged with glaucoma progression. We measured the RNFLD angle and investigated whether it was correlated with deterioration of the visual field in patients with glaucoma. METHODS: This study included 84 eyes of 84 patients with open-angle glaucoma (mean deviation [MD] = -6.51 ± 5.91 dB, follow-up period = 2.82 ± 0.74 years) with the RNFLDs, who underwent en face swept-source optical coherence tomography (SS-OCT) wide scans (12 × 9 mm) at least 6 times. The RNFLD angle was measured as the intersection between the RNFLD and a circle centered on the disc with a radius half the distance between the disc and the fovea. Slopes for the RNFLD angle, macular ganglion cell layer thickness (GCCT), and circumpapillary RNFL thickness (cpRNFLT) were compared with the MD slope, as measured with the Humphrey field analyzer 24-2 program. RESULTS: The correlation coefficients with MD slope were -0.67 for the RNFLD angle slope (P < 0.001), 0.15 for the macular GCCT slope (P = 0.163), and 0.04 for the cpRNFLT slope (P = 0.719). The RNFLD angle tended to increase as the number of disc hemorrhage occurrences increased (rs = 0.31, P = 0.004). The RNFLD angle slope also had good predictive power for glaucoma progression (area under the receiver operating characteristic curve = 0.88, 95% confidence interval = 0.81-0.95). CONCLUSIONS: We found that the RNFLD angle slope was more closely associated with the MD slope than were other OCT parameters. This suggests that measurement of the RNFLD angle with en face OCT images could be effective in evaluating glaucoma progression. TRANSLATIONAL RELEVANCE: Our study provides a method for monitoring glaucoma progression with SS-OCT.

Detection, diagnosis, and treatment of ophthalmic diseases depend on extraction of information (features and/or their dimensions) from the images. Deep learning (DL) model are crucial for the automation of it. Here, we report on the development of a lightweight DL model, which can precisely segment/detect the required features automatically. The model utilizes dimensionality reduction of image to extract important features, and channel contraction to allow only the required high-level features necessary for reconstruction of segmented feature image. Performance of present model in detection of glaucoma from optical coherence tomography angiography (OCTA) images of retina is high (area under the receiver-operator characteristic curve AUC ~ 0.81). Bland-Altman analysis gave exceptionally low bias (~ 0.00185), and high Pearson's correlation coefficient (p = 0.9969) between the parameters determined from manual and DL based segmentation. On the same dataset, bias is an order of magnitude higher (~ 0.0694, p = 0.8534) for commercial software. Present model is 10 times lighter than Unet (popular for biomedical image segmentation) and have a better segmentation accuracy and model training reproducibility (based on the analysis of 3670 OCTA images). High dice similarity coefficient (D) for variety of ophthalmic images suggested it's wider scope in precise segmentation of images even from other fields. Our concept of channel narrowing is not only important for the segmentation problems, but it can also reduce number of parameters significantly in object classification models. Enhanced disease diagnostic accuracy can be achieved for the resource limited devices (such as mobile phone, Nvidia's Jetson, Raspberry pi) used in self-monitoring, and tele-screening (memory size of trained model ~ 35 MB).

Purpose: Maintaining visual acuity in glaucoma patients is an important part of preventing the deterioration of quality of vision. We identified specific areas of the papillomacular bundle (PMB) that were strongly associated with visual acuity, based on en-face images derived from optical coherence topography (OCT) wide scans. Methods: The study recruited 23 eyes of 21 glaucoma patients (age: 61.3 ± 13.0 years, M: F = 9:12, Humphrey field analyzer-measured mean deviation: -19.9 ± 6.5 dB) with good best-corrected visual acuity (20/20 or more) and a remaining PMB with a maximum width no more than half that of the vertical disc diameter. En-face images were derived from 12 × 9 mm wide-scan images made with DRI-OCT (Triton, Topcon). Averaged en-face images were created by identifying the disc center and fovea line (DFL) and aligning it between images. We then measured the frequency of remaining PMB at 10 µm intervals along a vertical line intersecting the DFL at its midpoint. Finally, we used a logistic analysis in a much larger group of patients to identify cases of glaucoma with low BCVA (<20/20).Results: In the averaged en-face image, the residual PMB area appeared as a high-intensity region above the DFL. Analysis showed that residual PMB was most common in an area 830-870 µm above the DFL. The correlation coefficient of residual PMB in this area to BCVA was -0.57 (p < .01), and among OCT parameters in this residual PMB area, the AUC to identify decreased BCVA was highest for ganglion cell complex thickness (0.85, p < .01), with a cutoff of 87.5 µm.Conclusions: This study identified specific areas of the PMB that were associated with BCVA in wide-scan, en-face OCT images from glaucoma patients. This suggests that it may be possible to identify visual impairment during glaucoma treatment by measuring this area.

PURPOSE: Assessing the papillomacular nerve fiber bundle (PMB) can identify glaucoma patients with decreased visual acuity. In this study, we explore efficient methods for evaluating PMB thickness in glaucoma patients, based on swept source-optical coherence tomography (SS-OCT). METHODS: This study included 347 eyes of 205 open-angle glaucoma (OAG) patients. Patients were excluded if they had best-corrected decimal visual acuity < 0.3, axial length >28 mm, non-glaucoma ocular disease, or systemic disease affecting the visual field. We obtained vertical 12.0 × 9.0 mm 3D volume scans covering both the macular and optic disc regions with SS-OCT (DRI OCT Triton, Topcon), and measured the thickness of the PMB, as well as average macular retinal nerve fiber layer thickness (mRNFLT) and macular ganglion cell complex thickness (mGCCT) in the macular map and temporal-quadrant circumpapillary RNFL thickness (tcpRNFLT). We also measured central-strip RNFLT (csRNFLT) and GCC (csGCCT) in a 1.5 × 6.6 mm area of the scan centered between the fovea and optic nerve head. CsRNFLT and csGCCT were divided lengthwise into three 1.5 × 2.2 mm sections. We then calculated Spearman's rank correlation coefficient between these OCT measurements and visual acuity. Logistic regression analysis was used to find the cutoff value for the OCT measurements to predict logMAR < 0. RESULTS: The correlation coefficients with logMAR were 0.38 for mRNFLT, 0.44 for mGCCT, 0.37 for middle csRNFLT, 0.50 for middle csGCCT, and 0.33 for tcpRNFLT (all P < .0001). For middle csGCCT, the area under the curve indicating decreased visual acuity was 0.80, with a cutoff value of 88.6 μm (P < .001). CONCLUSIONS: We found strong associations between OCT parameters in the PMB, especially middle csGCCT, and visual acuity in patients with OAG. The thickness of the PMB may therefore be valuable information for glaucoma care and may help prevent visual acuity disturbance.
