The decline in mental health among workers in Japan has become a serious concern. Autism spectrum disorder (ASD) traits are increasingly recognized as a potential risk factor for mental health issues, yet few individuals receive a formal ASD diagnosis. This study aims to examine whether undiagnosed but working individuals with strong ASD characteristics differ in factors related to presenteeism (low productivity) compared to their counterparts with fewer ASD traits. In particular, we investigated the impact of social camouflaging—the behavioral adaptation used to mask ASD traits—especially on workers with strong ASD characteristics. An online survey was conducted among white-collar workers aged 20 to 60. The survey included items collecting demographic information (gender, age, marital status, highest level of education completed, employment status, job title, length of employment, and working hours) and included the Japanese versions of four validated instruments to measure aspects of autism traits, social camouflaging, work functioning, and occupational stress. Of the 543 respondents, 139 (25.6%) exhibited strong ASD characteristics. These individuals reported lower productivity than their counterparts. While social camouflaging was observed across all ASD trait levels, it was specifically linked to poorer mental health and reduced productivity among workers with strong ASD characteristics. To support workplace well-being and productivity, it is crucial to increase awareness and understanding of ASD traits in professional settings and create an environment that accommodates diverse work styles.

Based on Antonovsky’s theory, we explored the importance of adolescent sense of coherence (SOC) in coping with stress and how it was affected by the pandemic. Using longitudinal data from junior and senior high school students in urban areas in Japan, we examined the trends in SOC and factors related to SOC in students enrolled before and after the COVID-19 pandemic. With the cooperation of the Tokyo Metropolitan Secondary Education School, we surveyed 97 students who enrolled in 2018 (G1) and 144 students who enrolled in 2020 (G2). Four surveys were conducted for G1 and three for G2. Survey items included SOC, psychosomatic symptoms scale, Athens insomnia scale, school belonging scale, and stress experience scale. We followed the trends in SOC scores by gender and performed t-tests and multiple regression analysis. G2 had higher baseline SOC scores than G1, but the significant difference between the two groups disappeared by 2022. From 2019, comprehensibility and manageability significantly increased in G1 for girls, but meaningfulness decreased in G2 for both boys and girls. Multiple regression analysis showed no correlation between baseline SOC and SOC in 2022 in G1, which differed from G2, suggesting that the pandemic may have changed their perception of the world.

Objective A longitudinal study was conducted among Japanese women raising adolescents to determine how the COVID-19 pandemic has affected their sense of coherence (SOC) and to provide suggestions for supporting them. Methods The SOC scores of 138 pairs of middle-aged women and their children (junior high school students) were compared at two time points: 2019 (before the pandemic, T1) and 2020 (during the pandemic, T2). Results Overall, the women’s SOC did not change, whereas the adolescents’ SOC increased. In contrast, 44% of the women’s SOC scores decreased during the pandemic; no differences were detected between the SOC maintenance and increase group (G1) and the SOC decrease group (G2) in mental health, subjective physical health, social capital, and job status, and the child variables were not related. Multiple regression analysis of G1 and G2 with SOC at T2 as the dependent variable showed that for G2, at T1, having good mental and physical health conditions, a rich social capital, and having a job were positively associated with SOC during the pandemic. Conclusion Middle-aged Japanese women, who often work as informal workers, are easily laid off and are involved in care work. Thus, the change in their socioeconomic status due to the pandemic may have been severe. Given the long-term social impact of the pandemic, it is necessary to consider support for women, including economic and social support such as employment and building human connections.

Japanese public health nurses (PHNs) at public health centers (PHCs) have played critical roles in infection prevention and control during the COVID-19 pandemic. This study aimed to examine the actual pandemic-related experiences of PHNs and the relation between their experiences, individual resilience, two components of organizational resilience (system and human resilience), and burnout. An analysis of the responses of 351 PHNs revealed that mid-level PHNs scored higher in experience and lower in organizational resilience compared with those in other positions. More than 80% of respondents experienced inappropriate staff allocation. Multiple regression revealed that burnout was positively associated with the components of the experience of PHNs and negatively with individual and human resilience. In hierarchical multiple regression with depersonalization as the dependent variable, the sign of system resilience reversed from negative to positive when human resilience was added. The results highlight the need to prepare for future health crises including establishing a system with enough personnel, promoting human resilience such as collaboration among staff members, and burnout prevention measures, especially among mid-level PHNs. The study also described alternative approaches to comprehend system resilience—namely, a suppression variable of human resilience, promotion of depersonalization, and multicollinearity—and the need for further research on organizational resilience.

<p lang="fr"><abstract><p>Public health nurses (PHNs) are among the few municipal civil servants who lead community infection control and prevention initiatives in Japanese public health centers (PHCs). This study aims to investigate the distress faced by PHNs and clarify their difficulties and working environment relative to infection prevention control activities during the COVID-19 pandemic. We adopted a qualitative description methodology in this study of 12 PHNs who were involved in COVID-19 prevention and control in PHCs in Prefecture A. The distress during the early phase of the pandemic was due to the uncertainty of the SARS-CoV-2 related disease, which caused panic in medical institutions and among residents. PHNs were overwhelmed, distressed and exhausted by their inability to control the ‘pandemic’, lack of patient cooperation for prevention control and the unsustainable organizational environment. They were also distressed because they were one of the specialized personnel responsible for saving residents' lives with limited medical resources and while having identity crises due to an inability to carry out the PHN's role of controlling infection in the community. For future crises, rapid, drastic innovation defying conventional organizational systems is critical to reform sustainable organizations so that they play an effective role in the community. Innovation in crisis communication and strengthening the medical system will help achieve a resilient community in a health crisis.</p></abstract></p>

We conducted a longitudinal study to clarify the changes in the sense of coherence (SOC); that is, the ability to cope with stress successfully, of 166 Japanese junior high school students and their mothers before and after the onset of the COVID-19 pandemic. First, we analyzed changes in SOC at three time points for all students and divided them into two groups: Group 1 included students with SOC scores that increased or maintained before and after the onset of the pandemic and Group 2 included students with decreased SOC scores after the onset of the pandemic. Second, we conducted a comparative analysis between the two groups. Overall, results indicated that student's SOC scores increased. Additionally, interpersonal stress scores were lower after the onset of the pandemic than before. There were almost no differences in family relationships, financial conditions, or personality tendencies between the two groups. However, Group 2 did not regain their sense of belonging to school. In this group, the frequency of stress experiences in club activities after the onset of the pandemic, troubles with the opposite gender, and inability to catch up with the contents of the subject lecture were high. The accumulation of small stressors may have hindered the maintenance of a sense of school affiliation. Mothers of students in Group 2 either were full-time employees at baseline or had started a new job after the onset of the pandemic. Their children may have been affected by the household's damaged financial budget and changes in mother's working styles. As COVID-19 reduced the number of days students went to school, students' SOC could have reduced had they not felt a sense of presence or belonging due to the lack of participation in club activities, school events, etc. Teachers and mothers should communicate carefully with their students and children, respectively, to develop a sense of belonging.

In Japan, there is a high incidence of family members caring for their elderly. To facilitate this, caregivers often quit their jobs, work reduced hours, and forfeit leisure activities. This study examined the relationship between the mental health of the caregivers and the sacrifices and adjustments they make to care for the elderly. A cross-sectional survey was conducted with responses from 171 caregivers. Referencing Pearlin's stress process model, the relationship among five types of work change, four types of leisure activity quitting, caregivers' subjective care burden, and depression were analyzed using -test and multiple regression analysis. Caregivers who quit their work or other home activities had significantly more daily living care responsibilities than those who did not. Moreover, caregivers who gave up leisure activities had a greater sense of subjective care burden than those who did not. The experience of giving up peer activities and taking leave of absence from work was significantly associated with increased depressive symptoms. This review aimed to understand the dimensions of self-stigma that are unique to the families of persons with autism spectrum disorder (ASD) and the conditions in which self-stigma in families of persons with ASD arises. We reviewed the self-stigma dimensions in families of persons with ASD. Seventeen studies met our inclusion criteria and provided qualitative information on the dimensions of self-stigma. The identified dimensions were social misunderstanding, negative prejudice, social rejection, isolation, emotional reactions, and stigma management. The dimension of social misunderstanding was unique to families of persons with ASD. This review adds insights into self-stigma theory, and, by clarifying the dimensions of self-stigma, suggests areas for future self-stigma research among families of persons with ASD.

Adolescence is marked by significant life stress. Recently, school refusal and dropouts as well as suicide among Japanese adolescents have increased. Sense of coherence (SOC) is recognized as a competency that helps people deal with stress. The purpose of this study was to examine the factors influencing SOC in male and female high school students. We conducted a survey with 203 pairs of high school students and their mothers, in Tokyo, to explore their SOC, family relationships, school belonging, and autistic traits. Analysis of the data revealed a weak relationship between female students' SOC and that of their mothers, and no relationship between male students' SOC and their mothers' SOC. Feelings of acceptance and recognition from teachers improved students' SOC, irrespective of gender. Low SOC in mothers had a negative impact on female students' SOC, and children's lack of imagination (an autism spectrum tendency) had a negative impact on male students' SOC. This study revealed the importance of support at home and school according to the needs of both genders.
Development and Preliminary Validation of the Couples' Stigma Scale to Assess Self-Stigma Among the Partners of Persons With Autism Spectrum Disorder in Japan 査読有り

Spouses of individuals with autism spectrum disorder (ASD) may struggle with self-stigma and may require attention and care; however, no scale exists to measure the stigma of spouses of persons with ASD. This study created and investigated the construct validity of the Couples Stigma Scale. This scale consists of 14 items and it was designed based on prior literature, interviews, and the self-stigma theory to assess the self-stigma experienced by spouses of people with ASD. A survey was conducted with spouses of persons with ASD who participated in a self-help group. Responses were obtained from 259 people, of which 253 women were included in the analysis. Exploratory factor analysis was performed separately with two independent groups, indicating a four-factor structure, to determine structural validity. The factor loadings of the items constituting the four factors were 0.39 or greater. Regarding external validity, the correlation coefficient between the Couples Stigma Scale and the World Health Organization Quality of Life (WHOQOL) score was -0.341 (p < 0.001), and the domain correlation coefficient was significant for all relevant WHOQOL domains. Our results suggest that the Japanese version of the Couples Stigma Scale is a valid instrument for assessing self-stigma in the spouses of persons with ASD.

The purpose of this study was to clarify the factors related to perceived positive change and sense of coherence in mothers ( = 293) of children with cleft lip and palate. The strongest negative correlation with sense of coherence was stigmatizing words from others; there were positive correlations with social capital and perceived positive change. Multiple regression showed that to be aware of positive changes, mothers share their feelings with mothers in the same position, are rooted in a safe community and do not feel isolated in parenting. People who have experienced adversity may have the opportunity to enhance sense of coherence by obtaining perceived positive change.

近年、広汎性発達障害(PDD)の増加が各国で報告されているが、PDD児に続く妊娠・出産に関する研究は少なく、母親の障害認識と次子妊娠・出産にどのような関連があるのか、いつどのような支援が求められているのかについては把握されてこなかった。本研究ではPDD児の母親の障害認識の有無により、次子妊娠・出産をめぐる体験にどのような相違があるのかを明らかにし、母親にとって望ましい環境構築のための示唆を得た。結果から、年長子の障害を認識していなかった母親の中には、次子妊娠・出産を否定的に受け止める場合もみられたこと、両群共に次子との比較が年長子の障害認識を深め、きょうだい間の違いが次子の障害の可能性を否定する根拠となっていることが明らかになった。以上から、障害認識の有無や次子の状態を考慮に入れた支援、次子誕生を考慮した障害受容研究、母親の不安軽減、社会的理解と家族に負担を強いない制度構築の必要性が示唆された。

中学生とその保護者を対象として、「生きる力」としてのストレス対処力Sense of Coherence（以下SOC）を指標として、COVID-19によるパンデミック前から継続している縦断研究により、パンデミック前後のSOCの変化をとらえ、またパンデミックが中学生に及ぼした影響について明らかにし、中学生SOC向上に向けての支援策を探求する。
通常学級に在籍する発達障がいのある中学生への健康支援プログラム開発競争的資金

本研究の目的は、発達障がい（疑いを含む）のある中学生に養護教諭が適切な支援をするためのプログラムの開発である。2021年度は発達障がいのある生徒に対する養護教諭の健康支援の実態を明らかにするために、2020年に実施した質問紙調査の有効回答193を分析した。養護教諭が実践している合理的配慮に関する質問10項目の因子分析を行った結果、『認知や理解の程度に応じた対応』、『服薬指導・健康診断時の対応』、『対人関係スキルの指導』の3因子が抽出された。さらに発達障がいのある生徒に接するときに養護教諭が工夫していることについて自由記述式回答の記入があった105人（52.5％）のデータを質的に分析した。その結果230のデータが抽出された。自由記述のデータを類似内容でコード化した結果、『話し方や説明の仕方の工夫』が92（40.0％）と最も多く、次いで『自己肯定感をあげるような関わり』37（16.1％）、『教職員との連携』21（9.1％）、『特性に応じた対応』13（5.7％）の順であった。少数意見としては『他の生徒が不平等感を感じないような対応』、『養護教諭自身が感情的にならない』などが記載されていた。一方発達障がいのある生徒への支援で養護教諭が感じている困難感に関する質問16項目の因子分析を行った結果では、『集中できない、不注意、衝動的などの注意欠如多動症の特徴』と『コミュニケーションの障害、対人関係や社会性の障害、こだわりや興味の偏りなどの自閉症スペクトラムの特徴』の2因子が抽出された。発達障がいの特性は個々に異なるため、その生徒の状態を見極めて個人に必要な支援を行うことが必要である。そのための基盤として発達障がいを理解すること、特性に応じた具体的な支援方法を学ぶことが重要であることが調査結果から示唆された。今後プログラム内容にこれらを反映し研究を進めていく。
保健師が活用できる発達障害児の養育者の子育て支援ガイドラインの開発競争的資金

本研究の目的は保健師が活用する発達障害児の養育者への子育て支援ガイドラインの開発である。2021年度は養育者が求める支援ニーズを明らかにするために調査を実施した。10名の養育者に対して半構成的インタビュー調査を実施した。8名は3密を回避して十分な空間を確保したうえで対面による調査、2名はオンラインで調査を実施した。同意を得てインタビュー内容を録音し逐語録を作成した。そして養育者が保健師に求める支援として語られた内容を類似性でまとめた結果、以下のようなカテゴリーが抽出された。『発達障害の十分な理解』、『早期からの正しい情報提供』、『乳幼児健診時の対応』、『乳幼児健診後の支援』、『母親への子育てサポート』、『安心して相談できる場づくり』があげられた。具体的な内容としては、乳幼児健診で発達障害が把握できるように保健師には『発達障害の十分な理解』をもってほしい、また『早期からの正しい情報提供』では乳幼児健診等で発達障害等を含めた子どもの発達の多様性に関する情報を提供してほしい、『乳幼児健診時の対応』では待っている時間の子どもの行動も観察して子どもの状況把握をしてほしい、母親の訴えに耳を傾けてほしいなどが挙げられた。またグレーゾーンの場合、様子をみましょうと言われ、次の健診まで何のアプローチもなく不安になるので、健診後のフォローをしてほしいという『乳幼児健診後の支援』が述べられた。さらに子どもの特性から母親は外出も思うようにできないことが多く、一人で頑張っている場合が多いので『母親への子育てサポート』をしてほしい、母親が孤独にならないように『安心して相談できる場づくり』をしてほしいが述べられた。これら養育者の支援ニーズ調査結果をガイドラインに反映させていく予定である。

中学生の生きる力としてのストレス対処力SOCについて、縦断研究によりその変化を明らかにした。①母親と思春期の中学生のSOCには殆ど相関関係がみられないこと、②中学時代においてSOCは変化することが明らかになった。また、パンデミック発出前と比較して発出後のほうが全体としてはSOCが高くなった一方で、およそ4割の生徒のSOCは下がったまま回復していないことが分かった。思春期の生徒のSOCのスコアやその変化に大きく影響しているのは、家庭よりもむしろ学校での友人、先生との関係、部活の出来事、そして「学校居場所感」であった。コロナ禍では、居場所感とSOCの下位尺度である有意味感を育む必要がある。

本研究は、通常学級に在籍する発達障がい(疑いを含む)のある小学生への支援において、養護教諭が活用する健康支援プログラムを開発することを目的とした。公立小学校の養護教諭に発達障がいについての質問紙調査を行った結果、具体的な支援技術を学びたい要望が多い結果が得られた。それを基に支援プログラムを編成した。プログラムに基づき2日間の研修を開催した結果、研修参加者からはプログラムの肯定的な評価が得られ、発達障がいのある小学生への健康支援プログラムとして有用であることが示唆された。

育児や介護を担う家族ケアラーとして、先天性障害児（口唇口蓋裂児）の母親と、高校生を養育する母親を対象とし、ストレス対処力SOCの関連要因を探索した。障害児の母親にとって、気持ちを分かってもらう経験や医療従事者からの丁寧な説明が、高いSOCや肯定的変化の獲得と関連した。「周囲の人からの辛い言葉」は、SOCの低さと有意な関連を示した。高校生の母親は、ソーシャルキャピタルや家族関係の良好さがSOCの高さとプラスの相関を示した。また、子どもの自閉傾向の特性と、子どもに関する悩みがあることがSOCとマイナスの相関を示した。

本研究は、家族介護者の社会活動の中でも特に就労と余暇活動に着目し、(1)面接調査において介護者の介護認識プロセスの変化と社会活動との関係を描き出し、(2)質問紙調査において、介護者の余暇活動が介護者の精神健康に及ぼす影響を明らかにすることを目的とした。その結果、面接調査により、介護者は【介護に対する心理的拘束の引き下げ】と【介護から離れる時間を確保する試み】を行い、【社会活動の再開・参加】をすることで【介護と自分の生活とのバランス再構築】に至ることがわかった。質問紙調査では、退職経験や仕事量を減らす経験の有無は、療養者のADLおよびIADL介護量と関連していることがわかった。社会活動を辞めた経験は介護負担感の増大とCES-D悪化と関連を持つことが示された。具体的には、自宅外グループ活動(町内会、趣味の会、スポーツの会)を辞めた経験があること(p<0.05)、介護のために休職した経験があること(p<0.05)が、CES-Dの悪化と有意な関連を示した。社会活動以外でCES-D悪化と有意な関連を示したのは、暮らし向きが良くないこと(p<0.05)であった。
