BACKGROUND: Chemotherapy-induced taste and smell alterations in cancer patients are associated with multiple adverse effects, namely, malnutrition, weight loss, and a diminished quality of life. The aim of this prospective study was to identify the incidence of taste alterations following epirubicin and cyclophosphamide (EC) chemotherapy in patients with breast cancer without previous history of cancer or chemotherapy. METHODS: Forty-one patients undergoing EC chemotherapy for breast cancer at Tokai University Hospital were included. A subjective (questionnaire) and an objective (filter paper disk method) assessment for 5 basic tastes were administered on day 4 post-chemotherapy and immediately before the subsequent cycle of chemotherapy for each cycle, in addition to an olfactory evaluation and oral examination. The correlation between subjective and objective taste alterations and factors influencing these alterations were analyzed by statistical means. RESULTS: The mean incidence of subjective taste alteration on the 4th day after chemotherapy was 53%. In each of the 4 cycles, taste alterations decreased to about 9.0% immediately before the next cycle. A significant correlation between subjective and objective assessments was seen only for salty taste, suggesting important differences in subjective versus objective assessment outcomes. A multivariate analysis indicated that age and body surface area influenced taste alterations. CONCLUSIONS: EC chemotherapy induced taste alterations in more than 50% of patients, which decreased to less than 10% immediately before the next chemotherapy cycle. A combination of objective and subjective assessments is essential to evaluate taste alterations induced by EC chemotherapy. These could be used in routine clinical practice.

OBJECTIVE: We analyzed the diagnostic performance of the MR imaging findings of the parotid, submandibular, and sublingual glands to discriminate between patients with and without Sjögren's syndrome. METHODS: We retrospectively analyzed the correlation between the MR imaging and histopathological findings obtained from 69 patients with clinically suspected Sjögren's syndrome. We evaluated the heterogeneous signal intensity distribution on T1- and T2-weighted images, the multiple high-signal-intensity spots on MR sialograms, and the volume of the parotid, submandibular, and sublingual salivary glands. RESULTS: The multiple high-signal-intensity spots in the parotid gland showed the highest sensitivity and diagnostic accuracy (82% and 83%, respectively). In addition, the multiple high-signal-intensity spots and the heterogeneous signal intensity distribution in the submandibular gland showed high specificity (100% and 88%, respectively). The volume of the submandibular gland, but not that of the parotid or sublingual gland, was smaller in patients with Sjögren's syndrome. CONCLUSIONS: The presence of multiple high-signal-intensity spots on an MR sialogram in the parotid gland should be considered the best diagnostic indicator for Sjögren's syndrome. The presence of spots, heterogeneity, and the change to smaller volumes in the submandibular gland were also helpful because of their high specificity, particularly in advanced cases.

Patients with dry mouth are increasing in Japan due to the country's super-aging society and stressful modern society. Dry mouth affects quality of life, including difficulty with speech or swallowing, and also causes aspiration pneumonia and, respiratory infection. Moreover, dry mouth closely relates to taste dysfunction, resulting in malnutrition, a common risk of nursing care in the elderly. Against these backgrounds, dry mouth has recently been a target of social concern. Drugs for dry mouth such as M3, a muscarinic agonist, have been widely used: however, these drugs have serious side effects, such as vomiting, sweating and/or digestive disorders. We examined the gustatory-salivary reflex to explore a remedy for dry mouth. We have demonstrated that umami stimulation increases saliva from both minor- and major salivary-glands, and that the effect is longer-lasting than with the other four basic tastes. Stimulating the umami response could therefore be a safe and useful remedy for dry mouth.

To measure blood flow in the dental pulp and the periodontal ligament (PL), a Laser Doppler Velocimetry that consists of helium and neon Laser was applied to the canine teeth of dogs and cats. The following results were obtained: 1. The pulpal blood flow (PBF) could be monitored noninvasively and continuously whenthe distance from the probe to the dental pulp was within 1 mm.According as this distancebecame shoter, PBF showed higher value. 2. Electrical stimulation of the ipsilateral cervical sympathetic nerve induced prompt decrease in PBF according to the frequency of stimulation. 3. To monitor PL blood flow, the labial bone overlying the mandibular canine tooth rootwas pared away, and apical area of PL blood flow was continuously monitored. Whenforce was applied labio-lingually to the incisal edge of the tooth, PL blood flow showedpromptly decrease, and PL blood flow promptly increased when force was applied linguoladiplly. These results indicate that Laser Doppler Velocimetry considered to be availablefor monitoring the pulpal and the periodontal ligament blood flow. (J.Jpn. Soc. 我が国では超高齢化や食生活の変化などを背景に味覚障害を有する高齢者が急増している。味覚障害は単なる感覚障害に留まらず、食欲不振から体重減少、さらに体調不良に直結することから社会問題になっている。我々は、他の基本４味（甘、酸、塩、苦）の感受性は正常でありながら、「うま味」の感受性のみが低下するため極度の食欲不振に陥る高齢者を数多く経験し報告している。 2020-2021年度は、味覚障害を主訴として東北大学病院を受診した患者を対象として、以下①～③のデータ収集を行った。 ①医療面接―患者背景、年齢、性別、原疾患、既往歴、QOL-ACDによるQOL評価、アンケート調査による味覚・食事の変化、生活の変化、②味覚検査：a)濾紙ディスク法（酸味、苦味、塩味、甘味）テーストディスク; 三和化学研究所、濾紙ディスク法（うま味）、本研究グループ開発、b)電気味覚検査、③唾液検査：a)総唾液分泌量の測定、b)小唾液腺の唾液分泌量測定に関する

It is known that over 60 % of patients with taste dysfunction are originally caused by zinc deficiency, and that zinc intake therapies are effective for these patients. But patients have to take blood tests with surgical stress to assay zinc deficiency. On the other hand, carbonic anhydrase (CA) 6 has significant relation to taste dysfunction. As it takes a long time to detect CA 6 by a previously established ELISA method, we have tried to develop a swift and simple new diagnostic method using the imunochromatographic analysis. We attempted to apply the CA 6 monoclonal antibody to the screening of the taste dysfunction, and to examine effects of zinc oral administration on the taste disc levels and the CA 6 values in parotid saliva. CA 6 values negatively correlated with taste disk levels. Those results suggest that the measurement of CA6 values in parotid saliva might be helpful to establish a new and rapid screening method for taste dysfunction caused by zinc deficiency.

We conducted epidemiological survey about oral dryness and taste disorder on victims of Great East Japan Earthquake. As for oral dryness, whole salivation volume and minor gland secretion were measured. As for taste disorder, recognition thresholds of five basic tastes were respectively measured. Mental stress caused by Great East Japan Earthquake was evaluated by POMS (Profile of Mood States) on each victim. This study showed that the greater mental stress decreased whole salivation and/or minor salivary gland secretion, and rose the recognition thresholds of five basic tastes. Our findings suggested that there was association between the mental stress of Great East Japan Earthquake, and oral dryness and/or taste disorder.

Expression of taste-related genes in the tongue was analysed to develop a technique for genetic diagnosis of umami taste disorders.Gustducin-positive cells were observed in the samples, indicating the presence of taste cells. Gene expression of b-actin, GNAT3, T1R1 and T1R3 was detected in all seven samples tested, while that of mGluR1 was detected in four samples. Sequence analysis by NCBI Blast showed that each polymerase chain reaction product had a 99% rate of identification of its target sequence. Stimulation of the tongue with monosodium glutamate significantly upregulated the gene expression levels of T1R1 and T1R3, indicating that this method can detect alterations in umami-related gene expression. Thus, evaluation of the expression of the umami receptor genes, T1R1 and T1R3, in the tongue may be clinically useful for objective genetic diagnosis of umami taste disorders.

To clarify that minor salivary gland flow rate (MF) seem to be as a key feature of xerostomia in the elderly, MF in the lower labial mucosa was compared in xerostomia and control subjects using the iodine-starch filter paper method. Moreover, ｓtimulated whole salivary flow rates were also measured. As the ｒesults, Both labial-MF and stimulated-WF were significantly lower in xerostomia subjects than in controls. There was no correlation between labial-MF and stimulated-WF in xerostomia subjects. In xerostomia subjects compared to controls, there was a significantly larger reduction in labial-MF than in stimulated-WF. Though after 3 years the great east japn earth earthquake,in the elderly patients with xerostomia, psychological stress lasted and MF has still redused, whereas stimulates-WF has recovered to normal level. Moreover, in elderly patients with xerostomia, inadequate nutrition, appetite loss and poor health were significantly seen than in control without xerostomia.

Dental pain often evokes referred pain and/or radiating pain, and thus, to find origin of the pain is sometimes difficult. Such complicated pain is considered to be due to the vascular reaction kin the surrounding periodontal tissues evoked by the tooth pain.We demonstrated that (1) there are nerve terminals supplying the gingiva originate from parent axons which have collaterals that innervate the tooth pulp, (2) the vasodilation and the extravasation are induced in the gingiva following electrical stimulation of the nearby tooth pulp. These results indicate that tooth pain evokes neurogenic inflammation and presumably pain in the surrounding periodontal tissues.

近年、総唾液分泌量の低下を伴わない口腔乾燥症患者の存在が明らかとなった。その原因として、総唾液分泌量とは別に、口腔内に広く分布している小唾液腺の分泌量低下が口腔乾燥感を惹起している可能性が考えられるが、その実態は不明である。本研究の目的は、口腔乾燥症の罹患頻度が多いことが知られている高齢者を対象として、小唾液腺分泌機能と口腔乾燥症状との関連を明らかとすることである。当該年度は、口腔乾燥感のために東北大学病院を受診した高齢者患者を対象として、当教室で開発した、簡易に小唾液腺分泌量を測定できる“ヨウ素デンプン濾紙法”を用いて、実際の小唾液分泌量を測定した。また、小唾液腺分泌量測定と同時に、小唾液腺分泌機能と口腔乾燥との関連を明らかにするために、口腔乾燥感の自覚調査、口腔内の乾燥程度と様々な口腔乾燥併発症状（味覚障害、歯肉炎、口腔粘膜疾患、口腔カンジダ症など）の有無に関する口腔内精査、口腔細菌培養検査、総唾液分泌量測定を行った。その結果、口腔乾燥感を訴えた６５歳以上の高齢者患者に、総唾液分泌量は正常であるにもかかわらず口腔内に口腔乾燥がみられるものが確認され、それらの患者では、小唾液腺分泌量が健常高齢者よりも低下していた。また、これらの高齢者患者に、口腔乾燥の併発症状として味覚障害を示す患者がみられ、特に基本５味の中で“うま味”に特化した味覚障害を示す患者がふくまれていた。 “うま味”感受性低下を示した患者は、食欲不振やそれに伴う体重減少を示すことが多く、体調不良を訴えていた。これらの患者に、唾液腺分泌量増加作用のある漢方薬を処方した結果、小唾液腺分泌量は増加し、“うま味”感受性は正常化し、患者の食欲と体重の改善が得られた。小唾液腺分泌量低下は、口腔乾燥を招き、またその併発症状として味覚障害を惹起する場合があることが判明した。

This study was designed to determine the crosstalk between sensory and sympathetic nerves in pulpitis. The sympathetic nerve fibers were demonstrated to sprout in the inflamed pulp tissues. In the second place, the P2X4 gene expression was shown to be the most suitable noticeptive marker because of its sensitive response to pain stimuli.

Pain is one of the physical stressor for humans. In this study, we performed quantitative analysis of CRF gene transcription in hypothalamic PVN under the condition of tongue pain stress in rats. Plasma adrenocorticotropic hormone (ACTH) secretion was peak at 30 min after the capsaicin injection to tongue prior to gradually increasing of plasma corticosterone, indicating that tongue pain is stressor enough to up-regulate hypothalamo-pituitary- adrenocortical axis. CRF hnRNA 15 min after the injection was up-regulated 3 fold than that of control rats. This report is the first study which succeeded to detect CRF hnRNA by quantitative real-time PCR analysis.

1. Purpose Mastication needs cooperation of masticatory muscles, but the exact role of the each muscle for mastication is not completely clear. The purpose of this study was to evaluate the change of masticatory muscles under the unilateral masseter muscle dysfunction induced by Botulinum toxin A (BTXA) injection in rabbits. BTXA binds to presynaptic nerve endings and inhibits the release of acetylcholine, leading to temporary partial denervation and reduction of muscle fiber. 2. Materials and Methods White Japanese rabbits were injected with 10 units of BTXA into the left masseter muscle. The effect of a neuromuscular blockade of masseteric activity was evaluated with serial computer tomography (CT) scans with 0.63mm slice thickness and 0.1mm slice interval taken after 4 weeks. 3. Results The masseter muscle size of BTXA injected was reduced to about 80% against the control side. This indicated the morphologic change caused by the neuromuscular blockade. 4. Coclusion To study the exact role of each muscles for mastication, masseter mucle dysfunction was made by BTXA injection into the masseter muscle. The most suitable dose, timing and the degree of dysfunction were established. The elrctro-myography is considered to investigate the functional change. This animal model helps the understanding of the mechanism of tempolo-mandibular dysfunction.

Taste disorder in youth is possibly related to the changes in their lifestyle, such as the reverse of night and day time, population of internet, short sleep, change in eating habit and so on. However, until now, there have been few epidemiological studies regarding the relationship between taste disorder and the changes in life style in youth. We suveyed the prevalence of taste disorder in Japanese youth, and examined to clarify the causes. The subjects of this study were 153 freshmen (first grade students) of Tohoku University Graduate School of Dentistery (male n=103, female n=50), who agreed with our project, aged from 18 to 31 years old (mean=19.4). The gustatory test by means of taste disc method was carried out to them for examining their taste disorder. Gum teste was also carried out for examining their secretory function of the whole saliva. Besides, we asked them detailed questions about their systemic disease, the medication and life styles including eating habit. The results were as follows : 1)Thirty-eight (24.8%) of the 153 subjects were diagnosed with taste disorder in slight or intermediate level. 2)Only three (7.9%) of the 38 subjects with taste disorder noticed subjective sensation of their taste abnormalities. 3)No subjects had chronic systemic diseases or medication. 4)In all of the 38 subjects with taste disorder, salivary flow rates were on normal level. 5)There were no correlation of the occurrence of taste disorder in youth with feeling of stress, the reversed night and day time, daily use of internet, sleeping hour and daily use of perfume. 6)All of the subjects with taste disorder cooked their own meals. 7)The subjects who have no breakfast was much more in taste disorder group than in normal taste sensation group. 8)The frequency, in eating bean products, fish, shells and seaweed, was lower in the subjects with taste disorder than in normal subjects. 9)The frequency, in eating the convenience store's foods, vegitable, meat, snack such as potato chips and first food, was similar in the subjects with taste disorder to in normal subjects. 10)Anemic symptom and/or loss in weight were more frequent in the subjects with taste disorder than normal subjects. These results suggest that the taste disorder in youth have relevalence to the imbalance of eating habit, and the guidance of dairy diet is necessary for maintiming taste sensation, and for the good health in youth.

Taste disorder is a common symptom in elderly persons. The purpose of this study was to survey the prevalence and causes of taste disorder in elderly. The subjects of this study were 71 elderly persons (male N=19, female n=52) who agreed with our project, aged from 65 to 94 years old (mean=8O), in 4 homes for senior citizens. The gustatory test by means of taste disc method was carried out to them for examining their taste disorder. And we asked them detailed questions about their systemic disease, the medication and taste sensation. The results were as follows : 1) Twenty-six (36.6%) of the 71 subj ects were diagnosed with taste disorder in slight or intermediate. There is no gender difference. Particularly, in their nineties, taste disorder was significantly frequent. 2) Sixty-three (88.7%) of the 71 subjects had chronic systemic diseases than those without systemic diseases. 3) All of the 26 subjects with taste disorder had some chronic systemic diseases, in which gastroenteritis, cerebral infarction and diabetes, such diseases are considered to induce taste disorder, and hypertension were frequent. 4) Twenty-five (96.2%) of the 26 subjects with taste disorder underwent such medication as to cause taste disorder or xerostomia. The occurrence of taste disorder was significantly higher in elderly with medicatjon, particular medication for hypertention, than those without medication. 5) Only five (19.2%) of the 26 subjects with taste disorder noticed subjective sensation of their taste abnormalities. These results suggest that a detailed research of the systemic disease, cpmmon used medication and gustatory test are essential for the analysis of ilderly taste disorder.

(1) Histamine is a classical, but still interesting inflammatory mediator. Many people have long believed that histamine is derived from mast cells or basophils alone. However, the hidtamine forming enzyme, histidine decarboxylase (HDC), is induced in a variety of tissues in responses to gram-positive and gram-negative bacterial components and to various cytokines. HDC is induced even in mast-cell-deficient mice. The histamine newly formed via the induction of HDC is released immediately and may be involved in a variety of immune responses. We conclude that histamine can be produced via the induction of HDC by a mechanism coupled with cytokine network. We call this histamine neohistamine to distinguish it from the classical histamine derived from mast cells or basophils. Neohistamine is involved in physiological reactions, inflammation, immune responses and a variety of diseases such as pulpitis. (2) Dynamic changes in pulpal blood flow (PBF) in cat mandibular canine teeth were elicited by electrical stimulation of the distal cut end of IAN, at various levels of baseline blood flow (BBF), with blood flow monitored by Laser Doppler Flowmetry. The amplitude of the PBF vasodilator response increased as BBF was adjusted downwards (by cooling down the tooth). In contrast, the amplitude of the PBF vasoconstrictor response increased as the BBF level rose. Vasodilator responses could be transformed into vasoconstrictor responses, and vice versa, by alterations in the BBF level. No systemic blood pressure changes were elicited throughout the experiment. These results indicate that a consideration of baseline vascular tone is important whenever we discuss or calculate the size of PBF responses to various stimuli.

(1) The dental pulp is rich in both nerves and blood vessels. These nerves and vessels are closely related to each other. Particularly in the dental pulp, which is densely innervated by sensory nerve axons, antidromic vasodilatation mediated via sensory nerves easily occurs as a result of the release of vasodilator neuropeptides such as substance P.Such vasodilator responses increase intra-pulpal pressure and disturbvenous blood outflow. Consequently, the dental pulp falls into circulatory disturbance. A sympathetic vasoconstriction system mediated via alpha-adrenoceptors also exists in the dental pulp, whereas a vasodilator system mediated via beta-adrenoceptors is absent. Parasympathetic vasodilator systems are apparently lacking in the dental pulp, although many other tissues in the oro-facial region have these systems. The lack of parasympathetic vasodilatation in the dental pulp may be considered a characteristic feature of the regulation of pulpal blood flow ; it may be beneficial for the dental pulp, with its enclosed and low-compliance system, to avoid circulatory disturbances. (2) We designed a new method for the assessment of human tooth pulp vitality by monitoringpulpal blood flow. Pulpal blood flow is measured by means of a transmitted laser light, which is transmitted from the buccal surface of the tooth and received from the palatalsurface. This apparatus does not assess sensitivity of the dental pulp, but pulp vitality itself. Further, it causes no pain to the patient and is therefore objective. This new system is expected to be useful in the diagnosis of oral diseases.

The purpose of this study was to determine if use of transmitted laser light might enable a better assessment of human pulpal vitality than does back-scattered light (LDF : laser Doppler flowmetry). The experiments were carried out on 10 upper central incisors in 6 subjects aged 23-28 years ; 5 of the teeth were vital with no restoration, and 5 were non-vital. For use with transmitted laser light, the fibers within the probe of a conventional LDF apparatus were used, one for transmitting light onto the buccal surface, the other for receiving it at the palatal surface of the same tooth. For LDF,the probe was fixed at the buccal surface. Blood flow (BF) was measured at three different locations on each experimental tooth : the incisal third, the center and the cervical third of the tooth crown. In non-vital teeth, (1) output signals with transmitted laser light all registered zero, and no oscillation could be seen in recordings from any location on the tooth, but (2) LDF signals were above zero, there were regular oscillations related to heart rate, and passive increases in BF (corresponding to BP increases) were recorded from both the center and cervical third of the tooth, indicating that LDF registered blood flow of non-pulpal origin. In vital teeth, LDF signals were significantly higher than in non-vital teeth at each location on the tooth. At the central site on vital teeth, the output signals for transmitted laser light were about twice those seen with LDF,and passive BF changes corresponding to BP increases were more clearly observed. These results indicate that transmitted laser light would be useful for the assessment of tooth pulp vitality both because the blood flow signals do not include flow of non-pulpal origin, and because its output signals and response to BF changes are larger and so can be more easily monitored.

The purpose of this study was to investigate sympathetic and parasympathetic regulation in the periodontal ligament blood flow (PLBF). Dynamic changes in PLBF of the mandibular canine teeth in 8 young anesthetized cats (2-4 kg) were continuously monitored using laser Doppler flowmetry. The systemic blood pressure (BP) was monitored during the experiment. Three different patterns of response, (i.e., the increase, the biophasic and the decrease response in PLBF) were observed by electrical stimulation of the distal end of the cut inferior alveolar nerve (LAN). The PLBF decrease following stimulation of the LAN was completely abolished by pretreatment with adrenoceptor alpha-antagonist (phentolamine). Electrical stimulation of the distal end of the cut cervical sympathetic trunks led to a PLBF decrease without an alteration of BP.After the cut of IAN,this PLBF decrease response was completely abolished. The peripheral cut ends of the facial and glossopharyngeal nerve roots were electrically stimulated intracranially. The cervical sympathetic trunks were sectioned bilaterally prior to stimulation in order to eliminate sympathetic effects. The PLBF increases were caused following such stimulations, without alterations of BP.These vasodilator responses in periodontal ligament were reduced by pretreatment with ganglion blockade (hexamethonium). These results indicate that the sympathetic vasoconstic and/or the parasympathetic vasodilator machanisms exist in the cat periodontal ligament.
血流量測定による感覚麻痺の診断法の確立と歯科臨床応用

歯科臨床において、局所麻酔による神経損傷、嚢胞等の神経圧迫で、歯や歯周組織に、感覚麻痺を生じることがある。しかし現在、麻痺の評価は麻痺領域の判定に留まり、神経自体の機能や麻痺の予後の判定は殆どなされていない。近年、感覚神経は情報を中枢へ伝達するほかに、末梢の血管拡張を起こすことが報告されている。そこで本研究では、痛み刺激によって惹起される感覚神経による血管拡張反応をパラメータとして、(1)感覚麻痺の程度と(2)麻痺の予後を定量的に測定する方法を確立することを目的とした。当該年度は、動物実験によって口腔内の痛みが感覚神経由来に血管拡張を発現する領域を確認し、さらにヒトにおいて、痛み刺激の測定を行うために、痛みの測定基準を設定し、歯科疾患による痛みの臨床応用を試みた。その結果、 A.動物実験によって上顎犬歯の侵害性電気刺激により刺激側と同側の上下顎歯肉、頬粘膜、口蓋、舌および下唇などの種々の組織に、感覚神経由来の血管拡張反応が観察された。 B.ヒトボランティアによって 1)歯髄の痛みをvisual analogue scale(VAS)を用いて測定したところ、同一人の痛みの測定には有効であったが、異なる人同士の間で痛みの比較は難しいことが判明した。そこで、(1)VASの最大値を"想像した最強の痛み"とし、(2)"これまでに経験した最強の痛み"をVAS上に比較基準としてのせることにより、安定したVAS値が得られるようになった。 2)上記方法に従って、舌の痛みを主訴とした患者の痛みの定量を行った結果、疾患の診断と治療効果の判定にVASは非常に有効であった。

歯髄は感覚神経を介して歯に及ぶ侵襲をいち早く危険信号として知らせる機能を有している。近年、感覚神経と血流との機能的相関が論じられており、感覚神経は、痛みなどの情報を中枢に伝える求心性機能の他に、侵害性感覚神経の興奮が支配領域の末梢血管の拡張を引き起こす遠心性機能を有することが皮膚組織などで明らかにされている。感覚神経が豊富な歯髄では、痛みが血管拡張を引き起こし歯髄炎のメカニズムに深く関与している可能性が考えられる。本研究では、先ず歯髄の感覚神経の遠心性機能を検討するために、実験動物としてネコを用い、下顎歯髄の感覚を支配している下歯槽神経を末梢方向に電気的に刺激し、歯髄の血流の変化を観察した。その結果、刺激強度に応じた血流の著明な増加反応が観察され、この反応は、歯肉と比較して増加量が大きくさらに長く持続する特徴的な反応であった。次に、歯髄と歯肉との間の感覚神経軸索の枝別れを介する軸索反射性血管拡張が生じるかどうかを検討するために、近接する歯肉を刺激したときの歯髄血流反応を観察した。この結果、電気的、機械的、温度的および化学的ないずれの刺激に対しても血流増加反応が生じる部位が歯肉の一部に存在した。また、上記の感覚神経を介した歯髄の血管拡張を引き起こす神経線維は、生理学的に侵害性ポリモダールC線維であるこが示唆された。これら感覚神経を介した血流増加がヒトにおいても生じるかどうかについて現在検討を進めている。

The present study was undertaken to eveluate the quantitative measurement of human dental pain using a visual analogue scale (VAS). Pulpal pain, elicited by electrical stimulation in sixty students (ranging 23 to 29 years old), was measured. And, clinical pain in a patient suffered from acute suppurative pulpitis was measured. The following results were obtained : 1. Toothache was the most frequent severe pain rather than pain of injury, stomachache, lumbago, pain of burn, or headache. 2. The VAS score of pulpal pain elicited by electrical stimulation is higher in incisor areas than molar areas. 3. The pain measured with VAS showed good reproducibility. 4. In proportion to the increase of the accelerated velocity in electrical stimulation, VAS score increased. 5. Before and after treatment, clinical pulpal pain in acute pulpitis was accurately observed by VAS. These results suggest that the VAS method is available for assessing dental pain and its clinical use.

In order to study the interrelationship between the dental pulp and the periodontal tissues, acute response of the periodontal ligament (PL) blood flow to an application of histamine solution to the dental pulp and the gingival sulcus was investigated using laser Doppler velocimetry. The results were as follows: 1. When histamine solution was directly applied to PL, PL blood flow increased immediately after its application. This increase disappeared after washing out the histamine. 2. PL blood flow also increased after applying the histamine solution to the exposed pulp surface and the gingival sulcus, respectively. These increases did not disappear after washing out the histamine in the applied area. These results indicate that a measurement of tissue blood flow following an application of histamine solution as a tracer is methodologically useful to evaluate an inflammation spread between the tissues.
三叉神経刺激による歯根膜血管拡張反応について

歯根膜は、歯髄および歯周組織における炎症のpathwayとして臨床に深く関わりを持つ組織である。しかしながら歯根膜は歯と歯槽骨の間に挟まれ、容積が小さいためにこの組織の血流動態を検索することは因難とされていた。そこで本研究では、まず歯根膜における血流動態の測定法を検討し、次にこの測定法を応用して歯根膜における血管拡張反応について検討した。結果: (1)歯根膜血流の測定にレ-ザ-ドプラ-血流計の応用を試み、ネコを用いて歯肉を剥離し歯槽骨を削除して歯根膜を露出させ、血流を持続的にモニタ-する方法を確立した。 (2)歯を押したときの歯根膜血流の反応を検討するために、キャリブレ-ションされた一定の荷重を種々の方向から歯に加えた。この結果、歯根膜血流の瞬時の反応が明確に観察され、歯根膜血流を変化させるための荷重の閾値および変化量の最大値などが明らかになった。さらに、歯根膜の外力に対する反応は、歯頚部と根尖部で異なり、また荷重の方向によっても異なることが明瞭に観察された。 (3)三叉神経を逆伝導性に刺激したときの歯根膜血流の変化を検討するために、中枢端を切断した上歯槽神経を逆伝導性に電気刺激した。この結果、歯根膜血流は、減少、増加およびそれらの複合という3つのパタ-ンとして観察された。この結果から、歯根膜血流は交感神経による血管収縮、感覚神経の逆伝導による血管拡張により支配されていることが示唆された。以上の結果から、レ-ザ-ドプラ-血流計は、歯根膜の血流動態の観察に有用であり、今後さらに歯根膜血流の神経調節機構について詳細に検討していきたいと考えている。
