    Introduction Cleft lip and/or palate (CL/P), the most prevalent congenital anomaly, has been associated with higher rates of child maltreatment. In particular, the presence of cleft lip has more of an impact on external appearance and may increase the risks of negative health outcomes such as parental postpartum depression; however, this concept remains controversial. Item #10 of the Edinburgh Postpartum Depression Scale is the assessment of parental self-harm ideation, and its presence in postpartum mothers merits risk assessments as an emergent issue that may affect the health of both mothers and infants. This study focused on the impact of CL/P on maternal self-harm ideation. Methods Of 100,300 live births from a nationwide birth cohort in Japan, 238 mothers of infants with CL/P [186 children born with cleft lip (CL ± P) and 52 born with isolated cleft palate (CP)] were included in the analyses. The prospective association of children with CL/P and maternal self-harm ideation, which were acquired using item #10 in the Edinburgh Postpartum Depression Scale at 1 and 6 months postpartum, was examined using binomial logistic regression analyses after multiple imputations and with adjustments for several maternal (age at delivery, smoking habit, and alcohol intake) and child-related (sex and prevalence of other congenital diseases) variables. Results The prevalence of self-harm ideation in 238 mothers of infants with CL/P at 1 and 6 months were 14.7% (35/238) and 18.8% (45/238) [8.2% (8,185/100,062) and 12.9% (12,875/100,062) in the control group], respectively. The odds ratio (95% confidence interval) for maternal self-harm ideation increased with CL/P prevalence [1.80 (1.22–2.65) and 1.47 (0.98–2.18)] at 1 and 6 months of age, respectively. After stratified by the prevalence of cleft lip, we found significant differences in the CL ± P group but not in the CP group. Furthermore, persistent self-harming ideation was associated with a higher risk in the CL ± P group [2.36 (1.43–3.89)]. Conclusion CL/P, particularly cleft lip, which is more noticeable externally, was associated with an increased prevalence of maternal self-harm ideation. The findings in this study indicate some potential benefits of increasing support for mothers who have infants with CL/P.

    Bruxism, the involuntary activity of masticatory muscles, is common among individuals with autism spectrum disorders (ASD). Although bruxism is bidirectionally associated with sleep issues, whether an infant's sleep duration contributes to the development of bruxism remains unknown. In this study, a dataset (n = 83,720) obtained from the Japan Environment and Children's Study, a nationwide birth cohort study, was subjected to multiple imputations using logistic regression analysis with adjustments for several maternal and child-related variables. The aim of this study was to assess whether shorter sleep duration in the neonatal period additively affected the high prevalence of parent-reported bruxism (PRB) among children with ASD. The prevalences of ASD and PRB in the participants were 1.2% and 7.2%, respectively, and the odds ratio of the increased risk of PRB prevalence in individuals with ASD (95% confidence interval) was 1.59 (1.31-1.94) after covariate adjustments. Importantly, shorter sleep duration in the neonatal period (at one month of age) was significantly associated with an increased risk of PRB prevalence in individuals with ASD. The increased occurrence of bruxism, known to be highly prevalent among children with ASD, is associated with shorter sleep duration, particularly in the neonatal stage. Based on our results, a better understanding of the development of bruxism in individuals with ASD would provide valuable information for the prevention of oral diseases.

    OBJECTIVES: Pain associated with orthodontic tooth movement reportedly reduces periodontal ligament tactile sensation. However, the mechanism associated with the central nervous system remains unclear. This study was conducted by measuring somatosensory evoked magnetic fields (SEFs) during mechanical stimulation of teeth as they were being moved by separator elastics. Findings clarified the effects of pain on periodontal ligament tactile sensation during orthodontic tooth movement. MATERIALS AND METHODS: Using magnetoencephalography, SEFs were measured during the application of mechanical stimuli to the mandibular right first molars of 23 right-handed healthy participants (0 h). Separator elastics were subsequently inserted into the mesial and distal interdental portions of the mandibular right first molars. The same mechanical stimuli were applied again 24 h later while the SEFs were measured (24 h). After each SEF measurements, pain was also evaluated using the Visual Analog Scale (VAS). RESULTS: The VAS values were significantly higher at 24 h than at 0 h (p < 0.05). No significant difference in the peak latencies was found between those obtained at 0 h and 24 h, but the intensities around 40.0 ms in the contralateral hemisphere were significantly lower at 24 h than at 0 h (p < 0.01). CONCLUSIONS: Pain associated with orthodontic tooth movement might suppress periodontal ligament tactile sensation in the primary somatosensory cortex. CLINICAL RELEVANCE: Pain associated with orthodontic tooth movement might affect periodontal ligament sensation, consequently causing discomfort during occlusion.

    Otitis media, one of the most common childhood diseases, is characterized by inflammation or infection of the middle ear. Due to their ease of access, daily probiotics are recommended for the prevention of early childhood otitis media. This study aimed to assess the impact of probiotics on the incidence of otitis media using a dataset (n = 95,380) from the Japan Environment and Children's Study, a nationwide birth cohort study. After multiple imputations, the association between the incidence of otitis media in early childhood and the daily frequency of yogurt intake in children and mothers was examined using a generalized linear model after adjusting for several confounders. Repeated incidence of otitis media during the 2 years after birth was found in 14,874 participants (15.6%). Based on participants with the lowest frequency of yogurt intake ("almost never") as the reference group, risk ratios for otitis media incidence decreased with higher frequencies of yogurt intake in children at one year of age, but also independently in mothers during pregnancy. The lowest risk ratio (95% confidence interval) for otitis media incidence at six months of age was observed with the most frequent yogurt intake (once/day or more) (0.54 [0.46-0.63]). Additionally, although a similar association was observed in the subgroup of those with cleft lip and/or palate (CL/P), a high-risk group for severe recurrent otitis media, no statistical significance was observed. Thus, increased regular yogurt intake in both children and mothers was associated with a decrease of otitis media during early childhood.

    Abstract Alveolar bone grafting (ABG) is broadly performed for cleft lip and palate patients. The rate of canine impaction post-ABG is much higher than the prevalence of canine impaction in non-cleft patients. This pilot study was designed to investigate factors involved in canine eruption failure after ABG and to predict the possibility of canine impaction in unilateral cleft lip and palate (UCLP) patients. This retrospective observational study examined 45 patients with UCLP (mean age 7.9 years) classified into an impacted group (n = 9) and a spontaneously erupted group (n = 36). From 3D images, we measured lateral incisor presence or absence, lateral incisor position on the cleft side, canine position, movement change, cleft volume, and canine and first premolar overlap-area. Multivariate logistic regression analysis using independent variables indicated significant differences in results, selecting highly relevant items. Multivariate analysis indicated a significant association between the overlap-area between the canine and the first premolar at pre-ABG (p = 0.038) and the distance between the cleft side cusp tips of canine and the lateral cleft margin of pre-ABG (p = 0.005). Results suggest that canine impaction is predictable at an early stage in pre-ABG and show the possibility of comprehensive diagnosis of canine impaction using computed tomography.

    Background Cleft lip is the most common congenital anomaly worldwide. Nevertheless, lip somatosensory characteristics of patients with cleft lip after cheiloplasty have not yet been determined. The present study used magnetoencephalography to objectively evaluate the lip sensation in patients with unilateral cleft lip to establish a new objective evaluation method. Methods Participants were 15 patients with unilateral cleft lip after cheiloplasty (UCL group), and 30 healthy young subjects (control group). Five points of the upper and lower lips were stimulated electrically to measure somatosensory evoked magnetic fields (SEFs). The sources of the magnetic fields were modeled as single equivalent current dipoles (ECDs). ECDs located on the central sulcus by superimposition on magnetic resonance images were analyzed. Latency and intensity at 50–75 ms (cP60m) observed in the UCL group were compared with those in the control group. Thresholds of tactile stimuli in both groups were obtained using Semmes–Weinstein monofilaments for subjective sensory evaluation. Results No significant difference was found in the intensity of the cP60m or subjective evaluation between the groups. However, the latency of the cP60m was significantly longer in the upper lip of the UCL group than in the control group. Conclusions SEFs showed a difference in lip sensation between the UCL group and the control group, suggesting that longer latency might be caused by the effects of surgical scarring on the neurotransmission pathway. These results suggest SEFs as useful for the objective evaluation of lip sensations. This study might improve future surgical procedures and lip functions of patients with cleft lip.

    Introduction Bone-anchored maxillary protraction (BAMP) was devised recently as a method of direct maxillary protraction using anchor plates implanted in the maxilla and mandible without involving the teeth. Although several reports have described orthognathic effects of BAMP on patients with cleft lip and palate (CLP) with skeletal crossbite, none has described a study of Japanese patients with CLP or of BAMP treatment effects on speech in patients with CLP. This study, by performing BAMP treatment, and by evaluating speech function and skeletal and soft tissues, is intended to clarify BAMP efficacy and safety for patients with unilateral CLP (UCLP) who have skeletal crossbite. Methods and analysis This single-arm, open-label, non-randomised prospective study examines 20 patients with UCLP with skeletal crossbite (Wits appraisal ≤−5.0 mm). These 10–15 year-old participants had already undergone cheiloplasty, palatoplasty and bone grafting. The anchor plates are implanted in the zygomatic process in the maxilla and in the anterior part of the mandible. Two weeks after anchor plate implantation, maxillary protraction is started using elastics. Protraction is performed at 150 g per side at the start of protraction, 200 g per side from 1 month after the start of protraction and 250 g per side from 3 months after the start of protraction. The treatment period will be approximately 1½ years. Pretreatment and post-treatment, cephalometric analysis, speech evaluation, nasopharyngeal closure function evaluation and facial soft tissue evaluation will be performed to ascertain the effects of BAMP on patients with UCLP.     The exact profiles of the clinical symptoms related to the SARS-CoV-2 Omicron variant (B.1.1.529) remain largely uncertain. Therefore, this study aimed to clarify the clinical manifestations of infection with this variant. We enrolled individuals who were tested by quantitative nasopharyngeal swab reverse transcription-polymerase chain reaction (RT-PCR) test at a large screening center in a city of Japan during the B.1.1.529 Omicron variant wave between January and May 2022, after contact with COVID-19 patients. Swab tests were planned to be performed approximately 4-5 days after contact. The presence of COVID-19-related symptoms was assessed at the swab test site. Among the 2,507 enrolled individuals, 943 (37.6%) were RT-PCR test-positive and 1,564 (62.4%) were test-negative. Among the 943 PCR test-positive participants, the prevalence of the symptoms was as follows: 47.3% with cough, 32.9% with sore throat, 18.4% with fatigability, 12.7% with fever of ≥ 37.5℃, 9.9% with dyspnea, 2.1% with dysosmia, and 1.4% with dysgeusia. The prevalence of cough, sore throat, dyspnea, and fatigability was higher among adults aged ≥ 18 years than among children and adolescents. The prevalence of dysosmia and dysgeusia remarkably decreased during the Omicron wave (1-3%) compared to during the pre-Omicron variant waves (15-25%). In summary, common COVID-19-related symptoms during the Omicron variant wave included cough and sore throat, followed by fatigability, fever, and dyspnea. The prevalence of most of these symptoms was higher in adults than in non-adults. The prevalence of dysosmia and dysgeusia remarkably decreased with the Omicron variant than with pre-Omicron variants.

    Abstract The administration of a third booster dose of messenger ribonucleic acid (mRNA) vaccines against coronavirus disease 2019 (COVID-19) has progressed worldwide. Since January 2022, Japan has faced a nationwide outbreak caused by the Omicron variant, which occurred simultaneously with the progression of mass vaccination with the third booster dose. Therefore, this study evaluated the effectiveness of the third dose of vaccine by reverse transcription-polymerase chain reaction (RT-PCR) test using nasopharyngeal swab samples from adults aged ≥ 18 years tested after having close contact with COVID-19 cases between January and May 2022. Participants who completed only one dose were excluded from the study. Among the 928 enrolled participants, 139 had never been vaccinated, 609 had completed two doses, 180 had completed three doses before the swab test, and the overall RT-PCR test positivity rate in each group was 48.9%, 46.0%, and 32.2%, respectively. The vaccine effectiveness of the third dose to prevent infection after close contact was approximately 40% (95% confidence interval: 20–60%), which was the highest at 10–70 days after receiving the third dose. In conclusion, the effectiveness of the three-dose mRNA COVID-19 vaccine after close contact during the Omicron outbreak is approximately 40%.

    OBJECTIVES: Daily toothbrushing behaviour is very effective for the prevention of early childhood dental caries (ECC), but is mostly dependent upon parental management. Intrapersonal association between lower toothbrushing frequency and depression is well known; however, the impact of maternal postpartum depression on child toothbrushing behaviour within the mother-child dyad remains unknown. The aim of this study was to determine the association between the prevalence of maternal postpartum depression and lower toothbrushing frequency in children aged two years. METHODS: A secondary analysis of a data set from the Japan Environment and Children's Study was performed. A total of 104 062 fetuses were enrolled after obtaining informed written parental consent, in which 84 533 mother-infant pairs were included after applying exclusion criteria. The Edinburgh Postpartum Depression Scale (EPDS) was used to evaluate maternal postpartum depression (a total score of ≥9 in EPDS) at one and six months postpartum. Indeed, the participants were classified based on the persistence of postpartum depression: 'Resilient' (no prevalence); 'Improving' (prevalence only at one month postpartum); 'Emergent' (prevalence only at six months postpartum); and 'Chronic' (prevalence at both time points). The association between postpartum depression and a toothbrushing frequency in children (the reference group: more than once per day, the low group: once per day, and the very-low group: less than once per day) was examined using Poisson regression models with adjustments for maternal and child characteristics after multiple imputations for missing data. RESULTS: The prevalence of maternal postpartum depression at one and six months postpartum was 13.9% and 11.4%, respectively; the proportions of each persistence group were 81.1% ('Resilient'), 7.5% ('Improving'), 5.0% ('Emergent') and 6.4% ('Chronic'). Concerning children's toothbrushing frequency, 51.6% and 0.5% of participants self-reported frequencies of once per day and less than once per day, respectively. The association of maternal postpartum depression with a lower toothbrushing frequency in children consistently had higher relative risks (RRs). However, these associations were weakened when adjusting for whether the child could self-perform toothbrushing or whether this was done under parental supervision. A key result is that participants with persistent postpartum depression at both one and six month(s) postpartum showed the highest adjusted RRs (95% CI) for lower toothbrushing frequency in children (1.08 [1.04-1.12] with a decrease in children's toothbrushing frequency). CONCLUSION: Maternal mental health provides valuable screening information for children with lower toothbrushing frequency for the purpose of preventing ECC.

    The coronavirus disease 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), remained a major global health concern in 2021. To suppress the spread of infection, mass vaccinations have been performed across countries worldwide. In Japan, vaccinations of the first and second doses for most of the nation were performed during the nationwide outbreak of the B.1.617.2 (Delta) variant with the L452R spike protein mutation, and the effectiveness of the vaccinations to suppress the spread of COVID-19 among the people in Japan remains uncertain. In this study, adults aged ≥18 years, who were in contact with patients with COVID-19 and underwent nasopharyngeal swab reverse transcription-polymerase chain reaction (RT-PCR) tests during August and September 2021 at a mass screening test center in Japan, were enrolled. In this period, more than 95% of the COVID-19 infections were reportedly caused by the Delta variant. As a result, a total of 784 adults with recent contact history, including 231 (29.5%) RT-PCR test-positive cases, were enrolled. The test positivity rate was lower in individuals who had been vaccinated twice than in unvaccinated individuals (12.5% vs. 39.0%, p < 0.0001), with the risk ratio of 0.32 (95% confidence interval 0.23-0.46). The vaccine effectiveness was the highest between 7-90 days after the second vaccine dose. In conclusion, two doses of mRNA COVID-19 vaccines effectively suppressed transmission in Japan during the nationwide pandemic of the Delta variant, estimated to have prevented 50-80% of the infection.

    The periodontal ligaments are very important sensory organ for our daily life such as perception of food size or hardness, determination of jaw position, and adjustment of masticatory strength. The sensory properties of the periodontal ligament, especially those of the maxillary and mandibular molars, have not yet been fully investigated. Somatosensory evoked magnetic fields (SEFs) can be measured and evaluated for latency and intensity to determine the sensory transmission characteristics of each body parts. However, previous reports on SEFs in the oral region have only reported differences in upper and lower gingival and lip sensations. In this study, the aim was to clarify these sensory characteristics by measuring SEFs during mechanical stimulation of the periodontal ligament in the maxillary and mandibular first molars. Somatosensory evoked magnetic fields were measured in the contralateral hemispheres of 33 healthy volunteers. Mechanical stimulation of the maxillary and mandibular right first molars, and the left wrist was performed with a specific handmade tool. The first peak latency for the mandibular first molars was 41.7 ± 5.70 ms (mean ± SD), significantly shorter than that for the maxillary first molars at 47.7 ± 7.36 ms. The peak intensity for the mandibular first molars was 13.9 ± 6.06 nAm, significantly larger than that for the maxillary first molars at 7.63 ± 3.55 nAm. The locations in the contralateral hemispheres showed no significant difference between the maxillary first molars and mandibular first molars. These locations were more anteroinferior and exterior than that of the wrist, as suggested by the brain homunculus. Neural signals from the mandibular periodontal ligaments pass faster and more intensely to the central nervous system than those from the maxillary periodontal ligaments, and may preferentially participate in adjustment of the occlusal force and the occlusal position.

    Cleft lip and/or palate (CL/P), the most prevalent congenital anomaly, is understood to negatively affect a wide range of child development. Since the concept remains controversial, because most published work is from cross-sectional studies, we examined the neurodevelopmental trajectories in participants with CL/P through a longitudinal comparison with the general population during early childhood using data from a nationwide birth cohort study in Japan. The linear mixed models for each domain of the Ages and Stages Questionnaire, third edition (ASQ-3), were used to detect differences in standardised mean scores between groups. The ASQ-3 is a general neurodevelopmental screening tool comprising communication, gross motor, fine motor, problem-solving, and personal-social domains. Participants' neurodevelopment was determined semi-annually from 6 to 36 months of age. The trajectories of standardised mean scores in each domain showed several significant differences between the control and CL/P groups, with the maximum difference at 24 months of age in the communication domain. Indeed, CL/P was associated with significantly lower scores in the communication (coefficient: -3.31, 95% CI: -5.09 to -1.14), problem-solving (coefficient: -3.13, 95% CI: -5.07 to -1.18), and personal-social domains (coefficient: -1.99, 95% CI: -3.87 to -0.11). Trajectories of ASQ-3 scores suggest neurodevelopmental delays in children with CL/P.

    The Tohoku University Hospital has been a clinical and research facility for all the related departments of Tohoku University. Medical-dental and interprofessional collaboration has resulted in special treatment teams, made up of members of departments such as the center for head and neck cancer, the center for dysphagia, and the cleft lip and palate center. Those treatment teams held conferences, case study meetings, reading sessions, and in-hospital seminars. The purpose of this study was to evaluate the outcomes of various medical-dental and the interprofessional collaboration at Tohoku University Hospital and training program to equip hospital dentists in higher medical institutions. The attainment targets are the acquisition of basic medical skills and knowledge under the guidance of supervising doctors. As a result, the hospital dentists could acquire their own specialized knowledge and skills certificated by each academic society. The smooth team treatment has been achieved, and the number of cases discussed by cancer boards and center for dysphagia has increased year by year due to the efficiency of their clinical pathways. On the dental care side as well, the wearing rates of maxillofacial prosthetic devices such as maxillofacial prostheses and palatal augmentation prostheses (PAP) have improved, which have contributed to improving patient's stomatognathic function. Tohoku University Hospital has been practicing collaboration between medical and dental professionals and it has produced mutual benefits. Our interprofessional training system based on the medical-dental collaboration could develop professionals who have acquired cross-disciplinary knowledge and skills from experienced doctors.

    The coronavirus disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is currently the world's largest public health concern. This study evaluated COVID-19 transmission risks in people in group living environments. A total of 4550 individuals with a history of recent contact with patients at different places (dormitory/home/outside the residences) and levels (close/lower-risk) were tested for SARS-CoV-2 viral RNA using a nasopharyngeal swab test between July 2020 and May 2021. The test-positive rate was highest in individuals who had contact in dormitories (27.5%), but the rates were largely different between dormitories with different infrastructural or lifestyle features and infection control measures among residents. With appropriate infection control measures, the secondary transmission risk in dormitories was adequately suppressed. The household transmission rate (12.6%) was as high as that of close contact outside the residences (11.3%) and accounted for > 60% of the current rate of COVID-19 transmission among non-adults. Household transmission rates synchronized to local epidemics with changed local capacity of quarantining infectious patients. In conclusion, a group living environment is a significant risk factor of secondary transmission. Appropriate infection control measures and quarantine of infectious residents will decrease the risk of secondary transmission in group living environments.

    BACKGROUND: Daily toothbrushing prevents early childhood caries, but reinforcement depends on facilitative parenting behaviours. Mother-to-infant bonding, the maternal affection towards the infant, is an environmental factor that strongly influences parenting. AIM: This study examined the association between maternal bonding and children's daily toothbrushing frequency. DESIGN: The sample consisted of 83 954 mother-infant pairs at two years postpartum, derived from the initial sample of JECS (cohort study), which included 104 062 foetuses. Maternal bonding disorders were assessed using the Mother-to-Infant Bonding Scale (MIBS). After multiple imputation for missing data, a multinomial logistic regression analysis was conducted with adjustments for several maternal (eg, age at delivery) and child-related (eg, self-performed toothbrushing) variables. RESULTS: The odds ratio (95% confidence interval) for the association of maternal bonding disorders with the low (once per day) and the very low child toothbrushing frequency (<1 per day) was 1.12 (1.07-1.17) and 1.23 (0.91-1.66), respectively, after covariate adjustments. Furthermore, the univariate general linear model showed that the mean MIBS scores significantly decreased as the daily child toothbrushing frequency increased. CONCLUSIONS: The prevalence of maternal bonding disorders at one year postpartum was prospectively associated with a lower frequency of child toothbrushing at two years of age.

    Bisphosphonates (BPs) are classified into two groups, according to their side chain structures, as nitrogen-containing BPs (NBPs) and non-nitrogen-containing BPs (non-NBPs). All examined NBPs (pamidronate, alendronate, incadronate, risedronate, zoledronate) stimulated lipopolysaccharide (LPS)-induced PGE2 and NO production by upregulating COX-2 and iNOS mRNA expression, whereas non-NBPs (etidronate, clodronate, tiludronate) suppressed PGE2 and NO production, by downregulating gene expression. Additionally, [4-(methylthio) phenylthio] methane bisphosphonate (MPMBP), a novel non-NBP with an antioxidant methylthio phenylthio group in its side chain, exhibited the most potent anti-inflammatory activity among non-NBPs. Furthermore, results of immunohistochemistry showed that the nuclear translocation of NF-κB/p65 and tyrosine nitration of cytoplasmic protein were stimulated by zoledronate, while MPMBP inhibited these phenomena, by acting as a superoxide anion (O2-) scavenger. These findings indicate that MPMBP can act as an efficacious agent that causes fewer adverse effects in patients with inflammatory bone diseases, including periodontitis and rheumatoid arthritis.

    口唇口蓋裂とは様々な遺伝的・環境的要因により口唇や顎堤、口蓋に裂を生じる先天性疾患である。出生後、口唇形成術・口蓋形成術によって裂は閉鎖されるが、術後の瘢痕によって上顎骨の成長抑制を生じることが多い。多くの医療施設では乳歯列期~混合歯列期に上顎前方牽引装置を用いて骨格的不調和を改善する治療が行われるが、その効果は患者の協力度に依存し、副作用も生ずるため、成長期における新たな治療法の開発が求められている。近年、上下顎骨に埋入したアンカープレートと顎間ゴムを用いて上顎骨を前方に牽引する方法が考案され、BAMPと呼ばれるようになった。この装置は患者の協力も得られやすく、歯性に生じる副作用もほとんど伴わずに骨格的不調和を改善することが可能である。本研究の目的は、口唇口蓋裂を有する骨格性反対咬合の患者に対するBAMPの効果と副作用について明らかにすることである。 本研究は、前年度末に認定臨床研究審査委員会(国立大学法人東北大学 東北臨床研究審査委員会;東北厚生局)から承認を受けている。2021年6月には東北大学病院から校費負担患者制度の利用についても承認を得ることができ、本臨床試験を開始するための準備が整った。直後から被験者の候補となる患者への説明を開始し、2021年度中に5名の患者から、研究への参加の同意が得られた。内4名はBAMP群として登録し、残り1名はコントロール群として登録した。BAMP群の患者にはアンカープレートを左右の上顎骨頬骨下稜部および下顎骨体部前方に埋入した。アンカープレート埋入2週間後から、上顎骨の前方牽引を開始している。4名中3名の患者で一部のアンカープレートの動揺が生じ、再埋入が必要となっており、対策が必要と考えている。

    破骨細胞分化過程において、病的骨吸収と生理的骨リモデリングのメカニズムには違いがあることが報告されている。また、骨のリモデリングにはT細胞が重要な役割を果たしていることが知られており、中でもγδT細胞は多様なサイトカインを産生し骨吸収と骨形成をコントロールしている。本研究の目的は、病的骨吸収において、骨吸収と骨形成の両方をコントロールするγδT細胞の関与の解明と生理的骨吸収を阻害することなく病的骨吸収のみをターゲットにした分子標的薬を用いた骨破壊性疾患への臨床応用の可能性を検討することである。当該年度は以下の内容について研究を実施した。1.野生型マウスから回収したγδT細胞をセルソーターにて受容体サブセットごとに分離を行なった。それらサブセットが産生するサイトカインをPCR法とELISA法にて確認をした。また破骨細胞前駆細胞との共培養により、γδT細胞のサブセットの違いによる影響が異なることを確認した。2.野生型マウス由来の破骨細胞前駆細胞とγδT細胞の共培養を行い、in vitroにおける認識抗原の検索を実施している。3.認識抗原の同定を現在行っているが、既に以前同定済みのγδT細胞のサブセット認識抗原に対するモノクローナル抗体は現在製作中であり、今後抗体機能を確認予定である。4.マウスモデルを使用して、γδT細胞が産生するサイトカインの病的骨吸収と生理的骨リモデリングに対する影響の検討を開始した。今後はさらなる詳細な検討及び、研究協力者と共にγδT細胞認識抗原に対するモノクローナル抗体の作製及び病的骨吸収と生理的骨リモデリングのメカニズム解明を進めていく予定である。

    It is still not well-known about the features and mechanisms of pain caused by orthodontic tooth movement. The purpose of this study was to objectively evaluate the pain associated with orthodontic tooth movement by measuring somatosensory evoked magnetic fields (SEFs).The results showed that orthodontic force application to mandibular first molar didi not affect the first peak latency, whereas significantly reduced the peak intensity immediately after 24 hours of the force application.Thus, it was suggested that pain signals induced by the mechanical stimulation to periodontal ligament might be decreased in the central nervous system.

    It is reported that subsets of αβ T cell receptor (TCR) T cells affect bone remodeling by the production of osteoclastogenesis up- or down-regulatory factors. On the other hand, possible enrolment of γδ TCR T cells which consist only 1-5% of whole T cell population in bone remodeling process remains unclear. The present study investigated the possible regulatory roles of γδ T cells in the osteoclastogenesis. γδ T cells regulate RANKL-induced osteoclastogenesis in vivo and in vitro. This regulation is not required cell-cell contact but activated γδ T cells by proteins produced from osteoclasts modulate osteoclastogenesis. It was suggested that controlling the antigens recognized by γδ T cells may be novel option for patients with metabolic bone diseases.

    It is known that periodontal tissue is activated by vibrational stimulation, but the mechanism is unknown. In this study, mouse osteoblast-like MC3T3-E1 cells were cultured and subjected to vibrational stimulation. Vibrational stimulation induced osteoblast differentiation and the upregulation of osteogenic gene expression, and the development of actin stress fibers in the cells was enhanced. These effects varied with vibration parameters: frequency and particularly, acceleration. The effects of vibrational stimulation in vivo were also analyzed by applying vibrational stimulation to teeth and periodontal tissue in model rats. Time-dependent changes in bone and tissue in the anesthetized animals were analyzed. Micro-computed tomography and ultrasound revealed quantitative effects.

    It is still not well-known about the features and mechanisms of pain caused by the orthodontic tooth movement. The purposes of this study are to characterize the pain associated with orthodontic tooth movement objectively by measuring somatosensory evoked magnetic fields. The result of the study suggested that the transmission speed of twenty-four hours after the orthodontic tooth movement was faster than that of non-orthodontic tooth movement in a part of subjects. And, the comparison of the sense between the upper and the lower first molar’s periodontal ligaments generated by the mechanical stimulation showed the transmission speed of the lower first molar was faster and its signal strength was stronger than those of the upper first molar. Thus, the sense of the lower first molar periodontal ligaments has become a more important role in oral function than that of the upper first molar.

    This research was conducted for 3 years at the center of cleft lip and/or palate at Tohoku university hospital. The purpose of this study is to investigate the psychological impact on parents after prenatal diagnosis of their infant's cleft lip, and construct the psychosocial support for parents understanding about cleft and raise motivation of parenting. After 3 years research, over 70 parents were recruited this research. Our psychosocial support is gradually constructed with setting working a psychologist at the outpatients. Evidence from the research was reported at the conferences of Japanese Cleft palate association and will be reported at several conferences and papers. Our psychological support also will be keep providing with constructing psychological support model.

    Lip closing functions is thought to be very important factor in maintaining a negative pressure in the oral cavity when performing a normal swallowing, and also to be affecting articulation and arch form of the teeth. In this study, we aimed to reveal the relationship between lip closing functions, and the masticatory ability and articulatory function of the cleft lip and palate patients. In order to achieve this purpose, a balloon type lip pressure measuring device and lip movement analyzing system were newly developed. In addition, optimization of the myoelectric measurement conditions specific to the lip closing function was performed. Furthermore, performing both the masticatory ability assessment by the chewing gum method and articulation assessment by speech therapist, it was possible to clarify the characteristics of the functional problems recognized in common by the entire cleft lip and palate patients.

    The current study was carried out to investigate by what mechanism a novel bisphosphonate, [4-(methylthio) phenylthio] methanebisphosphonate (MPMBP), can promote bone formation, and whether this compound can be effective on the periodontitis. Our results show that MPMBP promotes the expression of mRNA that relates osteogenesis, and that this compound was effective on the experimental periodontitis in rats and dogs. MPMBP may be used not only for the treatment of periodontitis, but also for various conditions that require the regeneration of bone tissues.

    Recent studies suggest that the amelogenins are expressed in various tissues including bone and affecting bone metabolism. In this study, we examined the effects of LRAP, one of the major splice isoform of amelogenins, on stimulated bone turnover under estrogen deficiency. Using mesenchymal cells isolated from wild type and LRAP transgenic mice with or without ovariectomy(OVX), gene expression levels of inter leukin-6(IL-6), cycrooxigenase-2(COX2), and nitric oxide (NO)synthase were examined by real-time PCR. In addition, ELISA or colorimetric assay was also performed to measure prostaglandin E_2(PGE_2)or NO production from the cells. We found that the LRAP inhibited the elevation of IL-6 and COX2 gene expression, which is typically found in the estrogen deficient condition. PGE_2 production was decreased in the ovariectomized LRAP TG derived cells, but not in the ovariectomized wild type derived cells. The production of NO was increased in the ovariectomized LRAP TG cells. The LRAP may play a significant role on the prevention from bone loss due to estrogen deficiency, by changing the interleukin and PGE_2 expression, and the production on NO

    Amelogenin is a major protein expressed in enamel tissue and playing an important role for enamel formation. However, recent studies show that the amelogenin is expressed in various tissues including cartilage. Especially, two amelogenins among a number of splice isoforms, a full length amelogenin (M180) and a leucine rich amelogenin peptide (LRAP), are implicated as signaling molecules in the mesenchymal cells. In this study, we examined the effects of M180 and LRAP on chondrogenesis. Using differentiation medium supplemented with various concentrations of the M180 or LRAP containing medium, ATDC5 cells, a chondrogenic cell line, were cultivated. Addition of a higher concentration of M180 or LRAP to ATDC5 cells increased the alkaline phosphatase (ALP) activity at 14 days of culture compared to that in control. Increased glycosaminoglycan (GAG) secretion was also observed by alcian blue staining at 21 days of culture. Gene expression levels of ALP, Aggrecan, Col10a1 and Osteopontin were significantly increased in the samples supplemented with amelogenin at 28 days of culture. M180 and LRAP may have significant role to accelerate the chondrogenic differentiation.

    The aim of this study was to develop a magneto-driven orthodontic wire and to apply this new wire for efficient tooth movement Orthodontic tooth movement is generated by the coupling of bone resorption on the compressed side of the periodontal ligament (PDL) and by bone formation on the stretched side of the PDL as a consequence of therapeutic mechanical stress. Since orthodontic treatment usually takes place over a long period of time, the risk of caries, periodontal disease, and prolonged treatment period are burdensome for the patient. Furthermore, it has been reported that total treatment duration proves to be highly correlated with root resorption. In this respect, it is important to accelerate alveolar bone remodeling during orthodontic treatment, to shorten the time required for successful therapy. Some studies revealed that mechanical stimulation by vibration accelerated the speed of tooth movement. On the other hand, the unique properties of shape memory and superelasticity have made shape memory alloys (SMAs) very useful biomaterials. Superelasticity is a phenomenon in which the stress value remains fairly constant up to a certain point of wire deformation. At the same time, when the deformed wire rebounds, the stress value remains fairly constant The property is very advantageous for orthodontic tooth movement, and the SMA wires have thus been widely used in orthodontic treatment. In this study, a titanium-niobium-aluminum (Ti-24Nb-3Al) SMA wire, which was considered to be biocompatible as a nickel free wire, was developed, and combined with a cobalt-based ferromagnetic SMA. Thus, a magneto-driven orthodontic wire was newly developed, and orthodontic palatal movement of maxillary first molars in Wistar-strain rats (age; 6 weeks) was performed with this new wire. The vibration was applied for ten minutes everyday by a wirelessly changing the magnetic field. The results suggested that the vibration accelerated orthodontic tooth movement.

    The purpose of this research is to analyze the expression of molecules induced by the mechanical stress, and then to select the functional bone remodeling regulatory molecules, and to transfer these genes into the periodontal tissue, and finally, to control the local bone remodeling mechanism. 1. The analysis of molecules induced by mechanical stress in the cells of the periodontal tissue (1) Both the osteoblasts differentiation-regulating factor and the osteoclast differentiation-regulating factor produced from the periodontal ligament (PDL) cells: Soluble factor produced from PDL cells stimulated by cyclic tension force inhibited osteoblast differentiation, suppressing the expression of transcription factor, osterix and runx-2. Osteoprotegerin (OPG) was also produced by PDL cells, and on the other hand, inhibited the osteoclast formation. (2) The differences of the response to continuous compression force between PDL cells and the osteoblasts: The PDL cells were resistance to the compression force, and promoted the production of the angiogenesis factor corresponding to the compression force. On the other hand, osteoblasts were caused apoptosis by activation of caspase-3 via caspase-8 signaling pathway. 2. Development of appropriate in vivo delivery system of in vivo gene transfer to periodontal tissue and functional analysis (1) Receptor activator of NF- r B ligand (RANKL) and the OPG gene transfer by the HVJ-envelop vector method: RANKL gene transfer promoted the experimental tooth movement (ETM). Moreover the OPG gene transfers inhibited ETM and relapse after ETM by regulating the osteoclast formation. (2) Bone morphogenetic protein (BMP) -4-gene transfer by the in vivo electroporation method: The alveolar bone density increased by the BMP-4 gene transfer, and the relapse after ETM inhibited. (3) The gene transfer by the ultrasound using nanobubbles together: Using the nanobubbles together, high efficiency and transient expression were observed in the experiment targeting to PDL cells in vitro, and to periodontal tissue in vivo.

    Human mesenchymal stem cells(hMSC) of born marrow have the potential to defferentiate into obsteogenic, adipogenic, chondrogenic cells. hMSC were isolated from human bone marrow and were cultured. In this study, the expression of maker gene mRNA (type I collagen, Runx 2/CbFal, ALPactivity) increased under the mechanical stress. We showed that mechanical stress in the presence of OS. Mechanical stress promoted to differentiate into the osteoblast from hMSC, when cultered in the presence of osteogenic suppulement(OS). For examine to the effect of mechanical stress on the osteoblastic differentiation of hMSC, the change of gene expression by DNA maicroarray were examined under the osteogenic differentiation by the mechanical stress. The expression of four genes were down-regulated by only mechanical stress. The number of genes expression under the 2% mechanical stress were increaced more than the 4% and 8% mechanical stress. The expression of six genes were down-regulated under the osteogenic differentiation by mechanical stress.

    Mechanical stimulus is an essential factor for the maintenance of bone homeostasis. It is important to clarify the signal transduction pathway of mechanical stimulation (mechanotransduction pathway) leading to bone formation. In the present study, we investigated the role of Rho, a small G protein that regulates cellular configuration, in the mechanotransduction using pharmacological approach. 1.Development of a micromechanical testing device and establishment of the assay system We developed an improved loading device capable of delivering loads with milli-Newton resolution and micrometer displacements to viable bone explants and determined conditions of loading which give the maximum anabolic responses. 2.Pharmacological study using a tissue culture system and the micromechanical testing device It was suggested that small G proteins of Rho family might act as a negative regulator in the mechanotransduction pathway that leads to bone formation. 3.Pharmacological study using a cell culture system and the Flexercell Strain Unit It was suggested that small G proteins of Rho family are involved in the nitric oxide production by osteoblasts. However, it is not known whether they have any role in the signal transduction of a cyclic tensional stimulus in osteoblastic cells. 4.Future prospect Although the present results suggest that small G proteins of Rho family, are involved in the mechanotransduction pathway in bone, their role remains unclear and roles of the other families of small G proteins are not known. Further studies using molecular biological approach as well as pharmacological approach would be required to solve these questions.

    Nitric oxide, a mediator of vascular function, neurotransmission, and immune system, has been recently shown to play an important role in adaptive response of bone to mechanical stress/strain. Periodontal tissue receives and adapts to various mechanical stimuli such as masticatory force and orthodontic force. Thus, it is possible that NO is a mediator of adaptive physiological response of periodontal tissue to these mechanical stimuli. In the present study, we tested this possibility both in vitro and in vivo. 1) Effect of cyclic tension force on NO and prostaglandin EィイD22ィエD2 production and mRNA expression of their synthases in human periodontal ligament cells. Application of cyclic tension force to human periodontal ligament cells induced a rapid and transient increase in NO release in the culture medium. Endothelial isoform of NO synthase (NOS) mRNA expression decreased. Release of prostaglandin EィイD22ィエD2 time-dependently increased by the application of force. Cyclooxygenase-2 mRNA expression increased, whereas cyclooxygenase-1 mRNA expression did not change. These results suggest that NO and PGEィイD22ィエD2 are involved in the adaptive response of periodontal ligament cells to mechanical stress/strain. 2) Effect of topical administration of inhibitors of NO synthase and spontaneous NO donors on orthodontic tooth movement in rats. Upper first molars of male 8-week-old Wistar rats were moved buccally for 21 days. The topical administration of L-NAME (10 mg/mL), a general inhibitor of NOS activity, caused a significant reduction of tooth movement. On the other hand, L-NIO (5 mg/mL), a selective inhibitor of inducible isoform of NOS, had no effect on tooth movement. There was a tendency toward a decrease in tooth movement in animals treated with NO donors (NOR4 and NOC). These results suggest that NO sythesized by constitutive isoforms of NOS has an important role in orthodontic tooth movement.

    We have reported that systemic administration of bisphosphonates could prevent alveolar bone resorption in rats with experimental periodontitis. We have also reported that both systemic and topical administration of bisphosphonates have potent anchorage and retentive effects on the orthodontic tooth movement in rats. These results suggest that bisphosphonates could be useful agents in the various dental therapies in which prevention of alveolar bone resorption is essential. However, when clinical dental application of bisphosphonates is considered, localized use at specific local site of alveolar bone without systemic effect would be desirable, since normal systemic bone metabolism should not be affected by the local dental therapies. The objectives of the present study were (1) to determine whether topical administration of bisphosphonates is effective on the prevention of alveolar bone resorption incident to periodontitis or orthodontic tooth movement and (2) to clarify the mode of preventive action of bisphosphonates on the bone resorption. The following results were obtained. (1) Topical injection of clodronate or risedronate into the sub-periosteum area adjacent to target teeth were effective in preventing bone resorption incident to periodontitis or orthodontic tooth movement in rats. (2) Iontophoretic administration of risedronate were also effective in the prevention of alveolar bone loss in the periodontitis, but the effect was small. (3) The inhibitory action of bisphosphonates on bone resorption is not due to the inhibition of osteoclast recruittment, but rather to the inhibition of osteoclast function, such as an induction of apoptosis and inhibition of acid phosphatase.

    The purposes of the present study were 1) to examine the morphological changes of osteocytes in rat alveolar bone during orthodontic tooth movement using immunohistochemistry for actin filaments and 2) to determine a molecular marker for osteocyte metabolism. Eight-week-old male Wistar rats were used in this study. The maxillary first molars were moved buccally by using a expansive spring of Ni-Ti wire with a nominal size of 0.012 inch as described by Igarashi et al. (1998). The animals were killed after 3, 6, 12, 24 hours, 2, 4, 7, 10 and 14 days of tooth movement. After experiments, the animals were fixed with 4 % paraformaldehyde plus 0.5 % glutaraldehyde in 0.1 M PB, pH 7.4, decalcified with 10 % EDTA solution and processed into paraffin embedding. Six-mu in-thick sections were stained with hematoxilin-eosin or antibodies for osteocalcin, typesI,II and X collagen and actin filaments. The results were as follows ; 1) Precursor and mature osteocalcin was a proper molecular marker for examining the metabolic changes of alveolar bone and bone-forming cells. 2) After 3 hours of orthodontic tooth movement, hyaline tissue appeared in the pressure side of the periodontal tissue of all five roots of the first molar. The hyaline tissue spread and almost dessapered at day 10. 3) Immunohistochemistry for actin filament showed intense staining in cytoplasm and cellular processes of osteocytes in control group. Especially young osteocytes showed intense staining. In contrast, the staining of osteocyte cytoplasm adjacent to the hyaline tissue became weak at day 1 and the staining of its cellular processes disappeared at day 4. These results demonstrated that osteocytes adjacent to the hyaline degeneration tissue became cell death during early stage of orthodontic tooth movement due to the disappearance of intercellular communication between the periodontal tissue cells.

    The purpose of this study was to examine the risk of root resorption during orthodontic treatment and develop the prevention method of that from the basic and the clinical background. Clinical studies showed almost 40% of patients that developed severe root resorption during orthodontic treatment had abnormally shaped roots before active treatment. To test the effects of deficiencies of mineral nutrients on the formation of roots and the development of tooth resorption during orthodontic tooth movement, 2 weeks-old Wistar rats were fed a low calcium diet during root development period and the molars were moved orthodontically. A hypomineralized dentin with interglobular dentin was observed in the experimental animals, but no significant difference was observed in the development of tooth resorption between control and experimental animals. Immunohistochemical studies showed cementblasts produced osteocalcin and glycosaminoglycans and have simillar characteristics with osteoblasts. The production of them was changed during orthodontic tooth movement and cementblasts suggested to be related with the formation of odontoclasts. In vitro studies showed human periodontal ligament (PDL) fibroblasts induced HSP 47,60,70 with the application of continuous compressive stress to protect cells from mechanical stress and regulated the formation of odontoclast-like cells from peripheral blood lymphocytes. In contrast, inhibitory effects of the topical administration of a bisphosphonate on root resorption incident to orthodontic tooth movement were observed in rats. These results indicate the risk of root resorption during orthodontic treatment may be related with the properties of the periodontium and the roots and some bone resorption inhibitory detergents can prevent it.

    1.Osteoblast-like cells, MC3T3-El or periodontal-ligament derived fibroblasts were cultured on Type I collagen-coated silicon membranes and subjected to cyclic mechanical stimulation. The cells assumed a spindle shape, and the cytoskeleton components, including F-actin filaments and tubulin, were aligned perpendicular to the stretch force vector. Tyrosine-phosphorylated molecules which are localized in focal adhesion plaques and MAP kinase might be related to signal transduction caused by mechanical stress. 2.When peripheral blood leukocytes/monocytes were cultured with M-CSF,the number of osteoclast like cells, which were multinuclear, lamellipodia-positive and TRAP-positive cells increased. When these cells were cultured on slices of bovine dental root, pit formations were observed. Macrophages of PBL proliferate by blastogenesis and a part of the blastgenic macrophages differentiate to osteoclasts by M-CSF and 1,25 (OH) _2D_3. Moreover the number of osteoclast-like cells shown in PBL treated in M-CSF or GM-CSF was suppressed by coculture fluid of PBL/PDL cells. 3.Mechanical stress of tooth movement differently affected the alveolar bone and cellular cementum ; the bone was resorbed whereas the cementum resisted resorption and its calcification is inhibited under the compressive force of tooth movement. The expression of CH-6S is related to the compressive force in non-hyalinized and hyalinized PDL,whereas that of CH-4S/DS is not influenced by the mechanical stress. Osteocalcin expression of cementblasts forming acellular cementum is similar to that of cells forming cellular cementum as well as osteoblasts and odontblasts, and has a role for calcification of acellular cementum. Topical administration of a bisphosphonate (Risedronate) inhibits orthodontic tooth movement and root resorption. 4.Localization and distribution of the ECMs ; Type I,II,III,X collagen, osteocalcin and GAGs were analyzed, using immunohistochemical techniques. The marked regional differences cxit in the phenotypic expression of ECMs in tissues, may reflect the local functional environment and cellular response.

    The purpose of this study was to examine the effect of bisphosphonates, potent blockers of bone resorption, on orthodonic tooth movements. In 1992, we examined the effect of topical administration of a bisphosphonate (risedronate) on orthodontic tooth movements in rats. The topical administration of risedronate caused a significant and dose-dependent reduction of tooth movement after the orthodontic force was applied. The topical administration of risedronate also inhibited relapse of the tooth in a dose-dependent manner. The results regarding the number of osteoclasts indicated that the formation of osteoclasts was inhibited by the topical administration of risedronate. These results suggest that topical application of risedronate may be helpful in anchoring and retaining teeth under orthodonic treatment. In 1993, we measured the sizes of active bone resorptive lacunae by image analysis of the microscopic image which was fed directly to a TV monitor with a video camera attached to the microscope. About a 50 % reduction of the active bone resorptive area was observed on the risedronate-injected side as compared with the control side. This suggests that the bone-resorbing activity of osteoclasts decreased on the risedronate-injected side. In 1994, we reexamined the histological sections, putting emphasis on root resorptive changes. On the risedronate-treated side, the area of root resorption was significantly smaller than that on the control side. After the tooth movement, resorbed root surfaces were gradually restored by apposition of repair cementum. There was no difference between the restoration on the risedronate-treated side and that on the control side. In another study, the effects of 3 bisphosphonates, AHBuBP,AHPrBP,and Cl_2MBP on growth and osteoblastic functions of the clonal osteoblast-like cell line MC3T3-E1 were examined. The results suggest that bisphosphonates facilitate the differentiation of osteoblasts and that the inhibition of PGE_2 production by osteoblasts may be involved in the mechanism of the inhibitory effect of bisphosphonates on bone resorption.

    The present study was aimed to clarify the role of platelet activating factor (PAF) in bone metabolism. The findings were as follows. 1. Compared with PGE2, PAF did not seem to have a potent bone resorbing activity. 2. PAF inhibited calcification in an osteoblast-like cell, MC3T3-E1 in vitro. 3. PAF did not affect an endogeneous PGE2 diosynthesis by MC3T3-E1. 4. PAF did not increase an intra-cellular calcium ion concentration of MC3T3-E1. 5. MC3T3-E1 did not produce PAF with the stimulation of a calcium ionophore, A23187, but metabolised 90% of exogeneous PAF to lyso-PAF and acyl GPC within a hour.
