血管柄付き遊離腓骨皮弁による下顎骨再建治療を受けた患者1名の臨床データを用いて、術後のフォローアップCT画像解析による再建下顎骨の骨接合部のリモデリング解析、CTデータベースの有限要素解析による再建下顎骨のメカニカルストレス分布解析、これらの解析をもとにした患者固有の骨リモデリングアルゴリズムの構築とその応用を試みた。下顎右側臼歯部の歯肉扁平上皮癌のため下顎骨区域切除術ならびに血管柄付き遊離腓骨皮弁による下顎骨再建術を受けた66歳男性の治療データを用いて一連の解析を行った。下顎骨再建手術直後にCT撮影を実施し、その後は4、16、28ヵ月後に術後フォローアップとしてCT撮影を実施した。オトガイ部の下顎骨-腓骨の接合部をDS1、1ヶ所の腓骨同士の接合部をDS2、顎関節部に近い下顎骨-腓骨の接合部をDS3と定義した。その結果、特に異種骨(下顎骨-腓骨)間接合であるDS1、DS3において骨形態の経時的変化は顕著であり、各接合部において歪みエネルギー密度分布は経時的に大きく変化していた。構築したアルゴリズムを用いたシミュレーション解析により、下顎骨-腓骨間に適切な骨接合が獲得されることで、咬合力が増大しても固定用チタンプレートの破折リスクは増加しないことが示された。

筋萎縮性側索硬化症(ALS)患者を対象に口腔機能、咽頭機能の調査と舌接触補助床(PAP)の必要性との関連性を探索し、ALS患者のPAPの介入時期や必要な評価項目について検討した。2020年7月～2021年6月の1年間に当科にて口腔機能検査を実施した患者のうち、ALSの診断を受けた28名を対象とした。開口量、最大舌圧、舌口唇運動機能測定(オーラルディアドコキネシス)の/pa/、/ta/、/ka/は四肢麻痺型と比べ球麻痺型で有意に低い値を示した。反復唾液嚥下テストには有意な違いは認められなかったが、球麻痺型で低い値となる傾向がみられた。PAPの必要性の有無と兵頭スコアでの比較では、合計スコアと兵頭スコア3(嚥下反射惹起)の項目でPAPの必要性がある群で高い値となり有意差を認めた。ALS患者において、PAP製作の必要条件の候補として最大舌圧10kPa以下、3回/秒以下のオーラルディアドコキネシス(/ta/、/ka/)が挙げられた。

Mandibular deviation (MD) is a common reconstruction sequela after segmental mandibulectomy. Although proper postoperative rehabilitation is critical for MD management and minimization, the information available is limited. This report describes postoperative rehabilitation with an occlusal splint fabricated using computer-aided design (CAD) and computer-aided manufacturing (CAM) (CAD/CAM-OS) and the results of a threedimensional occlusal analysis using an intraoral scanner after hemimandibulectomy and plate reconstruction. Despite the short follow-up, adherence to postoperative rehabilitation with CAD/CAM-OS for MD correction, even during radiotherapy, was demonstrated by the digital workflow and analysis results.

PURPOSE: Using multivariate analysis, this study investigated the effectiveness of maxillofacial prosthetic treatment in relation to oral health-related quality of life (OHR-QoL), masticatory ability (food intake status score and gum-jelly test score) and related factors in patients who had undergone surgery for head and neck tumors. METHODS: The study cohort comprised 112 patients who underwent surgical resection and prosthetic treatment among 224 patients with head and neck tumors seen at the Maxillofacial Prosthetics Clinic of Tohoku University Hospital in a 2-year period. Correlations between OHR-QoL, food intake status score, and gum-jelly test score (criterion variables), and age, sex, maxillary defect, tongue/soft tissue defect, reconstructive surgery, and chemoradiotherapy (explanatory variables) were investigated, and the data were examined statistically. RESULTS: Maxillary defect, tongue and soft palate tissue defect, and chemoradiotherapy were identified as factors that hindered the effectiveness of maxillofacial prosthetic treatment for improvement of the OHR-QoL, food intake status score, and gum-jelly test score. On the other hand, reconstructive surgery was a factor that facilitated the improvement of OHR-QoL and masticatory ability with maxillofacial prosthetic treatment. CONCLUSION: The factors identified to be related to the success or failure of maxillofacial prosthetic treatment suggest the importance of combining prosthetic intervention with surgical reconstruction.

Purpose To investigate the effect of prosthodontic treatment on the ingestible food profile in adult Japanese outpatients, and to identify the related risk factors that can deteriorate the profile.Methods The participants were 277 outpatients who visited university-based specialty clinics in Japan for prosthodontic treatment. The demographic data, number of present teeth assessed via intraoral examination, and oral health-related quality of life assessed by the total Oral Health Impact Profile (OHIP-J54) scores of all participants were recorded before treatment. Ingestible food profile score (IFS) was recorded using a validated food intake questionnaire. Eligible participants who answered the questionnaire before and after treatment were categorized into five groups based on the prosthodontic treatments they received (i.e., crowns, bridges, removable partial dentures, removable complete dentures, and removable complete and partial dentures).Results Multivariate analysis of covariance revealed a statistically significant main effect of prosthodontic intervention (time course: before and after treatment) on mean IFS (p=0.035, F=4.526), even after adjusting for covariates (age, number of present teeth, and treatment modality). Multiple linear regression analysis revealed that the low number of present teeth (r=0.427, p<0.001) and a high OHIP-J54 total score (r=-0.519, p<0.001) of the patients at the baseline were significantly associated with their baseline IFSs, even after adjusting for confounding variables.Conclusions The findings of this multicenter follow-up study indicate the importance of prosthodontic rehabilitation in improving patients' ingestible food profiles.

The aim of this study was to investigate the clinical course of masticatory function recovery following arthrocentesis. Patients with a unilateral condylar head fracture who underwent arthrocentesis for therapeutic reasons were evaluated and compared with patients with a unilateral condylar head fracture who did not undergo arthrocentesis. At 3 months after treatment, the occlusal contact area and maximum bite force in patients with a fracture treated with arthrocentesis were greater than in those who did not receive arthrocentesis at the same time points, although the differences were not significant. Moreover, at 1 and 3 months following arthrocentesis, mean (±SD) occlusal contact area (1 month: 1.99 ± 0.55 mm2, p = 0.01; 3 months: 2.90 ± 1.36 mm2, p = 0.03) and maximum bite force (1 month: 82.45 ± 15.04 N, p = 0.01; 3 months: 101.11 ± 14.53 N, p = 0.01) on the fractured side in patients who underwent that treatment were significantly reduced when compared with those on the non-fractured side. The authors conclude that if the priority is to avoid open reduction and internal fixation, then the arthrocentesis approach might be a less invasive alternative, albeit with the price of a prolonged healing interval.

The Tohoku University Hospital has been a clinical and research facility for all the related departments of Tohoku University. Medical-dental and interprofessional collaboration has resulted in special treatment teams, made up of members of departments such as the center for head and neck cancer, the center for dysphagia, and the cleft lip and palate center. Those treatment teams held conferences, case study meetings, reading sessions, and in-hospital seminars. The purpose of this study was to evaluate the outcomes of various medical-dental and the interprofessional collaboration at Tohoku University Hospital and training program to equip hospital dentists in higher medical institutions. The attainment targets are the acquisition of basic medical skills and knowledge under the guidance of supervising doctors. As a result, the hospital dentists could acquire their own specialized knowledge and skills certificated by each academic society. The smooth team treatment has been achieved, and the number of cases discussed by cancer boards and center for dysphagia has increased year by year due to the efficiency of their clinical pathways. On the dental care side as well, the wearing rates of maxillofacial prosthetic devices such as maxillofacial prostheses and palatal augmentation prostheses (PAP) have improved, which have contributed to improving patient's stomatognathic function. Tohoku University Hospital has been practicing collaboration between medical and dental professionals and it has produced mutual benefits. Our interprofessional training system based on the medical-dental collaboration could develop professionals who have acquired cross-disciplinary knowledge and skills from experienced doctors.

BACKGROUND: The palatal augmentation prosthesis (PAP) is an intraoral prosthesis used in the treatment of dysphagia. AIM: The objective of the study is to examine the effect of PAP using tongue pressure and the Videofluoroscopic Dysphagia Scale (VDS) to understand the precise mechanism for improvement in swallowing function with PAP for oral cancer at retrospective survey. METHODS AND RESULTS: Fifteen patients were provided PAPs. Tongue pressure and VDS were evaluated with and without PAP. After intervention with PAP, tongue pressure significantly increased as compared to when without PAP (p < .05). The total mean VDS score with PAP was found to have significantly improved (p < .05). The mean VDS score of the oral phase also significantly improved with the PAP compared to without the PAP group (p < .05). Significant differences (p < .01) were found in each category, such as tongue to palate contact and pyriform sinus residue. CONCLUSION: PAP can improve tongue pressure, tongue to palate contact, and pyriform sinus residue.

OBJECTIVE: The objective is to conduct a questionnaire survey regarding pharyngolaryngeal sensation evaluation in dysphagia to understand the current situation in Japan. METHOD: The questionnaire was sent to the councilor of the Society of Swallowing and Dysphagia of Japan and the Japanese Society of Dysphagia Rehabilitation-Certified Clinician. The prospective questionnaire survey included the questions listed below: Q1: What do you think of the importance of pharyngolaryngeal sensory evaluation? Q2: Select one of the essential swallowing sensations. Q3: Select one of the following regarding the frequency of sensory examination of the larynx. Q4: Select the proportion of cases the sensory test results affect. Q5: As a pharyngolaryngeal sensory evaluation method in swallowing function evaluation, please fill in the table below for the frequency, difficulty, and effectiveness of the following tests, such as gag reflex, touching the larynx by endoscopy, touching the larynx by the probe with endoscopy, cough reflex test, swallowing provocation test. RESULTS: The essential swallowing sensations of mechanical stimulation, chemical stimulation, thermal stimulation were 84.9%, 5.4%, and 9.7%, respectively. The frequency of touching the larynx by endoscopy in the otolaryngology group and cough reflex test in dentistry was significantly higher than the other groups (p < 0.05). The correlation between the frequency and difficulty or effectiveness of the sensory tests indicated that the frequency and difficulty are significantly correlated between each item. CONCLUSION: Our results aid in increasing understanding and selection of pharyngolaryngeal sensation evaluation for dysphagia patients.

Maxillofacial prosthetic treatment with obturator prostheses has contributed to improvements in speech, mastication, swallowing, and early return of patients to society. To establish more effective maxillofacial prosthetic treatment, it is desirable to develop a new material with high reproducibility of the detailed shape of maxillary defects, high adaptability to tissue changes over time, and high biocompatibility. This study focused on shape memory gel (SMG). SMG has unique properties: it softens when heated, maintains its shape when cooled, and returns to its original shape when reheated. The aim was to evaluate the physical properties of SMG for dental application. Specimens were prepared by changing the polymerization initiator camphorquinone (CQ) and the mixing ratio of the two monomers stearyl acrylate (SA) and lauryl acrylate (LA) of SMG, and the memory properties were examined. Switching the polymerization initiator of SMG to CQ enabled polymerization with dental photopolymerization devices. Changing the combination ratio of SA to LA enabled adjustment of the tensile strength. It was confirmed that the shape memory property, an original characteristic of SMG, was maintained, and physical properties similar to those of existing dental materials were obtained.

BACKGROUND: The impact of the jaw bone condition, such as bone quantity and quality in the implant placement site, affecting the accuracy of implant placement with computer-guided surgery (CGS) remains unclear. Therefore, this study aimed to evaluate the influence of bone condition, i.e., bone density, bone width, and cortical bone thickness at the crestal bone on the accuracy of implant placement with CGS. METHODS: A total of 47 tissue-level implants from 25 patients placed in the posterior mandibular area were studied. Implant placement position was planned on the simulation software, Simplant® Pro 16, by superimposing preoperative computed tomography images with stereolithography data of diagnostic wax-up on the dental cast. Implant placement surgery was performed using the surgical guide plate to reflect the planned implant position. The post-surgical dental cast was scanned to determine the position of the placed implant. Linear and vertical deviations between planned and placed implants were calculated. Deviations at both platform and apical of the implant were measured in the bucco-lingual and mesio-distal directions. Intra- and inter-observer variabilities were calculated to ensure measurement reliability. Multiple linear regression analysis was employed to investigate the effect of the bone condition, such as density, width, and cortical bone thickness at the implant site area, on the accuracy of implant placement (α = 0.05). RESULT: Intra- and inter-observer variabilities of these measurements showed excellent agreement (intra class correlation coefficient ± 0.90). Bone condition significantly influenced the accuracy of implant placement using CGS (p < 0.05). Both bone density and width were found to be significant predictors. CONCLUSIONS: Low bone density and/or narrow bucco-lingual width near the alveolar bone crest in the implant placement site might be a risk factor influencing the accuracy of implant placement with CGS.

OBJECTIVE: Current interventions of dysphagia are not generalizable, and treatments are commonly used in combination. We conducted a questionnaire survey on nurses and speech therapists regarding dysphagia rehabilitation to understand the current situation in Japan. METHODS: The questionnaire was sent to 616 certified nurses in dysphasia nursing and 254 certified speech-language-hearing therapists for dysphagia. Based on "Summaries of training methods in 2014" by JSDR, 24 local indirect exercises, 11 general indirect exercises, and 13 direct exercises were selected. The Likert scale "How do you feel about each method" was used as follows: A; Frequency, B; Ease, C; Adherence, D; Effectiveness (1-5))?". RESULTS: Two hundred fifty (40%) nurses and 145 (57%) speech-language-hearing therapists (ST) responded to the questionnaire. The direct exercise was associated with a significantly high score in every question. In indirect exercises, "Cervical range of motion exercise," "Orofacial myofunctional exercise," "Lip closure exercise." "Ice massage of pharynx" and "Huffing" were used relatively frequently. "Balloon dilatation therapy" and "Tube exercise" was associated with a relatively high discrepancy for two questions. Frequency" and the sum of "Ease," "Adherence," and "Effectiveness." was significantly correlated for local indirect exercises (r2 = 0.928, P < 0.01), general indirect exercises (r2 = 0.987, P < 0.01), and direct exercises (r2 = 0.996, P < 0.01) (Fig. 5). CONCLUSION: This study examined the current situation of dysphagia rehabilitation in Japan. Our results aid to increase understanding and selection of rehabilitative treatments for dysphagia patients in Japan.

STATEMENT OF PROBLEM: Although closed hollow obturator prostheses provide the benefit of minimized weight, they also pose challenges. They are complex to fabricate, and contaminated water can easily enter the hollow section through the joined part, making them unsanitary and leading to malodor and increased weight. PURPOSE: The purpose of this in vitro study was to investigate the hermeticity and durability of a hollow obturator model fabricated by using computer-aided design (CAD) and rapid prototyping (RP) techniques and to evaluate the possibility of its clinical use. MATERIAL AND METHODS: Leak testing was used to evaluate the hermeticity and durability of hollow spherical obturator specimens with an outer diameter of 30 mm and 2 different wall thicknesses (1.5 and 2.0 mm). Six specimens were fabricated for each of the wall thicknesses by using CAD and RP techniques. The accumulation of fluids in the hollow obturator specimens was evaluated every day by using megascopic observation with photoirradiation from the base of the specimens. The amount of water absorption and the rate of increase in the weight of the 2 specimens were calculated and compared. Statistical analysis was performed by using the Mann-Whitney U test (α=.05). RESULTS: The application of CAD and RP techniques made it possible to fabricate a hollow obturator model specimen with completely unified parts. The 1.5-mm specimen showed an absorption rate (2.61%) that significantly exceeded that of the 2.0-mm specimen (2.53%) on day 130 (P=.006). By the end of the observation period, the 1.5-mm specimen exhibited large amounts of water absorption and destruction. The 1.5-mm-thick wall had reduced hermeticity than the 2.0-mm-thick wall. CONCLUSIONS: A fully unified hollow obturator model with 2.0-mm-thick walls was fabricated by using CAD and RP techniques. The absence of any joints prevented fluid accumulation, making this method suitable for the fabrication of hollow prostheses.

The biomechanics associated with buccal bone thickness (BBT) augmentation remains poorly understood, as there is no consistent agreement in the adequate BBT to avoid over-loading resorption or over-augmenting surgical difficulty. This study utilizes longitudinal clinical image data to establish a self-validating time-dependent finite element (FE)-based remodeling procedure to explore the effects of different buccal bone thicknesses on long-term bone remodeling outcomes in silico. Based upon the clinical computed tomography (CT) scans, a patient-specific heterogeneous FE model was constructed to enable virtual BBT augmentation at four different levels (0.5, 1.0, 1.5, and 2.0 mm), followed by investigation into the bone remodeling behavior of the different case scenarios. The findings indicated that although peri-implant bone resorption decreased with increasing initial BBT from 0.5 to 2 mm, different levels of the reduction in bone loss were associated with the amount of bone augmentation. In the case of 0.5 mm BBT, overloading resorption was triggered during the first 18 months, but such bone resorption was delayed when the BBT increased to 1.5 mm. It was found that when the BBT reached a threshold thickness of 1.5 mm, the bone volume can be better preserved. This finding agrees with the consensus in dental clinic, in which 1.5 mm BBT is considered clinically justifiable for surgical requirement of bone graft. In conclusion, this study introduced a self-validating bone remodeling algorithm in silico, and it divulged that the initial BBT affects the bone remodeling outcome significantly, and a sufficient initial BBT is considered essential to assure long-term stability and success of implant treatment.

BACKGROUND: Self-care and professional care of implants may prove difficult for elderly people who require nursing care. However, the actual state of care and problems remains unknown. In this study, we investigated the actual state of implant problems in elderly people living in their own home or in a nursing home who received visiting dental treatment. METHODS: We mailed questionnaire survey forms to 2339 representatives or specialists who were members of the Japanese Society of Oral Implantology, the Japanese Society of Gerodontology or the Japan Prosthodontic Society. We narrowed down the respondents to those who provided visiting dental treatment, and analyzed the actual state of implants observed during visiting dental treatment (type, care, problems, countermeasures, etc.). RESULTS: Of the 924 dentists who responded to the questionnaire survey, 291 (22%) provided visiting dental treatment. While the majority of implant types encountered in the previous 12 months were root-form implants, there were still a certain number of blade and subperiosteal implants. Daily implant care involved mostly cleaning with a toothbrush + auxiliary tools. The most frequent implant problems encountered in the past were difficulty in cleaning and peri-implantitis. Medication and antiphlogistic treatment were most frequently adopted as countermeasures to implant problems, followed by observation. When we classified the results into those for the dentists who provided implant treatment and those for the dentists who did not, we found that many of the dentists who did not provide implant treatment opted for observation or medication, while those who provided implant treatment also implemented removal of superstructure, retightening of screws, repair and so forth. CONCLUSIONS: We found that many of the implant troubles encountered by dentists who provided visiting dental care were difficulty in cleaning or peri-implantitis, and that the actions taken against these troubles varied depending on the experience of the dentist performing the implant treatment. Our study also revealed that dentists who provide visiting dental care need to acquire knowledge and skills of implant treatment, to have actions prepared in case they encounter such cases, or to closely coordinate with dentists who specialize in implants.

OBJECTIVES: Although inequalities in dental implant use based on educational level have been reported, no study has used income as a proxy for the socioeconomic status. We examined: (i) income inequalities in implant use; and (ii) whether income or education has a stronger association with implant use in elder Japanese. METHODS: In 2016, a self-reported questionnaire was mailed to participants aged 65 years or older living across Japan as part of the ongoing Japan Gerontological Evaluation Study. We used data from 84,718 respondents having 19 or fewer teeth. After multiple imputation, multi-level logistic regression estimated the association of dental implant use with equivalised income level and years of formal education. Confounders were age, sex, and density of dental clinics in the residential area. RESULTS: 3.1% of respondents had dental implants. Percentages of dental implant use among the lowest (≤ 9 years) and highest (≥ 13 years) educational groups were 1.8 and 5.1, respectively, and among the lowest (0 < 12.2 '1,000 USD/year') and highest (≥ 59.4 '1,000 USD/year') income groups were 1.7 and 10.4, respectively. A fully adjusted model revealed that both income and education were independently associated with dental implant use. Odds ratios for implant use in the highest education and income groups were 2.13 [95% CI = 1.94-2.35] and 4.85 [95% CI = 3.78-6.22] compared with the lowest education and income groups, respectively. From a model with standardised variables, income showed slightly stronger association than education. CONCLUSION: This study reveals a public health problem that even those with the highest education but low income might have limited accessibility to dental implant services.

Oral rehabilitation with prosthodontic treatment considerably influences the well-being and quality of life of patients after ablative oral tumor surgery. This study evaluated the effects of implant-supported prostheses (ISPs) on oral health-related quality of life (OHRQoL) and chewing ability in 10 patients who requested ISPs after mandibular oral tumor resection. OHRQoL was assessed using the Japanese version of the Oral Health Impact Profile (OHIP-49) before and one year after ISP placement. Chewing ability, including self-assessed masticatory ability and occlusal force, was examined at one year after ISP placement. The initial mean total OHIP-49 score of 65.3 ± 9.79 decreased to 46.0 ± 8.14 at one year after ISP placement. Mean OHIP-49 score decreased in all domains, whereas self-assessed masticatory ability increased within one year of ISP placement. There were no significant differences between prosthesis types with respect to the mean OHIP-49 score or self-assessed masticatory ability. In conclusion, ISP placement improves OHRQoL and the self-assessed masticatory ability. Moreover, the prosthesis type might not significantly affect OHRQoL.

Fluid accumulation in the hollow spaces of obturator is a continuing problem when fabricating hollow obturator prostheses using the conventional method. To address this problem, the three-dimensional (3D) digital technology was used to evaluate water absorption in the inner hollow obturator spaces. Solid and hollow obturator specimens were fabricated using a 3D printer with photocurable resin. Then, the hermeticity was examined by leak testing. These specimens were immersed in distilled water at 37°C. Each specimen was weighed every 24 h for 120 days, and weight changes between each group were compared. Water accumulation in the hollow obturator was not visually observed. Although water absorption was significantly higher in solid specimens, the weight increase rate was also significantly higher in hollow specimens. Applying a laminating 3D photo fabrication made the fabrication of a completely unified hollow obturator model possible.

PURPOSE: Patients treated with maxillofacial prosthetics often experience emotional problems because of the remaining facial skin concavity such as a surgical scar. In such cases, cosmetic treatment can potentially correct their skin tone imperfections and deformities. This study aimed to evaluate the clinical availability of novel cosmetics-based material for craniofacial small concavity by initiating a cosmetic treatment in a preliminary case. METHODS: Eighteen patients with aesthetic problems such as craniofacial deformities, small defects, and concavities on their faces underwent cosmetic treatment that was performed by makeup practitioners. Data were collected from the patient's charts and a survey questionnaire. A visual analog scale was used to conduct a survey regarding the satisfaction levels of the patients following cosmetic treatment with a novel cosmetics-based material. The cosmetic treatment was performed for a concavity on the left midface of a 67-year-old woman with partial maxillectomy. The novel cosmetics-based material was manufactured from a semi-translucent oil base. RESULTS: The satisfaction level of the patient increased after undergoing the cosmetic treatment. Regarding clinical applications, the novel cosmetics-based material can help reduce their cosmetic disturbance and restore the small deformity. CONCLUSIONS: These results suggest that the cosmetic treatment with the novel cosmetics-based material can be used as a subsidiary method for facial prostheses or an independent new method for correcting patients' small craniofacial concavity and for reducing visible deformity.

Whilst the newly established biomechanical conditions following mandibular reconstruction using fibula free flap can be a critical determinant for achieving favorable bone union, little has been known about their association in a time-dependent fashion. This study evaluated the bone healing/remodeling activity in reconstructed mandible and its influence on jaw biomechanics using CT data, and further quantified their correlation with mechanobiological responses through an in-silico approach. A 66-year-old male patient received mandibular reconstruction was studied. Post-operative CT scans were taken at 0, 4, 16 and 28 months. Longitudinal change of bone morphologies and mineral densities were measured at three bone union interfaces (two between the fibula and mandibular bones and one between the osteotomized fibulas) to investigate bone healing/remodeling events. Three-dimensional finite element models were created to quantify mechanobiological responses in the bone at these different time points. Bone mineral density increased rapidly along the bone interfaces over the first four months. Cortical bridging formed at the osteotomized interface earlier than the other two interfaces with larger shape discrepancy between fibula and mandibular bones. Bone morphology significantly affected mechanobiological responses in the osteotomized region (R2 > 0.77). The anatomic position and shape discrepancy at bone union affected the bone healing/remodeling process.

BACKGROUND: The objective of this study was to ascertain the situation relevant to implants, the status of oral self-care, the status of aftercare provided by the dentist who placed the implant, and the usage status of the implant card, in homebound or institutionalized older adults who are receiving home-visit dental care due to the inability to visit a dental clinic on their own. METHODS: A survey questionnaire was sent by post mail to 2339 people who are representative members or dental specialists belonging to any of the following three academic societies: Japanese Society of Oral Implantology, Japanese Society of Gerodontology, and Japan Prosthodontic Society. The survey questions asked were about provision/no provision of implant treatment, provision/no provision of home-visit dental care, the situation of patients after implant treatment, the situation of implants in the context of home-visit dental care, and the usage status and recognition of the implant card. RESULTS: No less than 30% of the dentists had patients who were admitted to the hospital or became homebound after receiving implant treatment at their clinic. Twenty-two percent of the dentists had been consulted about the implants. Dentists who continued to provide post-operative implant care through home-visit dental care accounted for approximately 80%. On the other hand, however, 40% of the dentists did not know the post-implantation status of their implant patients. Of the patients receiving home-visit dental care, approximately 3% had implants (identified mainly by visual inspection). It was found that more than 50% of the dentists offering implant treatment did not use the implant card, and even in cases where it was used, most of the cards were not in the standardized format. CONCLUSIONS: Within the limitation of low response rate to the questionnaire in this preliminary study, we concluded that many of practitioners including specialists indicated the need of universal record of implant for dependent elderly cares.

PURPOSE: The Japan Prosthodontic Society developed a multi-axis assessment protocol to evaluate the complex variations in patients who need prosthodontic care, and to classify the level of treatment difficulty. A previous report found the protocol to be sufficiently reliable. The purpose of this multi-center cohort study was to evaluate the validity of this multi-axis assessment protocol. METHODS: The treatment difficulty was evaluated using the multi-axis assessment protocol before starting prosthodontic treatment. The time required for active prosthodontic treatment, medical resources such as treatment cost, and changes in the oral health-related QOL before and after treatment, were evaluated after treatment completion. The construct validity of this protocol was assessed by the correlation between the dentist's pre-operative subjective assessment of the treatment difficulty, and the level of difficulty determined by this protocol. The predictive validity was assessed estimating the correlations between a "comprehensive level of treatment difficulty" based on the four axes of this protocol and total treatment cost, total treatment time, and changes in the oral health-related QOL before and after treatment. RESULTS: The construct validity of this protocol was well documented except for psychological assessment. Regarding the predictive validity, the comprehensive level of treatment difficulty assessed before treatment was significantly correlated with the three surrogate endpoints known to be related to the treatment difficulty (total treatment cost, treatment time, and improvement in the oral health-related QOL). To further clarify the validity of the protocol according to patients' oral condition, a subgroup analysis by defects was performed. Analyses revealed that treatment difficulty assessment before treatment was significantly related to one or two surrogate endpoints in the fully edentulous patients and the partially edentulous patients. No significant relationship was observed in the patients with mixture of full/partial edentulism and the patients with teeth problems, possibly due to the small sample size in these groups. CONCLUSION: This study revealed that the multi-axis assessment protocol was sufficiently valid to predict the level of treatment difficulty in prosthodontic care in patients with fully edentulous defects and with partially edentulous defects.

Melanotic neuroectodermal tumor of infancy (MNTI) is an extremely rare, pigmented neoplastic entity of neural crest origin. Histological and immunohistochemical profiles indicate the presence of two components, small rounded neuroblast-like cellular areas and areas with large melanin-containing cells which consist of combination of neural, melanocytic, and epithelial cell types. Here we present two interesting cases of infants with MNTI which have different clinicopathological features. The first case is a 3-month-old female with rapidly growing MNTI involving the lacrimal sac and inferior wall of the orbital cavity, treated with total maxillectomy without orbital exenteration followed by chemotherapy. The second case is a 7-month-old male with slow-growing maxillary MNTI treated with complete surgical excision. In the female patient, histological findings revealed a predominance of neuroblast-like cellular areas and a high Ki67 index indicating rapid cellular proliferation. In the male patient however, large melanin-containing cells were dominant in this slow-growing tumor. These findings support the presence of two different types of MNTI, rapid-growing and slow-growing types, determined by the component of neuroblast-like cellular areas.

3Dデジタル技術を利用した栓塞部中空型顎義歯の製作法とその物性について検討した。光学式3Dスキャナーと積層造形装置を応用して、完全一体の中空型の栓塞部を有する顎義歯の製作が可能であった。3D造形材料と床用アクリルレジンの圧縮せん断接着強さは、プライマー処理をした試料は15.2±4.9Mpa、プライマーなし群は13.5±1.4Mpaの結合強さを示し、両者に有意差は認めなかった。またプライマー処理に関係なく、全ての試料でDSとMCの界面剥離を認めず、DS側に試料内部へむかって線状の亀裂が存在し、凝集破壊を観察した。DSのみの試料では13.7±1.0MPaの結合強さで、プライマーなしの試料に近い値を示したが、DSを加熱処理した試料は29.2±3.8MPaの結合強さで他の3条件の2倍の値を示した。DSのみの試料では内部に亀裂が生じたが破断には至らず、加熱処理をした全ての試料では内部に向かって亀裂が生じ破断した。

頭頸部腫瘍リハビリテーション期患者への歯科衛生士による専門的口腔ケアの実態について調査した。歯科衛生士専用ユニット(DHユニット)で処置した頭頸部腫瘍術後患者を対象とした。専門的口腔ケア担当の歯科衛生士9例へ、対象患者に対して行った口腔ケアの実態に関してアンケートを行った。口腔ケアを行った延べ患者数は3787件、実患者数は1158例であった。そのうち頭頸部腫瘍術後の専門的口腔ケアを受けている患者は18例(男性10名、女性8名、41〜85歳)であった。頭頸部腫瘍に対する処置は、腫瘍摘出手術11例、腫瘍摘出手術+放射線療法5例、腫瘍摘出手術+化学療法1例、腫瘍摘出手術+化学療法+放射線療法1例であった。最終来院時のPCR値は初診時と比較し有意に低下した。対照患者を担当していた8例中7例から、口腔ケアの処置中に、課題、疑問と感じることがあると回答した。

頭頸部領域の腫瘍、先天性疾患ならびに外傷を有する患者の実態調査について報告した。頭頸部腫瘍・先天性疾患・外傷を有する288例を対象とし、頭頸部腫瘍250例、嚢胞6例、先天性疾患28例、外傷3例であった。初診時年齢は、29歳以下が全体の8.3%、30〜59歳が全体の27.4%で、60歳代と70歳代は全体の55.5%を占めた。紹介元は、口腔外科100例、予防歯科80例、頭頸部外科52例であった。悪性腫瘍が232例、86.8%と圧倒的に多かった。上顎癌は74例(27.8%)、下顎癌は103例(38.7%)で、歯肉癌58例、骨肉腫1例、舌癌32例、口底癌12例、頬粘膜癌14例、中咽頭癌16例などであった。また、良性腫瘍は30例、嚢胞6例であった。

宮城県立精神医療センターに入院中の患者79名(男性46名、女性33名、平均54歳)を対象に、歯科検診・機能検査を行った。入院期間は平均2808日、疾患は統合失調症が87%と最も多く、薬剤服用者は92%、院内歯科受診者は20名(25%)であった。平均値で口腔清掃状態(PI値)は前歯部1.7、臼歯1.3、う蝕・歯周疾患(CPI値)は最大値1.9、歯周ポケット4mm以上が50.6%であり、現在歯数は17.2歯、喪失歯は13.1歯、健全歯数は7.6歯、未処置歯数は4.1歯、処置歯数は4.1歯、欠損補綴歯数は5.3歯、DMFT指数は22.6であったが、義歯装着者は4名のみであった。口腔乾燥度は26.6、咀嚼機能検査は91.8mg/dl、嚥下機能検査(RSST)は5.4、RSST:3回未満の陽性率は42.9%であった。精神疾患患者の口腔環境は劣悪であることが判明し、積極的な口腔ケア指導、摂食・嚥下指導の必要性が示唆された。

可動域を含む顔面欠損に対応する2ピースエピテーゼの製作と臨床応用について検討した。症例は63歳男で、左側上顎癌により拡大上顎全摘術を施行し、中顔面部欠損および頬骨部および上唇頬部に皮膚残存部の陥凹を認めた。口腔内は、上顎無歯顎で、硬口蓋および歯槽部欠損が正中を超え反対側におよび、軟口蓋の欠損は後縁を含んだ。咀嚼困難および顔面欠損部の審美不良を訴え、2ピースエピテーゼを製作した。2ピースエピテーゼでは患部皮膚可動域に装着された内側エピテーゼが可動部の動きに追随し、可動域を避けて装着された外側エピテーゼの下方部内でスライドし、開口時のエピテーゼ辺縁部の間隙量が減少した。2ピースエピテーゼは開口運動に影響されることなく装着後5〜6時間の維持が得られた。また、内側エピテーゼは患者皮膚可動域に接着されているものの内側エピテーゼ自体が小さく皮膚可動域の複雑な動きに追随し分離することがなかった。

試作エピテーゼ用シリコーン材料の引張り応力に対する永久歪みについて検討した。試作エピテーゼ用シリコーン材料KRS-10、KRS-C1、エピテーゼ用シリコーン材料A-2186Fを用いた。引張り後の永久歪みはKRS-C1が有意に小さかった。引き伸ばす量を、130%、150%、200%と変化させ比較すると、二元配置分散分析の結果、各変形量間のみ有意差を観察したが、各材料間には有意差は観察しなかった。二元配置による異なる繰り返し応力での比較では、各材料間のみ有意差があったが、回数間には有意差は確認できなかった。各材料間における多重比較検定の結果、KRS-C1群にのみ有意差が存在した。繰り返し負荷での比較(500mm/min、130%)で比較すると、各材料間および回数間のいずれにおいても有意差を確認した。また、各材料間における多重比較の結果からKRS-C1のみ他の材料に比べて有意に小さかった。

PURPOSE: To quantify and identify bacteria detected in acrylic resin dentures and dento-maxillary obturator-prostheses after long-term use. METHODS: The internal layer of denture bases from 13 daily-use removable acrylic resin dentures was sampled, while the inner fluid samples/no-fluid samples of obturators were collected from 11 in-use acrylic resin dento-maxillary obturator-prostheses. Samples were cultured, and isolated bacteria were counted and identified by molecular biological methods. RESULTS: Bacteria were detected in five (38.5%) acrylic resin dentures and six (54.5%) acrylic resin obturators. Four Lactobacillus species and one Propionibacterium species were isolated from three repaired denture bases, and from two non-repaired dentures, two Actinomyces species and Streptococcus mutans were isolated. On the other hand, 17 bacterial species, belonging to the family and genera of Olsenella, Bacillus, Citrobacter, Enterobacteriaceae, Lactobacillus, Pantoea, Peptoniphilus, Klebsiella and Pseudomonas, were isolated from obturators. Several species of viable bacteria were detected in acrylic resin denture bases and obturators.

PURPOSE: Prosthetic reconstruction of a facial defect can help to reduce disfigurement and restore the social functioning of the patient. Several methods for holding a prosthesis in place exist, including the use of osseointegrated implants and medical adhesive agents; however, since the treatment options for some patients may be restricted by various health conditions and other limitations, including allergies to adhesive agents, a history of radiation therapy, and financial issues, other options that suit individual demands are required. The objectives of this study were to test the hypothesis that adhesive characteristics could be bestowed on silicone elastomers by altering their catalyst/base silicone ratios (CBR) and to examine the effect of the thickness of the cohesive silicone layer of a prosthesis on its initial adhesive strength. MATERIALS AND METHODS: The adhesive strengths of specimens with CBRs ranking from 1/10 to 1/70 were examined by the rolling ball tack test. A tensile test was used to evaluate the tensile adhesive strengths of specimens made of layers of cohesive silicone (CBR 1/60) and normal silicone (CBR 1/10) with different thicknesses. Auricular prostheses containing cohesive silicone on the skin side were applied to a 50-year-old man with defects in both auricular regions and with reduced manual dexterity due to serious burns. RESULTS: The rolling distance was reduced with a decrease in CBR, and a thinner cohesive silicone (CBR 1/60) layer demonstrated a higher peak load. On clinical application, the adhesion of the auricular prosthesis containing cohesive silicone was improved by expanding the adhesive area and altering the thickness of the cohesive silicone layer, resulting in sufficient adhesion and easier handling than that achieved using an adhesive agent 1 year post delivery. CONCLUSION: These results suggest that cohesive silicone can be used as a glueless retentive material for facial prostheses.

PURPOSE: The purpose of this retrospective study was to investigate factors that affected the continuing use of RPDs and the patients' satisfaction 5 years after delivery. METHODS: Sixty-seven patients treated with 90 RPDs delivered at the Tohoku University Hospital (Sendai, Japan) between 1996 and 2001 participated in this study. The patients were re-examined 5 years after delivery. Data were collected from clinical records and a questionnaire was used to evaluate the patients' delivery use of the RPDs. The RPD treatment was divided into three categories labeled as 'successful', 'remake', and 'failure'. Twelve factors that might affect the continuing use and 15 factors regarding satisfaction were evaluated. Stepwise logistic regression analysis was used to assess statistical significance. RESULTS: Fifty-five RPDs were regarded as successful, 21 were remake, and 14 failure. Statistically significant associations were found between the continuing use and the patient's age (P=0.002), location of the edentulous area (P=0.047), number of occluding pairs of teeth (P=0.038), number of occlusal rests (P=0.038), pain while using RPDs (P=0.006), color of the artificial teeth (P=0.021), and tooth shape and set-up (P=0.022). CONCLUSIONS: These findings suggest that the continuing use of RPDs is related to factors such as the patient's age, location of edentulous area, number of occluding pairs of teeth, and number of occlusal rests, satisfaction including pain while using RPDs, color of the artificial teeth, and tooth shape and set-up.

臨床的に顔面および顎口腔に異常が認められない男性6名(平均27±2歳)を被験者として、正常者顔面の三次元像を用いて4種類の正中基準平面について距離・角度・左右計測点の非対称率分析を指標にして比較検討した。その結果、顎顔面対称性の評価に際しては、顎顔面欠損部位、範囲および治療目的に応じて、適切な正中基準平面を選択する必要性が示された。また各正中標準平面の特徴を把握することにより、様々な症例の顎顔面対称性評価にも使用できることが示された。

日本顎顔面補綴学会、日本口腔外科学会、日本補綴歯科学会所属の計209施設を対象としてアンケート調査を実施し、105施設(50%)からの回答を得た。多くの施設が、口腔の組織内および組織外の補綴処置、咽頭の組織外の補綴処置、顎顔面の組織内および表面の補綴処置、頭頸部の組織外の補綴処置を顎顔面補綴あるいは自らの科の領域の処置と考えていたが、頭頸部の組織内の補綴処置、その他の組織内の体表面や組織内の補填処置は顎顔面補綴あるいは自らの科の領域の処置と考えていなかった。特に顔面補綴では実施すべきと考える率と実施率との差が大きく、制度的・資格的などの整備が必要と思われた。

顔面欠損患者に対するシリコーン材料自体が粘着性を有するエピテーゼを考案した。シリコーン材料の粘着性は、ベースシリコーンとキャタリストの混合比は通法の10対1では全く粘着性を有さなかったが、キャタリストの割合を減じることにより粘着性を帯び、60対1のとき、ローリングボールタック試験での剛球停止距離は13.2±1.7mmで、「最適」と判断した。生体為害性試験では、8時間後も被験者全員に皮膚変化はみられず、安全性を確認した。扁平上皮癌のため鼻部摘出術を受けた鼻部欠損患者2名に粘着性を有するシリコン製エピテーゼを装着したところ、接着剤を使用しなくても十分な粘着性を認め、皮膚の発赤・炎症、エピテーゼ辺縁部の破損は観察されなかった。また、患者の審美的な満足も得られた。粘着性シリコンエピテーゼは、接着剤を利用しなくても良いという大きな利点を持ち、今後の顔面補綴治療の主流になりうる可能性が示唆された。

PURPOSE: The purpose of this retrospective cohort study was to investigate factors that affected the status of wearing removable partial dentures (RPDs) using a multivariate analysis. METHODS: One hundred and sixty-one patients were treated with definitive dentures that were delivered by dental students under teacher supervision at Tohoku University Hospital between 1996 and 2001. Of the 161 patients, 67 patients who agreed to undergo a follow-up examination participated in this study. The subjects were 18 men and 49 women with a mean age of 66.0+/-9.5 years. They were re-examined 5 years after treatment. The status of wearing the RPDs was categorized into three groups, i.e: successful, wearing their original RPDs constantly for 5 years; replaced, wearing re-fabricated RPDs within 5 years, and failed; interrupted wearing the RPDs within 5 years. Factors that affected the status were also examined. The analyzed variables in this study were sex and age of subjects, previous experience in wearing RPDs, location of RPDs and edentulous area, number of occluding pairs, number of missing teeth, placement of RPDs on opposite jaw, denture base type, number of abutment teeth, type of clasp, and number of rests. A stepwise multiple logistic regression analysis was used to determine factors affecting the status of wearing RPDs. RESULTS: The response rate was 41.6%. Of the 90 RPDs, 55 RPDs were regarded as successful, 21 as replaced, and 14 as failed. Statistically significant correlations were found between denture usage and the age of subjects, the location of edentulous area, number of occluding pairs, and number of rests (p<0.05). CONCLUSION: The results suggest the importance of considering the significant factors mentioned above for treatment planning and designing of RPDs.

当院では2015年10月から多職種による摂食嚥下治療センターの運用を開始した。その概要は、初診時に耳鼻咽喉・頭頸部外科で嚥下内視鏡検査(VE)と嚥下造影検査(VF)を行うとともに、歯科で口腔内診査と口腔機能検査を行い、これらの検査結果についてリハビリテーション科医や言語聴覚士、看護師などが加わったカンファレンスにより検討し、多角的に評価する。今回、2016年7月までに同センターを受診した287例を対象に、「性別」「年齢」「原疾患」「義歯使用の有無」「補綴処置を要する欠損の有無」「VE所見」「VF所見」などについて調査した。性別は男性178例、女性109例、年齢は平均65.4±15.3歳であった。原疾患は頭頸部腫瘍が最も多く62%、次いで神経筋疾患9%、脳血管障害6%の順であった。義歯を使用していたのは32.0%で、補綴処置を要する欠損は35.6%が有していた。VE所見は、兵頭スコアで概ね問題なしとされるスコア4以下の症例が68.1%、誤嚥のリスクがあるものが29.5%、経口摂取不可と評価されたものが2.4%であった。VF所見は、AsRスコアから経口摂取のみでの栄養摂取が可能と評価されたものが35.1%、補助栄養を要するが直接訓練は可能とされたものが6.5%、直接訓練は避けるべきとされたものが58.4%、早期咽頭流入など口腔期に障害を認めたものが44.2%であった。

広範囲顎骨支持型装置と広範囲顎骨支持型補綴について検討した。「広範囲顎骨支持型装置および広範囲顎骨支持型補綴」を適用した患者および補綴装置を調査した。適用患者数は12例(男性7名、女性5名、平均56.9歳)であり、適用したインプラント部位は上顎4例、下顎7例、1例は上下顎に適用であった。良性腫瘍5例、悪性腫瘍6例、先天欠損1例で、放射線療法の既往ある者はいなかった。骨造成は床義歯形態に1例、ブリッジ形態に5例に行われた。床義歯形態は6例、7個、ブリッジ形態は6例、6個に適用された。埋入インプラント平均数は可撤性装置が4個、固定性装置が2.3個であった。全顎補綴装置数91個に対する割合は13%であった。悪性腫瘍以外の6例中5例で、骨造成PCMB(チタンメッシュ+腸骨海綿骨)が行われていた。

Functional adaptation with reformation of bone tissue structure occurs after changes in mechanical stress distribution. To examine how occlusal changes affect the dynamics of bone metabolism in the mandibular condyle, bone scintigraphy of rat condyles was taken using (99m)technetium-methylene-diphosphonate (99mTc-MDP) after extraction of maxillary molars resulting in unilateral loss of occlusal support. Accumulation of 99mTc-MDP was significantly higher in the condyles on the extracted side than on the intact side 3 d after molar extraction. In addition, bone mineral density (BMD) and osteoprotegerin expression in extracted-side condyles were significantly increased while osteoclast numbers were significantly decreased when compared with intact-side condyles. These differences were not detected 28 d after molar extraction. These findings suggest that occlusal change transiently results in changes in the dynamics of bone metabolism at the mandibular condyles through the downregulation of osteoclastogenesis. These changes may be involved in functional adaptation of the temporomandibular joint.

本研究では形状記憶ゲル材料（SMG）の歯科材料への応用探索を行い、口腔内における材料特性挙動や生体安定性を検証した。形状記憶ゲルは、アクリルアミドモノマーを主骨格とする、複数のモノマーにより架橋重合された3次元網目構造を有している。結晶融点の異なる2種類のアクリレートモノマー（ステアリル酸アクリレート（SA）およびラウリル酸アクリレート（LA））を含むことで、低温～室温環境下で形状を維持し、温度上昇に伴い生じる結晶融解によりヤング率が急激に低下する。これにより、重合体が瞬時に元の形状に戻る性質を示す（形状記憶・回復特性）。これらの特性を口腔内に適用するため以下の必要とされる材料特性評価を行った。１．圧縮変化による形状記憶・回復特性を検討するため、JIS K 6262を参考に圧縮永久歪試験を行った。結果、SMGは圧縮変形に対して、形状記憶・回復特性を示したが、異なるLA(SA)配合比率間で有意な差を示さなかった。形状固定後、37℃へ温度変化しても、50~60%の形状回復性を示した。今後、口腔内環境下での最適な材料設計を検討する必要があることが示唆された。２．生体内での安定性を検討するため、JIS T 6520 に準拠してSMGの吸水溶解性試験を行った。比較的、物性の高い配合においても、基準値の15倍以上の吸水量を示した。一方、溶解量は基準値以下であった。これは親水性の高いアクリルアミドモノマーが水分を含むためだと考えられる。アクリルアミドの配合量を低減するか、マトリックス重合率あるいは架橋密度の向上が必要であることが示唆された。３．新しい材料を配合することにより、特性が向上した。①CQ/3級アミンにより、37℃条件下での形状固定性が向上した。②MBAAの増量配合により、37℃条件下での形状回復性が向上した。

The aim was to evaluate the physical properties of shape memory hydrogel (SMG) for application of dental impression material in the use of maxillofacial prosthodontics. The SMG has the unique properties that it softens when heated, can maintain its shape when cooled, and returns to its original shape when reheated. Specimens were prepared by changing the polymerization initiator and the mixing ratio of the two monomers (SA, LA) of SMG. Physical properties were evaluated using a tensile test and evaluation of shape memory characteristics (fixation rate, recovery rate). The switching of the polymerization initiator of SMG to CQ enabled polymerization with the dental photopolymerization devices. It was confirmed that the shape memory property, an original characteristic of SMG.

It is difficult to completely remove plaque and calculus substance formation attached the dental implant without damage the surface of its implants. We are proceeding to develop a novel device, an actuator-driven pulsed water jet (ADPJ). ADPJ is an emerging technology with remarkably reduced water consumption and has the ability to selectively remove tissues based on their material stiffness. The aim of this study was to assess the removal effect of the artificial dental calculus and the surface scratches of the dental implants after using ADPJ in proper output. The effectiveness of dental calculus removal by the ADPJ at a constant jet pressure was dependent on the voltage output. Moreover,no damage was observed on the surface of the titanium disc or dental implants following the use of ADPJ.

For the monitoring of chew and swallowing condition in long period, the silk electrode has been used on the neck surface. Because of artifacts by the phonation and respiration movements, it was difficult to achieve a steady monitoring for the swallowing. The structure of the larynx and pharynx focusing those movement was evaluated histologically to assume the reason of the artifacts, and novel findings of distribution of elastic fibers and enteric plexus are published. To increase the stability of monitoring, the soft, paper type electrode which can detect surface movements of the skin by changing its electronic resistance has been developed and the experiment of monitoring in healthy subject is starting. Also the swallowing functions of head and neck cancer patients (the study will be carried out in those cases) has been evaluate and published.

During pulling the corner of mouth, the RMS values of buccinator and levator anguli oris muscles were highest at mandibular rest position. During contacting upper and lower teeth, the RMS values of these facial muscles were proportional to the clenching level. On the other hand, these RMS values of these facial muscles were highest at 50% of maximum voluntary clenching of the teeth. The RMS values of these facial muscles were observed to be lowest at the mandibular rest position and increased as the level of the clenching increased. It was suggested that the activities of the buccinator and levator anguli oris muscles were decreased due to the influence of contact between the upper and lower teeth.

In the present study, to develop a glueless retentive material for the denture, two prototype of cohesive silicone were made and their adhesive properties and atacchemnt forces were investigated. Although two prototypes improved manipulation of silicone, their adhesive properties and atacchemnt forces were lower than those for control. Besides, all samples didn't show adhesive property in the presence of artificial saliva. To improve adhesive property of the prototypes with water, the cohesive silicone applied with hydrophilic materials and developing.

To evaluate the recovery level of the craniofacial and oral functions after the treatment of the head and neck cancer, new estimation approach for healing of soft tissues, which had created by the surgical operation, had been studied. The index of oral functional rehabilitation was composed with the measurement values of oral functions and surgical wound healing. The patients who suffered the head and neck cancer were evaluated and compared the processes of oral functional recovery by using this evaluation method, with the observation of scar contraction and the oral functional rehabilitation, such as exercises of jaw opening.

The purpose of this study was to clalify the effect of maxillary obturator prostheses on facial morphology in unilateral maxillary defect patients. The facial data of 8 patients were acquired with a 3-dimensional digitizer, both with and without prostheses. Displacement at 18 measurement points, including 7 bilateral pairs, was compared between the 2 sides with and without the prostheses. Asymmetry indices of these 7 pairs were also calculated. Displacements of the lateral and inferior points at the ala of the nose were significantly greater on the defect side than on the normal side. The distances between the ideal and defect side points at the superior ala, the upper lip, and the angle of the mouth decreased significantly with the prosthesis. The angle of the upper lip line to the midsagittal plane increased significantly with the prosthesis. The prostheses affect the region of the nasal ala, the upper lip, and the angles of the mouth.

It prescribed the capacity of the required balloon for impression of maxillary defect by measuring the change of the intraoral capacity of the maxillary defect.The experiment model was modeled with 3D printer using the intraoral 3-D data and CT DICOM data. The expansion method of the balloon inserted the basket catheter using the guide wire in the balloon inside and tried a method to expand the balloon by support from the inside of the basket catheter. It was possible to expand or shrink the balloon easily and quickly. This study was able to contribute to development by the novel maxillofacial impression system that it was possible for with a nasal cavity and maxillary sinus safely by reducing a balloon again, and removing it after having recorded the complicated intraoral form of the maxillary defect patient.

The purpose of this study was to investigate the influence of denture base volume and vertical dimension of maxillary complete denture on facial morphology in individuals with maxillary edentulous by using three-dimensional digital stereophotogrammetry. In the case of removing the denture flange, unilateral concave surface corresponding to the area without denture flange was observed in all subjects. On the other hand, when the buccal flanges were swollen 3 mm, the bulge corresponding to swollen area of denture flange was observed in all subjects faces bilaterally. When changing the vertical dimension, the morphological change surrounding the ala of nose was not observed. On the other hand, it was suggested that the morphologies surrounding the subnasale, labrale superius, and cheilion points had the potential to be changed with a change in the vertical dimension.

In the present study, to develop a glueless retentive material for the denture, two prototypes of cohesive silicone were made and their mechanical properties and level of instantaneous adhesions were investigated. Two prototypes, High cohesion (H) and Middle cohesion(M), were made to be cured by using equivalent amount of a catalyst and base silicone, which improved manipulation of the silicone and bubble entrapment. Mechanical properties and the level of adhesions for (H) were same level as those for control. Furtehr investigation and improvement are necessary, for example, put in hydrophilic materials to show adhesive characteristics even when water is existent.

The purpose of this study was to develop the prosthodontic clinical guideline based on treatment difficulty indices. A multi-centered epidemiological study showed the newly developed multi-axis assessment protocol was useful and reliable to estimate the level of difficulty in treating patients who needed prosthodontic care. The results suggest that the established evidence-based clinical guideline is expected to have a significant contribution to the practice of qualified prosthodontic treatment with sufficient safety and efficacy.

The aim of this study was to investigate efficacy of prosthetic rehabilitation for shortened dental arch. Oral health-related quality of life and masticatory function were evaluated in patients with shortened dental arch before and after prosthetic treatments and in patients without treatment. Oral health-related quality of life and masticatory function were impaired with increase of missing posterior teeth, and they were improved after prosthetic treatment, suggesting that prosthetic rehabilitation for shortened dental arch is effective.

This study aimed to reveal the dynamic change of bone metabolism under the denture base using SPECT. Wistar rats were anesthetized with pentobarbital and the right maxilla teeth were extracted. To observe the bone metabolic activity, SPECT of rat cranium was taken using a gamma-camera. Tc-99m-MDP (74MBq/rat) was injected intravenously. Although the SPECT images were lower quality than planer bone scitigram, quantitative analysis of the the images provided sufficient information to examine the bone metabolic activity of alveolar bone beneath the denture base.

Loading before and after osseo-integration with bone scintigraphy observation study, following results were obtained : 1.the bone metabolic activity around dental implants is elicited again, and it is differed according to the degree of loading. The peak of bone metabolic turnover is differed according to the timing of loading and it rise when the timing of loading would be late. It suggested in nuclear medicine research that "the timing of loading" would affect the bone metabolic activity and an "osseo-integration" acquisition process.

The purpose of this study is to clarify the behavior of the load applied to the abutment tooth by means of the 3-dimensional force measuring device with a piezo-electric transducer that we have developed. It is important to know that load for partial denture design including the selection of the abutment tooth and occlusal scheme. The custom-made force measuring device comprised an inner part forming a metal core, a force transducer, and an outer part forming a metal crown. The subjects were a young male with natural dentition and an old female with partial edentulous dentition. Informed consent was obtained after full explanation of this study. The experimental teeth were the maxillary left second molar of dentulous subject and mandibular right second premolar of partial edentulous subject. When the partial edentulous subject wearing the denture with distal rest clenched maximally, the magnitude of the load applied to the abutment tooth decreased significantly than without the denture. When the partial edentulous subject wearing the denture with medial rest clenched maximally, the angle of the load direction was decreased significantly than without the denture. On the other hand, there was significant difference between the angles with the medial rest and with the distal rest. When the partial edentulous subject wearing the denture with distal rest clenched maximally, the angle of the load direction was increased significantly than the denture with medial rest. The force vector with the medial and distal rests directed most laterally, and that with the medial rest directed most downward. There was not significant difference among the directions of four different conditions in the frontal plane. In the dentulous subject, The maximum magnitude was 173.3±15.3N during MVC. As the magnitude of the compressive force increased, the load direction changed three dimensionally. Tractive forces were recorded throughout the chewing the sticky food.

Bone changes around the dental implants have not been fully elucidated. This study amid to reveal the dynamic change of bone metabolism around the implant using bone scintigraphy. Experimental implants were inserted in the tibiae of Wister rats. To observe the bone metabolic activity, bone scintigraphy of the cranium was taken using a gamma-camera with a modified high-resolution pinhole collimator (diameter of the pinhole: 2mm). 99mTc-MDP was injected intravenously (2 mCi/rat). Static planer acquisition in coronal direction was executed at 2 hours after the injection. The image takings were made at 4 days, 1, 2, 3, 4 weeks and 5 weeks after the implant insertion. After obtaining osseointegration, implants were loaded with coil springs (100,150,200g). The image takings were also made at 4 days, 1, 2, 3, 4 weeks and 5 weeks after loadings. Accumulation of 99mTc-MDP to the tibiae bone around the implant and the contralateral intact site were counted. Uptake ratio of the tibiae bone around the implant to the intact, site was calculated in each group. The study revealed that bone metabolism at, the implant site was returned to the same level as the contralateral intact site at Sweeks after the insertion. High peak accumulation of 99m-MDP was seen around the implant at from 7 to 12 days after insertion. Uptake ratio around the implant was differing among the different kind of loading. Furthermore, there was significant difference between the loading side and the traction side of implant after loading to implants. Turn-over of the tibiae bone around the dental implant was demonstrated with the active uptake of 99mTc-MDP by bone scintigraphy.

The aim of this study was to investigate the modality of 3-D force onto abutment tooth in vivo with a special miniaturized piezo-electric force transducer In order to clarify the characteristic of a small crystal piezo-electricity formula load sensor, a rigid proofreading equipment, made with steel material, that hate 1 axis variable vice of angle change by 0.05-degree was developed. This was enable to load to the transducer fixed with a brass jig from horizontal direction to vertical direction. When regression analysis between sensor output and the known amount of loads was applied to investigate the linearity, correlation coefficients of X-axis, Y-axis and Z-axis are 0.99999, 0.99999 and 0.99996, respectively. The effect of the hysteresis on sensor output for X-axis, Y-axis and Z-axis were 0.9%, 1.1% and 1.7% at the maximum, respectively. As the effect of temperature on sensor output was examined by 10-degree rise, the increase of output on X-axis, Y-axis, and Z-axis were 0.5%, 0.46%, respectively. These results showed the transducer hadquite precision, and was suitable for in vivo measurement of functional loads onto the tooth. A load measuring device that consists of an inner part similar to a metal core, a force transducer and an outer part similar to a tooth crown was manufactured. These were united by a steel screw and set with temporary cement on the tooth, so it enabled to measure the load onto the tooth in vivo. Informed consent was obtained from subjects after explaining the aim of this study. During clenching, force magnitudes and directions in 3-D on tooth crown were recorded. When the bite force increased, the direction of load changed in the mesial palate side in natural dentition. For the abutment tooth of a partial denture which have a removable occlusal rest, when with the occlusal rest, the amount of load became lager than not with the occlusal rest and the direction of load changed in the distal buccal side. Conclusion : this system enables to measure a 3-D functional load onto abutment tooth in vivo.
