    AIMS: The molecular genetic mechanisms underlying postoperative nausea and vomiting (PONV) in the brain have not been fully elucidated. This study aimed to determine the changes in whole transcriptome in the nucleus of the solitary tract (NTS) in an animal model of PONV, to screen a drug candidate and to elucidate the molecular genetic mechanisms of PONV development. METHODS: Twenty-one female musk shrews were assigned into three groups: the Surgery group (shrew PONV model, n = 9), the Sham group (n = 6), and the Naïve group (n = 6). In behavioral studies, the main outcome was the number of emetic episodes. In genetic experiments, changes in the transcriptome in the NTS were measured. In a separate study, 12 shrews were used to verify the candidate mechanism underlying PONV. RESULTS: A median of six emetic episodes occurred in both the Sham and Surgery groups. Whole-transcriptome analysis indicated the inhibition of the GABAB receptor-mediated signaling pathway in the PONV model. Baclofen (GABAB receptor agonist) administration eliminated emetic behaviors in the shrew PONV model. CONCLUSIONS: Our findings suggest that the GABAB receptor-mediated signaling pathway is involved in emesis and that baclofen may be a novel therapeutic or prophylactic agent for PONV.

    BACKGROUND: Genetic factors such as single-nucleotide polymorphisms (SNPs) play a key role in the development of postoperative nausea and vomiting (PONV). However, previous findings are not widely applicable to different populations because of population-specific genetic variation. We developed a Japanese-specific DNA microarray for high-throughput genotyping. The aim of the current study was to identify SNPs associated with PONV on a genome-wide scale using this microarray in a sample of Japanese surgical patients. METHODS: Associations between 659,636 SNPs and the incidence of PONV 24 h after surgery in a limited sample of 24 female patients were assessed using the microarray. After imputation of genotypes at 24,330,529 SNPs, 78 SNPs were found to be associated with the incidence of PONV. We chose 4 of the 78 SNPs to focus on by in silico functional annotation. Finally, we genotyped these 4 candidate SNPs in 255 patients using real-time PCR to verify association with the incidence of PONV. RESULTS: The T > C variant of rs11232965 in the long non-coding RNA MIR4300HG was significantly associated with reduced incidence of PONV among genotypes and between alleles (p = 0.01 and 0.007). CONCLUSIONS: We identified a novel SNP (rs11232965) in the long non-coding RNA MIR4300HG that is associated with PONV. The rs11232965-SNP variant (T > C) is protective against the incidence of PONV.     BACKGROUND: Underbody blankets have recently been launched and are used by anesthesiologists for surgical patients. However, the forced-air warming effect of underbody blankets is still controversial. The aim of this study was to determine the effect of forced-air warming by an underbody blanket on body temperature in anesthetized patients. METHODS: We retrospectively analyzed 5063 surgical patients. We used propensity score matching to reduce the bias caused by a lack of randomization. After propensity score matching, the change in body temperature from before to after surgery was compared between patients who used underbody blankets (Under group) and those who used other types of warming blankets (Control group). The incidence of hypothermia (i.e., body temperature < 36.0 °C at the end of surgery) was compared between the two groups. A p value < 0.05 was considered to indicate statistical significance. RESULTS: We obtained 489 propensity score-matched pairs of patients from the two groups, of whom 33 and 63 had hypothermia in the Under and Control groups, respectively (odds ratio: 0.49, 95% confidence interval: 0.31-0.76, p = 0.0013). CONCLUSIONS: The present study suggests that the underbody blanket may help reduce the incidence of intraoperative hypothermia and may be more efficient in warming anesthetized patients compared with other types of warming blankets.     We describe a case of fulminant intraoperative thrombosis during deceased donor liver transplantation. Despite significant medical bleeding, the patient suddenly developed diffuse thrombosis in all chambers of the heart and pulmonary vasculature resulting in intraoperative death. The patient's postmortem genetic analysis demonstrated a deleterious missense mutation in a coagulation pathway gene, SERPINC1, which codes for antithrombin III. The level of antithrombin III was not available to directly prove the causality of thrombosis, but our findings suggest that this mutation, in combination with antifibrinolytic administration in a hypercoagulable cirrhotic patient, might have contributed to the development of this catastrophic thrombotic event.

    A 29-year-old patient, 32 weeks' pregnant, with a history of familial interstitial fibrosis, was treated for acute hypoxemia after admission to the intensive care unit. Within 48 hours, this was followed by an emergent cesarean delivery, under general anesthesia, due to acute respiratory failure. Successful perinatal obstetric and anesthetic management resulted in the delivery of a baby and recovery of the mother. Subsequent genomic analysis using next-generation sequencing of the patient's entire exome revealed that she was a carrier of a deleterious mutation in the TERT gene, previously associated with the hereditary forms of interstitial fibrosis.

    Various preclinical rodent models have been established and utilized to elucidate the mechanisms underlying neuropathic pain. However, the utility of existing major models faces challenges, such as reproducibility and surgical complexity. Here, we introduce a new rat model of neuropathic pain created by simply clamping the sciatic nerve for 10 min. First, we investigated pain-related behavior in 12 male Wistar rats after surgery to clamp the sciatic nerve that induced mechanical allodynia for 21 days and thermal hyperalgesia for 7 days (2-way ANOVA compared with sham-operated rats, p < 0.01). We observed only small interindividual differences in response to stimuli among the rats after sciatic nerve clamp surgery. The L5 dorsal root ganglion and spinal dorsal horn of the other 14 rats were subsequently sectioned and stained for Atf3 and Iba-1 immunoreactivity, respectively. Compared with sham surgery, nerve clamping induced peripheral and central sensitization (Mann-Whitney U test, p < 0.01). This new preclinical rat model of neuropathic pain offers procedural simplicity and high reproducibility.

    PURPOSE: Minimally invasive cardiac surgery (MICS) involves fewer complications than median sternotomy. However, difficulties in post-MICS analgesia can undermine these advantages. The serratus anterior plane block (SAPB), an effective analgesic for thoracic surgery, could benefit post-MICS analgesia using programmed intermittent bolus infusion (PIBI). We investigated whether PIBI with SAPB can reduce post-MICS fentanyl administration and evaluated its safety profile. METHODS: This randomized, double-blind, controlled trial included 20 patients (age 20-80 years; Eastern Cooperative Oncology Group Performance Status 0-II; scheduled for elective MICS) randomly allocated to two groups (SAPB or control). All patients underwent preoperative SAPB with catheterization, followed by either 20 mL 0.25% ropivacaine or saline bolus every 6 h postoperatively. All patients received intravenous fentanyl via patient-controlled analgesia. Blood samples were collected 10, 20, 30, and 60 min after preoperative ropivacaine infusion; during and after cardiopulmonary bypass; and on postoperative days 1-5. The primary outcome was cumulative fentanyl consumption up to postoperative day 5. Secondary outcomes included numerical rating scale scores, rehabilitation preoperatively and postoperatively, postoperative nausea and vomiting, ropivacaine toxicity, and PIBI with SAPB complications. RESULTS: After excluding three patients, data from 17 patients were analyzed. No significant difference in cumulative fentanyl consumption on postoperative day 5 was observed (SAPB: median [interquartile range], 512 µg [457-753] vs. control: 654 µg [439-982], P = 0.96). Serum ropivacaine concentration remained below the toxicity threshold. CONCLUSION: PIBI with SAPB did not reduce post-MICS fentanyl consumption, and serum ropivacaine concentration did not reach the toxicity threshold.

    OBJECTIVES: To identify the clinical, ethnic, and genetic factors contributing to the varying risks of postoperative nausea and vomiting (PONV) among a Brazilian population undergoing cancer surgery. METHODS: A case-control study was conducted involving 152 patients who experienced vomiting and/or retching (cases) and 158 patients who did not report nausea, vomiting, or retching (controls) within 24 h following oncological surgeries. This study is registered as 'Genetic Polymorphism and postoperative nausea and vomiting (PONV)' under registration number NCT03627780 (https://clinicaltrials.gov/study/NCT03627780). Thirty-two polymorphisms associated with PONV predisposition and 15 polymorphisms for ancestry analysis were genotyped via real-time polymerase chain reaction (PCR) with customised TaqMan low-density array (TLDA) cards. RESULTS: The C allele of the rs208294 polymorphism (P2RX7 gene) was observed at a significantly higher rate in the control group than in the case group across the genotype (P=0.035), dominant (P=0.010) and allele (0.032) models, thus suggesting a protective effect against PONV. The genotype results for rs208294 were validated via Sanger sequencing, which confirmed the association in the dominant model (P=0.027). In a multivariate regression analysis that included rs208294 and clinical variables that were identified in the univariate analysis, only a prior history of PONV or motion sickness was observed to be a significant predictor of PONV (P<0.05). No association between rs208294 and PONV was detected in an external cohort consisting of 198 cases and 56 controls of Japanese descents (P>0.05). Additionally, ancestry analysis indicated a predominantly European genetic composition in the Brazilian cohort, which differed with the Asian composition of the independent validation cohort. CONCLUSIONS: A previous history of PONV or motion sickness was identified as being the strongest predictor of PONV in our analysis. Genetic association, ancestry and external validation analyses suggest that genetic factors for PONV may significantly differ across populations of different continental origins.

    BACKGROUND: Postoperative delirium (POD) is a severe complication associated with various adverse outcomes, especially in older patients. Although the incidence and risk factors for POD have been explored in general surgery, they have not been fully elucidated. Early identification of high-risk patients and active preoperative intervention are considered essential for the prevention of POD. Recently, psychiatric consultation intervention have been shown to prevent delirium. This study investigated the effect of preoperative psychiatric interventions on preventing POD in our specific surgical context. MATERIALS AND METHODS: This retrospective, single-center observational study included 86 patients who underwent major oral and maxillofacial surgery with free flap reconstruction between 2016 and 2023. The effect of psychiatric intervention were compared between patients with and without delirium. RESULTS: Preoperative psychiatric intervention did not reduce the incidence of POD. The incidence of POD was 29.1%. Univariate analyses showed no significant associations between POD and any clinical variables. CONCLUSION: There was no difference in the incidence of POD between patients who received preoperative psychiatric intervention and those who did not, and further investigation is needed to determine the efficacy of preoperative psychiatric intervention in the prevention of POD.

    Non-alcoholic fatty liver disease is a common liver disease worldwide, and is associated with dysregulation of lipid metabolism, leading to inflammation and fibrosis. Acanthopanax senticosus Harms (ASH) is widely used in traditional medicine as an adaptogen food. We examined the effect of ASH on steatohepatitis using a high-fat diet mouse model. Mice were fed a choline-deficient, L-amino acid-defined, high-fat diet with ASH extract (ASHE). After 6 weeks, liver RNA transcriptome sequencing (RNA-Seq) was performed, followed by Ingenuity Pathway Analysis (IPA). Our findings revealed that mice fed a high-fat diet with 5% ASHE exhibited significantly reduced liver steatosis. These mice also demonstrated alleviated inflammation and reduced fibrosis in the liver. IPA of RNA-Seq indicated that hepatocyte nuclear factor 4 alpha (HNF4 alpha), a transcription factor, was the activated upstream regulator (P-value 0.00155, z score = 2.413) in the liver of ASHE-fed mice. Adenosine triphosphate binding cassette transporter 8 and carboxylesterase 2, downstream targets of HNF4 alpha pathway, were upregulated. Finally, ASHE-treated HepG2 cells exposed to palmitate exhibited significantly decreased lipid droplet contents. Our study provides that ASHE can activate HNF4 alpha pathway and promote fat secretion from hepatocytes, thereby serving as a prophylactic treatment for steatohepatitis in mice.

    OBJECTIVES: The quantitative assessment of muscle stiffness or weakness is essential for medical care. Shear wave elastography is non-invasive ultrasound method and provides quantitative information on the elasticity of soft tissue. However, the universal velocity scale for quantification has not been developed. The aim of the study is to determine the shear wave velocities of abdominal muscle during anesthetic induction and to identify methods to cancel the effects of confounders for future development in the quantitative assessment of muscle tone using the universal scale. METHODS: We enrolled 75 adult patients undergoing elective surgery with ASA-PS I - III in the period between December 2018 and March 2021. We measured and calculated the shear wave velocity (SWV) before and after opioid administration (i.e., the baseline at rest and opioid-induced rigidity condition), and after muscle relaxant administration (i.e., zero reference condition). The SWV value was adjusted for the subcutaneous fat thickness by our proposed corrections. The SWVs after the adjustment were compared among the values in baseline, rigidity, and relaxation using one-way repeated-measures ANOVA and post hoc Tukey-Kramer test. A p-value of < 0.05 was considered to be statistically significant. UMIN Clinical Trials Registry identifier UMIN000034692, registered on October 30, 2018. RESULTS: The SWVs in the baseline, opioid-induced rigidity, and muscle relaxation conditions after the adjustment were 2.08 ± 0.48, 2.41 ± 0.60, and 1.79 ± 0.30 m/s, respectively (p < 0.001 at all comparisons). CONCLUSION: The present study suggested that the SWV as reference was 1.79 m/s and that the SWVs at rest and opioid-induced rigidity were ~ 10% and ~ 30% increase from the reference, respectively. The SWV adjusted for the subcutaneous fat thickness may be scale points for the assessment of muscle tone.

    Coronavirus disease 2019 (COVID-19) is associated with coagulopathy. However, the underlying mechanisms are not completely understood. We evaluated the association between COVID-19 coagulopathy and extracellular vesicle (EV) levels. We hypothesized that several EV levels would be higher in COVID-19 coagulopathy patients than in non-coagulopathy patients. This prospective observational study was conducted in four tertiary care faculties in Japan. We enrolled 99 COVID-19 patients (48 with coagulopathy and 51 without coagulopathy) aged ≥20 years who required hospitalization, and 10 healthy volunteers; we divided the patients into coagulopathy and non-coagulopathy groups according to the D-dimer levels (≥1 μg/mL and <1 μg/mL, respectively). We used flow cytometry to measure the tissue-factor-bearing, endothelium-derived, platelet-derived, monocyte-derived, and neutrophil-derived EV levels in platelet-free plasma. The EV levels were compared between the two COVID-19 groups as well as among the coagulopathy patients, non-coagulopathy patients, and healthy volunteers. No significant difference was found in EV levels between the two groups. Meanwhile, the cluster of differentiation (CD) 41 + EV levels were significantly higher in COVID-19 coagulopathy patients than in healthy volunteers (549.90 [255.05-984.65] vs. 184.3 [150.1-254.1] counts/µL, p = 0.011). Therefore, CD41+ EVs might play an essential role in COVID-19 coagulopathy development.

    Abstract Background Intraoperative motor-evoked potential (MEP) monitoring reduces postoperative motor deficits. Propofol-based total intravenous anesthesia is the gold standard for intraoperative myogenic MEPs. Although there is no contraindication to administering propofol in adults with peanut, soy, or egg allergies, its safety in children with these allergies remains unclear. Case presentation A 12-year-old girl required general anesthesia under intraoperative direct cortical MEP (dc-MEP) monitoring due to supratentorial glioma. Remimazolam-based anesthesia was selected, instead of propofol, due to the patient’s egg hypersensitivity. Stable myogenic MEPs were recorded throughout the surgery with remimazolam at 0.9 mg/kg/h and remifentanil at 0.35 μg/kg/min, following adjustments of stimulation intensity and titration of remimazolam infusion. Neither intraoperative memory nor motor deficits were present after surgery. Conclusions We present a pediatric case whose dc-MEP was recorded under remimazolam anesthesia. The cardiovascular stability and avoidance of propofol infusion syndrome with remimazolam were superior to propofol.

    PURPOSE: Clear visualization of ultrasound (US) images is crucial for successful US-guided nerve block. However, accurate determination of local anesthetic (LA) distribution from US images remains difficult. Sonazoid®, which comprises perflubutane microbubbles, is used to diagnose hepatic and breast tumors. This study aimed to investigate the visibility of Sonazoid® in perioperative US-guided nerve block. METHODS: We performed rectus sheath block (RSB) in patients scheduled for laparoscopic abdominal surgery (n = 10). 10 mL of a mixture containing equal amounts of 0.75% ropivacaine and iohexol with the addition of Sonazoid® diluted 100-fold was administered. We investigated the correlation and agreement between Sonazoid® and iohexol distributions. The brightness of the solution and tissues was calculated: a grayscale value between 0 (dark) and 255 (bright) was measured in all pixels of the region of interest. Adverse events were also investigated. RESULTS: Sonazoid® was clearly visualized and distinguished from the surrounding tissues both during and after RSB. The spread of Sonazoid® and iohexol was significantly correlated (spearman's ρ = 0.53, p = 0.0004). Bland-Altman analyses revealed significant mean difference between two methods (15.6 mm; 95% confidence interval [CI]: 10.6, 20.6; standard deviation (SD) 15.65; p < 0.0001). Limits of agreement were - 14.94 to 46.24 mm. Sonazoid® significantly increased the mean grayscale values at the posterior rectus sheath (93.7 vs. 201.9, p < 0.0001). There were no complications. CONCLUSION: Sonazoid diluted 100-fold® was clearly visualized real-time, and the enhancement was sustained and measurable after RSB. Sonazoid® could potentially be used for the contrast agent of US-guided nerve block.

    Oxytocin is known as a social bonding hormone, but it also functions as an anxiolytic or analgesic neurotransmitter. When oxytocin regulates pain or anxiousness centrally as a neurotransmitter, it is secreted by neurons and directly projected to targeted regions. Although the function of oxytocin at the spinal level is well studied, its effects at the supraspinal level are poorly understood. We aimed to investigate the effect of oxytocin at the supraspinal level in vivo using C57BL/6J (wild-type [WT]), oxytocin-deficient (Oxt-/-), oxytocin receptor-deficient (Oxtr-/-), and oxytocin receptor-Venus (OxtrVenus/+) mice lines. Response thresholds in Oxtr-/- mice in Hargreaves and von-Frey tests were significantly lower than those in WT mice, whereas open field and light/dark tests showed no significant differences. Moreover, response thresholds in Oxt-/- mice were raised to those in WT mice after oxytocin administration. Following the Hargreaves test, we observed the co-localisation of c-fos with Venus or the oxytocin receptor in the periaqueductal gray (PAG), medial amygdala (MeA), and nucleus accumbens (NAc) regions in OxtrVenus/+ mice. Furthermore, in the PAG, MeA, and NAc regions, the co-localisation of oxytocin with c-fos and gamma-aminobutyric acid was much stronger in Oxtr-/- mice than in WT mice. However, following von-Frey test, the same findings were observed only in the MeA and NAc regions. Our results suggest that oxytocin exerts its analgesic effect on painful stimulation via the PAG region and a self-protective effect on unpleasant stimulation via the MeA and NAc regions.

    Postoperative pain and consequent inflammatory responses after tissue incision adversely affects many surgical patients due to complicated mechanisms. In this study, we examined whether activation of protease-activated receptor 2 (PAR-2), which is stimulated by tryptase from mast cells, elicits nociception and whether the PAR-2 antagonist could reduce incisional nociceptive responses in vivo and in vitro. The effects of a selective PAR-2 antagonist, N3-methylbutyryl-N-6-aminohexanoyl-piperazine (ENMD-1068), pretreatment on pain behaviors were assessed after plantar incision in rats. The effects of a PAR-2 agonist, SLIGRL-NH2, on nociception was assessed after the injection into the hind paw. Furthermore, the responses of C-mechanosensitive nociceptors to the PAR-2 agonist were observed using an in vitro skin-nerve preparation as well. Intraplantar injection of SLIGRL-NH2 elicited spontaneous nociceptive behavior and hyperalgesia. Local administration of ENMD-1068 suppressed guarding behaviors, mechanical and heat hyperalgesia only within the first few hours after incision. SLIGRL-NH2 caused ongoing activity in 47% of C-mechanonociceptors in vitro. This study suggests that PAR-2 may support early nociception after incision by direct or indirect sensitization of C-fibers in rats. Moreover, PAR-2 may play a regulatory role in the early period of postoperative pain together with other co-factors to that contribute to postoperative pain.

    Chronic postsurgical pain (CPSP) is a serious issue for many postoperative patients. Though there are numerous treatment options for the prevention of CPSP, none of them is optimal as the mechanisms of the transition from acute to chronic postoperative pain have not been elucidated. Ketamine and opioids have been administered for chronic postoperative pain treatment but induce severe adverse reactions and/or physical dependency. Here, we examined whether pre-administration of the nonselective N-methyl-d-aspartate (NMDA) receptor antagonist magnesium sulfate attenuates CPSP behavior and alters the expression of glutamate ionotropic receptor NMDA type subunit 1a (Grin1 mRNA) in a rat skin/muscle incision and retraction (SMIR) model. We assessed the effects of a single subcutaneous magnesium sulfate injection on nociceptive behaviors including guarding pain, mechanical hyperalgesia, and heat hypersensitivity in rats after SMIR surgery. We used reverse transcription-quantitative PCR (RT-qPCR) to evaluate Grin1 mRNA expression in the dorsal horn of the spinal cord on postoperative day 14. Compared with the vehicle, magnesium sulfate administration before SMIR surgery reduced mechanical hyperalgesia for 17 d Grin1 gene expression was significantly higher on the ipsilateral side than the contralateral side (P = 0.001) on postoperative day 14. The magnesium sulfate injection prevented Grin1 mRNA upregulation in the spinal cord dorsal horn. A single magnesium sulfate injection mitigated SMIR-induced mechanical hyperalgesia possibly by modulating Grin1 expression. Preoperative magnesium sulfate administration could prove to be a simple and safe CPSP treatment.

    Inflammatory bowel disease (IBD) is a group of chronic inflammatory disorders of the gastrointestinal tract including ulcerative colitis (UC) and Crohn's disease. Pain management can be challenging in patients with IBD because there are limitations on the use of analgesics. Use of nonsteroidal anti-inflammatory drugs is not recommended in patients with IBD because there is risk of relapse of IBD and an overall increase in disease activity. Opioids, although frequently used for treating severe acute pain, can have additional risks and complications in patients with IBD such as ileus, toxic megacolon, and narcotic bowel syndrome. Furthermore, little information is available in the literature on pain management in these patients undergoing noncolorectal surgery. This report describes 2 patients with UC in whom postoperative pain following oral and maxillofacial surgery was managed by intravenous patient-controlled analgesia with pentazocine. Apart from the development of acute dystonia in 1 case that was likely due to the use of droperidol for prevention of postoperative nausea and vomiting, postoperative pain was well controlled by pentazocine in both patients without any complications or UC exacerbations.

    BACKGROUND: Wilms' tumor with hyperreninemia may result in critical cardiovascular decompensation. We report a case of severe hypertensive heart failure followed by tumor resection in a 3-month-old infant with Wilms' tumor. CASE PRESENTATION: A 3-month-old girl was admitted to the intensive care unit for Wilms' tumor with hypertension and hypoxia. Her systolic blood pressure was 110 mmHg, and her SpO2 was 92%. She presented with severe hypertensive heart failure and received mechanical ventilation and antihypertensive therapy for hypertension and heart failure. An alpha 2-adrenergic receptor agonist was used for sedation as part of her antihypertensive therapy. On hospital day 16, nephrectomy with tumor resection was performed under general anesthesia. Her systolic blood pressure did not vary more than 20 mmHg during surgery due to appropriate preoperative management. Hemodynamic collapse did not occur. CONCLUSIONS: The highlight of this case report is the successful management of an infant with Wilms' tumor, particularly with respect to preoperative hemodynamic control and sedation.

    The impact of rare and damaging variants in genes associated with platelet function in large‑vessel ischemic stroke (LVIS) remains unknown. The aim of this study was to investigate the contribution of some of these variants to the genetic susceptibility to LVIS in Polish patients using a deep re‑sequencing of 54 selected genes, coding for proteins associated with altered platelet function. Targeted pooled re‑sequencing (Illumina HiSeq 2500) was performed on genomic DNA of 500 cases (patients with history of clinically proven diagnosis of LVIS) and 500 age‑, smoking status‑, and sex‑matched controls (no history of any type of stroke), and from the same population as patients with LVIS. After quality control and prioritization based on allele frequency and damaging probability, individual genotyping of all deleterious rare variants was performed in patients from the original cohort, and stratified to concomitant cardiac conditions differing between the study and stroke groups. We demonstrated a statistically significant increase in the number of rare and potentially damaging variants in some of the investigated genes in the LVIS pool (an increase in the genomic variants burden). Furthermore, we identified an association between LVIS and 6 rare functional and damaging variants in the Kv7.1 potassium channel gene (KCNQ1). The predicted functional properties (partial loss‑of function) for the three most damaging variants in KCNQ1 coding locus were further confirmed in vitro by analyzing the membrane potential changes in cell lines co‑transfected heterogeneously with human muscarinic type 1 receptor and wild‑type or mutated KCNQ1 cDNA constructs using fluorescence imaging plate reader. The study demonstrated an increased rare variants burden for 54 genes associated with platelet function, and identified a putative role for rare damaging variants in the KCNQ1 gene on LVIS susceptibility in the Polish population.

    Epistaxis is one of the most common complications of nasotracheal intubation and can be life-threatening. However, there is little discussion in the current literature on the acute management of massive epistaxis after nasotracheal extubation. This is a report of 2 patients who experienced severe unanticipated nasal bleeding immediately after extubation, 1 after a surgical procedure for oral cancer and another after restorative dental treatment. In both cases the significant epistaxis was managed successfully with a Foley balloon catheter used to pack the posterior nasal cavity. The Foley catheter technique may be useful for managing and arresting sudden postextubation epistaxis.

    Purpose: Remifentanil is associated with acute opioid tolerance that can lead to increased postoperative consumption of opioid analgesics. The purpose of this study was to determine whether a low dose of ketamine prevents remifentanil-induced acute opioid tolerance and affects the neutrophil-lymphocyte ratio (NLR), a newly recognized biomarker of inflammation. Materials and methods: Forty patients undergoing orthognathic surgery were enrolled in this prospective, randomized, double-blind study and randomly assigned to intraoperative administration of one of the following anesthetic regimens: high-dose remifentanil (0.6 µg/kg/minute); low-dose remifentanil (0.2 µg/kg/minute); or high-dose remifentanil with ketamine (remifentanil 0.6 µg/kg/minute with 0.5 mg/kg ketamine just after induction followed by an intraoperative infusion of ketamine 5 µg/kg/minute until wound closure). Fentanyl by intravenous patient-controlled analgesia was used for postoperative pain control. Visual Analog Scale pain scores and fentanyl consumption were recorded in the first 24 hours postoperatively. Perioperative serum C-reactive protein level and NLR were also determined. Results: Baseline characteristics were similar in the three study groups. There were no between-group differences in Visual Analog Scale pain scores during the study period. The high-dose remifentanil group had a significantly higher requirement for fentanyl than the other two groups. Addition of ketamine did not affect the C-reactive protein level but increased the NLR; this increase was associated with decreased fentanyl consumption. Conclusion: High-dose intraoperative remifentanil induced postoperative acute opioid tolerance that was prevented by infusion of low-dose ketamine. Ketamine increased the postoperative NLR associated with decreased fentanyl requirement for postoperative pain control.

The predicted properties of the most damaging rare variants in P2RY1 and P2RY12 were confirmed by using mouse fibroblast cell cultures transfected with plasmid constructs containing cDNA of mutated variants (FLIPR on FlexStation3). This study identified a putative role for rare variants in P2RY1 and P2RY12 genes involved in platelet reactivity on large-vessel IS susceptibility in a Polish population.

    FcγRII and FcγRIII are low-affinity Fcγ receptors that are encoded by the FCGR2A and FCGR3A genes, respectively. These genes contain functional single-nucleotide polymorphisms (SNPs), which alter the binding affinities of these receptors for the γ chain of the Fc fragment of immunoglobulin G. The known SNPs in FCGR2A and FCGR3A are rs1801274 (A>G; H131R) and rs396991 (T>G; F158V), respectively. It is also known that there are copy number variations (CNVs) in the genetic locus (1q23) where FCGR2A and FCGR3A are located. However, the frequencies of these SNPs and CNVs have not been determined in the Japanese population. The aim of this study was to investigate SNPs and CNVs in FCGR2A and FCGR3A among 113 healthy individuals. The SNPs and CNVs in FCGR2A and FCGR3A were determined using the TaqMan® SNP Genotyping and the TaqMan® Copy Number assays. Our results revealed that the incidence of FCGR2A (rs1801274) genotypes were as follows: A/A, 69.9%; A/G, 29.2%; and G/G, 0.9%. The incidence of the FCGR3A (rs396991) genotypes were as follows: T/T, 56.7%; T/G, 38.9%; and G/G, 4.4%). No CNVs were detected for FCGR2A. To the best of our knowledge, this finding has not been previously reported in the Japanese population. By contrast, CNVs were observed in FCGR3A (3 subjects were found to harbour a gene deletion and 5 subjects had 3 copies of the gene). Using simple commercially available assays we were able to confirm previous findings regarding FCGR2A and FCGR3A alleles and CNVs. These assays may provide a basis for the investigation of the role of these genes in the efficacy of antibody-based drugs, such as trastuzumab and rituximab, in Japanese subjects.

    A 60-year-old woman declared brain dead was scheduled for organ donation. We continuously measured total hemoglobin values (SpHb) using a Radical-7 monitor (Masimo Co, Irvine, CA, USA) to maintain the functions of organs and oxygen delivery. At the start of surgery, the SpHb value was 9.3 g x dl(-1). Packed red blood cells were transfused immediately No anesthetics or opioids were used during the operation. Blood pressure suddenly decreased to below 80 mmHg because of bleeding, manipulation of organs, and/or compression of the vena cava. Six units of red blood cells and 900 ml of colloids were rapidly transfused with real-time monitoring of SpHb values. On cross-clamping of the aorta, the SpHb value increased up to 10.2 g x dl(-1). The heart, lungs, liver, pancreas, and kidneys were donated from the patient without organ dysfunction. The highlight of this case report is that anesthesiologists could use SpHb monitoring for management of hemodynamics in a brain-dead organ donor.

    A 60-year-old man with angina was scheduled for total gastrectomy, splenectomy, and cholecystectomy. Bare-metal stents were implanted into his left anterior descending coronary artery four weeks before the operation. Aspirin and clopidogrel were administered until one week before the operation and then injection of to 15,000 units of heparin per day was given. Anesthesia was maintained with sevoflurane, remifentanil and fentanyl. At 330 minutes after starting the operation, 2-mm ST segment elevation was observed and it recovered immediately. After the operation, new 9-mm ST segment elevation in leads V2-V6 was observed. Emergent cardiac catheterization showed occlusion of the coronary artery with in-stent thrombosis. An additional stent was implanted and 10,000 units of heparin per day was injected. After five days, new stent thrombosis occurred and an additional stent was implanted. Administration of aspirin, clopidogrel and cilostazol was started immediately. Anesthesiologists should pay attention to the kind of coronary stent, consider the timing of the operation, and continue administration of aspirin.

    60歳代女。自宅で意識消失し、高度救命救急センターに搬入された。頭部CTで、くも膜下出血とびまん性脳浮腫を認めた。第2病日には、脳幹反射の消失、平坦脳波、さらに尿崩症の出現を認め、臨床的に脳死と判断した。臓器提供意思表示カードの所持はなかったが、家族に臓器提供の意思があったため、2回の法的脳死判定後、第4病日に脳死下臓器摘出術を予定した。麻酔導入時の動脈血液ガス分析装置を用いたヘモグロビン値は、目標値である10g/dlを下回っていたため、ただちに赤血球濃厚液の投与を開始した。大動脈遮断後はすべての輸液・薬物の投与と人工呼吸を中止し、中心静脈カテーテルを抜去した後、心臓、肺、肝臓、膵臓、ならびに腎臓を摘出し、気管チューブを抜去した。

    60歳男。吐血で救急搬送され、緊急開腹止血術を行った。病理検査により出血の原因は胃癌と診断され、胃全摘術が予定された。術前の運動負荷心電図でST変化を認め、冠動脈造影を行ったところ3枝変化を認めたため、左前下行枝に2本のbare-metal stent(BMS)を留置した。留置後3週間はアスピリンとクロピドグレルを服用し、その後手術開始直前まで未分化ヘパリンを持続静注した。第92病日に全身麻酔下で胃・脾・胆嚢摘出術を行い、手術開始5時間半経過ごろに心電図でST変化を数回認めた。約6時間で手術終了し、術後1時間に行った12誘導心電図ではV2-6で最大9mmのST上昇を認め、ステント血栓症を疑って緊急冠動脈造影を行った。BMSの末梢から先が血栓で閉塞しており、血栓を吸引しBMSを留置した。しかし、5分後の再造影で血栓を認め、BMSを再留置した。術後ヘパリンを使用したが、第97病日に再び血栓症を生じ、BMS再留置となった。同日よりアスピリン、クロピドグレル、シロスタゾールを開始し、その後血栓症はなく、第149病日に退院した。

    初期臨床研修医が小児症例および肥満症例の硬膜外麻酔を、超音波画像(エコー)を用いることでスムーズに行った症例を経験した。症例1(11歳女児)。右距骨骨軟骨損傷に対し観血的骨接合術を予定した。麻酔は全身麻酔とL5-S1棘間の硬膜外麻酔の併用とした。良好な鎮痛効果を得ることができた。硬膜外穿刺および麻酔管理上の合併症はなかった。症例2(36歳女)。腹式子宮筋腫核出術を予定した。前投薬は施行せずに手術室入室とした。麻酔方法は全身麻酔とT12-L1からの硬膜外麻酔の併用とした。術後はフルルビプロフェンの経静脈投与を一度行ったのみで、副作用や合併症はなかった。

    2歳4ヵ月男児。出生後からRDS、気管支痙攣、慢性肺疾患、副腎不全のため小児科でフォローされており、今回、尿道下裂に対して尿道形成術を予定した。前投薬としてトリクロホスナトリウムを服用させ、酸素・亜酸化窒素・セボフルランによる緩徐導入とベクロニウム静脈内投与で気管挿管を行った後、右側臥位として仙骨硬膜外穿刺を行った。仙骨裂孔を触診で確認し、抵抗消失法で18GのTuohy針を進め、皮膚から2cmで硬膜外腔に到達した。他の麻酔医が5〜10MHzリニア型超音波プローブを用いてL5/S1の水平断像を描出し、穿刺者が硬膜、黄色靱帯を観察しながら硬膜外カテーテルを挿入した。カテーテルを硬膜外に11cm進めた時、カテーテルの動きに同期したL5/S1の動きを確認し、その部位に留置した。硬膜外腔へは20万倍アドレナリン添加1%リドカインを執刀20分前に8ml、手術開始75分後、140分後に4mlずつ投与した。鎮痛効果は良好で、手術は問題なく終了した。

    症例は66歳の男性で、腹部大動脈瘤に対するステントグラフト内挿術が予定されたが、63歳時から肺気腫に対する在宅酸素療法中であった。検査により、呼吸機能は最重症であると判定され、麻酔は脊髄クモ膜下麻酔と浸潤麻酔を選択した。術中には鎮静剤を使用せず、経鼻にて酸素投与を行い、周術期における呼吸状態の悪化は認められなかった。手術時間は3時間30分、麻酔時間は4時間40分であった。

    63歳女、Bowen病と診断され、肛門部周囲切除と人工肛門造設が予定された。C型肝炎、子宮体癌に対する子宮全摘術の既往があった。全身麻酔導入後、感染予防目的にセフメタゾール(CMZ)を点滴静注した。5分後、急激な血圧低下、顔面・手掌の発赤・浮腫を認め、CMZによるアナフィラキシーショックが疑われた。直ちにCMZを中止し、血圧低下に対して細胞外液、エピネフリンの静脈内投与、ドパミン投与を行った。気道内圧上昇を認めたため、テオフィリンを30分間持続投与した。さらにメチルプレドニゾロン、クロルフェニラミンを静注した。手術は人工肛門の作成に留め終了した。ショック発症70分後には顔面・手掌の発赤・浮腫は消退した。ショック状態が遷延したため、術後気管挿管されたままICU入室となり、離脱に2日を要した。術後3日目に経過良好でICU退室となり、一般病棟に転床した。本症例は「抗菌薬投与に関連するアナフィラキシー対策のガイドライン」に基づき迅速に対応することが可能であった。

    5歳3ヵ月,女児.出生時に顔面の変形,心奇形(心室中隔欠損,僧帽弁閉鎖不全),左母指低形成を指摘されGoldenhar症候群と診断された.左母指対立筋再建術を予定した.慢性副鼻腔炎で通院中であった.顔貌は左前額,下顎の低形成を認めたが,ほぼ正常でマスク保持は可能であると予想された.麻酔は全身麻酔を選択した.気道確保はPLAを用いることとした.十分な酸素化ののち,チアミラールを投与した.入眠後,マスクの保持は可能であった.亜酸化窒素とセボフルラン5%で十分に麻酔が深くなったあと,ビデオ喉頭鏡で試験的に咽頭内を観察した.声帯はディスプレー下で観察可能であり,緊急時に気管挿管は可能であると思われた.術中,著変を認めず,吸入麻酔薬を中止後,覚醒は速やかで,すぐにPLAを抜去することができた.術後の咽頭痛は認めなかった.術後は問題なく経過した

    53歳男性.アセチルサリチル酸(ASA)と大量バルプロ酸ナトリウム(VPA)の服用後に重症急性膵炎と急性肝不全を生じた1例を経験した.自宅で妻とともに倒れているのを発見され救急搬入(妻は他院に)された.意識レベルはJCS300,嘔吐,尿失禁を認め,検査で高ナトリウム血症,高乳酸血症,高アンモニア血症を認めた.搬入8時間後に妻へのVPA処方が判明した.患者は循環血液量減少性ショック状態となり,ヒドロキシエチルデンプン投与で血圧は上昇した.搬入6時間後の血液検査でVPA血中濃度1555μg/ml,ASA濃度114μg/mlでVPA中毒と診断し,直接血液灌流(DHP)と血液濾過透析(HDF)を施行し,VPA血中濃度237μg/mlに低下し,意識レベルはJCS100まで改善した.第2病日に膵酵素と肝逸脱酵素の上昇,DICを認め,CTの肝部分壊死と膵全体壊死像から重症急性膵炎と診断した.HDF等の諸治療に反応せず,第3病日多臓器不全で死亡した

    73歳男.大量下血をきたし,搬入時には出血性ショックを呈していた.腹部造影CTおよび血管造影にて,内腸骨動脈瘤破裂とその下部消化管穿破を疑い,止血と再破裂予防のため,2度の経カテーテル動脈塞栓術を施行した.その後イレウス症状が継続,術後5日目の腹部造影CTにて腹腔内遊離ガスを認め,緊急開腹術を施行した.術中所見にて直腸前壁に穿孔を認め同部位を切除,直腸断端を閉鎖し人工肛門を造設した.その4日後には腹腔内ドレーンから便臭を伴う多量の排液を認め,再開腹下に直腸を再切断し,腹腔内洗浄ドレナージ術を施行した.術中所見にて直腸断端部の離解を認め,腹膜炎を来したものと考えられた.術後,多臓器不全の合併なくICUを退室,その後数度の腹腔内膿瘍穿刺を要したが第48病日に外出可能となった.しかし翌日に右下腹部痛が出現,第50病日には救急車で搬送され,センター到着時には心肺停止状態であった.内腸骨動脈瘤再破裂による出血性ショックと思われ,心拍再開せず永眠した

    8年6ヵ月間に鈍的腹部外傷に対して経カテーテル的動脈塞栓術(TAE)を施行した25例について検討し,urban areaと状況が違うrural areaでのTAEの適応範囲の拡大を提言した.Injury Severity Score(ISS)の平均は32であり,高エネルギー外傷が多く,緊急TAEとなった17例のうち12例でTAE前に輸血ができなかった.初期輸血に反応しなかった8例中,5例でTAEにより救命でき,3例でTAE後に外科的止血術を加えた.術中の血圧,心拍数は安定していた.rural areaでは輸血の準備に時間を要するため,循環動態が不安定な症例はTAEを施行し開腹止血術を行うことが最善であると考えられた

    68歳女.右肺癌に対し,胸腔鏡下肺切除術(VATS)を予定した.血圧は150/68mmHgと高値で,胸部X線検査では分岐異常は不明瞭であった.しかし,胸部CTで気管分岐異常が指摘されていたため,前投薬としてゾピクロンを経口投与し,心電図,非観血的血圧,観血的動脈圧,SPO2をモニターし,手術室入室後,T4/5より硬膜外カテーテルを挿入留置した.プロポフォール80mgを静注投与で導入し,ベクロニウムを用い,左用二腔気管チューブを挿管し,.挿管後,気管支ファイバーで気管内を観察し,気管分岐部手前1cmの部位に右上葉支が確認された.腫瘍は右上葉であったため,挿管チューブを深めに留置しカフで閉塞した.硬膜外腔へ1.5%リドカイン間欠投与,酸素33%,亜酸化窒素66%,セボフルラン0.1〜1.6%で維持し,分離肺換気中は低酸素血症に陥ることもなく,麻酔時間8時間30分,出血量500mlで手術を終えた

    14歳男.釣りの準備中に突然意識を消失し海へ転落した.水没後約15分で救助されたが,救助者による心肺蘇生は施行されなかった.水温は9℃であった.救急隊が到着した時は心肺停止状態であり,直ちに一次救命処置を開始して搬送された.二次救命処置及び復温を施行し,エピネフリン,硫酸アトロピン,リドカインを投与して,搬入45分後に心拍が再開してICUに収容した.蘇生後の神経学的回復を期待し,前額温が30.2℃,直腸温が32.9℃と低体温でもあったことから,復温せずに軽度脳低温療法に移行した.第2病日には対光反射が出現した.第3病日の頭部CTには軽度の脳浮腫を認めたが頭蓋内占拠性病変はなかった.頭部MRIではT2強調画像で大脳基底核に高信号域を認めた.第4病日より復温を開始した.4ヵ月後の現在,会話は支障なく車椅子で移動できる程度に回復した

    65歳女.CMTDを有する患者の腹臥位手術を,プロポフォールと亜酸化窒素と少量のフェンタニルを用いて麻酔管理を行った.腹臥位であり気管挿管は必須と考えた.亜酸化窒素は静脈麻酔薬の減量と鎮痛作用を期待して用いた.術前の呼吸機能は良好であったが,鎮静薬投与による呼吸抑制を避ける為,前投薬は用いなかった.術中は著変なく経過したが,腹臥位であり気管管理には十分な注意が必要であった.本症例においてもプロポフォール持続投与を中止して10分後に抜管できる状態になった.覚醒が速やかで,全身麻酔維持にも使用できるプロポフォールはこのような神経筋疾患に対して有用と考えられた

    In the present study, obese rats induced by high-fat diet intake had enhanced and prolonged postoperative pain including elevated spontaneous pain score and lower mechanical threshold. Transcriptome analysis of peripheral and central nervous tissue revealed no major gene expression changes in the spinal cord and 19 gene expression changes in the dorsal root ganglia. Increased peripheral sensitization by genes related to lipid metabolism was suggested as one of the mechanisms by which obesity enhances postoperative pain.

    Chronic postsurgical pain (CPSP) is an unpleasant complication after surgery. However, the molecular genetic mechanisms underlying CPSP have not been fully elucidated. The aims of this study were to determine the whole genome-wide change in long non-coding RNA gene expression in the rat spinal cord and dorsal root ganglions and to try to determine the involvement of long non-coding RNAs in CPSP. The results of a behavioral study showed that pain-related behaviors persisted for over 60 days in the CPSP model rat but not in sham-operated rats. Whole transcriptome sequencing showed that AABR07007055.1 gene expression was significantly increased in both spinal cord and dorsal root ganglions of the CPSP model. We focused on AABR07007055.1 gene as a candidate associated with CPSP. The findings suggested that the change in AABR07007055.1 gene expression level may be involved in the molecular mechanisms underlying the development of CPSP.

    Postoperative nausea and vomiting (PONV) is a serious complication after emergence from general anesthesia. However, previous findings are not widely applicable to different populations because of population-specific genetic variation. We developed a Japanese-specific DNA microarray for high-throughput genotyping. The aim of the current study is to identify causal SNPs associated with PONV on a genome-wide scale using this microarray in a sample of Japanese surgical patients. The results of the study showed that T > C variant of rs11232965 in the long non-coding RNA MIR4300HG was significantly associated with reduced incidence of PONV among genotypes and between alleles (p = 0.01 and 0.007). The findings suggested that the rs11232965-SNP variant (T > C) is protective against the incidence of PONV.

    I constructed integrated big data in the intensive care unit (ICU) of Tohoku University Hospital, analyzed the relationship between the severity and the cost of each patient in the ICU. I planned to develop the original algorithm to derive the optimal treatments that would achieve optimal medical resource distribution and maximum medical benefits, depending on the changes in each patient's condition of severity. I set up a server to construct panel database. In regards to the patient’s severity, I gathered the ICU system data, and bed-side monitoring data of each patients admitted to our ICU for the past five years. In regards to the cost, I gathered material cost, labor cost, expense data, medical record and DPC/PDPS data with the cooperation of the university hospital medical affairs department. I cleaned and integrated the all above data, transferred to my server, and constructed the panel database. Finally, I calculated departmental costing and cost forecasting with this database.

    平成28年度は遺伝子実験の環境整備と陽性対照実験の実施のみで終了した.分子遺伝実験環境に問題がないことを確認できた.平成29年度は詳細な研究プロトコルを最終決定し,プロトコルのみを論文(プロトコル論文)としてOpen Accessジャーナルへ投稿・発表予定であった.しかし研究協力者が本務多忙につき,論文投稿までには至らなかった.研究期間の半ばを過ぎてもデータ獲得が為されていないこの危機的状況から脱するため,われわれは大きな研究戦略の変更を行った.まず新たな研究戦略の方向性を模索するため,平成29年10月に米国麻酔学会(マサチューセッツ州,ボストン市)へ出張し,いくつかの着想を得た.当初の予定はDNAマイクロアレイで少数のサンプルを用いた疑似ゲノムワイド関連解析を行う予定であったが,過去に取得したデータのインシリコ解析を最初に行うことでゲノム上の対象領域を狭めて遺伝子解析を行う方針とした.しかし,あまり有意な知見は得られなかった.また臨床データ取得のための研究協力者が本務多忙な状況は依然として変わらず,新たに協力者を探したが適任者が見つからなかった.研究期間内(平成31年3月まで)に解析に値する臨床データ,遺伝データを得ることはできないと考え,研究の遂行中止を決断した.そのため科学研究費の補助をこれ以上,受益するわけにはいかないため,日本学術振興会への補助金返上を最後に行った.研究は研究期間内に遂行できなかったが,あらためて研究資金を獲得できれば再開する予定である.

    Celiac-superior mesenteric ganglion (CSMG) is involved in regulating abdominal organ function, and is also involved in visceral pain transmission. We investigated the modulation of Ca2+ and M-type K+ currents by the tachykinin substance P (SP)/neurokinin-1 receptor signaling pathway in acutely dissociated rat CSMG neurons (with Prof. Ruiz-Velasco at Penn State University). The results of an electrophysiological study showed that SP blocked the Ca2+ and M-currents. Silencing the Gα14 subunit, a member of the Gαq/11 subfamily, significantly attenuated the Ca2+ and M-current blocks. The results suggest that in CSMG neurons, SP-stimulated NK-1 receptors modulate Ca2+ and M-currents by employing a single pathway requiring Gα14 subunits. Moreover, to identify genetic mechanisms in the pathway, we have developed a single-cell whole transcriptome sequencing method using a neuron after electrophysiological recording. We will find changes in the transcriptome of the neurons in near future.

    Postoperative nausea and vomiting (PONV) is a serious complication after emergence from general anesthesia. However, the molecular mechanisms underlying PONV have not been fully elucidated. Musk shrews are used for emetic research worldwide because standard laboratory animals, such as rats and mice, are not capable of vomiting. The aims of this study were to determine changes in gene expression in the shrew brainstem and to try to determine the causative genes for PONV. The results of a behavioral study showed that emetic episodes occurred in the PONV model shrews but not in naive shrews. Whole transcriptome sequencing showed that EGR1 gene expression levels in the gonadotropin-releasing hormone signaling pathway were significantly altered in the PONV model. We focused on EGR1 transcription factor as a candidate associated with PONV. The findings suggested that the change in EGR1 gene expression level may be involved in the molecular mechanisms underlying the development of PONV.

    We investigated whether volatile anesthetics caused postoperative cognitive dysfunction (POCD). In humans, desflurane anesthesia showed a better mini mental examination score than did sevoflurane anesthesia. However, we could not find any genetic polymorphisms associated with cognitive function in humans. To examine the association between general anesthesia and POCD, we investigated whole mRNA expression in mouse hippocampal cells by a transcriptome-wide association study. While expression of the Lhx9 gene was suppressed by exposure to volatile anesthetics in juvenile mice, expression of Lhx9 gene was greatly enhanced by exposure to volatile anesthetics in adult mice. These results suggest that volatile anesthetics affect the differentiation of stem cells in the mouse hippocampus, and the results should be useful for exploring biomarkers of POCD.

    We performed a transcriptome-wide association study, a total mRNA analysis, in the mouse brain to try to determine the molecular mechanism of volatile anesthetics. We also examined the behavior of mice to confirm the association between inhalation of volatile anesthetics and undesirable side effects such as nausea, vomiting, and cognitive dysfunction. Exposure to volatile anesthetics caused an increase in the expression level of the Rtn4rl2 gene in all mice and an increase in the expression level of the Lhx9 gene in aged mice. These results indicated that inhalation of volatile anesthetics causes neuroinflammation and induces apoptosis in juvenile mice. However, we did not find undesirable side effects of exposure to the volatile anesthetics in any of the mice. The results suggest that the mechanism by which exposure to volatile anesthetics causes nausea or cognitive dysfunction in mice is different from that in humans.

    We hypothesized that regulation of the TACR1 gene is involved in the individual differences in severity of postoperative nausea and vomiting (PONV). We examined CpG methylation of the promoter region of the TACR1 gene, which modulates NK1 receptor expression in both human blood and mouse the brain. The results of the current study showed that the level of methylation was inversely correlated with the intensity of PONV. This new finding may contribute to preoperative prediction of the severity of PONV in surgical patients.

    We hypothesized that the genetic polymorphisms in the GABRB3 gene are associated with the individual differences in concentration of propofol at loss of consciousness. We examined the genotypes of single nucleotide polymorphisms (SNPs) in the coding sequence of the GABRB3 gene. However, none of the genotyped SNPs were associated with the concentration at loss of consciousness. These results raised the possibility that we need to change the experimental design or to increase the size of the cohort.

    We studied the individual differences in postoperative pain and opioid sensitivity. We focused on the single nucleotide polymorphisms (SNPs) of oprm1 gene. We showed the associations of opioid-induced emesis with haplotypes of eight SNPs of oprm1 gene. This new finding may contribute to prediction of the occurrence of nausea and vomiting in the preoperative period.

    本研究では延髄の疼痛伝達における制御中枢であるRVMの機能を行動学的,電気生理学的に詳細に検討した。特に近交系マウスの系統(CBA/JとA/J)間で行動学的研究と電気生理学的研究の機能特性に差があることが示された。さらに注目すべきことに,RVMニューロンのタイプは電気生理学的に3種類存在するが,2系統間で存在する割合に差があることが判明した。さらにmu-opioidレセプタ作動薬であるDAMGOを脳室内投与したが,機能特性の変化に差があることが明らかになった。本研究から疼痛やオピオイドの反応性の個人差を予測するための研究で重要なのは神経ネットワークの機能の遺伝的差異が重要であることが強く示唆された。 今後,本研究結果の機序を分子生物学的に検討するために,mu-opioidレセプタの遺伝子多型をターゲットにして研究を進める予定である。同時に手術患者におけるオピオイド鎮痛の差異と遺伝的差異の関連を検討する予定である。
