    OBJECTIVES: This study evaluated and compared the effectiveness of various predictive models for forecasting occult lymph node metastasis (LNM) in tongue squamous cell carcinoma (TSCC) patients. METHODS: In this retrospective diagnostic experiment, 268 patients were recruited from three medical centers. Based on the different hospitals from which the patients were recruited, they were divided into a training set, an internal testing set, and two external testing sets, comprising 107, 53, 63, and 45 patients, respectively. Several predictive models were developed using patients' contrast-enhanced magnetic resonance imaging (CEMRI), including two-dimensional deep learning (2D DL), conventional radiomics (C-radiomics), and intratumoral heterogeneity radiomics (ITH-radiomics). Univariate and multivariate logistic regression analyses were conducted on the clinical data. Finally, two fusion strategies were used to construct the model. RESULTS: The ITH-radiomics model exhibited superior discriminative power compared to C-radiomics model. The late fusion model had the highest area under the curve (AUC) across all test sets (0.81-0.85). Compared to the late fusion model, the AUC values for the early fusion, 2D DL, C-radiomics, and ITH-radiomics models in the test sets ranged from 0.77 to 0.82, 0.64 to 0.81, 0.66 to 0.77, and 0.77 to 0.80, respectively. Additionally, the late fusion model demonstrated the highest accuracy (76-89%) and specificity (87-100%) across the test sets. CONCLUSIONS: The evaluation of the models' effectiveness revealed that the decision-based late fusion model, which integrated 2D DL, C-radiomics, ITH-radiomics, and clinical data, achieved the best results. This predictive approach can more accurately assess patients' conditions and aid in selecting surgical plans. KEY POINTS: Question How well does fusing multiple models work for predicting occult lymph node metastasis in patients with tongue squamous cell carcinoma? Findings The late fusion model, incorporating two-dimensional deep learning, conventional-radiomics, intratumoral heterogeneity-radiomics, and clinical features, achieved the best results compared to each individual model. Clinical relevance Patients with a high intratumoral heterogeneity-radiomics index exhibit an increased risk of occult lymph node metastasis in tongue squamous cell carcinoma patients, which showed that the late fusion model achieves superior predictive performance compared to the early fusion model.

    OBJECTIVES: This study aims to develop an automated radiomics-based model to grade the pathological differentiation of head and neck squamous cell carcinoma (HNSCC) and to assess the influence of various magnetic resonance imaging (MRI) sequences on the model's performance. MATERIALS AND METHODS: We retrospectively analyzed MRI data from 256 patients across two medical centers, including both contrast-enhanced T1-weighted images (CET1WI) and T2-weighted images (T2WI). Regions of interest were delineated for radiomics feature extraction, followed by dimensionality reduction. An XGBoost classifier was then employed to build the predictive model, with its classification efficiency assessed using receiver operating characteristic curves and the area under the curve (AUC). RESULTS: In validation cohort, the AUC (macro/micro) values for models utilizing CET1WI, T2WI, and the combination of CET1WI and T2WI were 0.801/0.814, 0.741/0.798, and 0.885/0.895, respectively. The AUC for the three differentiations, ranging from well-differentiated to poorly differentiated, were 0.867, 0.909, and 0.837, respectively. The macro/micro precision, recall, and F1 scores of 0.688/0.736, 0.744/0.828, and 0.685/0.779 for the CET1WI + T2WI model. CONCLUSION: This study demonstrates that constructing a radiomics model based on CET1WI and T2WI sequences can be used to predict the pathological differentiation grading of HNSCC patients. CLINICAL RELEVANCE: This study suggests that a radiomics model integrating CET1WI and T2WI MRI sequences can effectively predict the pathological differentiation of HNSCC, providing an alternative diagnostic approach through non-invasive preoperative methods.

    Abstract Background Oral Squamous Cell Carcinoma (OSCC) presents significant diagnostic challenges in its early and late stages. This study aims to utilize preoperative MRI and biochemical indicators of OSCC patients to predict the stage of tumors. Methods This study involved 198 patients from two medical centers. A detailed analysis of contrast-enhanced T1-weighted (ceT1W) and T2-weighted (T2W) MRI were conducted, integrating these with biochemical indicators for a comprehensive evaluation. Initially, 42 clinical biochemical indicators were selected for consideration. Through univariate analysis and multivariate analysis, only those indicators with p-values less than 0.05 were retained for model development. To extract imaging features, machine learning algorithms in conjunction with Vision Transformer (ViT) techniques were utilized. These features were integrated with biochemical indicators for predictive modeling. The performance of model was evaluated using the Receiver Operating Characteristic (ROC) curve. Results After rigorously screening biochemical indicators, four key markers were selected for the model: cholesterol, triglyceride, very low-density lipoprotein cholesterol and chloride. The model, developed using radiomics and deep learning for feature extraction from ceT1W and T2W images, showed a lower Area Under the Curve (AUC) of 0.85 in the validation cohort when using these imaging modalities alone. However, integrating these biochemical indicators improved the model’s performance, increasing the validation cohort AUC to 0.87. Conclusion In this study, the performance of the model significantly improved following multimodal fusion, outperforming the single-modality approach. Clinical relevance statement This integration of radiomics, ViT models, and lipid metabolite analysis, presents a promising non-invasive technique for predicting the staging of OSCC.

    OBJECTIVES: Arrested pneumatisation (AP) is an anatomic variant of the sphenoid sinus. Since AP remains underrecognised, otolaryngologists and radiologists may mistake AP for a lesion and perform follow-up imaging studies. We investigated the imaging findings of CT, MRI, and F-18 fludeoxyglucose (FDG)-positron emission tomography (PET) for AP, and discussed the differences between AP and other skull base lesions. METHODS: We reviewed multidetector low CT imaging of 442 patients (285 men and 157 women; age range, 19-93 years; mean age, 67.8 years) who underwent FDG-PET/CT for head and neck tumours between January 2019 and December 2019. The imaging findings of AP were reviewed on CT, MRI, FDG-PET/CT, and compared with those of fibrous dysplasia, chordoma, chondrosarcoma, multiple myeloma, and bone invasion of nasopharyngeal carcinoma. RESULTS: AP was identified in 22 patients (14 men and 8 women; age range, 24-93 years; mean age, 67.0 years) based on criteria from previous reports. AP manifested with well-circumscribed sclerotic margins on CT, without evidence of expansion. AP showed high-signal intensity on T1-/T2 weighted MRI. FDG-PET revealed non-significant uptake [maximum standardised uptake value (SUVmax): 0.85 (range, 0.4-1.27)] in AP. Contrastingly, skull base lesions showed expansion, poorly circumscribed boundaries without osteosclerotic margins, and moderate-to-severe FDG uptake (SUVmax: 1.8-8.4). CONCLUSIONS: The characteristic imaging findings of AP, namely non-expansile on CT and non-uptake on FDG-PET, may aid in its differentiation from other skull base lesions.

    Children are sensitive to radiation; therefore, it is necessary to reduce radiation dose as much as possible in pediatric patients. In addition, it is crucial to investigate the optimal imaging conditions as they considerably affect the radiation dose. In this study, we investigated the effect of different imaging conditions on image quality and optimized the imaging conditions for dental cone-beam computed tomography (CBCT) examinations to diagnose ectopic eruptions and impacted teeth in children. To achieve our aims, we evaluated radiation doses and subjective and objective image quality. The CBCT scans were performed using 3D Accuitomo F17. All combinations of a tube voltage (90 kV), tube currents (1, 2, 3 mA), fields of view (FOVs) (4×4, 6×6 cm), and rotation angles (360°, 180°) were used. Dose-area product (DAP) values were measured. SedentexCT IQ cylindrical phantom was used to physically evaluate the image quality. We used the modulation transfer function (MTF) as an index of resolution, the noise power spectrum (NPS) as an index of noise characteristics, and the system performance (SP) function as an overall evaluation index of the image. Five dentists visually evaluated the images from the head-neck phantom. The results showed that the image quality tended to worsen, and scores for visual evaluation decreased as tube currents, FOVs and rotation angles decreased. In particular, image noise negatively affected the delineation of the periodontal ligament space. The optimal imaging conditions were 90 kV, 2 mA, 4×4 cm FOV and 180° rotation. These results suggest that CBCT radiation doses can be significantly reduced by optimizing the imaging conditions.

    Positron emission tomography (PET)/computed tomography (CT) has improved sensitivity and resolution using silicon photomultiplier as a photosensor. Previously, only a fixed setting was available for the shooting time of 1 bed, but now, the shooting time can be changed for each bed. Time can be shortened or extended depending on the target area. A few studies reported on image reconstruction conditions for head and neck cancer in whole-body PET/CT examinations. Thus, this study aimed to optimize the imaging conditions of the head and neck region during whole-body imaging. A cylindrical acrylic container with a 200 mm diameter was used to simulate the head and neck area using a PET/CT system equipped with a semiconductor detector. Spheres of 6-30 mm in diameter were enclosed in the 200 mm diameter cylindrical acrylic vessel. Radioactivity in 18F solution (Hot:BG ratio 4:1) was enclosed in a phantom following the Japanese Society of Nuclear Medicine (JSNM) guidelines. Background radioactivity concentration was 2.53 kBq/mL. List mode acquisition of 1,800 s was collected at 60-1,800 s with the field of view of 700 mm and 350 mm. The image was reconstructed by resizing the matrix to 128 × 128, 192 × 192, 256 × 256, and 384 × 384, respectively. The imaging time per bed in the head and neck should be at least 180 s, and the reconstruction conditions should be a field of view (FOV) of 350 mm, matrix sizes of ≥ 192, and a Bayesian penalized likelihood (BPL) reconstruction with a β-value of 200. This allows detection of > 70% of the 8-mm spheres in the images.

    OBJECTIVE: The higher sensitivity of the new-generation positron emission tomography/computed tomography (PET/CT) with silicon photomultiplier (SiPM) may increase false-positive rates in detecting metastatic lymph nodes (LNs). This study aimed to clarify the usefulness of the SiPM PET scanner in diagnosing LN metastasis of oral squamous cell carcinoma (SCC). METHODS: We retrospectively reviewed consecutive F-18 fluorodeoxyglucose PET/CT images of 39 SCC patients using SiPM PET and 31 SCC patients using non-SiPM PET. We measured the maximum standardized uptake value (SUVmax) of the LNs on PET images and maximum short-axis diameter on transverse CT images. RESULTS: The sensitivity and specificity of SiPM PET were 86.2% and 95.6%, respectively (cut-off SUVmax, 4.6). The area under the curve (AUC) of SiPM PET (0.977; 95% confidence interval [CI], 0.958-0.995) was significantly higher than that of non-SiPM PET (0.825; 95% CI 0.717-0.934) (P < 0.01). In a size-limited analysis of diameter, the AUC of SiPM PET (≥ 0.96 for all diameters) was significantly higher than that of non-SiPM PET (tended to decrease as the LN diameter decreased) for the diagnosis of LN metastasis by SUVmax. CONCLUSION: SiPM PET had higher diagnostic accuracy for LN metastasis of oral SCC than non-SiPM PET, even for small LN metastasis without increasing false-positives.

    OBJECTIVES: The most typical maxillofacial feature of patients with acromegaly is mandibular protrusion. This study aimed to determine differences in maxillofacial morphology between skeletal Class III patients with and without acromegaly using cephalometric analysis. METHODS: Cephalograms of 37 patients with acromegaly (Acro), 37 age-matched non-acromegalic patients with skeletal Class III malocclusion (C-III), and 37 age-matched Class I malocclusion patients (C-I; control) were retrospectively collected. The skeletal and dental morphology of each group was analyzed using cephalometric analysis, which included linear and angular measurements and facial profilograms. In addition, we analyzed diagnostic performance and cutoff values for discriminating acromegaly from skeletal Class III malocclusion using receiver operating characteristic (ROC) curve analysis. RESULTS: The mandibular ramus height was larger in the Acro group than in the other groups. The increase in L1/MP in the Acro group, which represented labial inclination of the mandibular central incisors, was the most characteristic feature in this study. ROC curve analysis indicated that a cutoff value of 88.4° for L1/MP had the highest diagnostic performance in discriminating acromegaly from non-acromegalic Class III malocclusion. CONCLUSIONS: Acromegaly was characterized by a greater degree of bimaxillary prognathism than was non-acromegalic Class III malocclusion. Focusing on labial inclination of the mandibular central incisors would be the most useful way to differentiate acromegaly from non-acromegalic Class III malocclusion.

    We investigated the sensitivities of 2-dimensional (2D) magnetic resonance sialography (MR-S) and unilateral sagittal and axial 3-dimensional (3D) MR-S using a surface coil and their combination in diagnosing patients with Sjögren's syndrome (SS). We retrospectively analyzed the 3D and 2D MR-S results of 78 patients with SS. We evaluated the sensitivities of multiple high-signal-intensity spots and staging on MR sialograms and analyzed the efficient imaging methods and cross section for diagnosing patients with SS. The sensitivities of MR-S for detecting abnormal findings (i.e., MR-S stage 1 or higher) were as follows: 65 cases (83.3% [95% confidence interval (CI): 73.2-90.8]) for unilateral sagittal 3D MR-S; 62 cases (79.4% [95% CI: 68.8-87.8]) for axial 3D MR-S; 66 cases (84.6% [95% CI: 74.7-91.8]) for combined unilateral sagittal and axial 3D MR-S; and 32 cases (41.0% [95% CI: 30.0-52.7]) for bilateral sagittal 2D MR-S. The ratio of the abnormal finding of MR-S was tested using the two-tailed Fisher's exact test. Unilateral sagittal, axial, and combined unilateral sagittal and axial 3D MR-S showed significantly higher sensitivity than bilateral sagittal 2D MR-S, respectively (P < 0.001). Most cases upstaged by 3D MR-S were those positive (stage 1 or higher) among the stage 0 cases detected by 2D MR-S. Axial 3D MR-S, compared with 2D MR-S, understaged four cases, which was due to the imaging range of the axial 3D MR-S. We concluded that a single unilateral sagittal 3D MR-S was sufficient and axial 3D MR-S was unnecessary for SS staging. T1- and T2-weighted images are essential for investigating the salivary glands in patients with SS. Therefore, we also concluded that bilateral sagittal 3D MR-S of the parotid glands in addition to T1- and T2-weighted imaging is necessary, sufficient, and most efficient for precise MR imaging examination of the salivary glands, including diagnosing SS.

    OBJECTIVES: To evaluate the ability of different imaging modalities to accurately detect bone invasion in oral squamous cell carcinomas. PATIENTS AND METHODS: Patients with oral squamous cell carcinoma, who were scheduled for mandibulectomy or maxillectomy, underwent clinical evaluation using five preoperative imaging diagnosis methods-contrast-enhanced MRI, CT, 99mTc scintigraphy (Tc scan), FDG-PET CT (PET/CT), and panoramic radiography. The sensitivity and specificity of each modality in detecting bone invasion were calculated by comparing the findings on the images with postoperative histopathological findings. In a subgroup of patients, we further assessed the ability of MRI and CT to detect the accurate extent of bone invasion, including the height, width, and depth in patients with pathological mandibular invasion. RESULTS: Overall, 50 patients were enrolled in this study, and nine patients with pathological mandibular invasion were included in our subgroup analysis. MRI was found to be the most useful method in detecting bone invasion, showing the highest sensitivity (88.9%) and negative predictive values (92.3%). CT (87.5% specificity and 77.8% sensitivity) was more specific than MRI, though less sensitive. Combined PET/CT was more sensitive (83.3%) and less specific (71.9%) than CT. Tc scan had high sensitivity (88.9%); however, the specificity was relatively low (71.9%). CONCLUSION: MRI was the most useful method in detecting bone invasion. A negative MRI result definitively excludes bone marrow invasion. In patients with positive MRI findings, a negative CT may be useful in ruling out bone marrow invasion.

    We present six patients with head and neck osteosarcomas. They ranged in age from 15 to 84 years with a median age of 63.5 years. There were five males and one female. The site of origin was the mandibular bone in four patients, the maxilla in one, and the larynx in one. Five patients were TNM stage ⅡA and one was stage Ⅲ. Histological findings revealed osteoblastic cancers in five patients and chondroblastic in one. All patients underwent surgical resection and adjuvant therapy was performed in five cases. Pathologically complete resections were obtained in five cases; however, only one patient was diagnosed with positive margins in the surgical specimen. In two cases, the cancer recurred, one locally at 2 months and the other by distant metastasis at 12 months. The case of local recurrence was non-responsive to preoperative chemotherapy and was stump-positive upon resection.

    We present a patient with a malignant lesion diagnosed as carcinoma ex pleomorphic adenoma of the parotid gland detected by advanced MRI techniques such as diffusion-weighted MR imaging (DWI) and apparent diffusion coefficient (ADC). A 64-year-old male was referred to our hospital due to a gradual growing left parotid tumor originally discovered 20 years earlier. MRI imaging detected a left parotid tumor 4 cm in the longer axis and 3 cm in the shorter axis. On two occasions, fine needle aspiration cytology and an echo-guided core needle biopsy indicated the possibility of a carcinoma. Total parotidectomy with facial nerve reconstruction was performed. Histological findings confirmed the presence of a carcinoma as well as hyaline degeneration. The tumor was finally diagnosed as a carcinoma ex pleomorphic adenoma by histological examination and gradual growth of the tumor. Advanced MRI techniques revealed low ADC and high DWI only in the regions of the carcinoma.

    This study was carried out to collect and analyze the data obtained from patients to establish the ideal education system for clinical training at university dental hospitals.<br>For this purpose, we obtained the information from the outpatients who visited the Tohoku University Dental Hospital using a questionnaire for our education and clinical training system.<br>The following results were obtained.<br>(1) "Referral from general practitioner" was the most common reason for visiting our hospital for new patients and patients aged younger than 40, while "reliable treatment" was the most common reason for revisiting patients and patients aged over 40.<br>(2) Most patients accepted the treatment and observation by interns and undergraduate students regardless of generation. Revisiting patients refused to see them less than new patients.<br>(3) The permissible time for clinical training purposes was longer in revisiting patients and patients aged over 50 compared with new patients and patients aged younger than 50, respectively.<br>These results suggested that elderly and revisiting patients were more cooperative with our clinical training program than younger and newer patients through the assurance of extensive safety and conscientiousness regarding treatment.

    To clarify the relationship between masseter muscle pain and its edematous changes, we investigated whether increased in thickness and plasma extravasation may occur in the rat masseter muscle as a result of capsaicin injection. In anaesthetized rat, capsaicin solution was injected into a masseter muscle on the one side, and vehicle into one on the other side. The thickness of the masseter muscle was measured by means of high-frequency ultrasound. Plasma extravasation was determined by means of the Evans blue dye method. As results, capsaicin injection into the masseter muscle induced thickening and plasma extravasation in the masseter muscle, and combination with local anaestesia counteracted the dye extravasation, indicating neurogenic inflammation change in rat masseter muscle.

    Purpose: To investigate morphological changes in the mandible using the recently developed acromegaly-like rat model, induced by exogenous administration of insulin-like growth factor I (IGF-I), and to clarify whether the changes are reversible after normalization of the circulating IGF-I level.<br>Material and methods: Human recombinant IGF-I (SOMAZON, Astellas Pharma Inc., Tokyo, Japan) (640μg/day) was subcutaneous infused continuously to 10-week-old male rats for 4 weeks (IGF-I group) (n=6). The control sham-operated animals were injected with saline alone (control group) (n=6). Rats were sacrificed immediately on ending administration (age 14 weeks). Another 12 rats (6 from each group) were housed for an additional 4 weeks after administration ended. Measurement of body weight and blood sampling were done before sacrificing rats (at age 14 and 18 weeks). Then, IGF-I serum level was measured from the sera. After sacrificing the rats, their mandibles were excised. Morphological analysis on the mandible was performed on lateral cephalometric image using soft X-ray equipment (SOFTEX Type CMB, SOFTEX Corp., Tokyo, Japan).<br>Results: Body weights, as well as total IGF-I (human+rat) serum level was markedly higher in the IGF-I group during administration of IGF-I, with a return to the control level after IGF-I administration ceased. Some linear mesurements of the mandible, i. e., Mental foramen (MeF) -Condyle (Cd), MeF-Infradentale (Id), Mandibular foramen (MaF)-Cd, MaF-Id, Cd-Id and Cd-Deepest point of the antegonial notch, significantly increased in the IGF-I group as compared with the control group both during and after IGF-I administration (age 14 and 18 weeks).<br>Conclusion: In our acromegaly-like rat model, specific enlargement of the mandible was found in the length of the condylar process and mandibular body. Such morphological changes were not reversible after normalization of the circulating IGF-I level.

    歯科用コーンビームCT(歯科用CT)は、X線不透過性の極めて高いインプラントや歯科材料が撮影範囲に含まれるため、アーチファクトの軽減と組織のX線吸収値の安定化(定量化)が課題である。近年、微弱なX線を感知する素子としてテルル化カドミウム(CdTe)半導体が注目されている。CdTe半導体は、X線を直接感知することができるため検出感度や画質の低下が少なく、光子(フォトン)をそのままカウントすることにより、物質の原子番号や電子密度によるエネルギースペクトルの変化を定量評価することが可能である。本研究は、CdTe半導体検出器を歯科用CTに搭載することで、歯科用CTに実用可能なフォトン・カウンティング機能を開発し、骨や歯のエックス線に対するモノクロマティック特性に依存した多元的分析が可能であり、かつアーチファクトの極めて少ない、高い濃度分解能、空間分解能を有した歯科用CT装置の実現化を目指すものである。 2020年度は 1)高電圧出力を可能とした管球をプロトタイプの歯科用CT開発用冶具(以下、開発用冶具)に設置した。 2)抜去歯に対し、CdTe半導体検出器によるフォトン・カウンティング機能を用いて、エナメル質と象牙質の実効原子番号及び電子密度の算出を可能とする計算式を検討し、そのカラー画像化を試みた。その結果、エナメル質と象牙質には明瞭な差が確認され、フォトンカウンティングを用いることで歯の実効原子番号、電子密度の算出及びカラー画像化が可能であることが示唆された。

    Preoperative imaging diagnosis of oral cancer requires accurate evaluation of the extension and lymph node metastasis, which affects treatment planning and outcomes. We investigated imaging conditions specific to the head and neck region using a PET with a silicon-semiconductor detector. We developed a new MR imaging method to improve the detection of focal defects that are suspicious for lymph node metastasis. Compared to conventional PET imaging, these imaging methods improved the detection of small primary tumors, the evaluation of lesion extension, and the diagnostic accuracy of small lymph node metastases. Furthermore, the use of PET quantitative values resulted in very high diagnostic accuracy. The comprehensive imaging method obtained from the results of this study enables imaging diagnosis with superior diagnostic accuracy compared to conventional methods.

    We analyzed the usefulness of black-blood MR imaging (MSDE-VISTA) to detect small lymph node metastases in head and neck cancer, and obtained stable and good quality imaging with high resolution 3D imaging and blood flow artifact suppression.The MSDE-VISTA was superior to the conventional MR imaging in the ability to visualize focal defect and extranodal extension of the metastatic lymph nodes, which are findings suggestive of lymph node metastasis. Further, subtracion images of before and after contrast enhanced the MSDE-VISTA showed the best diagnostic performance. Thus, these results suggested that the MSDE-VISTA can detect early and accurately lymph node metastasis in imaging diagnosis of head and neck cancer.

    To develop an image analysis system to detect niche formation leading to lymph node (LN) metastasis formation, changes in vascular architecture in the process of LN metastasis formation were analyzed using contrast micro CT imaging and contrast high frequency ultrasound imaging. In the present study, tumor cells were inoculated into the subiliac LN (SiLN) of LN-swollen mouse, MXH10/Mo/lpr, and then metastasis formation process in the proper axillary LN located downstream of SiLN in mouse lymph network was analyzed. The results showed that, in the early stage of LN metastasis, blood vessel density in the LN prior to LN size change was increased. However, angiogenesis in the LN at the early stage of LN metastasis could not be confirmed.

    To detect lymph node micrometastasis, we developed a volume registration method for analyzing the vessel structure in the lymph node by using Scale-Invariant-Feature-Transform and Phase-Only Correlation methods. The algorithm developed here was for correcting images for evaluation of ultrasound images of the experimental metastatic lymph node, and also for calculating highly accurate information of blood vessel considering the dispersion value due to temporal change of luminance information and those before and after contrast agent administration. As a result, it was possible to eliminate misalignment and noise of the ultrasound images, and also extract the microstructure of the blood vessels in the metastatic lymph node.

    Taste perception is influenced by several factors. However, it was unclear how food culture affects taste perception. In this study, we compared recognition taste thresholds between populations with different food culture, i.e. Thai and Japanese Adults. A matched case-control study was conducted in 168 healthy older adults (84 for each country). Recognition thresholds of sweet, salty, sour, bitter and umami were measured using Filter Paper Disc by stimulation at anterior and posterior tongue area. Spicy preference was measured by calibrated questionnaires. Interestingly, Thai older adults had much higher recognition thresholds of all tastes than those of Japanese (p<0.0001). Further, Thai adults had stronger preference on spicy food (p<0.0001). Our findings suggest that population from culture with stronger preference on spicy food such as Thai had higher recognition thresholds of all tastes than population with milder preference on spicy food like Japanese.

    Recently, patients with atypical tooth pain, glossodynia and tempolomandibular disorder have been increasing and this phenomenon was recognized as social issues. This study was designed to determine whether sialorphin that displays analgesic activity can be effective for pain control in the maxillofacial regions both of the acute-pain model rats induced by formalin injection and of the chronic-pain ones induced by trigeminal (maxillary) nerve constriction. In conclusion, sialorphin had inhibitory effect both on the formalin-induced acute pain and on the chronic pain induced by nerve constriction.

    We analyzed the longitudinal morphological changes in extraction sockets following sustained continuous subcutaneous infusion of insulin-like growth factor I (IGF-I). rats were subjected to right mandibular first molar extraction. Experimental rats received a continuous subcutaneous infusion of IGF-I by minipump. Control animals were treated with saline via the same method. All rats were then housed after stop of the infusion. Micro-CT scanning was performed immediately after tooth extraction and at regular intervals. New bone formation was markedly higher in the IGF-I-treated group as compared with the control group. The loss in alveolar ridge height in the IGF-I group was significantly lower than that in the control group at each time point after extraction on the buccal side and at 2, 3, and 6 weeks on the lingual side. IGF-I treatment increases the volume of newly formed bone and reduces the loss in alveolar ridge height following tooth extraction.

    The aim of this study is to develop a new diagnostic imaging technique which can detect the head and neck cancer metastasis routes. In this study, MXH10 / Mo / lpr mice, which develop systemic swelling of lymph nodes up to 10 mm in diameter, were used as a lymph node metastasis model. As a method for visualization of lymph node network in real time, we examined a contrast ultrasound imaging system using nano-microbubbles. As a result, it was suggested that contrast ultrasound image analysis method using a nano-micro-bubbles might be a potential method clinically applicable to detection of lymphatic network.

    最近、非定型歯痛、舌痛症および顎関節症などの慢性痛患者は増加傾向にあり社会問題となっている。しかしながら、慢性痛に対する治療法は確立されておらず、診断および治療法の開発は急務であり責務である。近年、パスツール研究所の研究員により、ラット唾液からモルヒネの約3~6倍の鎮痛効果があるシアロルフィンが発見された。しかしながら、いまだこれら唾液由来の鎮痛物質が慢性痛にどのような効果をもたらすかについては明らかにされていない。また、これらの鎮痛効果の評価はラットの行動薬理学的評価であり、疼痛そのものを評価していない等の問題があるため、シアロルフィンを臨床に応用するためには、鎮痛効果の判定(定量)が不可欠である。そこで本研究では、我々が痛みストレスマーカーになることを発見した視床下部CRH(Corticotropin Releasing Hormone)遺伝子、下垂体POMC(proopiomelanocortin)遺伝子(以上、ストレス関連遺伝子)、三叉神経節P2×4レセプター遺伝子(慢性疼痛原因遺伝子)を指標としてシアロルフィンの鎮痛効果を定量的に評価することを目的とした。 H23年度はマウス頬部へホルマリン注射することにより疼痛刺激を与え、その疼痛モデルに対しシアロルフィンを投与しその鎮痛効果について疼痛行動回数を指標として検討した。その結果、シアロルフィン投与マウスの方がコントロールより疼痛行動回数が少なかった。また、シアロルフィンを投与したラットと生食を投与したラットで疼痛行動の回数の差が大きいのは5-10分と30-35分の時間帯であった。以上より、シアロルフィンが前者の時間帯はホルマリンによる第1相の疼痛を後者の時間帯は第2相の疼痛を抑制していることを示唆した。

In the present study, by using experimental tumor model, we developed a high-resolution diagnostic imaging system utilizing nanobubbles and high-frequency ultrasound image analysis equipment. In addition, we were able to present the possibility of a new gene therapy method for hemangioma by transfer of a plasmid DNA, which inhibited the tumor growth, by utilizing nanobubbles and ultrasound.

    口腔には顎骨の破壊や吸収を伴う疾患が多く存在し、自家骨や人工骨を用いた再建治療が行われているが、必ずしも満足な結果は得られていない。これまで我々は、「自らが持つ骨形成能による骨再生」を目指し、細胞増殖作用を有するInsulin-like growth factor I(以下IGF-1)に着目した一連の研究を行ってきた。そこで本研究は、IGF-1の骨形成促進作用に着目し、ナノバブルと高周波超音波装置による最新の遺伝子導入システムを用いて、新たな顎骨再生治療を目指すものである。平成23年度において、1)TNF-α発現プラスミドDNAを作製し、ナノバブルと超音波を用いた分子導入法による遺伝子治療実験を行った。その結果、腫瘍の縮小と腫瘍血管の密度の減少が確認でき、この技術をIGF-1の遺伝子導入システムにも応用できるものと思われる。2)IGF-1の全身投与を行なったラットを用いて、エックス線検査を行ない、顎骨、特に歯槽骨に与える影響について検討を行なった。その結果、IGF-1は歯槽骨の海面骨に比べ、皮質骨の骨密度を増加させ、その効果は投与後も持続することが明らかとなった。この結果は、将来の顎骨再生部へのインプラント治療への応用を考えると、非常に有用な情報である。3)顎骨欠損モデルの作製のために、各週齢のラット下顎臼歯抜歯と歯槽骨削除を行なってみた。その結果、ラットは5週齢を越すと抜歯が非常に難しくなることや、抜歯窩の骨形成は4週間ほどで単純エックス線写真においても確認できることが明らかとなった。今後は、この結果をもとに顎骨欠損モデルを作製していくこととする。

    The results of IGF-I administration for normal rat were as follows. 1)Bone mineral density (BMD)analyzed by X-ray photograph increased in lower area of mandibular condyle. 2)On measurement about bone matrix ratio (BMR), BMR was greater in lower part of mandibular condyle (Cd)vertically divided into three parts, and was greater in central part of laterally divided Cd. 3)On measurement about osteoblast, the number of osteoblasts was greater in central and lower parts of vertically divided Cd, and in inside and outside parts of laterally divided Cd. 4)On measurement about osteoclast, the number of osteoclasts has no change in any parts of Cd. 5)These changes during IGF-I administration disappeared after IGF-I administration ended.
