Myeloid-derived suppressor cells (MDSCs) help establish the tumor microenvironment by suppressing T-cell response in tumor-bearing hosts. Plasmacytoid dendritic cells (pDCs) activate antigen-specific T cells, thereby, maximizing their antitumor effects. IDO1 is associated with both MDSCs and pDCs and plays a major role in the formation of the tumor-mediated immunosuppressive environment. We utilized immunohistochemistry to examine the involvement of IDO1 in oral squamous cell carcinoma (OSCC) and oral potentially malignant disorders (OPMDs, precancerous lesions). We examined the expression of MDSC markers, CD11b and CD33, as well as pDC markers, CD303 and IDO1, in 60 OSCC and 45 precancerous lesion specimens and analyzed their association with clinicopathological parameters. Expression of these biomarkers identifying MDSCs and pDCs was high in precancerous lesions in patients with severe dysplasia and OSCC. While detecting pDCs, high CD303 and IDO1 expression levels were frequently observed in moderately or poorly differentiated OSCCs. CD11b, CD33, and CD303 levels were significantly correlated with the mode of invasion; CD33 was correlated with OSCC invasion depth while the other three markers tended to be highly expressed in superficial cancer cases showing microinvasion. Expression levels of all four biomarkers were significantly associated with the cancerization of OPMDs to OSCCs. We show, for the first time, that the infiltration of MDSCs and pDCs is significantly associated with progression of premalignant lesions to OSCC. This suggests that these cells may act as prognostic biomarkers for premalignant lesion progression and that immunotherapeutic approaches that control each of these immunosuppressive cells may protect against progression to malignancy.

OBJECTIVES: To evaluate the ability of different imaging modalities to accurately detect bone invasion in oral squamous cell carcinomas. PATIENTS AND METHODS: Patients with oral squamous cell carcinoma, who were scheduled for mandibulectomy or maxillectomy, underwent clinical evaluation using five preoperative imaging diagnosis methods-contrast-enhanced MRI, CT, 99mTc scintigraphy (Tc scan), FDG-PET CT (PET/CT), and panoramic radiography. The sensitivity and specificity of each modality in detecting bone invasion were calculated by comparing the findings on the images with postoperative histopathological findings. In a subgroup of patients, we further assessed the ability of MRI and CT to detect the accurate extent of bone invasion, including the height, width, and depth in patients with pathological mandibular invasion. RESULTS: Overall, 50 patients were enrolled in this study, and nine patients with pathological mandibular invasion were included in our subgroup analysis. MRI was found to be the most useful method in detecting bone invasion, showing the highest sensitivity (88.9%) and negative predictive values (92.3%). CT (87.5% specificity and 77.8% sensitivity) was more specific than MRI, though less sensitive. Combined PET/CT was more sensitive (83.3%) and less specific (71.9%) than CT. Tc scan had high sensitivity (88.9%); however, the specificity was relatively low (71.9%). CONCLUSION: MRI was the most useful method in detecting bone invasion. A negative MRI result definitively excludes bone marrow invasion. In patients with positive MRI findings, a negative CT may be useful in ruling out bone marrow invasion.

This study was conducted in order to investigate biological compatibility of a thin and flexible hydroxyapatite (HAP) paper which consists of ultralong hydroxyapatite nanowires. Circular-shaped cranial bone defects with a diameter of 8.8 mm were prepared to expose the dura maters in Wistar rats. The similar-sized, circular-shaped HAP paper was placed at the bottom of the bone defects. After 2, 4, and 8 weeks, the rats were sacrificed, and the experimental sections were examined by micro-CT scanning and histological observation. The HAP paper covered with fibrous tissues showed no inflammatory cell infiltration, and their thicknesses decreased over time. Tartrate-resistant acid phosphatase-positive osteoclast-like cells were induced around the edges of the HAP paper along with the exfoliation of the HAP paper. The newly-formed bones were observed in the bone-defected areas, either with a direct contact with the HAP paper or through thin fibrous tissues. The HAP paper-induced osteoblast differentiation was confirmed since the alkaline phosphatase activities were detected on the surfaces of the HAP paper. These results indicated that the HAP paper may induce osteogenesis without causing any harmful effects. The highly flexible HAP paper can contribute to further development of bone regenerative therapy.

To characterize concentrated growth factors (CGFs) in vivo, we examined the degradation of implanted CGF in rabbits. Untreated CGF (U-CGF) and compressed CGF (C-CGF) were subcutaneously implanted into the dorsum. Histological analyses showed that the U-CGF and C-CGF induced very few inflammatory cells and that the U-CGF and C-CGF were subsequently degraded with dendritic invasion of granulation tissue. The C-CGF histopathologically remained for longer term than the U-CGF. Aggregated CD31+ and RAM11+ cells appeared in and around the implanted CGF. The number of macrophages and blood vessels in the CGF-implanted groups was greater than that in the sham group. There were more blood vessels in the U-CGF group than that in the C-CGF and sham group. We showed that CGF was degraded by macrophages in 4 weeks and enhanced angiogenesis with dendritically branching new capillaries. Therefore, the U-CGF and C-CGF can be clinically applied as a biomaterial inducing angiogenesis.

症例は69歳男性で、数年前にパノラマエックス線画像上で左下78番根尖下方、下顎下縁付近に小豆大、類円形の透過像を指摘されていたが、その後母指頭大まで拡大したため、精査加療目的で当科紹介となった。CTで左下7番部の下顎下縁付近に舌側から陥凹する類縁形の骨欠損を認めた。MRIでは、左側下顎骨臼歯部で下顎管より下方に径12mm程度の類円形構造が観察された。左側下顎骨静止性骨空洞と左側下顎骨腫瘍が疑われた。生検を計画し、この際に腫瘍性病変の可能性も術前に説明し、二期的な全摘出術も予定し手術を行う計画とした。手術時間は42分、出血量は5mLで、術後に舌神経の麻痺症状はなく、口底部の腫脹も軽度で、術後6日目で退院となった。術後1年7ヵ月の現在まで再発所見はなく、CT画像上でも摘出腔は縮小し、同部には骨再生を認めた。また、パノラマエックス線画像では骨欠損部の骨造成を認めた。病理組織学的所見はLipoma、compatibleで、摘出標本は少量の結合組織に覆われた脂肪組織であった。

A simple bone cyst (SBC) is an intrabone cavity without epithelial lining, which occasionally occur with fibrous-osseous lesions. The imaging finding of the scalloped border and conserving the lamina dura, periodontal ligament spaces, or follicular spaces are considered radiographically pathognomonic of SBC. SBC has been reported to occur with fibrous-osseous lesions, including cemento-osseous dysplasia (COD). Computed tomography (CT) imaging findings are sparse, and there are no reports of magnetic resonance imaging (MRI) findings, including apparent diffusion coefficient (ADC) value calculated by diffusion-weighted MRI for the florid COD-associated SBC. We report a case of a 39-year-old woman who was referred to our hospital because a panoramic radiograph of her right mandible showed asymptomatic radiolucency in the apical molar area. CT images showed a low-density lesion in the periapical areas of the right mandible, which accompanied a well-defined, high-density lesion, and a mixed low/high-density lesion with an unusual marked irregular border in the left mandible. The MRI showed cyst-like extremely hyperintense signals on fat-suppressed T2-weighted images at the right mandibular low-density and the left mandibular mixed low/high-density areas. ADC map showed high values (over 2.5 × 10-3 mm2/s) at the cystic areas in the bilateral mandible. We performed incisional biopsies of the bilateral mandibular lesions and resections of the bilateral maxillary lesions. Surgical and histopathological findings established a diagnosis of florid COD in the bilateral mandible and maxilla, simultaneously complicated by multiple SBCs in the bilateral mandible. The ADC value of cystic component is useful for the diagnosis, if the COD-associated SBC exhibits solitary, nonspecific, or rare imaging findings.

症例は81歳女性で、近在歯科にて下顎臼後部の腫瘤を指摘され、生検により左側下顎臼後部乳頭状唾液腺腺腫の病理組織診断を得た。その後、同病変の精査加療目的に当科紹介受診となった。全身麻酔下に腫瘍切除術を施行した。術前に前医によって施行された生検において乳頭状唾液腺腺腫の診断を得ていたことより、腫瘍の基部より5mmの安全域を設定し左側下顎臼後部にメスにて切開を加え、骨膜を含めて切除した。切除後の骨露出部については人工真皮にて被覆し軟膏付きガーゼを縫着した。切除標本は弾性軟で大きさは約20×10mmであった。表面は顆粒状で白色を呈していた。HE染色では乳頭状増殖を示す外方隆起性の粘膜病変で、被膜形成はなく、重層扁平上皮の肥厚とそれに連続する比較的境界明瞭な腫瘍性病変として観察され、特徴的な2層性の増殖を示していた。

症例は35歳男性で、定期検診のために近医歯科医院を受診した際に撮影されたパノラマX線写真で左側上顎前歯部にスリガラス様の不透過像を認めたため、当科紹介受診となった。病理組織学的に類腱型エナメル上皮腫と診断し、上顎部分切除術および遊離腸骨移植術を施行した。その後、3Dプリンターにて顎骨模型を作製し、カスタムメイド・チタンメッシュトレーと自家腸骨海綿骨細片を用いた上顎骨再建術を施行した。術後6ヵ月目のCT所見で良好な骨形成を確認した後、カスタムメイド・チタンメッシュトレー除去術および4本のインプラントの埋入手術を施行した。さらに、4ヵ月後に二次手術を行い、ヒーリングアバットメントを装着した。現在、初回手術後5年および最終補綴物装着後2年8ヵ月経過しているが、腫瘍の再発所見およびインプラント体周囲に異常な骨吸収は認めず、十分な審美性と咬合機能の回復が得られている。

症例は11歳女児で、非化膿性のび漫性硬化性下顎骨骨髄炎に罹患し、1ヵ月間にわたり開口障害が生じていた。パノラマX線写真では下顎骨左側下縁に新生骨形成が、CT画像では下顎左側にび漫性硬化像、皮質骨の吸収像、新生骨膜骨を骨表面から隔てている透亮帯が示された。抗生物質と非ステロイド性抗炎症薬で加療されたが症状の改善は不完全で繰り返し再発を生じていた。初診から1年後に行ったパノラマX線評価では下顎骨の左側がすりガラス様の様相を呈して肥大している所見が、CT検査では骨硬化の進行、散在する骨融解像、本来の皮質骨の破綻、骨の肥大が示された。初診から1年3ヵ月後に下顎骨のdecorticationと高気圧酸素治療を施行したが効果はみられなかった。初診から1年11ヵ月後、抗生物質としての効果とは別に炎症性サイトカイン低下を期待してミノマイシンを投与したところ、症状は著明に軽減した。経過と所見から慢性再発性多発性骨髄炎の重症型としての慢性非細菌性骨髄炎であることが示唆された。

症例は71歳女性で、左側下顎歯肉の粗造感を自覚した。その後、左側下顎歯肉部が徐々に腫瘤様に増大し、腫瘤の存在により下顎義歯の装着が困難となった。上下顎ともに無歯顎で、左側下顎小臼歯相当部の歯肉に、25×17×16mm大、弾性硬、有茎性、境界明瞭な腫瘤を認めた。表面性状は滑沢で粘膜色よりやや赤色を呈し、周囲に硬結は認めなかった。パノラマX線写真で、左側下顎小臼歯相当部の腫瘤部下顎骨に軽度の骨吸収像を認めた。精査のため腫瘤の前方部から生検を行った。病理組織学的に角化を伴うエナメル上皮腫様の腫瘍胞巣を認めたため、周辺性エナメル上皮腫と診断し、全身麻酔下に腫瘍切除術を施行した。術後は左側下唇部に軽度知覚障害が出現したが、術後6日目に消失した。術後19日目に経過良好のため退院した。術後約1ヵ月目で上皮化を認め、術後約2ヵ月目に新製した上下顎義歯が装着再能となった。術後約5年経過しているが、再発は認めず経過良好である。病理学的組織診断は象牙質形成性幻影細胞腫となり、発生部位から周辺性象牙質形成性幻影細胞腫と診断した。

症例は生後4週女児で、生後3週目より母親が患児の舌背に舌苔様の白色病変を認めた。生後4週目に病変がやや隆起し黒色の着色を認めた。口腔内所見では、表面に黒又は青の色彩を帯びた舌苔様の白色病変を舌尖及び舌縁を除いた舌背部に認めた。初診時に舌カンジダ症を疑い、舌背部の擦過により検体を採取し、細菌検査を施行し、グラム陽性球菌及びグラム陰性桿菌を検出した。11日後、舌背部病変が不変のため、舌背部を擦過して糸屑様、繊維状の標本を採取した。患児母に、舌ブラシでの清掃を指示したところ、時折、舌背に自色病変がみられることはあったが、黒色調を示すことはなくなり、6ヵ月後の時点において、病変の再燃を認めなかった。擦過標本から、外来性の繊維性異物を伴った黒毛舌と診断した。

75歳男。2年ほど前より右側顎関節部の違和感を自覚するも放置した。最近になり同部の圧痛が出現した。X線CT画像にて右側顎頭を取り囲むように巨大な腫瘤を認め、腫瘤内には石灰化物の散在を認めた。また、右側下顎頭の高度骨吸収と著明な変形を確認した。MRIにて右側顎関節部に、T1強調画像にて低信号、T2強調画像にて不均一な高信号を示す腫瘤を認めた。右側下顎頭腫瘍の臨床診断で、全身麻酔下での腫瘍摘出術を予定した。術後約2年の3D-CT画像所見においては、腫瘍の再発を疑う所見や、整復した形成骨片ならびに整形した関節突起の著しい吸収は認めず、良好な術後状態であることを確認した。退院後、定期的な外来通院にて診察を継続し、現在術後約2年半が経過するが、局所に再発を示唆する所見は確認されておらず、咬合状態は安定しており、開口障害や機能障害も認めていない。病理組織学的診断は滑膜性軟骨腫症であった。

Ameloblastoma is one of the most common odontogenic epithelial tumors and occurs chiefly between the ages of 20 and 39 years. Reports of ameloblastoma developing before the age of 6 years are rare. We report a rare case of ameloblastoma in the left mandibular region of a 3-year-old Japanese boy, with a review of the Japanese and English literature on ameloblastoma in children younger than 6 years. Intraoral examination showed a marked painless swelling in the left side of the mandible. Radiographic examination revealed a unicystic radiolucency. To treat the tumor, marsupialization was performed twice, with the patient under general anesthesia. There have been no clinical or radiologic signs of recurrence for 4 years 8 months after the second marsupialization.

Intravenous injection of Klebsiella O3 lipopolysaccharide (LPS) into BALB/c mice induces an anaphylaxis-like shock within minutes. Using 5-hydroxytryptamine as a marker for platelets, we previously suggested that a rapid platelet accumulation in the liver and lung precedes the shock, and that a complement-dependent platelet-degradation is involved in the shock. Here, we examined (i) the effect of platelet-depletion (using an anti-platelet monoclonal antibody) on the shock and (ii) the contribution of macrophages to the platelet-accumulation in those organs. LPS-induced platelet-accumulations in the liver and lung were confirmed by immunostaining. In platelet-depleted mice, the shock was largely prevented. The number of F4/80-positive macrophages was much greater in liver than in lung, and the hepatic macrophages were largely lost in mice given clodronate-encapsulated liposomes. In mice treated with such liposomes, both the LPS-induced accumulation of platelets in the liver (but not in the lung) and the shock were largely prevented, and repopulation of hepatic macrophages restored these LPS-induced responses. These results suggest that (i) platelets are indeed involved in the shock, (ii) Kupffer cells mediate the hepatic platelet accumulation, and (iii) preventing this hepatic accumulation can largely prevent rapid shock being induced by LPS (at the dose used here).

11歳男児,14年間の長期経過観察を行った基底細胞母斑症候群例で,異時性に発生した顎骨嚢胞の増殖細胞の増殖細胞活性能を検討した.その結果,本症例の顎骨嚢胞では対照とした歯原性角化嚢胞に比べ,免疫組織化学的に裏装上皮の増殖活性能が高く,長期に亘る注意深い経過観察を行う必要がある

56歳男,舌の接触痛を主訴として来院した.口腔内診査では舌右側辺縁部に不整形の潰瘍がみられ,潰瘍周囲は硬結を呈し,右側頸部リンパ節に腫脹を触知した.舌生検で高分化型の扁平上皮癌と診断され,術前化学療法の後,舌部分切除術及び右側全頸部郭清術が施行された.手術標本では,舌の筋層深部に達する扁平上皮癌及びリンパ節転移がみられた.術後7ヵ月に左側頸部リンパ節への転移が疑われ,左側全頸部郭清術が施行された.摘出組織標本では舌扁平上皮癌のリンパ節転移と共に耳下腺部にWarthin腫瘍がみられた

生後3ヵ月時に血液検査,免疫学的検査によりSCIDと診断され,4ヵ月時に骨髄移植を施行したが急性心不全で死亡した.剖検では,顕著なリンパ器官の形成不全がみられ,真菌及びCMV感染を合併していた.抗CMVモノクローナル抗体を用いた免疫組織化学的検索では,顎下腺で封入体保有巨細胞と封入体のみられなかった一部の細胞に反応がみられた.後舌腺にCMV陽性反応が検出された.これらの結果より,唾液腺でのCMV感染は分泌唾液の性状や導管の解剖学的構造に影響されることが示唆された.定量的免疫組織化学検索は,通常の組織検索よりも検出数が多く,この手法はCMV感染の早期診断や確定診断に有用と考えられた

non-RI ISH法により,舌粘膜上皮増殖性疾患について検討した。プローブはHPV 6・11・16・18・30・31・32・45・51型に共通するcDNAをビオチン化したものを用いた。HPV DNAは上皮細胞の核ないし核および核周囲の細胞質で検出された。正常上皮では3例中1例にHPVが検出され,潜伏感染が示された。乳頭腫では3例すべてにHPVが検出され,その病原性が示唆された。白板症では3例中1例に,疣贅性癌では2例中1例に,扁平上皮癌では3例中1例にHPVが検出された

Regulator molecules associated with neovascularized condition, development, and cell stemness were examined to clarify the effect of transcription system in oncogenesis of odontogenic epithelium under the intraosseous microenvironment. Neovascularization factors, EphA2, EphB4, and EphrinB2, were greater in ameloblastoma than in tooth germ, and measurement of CD34-positive microvessels and D2-40-positive lymphatic vessels was correlated positively with the Eph/Ephrin molecules. Development-related Hippo pathway signaling molecules, YAP and TAZ, were expressed higher in ameloblastoma than in tooth germ, whereas stem cell-associated transcription factors, Sox2 and Klf4, were lower in ameloblastoma than in tooth germ. These transcription factors also showed differences among histological variation and clinical behavior.

We studied the effects of the stimulating ingredients on TRPA1 channel recombinantly expressed in HEK293 cells using the Ca2+ imaging analysis and the patch-clamp technique. Formaldehyde, butylaldehyde, methyl ethyl ketone, and NNK (4-(methylnitrosoamino) -1-(3-pyridyl)-1-butanoe) activated TRPA1 channel, although acetone, crotonaldyhyde, isoprene, and nicotine did not induce Ca2+ transient. Acrylonitrile induced TRPA1 channel current. We examined the responses to the cigarette smoke elements in trigeminal ganglion neurons and human gingival fibroblast 1. TRPA1 channel was expressed in TRG neurons and HGF-1 cells. Similar to the selective TRPA1 agonist allyl isothiocyanate (AITC), the chemical substances contained in the smoke of tobacco increased intracellular Ca2+ concentration, and induced the outwardly rectifying currents. The responses were blocked by the selective TRPA1 antagonist HC-030031. Therefore it is suggested that TRPA1 channel is activated by the ingredients.

Hypoxia-related molecules, HIF-1 and CA IX, were greater in ameloblastoma than in tooth germ, and solid ameloblastoma showed high reactivity as compared with unicystic ameloblastoma. Measurement of CD34-positive microvessels were correlated positively with HIF-1 and CA IX. Cell proliferation signaling pathways , MAPK, Akt, and STAT3, were activated in ameloblastoma rather than tooth germ, and BRAF mutations were detected only in ameloblastoma but not in tooth germ. Translocation of cell development-related gene EWSR1 was confirmed in clear cell odontogenic carcinoma but not in other celar cell tumors by FISH method.

A large-scale survey of odontogenic tumors in Asia was conducted. We collected information on 5600 odontogenic tumors from 14 universities in 12 countries. The frequency of occurrence was ameloblastoma (2232 cases; 39.9%), keratotic cystic odontogenic tumor (2083 cases; 37.2%), odontoma (638 cases; 11.4%), adenomatoid odontogenic tumor (142 cases ; 2.5%) and odontogenic myxoma (122 cases; 2.2%). There were 96 malignant odontogenic tumors. Although all institutions visited are education and research institutes that represent the respective countries, the environment surrounding the pathology diagnosis and the diagnostic level are diverse, and it is important to elimination of disparities in pathological diagnosis of odontogenic tumors in Asia. These results were reflected into WHO international classification revision.

The objective of this research is to investigate the effect to bone regeneration of condensing platelet fibrin gel derived from autologous blood (Concentrated Growth Factors: CGF) and adipose-derived stem cells conditioned medium(ADSCs-CM). In culture examination, the behavior of angiogenesis of CGF was confirmed. The CGF implanted subcutaneously in rabbit back was disappeared before 4 weeks after implantation. In process of disappearance, many macrophages and newly formed vessels was seen around the CGF. The compressed and membranal CGF showed excellent mechanical properties characterized by high ductility with wide clinical application. The ADSC-MC contained PDGF, VEGF, HGF,and TGF-b, which are known to be necessary for tissue regeneration. This medium contained a high level of VEGF, HGF and TGF-b.

Regulator molecules associated with cell proliferation and degradation as well as hypocic condition under the intraosseous microenvironment were examined in oncogenesis, cell differantiation, and progression. Growth factor receptors, EGFR and HER2-4, were expresssed in odontogenic epithelial cells, and reactivity for EGFR and HER4 was prominent. Gene alterations of these molecules were obscure. Autophagy-related molecules, ATG7, LC3, and p62, were recognized chiefly in odontogenic epithelial cells neighboring the basement membrane. Granula cell ameloblastoma showed high reactivity for these molecules. Hypoxia-related molecules, HIF-1 and CA IX, were found greater in ameloblastoma than in tooth germ, and solid type ameloblastoma showed high reactivity as compared with unicystic type ameloblastoma. Biodegradaion mechanism and effect of new bone formation of magnesium alloys were examined by animal implant experiments. In subcutaneous implant model, Two magnesium alloys of different composition showed different degradation rate. Magnesium alloy of slow rate degradation revealed rapid organization of surrounded tissue. In intraosseous implant model, magnesium alloys showed similar degradation behavior to subcutaneous implant model. New bone was formed around magnesium alloys. New bone formation showed gradual increase in magnesium alloy with high rate degradation. In magnesium alloy with slow rate degradation, bone showed initial increase and afterwords gradual decrease. Magnesium alloys showed high contact rate with bone. Magnesium alloy were demonstrated the high compatibility to bone tissue.

Preliminary, we examined jaw cysts in heterozygous ptc knockout mouse. Mandibular cysts were often histologically detected in the alveolar bones or periodontal ligaments of the molars, but maxillary cysts were not detected. And we analyzed the expression of CD31, CD34 and D2-40 in human odontogenic cystic lesions of dentigerous cysts, radicular cysts and keratocystic odontogenic tumors. Distinctly positive staining for D2-40 was found in lymphatic vessels located in the cystic walls. Small blood vessels stained for CD31 and CD34 were also present in the cystic walls. Especially, numerous lymphatic vessels and blood vessels were present in the radicular cysts.

Regulators of tooth development were examined in oncogenesis and differetiation of odontogenic epithelium. Tooth germs and ameloblastomas expressed Notch receptors and their ligands chiefly in epithelial cells detached from the basement membrane. A stem cell-related molecule CD133 expression in malignanr ameloblastic tumors was significantly higher in tooth germs and ameloblastomas. ABCG2 did not react with toth germ epithelium, while some cases of ameloblastomas and malignant ameloblastic tumors showed positive reaction in neoplastoc cells. An autophagy-related molecule ATG5 expression in ameloblstomas and metastasizing ameloblastomas was slightly lower than in tooth germs. Granular neoplastic cells in ameloblastomas showed evident Beclin1 and ATG5 expression.

To clarify the possible roles of interactions with bone microenvironment in oncogenesis of odontogenic epithelium, expression of matrix-degrading proteinases, angiogenic factors, growth factors, and their associates was analyzed in ameloblastomas and malignancy ameloblastomas as well as in tooth germs. 1. uPA, uPAR, PAI-1, and maspin: uPA was recognized predominantly in mesenchymal cells, uPAR was evident in epithelial cells, PAI-1 was found in both epithelial and mesenchymal cells, and maspin was expressed only in epithelial cells. Expression of uPA and uPAR tended to be higher in ameloblastic tumors than in tooth germs, while PAI-1 was slightly lower in ameloblastic tumors than in tooth germs. Maspin expression was significantly higher in ameloblastomas than in tooth germs. 2. PD-ECGF/TP and angiopoietin1, 2: PD-ECGF/TP was expressed in mesenchymal cells, and showed higher reactivity in ameloblastomas than in tooth germs. Angiopoietin1 and 2 were expressed mainly in epithelial cells neighboring the basement membrane, and malignant ameloblastic tumors showed low angiopoietin1 expression and high angiopoietin2 expression as compared with tooth germs and ameloblastomas. 3. IGF, PDGF, and their receptors: These molecules were expressed mainly in epithelial cells neighboring the basement membrane. Expression of IGF-II was higher in ameloblastomas than in tooth germs, and desmoplastic ameloblastomas showed high expression of IGF and the receptor. Malignant ameloblastic tumors showed high expression of PDGF and the receptor, and PDGF expression was higher in follicular ameloblastomas than in plexiform ameloblastomas.

To clarify the possible roles of the apoptosis signaling pathways in oncogenesis of odontogenic epithelium, expression of death receptor- and mitochondria-mediated pathway signaling molecules was analyzed in ameloblastomas and malignancy ameloblastomas as well as in tooth germs. 1.Death receptor-mediated apoptosis signaling molecules : Representative death receptors, TNFRI and TRAIL-R1,2, and their downstream apoptosis initiator, caspase-8, were examined in tooth germs and benign and malignant ameloblastomas by RT-PCR and immunohistochemistry. Expression of TNFRI and TRAIL-R1,2 was detected in both normal and neoplastic odontogenic tissues, and they were distributed widely in epithelial components. In contrast, expression of caspase-8 was detected in a few cases of ameloblastomas, and its distribution is limited. 2.Mitochondria-mediated apoptosis signaling molecule : Representative mitochondrial apoptotic pathway signaling molecules, cytochrome c, APAF-1, and caspase-9, were examined in tooth germs and benign and malignant ameloblastomas by RT-PCR and immunohistochemistry. Expression of cytochrome c, APAF-1, and caspase-9 was detected in both normal and neoplastic odontogenic tissues. Cytochrome c distribution was evident in odontogenic epithelial cells neighboring the basement membrane, whereas APAF-1 and caspase-9 were expressed in most odontogenic epithelial cells. Immunoreactivity for cytochrome c in tooth germs was slightly weaker than that in ameloblastomas and malignant ameloblastomas. 3.Conclusions : The mitochondria-mediated apoptotic pathway has a role in apoptotic cell death of normal and neoplastic odontogenic epithelium, rather than the death receptor-mediated apoptotic pathway. Alterations of these apoptosis signaling molecules might be involved in oncogenesis of odontogenic epithelium.

歯原性上皮の腫瘍化における細胞増殖因子とそのシグナル伝達機構の異常を解明するため、歯原性上皮に由来する代表的な腫瘍であるエナメル上皮腫におけるチロシンキナーゼ型リセプターを介する細胞増殖因子HGF,VEGFについて解析した。今回、更にこれらの増殖因子の重要なシグナル伝達経路であるRas/MAPK経路について検索した。 1.Ras/MAPK経路のシグナル伝達分子K-Ras,Raf1,MEK1,ERK1/2の発現検索した全組織(歯胚10,エナメル上皮腫46,悪性エナメル上皮腫6)において、これらのシグナル伝達分子の免疫組織化学的発現が確認された。これらの発現は、主として基底膜近傍の歯原性上皮性細胞で高かった。エナメル上皮腫におけるこれらのシグナル伝達分子は、叢状型で濾胞型より高い発現傾向を示し、角化部や顆粒細胞での発現は明らかに低かった。これらの所見よりRas/MAPK経路は正常および腫瘍性の歯原性上皮において機能し、細胞の増殖や分化に関与することが示唆された。 2.Ras/MAPK経路のシグナル伝達分子K-Rasの変異癌原遺伝子としても知られているK-Rasはエクソン1,2より構成され、それぞれ変異のホットスポットであるコドン12,13および61を含んでいる。新鮮材料の得られたエナメル上皮腫22例、悪性エナメル上皮腫1例よりDNAを抽出し、DNAダイレクトシークエンス法でK-Ras遺伝子の異常について検討した。その結果、エナメル上皮腫1例においてエクソン1のコドン12にGGT→GCTの点変異がみられた。これらの腫瘍におけるK-Ras変異の頻度は低く、腫瘍発生や悪性化との関係は明らかでなかった。

歯原性上皮の腫瘍化における細胞周期制御機構の異常を解明するため、歯原性上皮に由来する代表的な腫瘍であるエナメル上皮腫において、テロメラーゼ活性をTRAP法でまたTERTおよびc-myc蛋白を免疫組織化学で検索した。 1.テロメラーゼ活性およびTERTの発現テロメアDNAを合成し細胞の不死化に関わるテロメラーゼの活性は、エナメル上皮腫全例において認められた。テロメラーゼの構成要素であるTERTは、腫瘍細胞の核に認められ、間質細胞ではみられなかった。腫瘍実質の周辺部円柱状ないし立方状腫瘍細胞に高頻度でみられ、中央部多角形腫瘍細胞は散在性に陽性を示した。これらの所見より、テロメラーゼが歯原性上皮の腫瘍化や増殖活性に関与することが示唆された。 2.c-myc蛋白の発現とテロメラーゼとの関連 c-myc蛋白は原癌遺伝子にコードされる多機能を有する転写因子で、テロメラーゼの転写に関与するとされている。エナメル上皮腫において、c-myc蛋白は周辺部円柱状ないし立方状腫瘍細胞の核に高頻度でみられ、中央部多面形腫瘍細胞は散在性に陽性だった。これらの所見はTERTの局在と類似しており、エナメル上皮腫においてc-myc蛋白がテロメラーゼを誘導する可能性が推察された。

歯原性上皮の腫瘍化における細胞接着の動態を解明するため、歯原性上皮由来の腫瘍における上皮性細胞同士の接着に関与する接着分子E-カドヘリンおよびカドヘリンの細胞質内裏打ち蛋白αカテニンの発現・局在の免疫組織化学的検索を追加するとともに、透過型電子顕微鏡を用い細胞間接着装置の微細構造を観察した。 1. 細胞接着分子の免疫組織化学的検討(前年度の追加) エナメル上皮腫24例(濾胞型13例、叢状型11例で、棘細胞腫亜型6例、顆粒細胞亜型4例を含む)、歯原性石灰化上皮腫2例、歯原性淡明細胞腫1例、悪性エナメル上皮腫2例について、E-カドヘリンおよびαカテニンに対する免疫染色を行った。エナメル上皮腫では、濾胞型・叢状型ともに、基底膜に接する部位を除く細胞膜領域にE-カドヘリンおよびαカテニンの発現がみられた。いずれも、胞巣周辺の円柱状細胞よりも胞巣中央の多角形細胞で発現は強かった。角化部で発現は消失し、顆粒細胞で発現は減弱した。歯原性石灰化上皮腫および歯原性淡明細胞腫では、基底膜に接する部位を除く細胞膜領域に一様にE-カドヘリンおよびαカテニンの発現がみられた。悪性エナメル上皮腫では、1例はE-カドヘリンおよびαカテニンの発現が保たれていたが、1例では発現の減弱が認められた。 2. 細胞間接着装置の微細構造的検討エナメル上皮腫7例(濾胞型4例、叢状型3例で、棘細胞腫亜型1例、顆粒細胞亜型3例を含む)、歯原性石灰化上皮腫1例、歯原性淡明細胞腫1例について、超薄切片を作製し透過型電子顕微鏡で観察した。エナメル上皮腫では、腫瘍細胞間にデスモゾームが認められ、胞巣周辺の円柱状細胞よりも胞巣中央の多角形細胞で多かった。角化部で細胞間接着装置はみられず、顆粒細胞で少数のデスモゾームがみられた。歯原性石灰化上皮腫および歯原性淡明細胞腫では、腫瘍細胞間に一様にデスモゾームが散見された。

歯原性上皮は本来歯の発生に関わる組織であるが,嚢胞や腫瘍への病的変化も稀ではない。これらの病変は、歯の形成後消失すべき歯原生上皮が残存・増殖したものと考えられ、その過程でのアポトーシスの関与が注目されている。本研究では、歯胚(14例)、エナメル上皮腫(29例)および悪性エナメル上皮腫(2例)におけるアポトーシス関連因子について、免疫組織化学およびTUNEL法で検索した。 1.免疫組織化学 1) bc1-2蛋白の発現は、歯胚上皮14例(100%)、エナメル上皮腫26例(90%)、悪性エナメル上皮腫2例(100%)にみられた。 2) p53蛋白の発現は、歯胚上皮ではみられず(0%)、エナメル上皮腫14例(48%)、悪性エナメル上皮腫1例(50%)にみられた。エナメル上皮腫(p<0.001)および悪性エナメル上皮腫(p<0.01)における発現率は、歯胚上皮より統計学的に有意に高かった。腸性症例での陽性細胞率は、エナメル上皮腫が平均7.7%、悪性エナメル上皮腫が36.2%だった。 3) Le^y抗原の発現は、歯胚上皮14例(100%)、エナメル上皮腫21例(72%)、悪性エナメル上皮腫1例(50%)にみられた。エナメル上皮腫における発現率は、歯胚上皮より統計学的に有意に低かった(p<0.05)。 2. TUNEL法 TUNEL法による陽性細胞率の平均は、歯胚のエナメル器が0.4%、歯堤が0.7%、エナメル上皮腫が2.2%、悪性エナメル上皮腫が0.9%だった。エナメル上皮腫における陽性細胞率は、エナメル器(p<0.001)および歯堤(p<0.01)より統計学的に高かった。以上の結果より,歯原性腫瘍の腫瘍化にアポトーシスの関与が示唆された。

歯原性上皮は本来歯の発生に関わる組織であるが,嚢胞や腫瘍への病的変化も稀ではない。エナメル上皮腫は歯原性上皮由来の疾患として発症頻度が高く,その特徴の一つに良性腫瘍であるにも関わらず局所浸潤の傾向が強く再発しやすいことがあげられる。本研究では,エナメル上皮腫の間質への侵襲機構を解明するため,基質蛋白質分解酵素であるmatrix metalloproteinase(MMP)の発現・局在について免疫組織化学的に検索した。 1.組織切片および一次抗体の検討本学歯学部附属病院口腔外科での生検・手術より得られた歯胚4例,エナメル上皮腫16例,悪性エナメル上皮腫2例,口腔扁平上皮癌3例を,4%パラホルムアルデヒドPBS溶液で一晩固定し,通法に従いパラフィン包埋し,3μmの連続切片として用いた。一次抗体としては,MMP-1(間質コラゲナーゼ),MMP-3(ストロムライシン),MMP-9(ゼラチナーゼB)を用いた。免疫組織化学における酵素処理や熱処理などの前処理および一次抗体の希釈濃度を検討した結果,前処理としてはマイクロウェーヴ10分間照射で,また希釈濃度はMMP-1,3,9ともに10μs/mlで良好な染色態度を得た。 2.MMPの発現・局在の検討歯胚の上皮細胞では,MMP-1が4例中2例(50%)にMMP-3が4例中0例(0%)にMMP-9が4例中1例(25%)に陽性を示した。エナメル上皮腫では,MMP-1が16例中11例(81%)にMMP-3が16例中2例(13%)にMMP-9が16例中9例(56%)に陽性を示した。MMP-1は腫瘍細胞および間質細胞に発現を認めたのに対し,MMP-3,9は間質細胞のみに発現を認めた。悪性エナメル上皮腫では,MMP-1が2例中2例(100%)にMMP-3,9が2例中1例(50%)に陽性を示した。エナメル上皮腫と同様に,MMP-1は腫瘍細胞および間質細胞に発現を認めたのに対し,MMP-3,9は間質細胞のみに発現を認めた。口腔扁平上皮癌では,MMP-1が3例中3例(100%)にMMP-3が3例中2例(67%)MMP-9が3例中3例(100%)に陽性を示した。MMP-1,9は腫瘍細胞および間質細胞に,MMP-3は間質細胞のみに発現を認めた。以上の結果より,MMPの発現態度は腫瘍により異なり,腫瘍細胞の侵襲性に関与することが示唆された。
パラフィン包埋組織を用いたエナメル上皮腫に対するモノクローナル抗体の作製と応用

歯原性上皮は歯の発生に関わる組織であるが,嚢胞や腫瘍への病的変化も稀ではない。これらの病変の分類・診断において,歯原性を客観的に示す組織マーカーはごく少数に限られている。本研究では,歯原性嚢胞・腫瘍を他の由来の異なる類似疾患と鑑別することを目的とし,エナメル上皮腫を免疫原としてモノクローナル抗体の作製をみた。 1.モノクローナル抗体の作製本学歯学部附属病院口腔外科での手術より得られたエナメル上皮腫のホルマリン固定パラフィン包埋組織を,脱パラフィン・ホモジナイズし,(BALB/cxMRL+/+)F_1マウスおよび(B10.AxA.By)F_1マウスを免疫した。その脾細胞をマウス骨髄腫細胞(X63-Ag8-6.5.3.)とポリエチレングリコールで融合させ,HAT培地で培養した。そしてハイブリドーマの培養上清を用いて,免疫組織化学的にエナメル上皮腫に特異性を示すクローンとしてY4,M11を得た。更にそのクローンを増殖させ、培養上清をモノクローナル抗体の解析・応用に用いた。 2.モノクローナル抗体の解析マウス免疫グロブリンタイピングキットを用いたELISAおよび免疫組織化学により,Y4,M11のサブクラスはIgMだった。免疫組織化学的にはエナメル上皮腫に対してY4は基底膜と細胞質の一部に,M11は細胞質全体に顆粒状に反応がみられた。一方,正常歯肉重層扁平上皮に対してY4は基底層の一部に,M11は顆粒層のみに反応がみられた。これらはケラチンやラミニンなど既知の抗原とは異なる分布を示した。ウエスタンブロット法による反応抗原の分子量の解析は現在進行中である。 3.モノクローナル抗体の応用免疫組織化学的にY4はエナメル上皮腫,石灰化歯原性嚢胞,歯牙腫において上皮性腫瘍細胞に反応し,石灰化歯原性嚢胞ではghost cellや石灰化成分に対しても反応した。M11はエナメル上皮腫,悪性エナメル上皮腫,石灰化歯原性嚢胞,歯牙腫において上皮性腫瘍細胞に反応した。以上より,得られたモノクローナル抗体は歯原性上皮の性格を反映すると考えられ,組織診断に有用であると考えられた。
