    The public assistance system in Japan provides detailed and comprehensive livelihood support for low-income families with various needs. As one example, and the beneficiaries of the public welfare program in Japan can receive the same medical treatments as those insured of the universal public health insurance without any financial burdens. This system has greatly contributed to maintaining and improving the health of public assistance beneficiaries but may cause excessive healthcare utilization: moral hazard. This study uses a large sample taken from two nationally representative claim data for public assistance and public health insurance patients to estimate the magnitude of moral hazard effect in basic outpatient utilization. The results of the fixed-effect regression analysis utilizing the concept of pseudo panel data analysis and those of propensity score matching show that the average treatment effect of public assistance assignment on healthcare utilization is significantly positive. Specifically, public assistance assignment increases monthly healthcare expenditure by 17.5 to 22.9 percent and the monthly number of doctor visits by 23.1 to 27.8 percent, respectively. In addition, the average treatment effects on the treated are also significantly positive, suggesting that monthly healthcare expenditure significantly decreases by 22.7 to 25.0 percent and the number of visits by 27.6 to 29.7 percent, respectively, when imposing a copayment on public assistance beneficiaries. However, the estimated price elasticity based on these results is very small, approximately −0.02, indicating that the level of copayment rate has little effect on the intensive margin of outpatient healthcare utilization.

    <title>Abstract</title> The Great East Japan Earthquake of March 2011 and it's massive tidal waves wreaked devastating damage on residences and communities along 400 km of the Pacific coast in Japan. Using the panel data from the Japanese Study of Aging and Retirement, we estimate the effects of this disaster on individual health, finding a significant negative impact on the health of those in the area. In addition, we exploit a discontinuous decrease in copayments for medical care at the age of 70 years and find that the reduction in copayment increased medical and long-term care utilization and significantly reduced health deterioration after the earthquake.

    In this research project, I have mainly conducted several empirical analyses on (1) the utilization of medical and long-term cares, (2) the Japanese medical assistance system, and (3) social environment and delivery choice. Specifically, I examine the effect of the health condition of the informal caregiver on their parent's (including those in-laws) care level in the project (1). In the project (2), I statistically compare the outpatient and inpatient utilization of the medical assistance patients with those of public health insurance. In the project (3), I examine the effects of recent public and social environment changes and Caesarean section delivery choice. In addition, I conducted other empirical analyses on smoking behavior and public finance of the health insurance system in Japan.

    Although the projection until the Fiscal Year 2025 has been used in discussions on social security finance, a longer time horizon is needed to deal with real issues caused by population aging, because the ratio of elderly to total population will not peek out in a first half of this century. This research discusses how to finance the future health care and long-term care costs, which will rise sharply. It also examines alternative financing schemes that aim to smooth insurance premiums during a century to lighten the future burdens. We identify a practically reasonable implementation of financing scheme to smooth burdens among generations.

    In this research project, I employ the nationally representative individual medical claim data, the Fact-finding Survey on Medical Assistance and the Survey of Medical Care Activities in Public Health Insurance from 2001 to 2007, conducted by the Ministry of Health, Labour and Welfare in Japan and examine the differences in the demand and supply of the medical care between the beneficiaries of the Public Assistance system and the enrollees of the public health insurance systems. I find that medical suppliers provide unnecessary inpatient and outpatient treatments to the beneficiaries and enrollees in response to the medical fee reductions and that the beneficiaries more visit medical institutions than the enrollees in their second and subsequent months.

    This research program projects the future financial status of health and long-term care insurances, and analyzes the shape of burdens among generations. At an earlier stage of this research, the estimation of lifetime burdens by generation implies that the transition from the present system to the funded insurance system will reduce the inequality in burdens. At a later stage, the stochastic simulation model is developed. Since the qualitative results are the same as in the deterministic model, the uncertain future economic conditions are not likely to be a hurdle for the reform that introduces the funded system.
