    Effects of long-term bisphosphonate (BP) administration on the metabolism of healthy bone and the concomitant changes in imaging are unclear. Hence, we aimed to retrospectively investigate the effects of long-term BP administration on the intact parietal bone using the standardised uptake value (SUV) derived from single photon emission computed tomography (SPECT). We enrolled 29 patients who had odontogenic infection, osteoporosis, bone metastasis cancer, or rheumatoid arthritis, and classified them into BP-naïve: A (14 patients) and BP-treated: B, < 4 years (7 patients) and C, ≥ 4 years (8 patients) groups. We measured the maximum bilateral SUV (SUVmax) of the parietal bone using quantitative bone SPECT software. There were significant differences in the duration of BP administration and SUVmax of the parietal bone among the diseases (P < 0.0001 and P = 0.0086, respectively). There was a positive correlation between the duration of BP administration and SUVmax of the parietal bone (rs = 0.65, P = 0.0002). The SUVmax was significantly different between A and B (P = 0.02) and between A and C (P = 0.0024) groups. This is the first report on the correlation between long-term BP administration and the SUVmax of the parietal bone using the quantitative bone SPECT analysis.

    当科で経験した術後せん妄のリスク因子について検討するとともに、口腔癌手術においてE-PASS(estimation of physiologic ability and surgical stress)を応用し、せん妄発症の予測因子としての有用性について検討した。2010年1月〜2017年5月に当科で口腔癌の手術を行った134名を対象とし、そのうち28名は2回以上の手術歴を有していたため1手術1例と考え合計164例とした。術後せん妄は164例中14例(8.5%)に発症した。せん妄発症の術前リスク因子ではBenzodiazepine(BZ)系薬服用歴と糖尿病で有意差を認め、手術に関するリスク因子では手術時間、出血量、気管切開術が、術後リスク因子では頸部ドレーンの留置と術後Alb値が抽出された。単変量解析で有意差を認めた術前リスク因子のうち、E-PASSの術前評価項目である術前リスクスコア(PRS)とE-PASSの構成要素に含まれていない術前リスク因子であるBZ系薬剤服用歴の2項目を説明変数とし多変量解析を行った結果、BZ系薬剤服用歴とPRS>0.36のそれぞれが独立したリスク因子であった。

    We report two cases of central venous catheter-related deep venous thrombosis (DVT) that occurred after oral cancer resection and reconstruction with vascularized free flap. In addition, we present a manual on coping with DVT which has been used by dentists in the Hokkaido University Hospital since July 2014.&lt;br&gt;First case: A 60-year-old man with oral floor cancer (cT4aN1M0) underwent tumor resection, neck dissection, and reconstruction with anterolateral thigh flap in May 2010. At the time of administration of general anesthesia, a central venous catheter (CVC) was inserted through the femoral vein and it was removed five days after the operation. In April, prior to the operation, early gastric cancer was detected endoscopically. Three months after the operation, enhanced CT was performed before endoscopic submucosal dissection for the gastric cancer. A 7-cm thrombus was detected in the inferior vena cava, which was diagnosed as deep venous thrombosis (DVT). Warfarin was administered as an anticoagulant. Two weeks later, the size of the thrombus decreased, and it disappeared after two more months of warfarin therapy.&lt;br&gt;Second case: A 69-year-old woman with tongue cancer (cT2N0M0) underwent tongue resection, neck dissection, and reconstruction with anterolateral thigh flap in February 2014. At the time of administration of general anesthesia, a CVC was inserted through the femoral vein; it was removed seven days after the operation. Drug-induced hepatopathy related to prostaglandin E1 occurred one week after the operation. Ultrasonography revealed DVT. Heparin was immediately administered intravenously as an anticoagulant. The thrombus disappeared after heparin therapy for a week.

    Patients receiving oral bisphosphonates (BP) are at risk for developing bisphosphonate-related osteonecrosis of the jaw (BRONJ), but to a much lesser degree than those treated with intravenous BP. Patients treated with oral BP were reported to have less extensive and / or refractory BRONJ. However, the risk of BRONJ associated with oral BP is reported to increase when the duration of therapy exceeds 3 years.&lt;br&gt;We performed 208 extractions in 99 patients who received oral BP for more than 3 years, and their postoperative course was evaluated retrospectively. These subjects were all female (average age, 72 years). BP was orally administered to 73 patients with osteoporosis, 13 with RA, 3 with SLE, and 10 with other diseases. The mean duration of oral BP exposure was 60 months (median 54 months). Treatment with oral BP was discontinued in 83 patients. There were 24 patients receiving steroids, 13 receiving immunosuppressants, and 14 with diabetes mellitus.&lt;br&gt;Six patients had delayed healing with bone exposure in sockets 4 weeks after tooth extraction ; furthermore, BRONJ developed in 2 other patients (2%). In these 8 patients, we studied the relations between delayed healing and various clinical factors such as age, the type, duration, and holidays of BP, risk factors, steroids, immunosuppressants, steroids plus immunosuppressants, anti-inflammatory treatment before extraction, site and type of extraction, and closure of the extraction socket. Discontinuation of BP, steroids plus immunosuppressants, and type of extraction were significant independent risk factors for delayed healing.

    Recently, zoledronic acid was approved for the indication of bone metastasis from solid tumors and multiple myeloma, and the use of bisphosphonates （BP） has been increasing every year. These drugs are associated with a risk of intravenous bisphosphonate-related osteonecrosis of the jaws （BRONJ）. This study was designed to evaluate the treatment and prevention of BRONJ in cancer patients in our hospital. We performed a retrospective analysis of 69 patients treated with intravenous bisphosphonates （IVBPs） during 5 years （2008 through 2012）. Among the 69 patients, 36 had received dental care before treatment with IVBPs and 33 had not received dental care. Of the 36 cases who were provided professional dental care before initiating treatment with IVBPs, none developed BRONJ. Thirteen patients underwent tooth extraction, and none of them had delayed healing. At the first medical examination in our hospital, of the 33 patients who had a prior history of IVBP exposure, 16 （48%） already had BRONJ and 4 （24% 4/17） developed BRONJ thereafter. Of these 20 patients, BRONJ was diagnosed after dental extraction in 11 patients and on spontaneous exposure in 9. Dental evaluation and treatment helped to prevent BRONJ before initiating IVBP treatment.

    Churg-Strauss syndrome is a rare, systemic inflammatory disease characterized by pre-existing type-I allergic symptoms, such as bronchial asthma or allergic rhinitis, and necrotic and granulomatous vasculitis with eosinophilia. We describe our experience with a case of this disease associated with cheek pain.&lt;br&gt;A 64-year old man was referred to our hospital because of pain of the cheek and fever of unknown. He had a past history of chronic sinusitis. Computed fomography and magnetic resonance imaging showed bilateral multiple cystic lesions in the maxilla, and a postoperative maxillary cyst with infection was diagnosed initially.&lt;br&gt;The patient received antibiotic therapy, but the symptoms did not improve. Blood examinations revealed eosinophilia and an increased serum IgE level. Symptoms of mononeuritis multiplex of the arm and leg, renal disorder, and micro-cerebral hemorrhage developed sequentially, and Churg-Strauss syndrome was given diagnosed. He received prednisolone and high-dose gamma globulin therapy, and the symptoms improved.

    症例は75歳男性で、転倒して下顎を強打し、疼痛および開口障害が生じた。X線で萎縮した無歯顎の両側下顎骨体部に骨折を認め、左側では小骨片の変位がみられた。手術適応と考えたが、口内法によるミニプレート固定では義歯の支持域を避けての強固な固定を得ることは困難と判断し、口腔外から固定を行う方針とした。下顎下縁の切開により骨折部を明示し、骨片を徒手整復した後、厚さ2mmの再建用6穴チタンプレート2枚およびロッキングスクリュー12本を用いてmonocorticalに固定を行った。さらに義歯支持域の歯槽頂を避けて、厚さ1mmのミニプレートおよび5mmのスクリュー8本で追加固定した。術後左側顔面神経下顎縁枝領域に軽度の麻痺を認めたが、7日目には消失し、総義歯装着も可能であった。10ヵ月経過して骨片間の良好な骨癒合を認め、プレートやスクリューの脱落などもない。
