    ObjectiveTo characterize the presurgical anthropometric features of Japanese patients with unilateral cleft lip and identify factors associated with lip-height and -width discrepancies.DesignRetrospective analysis of operative records.SettingSingle-university hospital in Japan.PatientsOne hundred and seventy-four Japanese patients with unilateral cleft lip who underwent primary cheiloplasty between July 2007 and September 2016.Main Outcome MeasuresVertical lip-height (n = 174) and horizontal lip-width (n = 73) measurements on cleft and noncleft sides.ResultsMean medial lip height was 10.5 mm on the noncleft side versus 5.5 mm on the cleft side (average discrepancy, 5.0 mm). Mean lateral lip width was 18.9 mm on the noncleft side versus 16.4 mm on the cleft side (average discrepancy, 2.5 mm). Cleft-side lip height was shorter in all cases, and the width was narrower in 81% of cases. Complete alveolar cleft (P = .0156) and male sex (P = .0357) were significantly associated with greater lip-height discrepancy, whereas cleft palate alone was not independently significant. Only laterality was significantly associated with lip-width discrepancy; right-sided clefts showed larger discrepancies than left-sided clefts (P = .0010).ConclusionsPatients with unilateral cleft lip demonstrated substantial lip-height discrepancies, considerably larger than those reported in other populations. Complete alveolar cleft and male sex were independently associated with greater lip-height discrepancy, whereas right-sided clefts showed greater lip-width discrepancy. These findings provide valuable reference data for surgical planning in Japanese patients. It is important to consider these factors when selecting cheiloplasty techniques.

    BACKGROUND: Pediatric orbital trapdoor and linear fractures may involve entrapment of the extraocular muscles (EOM), potentially causing irreversible ocular motility impairment if surgical intervention is delayed. The optimal timing for surgery in cases with EOM strangulation remains uncertain. METHODS: We retrospectively reviewed 103 pediatric cases from our medical records of 389 patients with orbital fracture. A systematic review was conducted following PRISMA 2020 guidelines. Data from 10 previous studies and our institutional cohort were combined, including 101 patients under 20 years old with radiologically confirmed EOM entrapment. Patients were stratified by time to surgery: within 24 hours, 24-72 hours, and beyond 72 hours. RESULTS: Surgical intervention within 24 hours was significantly associated with better postoperative ocular motility outcomes compared to delayed surgery (p < 0.05). In our cohort, cases treated within 8 hours achieved excellent recovery, with Hess Area Ratio (HAR%) exceeding 85% and no residual diplopia. Patients without EOM entrapment improved with an average of 7 weeks of conservative management. CONCLUSIONS: This study supports urgent surgical repair within 24 hours for pediatric orbital fractures with EOM entrapment to maximize functional outcomes. In cases of soft tissue entrapment without sustained oculocardiac reflex, non-operative management with close follow-up may be appropriate. Prompt diagnosis and timely referral remain critical to avoid permanent sequelae.

    In bilateral cleft lip and palate patients, secondary rhinoplasty is often complicated by a limited skin envelope, which restricts nasal tip projection despite the use of septal extension grafts (SEGs). We proposed a technique based on the concept of rebalancing the existing skin envelope, without the use of local flaps, skin grafts, or composite grafts. We reported 2 cases of adult male patients with bilateral cleft lip and palate who underwent secondary cleft rhinoplasty. Both patients exhibited a cleft-specific configuration characterized by an underprojected nasal tip and a relatively high dorsum at the rhinion. In addition, 1 patient presented with a broad nose deformity, and the other exhibited a crooked nose. A dorsal preservation technique-either push-down or let-down-was applied to reduce dorsal height at the rhinion and create redundancy in the skin envelope. An SEG using costal cartilage was then used to reconstruct and project the nasal tip. Structural grafts were additionally used to correct nasal asymmetries when necessary. At 1-year postoperative follow-up, both patients demonstrated marked improvements in nasal profile, symmetry, and tip projection. No complications were noted. Rebalancing the skin envelope through targeted reduction at the rhinion may allow for improved tip projection in cleft rhinoplasty. This approach may expand the applicability of SEGs in challenging cleft cases and warrants further study.

    The corrosion rate of these alloys is observed to be the fastest immediately after implantation. In addition, bleeding can accelerate corrosion and cause treatment failure in vivo, and therefore, it is necessary to optimize the corrosion rate. The brushite (CaHPO4・2H2O) coating treatment, a type of calcium phosphate, is used as a method to optimize the corrosion rate of magnesium and is expected to have the same coating effect under hematoma as under non-hematoma conditions. The purpose of this study was to evaluate the effect of brushite coating treatment with and without hematoma up to 14 days of implantation in vivo experiments. Specimens of cylindrical Pure Magnesium (99.9%, 1.2 mm × 6 mm), treated with a brushite coating by precipitation, were implanted in hematoma or non-hematoma rat femurs. The insoluble salts that precipitated on the specimens were analyzed by inductively coupled plasma mass spectrometry, scanning electron microscopy-energy dispersive X-ray analysis, and Raman spectroscopy. Results showed that weight loss at 14 days of implantation in the hematoma group (24.83 ± 2.26 μg mm-2) was significantly greater than in the non-hematoma group (18.88 ± 1.95 μg mm-2). The hematoma group had increased precipitation of insoluble salt magnesium and phosphorus. However, the amount of magnesium contained in insoluble salt per weight loss was significantly greater in the non-hematoma group at all time points, particularly after 1 days of implantation (non-hematoma group: 19.42% ± 3.56%; hematoma group: 10.73% ± 0.41%, p < 0.01). The brushite coating transformed the insoluble salts in the surface layer to an optimum structure compatible with the implantation environment. This enhanced the coating's protective function and more effectively inhibited magnesium corrosion. Given the clinical challenge of fully controlling intra-and postoperative bleeding, a brushite coating adaptable to hematoma conditions offers significant utility.

    The corrosion of magnesium (Mg)-based bioabsorbable implanting devices is influenced by implantation environment which dynamically changes by biological response including wound healing. Understanding the corrosion mechanisms along the healing process is essential for the development of Mg-based devices. In this study, a hematoma model was created in a rat femur to analyze Mg corrosion with hematoma in the early stage of implantation. Pure Mg specimen (99.9%,ϕ1.2 × 6 mm) was implanted in rat femur under either hematoma or non-hematoma conditions. After a designated period of implantation, the specimens were collected and weighed. The insoluble salts formed on the specimen surfaces were analyzed using scanning electron microscopy, energy-dispersive x-ray spectroscopy, and Raman spectroscopy on days 1, 3, and 7. The results indicate that hematomas promote Mg corrosion and change the insoluble salt precipitation. The weight loss of the hematoma group (27.31 ± 5.91 µg mm-2) was significantly larger than that of the non-hematoma group (14.77 ± 3.28 µg mm-2) on day 7. In the non-hematoma group, carbonate and phosphate were detected even on day 1, but the only latter was detected on day 7. In the hematoma group, hydroxide was detected on day 1, followed by the formation of carbonate and phosphate on days 3 and 7. The obtained results suggest the hypoxic and acidic microenvironment in hematomas accelerates the Mg corrosion immediately after implantation, and the subsequent hematoma resorption process leads to the formation of phosphate and carbonate with organic molecules. This study revealed the risk of hematomas as an acceleration factor of the corrosion of Mg-based devices leading to the early implant failure. It is important to consider this risk in the design of Mg-based devices and to optimize surgical procedures controlling hemorrhage at implantation and reducing unexpected bleeding after surgery.

    This study aimed to evaluate the advantages of using a high-powered ultrasonic vibration cutting device (HP-UVCD) in orthognathic surgeries. We analyzed 22 consecutive patients who underwent Le FortⅠosteotomy and bilateral sagittal split osteotomies for cleft lip and palate. We used conventional power instruments, such as reciprocating saws and rotating burrs in the first 10 patients, and an HP-UVCD in the remaining 12 patients. Surgical time, intraoperative blood loss, and postoperative inferior alveolar nerve affection were retrospectively investigated. The surgical time in the ultrasonic group (223±12.217 min) was significantly shorter than that in the conventional group (278±11.153 min, P=0.008). The amount of intraoperative blood loss in the ultrasonic group was an average of 230±45.154 g, and that in the conventional group was an average of 343±49.463 g. Although the blood loss in the ultrasonic group was reduced compared with that in the conventional group, this difference was not statistically significant (P=0.118). All patients in both groups recovered normal sensation by 6 months postoperatively. Our findings suggest that the HP-UVCD can be used in place of a reciprocating saw or rotating burrs in orthognathic surgeries, resulting in reduced surgical time and minimal blood loss. The use of an ultrasonic bone-cutting device may allow for a safer and less invasive orthognathic surgery.

    OBJECTIVE: Children with cleft palate (CP) are at high risk of developing otitis media with effusion (OME) due to Eustachian tube (ET) dysfunction. Palatoplasty has been reported to decrease the frequency of middle ear disease and improve ET function, and although various techniques have been developed, there is no consensus on the differences in the impact of different techniques on the middle ear. The purpose of this study was to determine the differential effects of palatoplasty on middle ear function and hearing. METHODS: We performed a retrospective observational survey of pediatric patients who underwent palatoplasty for CP between June 2010 and October 2018 at Tohoku University Hospital. Cases were divided into three groups depending on the palatoplasty procedures performed: the push-back palatoplasty group, the two-flap palatoplasty group, and the Furlow double-opposing Z-plasty group. We examined the differences in clinical characteristics between patients who underwent each procedure. The primary outcome variable was tympanic membrane (TM) findings, and the secondary outcome was hearing test results. RESULTS: Children who underwent the two-flap palatoplasty had a higher tympanostomy tube (TT) insertion rate and a higher total number of TT insertions than those who underwent the Furlow double-opposing Z-plasty or the push-back palatoplasty. The TM retraction rate tended to be lower in the Furlow double-opposing Z-plasty group than in the push-back palatoplasty group or the two-flap palatoplasty group. The hearing test results at the last visit were not significantly different among the three groups. CONCLUSIONS: Children who underwent the two-flap palatoplasty had a higher rate of TT insertions, potentially increasing the risk of TM perforation. In contrast, the Furlow double-opposing Z-plasty group had a lower tendency for TM regression, possibly due to improved ET function and reduced incidence of OME. It is important to understand the advantages and disadvantages of each technique before selecting one suitable for the child's cleft and arch width. Additionally, it is important to conduct regular follow-up of TM findings and hearing test results even after palatoplasty.

    Background Cleft lip is the most common congenital anomaly worldwide. Nevertheless, lip somatosensory characteristics of patients with cleft lip after cheiloplasty have not yet been determined. The present study used magnetoencephalography to objectively evaluate the lip sensation in patients with unilateral cleft lip to establish a new objective evaluation method. Methods Participants were 15 patients with unilateral cleft lip after cheiloplasty (UCL group), and 30 healthy young subjects (control group). Five points of the upper and lower lips were stimulated electrically to measure somatosensory evoked magnetic fields (SEFs). The sources of the magnetic fields were modeled as single equivalent current dipoles (ECDs). ECDs located on the central sulcus by superimposition on magnetic resonance images were analyzed. Latency and intensity at 50–75 ms (cP60m) observed in the UCL group were compared with those in the control group. Thresholds of tactile stimuli in both groups were obtained using Semmes–Weinstein monofilaments for subjective sensory evaluation. Results No significant difference was found in the intensity of the cP60m or subjective evaluation between the groups. However, the latency of the cP60m was significantly longer in the upper lip of the UCL group than in the control group. Conclusions SEFs showed a difference in lip sensation between the UCL group and the control group, suggesting that longer latency might be caused by the effects of surgical scarring on the neurotransmission pathway. These results suggest SEFs as useful for the objective evaluation of lip sensations. This study might improve future surgical procedures and lip functions of patients with cleft lip.

    11歳女児。脳器質疾患を有する17歳実兄より木製ハンガーで顔面頭部を殴打され裂傷し、翌日に母親とともに当院形成外科を受診した。受診時、顔面外傷そのものは創処置を要さないものであり保存的加療のみとしたが、患児のおびえが強く、母親も混乱している状態であったため、保護的観点から形成外科入院を勧めた。その後、小児科にて児童虐待防止対策チームとしての対応が可能であることが判明したため、小児科入院となった。小児科にて5日間入院し、母親の実家へ退院となり、その後臨床心理士の関与を含め小児科外来で経過観察中である。

    症例は18歳男子で、9歳時に原因不明の貧血、16歳時に胃十二指腸潰瘍の既往があった。顔面、頭部の皮膚肥厚、深い皺襞を自覚し、17歳時に肥厚性皮膚骨膜症と診断された。局所麻酔下に前額部、眉間部、両側頬部の除皺術を受け、18歳時に顔面、頭部形成のため受診した。頭部CTでトルコ鞍の拡大、頭蓋内病変は認めなかった。四肢単純X線で前腕、下腿の長管骨に骨膜性の骨肥厚が認められた。完全型肥厚性皮膚骨膜症と診断し、19歳時に局所麻酔下に両側頬部、オトガイ部の除皺術を施行した。両側頬部は1×5cm、オトガイ部は1×4cmほど皺襞部分を舟形に切除し縫縮した。創傷治癒に問題はなかった。21歳時に全身麻酔下に頭部の除皺術、前頭部の吊り上げ術を施行した。初回手術から7年、26歳時に顔面は再び頬部から下顎部にかけ深い皺襞が認められたが、頭部は良好な形態を保っており、以前の手術で生じたドッグイヤーを残すのみであった。再度、顔面の除皺術、頭部のドッグイヤーの修正を全身麻酔下に施行した。術後1年、顔面、頭部共に良好な形態を保っており患者の満足が得られた。

    近年経験した眼窩内壁・下壁合併骨折8例(男性7例、女性1例、平均32歳)の手術症例について検討した。8例全例で経結膜切開を延長し、涙丘下切開とつなげて骨折部へのアプローチを行っていた。再建・骨固定材料は1例が自家骨(腸骨)、1例がチタンメッシュプレート、ほかの6例はハイドロキシアパタイト・ポリL-乳酸複合体シートを用いていた。術後の眼球陥凹については記録のあった6例すべてで2mm以内の陥凹であった。術前の眼球陥凹については、腫脹の影響もあり、ヘルテル氏眼球突出計での計測結果が不良となった症例がほとんどであり、今回の検討では評価しなかった。術後のヘスコージメータの結果は2例に記録がなかったが、記録がある6例全例で眼球運動制限の残存を認め、そのうちの2例では自覚的複視も残存していた。

    症例1は17歳男性で、出生時より左唇顎口蓋裂を認めた。前医にて口唇形成術、口蓋形成術、外鼻修正術、顎裂部骨移植を施行された。当科にて12歳時に咽頭弁形成術を施行し、歯科にて顎矯正手術を前提とした咬合管理が行われてきた。成長終了を待って顎矯正手術を予定し、手術時の気道確保は経鼻挿管ではなくオトガイ下挿管で行う方針とした。術後1年、咬合は矯正加療中であるが正被蓋に改善している。オトガイ下挿管の瘢痕は目立たない。症例2は18歳女性で、出生時より顔面正中部に発生した過誤腫による顔面の多発異常を認めた。今回、反対咬合を主体とする咬合の異常に対して顎矯正手術を行い、麻酔科により経鼻挿管は困難と判断されたため、オトガイト挿管で術中気道管理を行う方針とした。術後1年、上下顎の関係および咬合は改善し安定している。オトガイ下挿管の瘢痕は目立たない。

    症例は62歳女性で、右前額部皮下腫瘤の増大を主訴とした。近医耳鼻咽喉科にて前頭洞炎を疑われたが抗生剤治療で改善を認めなかった。前頭洞粘膜の生検でmeningiothelial meningiomaの病理診断であったため、脳神経外科へ紹介となった。年1回のMRI撮影による経過観察で頭蓋内病変は認めなかったが、前額部の隆起が徐々に醜状を呈したため整容改善を目的に形成外科へ紹介となった。術前画像診断では前頭洞原発の頭蓋外髄膜腫が前頭骨内・蝶形骨を含む頭蓋底・皮膚に浸潤が疑われ、全切除は困難な状態であり、前頭部皮下の腫瘍切除減量手術を行った。腫瘍は頭蓋骨・皮膚側いずれからも剥離可能であり、ほとんど出血を認めなかった。病理組織診断は前頭洞粘膜の生検と同じであり、切除断端は骨側・皮膚側ともに腫瘍細胞が露出していた。術後外観上の問題が軽減して患者は満足している。

    β-TCP含有マグネシウム合金の腐食挙動のメカニズムを解明するに先立ち、マグネシウムの分解挙動を把握する必要性が生じたため、予備的に腐食挙動の試験を開始した。これまで腐食のメカニズムはin vitroとin vivoではデータに解離があることが多かったが、臨床応用をする際に影響が避けられない出血による影響が十分に検討されていなかったため、出血の影響を確認した。ラットの大腿骨で出血モデルを作成し、マグネシウム試料を埋植し解明を行った。 出血はマグネシウムの腐食を促進すること分かった。また、腐食による不溶性塩がマグネシウム試料の表面を覆うにつれて、経時的に腐食は抑制されたが、出血がない環境と比べて腐食速度が速いことが確認できた。さらに、SEM-EDX、ICP-MS、ラマン分析により出血環境下では埋植初期に生じる不溶性塩の組成や構造が異なることが確認され、腐食速度にも影響を与えていることが確認できた。出血環境下でのpHの変化やイオン拡散速度が異なることで、析出が異なることが示唆された。 また、高解像度マイクロCTにより、血液にマグネシウムを浸漬した場合に、埋植1日までにガス空隙を形成することが確認され、経時的に縮小することが確認された。これは、臨床応用時にマグネシウム製の骨固定材の周囲に出血がある場合には腐食の促進とともにガス空隙が生じやすく、骨壊死などの合併症のリスクが高くなる可能性を意味している。

    In the development of biodegradable osteosynthesis materials using magnesium, suppression of gas generation during decomposition is an issue to be solved.In this study, we focused on the fact that the initial decomposition reaction is suppressed by applying a film treatment to the alloy surface. Then, the degree of decomposition suppression was examined from the amount of gas generated.Although the composition of the corrosion-resistant film treatment and the actual number of gas generated are confidential, the film treatment significantly suppressed gas generation at all time points.This study suggests the usefulness of corrosion-resistant film treatment for optimizing the corrosion rate.
