    Abstract We have studied a noninvasive and quantitative method for measuring the aggregate size of red blood cells (RBCs). In our previous study, the scattering power spectrum obtained from the vascular lumen was fitted to the reference scattering spectra calculated from the scattering spectrum of a single-sphere scatterer. In this paper, we propose a method for calculating the reference scattering spectra by summing the scattering spectra for numerous scatterers in the ultrasound focal region. By applying this method to the size estimation of microparticles simulating RBC aggregates, the estimated sizes are found to be close to the true values. In in vivo measurements, the estimated sizes at rest are equivalent to the size of a single sphere with the same volume as an RBC, and those during avascularization are larger than those at rest, which is reasonable. The proposed method has the potential to accurately estimate the size of RBC aggregates.

    Abstract Analysis of the envelope statistics of ultrasound echo signals contributes to quantitative tissue characterization in medical ultrasound. Many probability distribution model functions have been studied, and the model function that should be used for tissue characterization depends on the type of disease, even in the same organ. Thus, an appropriate model selection is important for an accurate diagnosis. In this study, we aimed to select a model using threshold processing for modeling errors instead of a simple selection by minimizing the modeling error. For this purpose, we compared several indicators of modeling errors using random number simulations, ultrasonic simulation, and phantom experiment. The results validated that the Mahalanobis distance of moments is an appropriate indicator because it enables the use of a constant threshold value, regardless of the type of model function and data length.

    Abstract This study proposes a method for precise measurement of time waveforms of minute sinusoidal displacements using continuous waves. The technique involves measuring the S-parameter of ultrasonic waves reflected from an object undergoing sinusoidal vibration with a network analyzer. When the sinusoidal displacement is not sufficiently small, second-order side peaks emerge in its inverse Fourier transform, alongside the main and first-order side peaks. This method, which incorporates second-order side peaks previously overlooked, enhances the accuracy of measurements. It facilitates precise measurement of sinusoidal displacements ranging from 0.3 to 50 μm at frequencies between 10 and 300 Hz.

    We developed a method for generating continuous sinusoidal displacements of an object to estimate viscoelastic parameters. However, the amplitude of the displacement caused by the ultrasonic excitation force under safe guidelines was small (a few micrometers), and it was difficult to stably measure the displacement. Therefore, to stably measure the amplitude of sinusoidal displacement as small as the order of micrometers, we proposed a novel method using a network analyzer. Ultrasonic waves were irradiated using an ultrasonic transducer to an object vibrating sinusoidally. The S parameter of the first reflected wave received from the surface of the object was measured using a network analyzer. The S parameter and the inverse Fourier transform were formulated theoretically, and the amplitude of the sinusoidal displacement of the object was estimated from the amplitude characteristics of the inverse Fourier-transformed signal. The proposed method was applied to measure sinusoidal displacements on the order of micrometers from 10 to 300 Hz on an object using a water tank experiment. The obtained sinusoidal displacement agreed well with the reference values measured using a laser displacement meter. The proposed method can accurately measure minute sinusoidal displacements that occur on an object.

    Abstract Purpose Estimating the speed of sound (SoS) in ultrasound propagation media is important for improving the quality of B-mode images and for quantitative tissue characterization. We have been studying a method for estimating the SoS by measuring the reception time distribution of waves scattered from a scatterer at the elements in a probe. Previously, the measurement cross section was assumed to be perpendicular to the long axis of the blood vessel. In this study, we experimentally investigated the relationship between rotation angle $$\theta$$ of the probe relative to the short-axis plane of the blood vessel and the estimated SoS, $$\widehat{c}$$. Methods Water tank and phantom experiments were conducted to investigate the characteristics of $$\widehat{c}$$ and element signals when the probe was rotated. Results The received signal powers at the elements around both edges greatly decreased as $$\theta$$ increased. We introduced a parameter representing the decrease in power, $${P}_{\mathrm{dec } }$$, in the received signal at the elements at both edges relative to the center element. $$\widehat{c}$$ was estimated to be larger as $$\theta$$ increased, especially for $$\theta \ge 30^\circ$$. $${P}_{\mathrm{dec } }$$ also increased as $$\theta$$ increased. An approximately proportional relationship existed between the errors in $$\widehat{c}$$ and $${P}_{\mathrm{dec } }$$. Conclusion Based on these results, we can distinguish between the presence and the absence of SoS misestimations using the difference in power among the elements in the received signal. In the absence of misestimation, we can obtain the true SoS, even if the target has a non-negligible size, by applying our previously proposed methods.

    Abstract Purpose Quantitative diagnosis of the degree of fibrosis progression is currently a focus of attention for fatty liver in nonalcoholic steatohepatitis (NASH). However, previous studies have focused on either lipid droplets or fibrotic tissue, and few have reported the evaluation of both in patients whose livers contain adipose and fibrous features. Our aim was to evaluate fibrosis tissue and lipid droplets in the liver. Methods We used an analytical method combining the multi-Rayleigh (MRA) model and a healthy liver structure filter (HLSF) as a technique for statistical analysis of the amplitude envelope to estimate fat and fibrotic volumes in clinical datasets with different degrees of fat and fibrosis progression. Results Fat mass was estimated based on the non-MRA fraction corresponding to the signal characteristics of aggregated lipid droplets. Non-MRA fraction has a positive correlation with fat mass and is effective for detecting moderate and severe fatty livers. Progression of fibrosis was estimated using MRA parameters in combination with the HLSF. The proposed method was used to extract non-healthy areas with characteristics of fibrotic tissue. Fibrosis in early fatty liver suggested the possibility of evaluation. On the other hand, fat was identified as a factor that reduced the accuracy of estimating fibrosis progression in moderate and severe fatty livers. Conclusion The proposed method was used to simultaneously evaluate fat mass and fibrosis progression in early fatty liver, suggesting the possibility of quantitative evaluation for discriminating between lipid droplets and fibrous tissue in the early fatty liver.

    Abstract Ultrasound echo envelope statistics have been widely studied for quantitative tissue characterization. In ultrasound measurements, the size of the region-of-interest (ROI) is limited by several factors, such as the locality of the tissue characteristics and the depth dependence of the acoustic field of the ultrasound beam. In this case, the evaluated echo envelope statistics vary even when the envelope amplitudes follow the same population without any noise. In this study, the statistical variance of the moments caused by this finite number of samples was quantified based on the central limit theorem and the law of error propagation. The proposed principles were validated by random number simulation and used to quantify the statistical variance of Nakagami parameter estimation. Finally, the effective number of independent samples in an ultrasonic measurement was quantified based on the relationship between the ROI size and the ultrasound spatial resolution.

    Abstract We compared the evaluation accuracy of amplitude envelope statistics under the transmission and reception conditions of compounded plane wave imaging (CPWI) and focused beam imaging (FBI). In a basic study using a homogeneous phantom, we found that the amplitude gradient in the depth direction and the point spread function in the lateral direction spread in the FBI reduced the accuracy of evaluation in amplitude envelope statistics. On the other hand, CPWI showed a more stable evaluation than FBI because of the elimination of sound field characteristics. In CPWI, the multi-Rayleigh model discriminated signals from two types of scatterer with high accuracy in the evaluation using phantoms mimicking fatty liver. It was confirmed that the combination of CPWI and the multi-Rayleigh model is effective for detecting early fatty liver disease. The results show that CPWI is effective for improving the robustness of amplitude envelope statistics.

    Abstract For early diagnosis of atherosclerosis, we have developed a method to measure the initial minute surface roughness utilizing the natural longitudinal displacement of the intima-media-complex (IMC) on the carotid artery caused by pulsation. However, the IMC displaces not only in the longitudinal direction but also in the axial direction due to the pulsation. In the present paper, we proposed a novel method to remove the pulsation component by spatially convolving a high-pass filter with the measured depths of IMC in the longitudinal direction. The proposed method was validated by a phantom experiment, and the surface roughness with ten saw teeth was correctly measured. Next, the surface roughness of the carotid artery wall was measured for two healthy volunteers. The proposed method reduced the variation of measured surface roughness among beams compared to the previous method.

    Abstract Improving the accuracy of heart wall motion measurement is essential to realise better cardiac function evaluation. This paper proposed a two-dimensional (2D) displacement estimation method with a high temporal resolution using the 2D complex cross-correlation of element RF signals of an ultrasonic probe between frames returned from the target scatterers. The application of the proposed method to the phantom displacement confirmed its principle. The estimated 2D displacement of the phantom was consistent with the set displacement. Subsequently, the method was applied to two healthy subjects to measure the 2D displacement of the interventricular septum during one cardiac cycle. Consequently, during systole and diastole, the movement of the myocardium was measured, and the results were validated.

    Abstract Previously, we proposed an ultrasonic measurement method of arterial wall elasticity for the early detection of arteriosclerosis. Since vascular wall elasticity depends on blood pressure, in this study, the elasticity and viscosity were estimated using the hysteresis loop determined between the incremental strain in the wall and internal pressure by altering the internal pressure in the phantom and in vivo experiments. Consequently, both the estimated elasticity and viscosity increased with the internal pressure. Moreover, the slope of each hysteresis loop was larger than that of the approximated curve for the lowest blood pressures of the hysteresis loops with different internal pressures, as the blood pressure changed in the order of several hertz within a single heartbeat. Hence, we conclude that measuring both the blood pressure and the elastic and viscous moduli would be beneficial in comprehensively investigating more vessel wall properties that change with the progression of arteriosclerosis.

    Abstract The thickness of the carotid arterial wall obtained from B-mode imaging using ultrasonic diagnostic devices is widely used for the diagnosis of atherosclerosis. However, the measurement interval in the lateral direction of the B-mode image depended on the beam interval (>100 μm). Therefore, the B-mode image is discrete in the lateral direction and cannot reflect changes in local and minute thicknesses. A method for measuring the roughness of the luminal surface of the wall was proposed using the displacement of the carotid arterial wall in the lateral direction during one heartbeat. In this method, the lateral measurement interval is much shorter than the beam interval, enabling a smooth measurement of the luminal surface. By simultaneously applying the method to the lumen-intima and medial-adventitia boundaries of the wall, we propose a novel method to measure the local and minute thicknesses of the carotid arterial wall.

    In this study, the attenuation coefficient of blood was measured in vivo in the frequency range of 10–45 MHz. A procedure to correct the distribution of sound pressure in the measurements was discussed. Further, in vivo measurements were applied on the dorsal hand vein of four healthy subjects at rest and during avascularization. As a preliminary result, less variation of the measured attenuation coefficients was achieved by the proposed method. The comparable results of the inclination of the attenuation coefficients were obtained at rest and during avascularization. Furthermore, the attenuation coefficients during avascularization were markedly higher than those at rest, reflecting the degree of red blood cell aggregation promoted by avascularization. This method may aid in the non-invasive evaluation of blood properties reflecting the degree of red blood cell aggregation.

    The heart wall has a multilayered structure and moves rapidly during ejection and rapid filling periods. Local strain rate (SR) measurements of each myocardial layer can contribute to accurate and sensitive evaluations of myocardial function. However, ultrasound-based velocity estimators using a single-frequency phase difference cannot realize these measurements owing to insufficient maximum detectable velocity, which is limited by a quadrature frequency. We previously proposed a velocity estimator using multifrequency phase differences to improve the maximum detectable velocity. However, the improvement is affected by a spatial discrete Fourier transform (DFT) window length that represents the locality of the velocity estimation. In this article, we theoretically describe that shortening the window increases the interference between different frequency components and decreases the maximum detectable velocity. The tradeoff between the maximum detectable velocity and the window length was confirmed through simulations and a water-tank experiment. Under the tradeoff, the Hanning window, which was used in previous studies, is not always appropriate for the local measurement of the velocity, which sometimes exceeds 100 mm [Formula: see text] depending on the subject, direction of the ultrasound beam to the heart wall, and cardiac periods. In the in vivo measurement with the short window, the Tukey window with a large flat part that has a high-frequency resolution and ameliorates the discontinuity at both edges of the windowed signal was appropriate to measure the maximum velocity. This study offers the potential for local measurements of each myocardial layer using the multifrequency velocity estimator with the appropriate window function and window length.

    Measurement of the myocardial strain rate (SR), with high spatial resolution, is useful in evaluation of the transmurality of myocardial infarction. As the SR distribution is calculated using velocities observed at multiple positions in the heart wall, it is necessary to estimate the local velocity to measure SR distribution. In the present study, our previously proposed local velocity estimator, with multifrequency phase differences, was used to measure SR distribution in the heart wall. The SR distribution measured with the proposed local velocity estimator revealed alternate layers of contraction and relaxation, which were not measured with the conventional velocity estimator with spatial averaging. The reproducibility of the SR distributions was confirmed in three consecutive heartbeats with three subjects. High-spatial-resolution SR measurement with the proposed local velocity estimator will allow myocardial layer-specific analysis in the transmural direction.

    Local high-accuracy velocity estimation is important for the ultrasound-based evaluation of regional myocardial function. The ultrasound phase difference at the center frequency of the transmitted signal has been conventionally used for velocity estimation. In the conventional method, spatial averaging is necessary owing to the frequency-dependent attenuation and interference of backscattered waves. Here, we propose a method for suppressing these effects using multifrequency phase differences. The resulting improvement in velocity estimation in the heart wall was validated by in vivo experiments. In the conventional method, the velocity waveform exhibits spike-like changes. The velocity waveform estimated using the proposed method did not exhibit such changes. Because the proposed method estimates myocardium velocity without spatial averaging, it can be used for measuring heart wall dynamics involving thickness changes.

    PURPOSE: The generalized coherence factor (GCF), an adaptive beamforming technique, can reduce unnecessary signals from an unfocused position without reducing the contrast-to-noise ratio. However, the computational complexity of this method is large compared to the conventional delay-and-sum (DAS) beamformer. In the present paper, we propose a novel method to achieve the same reduction effect of unnecessary signals with a smaller computational load than that of the conventional GCF approach. METHODS: One of the factors increasing the computational complexity of the GCF-based beamformer compared with DAS is the generation of analytic signals at receiving elements. We clarified the mechanism of generating unnecessary signal components to enable the calculation of the GCF value directly from real signals without generating analytic signals. Furthermore, we proposed a method to filter out these components without generating analytic signals. RESULTS: The GCF values obtained using the proposed and conventional methods were compared and verified using the actual data acquired from a phantom with an ultrasound diagnostic system. We also compared the B-mode images. As a result, equivalent GCF values and similar B-mode image quality were achieved with the proposed method with reduced computational complexity. CONCLUSION: With the proposed method, generation of analytic signals at receiving elements can be omitted, and as a result, the computational load of the GCF method can be greatly reduced, while preserving the effect of reducing unnecessary signals like with the conventional method.

    PURPOSE: Medical ultrasound is often used to specify the puncture position during epidural anesthesia. However, visualization of the thoracic spine is difficult because of the complex structure, i.e., it is difficult to determine whether the thoracic spine or muscle is depicted. Therefore, this study aims to distinguish bone from muscle tissue using the differences in reflection and scattering characteristics of ultrasound. METHODS: We experimentally investigated the difference in signals received from bone and muscle. We proposed a new parameter utilizing the ratio of the amplitude of the received signals averaged in a wide range around the ideal delay line and that only along the ideal delay line, to emphasize the bone. RESULTS: First, we confirmed the difference in signals received from bone and muscle tissue by basic experiments. We also investigated the difference by in vitro experiments using chicken thigh and in vivo experiments in humans. In both experiments, the proposed method succeeded to clearly depict bone, suppressing the depiction of muscle, compared with conventional B-mode imaging. CONCLUSION: Using the difference in the characteristics of reflection from bone and scattering from muscle tissue, we could distinguish bone from muscle tissue with the proposed method.

    In epidural anesthesia, in which an anesthetic needle is inserted into the thoracic space from the skin surface, it is difficult to puncture the appropriate position because the space is narrow and the spinous process extends. In clinical practice, ultrasound is sometimes used to confirm the puncture position, but the visualization of the puncture position is unclear because it is not possible to distinguish between muscle that scatters ultrasound and bone that reflects ultrasound. In the previous research, the influence of the reflected wave from the object was superimposed and the spatial resolution decreased. Therefore, in this research, (1) the difference between scattering and reflection is identified from the amplitude angle characteristics obtained from the instantaneous value of the envelope amplitude of each element, and (2) using the difference in the amplitude angle characteristics of scattering and reflection, The thoracic interspace is sharpened by emphasizing bones.

    We have developed a method to exclude unnatural shapes at both ends of the surface shape measured for each beam by limiting the analysis range to the monotonically increasing part of the lateral displacement when measuring the surface shape for each beam. Furthermore, by interpolating the surface shape measured for each beam for each 1-micron lateral position, the error power is calculated with equal weight at all lateral positions of the surface shape measured for each beam, and the height is adjusted appropriately. We also developed a method. As a result, it became possible to obtain carotid artery surface roughness in vivo by reducing the beam-to-beam variation after adjusting the height of the surface shape measured for every single beam. These results were obtained by referring to Professor Wilhjelm's many years of research.

    In the early stage of ischemic heart disease, the ischemic region can avoid myocardial necrosis by rapidly reperfusing the coronary arteries. Therefore, rapid identification of the ischemic region is important, and local and highly accurate velocity estimation is the key to the evaluation of local myocardial contraction characteristics using ultrasound. In this study, we analyzed the received ultrasonic signal at multiple frequencies and proposed a speed estimation method using ultrasonic phase differences at multiple different frequencies. This proposed method can reduce the effects of attenuation and interference as described above without performing spatial averaging, and local velocity estimation is possible. The proposed method can estimate the velocity with a shorter window width than the conventional method, and can measure the local velocity distribution and the local myocardial strain rate.

    For a quantitative and early diagnosis of chronic liver disease, we examined a mechanical property measurement method and a tissue structure estimation method of liver tissue. In the mechanical property measurement, a local excitation method was constructed for the local measurement of viscoelasticity. In the estimation of liver tissue structure, the possibility of discrimination of the fibrotic liver tissue and fatty liver tissue based on the analysis of envelope statistics property was evaluated. This study demonstrated the possibility of the simultaneous evaluation of the mechanical property and the morphometric characteristics of liver tissue by ultrasound.

    平成29年度は,世界的にも先進的な振幅分布関数モデルとして提案されたマルチレイリーモデルを基本として,実際の組織構造が未知の臨床超音波画像に対して肝線維化を高精度に評価する手法を開発した。 マルチレイリーモデルを用いることで,与えられた肝超音波画像から正常組織,線維組織,低エコー組織などの各組織を個別かつ定量的に抽出・可視化できるが,マルチレイリーモデルにより各組織を正しく評価するためには実際の肝組織の構成要素数に合わせた成分数のマルチレイリーモデルを用いる必要がある。しかし,実際の肝組織の構成要素数は肝線維化の進行度や解析領域の位置に応じて変化するため,組織構造が未知の臨床超音波画像に対してマルチレイリーモデルを用いた肝線維化評価を正しく行うためには解析領域内の組織構成要素数をあらかじめ推定する必要がある。 解析領域内の組織の構成要素数を推定するため,マルチレイリーモデル推定の入力値であるエコー振幅統計量に着目した。エコー振幅統計量の分布領域が組織の構成要素数に応じて変化することに着目し,エコー振幅統計量に対する閾値処理で組織の構成要素数を推定する新たな手法を提案した。閾値の決定法としてマハラノビス距離を導入することで,組織の構成要素数を正しく判定する統計的な確率を制御することが可能になり,定量的に組織の構成要素数を推定できた。推定した組織構成要素数に応じた成分数のマルチレイリーモデルを用いることで,実際の組織構造が未知の臨床超音波画像に対して肝線維化を正しく定量的に評価することが可能になった。本成果は,臨床的に十分な精度をもつ定量診断システムを実現する上で重要な成果であり,その意義は学術集会での受賞や学術論文への採択により認められた。
