BACKGROUND: The traditional curriculum for medical students in Japan does not include sufficient opportunities for students to develop their skills for musculoskeletal (MSK) examination and clinical reasoning and diagnosis. Therefore, an effective programme is required to help medical students and residents improve their clinical skills in MSK. This paper aims to assess the clinical skills of medical students who have participated in a peer role-playing simulation programme using a mini clinical evaluation exercise (mini-CEX). METHODS: Participants were 90 female medical students who were completing their first orthopaedic clinical clerkship. They were divided into two groups. The simulation group participated in a role-play focussed on MSK cases as low-fidelity simulation, a structured debriefing with the course supervisor, and a self-reflection on Day 1 (n = 64). The control group did not participate in the role-play due to randomised clerkship schedules (n = 26). On Day 2 of the intervention, we observed and assessed all participants' performances during MSK outpatient encounters using the mini-CEX. We compared the mini-CEX score between the simulation group and the control group; the Wilcoxon rank-sum test was used for statistical analysis. RESULTS: The mini-CEX scores for physical examination, clinical reasoning and diagnosis, and overall clinical competency were significantly higher in the simulation group than in the control group (p < .05, physical examination: p = .014, clinical reasoning: p = .042, overall: p = .016). These findings suggest that medical students who partake in a peer role-playing simulation programme could experience improved clinical skills for physical examination, clinical reasoning and diagnosis, and overall clinical competency in real-life MSK outpatient encounters. CONCLUSIONS: Through a mini-CEX assessment, our findings indicate that medical students who participated in our peer role-playing simulation programme have improved clinical skills. Peer role-playing as a low-fidelity simulation and practical educational opportunity will enable educators to polish the competency of medical students in musculoskeletal physical examinations and clinical reasoning and diagnosis in a clinical setting.

[Aim] The aim of this study is constructing predictive models for vital and quality of life (QOL) prognosis and dementia risk in the elderly, and examining the prediction accuracy of these models. We also proposed a mobile Social Diagnosis Instrument (m-SDI), a tool that can be used practically in the medical, caregiving, and insurance fields, by utilizing the results of the prediction models to evaluate preventative care and support strategies for independent living of the elderly. [Method] We conducted a regression analysis based on the results of a survey on activities of daily living, QOLs (physical component summary; PCS and mental component summary; MCS), cognitive function, low nutrition risk, and actual living circumstances of the residents in a village in Nagano prefecture (2007, 2014). [Results] For men and women respectively: (1) vital prognosis: R²=0.324 and R²=0.302 (2) QOL (PCS): R²=0.325 and R²=0.090. [Summary] Predictive models require further improvements, including addition of other question items, to make it more practical. m-SDI uses a tablet device and allows responses to be input easily during a home visit, thereby appropriate lifestyle guidance can be provided to individuals while perusing their diagnostics results.

The purpose of this study is to propose a method of factor analysis for analyzing contingency tables developed from the data of unlimited multiple-choice questions. This method assumes that the element of each cell of the contingency table has a binominal distribution and a factor analysis model is applied to the logit of the selection probability. Scree plot and WAIC are used to decide the number of factors, and the standardized residual, the standardized difference between the sample, and the proportion ratio, is used to select items. The proposed method was applied to real product impression research data on advertised chips and energy drinks. Since the results of the analysis showed that this method could be used in conjunction with conventional factor analysis model, and extracted factors were fully interpretable, and suggests the usefulness of the proposed method in the study of psychology using unlimited multiple-choice questions.

This paper presents an approach leading to unique interpretations about reliability and validity in the confirmatory factor analysis model for dealing with multitrait-multimethod matrices. The correlated trait-correlated method (CT-CM) model does not necessarily identify the best solution, although it gives a unique interpretation for the data. On the other hand, the correlated trait-correlated method minus one (CT-C (M-1)) model (Eid, 2000) invariably identifies a solution, but the solution is not unique, depending on the arbitrary choice of which method factor is eliminated from the model. In this study, the constraint that the sum of the method factor scores equals zero is introduced into the CT-CM model, which then ensures a unique solution. These three models are applied to 12 actual correlation matrices, and a simulation study to verify the proposed model. The results indicate that the proposed model is more practical than the other conventional models.

The purpose of this study is to propose an exploratory positioning analysis method for increasing the number of targets and scales efficiently utilizing multi-sample analysis in structural equation modeling. We applied this method to semantic differential data that collected to investigate images of foods at izakaya, or Japanese-style pubs. From real data examples, it is shown that we could increase the number of targets and scales without imposing a burden on subjects and that this method enable us to integrate multiple maps plotted by different groups into one new map and to examine targets and scales on it.

本研究は，高大接続改革に並行する形で平成28（2016）年度から５年間実施された前研究課題［基盤研究（A）JP16H02051］の後継である。前研究課題では，進行中の改革に即応しながら方針転換までを見届け，政策にも一定の影響を及ぼした。本研究課題では，突然の新型コロナウイルス感染症（COVID-19）の蔓延の大学入試への影響に対して，前研究課題で採用した方法論と成果を継承するものである。具体的には「大学入試政策の検証」「COVID-19への対応」「多面的・総合的評価の追究」「評価測定技術の追求」という４つの柱を基に進める研究成果を，本科研費のために構築するウェブサイトの「研究報告」のページを駆使し，エビデンスを重視した対応策の提案を行うことを旨とする。令和２（2020）年１月頃から流行が始まったCOVID-19は令和３（2021）年度入試を直撃した。５年計画の本研究の初年度は五里霧中でコロナ対策が行われた令和３（2021）年度入試の検証から始まることとなった。令和３（2021）年度入試は高大接続改革の初年度と目されていた時期であり，コロナ対策と高大接続改革の方針転換は輻輳的に大学入試に対する影響を及ぼした。５月には対面とオンラインのハイブリッド形式で開催された第34回東北大学高等教育フォーラム「検証コロナ禍の下での大学入試」において本研究の研究分担者が基調講演者及び現状報告者として３名登壇し，令和３年度入試におけるコロナ対応について実践報告を行った。12月には大学入試センター理事長裁量経費と共催でオンラインによる「プレイバック座談会大学入試におけるコロナ対策 ――令和3年度入試の舞台裏――」を開催し，令和3年度入試の検証を行った。その他，大学入試に対するコロナ禍の影響に関する研究論文２編を含む計４編の論文を成果として輩出した。

本研究の目的は、高校教員の「主体的に学習に取り組む態度（主体性）」の評価を支援するために、ＣＢＴ（computer based testing）による高校生向けのメタ認知標準検査を開発することである。具体的には、「① 標準検査の開発」、「② 項目反応理論（item response theory：ＩＲＴ）に基づく項目プールの構築」、「③ ＣＢＴシステムの開発」の３つの課題を通して達成される。それぞれに対応した３つの班（標準検査開発班、統計分析班、システム開発班）を組織し、各課題に取り組んでいる。「①標準検査の開発」については、国内外のメタ認知に関する先行研究を収集・検討した。また、国内で定評のあるメタ認知に関する項目を用いて質問紙調査を実施し、信頼性と妥当性の高さ、及び課題を明らかにした。これらを踏まえて、新たな項目を作成するための枠組みと項目の仕様を策定した。「②ＩＲＴに基づく項目プールの構築」については、次年度の標準化のための予備調査で使用する重複テスト分冊法のデザインを策定した。また、予算面からＩＲＴに基づく検査の実施方法の見直しが余儀なくされた。本研究が目指すメタ認知のくり返しの測定が可能であることから、当初予定のアダプティブ方式（１問ごとに受検者の能力を推定しながら実施）からリニア方式（統計的品質のそろった検査問題セットをあらかじめ複数作成して実施）に変更することとした。「③ＣＢＴシステムの開発」については、国内外の先行研究を参考にしてＣＢＴシステムの仕様を策定し、それに基づきプロトタイプの開発を行った。以上に加えて、本研究の成果を常時かつ即時に公開し、また、調査の補助的ツールとして使用するために、ホームページを作成した。

高大接続改革の方針転換が起こり，大学入試センター試験に代わって大学入学共通テストが導入された令和３（2021）年度入試に新型コロナウイルス感染症（COVID-19）が襲った。大学は様々な特別対応に追われ，実施可能性（feasibility）の課題が例年にも増して大きな課題となる年となった。令和４（2022）年度入試は落ち着いた環境で穏やかな実施が望まれたが，実際にはCOVID-19特別対応の継続に加え，大学入学共通テストが津波や障害事件に見舞われた。ひいてはスマートフォンを用いた不正事件が発生するなど，大学入試の信頼性，公平性が問われる事態が頻発した。さらに，大学入学共通テストの予想外の難化が受験行動に大きな影響を与えた。大学入試学の基盤確立を目指す本研究の方向性にも大きな影響を与える出来事であった。研究環境自体が計画時期の予想を超えて大きく変化している。その中で，当初の方向性に則った成果としては東北大学大学入試研究シリーズ第４巻「大学入試の公平性・公正性」及び第５巻「大学入試を設計する」の上梓が挙げられる。「真正な評価と実施可能性の両立」という本研究の根本理念の深化を示す成果であったと言える。「堅実型選抜方法」については，「書類審査開発班」及び「面接試験開発班」の実績となる研究を個別大学の選抜方法の評価という形で実施した。「挑戦型選抜方法」に関わる成果としては，英語のスピーキングテストに関わる研究，項目反応理論（Item Response Theory）の理論的発展につながる研究を発表した。現地に赴いての国際調査が困難な中，昨年度に収集した資料に基づき，日本，中国，韓国における大学入試のCOVID-19対策に関わる研究を本研究の成果として公表した。当初の想定とはやや異なる経過をたどっているが，厳しい研究環境の下，一定の研究成果を輩出することができた。

「よき医師」になるには、学識を修得する能力と医師としての資質や適性が求められるが、医師の資質や適性については、その教育方法および測定方法が確立されていない。入学者選抜試験では学力評価だけでなく、医師への適性を判断することが必要となる。本研究は入学者選抜試験および入学後の学識の成績と資質特性を解析すること、「よき医師」となる資質特性の測定法を開発し検証することを目的としている。 2018年度は本学の入試の3区分（指定校推薦、一般公募推薦、一般）による入学後の学科試験成績、問題発見解決をグループ学修で行うテュートリアル科目成績、プロフェッショナリズム・コミュニケーション・キャリア学修である「『至誠と愛』の実践学修」科目の成績、英語の成績を比較した。入試区分により各科目の成績に特性があり、低学年での学科試験、テュートリアル、「至誠と愛」の実践学修の成績は、指定校推薦で入学した学生で高かった。学年が進むと入試区分による差は少なくなった。医師となる資質を測定する目的で、投影法による心理検査を入学者選抜試験に導入した。従来の面接試験による評価の分布や評価者特性、新規検査による評価の分布や評価者特性、および面接試験と新規検査との相関、同一受験者への評価の平均値や級内相関を比較し、両者の一致度・一貫性、信頼性を検討した。新規の検査の収束的妥当性が示されたが、両者は受験者の適性の異なる側面を測定している可能性が示唆された。さらに「よき医師」の評価として、卒業後の医師の評価法についてアメリカで調査を行った。日本の生涯教育制度と同様にアメリカでも医師個人が自己研鑽を続ける制度がある。しかし、社会・患者・雇用主など「よき医師」に求めるものはstakeholderにより異なり、明確な評価法は認めなかった。研究計画には無かったが、本学の卒前教育と医師としての活動との関係を卒業生に調査することが出来た。

The range of Pearson's correlation coefficient r is said to be [-1,1], but we found a serious bias that the absolute value cannot attain 1 when the number of categories of the two variables are different. So we did the following: We first thoroughly investigated the reality of the bias. The expected value of the sample correlation coefficient was robustly underestimated, and it was revealed that this bias decreased as the number of categories increased. We attempted to convey this fact to everyone who uses the correlation coefficient. We also investigated how the bias affects other statistical methods. Finally, in order to remove the bias, we proposed a new calculation method: the polychoric correlation coefficient using the EM algorithm. We also conducted a historical study of the rating scale.

The purpose of this research was to closely observe the currently ongoing reform of university admissions and to contribute to the individual universities interdisciplinarily. As is well known, the reform policy reached a change of direction at the end of 2019 just before its introduction, but this research saw the whole story and influenced policy making, which is more than expected. It can be summarized that there was a fruit. Studies related to individual academic ability tests at national universities, such as Miyamoto and Kuramoto (2017), were reported in the media from the beginning, and provided evidence that would be the decisive factor in abandoning the introduction of descriptive questions into the common test during the policy change period. Regarding the evaluation of proactive learning attitude, the results of interdisciplinary approaches such as comparative education and educational psychology were produced, which influenced individual universities’ admission system.

The purpose of this study is to assess the cognitive characteristics of children with spina bifida using WISC-IV intelligence tests etc. and analyze the relationship with the learning status of each subject to provide specific support strategies. Seven studies were conducted for that purpose. As a result, children with spina bifida had difficulties in visual perception and processing speed. It could be divided into four groups according to cognitive characteristics. In arithmetic, it was related to the poor performance of perceptual reasoning and the difficulty of processing figures and calculations. In the Japanese language, they were stumped in terms of vocabulary, and stumped in discussions, writing, understanding, and writing.

In this study, two methods of analysis in order to examine reliability, convergent validity and discriminant validity of measurement using multitrait-multirater data are proposed in the framework of confirmatory factor analysis model. The first method is to evaluate reliability and validity using weighted sum scores of ratings by multiple raters. The second method is to deal with the data including a number of raters within a same rating source. As a result of application example to actual data, the usefulness of these methods was suggested. In the future, practical use of these methods in various fields such as 360-degree feedback is expected.
