    OBJECTIVES: Positron emission tomography (PET) images are reconstructed using computed tomography (CT)-based attenuation correction, which may cause artefactual errors due to CT artefacts. This study aimed to investigate the effects of metallic dental prostheses and metal artefacts on the incidence of nodular overcorrection in PET/CT images of the oral cavity in patients with head and neck tumours. METHODS: We retrospectively reviewed the F-18 fluorodeoxyglucose PET/CT images of 27 and 20 patients using silicon photomultiplier (SiPM) and photomultiplier tube (PMT) PET, respectively. We evaluated the presence or absence of overcorrection of tumour-like nodular uptake, number of metallic dental prostheses, number of metallic prosthetic blocks, mean CT values, bright-band artefact area/ratio, and dark-band artefact area/ratio. RESULTS: There was no significant difference in the incidence of overcorrections between SiPM and PMT PET; however, overcorrections in the former were significantly smaller in size than those in the latter. Overcorrection in PMT PET was more likely to occur when the number of metallic dental prostheses, number of metallic dental prosthetic blocks, mean CT value, and bright-band artefact area/ratio were high. However, there was no significant difference in SiPM PET overcorrection between the positive and negative overcorrection groups for the above conditions. CONCLUSION: Overcorrection was associated with metallic dental prostheses and CT artefacts; however, the conditions may have scanner-specific characteristics. ADVANCES IN KNOWLEDGE: Small tumour-like overcorrections appear on SiPM PET, which should be noted because it is more difficult to notice overcorrections. It is important to analyse the images while checking the pre-attenuation correction PET.

    INTRODUCTION: A clinical significance of extra-cardiac uptake of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG-PET), particularly in the brain, is unclear in cardiac sarcoidosis (CS). METHODS AND RESULTS: We analysed data from 84 CS patients (mean age 58.3 years, 70.2 % woman) who underwent 18F-FDG-PET scans, including brain imaging, between May 2008 and December 2021, with follow-ups completed in July 2023. Brain 18F-FDG-PET images were processed using SPM12 software, with regional brain 18F-FDG uptake normalised to whole-brain uptake. Voxel-wise analysis identified the left insula, whose 18F-FDG uptake was lower in patients who experienced fatal ventricular arrhythmic events (FVAE) (N = 30) compared to those without FVAE (N = 54) (1.15 ± 0.06 vs. 1.20 ± 0.06, P < 0.001). Kaplan-Meier curve analysis showed that patients with lower left-insular 18F-FDG uptake (<1.18) had a higher FVAE incidence compared to those with higher uptake (≥1.18) (P = 0.003). Cox proportional hazard analysis demonstrated that the FVAE risk decreased with a hazard ratio of 0.538 (95 % confidence interval 0.359-0.808) for every 1 standard deviation increase in left-insular 18F-FDG uptake (P = 0.003). Combining both cardiac and brain 18F-FDG uptakes allowed a better discrimination of FVAE risk. CONCLUSIONS: Brain 18F-FDG-PET, particularly evaluating the left insular cortex involved in parasympathetic regulation, may help to assess the FVAE risk in CS patients.

    This phantom study aimed to optimize the noise-suppression parameter (β) and the edge-preservation parameter (γ) values in the block sequential regularized expectation maximization (BSREM) reconstruction method for 123I-2-β-carbomethoxy-3β-(4-iodophenyl)-N-(3-fluoropropyl)nortropane striatal SPECT using a ring-type cadmium-zinc-telluride camera. Methods: We used a striatum phantom filled with an 123I solution, with adjusted concentrations in the left and right caudate and left and right putamen. The phantom was scanned for 120 min using the StarGuide system, and 20-min SPECT data were extracted from the total acquisition. Using the BSREM method, we generated 42 reconstructed images by varying β and γ. We calculated the percent coefficient of variation and percent contrast for each image. Visual evaluations were conducted by a nuclear medicine physician and 2 technologists. Additionally, we calculated the striatal uptake ratio (SUR) and the ratio of the left caudate SUR to putamen SUR in the left striatum (left striatal ratio). Results: The percent coefficient of variation decreased as the β value increased, whereas percent contrast increased as the γ value increased. We conducted a visual evaluation of 15 reconstructed images selected by physical assessment. Images reconstructed with a β value of 0.03 with a γ value of 5 and those with a β value of 0.05 with a γ value of 10 were deemed suitable for diagnostic purposes because of their accurate depiction of tracer accumulation in each striatal region. SUR and left striatal ratio were comparable for these 2 image sets. Conclusion: Evaluation of the optimal conditions for striatal SPECT imaging with BSREM reconstruction revealed that the optimal β value and γ value combinations were a β value of 0.03 and γ value of 5 and a β value of 0.05 and γ value of 10.

    BACKGROUND: Patients with cardiac sarcoidosis (CS) are at an increased risk of fatal ventricular arrhythmic events (FVAE). However, the predictive value of 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) in assessing the risk of FVAE in patients with CS remains uncertain. METHODS: We included data from 121 patients with CS (39 men and 82 women; mean age: 59.5 years) who underwent FDG-PET imaging between March 2008 and November 2020, with follow-ups completed in July 2023. Of these, 82 patients had available cardiac magnetic resonance imaging data, including late gadolinium enhancement (LGE). FDG-PET images were analysed using a polar-map model to determine the regional mean percentage uptake relative to the maximal cardiac 18F-FDG uptake in each of the 17 segments defined by the American Heart Association. RESULTS: Patients experiencing FVAE after FDG-PET (n = 43) showed lower percent uptake in the basal inferoseptal segment compared to those who did not (n = 78) (41.8 ± 15.2 % vs. 54.4 ± 13.8 %, P < 0.001). Patients with a basal inferoseptal percent uptake below the median had a lower FVAE-free survival rate than those with a higher percent uptake (58.1 % vs. 78 % at 5 years, P = 0.007), which was consistent in patients with LGE in the same regions with reduced 18F-FDG uptake. A Cox hazard model indicated that the FVAE risk decreased with a hazard ratio of 0.862 (95 % CI 0.770-0.964) for every 5 % increase in basal inferoseptal percent uptake (P = 0.009). CONCLUSION: Reduced 18F-FDG uptake in the basal interventricular septum, including the inferoseptal segment, may be a valuable predictor of future FVAE in patients with CS.

    PURPOSE: This research aims to optimize the block sequential regularized expectation maximization (BSREM) reconstruction in bone single photon emission computed tomography (SPECT) images using a ring-type SPECT/computed tomography (CT) system in terms of image quality and accuracy of quantitative evaluation. METHODS: SPECT images were acquired using a ring-type SPECT/CT and from a SIM2 bone phantom containing a 99m-Tc solution. BSREM parameters were assessed with coefficient of variation (CV), recovery coefficient (RC), contrast noise ratio (CNR), standardized uptake value max (SUVmax), and its root mean square error (RMSESUVmax). Clinical images were reconstructed, and the reconstructions were evaluated as superior based on phantom results. RESULTS: Increasing γ led to higher RC and CV, while increasing β lowered these values. CNR showed no consistent trend. In terms of quantitative evaluation, γ value 5.0, β values 0.3-0.9, and γ value 10, βvalues 0.3-0.9 resulted in lower RMSESUVmax compared to Anger type. Based on the phantom experiment, γ value 5.0, β values 0.5-0.9, and γ value 10, β values 0.7-0.9 were evaluated as superior. Clinical images under these reconstructions, determined from the phantom experiments, with γ value 5.0, β value 0.9, showed the smoothest of normal bone and the clearest separation of neighboring tumors. CONCLUSION: As reconstruction parameters for BSREM reconstruction in bone SPECT on a ring-type semiconductor SPECT/CT system, a γ value of 5.0 and a β value of 0.9 were considered suitable for image quality and quantitative evaluation.

    BACKGROUND: Disproportionately enlarged subarachnoid space hydrocephalus (DESH) is one of the neuroradiological characteristics of idiopathic normal pressure hydrocephalus (iNPH), which makes statistical analyses of brain images difficult. This study aimed to develop and validate methods of accurate brain segmentation and spatial normalisation in patients with DESH by using the Computational Anatomy Toolbox (CAT12). METHODS: Two hundred ninety-eight iNPH patients with DESH and 25 healthy controls (HCs) who underwent cranial MRI were enrolled in this study. We selected the structural images of 169 patients to create customised tissue probability maps and diffeomorphic anatomical registration through exponentiated Lie algebra (DARTEL) templates for patients with DESH (DESH-TPM and DESH-Template). The structural images of 38 other patients were used to evaluate the validity of the DESH-TPM and DESH-Template. DESH-TPM and DESH-Template were created using the 114 well-segmented images after the segmentation processing of CAT12. In the validation study, we compared the accuracy of brain segmentation and spatial normalisation among three conditions: customised condition, applying DESH-TPM and DESH-Template to CAT12 and patient images; standard condition, applying the default setting of CAT12 to patient images; and reference condition, applying the default setting of CAT12 to HC images. RESULTS: In the validation study, we identified three error types during segmentation. (1) The proportions of misidentifying the dura and/or extradural structures as brain structures in the customised, standard, and reference conditions were 10.5%, 44.7%, and 13.6%, respectively; (2) the failure rates of white matter hypointensity (WMH) cancellation in the customised, standard, and reference conditions were 18.4%, 44.7%, and 0%, respectively; and (3) the proportions of cerebrospinal fluid (CSF)-image deficits in the customised, standard, and reference conditions were 97.4%, 84.2%, and 28%, respectively. The spatial normalisation accuracy of grey and white matter images in the customised condition was the highest among the three conditions, especially in terms of superior convexity. CONCLUSIONS: Applying the combination of the DESH-TPM and DESH-Template to CAT12 could improve the accuracy of grey and white matter segmentation and spatial normalisation in patients with DESH. However, this combination could not improve the CSF segmentation accuracy. Another approach is needed to overcome this challenge.

    OBJECTIVE: To investigate the prevalence and intensity of grasp reflexes and to examine changes in these reflexes after shunt surgery in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: We enrolled 147 patients with probable iNPH. A standard procedure was used to determine the presence of grasp reflexes, and the intensity of these reflexes was assessed using a four-category classification. Clinical rating scales and their correlation with grasp reflexes were also evaluated. Grasp reflexes were reassessed in 72 patients 1 year after surgery. RESULTS: We found that approximately 50.3% of patients with iNPH exhibited a positive grasp reflex. Among these patients, 69% exhibited bilateral positivity, while the remaining patients showed unilateral positivity. Furthermore, the intensity of the grasp reflex was significantly correlated with the severity of gait and with cognitive, urinary, motor, and behavioural symptoms. Surgical interventions led to a reduction (41.7%) or maintenance (30.6%) of the reflex intensity in 72.3% of iNPH patients. The changes in reflex intensity showed significant positive correlations with changes in the number of steps of the Timed Up and Go test and Trail Making Test-A scores but not with changes in total scores on the iNPH Grading Scale. CONCLUSION: This retrospective study identified grasp reflexes as a highly prevalent phenomenon in patients with iNPH. These reflexes can assist in evaluating the severity of various symptoms, including cognitive, gait, urinary, motor and emotional symptoms.

    INTRODUCTION: Brain positron emission tomography/computed tomography (PET/CT) scans are useful for identifying the cause of dementia by evaluating glucose metabolism in the brain with F-18-fluorodeoxyglucose or Aβ deposition with F-18-florbetaben. However, since imaging time ranges from 10 to 30 minutes, movements during the examination might result in image artifacts, which interfere with diagnosis. To solve this problem, data-driven brain motion correction (DDBMC) techniques are capable of performing motion corrected reconstruction using highly accurate motion estimates with high temporal resolution. In this study, we investigated the effectiveness of DDBMC techniques on PET/CT images using a Hoffman phantom, involving continuous rotational and tilting motion, each expanded up to approximately 20 degrees. MATERIALS AND METHODS: Listmode imaging was performed using a Hoffman phantom that reproduced rotational and tilting motions of the head. Brain motion correction processing was performed on the obtained data. Reconstructed images with and without brain motion correction processing were compared. Visual evaluations by a nuclear medicine specialist and quantitative parameters of images with correction and reference still images were compared. RESULTS: Normalized Mean Squared Error (NMSE) results demonstrated the effectiveness of DDBMC in compensating for rotational and tilting motions during PET imaging. In Cases 1 and 2 involving rotational motion, NMSE decreased from 0.15-0.2 to approximately 0.01 with DDBMC, indicating a substantial reduction in differences from the reference image across various brain regions. In the Structural Similarity Index (SSIM), DDBMC improved it to above 0.96 Contrast assessment revealed notable improvements with DDBMC. In continuous rotational motion, % contrast increased from 42.4% to 73.5%, In tilting motion, % contrast increased from 52.3% to 64.5%, eliminating significant differences from the static reference image. These findings underscore the efficacy of DDBMC in enhancing image contrast and minimizing motion induced variations across different motion scenarios. CONCLUSIONS: DDBMC processing can effectively compensate for continuous rotational and tilting motion of the head during PET, with motion angles of approximately 20 degrees. However, a significant limitation of this study is the exclusive validation of the proposed method using a Hoffman phantom; its applicability to the human brain has not been investigated. Further research involving human subjects is necessary to assess the generalizability and reliability of the presented motion correction technique in real clinical scenarios.

    OBJECTIVES: Arrested pneumatisation (AP) is an anatomic variant of the sphenoid sinus. Since AP remains underrecognised, otolaryngologists and radiologists may mistake AP for a lesion and perform follow-up imaging studies. We investigated the imaging findings of CT, MRI, and F-18 fludeoxyglucose (FDG)-positron emission tomography (PET) for AP, and discussed the differences between AP and other skull base lesions. METHODS: We reviewed multidetector low CT imaging of 442 patients (285 men and 157 women; age range, 19-93 years; mean age, 67.8 years) who underwent FDG-PET/CT for head and neck tumours between January 2019 and December 2019. The imaging findings of AP were reviewed on CT, MRI, FDG-PET/CT, and compared with those of fibrous dysplasia, chordoma, chondrosarcoma, multiple myeloma, and bone invasion of nasopharyngeal carcinoma. RESULTS: AP was identified in 22 patients (14 men and 8 women; age range, 24-93 years; mean age, 67.0 years) based on criteria from previous reports. AP manifested with well-circumscribed sclerotic margins on CT, without evidence of expansion. AP showed high-signal intensity on T1-/T2 weighted MRI. FDG-PET revealed non-significant uptake [maximum standardised uptake value (SUVmax): 0.85 (range, 0.4-1.27)] in AP. Contrastingly, skull base lesions showed expansion, poorly circumscribed boundaries without osteosclerotic margins, and moderate-to-severe FDG uptake (SUVmax: 1.8-8.4). CONCLUSIONS: The characteristic imaging findings of AP, namely non-expansile on CT and non-uptake on FDG-PET, may aid in its differentiation from other skull base lesions.

    PURPOSE: To present a novel clinical application of silicon-photomultiplier-based positron emission tomography (SiPM-based PET)/computed tomography (CT), detecting a type II endoleak 5 years after endovascular aneurysm repair (EVAR). TECHNIQUE: SiPM-based PET/CT scans with a standard whole-body protocol were performed for a 73-year-old man with a past medical history of abdominal aortic aneurysms treated with EVAR and currently under investigation of his duodenal papillary carcinoma. The PET/CT demonstrated 18F-fluorodeoxyglucose (FDG) accumulation outside the stent graft in the native sac of the aneurysm. The site of accumulation corresponded to that of the contrast enhancement depicted in the CT angiography taken 1 month earlier. Another CT scan performed 3 months later revealed enlargement of the aneurysm. CONCLUSION: SiPM-based PET/CT, with its superior sensitivity and spatial resolution over conventional PET/CT, can detect type II low-flow endoleaks. CLINICAL IMPACT: Abnormal intra-aneurysmal FDG activity incidentally detected on SiPM-based PET/CT is worthy of attention because it may be indicative of endoleaks. Additional imaging using different modalities should be considered so that the patient would not miss the additional treatment opportunity upon observing sac enlargement. For patients with contraindications for iodine CT contrast media, SiPM-based PET/CT would serve as a suitable alternative.

    Positron emission tomography (PET)/computed tomography (CT) has improved sensitivity and resolution using silicon photomultiplier as a photosensor. Previously, only a fixed setting was available for the shooting time of 1 bed, but now, the shooting time can be changed for each bed. Time can be shortened or extended depending on the target area. A few studies reported on image reconstruction conditions for head and neck cancer in whole-body PET/CT examinations. Thus, this study aimed to optimize the imaging conditions of the head and neck region during whole-body imaging. A cylindrical acrylic container with a 200 mm diameter was used to simulate the head and neck area using a PET/CT system equipped with a semiconductor detector. Spheres of 6-30 mm in diameter were enclosed in the 200 mm diameter cylindrical acrylic vessel. Radioactivity in 18F solution (Hot:BG ratio 4:1) was enclosed in a phantom following the Japanese Society of Nuclear Medicine (JSNM) guidelines. Background radioactivity concentration was 2.53 kBq/mL. List mode acquisition of 1,800 s was collected at 60-1,800 s with the field of view of 700 mm and 350 mm. The image was reconstructed by resizing the matrix to 128 × 128, 192 × 192, 256 × 256, and 384 × 384, respectively. The imaging time per bed in the head and neck should be at least 180 s, and the reconstruction conditions should be a field of view (FOV) of 350 mm, matrix sizes of ≥ 192, and a Bayesian penalized likelihood (BPL) reconstruction with a β-value of 200. This allows detection of > 70% of the 8-mm spheres in the images.

    A 37-year-old man with previous heart transplantation for dilated cardiomyopathy underwent screening for malignancy under posttransplantation immunosuppression. 18 F-FDG PET/CT revealed uptake in 2 peritoneal sites of the pericardium that corresponded to the insertion sites of a left ventricular assist device that was used before transplantation. Additional abnormal uptake in the right axillary artery, aortic arch, and left femoral artery corresponded to the insertion sites for arterial inflow during cardiopulmonary bypass. Knowledge that FDG accumulation may occur at the insertion sites of an extracorporeal-circulation device enables unnecessary tests to be avoided.

    OBJECTIVE: The higher sensitivity of the new-generation positron emission tomography/computed tomography (PET/CT) with silicon photomultiplier (SiPM) may increase false-positive rates in detecting metastatic lymph nodes (LNs). This study aimed to clarify the usefulness of the SiPM PET scanner in diagnosing LN metastasis of oral squamous cell carcinoma (SCC). METHODS: We retrospectively reviewed consecutive F-18 fluorodeoxyglucose PET/CT images of 39 SCC patients using SiPM PET and 31 SCC patients using non-SiPM PET. We measured the maximum standardized uptake value (SUVmax) of the LNs on PET images and maximum short-axis diameter on transverse CT images. RESULTS: The sensitivity and specificity of SiPM PET were 86.2% and 95.6%, respectively (cut-off SUVmax, 4.6). The area under the curve (AUC) of SiPM PET (0.977; 95% confidence interval [CI], 0.958-0.995) was significantly higher than that of non-SiPM PET (0.825; 95% CI 0.717-0.934) (P < 0.01). In a size-limited analysis of diameter, the AUC of SiPM PET (≥ 0.96 for all diameters) was significantly higher than that of non-SiPM PET (tended to decrease as the LN diameter decreased) for the diagnosis of LN metastasis by SUVmax. CONCLUSION: SiPM PET had higher diagnostic accuracy for LN metastasis of oral SCC than non-SiPM PET, even for small LN metastasis without increasing false-positives.

    The International Study of Comparative Health Effectiveness with Medical and Invasive Approaches (ISCHEMIA) trial has had a great impact on the management of chronic coronary artery disease (CAD). One of the implications of this trial is the importance of close patient follow-up. To improve patient prognosis, evaluation of the residual extent of ischemia after treatment may be important because several studies have shown a close relationship between residual ischemia and cardiac events. For this assessment, myocardial perfusion single-photon emission computed tomography (MPS) has been utilized and is almost the only modality. Among the participants in the ISCHEMIA trial, more than 10% were excluded due to the absence of obstructive CAD. The pathophysiology of ischemia without non-obstructive coronary artery disease (INOCA) is gaining recognition; however, diagnosis is difficult, except for the assessment of myocardial flow reserve (MFR). Myocardial perfusion positron emission tomography (PET) is the most common modality for noninvasive evaluation of MFR; however, its availability in Japan is limited. For a breakthrough in this situation, a novel gamma camera with a cadmium zinc telluride (CZT) semiconductor might be one of the solutions that enables the evaluation of MFR with a commercially available perfusion tracer, similar to PET. Another solution is a novel PET tracer with a longer half-life. Clinical trials with 18F labeled perfusion agents have been initiated in Japan, and in a few years, delivery of this perfusion tracer will result in more frequent and easier assessment of MFR.

    OBJECTIVES: Delirium is an important prognostic factor in postoperative patients undergoing cardiovascular surgery and intervention, including transcatheter aortic valve implantation (TAVI). However, delirium after transcatheter aortic valve implantation (DAT) is difficult to predict and its pathophysiology is still unclear. We aimed to investigate whether preoperative cerebral blood flow (CBF) is associated with DAT and, if so, whether CBF measurement is useful for predicting DAT. METHODS: We evaluated CBF in 50 consecutive patients before TAVI (84.7±4.5 yrs., 36 females) using 99mTc ethyl cysteinate dimer single-photon emission computed tomography. Preoperative CBF of the DAT group (N = 12) was compared with that of the non-DAT group (N = 38) using whole brain voxel-wise analysis with SPM12 and region of interest-based analysis with the easy-Z score imaging system. Multivariable logistic regression analysis with the presence of DAT was used to create its prediction model. RESULTS: The whole brain analysis showed that preoperative CBF in the insula was lower in the DAT than in the non-DAT group (P<0.05, family-wise error correction). Decrease extent ratio in the insula of the DAT group (17.6±11.5%) was also greater relative to that of the non-DAT group (7.0±11.3%) in the region of interest-based analysis (P = 0.007). A model that included preoperative CBF in the insula and conventional indicators (frailty index, short physical performance battery and mini-mental state examination) showed the best predictive power for DAT (AUC 0.882). CONCLUSIONS: These results suggest that preoperative CBF in the insula is associated with DAT and may be useful for its prediction.

    INTRODUCTION: Takotsubo syndrome (TTS) is triggered mostly by physical and/or emotional stress that is processed in stress-associated brain regions, including the amygdala. However, it remains unclear whether such stress-induced brain activity is associated with TTS onset. METHODS AND RESULTS: We acquired brain [18F]-2-fluoro-deoxy-d-glucose (18F-FDG) positron emission tomography in 4 TTS patients (44-82 yrs., 3 women) on days 2-4 (acute phase) and days 29-40 (recovery phase) after diagnosis of TTS was made by coronary angiography and left ventriculogram. The 18F-FDG uptake was measured globally and also in the pre-defined regions of interest of the bilateral amygdala on the common Montreal Neurological Institute space; all 18F-FDG images were normalized using automated image pre-processing. Amygdalar activity was calculated by dividing the 18F-FDG uptake of the amygdala by the global brain uptake. Left ventriculograms showed that apical ballooning was typical at diagnosis and was then relieved in the recovery phase. Amygdalar activity in the acute phase (0.872 ± 0.032) was higher than in the recovery phase (0.805 ± 0.037) (P = 0.013). CONCLUSIONS: We report here 4 cases of TTS showing higher amygdalar activity in the acute phase as compared with the recovery phase, suggesting that increased stress-induced neurobiological activity is associated with TTS onset.

    Silicon photomultiplier(SiPM)半導体検出器を搭載したPET/CT装置で、位相限局法における最適位相幅とその位相幅を使用する際に必要な収集時間に関してファントム実験を行った。集積体積は位相幅を狭く設定した画像ほど静止画像において測定した集積体積0.26mlに近似していき、位相幅30%以下の範囲においては大きな変動がみられなかった。有意差検定を行った結果から、集積体積とSUVmaxの両者が変化しなくなったのは位相幅20%以下の画像であった。最適位相幅の呼吸同期画像における視覚評価と物理評価に関して、描出能スコアは採用時間0.5、1、1.5、2、2.5、3分の画像においてそれぞれ1、1.7、2、2、2、2点となった、描出能スコアが1.5点を上回り10mm球が描出されたのは採用時間1分以上の画像であった。採用時間が1分の画像におけるN 10minは平均14.24%であり、10mm球が描出されるための画質の目標値とされているN 10min<5.6%は満たされていなかった。

    症例は75歳男性で、6年前よりうつ病で近医通院中であり、自殺企図歴があった。また入退院を繰り返しており、内服薬としてリフレックス服用中であった。4〜5年前より近時記憶障害を自覚していたが、ここ数ヵ月で増強しており、認知症を心配され精査希望で受診となった。I-123 MIBG心筋シンチでは心筋集積がび漫性に低下し、washout亢進が認められた。I-123 IMP脳血流SPECTでは、両側前頭葉の集積低下が認められ、軽度血流低下が疑われるが認知症に特異的なパターンは指摘できなかった。胸部単純写真にて胸骨切開術後のワイヤー、ステントグラフトが認められたため病歴を確認したところ、前年、急性大動脈解離にて他院で上行〜弓部置換術+オープンステントグラフト留置、大動脈弁形成術後であり、左反回神経麻痺も認められた。そのため、MIBG心筋集積の高度低下は上行・弓部大動脈置換術後に伴う心交感神経機能低下の可能性が高いと考えられた。

    胸部悪性腫瘍に対して、化学療法と放射線療法を併用する化学放射線療法(CRT)が広く行われており、長期生存が期待できるようになった。それに伴い、化学療法に用いられる薬剤による心臓疾患、放射線照射野内に心臓が含まれることによる心臓疾患が長期生存例で問題となっている。これらCRT関連心臓疾患について概説し、それらの画像所見と画像を用いた早期検出と予防の可能性について述べた。

    Lewy小体病は、その他パーキンソニズムを呈する神経変性疾患やアルツハイマー病などと区別されるが、その鑑別は難しい。鑑別に当っては画像診断の果たす役割は大きく、その中でも核医学分野の機能画像診断は有用である。123I-FP-CIT SPECTと123I-MIBG心筋シンチグラフィーを組み合わせることで、Lewy小体病の診断精度が上がるだけでなく、その類縁疾患などを診断する助けになる。その二つの検査を使った鑑別法を「4分割法」とし、理論と運用法を紹介した。

    Translational research is needed to apply the PET agents developed in basic research to clinical research, which involves establishing a system to produce high-quality PET agents for routine clinical use. In this study, we aimed to establish a reliable method to produce 18F-TAP-X PET agents for clinical use, which we developed for myocardial blood flow imaging, by using a globally available commercial radiolabeling synthesizer. We were able to purify 18F-TAP-X to high purity in this study using only solid-phase extraction column cartridges without HPLC purification. Then, we succeeded in producing an 18F-TAP-X solution available as an injection agent by FASTlab, leading the way to its clinical application.

    Preoperative imaging diagnosis of oral cancer requires accurate evaluation of the extension and lymph node metastasis, which affects treatment planning and outcomes. We investigated imaging conditions specific to the head and neck region using a PET with a silicon-semiconductor detector. We developed a new MR imaging method to improve the detection of focal defects that are suspicious for lymph node metastasis. Compared to conventional PET imaging, these imaging methods improved the detection of small primary tumors, the evaluation of lesion extension, and the diagnostic accuracy of small lymph node metastases. Furthermore, the use of PET quantitative values resulted in very high diagnostic accuracy. The comprehensive imaging method obtained from the results of this study enables imaging diagnosis with superior diagnostic accuracy compared to conventional methods.

    In this study, we improved the examination method of three-dimensional (3D) thoracic duct scintigraphy by the oral administration of the radioisotope-labelled BMIPP and conducted a basic study on healthy subjects. In all cases, the thoracic duct was clearly visible and no adverse events were observed. The results of this study demonstrated the necessity to start scanning at less than 100 minutes after the administration. Compared to the effective dose of 2.8mSv / 111MBq for the intravenous administration of BMIPP, the effective dose associated with thoracic duct scintigraphy was slightly higher at 4.7mSv. Since the exposure dose to the thyroid gland and intestinal tract was relatively high, it was considered that exposure could be reduced by using an iodine block before the test or a laxative after the test. The safety and exposure dose of 3D thoracic duct scintigraphy were confirmed, and the possibility of exposure reduction was clarified.

    The purpose of this study was to investigate the role of nuclear medicine imaging in the prediction of radiation-induced heart disease (RIHD). This study included 12 esophageal cancer patients who underwent SPECT/CT using I-123-labelled fatty acid analogue (BMIPP) at pre-chemoradiation therapy (CRT), pre-boost irradiation, 3-month post-CRT, and 1-year post-CRT. The correlations between radiation dose to the heart and the change of myocardial BMIPP uptake from pre-CRT (Δmyocardial BMIPP uptake) were assessed. As a result, at pre-boost and 3-month post-CRT, Δmyocardial BMIPP uptake was significantly correlated with radiation dose to LV and WH. However, there were no statistically significant correlations at 1-year post-CRT. The results of this study indicated that, in the long term after CRT, myocardial metabolism might affected by factors other than the radiation dose to the heart. Nuclear medicine imaging may provide additional information regarding RIHD.
