    The Cryptococcus neoformans species complex (CNSC), responsible for cryptococcosis, is controlled by Th1-type immunity, in which IFN-γ-activated macrophages form granulomas that contain infection. Nevertheless, CNSC can evade host immunity and, after the primary infection phase, can shift to a latent infection similar to tuberculosis. Reactivation is thought to occur when immune control fails, but the underlying mechanisms remain poorly understood. FTY720, a functional antagonist of sphingosine-1-phosphate receptors, is primarily used in the treatment of multiple sclerosis. However, there have been reports linking its use to instances of cryptococcosis in patients undergoing treatment. To explore this, we established a novel mouse model using CnT-II mice, which express CD4+ T cell receptors specific for chitin deacetylase 2 (Cda2), a major T cell antigen of CNSC. Following infection with encapsulated CNSC, mice developed persistent pulmonary fungal burdens (10²-10³ CFU) accompanied by granulomatous responses, modeling latent infection. Upon FTY720 administration, mice exhibited increased pulmonary fungal burdens, disrupted granuloma integrity, and reduced IFN-γ and IL-12 production. FTY720 also markedly decreased CD4+ effector memory (Tem) and effector T cells (Teff) in the lungs, with a particularly profound loss of IFN-γ-producing Tem cells. These findings indicate that our model successfully recapitulates latent cryptococcal infection and reactivation, and demonstrate that FTY720 promotes relapse by depleting protective IFN-γ-producing CD4+ Tem cells and impairing Th1-mediated immunity. Given the rising use of FTY720, our study highlights its potential risk in patients with subclinical cryptococcosis and underscores the need for preventive strategies to avert disease progression.

    Obesity is recognized as a chronic low-grade inflammation that contributes to metabolic disorders. Weight loss (WL) is well known to improve metabolic disorders. However, recent studies have shown that the changes in immune cells associated with obesity exhibit immunological memory even after WL. Obesity impairs skin barrier function and exacerbates inflammatory skin diseases, such as psoriasis. While skin immune cells maintain homeostasis and defense, the impact of obesity on these cells in intact skin is understudied, as research mainly focuses on skin with an inflammatory skin disease model in the context of obesity. The effect of WL on skin immunity is even less clear. Therefore, this study aimed to investigate these discrepancies. Mice were assigned to the lean group (regular diet), the obese group (high-fat diet for 18 weeks), and the WL group (high-fat diet for 9 weeks followed by regular diet for 9 weeks). Following 18 weeks of feeding, mouse skin was excised, and immune cell populations within the skin were analyzed using real-time PCR and flow cytometry. In addition, the effects of WL on psoriasis were examined using an imiquimod-induced psoriasis mouse model. We observed increased expression of RORγt, IL-17A, and CCL20 in intact skin in both the obese and WL groups. In the psoriasis model, disease severity was further exacerbated by WL. Moreover, whereas the Vγ4+Vγ5- γδ T cell population was increased in the lean group, the Vγ4-Vγ5- γδ T cell population remained elevated following WL, similar to levels observed in the obese group. These observations indicated that obesity may imprint a memorized immune response in the skin, which is not abrogated by WL. This study is the first to demonstrate this persistent effect on skin immunity. These findings imply that individuals who have experienced obesity may remain at increased risk for inflammatory skin conditions, even after WL.

    ABSTRACT Carbapenemase-producing Enterobacter cloacae complex (CPEC) represents an emerging threat in healthcare settings; however, its spread across local and regional hospitals remains elusive. Between 2014 and 2022, 211 non-duplicate E. cloacae complex isolates (ECC) with reduced carbapenem susceptibility based on automated susceptibility testing were collected from 58 hospitals in 14 prefectures across Japan. PCR detected the bla IMP-1 group in 92.9% of isolates. Among these, 80 representative CPEC isolates underwent whole-genome sequencing. Two dominant high-risk clonal groups, ST78 and ST133 (including single-locus variants), accounted for 62.5% of sequenced isolates. Notably, ST133—rarely reported in Japan—was the second most common type and detected widely, suggesting gradual and inconspicuous dissemination. ST78 exhibited substantial genomic diversity, frequent co-carriage of extended-spectrum beta-lactamase genes, and multiple plasmid replicons, while ST133 formed a more uniform lineage with limited resistance determinants. Most isolates remained highly susceptible to amikacin and cefiderocol. Core-genome multi-locus sequence typing suggested possible intra-facility persistence and inter-facility transmission of closely related strains (≤10 allele differences) in some hospitals over time. Among the 46 facilities with bla IMP-1 group-positive isolates, 73.9% (34/46) were small hospitals (<200 beds). The proportion among screened ECC isolates was similarly high in small and large hospitals (90.5% vs 94.8%), with no significant difference, suggesting the need to extend molecular surveillance even in resource-limited settings. These findings highlight the utility of molecular confirmation and targeted genomic analysis, particularly in regions or settings where high-risk CPEC clones may circulate undetected, to support timely detection and containment efforts.

    Cycle threshold (Ct) values obtained from real-time reverse-transcription polymerase chain reaction (RT-PCR) tests are commonly used to determine COVID-19 isolation discharge from hospitals. However, not all patients with low Ct values are infectious, and studies evaluating infectivity through virus cultures in Japan remain limited. This study assessed patients with Ct values <30 at 10 days after symptom onset and compared the clinical characteristics of seven patients with positive virus culture results and 11 patients with negative virus culture results to validate the Centers for Disease Control and Prevention (CDC) isolation criteria in Japan. Among non-immunosuppressed patients with mild-to-moderate COVID-19, 80 % were virus culture-negative 10 days after symptom onset, even when the Ct values were <30. In contrast, only 33 % of patients with severe-to-critical symptoms tested negative on virus culture. All immunosuppressed patients with severe-to-critical symptoms consistently tested positive in virus cultures. Notably, three patients with hematological disorders remained virus culture-positive 20 days after symptom onset. These findings generally align with the CDC criteria. For non-immunosuppressed patients with mild-to-moderate COVID-19, isolation release after 10 days is supported without the need for additional RT-PCR testing. However, for non-immunosuppressed patients with severe-to-critical symptoms and immunocompromised patients, particularly those with hematological disorders, isolation may be safely discontinued when Ct values exceed 30 after 10 days. This study provides valuable insights into identifying individuals suitable for isolation release based on SARS-CoV-2 testing in Japan.

    The use of extracorporeal membrane oxygenation (ECMO) increased during the COVID-19 pandemic. However, complications associated with the long-term use of ECMO are poorly understood. This case report describes the autopsy findings of a perihepatic abscess in a patient with long-term COVID-19, which could not be diagnosed before death. In cases where the source of infection remains elusive but uncontrolled infections occur, we recommend the combined use of ultrasonography and contrast-enhanced computed tomography (CT) in patients with COVID-19 undergoing prolonged ECMO support, with particular consideration given to the potential development of cholecystitis.

    Staphylococcus aureus bacteremia results in substantial mortality. Rapid identification and the determination of methicillin susceptibility are crucial for immediate treatment with appropriate antibiotics. In the present study, we aimed to evaluate the basic assay performance of GeneSoC®, a novel rapid quantitative polymerase chain reaction (qPCR) method, for the detection of methicillin-susceptible (MS) or -resistant (MR) S. aureus in blood culture (BC) bottles. qPCR pimers and probes were desinged for femA and mecA genes to diagnose S. aureus and its methicilline-resistance status. GeneSoC® system can detect target genes within 12 min per sample using microfludic thermal cycling. A total of 100 BC-positive samples, showing clusters of gram-positive cocci using microscopy, were tested. The analytical sensitivity was demonstrated for the target sequence of femA and mecA genes at 10 copies/μL, respectively. The detection limit of the MRSA bacterial burden using this system was 104 and 103 CFU/mL for femA and mecA, respectively. Compared with culture-based identification and susceptibility testing, the sensitivity and specificity for the detection of femA (+)/mecA (+) MRSA using GeneSoC® were 90.9 and 98.9%, respectively, whereas the sensitivity and specificity for detection of femA (+)/mecA (-) MSSA were 96.2% and 97.3%, respectively. In conclusion, although this was a small sample and pilot study, the GeneSoC® system is beneficial for rapid, reliable, and highly sensitive real-time testing of MRSA and MSSA in BC bottles.

    Coronavirus disease 2019 (COVID-19) is associated with coagulopathy. However, the underlying mechanisms are not completely understood. We evaluated the association between COVID-19 coagulopathy and extracellular vesicle (EV) levels. We hypothesized that several EV levels would be higher in COVID-19 coagulopathy patients than in non-coagulopathy patients. This prospective observational study was conducted in four tertiary care faculties in Japan. We enrolled 99 COVID-19 patients (48 with coagulopathy and 51 without coagulopathy) aged ≥20 years who required hospitalization, and 10 healthy volunteers; we divided the patients into coagulopathy and non-coagulopathy groups according to the D-dimer levels (≥1 μg/mL and <1 μg/mL, respectively). We used flow cytometry to measure the tissue-factor-bearing, endothelium-derived, platelet-derived, monocyte-derived, and neutrophil-derived EV levels in platelet-free plasma. The EV levels were compared between the two COVID-19 groups as well as among the coagulopathy patients, non-coagulopathy patients, and healthy volunteers. No significant difference was found in EV levels between the two groups. Meanwhile, the cluster of differentiation (CD) 41 + EV levels were significantly higher in COVID-19 coagulopathy patients than in healthy volunteers (549.90 [255.05-984.65] vs. 184.3 [150.1-254.1] counts/µL, p = 0.011). Therefore, CD41+ EVs might play an essential role in COVID-19 coagulopathy development.

    Tuberculosis remains a major public health concern. Millions of tuberculosis cases and associated deaths have been reported worldwide. The Indo-Oceanic lineage Mycobacterium tuberculosis is common in Southeast Asia and causes extrapulmonary lesions. Only a few case studies on this lineage with genetic analysis using whole-genome sequencing have been reported in the literature. We present a case of disseminated tuberculosis, characterized by a variety of extrapulmonary lesions and paradoxical reactions, caused by the Indo-Oceanic lineage M. tuberculosis in a woman in Myanmar. A 22-year-old Burmese woman had arthritis in the right knee, with unknown aetiology, and was referred to our hospital. Computed tomography of the trunk revealed multiple nodular shadows in both lungs; swollen mediastinal lymph nodes; and small, low-density areas in the spleen. M. tuberculosis was detected in the sputum sample, joint aspirate, subcutaneous tumor, and exudate. She experienced a variety of paradoxical reactions together with aggressive tuberculosis dissemination in all areas of the body. Whole-genome sequencing of the DNA of MTB obtained from sputum and the right cervical subcutaneous abscess confirmed the Indo-Oceanic lineage of M. tuberculosis, the predominant strain in Myanmar. The Indo-Oceanic lineage M. tuberculosis causes disseminated tuberculosis all over the body including the periungual region. When patients show unusual symptoms, physicians should consider the introduction of new strains from foreign countries. Genetic analyses of the strains are recommended to define and confirm the lineages.

    Patients with severe Coronavirus disease 2019 (COVID-19) are at high risk for secondary infection with multidrug-resistant organisms (MDROs). Secondary infections contribute to a more severe clinical course and longer intensive care unit (ICU) stays in patients with COVID-19. A man in his 60s was admitted to the ICU at a university hospital for severe COVID-19 pneumonia requiring mechanical ventilation. His respiratory condition worsened further due to persistent bacteremia caused by imipenem-non-susceptible Klebsiella aerogenes and he required VV-ECMO. Subsequently, he developed a catheter-related bloodstream infection (CRBSI) due to Candida albicans, ventilator-associated pneumonia (VAP) due to multidrug-resistant Pseudomonas aeruginosa (MDRP), and a perianal abscess due to carbapenem-resistant K. aerogenes despite infection control procedures that maximized contact precautions and the absence of MDRO contamination in the patient's room environment. He was decannulated from VV-ECMO after a total of 72 days of ECMO support, and was eventually weaned off ventilator support and discharged from the ICU on day 138. This case highlights the challenges of preventing, diagnosing, and treating multidrug-resistant organisms and healthcare-associated infections (HAIs) in the critical care management of severe COVID-19. In addition to the stringent implementation of infection prevention measures, a high index of suspicion and a careful evaluation of HAIs are required in such patients.

    Macrolide-based combination chemotherapy is recommended for the treatment of Mycobacterium avium complex (MAC) pulmonary disease (MPD). The susceptibility of the MAC to macrolide antibiotics (MAs) determines the efficacy of treatment and clinical course of MPD. However, MAs cause several adverse effects, resulting in the discontinuation of macrolide-based combination chemotherapy. We encountered two women aged 65 years and 66 years diagnosed with MPD based on bronchoscopic examinations. They were initially treated with clarithromycin-based combination chemotherapy. However, neither patient could continue with chemotherapy owing to adverse events such as rash and edema. We switched clarithromycin with azithromycin, and the patients were able to continue chemotherapy without adverse events. Both patients completed their treatment successfully. Azithromycin, which also belongs to the class of MAs, can be a promising therapeutic option for MPD in case of clarithromycin intolerance.

    Acute respiratory distress syndrome (ARDS) is the leading cause of mortality in hospitalized patients with coronavirus disease 2019 (COVID-19) because of limited effective therapies. During infection, the accumulation and activation of macrophages and monocytes in the lungs induce inflammatory mediators and contribute to tissue injury, leading to ARDS. However, therapeutic strategies that directly target activated macrophage and monocytes have not been reported. Combination treatment with etoposide (a cytotoxic agent) and a corticosteroid has been widely used for treating hemophagocytic lymphohistiocytosis characterized by the systemic activation of macrophages with overwhelming inflammation. Herein, we present five cases of COVID-19-associated ARDS treated with etoposide and corticosteroids. Three of the five patients were over 65 years of age and had various underlying diseases, including multiple myeloma. Four patients required invasive mechanical ventilation (MV), and one patient refused to be placed on MV due to underlying diseases. All patients were pre-treated with antiviral and/or other anti-inflammatory agents, but their condition deteriorated and hyperinflammation was noted. All five patients responded well to treatment and had an immediate response, as reflected by improvement in their respiratory condition and inflammatory marker levels and rapid resolution of fever after etoposide administration; however, some patients required a second dose of etoposide and longer course of steroids. All patients recovered, and there were no severe adverse events related to the drugs. Following successful treatment in these five patients, we plan to conduct a clinical trial to evaluate the efficacy and safety of combination therapy with etoposide and corticosteroid for treating COVID-19 patients in Japan.

    2006〜2019年の宮城県内の三次医療施設4施設における侵襲性リステリア症症例について後ろ向きに調査した。網羅的病原遺伝子解析の結果、検出された全18株のListeria monocytogenes中、血清型IVbが10株(55%)と最も多く、その内訳はIVb-CC6型が4株(22%)、IVb-CC1およびIVb-CC2がそれぞれ3株(17%)であった。病原遺伝子の分布はそれぞれの遺伝子型毎に特徴を認めたが、遺伝子型と疾患とに関係性は見られなかった。分子系統解析では、2007年に菌血症患者から検出された株と、2012年に感染性大動脈瘤(IAA)患者から検出された株、および2015年にIAA患者から検出された株と2017年に髄膜炎患者から検出された株はそれぞれ同じCT型(IVb-CT1およびIVb-CT6)に属し、それぞれ異なる遺伝子が10個以下と相同性が非常に高かった。NCBIデータベース上の株と、宮城県で得られた株計277株の中で、遺伝子型IVb-CC1・2・6型と、それ以外の遺伝子型の株との病原遺伝子の分布を比較した。その結果、IVb-CC1・2・6型株は細胞壁に存在し宿主細胞への接着に関与するタイコ酸合成遺伝子gltA・gltBおよびLIPI-3遺伝子や、宿主細胞への侵入に関わるaut_IVbやvipの各病原遺伝子を他の遺伝子型株に比べ有意に多く保有していた。分子系統解析では、2013年宮城県・胎内感染症例由来株と2003年ブラジル・ポークソーセージ由来株、1931年アメリカ合衆国・由来不明株と1961年イギリス・鶏由来株、アメリカ合衆国・採取年・由来不明株と2005年ロシア・Myodes rufocanus由来株の遺伝的相同性がそれぞれ非常に高かった。

Strains genetically related to those from other countries caused various types of listeriosis, including vascular listeriosis in immunocompetent elderly people.     We report a coronavirus disease 2019 (COVID-19) case with rheumatoid arthritis taking iguratimod. The patient who continued iguratimod therapy without dose reduction was treated with ciclesonide had an uneventful clinical course, but prolonged detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was observed after resolution of symptoms. The effects of disease-modifying antirheumatic drugs (DMARDs) and ciclesonide on clinical course and viral shedding remain unknown and warrant further investigation.

    Introduction.Bacillus cereus harbouring Ba813, a specific chromosomal marker of Bacillus anthtacis, is found in patients with severe manifestations and causes nosocomial outbreaks.Aim. We assessed the genetic characteristics and virulence of Ba813(+) B. cereus in a hospital setting.Methodology. Three neutropenic patients with haematological malignancy developed B. cereus bacteraemia within a short period. Fifteen B. cereus were isolated from different sites in a haematology ward. A total of 18 isolates were evaluated for Ba813- and B. anthracis-related virulence, food poisoning-related virulence, genetic diversity, bacteria motility and biofilm formation.Results.Ba813(+) B. cereus was detected in 33 % (1/3) of patients and 66 % (9/15) of the hospital environment. The 18 strains were divided into 2 major clusters (clade 1 and clade 2), and 14 strains were classified into clade 1. All Ba813(+) strains, including four sequence types, were classified into clade 1/the cereus III lineage, which is most closely related to the anthracis lineage. Two strains belonging to clade 1/non-cereus III carried the B. anthracis-associated cap gene, but not Ba813. B. cereus, including Ba813(+) strains, had significantly lower prevalence of enterotoxin genes than clade 2 strains. In clade 1, B. cereus, Ba813(+) strains showed significantly higher swimming motility and biofilm formation ability than Ba813(-) strains.Conclusion.Ba813(+) B. cereus, which are genetically closely related to B. anthracis, were abundant in a haematological ward. Ba813(+) B. cereus with high motility and biofilm formation abilities may spread easily in hospital environments, and could become a hospital-acquired infection.

    Bacillus cereus commonly causes catheter-related bloodstream infections (BSIs) in hospital settings, and occasionally occurs fatal central nervous system (CNS) complications. B. cereus harboring Ba813, a specific chromosomal marker of Bacillus anthracis, has been found in patients with severe infection and nosocomial BSI. However, the bacteriological profile and clinical feature of Ba813 (+) B. cereus are unclear. Fifty-three patients with B. cereus BSI were examined. Isolates were evaluated for Ba813, B. anthracis-related and food poisoning-related virulence, multilocus sequencing typing, and biofilm formation. Patients' clinical records were reviewed retrospectively. The 53 isolates were comprised of 29 different sequence types in two distinct clades. Seventeen of the 53 (32%) B. cereus isolates including five sequence types possessed Ba813 and were classified into Clade-1/Cereus-III lineage which is most closely related to Anthracis lineage. No B. cereus possessed B. anthracis-related virulence genes. Ba813 (+) strains showed a lower prevalence of enterotoxin genes than Clade-2 strains (n = 4), but no difference from Clade-1. Ba813 (+) strains showed significantly lower biofilm formation than Clade-1/non-Cereus-III (n = 22) and Clade-2 strains, respectively. Compared to Clade-1/non-Cereus-III and Clade-2 B. cereus, Ba813 (+) strains were isolated more frequently from elderly patients, patients with indwelling central venous catheter rather than peripheral venous catheter, and patients who remained in the hospital for longer before BSI onset. No significant differences in disease severity or mortality were observed. Though two of the ten Ba813 (-) strains in Clade-1/Cereus III were isolated from the patients with CNS complication, no significant difference was observed in the bacterial profile and clinical characteristics among Clade-1/Cereus III strains. In conclusion, our report suggested that Ba813-harboring B. cereus strains, genetically closely related to B. anthracis, were abundant among B. cereus strains in the hospital setting, and might cause catheter-related nosocomial BSI. However, it did not affect the clinical outcomes.

    The gut microbiota may play a pivotal role in controlling the antimicrobial resistant (AMR) organisms although the evidences are limited. We investigated the effects of gut microbiota on the growth of AMR organisms, β-lactamases activity and transmissibility of antimicrobial resistant properties of the extended spectrum β-lactamase (ESBL)-producing Escherichia coli and carbapenem-resistant Enterobacteriaceae. CTX-M-15-positive, ESBL-producing E. coli and carbapenem resistant Enterobacteriaceae, Bacteroides fragilis, Bifidobacterium longum, Clostridium butyricum, Clostridioides difficile, Clostridium perfringens, Enterococcus faecium, Lactobacillus plantarum and probiotic strain of C. butyricum MIYAIRI 588 were used in this study. The growth of AMR organisms was suppressed by the supernatant of C. butyricum, C. difficile, C. perfringens, E. faecium and L. plantarum in a dose dependent manner but not by that of B. fragilis and B. longum. The β-lactamase activity produced by E. coli was reduced by the presence of culture supernatant of certain gut microbiota during stationary phase of E. coli. Importantly, C. butyricum MIYAIRI 588 culture supernatant suppressed the transcription of blaCTX-M gene during growth phase of E. coli. The conjugation assay showed the reduction of transmissibility of antibiotic resistant gene by gut microbiota. These findings suggest that certain gut microbiota affect the antibiotic resistant activities of AMR organisms. Further studies are needed to identify the specific mechanism(s) of these actions between AMR organisms and gut microbiota.

    Excessive inflammation reactions with a cytokine storm in the lungs have historically been thought as the primary cause of fatal acute respiratory distress syndrome (ARDS). However, interruption of inflammatory cytokine activation failed to attenuate ARDS, suggesting that other therapies are required to treat this illness and improve survival. Etoposide (ET), a cytotoxic agent, and prednisolone (PSL), a corticosteroid with strong anti-inflammatory activity, have been used to treat other disease involving similar cytokine-activated macrophages and hemophagocytic activity. However, they have not been previously tested as ARDS therapeutics alone or in combination. In the present study, we used a fatal ARDS mouse model induced via administration of α-galactosylceramide and lipopolysaccharide, which resulted in the development of severe lung injury with hypercytokinemia and hemophagocytosis, all of which were observed in ARDS patients infected with highly pathogenic respiratory viruses. The ET and PSL combination therapy, but not ET or PSL alone, reduced the recruitment and activation of inflammatory cells including macrophages, natural killer T cells, and neutrophils, and significantly improved the survival rate in this model. Furthermore, whereas ET alone improved lung edema, it did not increase the survival rate, indicating the necessity of PSL in the treatment of ARDS. Surprisingly, combination therapy did not reduce the production of cytokines and chemokines in the lungs, demonstrating that inflammatory cells, rather than hypercytokinemia, are the direct target of these compounds and primary cause of ARDS-related death. Thus, combination therapy with ET and PSL that targets inflammatory cells has the potential to attenuate fatal ARDS.

    OBJECTIVES: The aim of this study was to determine the mutant selection window (MSW) of various disinfectants against Staphylococcus aureus and Pseudomonas aeruginosa clinical isolates to determine the tendency of these strains to acquire resistance to disinfectants. METHODS: A total of 60 S. aureus isolates [30 methicillin-resistant S. aureus (MRSA) and 30 methicillin-susceptible S. aureus (MSSA)] and 30 P. aeruginosa, including 2 multidrug-resistant P. aeruginosa (MDRP), were collected in Japan. The minimum inhibitory concentrations (MICs) and mutant prevention concentrations (MPCs) of disinfectants, including sodium hypochlorite (NaOCl), against these strains were established to determine the MSW. RESULTS: The MSW50, MSW80 and MSW100 for sodium hypochlorite against S. aureus and P. aeruginosa were 4×, 8× and 16× MIC, respectively. Strains surviving in the sodium hypochlorite MSW remained at a concentration of ≤0.3% (≤3072μg/mL). CONCLUSIONS: This is the first evaluation of the bactericidal activity against S. aureus and P. aeruginosa strains surviving in the MSW of disinfectants. Environmental disinfection at low concentrations of sodium hypochlorite does not kill micro-organisms. Proper use of sodium hypochlorite shows a bactericidal effect against various pathogenic micro-organisms and is inexpensive, making it frequently used globally.

    Shiga toxin-producing Escherichia coli (STEC) can cause severe gastrointestinal disease and colonization among food handlers. In Japan, STEC infection is a notifiable disease, and food handlers are required to undergo routine stool examination for STEC. However, the molecular epidemiology of STEC is not entirely known. We investigated the genomic characteristics of STEC from patients and asymptomatic food handlers in Miyagi Prefecture, Japan. Whole-genome sequencing (WGS) was performed on 65 STEC isolates obtained from 38 patients and 27 food handlers by public health surveillance in Miyagi Prefecture between April 2016 and March 2017. Isolates of O157:H7 ST11 and O26:H11 ST21 were predominant (n = 19, 29%, respectively). Non-O157 isolates accounted for 69% (n = 45) of all isolates. Among 48 isolates with serotypes found in the patients (serotype O157:H7 and 5 non-O157 serotypes, O26:H11, O103:H2, O103:H8, O121:H19 and O145:H28), adhesion genes eae, tir, and espB, and type III secretion system genes espA, espJ, nleA, nleB, and nleC were detected in 41 to 47 isolates (85-98%), whereas isolates with other serotypes found only in food handlers were negative for all of these genes. Non-O157 isolates were especially prevalent among patients younger than 5 years old. Shiga-toxin gene stx1a, adhesion gene efa1, secretion system genes espF and cif, and fimbrial gene lpfA were significantly more frequent among non-O157 isolates from patients than among O157 isolates from patients. The most prevalent resistance genes among our STEC isolates were aminoglycoside resistance genes, followed by sulfamethoxazole/trimethoprim resistance genes. WGS revealed that 20 isolates were divided into 9 indistinguishable core genomes (<5 SNPs), demonstrating clonal expansion of these STEC strains in our region, including an O26:H11 strain with stx1a+stx2a. Non-O157 STEC with multiple virulence genes were prevalent among both patients and food handlers in our region of Japan, highlighting the importance of monitoring the genomic characteristics of STEC.

    Legionella pneumophila causes life-threatening pneumonia culminating in acute lung injury. Innate and adaptive cytokines play an important role in host defense against L. pneumophila infection. Interleukin-36 (IL-36) cytokines are recently described members of the larger IL-1 cytokine family known to exert potent inflammatory effects. In this study, we elucidated the role for IL-36 cytokines in experimental pneumonia caused by L. pneumophila Intratracheal (i.t.) administration of L. pneumophila induced the upregulation of both IL-36α and IL-36γ mRNA and protein production in the lung. Compared to the findings for L. pneumophila-infected wild-type (WT) mice, the i.t. administration of L. pneumophila to IL-36 receptor-deficient (IL-36R-/-) mice resulted in increased mortality, a delay in lung bacterial clearance, increased L. pneumophila dissemination to extrapulmonary organs, and impaired glucose homeostasis. Impaired lung bacterial clearance in IL-36R-/- mice was associated with a significantly reduced accumulation of inflammatory cells and the decreased production of proinflammatory cytokines and chemokines. Ex vivo, reduced expression of costimulatory molecules and impaired M1 polarization were observed in alveolar macrophages isolated from infected IL-36R-/- mice compared to macrophages from WT mice. While L. pneumophila-induced mortality in IL-36α- or IL-36γ-deficient mice was not different from that in WT animals, antibody-mediated neutralization of IL-36γ in IL-36α-/- mice resulted in mortality similar to that observed in IL-36R-/- mice, indicating redundant and overlapping roles for these cytokines in experimental murine L. pneumophila pneumonia.

    Introduction. This is the first case report of septic abortion due to β-lactamase-negative ampicillin-resistant (BLNAR) non-typeable Haemophilus influenzae infection. In Japan, BLNAR H. influenzae is widespread and has become a clinical concern, especially in paediatrics and otolaryngology, but H. influenzae has not been previously recognized as a causative agent of obstetric or gynaecological infection. Case presentation. A 31-year-old pregnant woman presented at 17 weeks and 6 days of gestation with a high fever; she was admitted with a diagnosis of threatened premature delivery. Despite tocolytic treatment, she aborted spontaneously 2 h after admission and then entered septic shock. BLNAR H. influenzae was detected in both blood and vaginal cultures. Her condition gradually improved after several days of treatment with cefotaxime, and she was ultimately discharged without sequelae or complaints. Conclusion. Although penicillin with a β-lactamase inhibitor is currently recommended for the treatment of septic abortion, this combination will probably lead to treatment failure in the case of BLNAR H. influenzae infection. As this study reveals, H. influenzae can cause septic abortion; hence, future efforts should be undertaken to detect and therapeutically target this pathogen during pregnancy.

    BACKGROUND:<br /> Over-expression of alpha-phenol-soluble modulins (PSMs) result in high virulence of community-associated methicillin-resistant Staphylococcus aureus (MRSA). The psm-mec gene, located in the mobile genetic element SCCmec-II, suppresses PSMαs production.<br /> METHODS:<br /> Fifty-two patients with MRSA bacteremia were enrolled. MRSA isolates were evaluated with regard to the psm-mec gene sequence, bacterial virulence, and the minimum inhibitory concentration (MIC) of vancomycin and teicoplanin. Fifty-one MRSA isolates were classified as SCCmec-II, and 10 had one point mutation in the psm-mec promoter. We compared clinical characteristics and outcomes between mutant MRSA and wild-type MRSA.<br /> RESULTS:<br /> Production of PSMα3 in mutant MRSA was significantly increased, but biofilm formation was suppressed. Wild-type MRSA caused more catheter-related bloodstream infections (30/41 vs. 3/10, P = 0.0028), whereas mutant MRSA formed more deep abscesses (4/10 vs. 3/41, P = 0.035). Bacteremia caused by mutant MRSA was associated with reduced 30-day mortality (1/10 vs. 13/41, P = 0.25), although this difference was not significant. The MIC90 of teicoplanin was higher for wild-type MRSA (1.5 mg/L vs. 1 mg/L), but the MIC of vancomycin was not different between the two groups. The 30-day mortality of MRSA with a high MIC of teicoplanin (≥1.5 mg/L) was higher than that of strains with a lower MIC (≤0.75 mg/L) (6/10 vs. 6/33, P = 0.017).<br /> CONCLUSION:<br /> Mutation of the psm-mec promoter contributes to virulence of SCCmec-II MRSA, and the product of psm-mec may determine the clinical characteristics of bacteremia caused by SCCmec-II MRSA, but it does not affect mortality.

    Activities of various enzymes including elastase were determined in sera from healthy control and from patients with systemic lupus erythematosus (SLE), rheumatoid arthritis (RA), IgA nephropathy (IgA NP), and other renal failures. In IgA NP, the activity of elastase increased significantly in parallel with the disease stage. It correlated well with the increase in the amount of polymorphonuclear leukocyte-elastase (PMN-elastase)·α1-antitrypsin complex assayed by ELISA, with the increase in myeloperoxidase (MPO) activity, and also with the increase in proteinuria. Studies on both substrate and inhibitor specificities of the elastase showed that the enzyme is indeed from PMN.

    2006〜2019年の宮城県内の三次医療施設4施設における侵襲性リステリア症症例について後ろ向きに調査した。網羅的病原遺伝子解析の結果、検出された全18株のListeria monocytogenes中、血清型IVbが10株(55%)と最も多く、その内訳はIVb-CC6型が4株(22%)、IVb-CC1およびIVb-CC2がそれぞれ3株(17%)であった。病原遺伝子の分布はそれぞれの遺伝子型毎に特徴を認めたが、遺伝子型と疾患とに関係性は見られなかった。分子系統解析では、2007年に菌血症患者から検出された株と、2012年に感染性大動脈瘤(IAA)患者から検出された株、および2015年にIAA患者から検出された株と2017年に髄膜炎患者から検出された株はそれぞれ同じCT型(IVb-CT1およびIVb-CT6)に属し、それぞれ異なる遺伝子が10個以下と相同性が非常に高かった。NCBIデータベース上の株と、宮城県で得られた株計277株の中で、遺伝子型IVb-CC1・2・6型と、それ以外の遺伝子型の株との病原遺伝子の分布を比較した。その結果、IVb-CC1・2・6型株は細胞壁に存在し宿主細胞への接着に関与するタイコ酸合成遺伝子gltA・gltBおよびLIPI-3遺伝子や、宿主細胞への侵入に関わるaut_IVbやvipの各病原遺伝子を他の遺伝子型株に比べ有意に多く保有していた。分子系統解析では、2013年宮城県・胎内感染症例由来株と2003年ブラジル・ポークソーセージ由来株、1931年アメリカ合衆国・由来不明株と1961年イギリス・鶏由来株、アメリカ合衆国・採取年・由来不明株と2005年ロシア・Myodes rufocanus由来株の遺伝的相同性がそれぞれ非常に高かった。

    OBJECTIVES: Streptococcus pyogenes causes various diseases in humans. While the prevalence of fluoroquinolone-resistant S. pyogenes isolates has been increasing since 2000 in the USA and Europe, it has remained very low in Japan. We isolated a fluoroquinolone-resistant S. pyogenes strain and analysed its genetics. METHODS: TU-296, a strain of S. pyogenes resistant to levofloxacin (MIC 16 mg/L), was isolated from the throat of a patient in their thirties with pharyngitis in autumn 2007. We carried out susceptibility tests for various antimicrobial agents and PCR analysis of the genes gyrA, gyrB, parC and parE in the quinolone resistance-determining region, followed by sequencing of the PCR products to find mutation(s) and the resulting amino acid substitution(s). We then sequenced the PCR product of the emm gene and determined the emm genotype. RESULTS: S. pyogenes TU-296 was found to have the following mutations and amino acid substitutions: adenine 476 to cytosine in gyrA and cytosine 367 to thymine in parC, resulting in Glu-85→Ala in GyrA and Ser-79→Phe in ParC. The genotype of the isolate was emm11. CONCLUSIONS: Amino acid substitutions in fluoroquinolone-resistant S. pyogenes have already been reported from Europe and the USA, including Ser-81→Phe or Tyr and Met-99→Leu in GyrA, as well as Ser-79→Phe, Tyr or Ala and others in ParC. Numerous point mutations were found in parC and parE of S. pyogenes TU-296. In addition, a new amino acid substitution was detected (Glu-85→Ala in GyrA). To our knowledge, there have been no previous reports of this substitution in a clinical isolate of S. pyogenes.

    Pseudomonas (P.) aeruginosa is a major opportunistic pathogen especially in immunocompromised patients. To evaluate the invasiveness of respiratory pathogens, we developed monolayer culture systems and examined the degree of invasion by P. aeruginosa and invasive Salmonella (S.) typhimurium strains using human respiratory cell lines: A549 (derived from lung cancer), BEAS-2B (normal bronchial epithelium), and Calu-3 (pleural effusion of a patient with adenocarcinoma of the lung). Cells were seeded into filter units containing 0.33 cm2 filter membranes with 3.0 μm pores, and were incubated at 37°C under 5% CO2 for 4-10 days. By monitoring the trans-monolayer electrical resistance (TER), we judged that BEAS-2B cells (TER values: 436.2 ± 16.8 to 628.8 ± 66.3 Ω cm2) and Calu-3 cells (TER values: 490.5 ± 25.2 to 547.8 ± 21.6 Ω cm2) formed monolayers with tight junctions, but not A549 cells. On day 8 of culture, monolayer cultures were infected with bacteria, and the number of microorganisms penetrating into the basolateral medium was counted. Wild-type P. aeruginosa PAO1 (PAO1 WT) and S. typhimurium SL1344 were detected in the basolateral medium of BEAS-2B monolayer system by 3 h after inoculation, while only P. aeruginosa PAO1 WT was detected in the basolateral medium of Calu-3 monolayer, indicating poor invasiveness of S. typhimurium SL1344 in the Calu-3 system. These findings suggest that BEAS-2B or Calu-3 monolayer system could be useful for evaluating the invasiveness of respiratory pathogens. Because of the difference in bacterial invasiveness, we may need to choose a suitable cell system for each target pathogen.

    Leukocidin (Luk), an exotoxin of Staphylococcus aureus consisting of LukF and LukS, is a hetero-oligomeric pore-forming cytolytic toxin toward human and rabbit polymorphonuclear leukocytes. However, it is uncertain how Luk affects the host immune response. In the present study, we investigated whether Luk has the ability to stimulate mouse bone marrow-derived myeloid dendritic cells (BM-DCs). LukF activated BM-DCs to generate IL-12p40 mRNA, induce intracellular expression and extracellular secretion of this cytokine and express CD40 on their surface, whereas LukS showed a much lower or marginal ability in the activation of BM-DCs than its counterpart component. Similarly, TNF-alpha was secreted by BM-DCs upon stimulation with these components. Combined addition of these components did not lead to a further increase in IL-12p40 secretion. IL-12p40 production caused by LukF was completely abrogated in BM-DCs from TLR4-deficient mice similarly to the response to lipopolysaccharide (LPS). Polymixin B did not affect the LukF-induced IL-12p40 production, although the same treatment completely inhibited the LPS-induced response. Boiling significantly inhibited the response caused by LukF, but not by LPS. Finally, in a luciferase reporter assay, LukF induced the activation of NF-kappaB in HEK293T cells transfected with TLR4, MD2 and CD14, whereas LukS did not show such activity. These results demonstrate that LukF caused the activation of BM-DCs by triggering a TLR4-dependent signaling pathway and suggests that Luk may affect the host inflammatory response as well as show a cytolytic effect on leukocytes.

    This study aims to investigate the role of IL-36 cytokines in sepsis. We found that the concentration of IL-36gamma elevated in the patients with sepsis,and IL-36Ra packed in extracellular vesicles was released from neutrophils. IL-36gamma enhanced inflammation in inflammatory cells and coagulation and cell injury in endothelial cells. Whereas, IL-36Ra inhibited IL-36gamma induced inflammation, coagulation and cell injury.Indicating that IL-36 cytokines deeply involved in the pathogenesis of sepsis. Finally, we confirmed that IL-36Ra is a potential therapeutic candidate for sepsis using a sepsis mouse model.

    The clinical course and pathophysiological feature of ARDS are characterized as three phases: acute exduative, proliferative and fibrotic. We investigated the role of claudins, a family of proteins in the components of the tight junction, in acute and fibrotic phase of lung injury. We induced nonviral ARDS by alpha-galactosylceramide(GalCer) and LPS and viral ARDS by influenza virus infection. Claudin-5 and -18 are expressed in naive lungs. In both ARDS model, expression of claudin-4 was elevated and claudin-18 mRNA decreased in acute phases of lung injury. Claudin-4 was expressed in not only epithelial cells but leukocytes recruited into the lungs. Micorparticles in BAL fluid after influenza virus infection was observed. Influenza virus induced MPs have a potential to induced the cytokines and chemokines in alveolar machrophasge. These results indicated that claudins may be involved in the pathogenesis of acute phase of lung injury.

    In recent years, environment of medical facilities, it is recognized that it becomes the risk in cross-infection as for the site felt well and the air. We conducted risk assessment as Hospital flora in this study using air sampling and ATP among general hospital, outpatient clinic including dentistry, a long-term care facilities. Multi-drug resistant pathogens (ex. MRCNS) were detected in environment in the general hospital, and, in addition, an outbreak due to an air conditioner might occur. In facility management for the medical facilities, periodical monitoring is recommend.

    Influenza virus is the reason of acute exacerbation bronchial asthma. However, it was lack of understanding for this mechanism. First, we have utilized mouse models of bronchial asthma induced by ovalbumin to experimentally examine a possible aggregation between influenza virus and asthma. We could not find any difference of clinical outcome, accumulation of inflammatory cells and Th1/Th2 related cytokines between normal and asthma mouse after influenza virus infection. We, next, examined whether tissue remodeling was observed after severe inflammation by influenza virus infection. Mice were inoculated non-lethal dose of influenza virus, histopathological analysis presented bronchiectasis and peribronchial collagen diposition and collagen level in the lung was significantly elevated compared to normal mice at the day 35 post-infection. This model could be a novel experimental tool for approaching the pathogenic mechanism of bronchial remodeling after sever inflammation.

    We addressed the possible involvement of claudins in the development of acute lung injury. In a real-time PCR analysis, expression of cluadin 2 and 4 was increased in the injured lungs, whereas that of claudin 5, 6, 9 and 18-1 was reduced. In an immunohistochemical analysis, expression of claudin 2 and 4 was detected in the bronchoepithelial cells of injured lungs. Lung edema caused by LPS administration tended to be worsened in claudin4 gene-disrupted mice. These results suggest that claudins may be involved in thedevelopment of ARDS.
