    PURPOSE: Evaluation of the clinical performance of computer-aided design/computer-aided manufacturing-produced resin composite crowns (CAD/CAM composite crowns) on molars with a particular focus on placement location. METHODS: A retrospective cohort study was performed based on the clinical records of patients with CAD/CAM composite crowns on molars (June 2016 to March 2021). The hazard ratios (HRs) and 95% confidence intervals (95% CIs) were estimated based the Cox proportional hazard model to evaluate the effect of tooth location on complication type and occurrence. Covariates included crown location (maxilla/mandible, distalmost tooth/not distalmost tooth, and first molar/second or third molar) and endodontically treated (nonvital) or untreated (vital) tooth. RESULTS: Overall, 362 crowns were evaluated (mean follow-up: 378 days, median: 286 days), and 106 crowns (29.3%) showed complications, most frequently crown debonding. The cumulative success and survival rates were 70.9% and 93.7%, respectively, after 1 year and 49.5% and 86.5%, respectively, after 3 years. There was no significant difference in the HRs and log-rank tests in the Kaplan-Meier curves based on crown location parameters (P > 0.05). However, placement on vital teeth was associated with higher risks than on nonvital teeth (HR, 1.55; 95% CI, 1.03-2.23). In addition, the cement as a covariate yielded a high HR. CONCLUSIONS: The location of CAD/CAM composite molar crowns is unlikely a risk factor for complications; therefore, these crowns can be clinically applied to all molars. However, the application of such molar crowns to vital teeth and the use of a cement other than adhesive resin cement present risks.

    PURPOSE: The primary purpose of this study was to examine the clinical performance of monolithic zirconia single crowns in terms of short-term failure or complications. The secondary purpose was to detect the originating flaws of clinically failed monolithic zirconia crowns to find the causes of failure. METHODS: A short-term prospective cohort study based on record evaluation and clinical examination of patients treated with tooth-supported monolithic zirconia crowns was performed in the Department of Fixed Prosthodontics, Tohoku University Hospital, Japan. The crowns were prepared during the follow-up period from April 2014 to July 2018. The 3.5-year cumulative success and survival rates were set as primary endpoints. Fractures of the crown or fragments were inspected under a scanning electron microscope for descriptive fractography. RESULTS: During the study period, 40 monolithic zirconia crowns were placed. Four crowns experienced clinical complications, including: 1) fracture of the crown (two crowns), 2) abrasion of the crown (one crown), and 3) fracture of the antagonist tooth (one crown). The estimated Kaplan-Meier 3.5-year success and survival rates were 90.5% (95% confi dence interval [CI]: 73.1-97.1) and 92.8% (95% CI: 74.1-98.3), respectively. Fractography revealed that all fractures were initiated from the wear phase on the occlusal surface. CONCLUSIONS: The results of this study suggest that the molar application of monolithic zirconia crowns requires detailed attention to interocclusal clearance and the restoration of the antagonist tooth.

  6. A Thick Frame Decreases the Fracture Toughness of Veneering Ceramics Used for Zirconia-Based All-Ceramic Restorations Peer-reviewed

    PURPOSE: To investigate the influence of firing condition and thickness of a yttria-stabilized tetragonal zirconia polycrystal (Y-TZP) frame on the fracture toughness of veneering ceramics. METHODS: Layering and pressable ceramics without frames or on 0.8- and 1.2-mm-thick Y-TZP frames were fired with 5 schedules (normal and 4 customized conditions). The fracture toughness of the ceramics was measured at 0.5-2.5mm from the frame by the indentation fracture method. Crystal structures at the measurement points were evaluated by X-ray diffraction (XRD) and scanning electron microscopy (SEM). RESULTS: Under customized firing conditions, the fracture toughness of the layering ceramic significantly decreased, particularly near the frame. Use of a 1.2-mm-thick frame significantly decreased the fracture toughness of both layering and pressable ceramics compared to ceramics without the frame. The decreased fracture toughness only occurred close (0.5 and 1.0mm) to the frame. XRD analysis showed identical diffraction patterns between points near and far from the frame, suggesting that the decreased fracture toughness was not due to crystal defects or impurities. SEM revealed many microcracks and large crystals in layering ceramic near the 1.2-mm-thick frame, possibly resulting from the thermal properties of the zirconia frame, such as large heat capacity and low thermal conductivity. CONCLUSIONS: Modification of the firing conditions from the manufacturer's instructions and inclusion of a thick Y-TZP frame decreased the fracture toughness of veneering ceramics at the interface, suggesting that a thick frame would pose a potent fracture risk in veneering ceramics used for zirconia-based all-ceramic restorations.

    OBJECTIVES: This study aims to investigate the incidence of clinical complications with tooth-supported zirconia-based all-ceramic single crowns and identify pertinent risk parameters. MATERIALS AND METHODS: A retrospective cohort study (May 2004 to April 2016) utilizing clinical records of patients receiving yttrium-oxide-partially stabilized zirconia (Y-TZP)-based all-ceramic crowns placed at Tohoku University Hospital was performed. The length of time of treatment success (complication event-free) and restoration survival (including minor complication events and remaining clinically functional) were estimated using Kaplan-Meier analysis. Multilevel survival analysis was used to identify risk factors. RESULTS: One hundred thirty-seven crowns were evaluated (mean follow-up time, 7.0 years). A total of 21 crowns experienced at least one complication with fracture of veneering ceramic being the most common (16 crowns). Estimated success and survival rates at 5 years (96.9 and 98.5%, respectively) decreased at 10 years to 62.1 and 67.2%, respectively. The risk of complications was significantly higher for molar crowns compared to anterior crowns (p < 0.01). A significant association of complications with metal antagonist restorations was shown by univariate analysis (p < 0.01). CONCLUSIONS: Given the study limitations, Y-TZP single crowns placed on anterior teeth demonstrated encouraging clinical results over a period of up to 10 years. However, there is a substantial risk of complications with posterior teeth within 10 years of restoration placement. CLINICAL RELEVANCE: Treatment with zirconia-based all-ceramic crowns for molar teeth with metal antagonist occlusion should be undertaken with caution.

    PURPOSE: Hard resins for crowns and bridges are widely used for esthetic restorations. The objective of this study was to evaluate the mechanical properties of new commercial hard resins and to compare the results with those of the other hard resins previously investigated. METHODS: Dentin and enamel made with two new hard resins (Epricord: EP, Kuraray, Co., Ltd., Osaka, Japan and Prossimo: PR, GC, Co., Ltd., Tokyo, Japan) were used in this study. Regarding the fundamental characteristics, the thermal expansion/shrinkage coefficient, the filler content, the polymerization shrinkage, and the wear were examined. Regarding the strength of resin, the bending strength, hardness, compression strength, elastic modulus, and fracture strength of a jacket crown were measured. RESULTS: These resins showed comparatively lower levels than the other hard resins regarding the bending strength, hardness, compression strength, and fracture strength of the jacket crown. The total filler content rate and wear amount of these resins exhibited similar values to those of the other resins. The thermal expansion/ shrinkage coefficients of these resins exhibited higher values than those of the other resins. EP showed a different tendency from PR about the compression strength, elastic modulus, and polymerization shrinkage. CONCLUSIONS: PR and EP did not show dramatically better physical properties. However, the results of each examination in this study may be acceptable clinically. The results of each investigation changed according to the products used, and proper use for each case and application was suggested.

    PURPOSE: The purpose of this study was to investigate the coefficients of thermal expansion and shrinkage of an all-ceramic system (cercon) smart ceramics, DeguDent) utilizing tetragonal stabilized zirconia. METHODS: The coefficients of thermal expansion and shrinkage of the core material and the layering porcelains (dentin and enamel) used in this study were measured according to the ISO 9693 standard. Five specimens for the core material and ten specimens for each layering porcelain were tested. The core specimens were milled, sintered, ground and polished. Five of the specimens for each layering porcelain were fired two times, and the remaining five specimens were fired four times. The fired layering porcelain specimens were ground and polished. The coefficients of thermal expansion and shrinkage were evaluated using a push-rod dilatometer at a heating rate of 5 degrees C /min over temperature ranges of 25-700 degrees C for the core, and 25-550 degrees C for the layering porcelain. For each specimen, the coefficients of thermal expansion was determined to be between 25 and 500 degrees C from the plotted curve of expansion versus temperature. RESULTS: For the core material, the coefficients of thermal expansion and shrinkage showed almost the same value (10.8 x 10(-6)/ degrees C). For the layering porcelains, the coefficients of thermal expansion and shrinkage ranged from 9.3-11.1 x 10(-6)/ degrees C. The difference of the coefficients of thermal expansion and shrinkage between the core materials and the layering porcelains was -0.3-1.5 x 10(-6)/ degrees C. The specimens fired two times and the specimens fired four times exhibited almost the same value for each layering porcelain. CONCLUSION: The core material and layering porcelains have a suitable relationship.

    Purpose: For the purpose of evaluating the flow properties influencing the formativeness of commercial hard resin pastes used in the construction of crowns and bridges, we examined the behaviors of various resin pastes and their temperature dependence during manipulation.<BR>Methods: Five types of hard resins currently in use for the construction of crowns and bridges in the clinical setting, namely, New Meta Color INFIS, Solidex, Ceramage, Gradia, and Estenia C&B, were examined. The components and flow properties of each resin paste were investigated in relation to the dentine and enamel colors. The viscosities of the resin pastes before curing were measured by a cone-plate viscometer.<BR>Results: Each resin paste demonstrated a non-Newtonian flow, and the flow curve showed a hysteresis loop, in which the upward and downward curves were different. When the shear stress at the beginning of the holding time of the flow curve (20°C, 25°C, 30°C) of each resin paste was measured, the shear stress at 20°C was found to be significantly higher than that at 25°C and 30°C, and the shear stress at 25°C was found to be significantly higher than that at 30°C. The viscosity was found to be higher at lower temperatures than at higher temperatures. Comparison of the shear stresses at the beginning and at the end of the holding time revealed that the stress at the beginning of the holding time was significantly higher than that at the end of the holding time for all except the Estenia C&B resin paste.<BR>Conclusions: Each of the resin pastes used in the present study showed temperature dependence of its characteristics in the temperature range examined (20-30°C) and thixotropic fluidity. However, the flow characteristics of these resin pastes differed significantly, suggesting that the flow characteristics may affect the manipulation of resin formativeness.

    The purpose of this study was to give polyaddition-type silicone impression materials (Regular Type) the characteristics that the pastes are easy to mix, and they are difficult to hang down from the tray. In a previous study, the inorganic filler significantly affected the flow characteristics of impression materials. As usual, polyaddition-type silicone impression materials (Regular Type) do not have a yield value in the flow characteristics. A material with a yield value has superior workability characteristics.<BR>We attempted to make pastes of mixtures having a yield value. The silicone prepolymers used were vinyl polydimethylsiloxanes with 56, 000 and 85, 000 molecular weights (Mw), and the inorganic fillers used were quartz and synthetic silica (micro filler). The flow properties of experimental pastes were investigated by using a cone-plate viscometer, from the rheological point of view. As a result, a little addition of synthetic silica in experimental pastes made a flow property having a yield value and was effective in working. This study suggests a direction in manufacturing impression materials toward including superior characteristics that had low viscosity and was easy to mix in working.

    The purpose of this study was to investigate flow characteristics of four commercial polyaddition-type silicone impression materials (Regular Type) before mixing.<BR>Base and catalyst pastes were filler and polymer separated by a centrifugal separator with benzene for component analysis. As a result, weight fraction of filler was 39-55% and the chief ingredient was silica. The main polymer of base and catalyst pastes was polydimethylsiloxane. Especially, base polymer included hydrogen polydimethylsiloxane which was a crosslinking agent. The molecular weight (Mw) of these polymer was mostly about 40, 000-70, 000.<BR>The viscosities of these pastes before mixing were measured by a cone-plate viscometer. Both base and catalyst pastes demonstrated pseudoplastic behavior which is the non-Newton flow. Particularly, base pastes showed thixotropic properties because of their clear hysteresis curves and time dependency on the viscosity. One of the base pastes had a yield value.<BR>The inorganic filler significantly affected on flow characteristics of impression materials. The results suggested a means to manufacture impression materials with superior characteristics for working.

    In this study, the relationship between the composition and the flow properties of three commercial tissue conditioners was investigated. Analysis of the composition was chiefly done by infrared spectroscopy (IR). As for the liquid, plasticizer was qualitatively determined and the content of alcohol was measured by distillation. The characteristics of polymer powders were obtained by means of gel permeation chromatography (GPC), particle size analyzer and SEM. As for the flow property, the apparent viscosity of a mixture of liquid and powder was measured by a cone and plate rotational viscometer.<BR>As a result, the liquid was a mixture of a phthalic ester plasticizer (dibutyl phthalate) with up to 13% ethyl alcohol. The polymer powders generally consisted of polyethyl methacrylate (PEMA), but one was a blend of PEMA and polyisobutyl methacrylate (iso-PBMA).<BR>The viscosities of mixtures were found to increase rapidly with an increase in the ethyl alcohol content, and an increase in temperature. Further, the viscosity curve of the tissue conditioner, of which the powder was a blend of PEMA and iso-PBMA, showed characteristic behavior. Thus it was suggested that the flow property of the tissue conditioner was affected by the molecular structure of the polymer powder.

    The aim of this study was to evaluate the mechanical properties of a flowable hard resin. Strength of this resin was investigated by three-point bending test. The samples were prepared made by laminating a paste type of this resin and a flowable type of this resin as well as samples made with only flowable type of resin. Samples made with only flowable type of this resin were exhibited sufficient strength. However, in order to avoid problems at the production of the facing crown, more to use a paste type of resin primarily would be better.

    The purpose of this study is to evaluate the dimensional accuracy of plaster model applying a micro X-rays CT which is measureable non-destructively. Evaluation was performed by comparing the plaster model applying the micro X-rays CT and a cut plaster model which is measured directly. From the results of this study, it was shown that there was non-significant difference between two methods. Therefore, the micro X-rays CT was shown to be a useful technique to assess the dimensional accuracy of plaster model.

    We aimed to develop Ti-Mg alloys with possibility that magnesium in their solid solution stimulated biomedical tissues as a trigger and that it resulted in leading bone morphogenetic growth. Although we invented a new alloying method applying gas-solid reaction, it did not work and we could not obtain the solid solution in which magnesium diffused. In concurrently running works, we succeeded to fix magnesium on the titanium surface easily as a compound, which was obtained by a surface modification of the magnesium ions on the alkaline heating treatment titanium surface. Sheet and cylinder types of specimens modified by magnesium ions were implanted into subcutaneous and thighbone of a rat, respectively. According to those tissues at the vicinity of the implants after two weeks, fiber structures on the modified specimens in the subcutaneous was thinner than those on pure titanium specimens. The improvement of the bone contact ratios could be confirmed by the modification of magnesium.

    This study was carried out by paying attention to the properties of titanium dioxide. Titanium dioxide applied to environmental technology can be expected to provide a self-cleaning effect, and it has good wettability with water. The samples were made by replacing the titanium dioxide part of the hard resin's composition. As a result, the sample had a mechanical strength comparable to commercially available material. In addition, improvemed wettability was observed in that sample. The improved of wettability of the sample is related to the fact that material is less likely to adhere to the surface. From these results, it was possible to obtain suggestions for developing a veneering resin to which dental plaque will have difficulty adhering.

    本研究の前年度の目的は,精密弾性印象材として臨床において広く用いられている重付加型シリコーンゴム印象材の操作性の向上,すなわち,硬化前ペーストに練りやすさと垂れにくさを付与することであった。これを受けて今年度は硬化体の物性の向上を目指して研究を行った。 前年度に得られた試作ペースト中,練りやすさと垂れにくさを兼ね備え,最も操作性が良いと思われたキャタリストペースト相当のフィラー含有率40%のペーストに希釈触媒液を加えて,フィラー含有率40%のベースペースト相当のペーストと練和することにより硬化体を試作した。 硬化体は,はじめに動的試験として粘弾性スペクトロメータによる動的粘弾性試験を行い,貯蔵弾性率と損失正接を測定した。その結果,貯蔵弾性率は市販の従来型よりも小さな値をとる傾向を示し,損失正接は市販の高弾性型よりも小さな値をとる傾向を示した。また,静的試験として弾性ひずみ,永久ひずみ,硬さ試験を行った。その結果,市販材料の従来型と比較して弾性ひずみは大きく,永久ひずみは同程度硬さは小さな値をとる傾向を示した。 レジンとの接着に関しては,3種類の市販印象材に附属した接着剤について赤外分光光度計を用いて赤外分光分析を行い,含まれている官能基を推定した。その結果,共通してメチル基を含む有機けい素化合物が含まれていることが推定された。また,レジンとの接着が比較的良好な市販材には脂肪族不飽和炭化水素の存在が推定された。これらを参考に接着剤を試作しレジンとの接着性を検討した。その結果,接着強さはレジンとの接着が比較的良好な市販材料と同程度を示し,破断面は界面破壊を認めた。

    The purpose of this study was to develop the new tissue conditioners, which consist of poly (butyl methacrylate) and the liquid plasticizer without an endocrine disrupter. 1.Syntheses of polymers Homopolymers (isoPBMA, PEMA) and copolymer of isobutyl methacrylate (isoBMA) and ethyl methacrylate (EMA) were synthesized by suspension polymerization in aqueous gelatin. 2.Particle size and molecular weight of polymers The particle size of polymer powder is related to flow properties and gelation time of powder/liquid mixtures. The particle sizes of polymer powders of isoPBMA, PEMA, and copolymer were 32μm, 29μm, and 35μm, respectively. The molecular weight of polymer is related to viscoelastic properties of powder/liquid mixtures after gelation. The weight average molecular weight. of isoPBMA and copolymer was 280,000, and that of PEMAwas 210,000. 3.Flow properties of powder and liquid mixtures The viscosity-time curves of mixtures of copolymer powder and plasticizer dibutyl sebacate (DBS) containing ethanol were measured by the cone-plate viscometer. After two or three minutes, the viscosities of mixtures increased rapidly with gelation. It was found that ethanol content of 13〜15% and powder/liquid ratio of 1.3〜1.4 were useful and practical for a tissue conditioner. 4.Dynamic viscoelasticity of powder and liquid mixtures The viscoelastic properties of mixtures of copolymer powder and DBS containing 13% ethanol after gelation were investigated by the dynamic method. The viscoelastic data of storage modulus G' and loss tangent tan δ versus frequencies at different temperatures were superimposed to a master curve by application of the time-temperature superposition principle. These viscoelastic properties of new experimental materials were similar to those of a commercially available tissue conditioner. This study indicates that a new tissue conditioner without an endocrine disrupter can be made by use of copolymer and DBS.
