    INTRODUCTION: The COVID-19 pandemic has exacerbated psychological distress among frontline and non-frontline nurses. However, several research findings suggest that workplace social capital may have been strengthened during the COVID-19 pandemic, thereby reducing the deterioration of psychological distress. OBJECTIVE: This study investigated the perceptions of change in workplace social capital from before the COVID-19 pandemic to the time of the survey. This study also examined the influence of variables related to the COVID-19 pandemic, such as experience of discrimination related to COVID-19, on psychological distress by targeting hospital nurses. METHODS: An online survey was conducted among nurses at 138 hospitals in a prefecture in the Tohoku region of Japan from July 7 to August 8, 2022; respondents worked in their respective hospitals before and during the pandemic (N = 1,075). Generalized estimating equations (GEE) was used with psychological distress as the dependent variable, perceptions of change in workplace social capital and variables related to the COVID-19 pandemic as independent variables, and personal attributes as control variables. RESULTS: Four hundred and fifteen (38.6%) participants perceived a deterioration in workplace social capital before the COVID-19 pandemic, whereas 194 (18.0%) perceived improvements. Their psychological distress was influenced by marital status, experience dealing with patients suspected of having COVID-19, experience of discrimination based on COVID-19, workplace social capital at the time of the survey, and the perceptions of change in workplace social capital. CONCLUSION: Some nurses perceived improvements in workplace social capital owing to the COVID-19 pandemic. Changes in workplace social capital and experiences of discrimination based on COVID-19 were factors affecting nurses' psychological distress. To prepare for future pandemics, it is necessary to examine the deterioration of workplace social capital and discrimination against medical workers caused by COVID-19 and consider strategies to prevent the worsening of nurses' psychological distress.

    Background: During the COVID‐19 pandemic, nurses’ workload and subjective work‐related burden increased. These issues interacted and exacerbated nurses’ psychological distress. This study aimed to clarify the relationship between nurses’ workload and subjective burden on psychological distress. Methods: We conducted an analytical cross‐sectional analysis using self‐administered questionnaires in December 2020. In total, 1299 nurses working at multiple hospitals in Prefecture A in Japan participated in this study through convenience sampling. Hierarchical multiple regression analysis was performed with psychological distress as the dependent variable and changes in workload and subjective work‐related burden as the independent variables. Results: The mean score of nurses’ psychological distress during the COVID‐19 pandemic was 6.73 ± 5.28. The hierarchical multiple regression analysis demonstrated that psychological distress was positively correlated with workload. However, after the subjective work‐related burden was included as an independent variable, the effect of the change in workload on psychological distress disappeared. An increase in subjective work‐related burden was found to be the biggest influencing factor of psychological distress, with the level of psychological distress higher when the subjective work‐related burden was high. Conclusions: The results suggest that nurses’ psychological distress was significantly affected by subjective burden but not by changes in workload. This study highlighted the impact of subjective burden on the psychological distress of nurses during the early period of the COVID‐19 pandemic. We recommend paying more attention to nurses’ subjective perceptions than issues such as increased workload. The practical suggestion offered is that nursing managers should pay attention to changes in nurses’ workloads and their perceptions of those changes, as subjective perceptions could help reduce psychological distress.

    Elucidating nurses’ work values will contribute to creating a work environment consistent with individual nurses’ work values and help them overcome challenging situations. This study aimed to create a short version of the Nurses’ Work Values Scale and verify its psychometric properties and usability among nurses, public health nurses, midwives, and licensed practical nurses. The short version was developed in three steps. In Step 1, a 12‐item scale was created from the original 30‐item Nurses’ Work Values Scale via a multidimensional item response theory based on data obtained from 1587 nurses. In Step 2, we modified the wording of the items and verified the scale’s content validity. In Step 3, we obtained new data from 1220 nurses, public health nurses, midwives, and licensed practical nurses who worked in various settings. Subsequently, we verified the psychometric properties of the 12‐item scale. The developed scale made it possible to measure work values among nurses working in hospitals, nursing care facilities, visiting nursing settings, and public health nurses. Simultaneously, the burden of responding to surveys was reduced. In the future, it is hoped that this scale will be used on a large longitudinal cohort of nurses working in various workplaces to track life events and changes in family background, thereby clarifying changes in work values and the factors that influence them. In addition, by knowing nurses’ values, nursing managers can take measures such as adjusting work to match each nurse’s work values. This could help promote alignment between the values of individuals and organizations and reduce turnover rates.

    ABSTRACT Background Day‐service centres offer valuable support for individuals with intellectual disabilities, often referred to as service‐users. Due to potential communication challenges, nurses must gather information from caregivers, particularly mothers. This study explores the collaboration between nurses at day‐service centres and the mothers of service‐users to enhance care quality. Methods This qualitative study collected data from six nurses working at day‐service centres in Japan and conducted an analysis using a descriptive narrative approach. Each nurse participated in two or three semi‐structured interviews, where nurses were encouraged to speak freely, resulting in 16 interviews. Findings Three key themes emerged from the collaboration between nurses and service‐users’ mothers: (1) ‘building good relationships with parents to ensure a smooth exchange of information about service‐users’; (2) ‘providing the same care at the day‐service centre as the service‐user would receive at home’; and (3) ‘identifying areas that nurses cannot compromise on while respecting how mothers care for their children’. Conclusions These findings provide insights into effective nursing care for individuals with intellectual disabilities in day‐service centres, emphasising the importance of productive engagement with their mothers.

Therefore, the following are suggested: For nurses working in day care facilities for individuals with disabilities, utilizing the "space" when they spend time with service users without relying on verbal communication can lead to awareness of the abilities of service users; and providing service users opportunities to experience a variety of things is important while believing in their potential and growth, not merely focusing on what each service user can do.

    Abstract Aim Although researchers have emphasized the importance of enhancing work engagement in nurses to ensure work‐related well‐being, the underlying mechanisms of the influencing factors of work engagement remain unclear. This study aimed to elucidate whether work‐to‐family spillover moderates the relationship between nurses' work values and work engagement. Methods In total, 2600 nurses from 52 hospitals in the Tohoku region of Japan were recruited, and 1587 nurses participated. The questionnaire included items on demographic characteristics, the Nurses' Work Values Scale, the Japanese version of the Work‐life Balance Scale, and the Work Engagement Scale. Data were analyzed using hierarchical multiple regression. Results The interaction terms of negative work‐to‐family spillover with extrinsic work values and prestige work values were significantly associated with work engagement. The interaction terms of nurses' work values and positive work‐to‐family spillovers did not show a significant association. Conclusions Nurses who emphasized extrinsic work values and those who did not emphasize prestige work values had the lowest work engagement in settings with high negative work‐to‐family spillover. To ensure nurses' work‐related well‐being and engagement, nursing managers could provide support for nurses in tackling work‐related negative events, so that these do not carry over to family settings.

    This study aimed to classify nurses with similar work values into subgroups by examining their intrinsic, extrinsic, social, and prestige work values. Additionally, we clarified the characteristics of the obtained subgroups using personal attributes, work engagement, and life satisfaction. Using a cross-sectional observational study design, we randomly sampled 52 hospitals in the Tohoku region of Japan and conducted a self-administered questionnaire survey with 2600 nurses. Latent profile analysis was performed to identify the number of subgroups. Of the 1627 collected questionnaires, 1587 were regarded as valid. The latent profile analysis revealed the following five subgroups with strong statistical significance: (1) self-oriented, (2) low, (3) medium-low, (4) medium-high, and (5) high types. The means of work engagement and life satisfaction gradually increased from the (2) low- to (5) high-type subgroups. There were significant differences among the subgroups in terms of marital status, child status, and job title. The (5) high-type subgroup had many nurses with job titles, high work engagement, and high life satisfaction. The (2) low-type subgroup included many nurses who were young, had few years of experience, were married, had children, and had low levels of work engagement and life satisfaction. Preregistration: This study was not registered.

    This study aimed to identify the learning requirements of nurses practicing diabetic foot care. Semi-constructive interviews were conducted with 15 nurses who had completed appropriate training in the teaching of diabetic foot lesion care and who were involved in the foot care of patients with diabetes at a medical institution. The obtained data were qualitatively and descriptively analyzed. Five categories of learning needs were generated: biomechanics-based foot care, assessment skills in foot care practice, techniques to facilitate multidisciplinary collaboration, direct foot care techniques performed on patients, and developing new educational tools to deepen learning. Our findings suggest learning needs that could not be fulfilled by conventional training programs alone. Thus, participants may feel a gap between the skills and abilities they acquired during training and those required in the field. Therefore, to address the complex and diverse needs of patients with diabetic foot lesions in the future, establishing an educational system that incorporates the perspectives of physical therapists, prosthetists, and other professionals, and also utilizes audiovisual aids and other educational materials is necessary.

    AIM: The study aimed to conceptually define "shame" within the field of nursing. BACKGROUND: Many nurses sometimes experience and struggle with shame in their professional lives. It reduces their sense of self-worth and hampers emotional well-being and efficacy at work. DESIGN: The conceptual analysis was performed using Walker and Avant's method. DATA SOURCES: We searched using Academic Search Complete, CINAHL, PsycINFO, MEDLINE, and Psychology and Behavioral Sciences Collection databases for literature published between 1980 and 2020. REVIEW METHODS: We searched for keywords "shame," "nurse," and "nursing," with the condition that the keywords must be included in the title or abstract. RESULTS: Shame in the nursing field was defined as a negative emotion, an experience of self-blame and anger, an emotion accompanied by social anxiety, loneliness, and influenced by society and culture. Shame in the nursing field has three antecedents: negative evaluation, the involvement of others, and social and affiliated-group norms. Consequences of shame in nursing include decreased senses of self-esteem and self-efficacy, escape through defense mechanisms, depressive states, and alleviation of distress through reaffirmation of self-promotion and reflection leading to personal growth. CONCLUSIONS: We clarified the significance of self, others, others' evaluations, and differences in socio-cultural contexts while defining shame.

    Objective: Research evidence demonstrating the effect of Partnership Nursing System (PNS) on nurses is limited because it has only recently been developed. Therefore, this study aimed to clarify the effect of PNS on nurses’ autonomy and nursing practical ability. Methods: A questionnaire survey was conducted among nurses with less than three years of experience working in general wards at six facilities with and without the PNS. We compared the scores of autonomy and practical nursing ability of 192 nurses with only PNS experience (PNS group) and 121 nurses without PNS experience (non-PNS group). Results: Scores on autonomous clinical judgment and control over working conditions were lower in the PNS group than in the non-PNS group. However, job-related independence was similar among the two groups. The PNS group showed significantly larger dispersion in scores on autonomous clinical judgment than the non-PNS group, indicating that nurses in the PNS group had particularly low autonomy. Scores on all factors of practical nursing ability did not differ significantly between the two groups. Conclusions: Results suggest that PNS may interfere with the development of nurses’ autonomy and that nurses with low autonomy may choose facilities that implement PNS.

    Abstract Background Career competencies, which are the knowledge, skills, and abilities essential for career development, have been shown to facilitate career success, fulfilling both work and life goals. In dynamically changing healthcare settings, nurses’ career competencies are key for successfully navigating their careers and improving their nursing practice abilities. However, limited studies have examined career competencies in the nursing profession. In particular, no research has been conducted on career competencies among Japanese nurses, which remains a major challenge as voluntary effort is the main factor promoting career and professional development. Therefore, the purpose of this study was to evaluate the validity and reliability of the Japanese version of the Career Competencies Questionnaire (CCQ-J). Methods In this cross-sectional study conducted between June 2020 and August 2021, the English CCQ was translated into Japanese using back and forward translation. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were conducted on separate samples. In the first step, item analysis and EFA were conducted with 276 nurses from one hospital. In the second step, CFA was conducted and concurrent validity and reliability were evaluated with 522 nurses from hospitals in the Tohoku region. Results Content validity was confirmed by the back-translation report, an expert panel, and a pilot test. The EFA showed that the CCQ-J consisted of a three-factor structure that explained 66.69% of the variance. The CFA revealed that all the fit indices were acceptable [chi-square value (CMIN) = 432.26, degree of freedom (df) = 153, chi-square fit statistic/degree of freedom (CMIN/df) = 2.83, goodness-of-fit index (GFI) = 0.93, adjusted goodness of fit index (AGFI) = 0.89, comparative fix index (CFI) = 0.96, and root mean square error of approximation (RMSEA) = 0.06]. Cronbach’s α for the 21-item CCQ-J and its subscales ranged from 0.85 to 0.95. Concurrent validity was demonstrated via the positive correlation between work engagement, life satisfaction, and the CCQ-J. Conclusions The CCQ-J is a valid and reliable instrument to assess the career competencies of Japanese nurses. We hope that the findings presented in this study will contribute to a better understanding of nurses’ career competencies and their successful career and professional development in the future.

    Nurses’ work motivation impacts their job satisfaction and work engagement, affecting their quality of care. Work motivation, a personal resource, can be categorized into intrinsic and extrinsic motivation, each of which may function differently in the job demands–resources (JD–R) model. To study the effect of nurses’ intrinsic and extrinsic work motivation on work engagement in long-term care (LTC) facilities, we randomly selected 1200 facilities from 6055 LTC facilities in eastern Japan. Two nurses from each facility completed a self-report questionnaire—newly developed for this study for evaluating intrinsic and extrinsic work motivation—to assess their work engagement, job satisfaction, and work motivation. Multiple regression analysis of 561 valid questionnaires investigated the relationship between work motivation and work engagement, indicating that intrinsic work motivation, job satisfaction, and age had a significant positive effect on work engagement, while extrinsic work motivation had no significant effect. However, half the nurses chose to work because of extrinsic work motivation, explaining the high turnover rate of nursing staff in LTC facilities. Findings indicate the importance of measures to foster nurses’ intrinsic motivation to improve work engagement. Further research should investigate how to improve the intrinsic motivation of nurses working in LTC facilities.

    This study examined the impact that the attractiveness of working in nursing homes and autonomous clinical judgment have on affective occupational commitment, and whether work engagement mediates these relationships. This analysis was based on the job demands-resources theory. The study setting was 1200 nursing homes (including long-term care welfare facilities and long-term care health facilities) in eastern Japan. An anonymous, self-report questionnaire survey was administered to two nurses from each facility, resulting in a prospective sample of 2400 participants. Overall, 552 questionnaires were analyzed, in which structural equation modeling and mediation analysis using the bootstrap method were performed. The results showed that the attractiveness of working in nursing homes does not directly affect affective occupational commitment; work engagement fully mediates the impact of attractiveness of working in nursing homes on affective occupational commitment. Additionally, autonomous clinical judgment showed a direct impact on both work engagement and affective occupational commitment, indicating that work engagement partially mediates the impact on affective occupational commitment. To increase the affective occupational commitment of nurses working in nursing homes, managers should help nurses recognize the attractiveness of working in nursing homes, and then provide appropriate support to help such nurses work in a motivated manner.

    AIM: We developed and psychometrically tested the Japanese version of the Professional Attitude Scale for Nurses (PASN-J). BACKGROUND: Nurses must recognize the importance of their professionalism; therefore, it is critical to quantitatively measure nurses' professional attitudes. INTRODUCTION: This instrument validation study was designed to generate an itemized scale and examine its content validity/psychometric testing using a sample of Japanese nurses. METHODS: Based on a trait approach focusing on the characteristic traits of the nursing profession, a 59-item draft scale was generated. During November 2017, 2657 nurses from 29 facilities in Japan were surveyed. The questionnaire included demographics, the 59-item draft scale, and a self-report scale of nurses' professional behaviour and nursing practice ability. Using exploratory and confirmatory factor analyses, we evaluated the construct, criterion-related, concurrent, and known-groups validity, and reliability of the PASN-J. RESULTS: Data from 1716 participants were analysed. The analyses yielded a 38-item, 3-factor scale that adequately fit the data. PASN-J scores were positively correlated with nurses' professional behaviour and nursing practice ability. CONCLUSION: The 38-item PASN-J has good reliability and validity, making it useful for measuring the current condition of nursing professionalism and evaluating nursing education. Implications for Nursing and Health Policy: This scale can evaluate nursing education and promote nurses' professionalism. The PASN-J will help identifying the elements of undergraduate nursing education that require further emphasis. Additionally, the PASN-J could facilitate the development of nursing policies to promote professional development in nurses. Ultimately, evaluating nursing education with the PASN-J enhances nurses' professional attitudes and subsequently improves their quality of nursing, nursing efficiency and patient outcomes.

    Background: Nurse practitioners' role is always expanding. The Japanese Nurse Practitioner (JNP) system was initiated in 2015 to shift some aspects of doctors' work to various other healthcare professionals, including nurses. JNPs' fulfillment of their roles was shown to have a certain degree of efficacy and provide positive outcomes for patients (e.g., shortening hospitalization period). Nurse practitioners are considered legally liable for their medical practices because they are performed on doctors' behalf; however, in real life, there is ambiguity regarding such practice. It is necessary to clarify nurse practitioners' legal liability in order to ensure the safety of their medical practice and protect them in medical procedures performed on physicians' behalf. This study aimed to clarify how JNPs understand their own legal liability in medical practice. Methods: A qualitative, inductive research design was adopted to record participants' opinions. The survey was conducted from October 2017 to February 2018. Participants were nurses working as JNPs at general hospitals in eastern Japan. We recruited participants via snowball sampling. Results: With regard to JNPs' legal liability in their medical practice, three themes understanding were observed: "determining whether the JNP has the ability to perform the assigned medical procedure," "exercising caution when performing medical procedures on a doctor's behalf" and "an urge to follow up with appropriate medical practice until the end of care." Conclusions: We demonstrated that JNPs recognized their own legal liability in medical practice. They had to protect themselves because their legal position was ambiguous. Furthermore, JNPs accepted that diagnosis and drug prescription could be performed on behalf of doctors if trusting relationships had been previously established.

    A病院の集中治療室(ICU)を対象に、看護師の引き継ぎを立位にすることで、坐位と比較して引き継ぎ時間が短縮するかについて準実験的研究を行った。病床数10床に対して2チーム制をとっており、2チームそれぞれの夜勤リーダー看護師が、同チームの日勤看護師全体に対して引き継ぎを実施していた。60日×2チームで述べ120回の引き継ぎを対象とし、引き継ぎ姿勢を立位および坐位に60回ずつ均等に無作為に割り付けたうち、合計106回のデータを分析対象とした。坐位群53回、立位群53回であった。立位で引き継ぎを実施することで、坐位で引き継ぎを実施する場合と比較して引き継ぎ時間が短縮することが示唆された。個人単位での比較では、看護師12名中9名が立位で引き継ぎ時間が短縮しており、短縮の傾向は認められたが、統計的には有意差はなかった。看護師によって引き継ぎ時間のばらつきが大きいため、背景要因とは別に個人要因の結果への影響を排除する必要があったと考えられた。

    本研究の目的は、専門職の構成要件を理論的背景として、看護師が専門職として確立するために必要な看護師の専門職意識を構成する概念を明らかにすることである。そのため文献レビューを用いた演繹的アプローチを用いた。次にそれらの記述を類似の内容ごとに整理して、専門職意識の理論的背景となる専門職の構成要件を導出した。結果、専門職の構成要件は、3つの要件に分類することができた。社会学の中で広く用いられ、専門職の有する特性を明確に論じている8つの文献から専門職が有すべき特性に関する記述を抽出した。その他のことから、専門職としての看護師は、雇用された組織ではなく職業集団に価値基準を準拠させて、判断や行動をしていく必要がある。

    2022年度は以下のことを実施した。(1)宮城県の病院に勤務する看護職を対象とした第3期質問紙調査、(2)COVID-19の感染を経験した患者の質問紙調査。 (1)からは以下のことを明らかにした。対象者全体では、COVID-19感染拡大前から調査時にかけて職場のソーシャルキャピタルが有意に低下していた。対象者を個別に見ると、一部の職場は職場のソーシャルキャピタルが改善していたが、悪化した職場が多かった。交互作用の結果から、COVID-19陽性疑い患者を対応した看護師にとって、特に職場のソーシャルキャピタルが悪化した場合に精神健康が悪化していた。看護師の精神健康の悪化を防ぐには、COVID-19陽性患者対応を行っている看護師だけではなく、疑いのある患者対応を行う看護師へも精神的サポートが必要。COVID-19感染拡大などの災害時にも職場のソーシャルキャピタルを低下させないことが重要。 (2)からは、新型コロナウイルス感染症の症状が強い患者の受診が妨げられていたことを明らかにし、限りある医療資源の有効活用のために、看護職等による軽症者への健康フォローアップや、体調悪化時の相談体制の強化が重要となることが示唆された。

    本研究は、看護提供方式が看護師・組織・患者アウトカムに及ぼす影響の解明を目的として実施している。日本の看護提供方式の中で主流であるチームナーシング、パートナーシップ・ナーシング・システム、セル型看護提供方式の3つにフォーカスを当て、それぞれの看護提供方式のアウトカムについて検討を計画している。研究の大きな特徴は、看護提供方式のアウトカムを複数の指標を用いて多面的に測定することである。特に測定が困難な患者アウトカムを評価指標に含めた点は、これまでの研究にはない新しい点であった。患者アウトカムの測定には、先行研究で開発した、Web版Patient eXperience(患者経験:以下PX)調査システムの使用を予定していた。 現在は、看護提供方式のアウトカム測定に先んじて、PX調査システムの項目の修正および妥当性・信頼性の検証に取り組んでいる。先行して開発したPX調査システムで収集したデータを分析し、課題を明確化した上で、それらの課題を克服した改訂版PX調査システムの開発を実施している。改訂したPX調査システムを使用することで、これまでより効率的に患者アウトカムの測定が行えるようになると期待できる。 また、2021年度に実施予定としていた、看護提供方式と看護師の成長および業務環境に関する横断調査の準備を進めた。仮説と測定項目を再検討し、調査計画の骨子を整えた段階である。2022年度の早い段階で複数の医療施設をフィールドとして調査を予定している。

    平成30年度は、地域完結型医療に関わる場で就業する看護職を対象とし、彼らの専門性、専門職性などに関する実態調査(予備調査と本調査)を実施した。 予備調査は、宮城県に所在する介護施設に勤務する看護職を対象に、半構成的面接を実施した。予備調査の結果、看護職の専門性として「利用者中心でのケア」「多職種・家族との連携・指導と関係構築」「医師のいない環境での医療的判断や処置」「医療行為に関する裁量の発揮」「医療現場とは異なる価値の徹底」などの領域が明らかになった。 本調査は、東北地方並びに関東甲信越地方の介護施設を無作為抽出し、そこに勤務する看護職2,400名を対象として無記名自記式質問紙調査を実施した。その結果、介護施設に勤務する看護職の専門職性の平均値は、病院の看護職と比べて、自律性は同等であるが分布に差があること、存続的/規範的職業コミットメントは介護施設の看護職のほうが得点が高いことが明らかになった。加えて、介護施設で働く看護職の心理社会的労働環境(蓄積疲労と精神健康)の平均点は、病院に勤務する看護職よりも低く状態が良好であることが示された。 本調査の質問紙の回収作業は終了していないため、引き続き回収しデータの解析を進める予定である。加えて、平成30年度は入所型の介護施設のみを対象としたが、平成31年度は、対象施設を訪問看護ステーションや診療所等に広げて調査する必要性もある。したがって、平成30年度に収集したデータの解析を進めつつ、追加調査を実施するか検討する。

    The purpose of this study was to examine a new method of assessing nursing quality. Considering the fact that nursing quality assessment tends to be biased toward the incidence of negative events as an issue, we attempted to assess nursing quality from positive aspects. As a result, we focused on patient-centeredness, which is one aspect of healthcare quality. The ability of patients to enjoy patient-centered medical care is an important issue for nursing. In order to measure patient-centeredness, the concept of patient experience was introduced. Patient experience is the patient's own assessment of whether or not they have had a desirable experience while receiving medical care. In this research project, we developed a web-based survey system to measure patient experience, and implemented and evaluated the system in a clinical setting.

    This study aimed to develop a scale to measure the professional attitude of nurses in Japan and to clarify predictors of the professional attitude of nurses. We conducted a conceptual analysis of the nurses' sense of profession and identified that the nurses' sense of profession consists of 12 subordinate concepts. Based on this concept, we created an item pool and developed a scale (nurse professional attitude scale) consisting of 38 items 8 subordinate factors through two questionnaire surveys. Factors predicting the nurses' sense of profession are currently being analyzed.
