    Poly(2-oxazoline) (POx) polymers are hydrophilic and biocompatible materials with tunable physicochemical properties for biomedical use. Postoperative hypotony, which currently lacks approved pharmacological or device-based treatment, requires minimally invasive and controllable methods for temporary intraocular pressure restoration. Herein, we present a modular strategy to control POx aggregation via thermally triggered co-assembly with thermoswitchable poly(2-n-propyl-2-oxazoline) (PnPrOx) and hydrophilic PEG-modified PnPrOx (PnPrOx-PEG). These mixtures remain dissolved at low temperatures but aggregate at physiological temperatures because of the thermo-switchable behavior of the PnPrOx segments, with PEG content modulating particle size, dispersibility, and fusion. Based on in vivo compositional screening in rabbits, we highlight a 98:2 (w/w) PnPrOx:PnPrOx-PEG formulation (POx2P) that induces transient intraocular pressure elevation without persistent turbidity or deposition. In a trabeculectomy-induced hypotony model, POx2P restored intraocular pressure without adverse effects. Microscopy and in vivo imaging suggest that this formulation forms flow-responsive aggregates that transiently occlude aqueous outflow pathways. We established a compositionally tunable and injectable POx platform for in situ aggregation and pressure modulation, offering a clinically relevant approach for localised intraocular therapy.

    BACKGROUND: Developing quantitative indicators of daily life that detect subtle cognitive decline is an important challenge in today's growing population of older adults worldwide. In this study, we aimed to explore novel candidates for indicators associated with subtle abnormalities in brief dementia screening test. METHODS: In this multifaceted cross-sectional study, data were collected from 35 older adults who had not been certified for long-term care or diagnosed with cognitive impairment using questionnaires, the Mini-Mental State Examination (MMSE), body composition measurements, sleep apnoea testing, activity monitoring, motor function assessments, blood tests, and nutrient analyses. Of the 89 factors examined in this study, several less invasive indicators for cognitive impairment were identified using Spearman's correlation analysis, two-group comparison, and multiple linear regression model analysis. RESULTS: An increased minimum pulse rate during sleep was found to be the most important non-invasive marker associated with both the correlation with MMSE scores and the risk classification of cognitive impairment (mild cognitive impairment or dementia). CONCLUSIONS: The minimum pulse rate during sleep is easily detectable owing to recent developments and widespread use of wearable devices. The results of this study will not only provide opportunities for older adults and their families to recognise slight cognitive decline but will also contribute to the early detection of cognitive decline, as a guide for the development of research evaluating the detection sensitivity and specificity of these indicators.

    PURPOSE: The retinal pigment epithelium (RPE) is crucial for photoreceptor function, and its dysfunction is associated with several retinal degenerative diseases. This study examines how different sugars in preservation media affect the viability of RPE cells, highlighting the need for effective storage solutions for cell transplantation. METHODS: Human RPE cells were cultured and suspended in modified media with various sugars. The survival rate was measured for cells cultured under adhesion for 3 weeks and for those stored at 37°C for 24 hours. Metabolism was evaluated using liquid chromatography, whereas the responses to hypoxia were assessed with specific markers. RESULTS: Fructose-containing media significantly enhanced RPE cell survival, even under hypoxic conditions. In adherent cultures, fructose showed higher survival rates compared to galactose, which had a notably low survival rate. Chromatography results indicated that fructose played a role in non-anaerobic metabolism, helping to explain its effectiveness. In suspension, fructose maintained higher cell viability than glucose and suppressed hypoxia markers, suggesting increased resistance to hypoxic stress. CONCLUSIONS: The study highlights the critical role of sugar composition in preservation media on RPE cell survival, positioning fructose as a potential enhancer. Its antioxidative properties suggest fructose could be effective in suspension preservation. These findings indicate that fructose-containing media are promising for preserving RPE cells and could have broader applications in preserving various cell types and tissues. TRANSLATIONAL RELEVANCE: The results of this study may allow for longer-term storage of RPE cells, potentially increasing the versatility of cell transplantation therapy.

    Retinal pigment epithelium (RPE) cells show heterogeneous levels of pigmentation when cultured in vitro. To know whether their color in appearance is correlated with the function of the RPE, we analyzed the color intensities of human-induced pluripotent stem cell-derived RPE cells (iPSC-RPE) together with the gene expression profile at the single-cell level. For this purpose, we utilized our recent invention, Automated Live imaging and cell Picking System (ALPS), which enabled photographing each cell before RNA-sequencing analysis to profile the gene expression of each cell. While our iPSC-RPE were categorized into four clusters by gene expression, the color intensity of iPSC-RPE did not project any specific gene expression profiles. We reasoned this by less correlation between the actual color and the gene expressions that directly define the level of pigmentation, from which we hypothesized the color of RPE cells may be a temporal condition not strongly indicating the functional characteristics of the RPE.

    Body mass index (BMI) is routinely used to ascertain health status, including activities of daily living (ADLs); however, the associations of ADLs with height and weight in older adults have not been elucidated. Therefore, we cross-sectionally investigated the correlations between ADLs and height, weight, and BMI in 155 participants aged 82 to 103 years and characterized the naïve Bayesian prediction for ADLs. Activities of daily living showed a significant negative correlation with height and weight and a positive correlation with age. In males, a shorter height was associated with an increased risk of falling and disability in phone calling independently, and losing weight was associated with an increased risk of disability in going out. Combining age, weight, and height improved the area under the receiver operating characteristic curve in the prediction of disability in going out and phone calling independently in males. Therefore, height and weight, not BMI, are potential predictors of ADL decline.

    INTRODUCTION: The retinal pigment epithelium (RPE) plays essential roles in maintaining retinal functions as well as choroidal capillaries and can lead to visual disorders if dysfunctional. Transplantation of human-induced pluripotent stem cell-derived RPE (hiPSC-RPE) is a promising therapy for such RPE impaired conditions including age-related macular degeneration. The challenge with cell suspension transplantation is targeted delivery of graft cells and undesired cell reflux. Gelatin hydrolysate, a soluble variant with specific molecular weight distribution, is examined in this study for its potential use in hiPSC-RPE suspension transplantation, particularly in reducing cell reflux and enhancing RPE engraftment. METHODS: A retinal bleb model was created using polydimethylsiloxane (PDMS) soft lithography to quantify cellular reflux. We examined the effects of gelatin hydrolysate on the hiPSC-RPE of various aspects of cell behavior and performance such as cell viability, hypoxia reaction, morphology, induction of inflammation and immune responses. RESULTS: Gelatin hydrolysate at 5 % concentration effectively mitigated cell reflux in vitro mimic, improved cell viability, reduced cell aggregation, and had an inhibitory effect on hypoxic reactions due to cell deposition with hiPSC-RPE. Additionally, gelatin hydrolysate did not affect cell adhesion and morphology, and decreased the expression of major histocompatibility complex class II molecules, which suggests reduced immunogenicity of hiPSC-RPE. CONCLUSION: Gelatin hydrolysate is considered a valuable and useful candidate for future regenerative therapies in hiPSC-RPE suspension transplantation.

    Obesity and ageing are the most important risk factors for sleep apnoea syndrome (SAS); however, the role of body mass index (BMI) on sleep status in healthy older adults is unclear. To explore sleep parameters according to BMI among active older adults, we cross-sectionally examined the relationship between sleep-related parameters and BMI in 32 Japanese adults aged from 83 to 95 years without long-term care who were unaware of having SAS. Correlation and linear regression analyses were performed. Moderate or severe SAS prevalence was high in both those with low (68.8%) and high (68.8%) BMI. A higher increase in apnoea-hypopnoea index (AHI) was negatively correlated with sleep depth in the high-BMI group. In the low-BMI group, the number of awakenings and age were positively correlated with AHI. Older adults may have SAS regardless of their BMI, and the sleep status of patients with SAS may vary by BMI.

    Study Objectives: Aging is a risk factor for sleep apnoea syndrome (SAS), which is associated with lower quality of life and sudden mortality. However, SAS is often overlooked in older adults without suspicions. Therefore, this study aimed to evaluate SAS incidence and 48 other general factors in older adults. Methods: This cross-sectional study included all non-caregiver-certified, healthy individuals (N = 32) who survived during the long-term cohort study and agreed to participate in apnoea-hypopnoea index (AHI) measurement (aged 83–95 years). AHI and 48 other general factors were evaluated, and simple linear regression analysis was used to identify potential AHI-related factors. Stepwise evaluation was further performed using multiple linear regression analyses. Results: Although no individuals were previously diagnosed with SAS, 30 (93.75%) participants had some degree of SAS (AHI > 5/h), and 22 (68.75%) had severe or moderate SAS (AHI > 15/h). Compared with typical single risk factors represented by body mass index, combining daily steps and other factors improved the fit to the multiple linear regression. Combining daily steps and body mass index improved the fit for males and combining daily steps and red blood cell count improved the fit for females. Conclusion: SAS was highly prevalent in unaware healthy Japanese older adults; combinations of daily steps and body mass index, and daily steps and red blood cell count may predict AHI in such individuals without the need for a specific AHI test.

    Abstract Several clinical studies have been conducted into the practicality and safety of regenerative therapy using hESC/iPSC-retinal pigment epithelium (RPE) as a treatment for the diseases including age-related macular degeneration. These studies used either suspensions of RPE cells or an RPE cell sheet. The cells can be injected using a minimally invasive procedure but the delivery of an intended number of cells at an exact target location is difficult; cell sheets take a longer time to prepare, and the surgical procedure is invasive but can be placed at the target area. In the research reported here, we combined the advantages of the two approaches by producing a quickly formed hiPSC-RPE strip in as short as 2 days. The strip readily expanded into a monolayer sheet on the plate, and after transplantation in nude rats, it showed a potency to partly expand with the correct apical/basal polarity in vivo, although limited in expansion area in the presence of healthy host RPE. The strip could be injected into a target area in animal eyes using a 24G canula tip.

    Retinal pigment epithelium (RPE) cells is the outermost layer of the retina and RPE dysfunction is a key factor in the disease pathogenesis of age-related macular degeneration (AMD). Transplantation therapy using induced pluripotent stem cell (iPSC)-derived RPEs has recently received much attention as a treatment for AMD. Preserving these cells under the best possible conditions is important, and preservation methods using Y-27632 have been reported. Rho-associated coiled-coil containing kinase (ROCK) inhibitors are known to inhibit cell death, emerging as important drug candidates for stem cell differentiation and regenerative medicine. However, it has recently been shown that ROCK inhibitors may have a vasodilatory effect on human retinal arterioles, a side effect that should ideally be avoided in RPE transplantation. Although ROCK inhibitors hold great potential, optimizing efficacy while minimizing adverse reactions is critical for translation into a clinical treatment. We examined the effect of transient exposure of RPE cells to ROCK inhibitor Y-27632 to determine whether the extracellular presence of the drug is necessary for ongoing Rho/ROCK downregulation. Human RPE cells were subcultured as a suspension for 4 h in drug-free medium following exposure to Y-27632 for 2 h. A Y-27632 concentration of >10 μM improved cell survival beyond 4 h and cell proliferation in recovery culture medium. ROCK2 expression levels were specifically downregulated by Y-27632 in the Rho/ROCK signaling pathway. In conclusion, we demonstrated that the effect of Y-27632 is not dependent on its extracellular availability and can last beyond the 2 h of exposure. The lasting Rho/ROCK signaling pathway downregulation by Y-27632 suggests that RPE cell transplantation with ROCK inhibitor-free media is possible, which can minimize side effects to host tissue and have wider implications for transplantation methods requiring ROCK inhibition.

    The ability to move cells and tissues from bench to bedside is an essential aspect of regenerative medicine. In this study, we propose a simple and static shipping system to deliver tissue-engineered cell sheets. Notably, this system is electronic-device-free and simplified to minimize the number of packing and opening steps involved. Shipping conditions were optimized, and application and verification of the system were performed using human iPS cell-derived or fetal retinal pigment epithelium (RPE) cell sheets. The temperature of the compartments within the insulated container was stable at various conditions, and filling up the cell vessel with medium effectively prevented turbulence-induced mechanical damage to the RPE cell sheets. Furthermore, no abnormal changes were observed in RPE morphology, transepithelial electrical resistance, or mRNA expression after transit by train and car. Taken together, our simple shipping system has the potential to minimize the costs and human error associated with bench to bedside tissue transfer. This specially designed regenerative tissue shipping system, validated for use in this field, can be used without any special training. This study provides a procedure for easily sharing engineered tissues with the goal of promoting collaboration between laboratories and hospitals and enhancing patient care.

    Human induced pluripotent stem cell (hiPSC)-derived retinal pigment epithelium (hiPSC-RPE) cells suspension have the potential for regenerative treatment. However, practical regenerative applications with hiPSC-RPE cells require the development of simple and cost-effective non-freezing preservation methods. We investigated the effect of non-freezing temperatures on suspended hiPSC-RPE cells in various conditions and analysed mechanisms of cell death, anoikis, Rho GTPases, hypoxia, microtubule destruction, and cell metabolism. Cells stored at 37 °C had the lowest viability due to hypoxia from high cell metabolism and cell deposits, and cells preserved at 4 °C were damaged via microtubule fragility. Cell suspensions at 16 °C were optimal with drastically reduced apoptosis and negligible necrosis. Moreover, surviving cells proliferated and secreted key proteins normally, compared to cells without preservation. hiPSC-RPE cell suspensions were optimally preserved at 16 °C. Temperatures above or below the optimal temperature decreased cell viability significantly yet differentially by mechanisms of cell death, cellular metabolism, microtubule destruction, and oxygen tension, all relevant to cell conditions. Surviving cells are expected to function as grafts where high cell death is often reported. This study provides new insight into various non-freezing temperature effects on hiPSC-RPE cells that are highly relevant to clinical applications and may improve cooperation between laboratories and hospitals.

    PURPOSE: To investigate the role of intravitreal injection of triamcinolone acetonide (IVTA) in preventing photoreceptor apoptosis in eyes with rhegmatogenous retinal detachment (RRD) by measuring cytokine levels in the aqueous humor before and after IVTA. DESIGN: Prospective, nonrandomized, interventional case series. METHODS: setting: Institutional. patients: Nineteen eyes of 19 consecutive patients with RRD. intervention: All 19 eyes underwent IVTA 1 day before 25-gauge vitrectomy. Seventeen eyes free of retinal vascular disease served as controls. main outcome measure: Both baseline and 1 day post-IVTA measurements were made of the relative concentrations of 15 soluble factors (3 cytokines, 7 chemokines, and 5 growth factors). The associations with clinical findings, including macular status, were then analyzed. RESULTS: Elevated monocyte chemotactic protein 1 (MCP-1), macrophage inflammatory protein 1β (MIP-1β), and interferon γ-induced protein 10 (IP-10) in eyes with RRD were significantly reduced after IVTA. MCP-1 levels were significantly correlated with MIP-1β and IP-10 before and after IVTA. The decreases in MCP-1, MIP-1β, and IP-10 were also closely correlated to each other. Both before and after IVTA, MCP-1 was higher in eyes with macula-off RRD than in eyes with macula-on RRD. CONCLUSIONS: IVTA suppressed elevated levels of intraocular MCP-1, MIP-1β, and IP-10 in eyes with RRD. The decrease in the aqueous levels of each of these factors was significantly correlated with the others. In addition to MCP-1, MIP-1β and IP-10 might potentially be additional target molecules for RRD therapy.

    There is a big need for estimating internal properties of eyeball, since they shoud be closely related to eye diseases, such as detached retina. Based on this background, this paper discusses a new bio-marker expressing the mechanical characteristics of internal eyeball. For this purpose, we focus on subjects having vitreum surgery in which vitreum of subject is replaced with succedaneum. With the assistance of high speed camera, the eye deformations before and after operation are captured during air-puff impartment. Through the analysis, we found the difference between the behaviors before and after operation, especially the shape of eye ball immediately after the tip of cornea recovers. We discuss why such different behaviors happen and how we can explain the physics.

    1. WTマウスと老齢サルコペニアモデルBrd4ヘテロ欠損マウスを比較したところ、対照食摂取群において、Brd4ヘテロ欠損マウスの体重、握力、臓器重量(肝臓、副睾丸脂肪、腓腹筋、ヒラメ筋、前脛骨筋)が顕著に低く、サルコペニアマウスとして使用できる可能性が示唆された。 2. 昨年度まで、カプリル酸が多い油脂[カプリル酸(C8):カプリン酸(C10)=75%:25%]を使用していたが、今年度は、カプリン酸が多い油脂[カプリル酸(C8):カプリン酸(C10)=25%:75%]を投与する実験を行った。具体的には、WT マウスとBrd4ヘテロ欠損マウスに対照食(長鎖飽和脂肪が多いラードを油脂源として使用)およびカプリン酸が多い油脂[カプリル酸(C8):カプリン酸(C10)=25%:75%]に置き換えた食餌を5ヶ月のあいだ投与した。その結果、中鎖脂肪が多い食餌を摂取したBrd4ヘテロ欠損マウスでは体重や副睾丸脂肪は低下傾向を示したが、腓腹筋、ヒラメ筋、前脛骨筋、長指伸筋の重量は増大傾向を示した。さらにBrd4ヘテロ欠損マウスへの中鎖脂肪の投与により握力が顕著に上昇した。

    2020年度は、前年度mRNA導入の効果検証系として選択したマウス視神経挫滅モデルへの神経保護物質の眼内注射実験系の検証を継続した。まず正常コントロールとして、細胞死抑制効果が報告されているROCK inhibitorまたは生理食塩水を視神経挫滅後のマウス眼内に注射した。網膜神経節細胞(RGC)数の評価方法として、処置5日後に摘出した網膜を展開して、代表的なRGCマーカーであるBrn3aとRbpmsを免疫染色した。未処置ガンに対する傾向陽性細胞の割合は、Brn3a染色では、生理食塩水で54.3%であったのに対して、ROCK inhibitor 注射眼が66.5%、Rbpms染色では、生理食塩水で48.5%であったのに対して、ROCK inhibitor 注射眼が60.3%と、双方共にROCK inhibitor注射眼でRGC保護効果が認められ、神経保護効果を評価できることを確認した。しかしながら、RGCに対する神経保護効果が報告されているBdnfのmRNAを内包したナノミセルを上記の視神経挫滅モデルに注射したが神経保護効果は認められなかった。そこでナノミセルを構成する生体親和性ポリマーを新たに検討して従来と異なるものを複数用意した。また、培養系での検証への移行の可能性を検証するために、網膜色素上皮細胞の細胞死保護効果検証系のFeasibilityを検証した。さらに。この生体親和性ポリマーを活用した新しい眼科治療法の提案をmRNAの導入に囚われずに検討して、眼圧制御を効果的に実現する新しいコンセプトの試薬の提案に繋げて特許出願や新しい予算申請等に繋げた。

    In this study, we examined two new detection methods: digital nucleic acid quantification method with reverse transcription and digital nucleic acid quantification method of identifying and quantifying nucleic acid amplified by melting curve analysis. The isothermal amplification method can amplify a nucleic acid only by maintaining it at around 70 ° C, but there is a problem that it can not be determined unless there is a 10-fold concentration difference, but digitization of the isothermal amplification kit with reverse transcription It has been found that it becomes possible to detect even a concentration difference of about 2 times. In addition, in order to detect genomic mutations used for quality control of cells utilized for regenerative medicine of retina, newly designed primers for isothermal amplification, digital nucleic acid amplification, and biotyped and mutated nucleic acid by melting curve analysis. It turned out that it can be identified and quantified.

    Glaucoma is a chronic ocular disease that is often influenced by high intraocular pressure, resulting in irreversible loss of visual field. Visual field loss caused by glaucoma is progressive, thus, we should diagnose glaucoma at its early stages and start the treatment if indicated. The goal of the present study is to identify biomarkers for glaucoma, through investigating the translational profiles of RNA derived from blood. Thirty-two patients with normal tension glaucoma and 32 controls with ocular diseases which have little association with genetic factors were recruited in the present study. RNA was extracted from the peripheral blood. Activation of promoters of all the genes were comprehensively quantitated using cap analysis gene expression (CAGE) technique. We selected the candidate genes as biomarker according to the result of the differential analysis of the two groups. The promoters identified hold promise for their clinical application as biomarkers for glaucoma.

    We found that at 0.025-ml/min, IOP stayed below 40 mmHg even after 3 min. Moreover, we found that a 0.025 ml/min ADPJ stream was stable at a pulse frequency of 10Hz, and that at this flow rate/frequency the ADPJ pressure was closely correlated with the applied voltage. The minimum voltage and distance to achieve intravenous displacement of the blood column (DBC) without causing retinal tearing or hemorrhage were 40 V and 0.5 mm, respectively. Furthermore, at this voltage (40V), consecutive OCT measurements revealed that this inner retinal swelling resolved within 1 week. Histological examination of the enucleated eyes 7 days after ADPJ massage showed that retinal areas near the massaged vessels were similar to areas near untreated vessels. With an appropriate flow rate and surgical time, ADPJ successfully induced massage of the retinal vessels and intravenous DBC while maintaining safe IOP and not causing retinal complications.

    We extracted a trace amount of RNA from anterior aqueous humor from patients with ophthalmopathy of just a few 100 μL and tried comprehensive RNA analysis using next generation sequencer etc. About 360 kinds of microRNAs were successfully detected from one clinical specimen, and about one third of them were commonly detected in eight cataract specimens. Of these, 75% of the top 40 in expression levels were consistent with other studies reported during the study period. As a study that tried comprehensive analysis individually without mixing samples, our microarray analysis reported at the beginning of the research period is the first time, and the analysis scale by the next generation sequencer is the largest. In addition, as a surrounding study, we have examined microRNA analysis method and pathology model evaluation method and obtained basic knowledge.

    We made alternative corneal stroma having strenght and tranparency the same as normal corneal stroma with transparent human amniotic membrane laminates. When these laminates were tranplanted to rabbit cornea, these membranes survived for a long time. These laminates were useful for filling scleral defect and covering glaucoma shunt device. We manipulated scleral contact lens with these laminates. When this contact lens was worn on chemical injured rabbit cornea, inflammation on the ocular surface was reduced significantly. This contact lens might be effective for severe ocular surface inflammation.

    The transplantation of dental pulp-derived cells to the eye was explored for the possibility of the novel neuroprotection and regenerative treatments to the critical retinal diseases that results in vision loss. Firstly, the isolated dental pulp-derived cells were directly injected into normal rat vitreous, and it was confirmed that some of the injected cells adhered to the retina and formed the neuronal process. Furthermore, several cell manipulation methods were examined to enrich stem cell population and up-regulate neuron-related molecular expression. Specifically, (1) gene expression of nerve and stem cell related molecule were activates by sphere-forming floating culture, (2) sphere-forming cells can be enriched by cell sorting of CD105-positive dental pulp-derived cells, (3) the expression of an axonal regeneration related molecule was promoted by applying hypoxic culture to the sphere-forming culture.

    For the ophthalmic application of optically-cleared tissues, their modification and animal experiments were investigated in this study. This study focused on the amnion that were widely used in the ophthalmology field, and examined their biocompatibility in vivo and also improvement in the tolerance to a suture and transparency. Firstly, it was confirmed that no inadequate biological responses to the optically-cleared amniotic membrane were observed during the transplanted into rabbit corneal stroma. Next, further functional refinements of the optically-cleared amnion, and additional tissue optical clearing were also induced. Further optical-clearing was effective to improve the transparency of the refinement processes. Furthermore, the improvements of mechanical properties were confirmed that it was possible to improve the mechanical property to about 10 times of the original amnion, and that the corneal transplantation, which requires a tight suture, was also possible.
