    The 2025 Myanmar earthquake caused extensive destruction. Beyond 3,800 direct disaster-related deaths (direct DRDs) and 5,100 injuries, indirect disaster-related deaths (indirect DRDs) remain a persistent concern. Indirect DRDs are deaths caused by secondary health effects after a disaster, rather than by the direct physical impact of the hazard itself. They may arise through disrupted healthcare delivery, deteriorating living and evacuation environments, interruptions to chronic-disease management, and psychosocial stress. Evidence from Japan, including the 2011 Great East Japan Earthquake and the 2024 Noto Peninsula Earthquake, shows that prolonged displacement and repeated disruptions in health and social systems can sustain mortality risk over time. Based on these insights, this Perspective synthesizes publicly available United Nations and humanitarian reports to identify risks of indirect DRDs in Myanmar. Months after the earthquake, many survivors remain in fragile shelters with degraded water, sanitation, and hygiene conditions. Health facilities were damaged or closed, healthcare access remains constrained, and vulnerable groups—including children, pregnant women, older adults, persons with disabilities, and patients with chronic diseases—face sustained risks. These conditions increase the likelihood of infectious disease outbreaks, chronic disease decompensation, heat illness, and psychosocial distress. Priorities include continuity of essential services, access to noncommunicable disease medicines, maternal–child health and psychosocial support, improved shelter and WASH conditions, standardized documentation of disaster relevance in medical records and death certification, and sustained, coordinated recovery. Framing indirect DRDs as preventable and measurable can strengthen protection of vulnerable groups, resilience, accountability, and dignity.

    Introduction: Disaster risk reduction significantly reduced direct disaster-related deaths, but indirect deaths are becoming a significant issue after recent disasters in Japan, an aging society. Some of these deaths were officially certified for condolence grants to bereaved families who applied, which may hinder the accuracy of mortality statistics. This study aimed to investigate the extent to which official medical certificates documenting disaster relevance were included in the municipal documents for indirect disaster-related deaths. Methods: This study conducted a retrospective cross-sectional analysis using anonymized data obtained from municipalities through information disclosure (IRB approval: 2023-1-489). The study assessed 755 certified indirect disaster-related deaths after the 2011 Great East Japan Earthquake in the Miyagi Prefecture, Japan, to determine whether the medical documents in the certification included references to disaster relevance. Results: Of the 755 cases, 74 (9.8%) death certificates and 145 (19.2%) medical documents mentioned disaster relevance. In 536 (71.0%) cases, the disaster relevance was only mentioned in the self-reported documents submitted by bereaved families. The median [interquartile range] time from the disaster onset to disaster-related death was 21 days [7–52 days]. The deaths occurred 61.1% within 30 days, 34.0% during 31-180 days, and 4.5% after 181 days. The mean age of the deceased was 79.7 years, with no gender difference. The predominant etiologies of these deaths included circulatory (32.7%) and respiratory (27.7%) diseases. Conclusion: Only 30% of official medical documents mentioned disaster relevance in 755 certified cases as indirect disaster-related deaths, suggesting the lack of systematic registration of disaster relevance. Especially, middle and long-term disaster-related deaths can be marginalized. After establishing social consensus, changing the medical practice and municipal handling of death certification for disaster-affected people is essential for accurate disaster mortality statistics.

    Disaster-related deaths can be classified as direct and indirect disaster-related deaths that are becoming more predominant in Japan. To reduce indirect disaster-related deaths, accurate statistics based on medical documentation are essential. However, the definition and reporting of such deaths in Japan present systemic challenges. These deaths are certified and recorded by municipalities at the request of bereaved families seeking condolence grants. This cross-sectional study assessed 755 certified indirect disaster-related deaths after the 2011 Great East Japan Earthquake in the Miyagi Prefecture, Japan, to determine whether medical documents mentioned disaster relevance. Of the 755 cases, 74 (9.8%) death certificates and 145 (19.2%) medical documents described the relevance of disasters. Relevance mentioned only in the self-reported documentation from bereaved families was 536 (71.0%). The median [interquartile range] time from the disaster onset to disaster-related deaths was 21 [7-52] days. The mean age was 79.7-years-old, and 346 (46.7%) of the victims were female. The predominant etiologies of these deaths included circulatory (32.7%) and respiratory (27.7%) diseases. The time periods of the deaths varied among the municipalities, suggesting a lack of consensus among the review committees. Establishing a medical and social consensus for indirect disaster-related deaths and recording them on death certificates is crucial for accurate disaster death data and social resilience.

    Recent trends of disaster present that disasters are more uncertain and complex with unpredictable cascading effects owing to global environmental and climate change. As one of the significant phenomena, natural hazards triggering technological accidents, so-called Natech, which cause acute and prolonged terrifying consequences, have also been increased. Considering low probability but high consequences, Natech accidents often exceed the capacity of disaster preparedness and response of local government and professional responders. As a result, the need to better manage Natech risks has become a challenge in disaster risk management among all stakeholders, including government, disaster and safety experts, industry, and citizens. Although there have been numerous efforts undertaken by international organizations and individual countries to reduce Natech risks, there are still not sufficient reflections to reduce the risks of natural hazard triggering technological disasters in South Korea. Thus, this study highlights lessons from Natech risk reduction practices undertaken by individual countries and recent minor Natech cases to fullfill gaps in effective risk management considering for cascading effects.
