    OBJECTIVE: To characterise oral lichen planus-associated oral microbiome alterations separately in saliva, supragingival dental plaque, and tongue coating. METHODS: In this cross-sectional study, samples from patients with oral lichen planus and healthy controls were analysed using bacterial amplicon sequencing. Microbial diversity, taxonomic composition, and genus-level differential abundance were compared within each sample type. RESULTS: Analyses included 47 control and 35 oral lichen planus saliva samples, 10 and 30 dental plaque samples, and 47 and 32 tongue coating samples, respectively. Significant alpha-diversity differences were observed for all four indices in saliva, three in tongue coating, and one in dental plaque. Beta-diversity differed between groups within all three sample types. Unweighted phylogenetic and presence-absence distances most consistently detected group differences, whereas abundance-weighted phylogenetic distances were not significant after correction. Unweighted phylogenetic differences remained significant after adjustment for Plaque Control Record. Descriptive taxonomic and differential-abundance patterns varied among sample types, with saliva yielding the largest number of significant taxa. CONCLUSION: Oral lichen planus was associated with microbiome alterations in all three sample types, mainly reflecting differences in microbial community membership. These sample types may provide complementary information for characterising oral lichen planus-associated dysbiosis.

    OBJECTIVE: To clarify the relationship between oral squamous cell carcinoma (OSCC), potentially malignant oral disease (OPMD), and bacterial flora using metagenomic analysis. METHODS: This cross-sectional observational study included 50 patients in the control group and 77 patients with OPMDs, 41 with early OSCCs, and 20 with advanced OSCCs. Patient saliva samples were subjected to high-throughput sequencing of 16S rRNA gene amplicons to evaluate the composition and diversity of the oral microbiome. RESULTS: No significant differences were observed in patient backgrounds, other than sex. Patients with advanced OSCCs had greater oral bacterial diversity than those with early OSCC or OPMD. The advanced OSCC group formed a distinct cluster separate from the other groups. Sixteen and 275 species were identified at the phylum and genus levels, respectively. Compared with the control group, Actinomycetia and Streptococcus were significantly elevated in the early OSCC and OPMD groups. Peptostreptococcus and Fusobacterium were significantly higher in the advanced OSCC group than in the control, OPMD, and early OSCC groups. CONCLUSIONS: The composition and diversity of oral microbiota may be associated with OPMD development and progression to OSCC. Consequently, the salivary microbiome may serve as a biomarker for oral cancer and help predict cancer progression.

    BACKGROUND: Although studies have demonstrated a relationship between pathogenic microorganisms and oral cancer, no study has demonstrated a relationship between changes in bacterial flora and oral squamous cell carcinoma (OSCC). Therefore, we investigated the association between oral microbiota and oral squamous cell carcinoma using metagenomic analysis. METHODS: Saliva samples from 64 patients with OSCC and 50 healthy controls who visited the Department of Oral Surgery, Tohoku University Hospital, were collected, and bacterial genomic DNA was extracted using polymerase chain reaction amplification. Single-end sequencing was performed using the Illumina MiSeq platform, and sequence data were analyzed using the Quantitative Insights Into Microbial Ecology 2 platform. The Steel-Dwass test was used for between-group comparisons, and Analysis of Compositions of Microbiomes with Bias Correction was used to detect significant differences in microbiome composition. RESULTS: Significant differences were observed in alpha-diversity indices of bacterial flora (richness, Faith- phylogenetic diversity, Shannon index) in the OSCC group compared to those in the control group. Among the OSCC group, patients with larger tumor diameters and lymph node metastases (T3/T4, N1 or greater) formed independent clusters in the beta diversity analysis of the bacterial flora. Bacteria of the Actinomycetia phylum, such as Actinomyces and Rothia, were significantly reduced in patients with higher stage and pathological grade. Conversely, bacteria of the phylum Spirochaetia and Proteobacteria, particularly those of the genus Treponema, were significantly elevated in advanced cancer cases. CONCLUSIONS: Our results suggest that changes in the oral microbiota may play a role in OSCC development and progression.

    BACKGROUND: We aimed to construct an artificial intelligence-based model for detecting oral cancer and dysplastic leukoplakia using oral cavity images captured with a single-lens reflex camera. SUBJECTS AND METHODS: We used 1043 images of lesions from 424 patients with oral squamous cell carcinoma (OSCC), leukoplakia, and other oral mucosal diseases. An object detection model was constructed using a Single Shot Multibox Detector to detect oral diseases and their locations using images. The model was trained using 523 images of oral cancer, and its performance was evaluated using images of oral cancer (n = 66), leukoplakia (n = 49), and other oral diseases (n = 405). RESULTS: For the detection of only OSCC versus OSCC and leukoplakia, the model demonstrated a sensitivity of 93.9% versus 83.7%, a negative predictive value of 98.8% versus 94.5%, and a specificity of 81.2% versus 81.2%. CONCLUSIONS: Our proposed model is a potential diagnostic tool for oral diseases.

    <p>Static bone cavities were first reported by Stafne in 1942 and are considered localized bone recesses in the lingual cortical bone of the mandible. This diseased tissue has been reported as salivary gland (mainly submandibular gland) tissue, but its etiology remains unclear; several mechanisms have been proposed, but a consensus has yet to be reached.</p><p> In addition, since it is rare to show enlargement in size, and this disease remains clinically asymptomatic, long-term follow-up observations are often made while evaluating images such as panoramic radiographs and CT without aggressive surgical invasion.</p><p> We studied a static bone cavity that increased in size from a red bean to the tip of the thumb at 4 years 3 months. We performed minimally invasive surgical removal by an endoscopic intraoral approach for definite diagnosis and early recovery after surgery. The pathological diagnosis was adipose tissue, and the patient has no symptoms as of 1 year 7 months after the operation.</p>

    We report a case of migration of a broken carbide bur into the inferior border of the mandible during surgical extraction of a third molar.<br>The patient was a 36-year-old woman who was referred to our outpatient clinic for the removal of a broken piece of carbide bur at the inferior border of her mandible. Prior to this, she had been to a private clinic for the extraction of an impacted third molar, during which the carbide bur for dividing the tooth crown broke two times. The surgeon tried to remove the broken bur but could not do so completely, therefore the patient was referred to our department 6 days after the tooth extraction.<br>X-ray examination revealed a broken bur lodged at the inferior border of the patient's mandible. Nevertheless, at her initial visit to our outpatient department, we found the presence of inflammation, so antibiotics were administered until the inflammation disappeared. 21 days after tooth extraction, we finally removed the piece of broken bur at the inferior border of the mandible under general anesthesia.
