    PURPOSE: To identify secular trends in the use of radiotherapy prostheses in the Clinic for Maxillofacial Prosthetics at Tokyo Medical and Dental University Hospital and to ascertain patient characteristics and future needs. METHODS: The study included all patients who visited the clinic between January 1, 2000, and December 31, 2019. Data on numbers of cases, gender and age, numbers and types of radiotherapy prostheses, primary cancer site, and time required for prosthesis fabrication during the study period were collected from medical records and retrospectively analyzed. RESULTS: During the 20-year study period, 1,066 prostheses were fabricated for 1,135 cases. In most years, at least 60% of cases were male. The mean age increased from 60.6 to 67.1 years during the study period, and the number of cases older than 70 years increased over time. The most common cancer site was the tongue, and the most common type of radiotherapy prosthesis was a spacer. The mean time for spacer fabrication was 11 days. CONCLUSION: During a recent 20-year period, mean age and the proportion of elderly patients requiring savers increased. When treating elderly patients, prosthodontists responsible for fabricating radiotherapy prostheses must collaborate effectively with radiation oncologists.

    Oral rehabilitation with prosthodontic treatment considerably influences the well-being and quality of life of patients after ablative oral tumor surgery. This study evaluated the effects of implant-supported prostheses (ISPs) on oral health-related quality of life (OHRQoL) and chewing ability in 10 patients who requested ISPs after mandibular oral tumor resection. OHRQoL was assessed using the Japanese version of the Oral Health Impact Profile (OHIP-49) before and one year after ISP placement. Chewing ability, including self-assessed masticatory ability and occlusal force, was examined at one year after ISP placement. The initial mean total OHIP-49 score of 65.3 ± 9.79 decreased to 46.0 ± 8.14 at one year after ISP placement. Mean OHIP-49 score decreased in all domains, whereas self-assessed masticatory ability increased within one year of ISP placement. There were no significant differences between prosthesis types with respect to the mean OHIP-49 score or self-assessed masticatory ability. In conclusion, ISP placement improves OHRQoL and the self-assessed masticatory ability. Moreover, the prosthesis type might not significantly affect OHRQoL.

  16. Evaluation of salivary cortisol levels in relation to dento-maxillary prosthesis adjustment Peer-reviewed

    PURPOSE: The purpose of this study was to investigate the influence of dento-maxillary prosthesis adjustment procedure on levels of salivary cortisol. METHODS: Nine participants (six men, three women, mean age 65.9 years) took part in this study. Saliva samples were collected before and after dento-maxillary prosthesis adjustment during the four different visits. Free cortisol levels were determined using a salivary cortisol immunoassay kit (expanded-range high-sensitivity salivary cortisol enzyme immunoassay kit, Salimetrics). Besides, original self-report sheets, a 35-item food intake questionnaire, the University of Washington Quality of Life (UW-QOL) questionnaire version 4, and the Geriatric Oral Health Assessment Index (GOHAI) questionnaire were also administered. The changes of salivary cortisol levels were analyzed using 2-level multilevel linear regression, with adjustment for age, sex, and time. Wilcoxon signed-rank test was used to compare scores of the food intake questionnaire, UW-QOL questionnaire, and GOHAI questionnaire. RESULTS: Salivary cortisol levels decreased significantly after carrying out the dento-maxillary prosthesis adjustment procedure. During the third adjustment, the salivary cortisol levels were significantly low. In addition, salivary cortisol levels of participants aged 70 years and over were significantly higher than other aged groups. The total scores for grade III-V of the food intake questionnaire increased significantly. Other questionnaires had a trend toward increasing scores, yet the differences were not significant. CONCLUSIONS: Within the limitations of this study, the results suggest that a reduction in symptoms of discomfort may have an influence on the decrease of salivary cortisol levels in dento-maxillary prosthesis wearers.

    The purposes of prosthodontic treatment in cleft lip and palate patients are to recover various functions, to prevent individual teeth and alveolar arches from relapsing, to equalize the occlusal force, and to close the fistula. The Department of Maxillofacial Prosthetics in Tokyo Medical and Dental University (TMDU) was established in 1979 as a Clinic for Stomatognathic Dysfunction and we have treated cleft lip and palate patients as a part of maxillofacial prosthetic treatment for 35 years since then. We report 4 cases that were treated in the early 1980s and were followed up with longevity for over 25 years.<br>Case 1: Left side of cleft lip and palate patient. An obturator and a hard resin facing fixed prosthesis were set as the abutment teeth were both maxillary central incisors, both maxillary canine teeth, and the left maxillary first premolar. The patient was followed for 31 years.<br>Case 2: Bilateral cleft lip and palate patient. An obturator, 2 plumpers and a hard resin facing fixed prosthesis were set as the abutment teeth were both maxillary central incisors, the right maxillary canine tooth, both maxillary first premolars and the left maxillary second premolar. The patient was followed for 29 years.<br>Case 3: Bilateral cleft lip and palate patient. An obturator, a plumper and a hard resin facing removable prosthesis by telescopic crowns were set as the abutment teeth were both maxillary central incisors, both maxillary canine teeth and the left maxillary first premolar. The patient was followed for 29 years.<br>Case 4: Left side of cleft lip and palate patient. An obturator, a plumper and a porcelain fused fixed prosthesis were set as the abutment teeth were the right maxillary central incisor, the right maxillary lateral incisor, the left maxillary canine tooth and the left maxillary first premolar. The patient was followed for 26 years.<br>In addition, individual teeth and alveolar arches were kept within normal limits in every case.

    PURPOSE: The purpose of this study was to use cortisol awakening response (CAR) to investigate the differences in daily life stress experienced by individuals wearing either a long-term (LT) or a short-term (ST) dento-maxillary prosthesis following head and neck cancer (HNC) resection. Also we used the University of Washington Quality of Life (UW-QOL) version 4 questionnaire to evaluate the differences in quality of life (QOL) scores between ST and LT wearers of a dento-maxillary prosthesis. METHODS: Salivary samples were collected from 11 LT and 10 ST prosthesis wearers on two consecutive days at two time points, immediately after waking up (T0) and 30min later (T30), by passive drool collection. Cortisol levels were measured using a high sensitivity salivary cortisol enzyme immunoassay kit (Salimetrics, LLC, State College, PA, USA) and CAR (the differences between the cortisol levels at T0 and T30) was compared between LT and ST prosthesis wearers. In addition, both the groups completed the UW-QOL questionnaire and the scores were compared. RESULTS: A significant difference was observed in CAR between the two groups.     無歯顎の上顎骨切除患者10名を被験者として、アラマニー分類I、II、IV、VIの4グループに分け、咀嚼混合能力試験を用いて咀嚼混合能力(MAI)の評価を行った。その結果、分類IIのMAIが高値で、分類IVにおいて、下顎残存歯数の減少に伴い、MAIが低下した。分類VIにおいて、腓骨再建された被験者10のMAIは、ほかの被験者より高値であった。分類VIの症例において、遊離腓骨皮弁による再建が咀嚼混合能力の向上につながることが示唆された。

    本研究は，腐敗・焼損・乾燥など死後の時間経過により高度に変化した顔貌から，直接生前の顔貌を復元するための客観的方法を提供しようとするものである。現在，死後変化が生じた遺体の顔貌からの生前の表情の推測には，警察の捜査官などが経験則によって似顔絵描写を行っている場合が多いため，このような経験則からなる顔貌再現の基本的データを客観的な数値として明らかにし，身元不明遺体の顔貌の速やかな復元に寄与できる。具体的には生体の頭蓋の骨形状（Ａ）とこれに対応する生体の軟部組織形状（Ｃ）の差分データ，また死後変化を遂げた遺体の頭蓋の骨形状（Ａ’）と死後軟部組織形状（Ｂ）の差分データを集積する。Ａ→Ｃの差分データに基づいてＡ’から遺体の生前軟部組織形状（Ｃ’）を推定し，これにより遺体の死後変化による顔貌（Ｂ）から生前の顔貌（Ｃ’）への関係式を導き出し，顔貌復元のためのデータとするものである。初年度は既にデータ取得済みのＡとＣとの関係性を精査すると共に，立体形状の解析手法である相同モデル化の工程およびプログラムのブラッシュアップを行った。相同モデル化とは，複数のサンプルの三次元的形状を，基準となるテンプレートデータの変形として扱って分析するもので，複雑な立体形状の定量的評価が容易となる。また新たに機械学習によるＡ→Ｃ，Ａ’→Ｂの推定の検討に着手した。機械学習を行うためには既知のサンプル数だけでは不足するため，まずはＡ→Ｂについて必要な追加データの記録を開始した。更にＡおよびＣの関係性に関するデータ解析結果について英文論文の投稿を行い，現在査読を受けている最中である。

    遺体の口腔内環境は乾燥、湿潤、高温、低温といった様々な状況により生前と大きく異なると考えられ、歯科所見採取に苦慮する場合がある。さらに近年の歯科材料および歯科治療技術の向上により歯冠色材料と天然歯との鑑別は益々困難となっており、歯科所見採取の際に注意が必要である。本研究では法医学的資料を想定し、特殊環境下に曝露された歯冠色材料の色調および蛍光変化について分光測色計、分光計を用いて客観的に評価し、歯科的個人識別に有用な指標を提示することを目的としている。 本年度は前年度実施した実験結果を踏まえ、歯冠色材料のうちコンポジットレジンおよびグラスアイオノマーセメントを用いて熱変性環境下における新たな実験を進める予定であった。具体的には前年度における実験にて400℃における熱性変化が著しく、多くのサンプルが黒褐色化し蛍光性の測定が不能となったことから、実際の現場においても活用が難しい条件とみなし、設定温度を200℃および300℃に限定することとした。加熱時間は前年度の実験と同様に10分、20分、30分に設定した。また前年度での実験において見られたコンポジットレジンおよびグラスアイオノマーセメントの蛍光変化の特徴より、蛍光の強さに影響を与える可能性が考えられる項目として①サンプルの厚み、②シェード、③歯科材料の性状（パテタイプ、フロータイプ）について検討することを目標としていた。さらに分光測色計を用いた色差計測も実施予定であった。 しかし研究者の妊娠・出産に伴う長期入院、産前産後休暇および育児休業により2021年4月30日から2022年4月14日までの期間中、研究活動を行う事は困難であった。 上記理由により本年度は実績がなく、研究の進捗状況としては遅れている。

    The purpose of this study was to investigate the phychological influence of dento-maxillary prosthesis adjustment using salivary cortisol as a stress marker. 10 subjects (7 male and 3 female) participated in this study. Saliva samples were collected before and after dento-maxillary prosthesis adjustment for different 4 days when they came to the clinic. All samples were collected at clinic of Maxillofacial Prosthetics in Tokyo Medical and Dental University (TMDU) Hospital Faculty of Dentistry. Results showed that salivary cortisol levels decreased significantly after dento-maxillary prosthesis adjustment. These results suggested that dento-maxillary prosthesis adjustment improved the discomfort symptoms of the prosthesis and influenced on the decrease of patient’s phychological stress.
