Despite established links between masticatory dysfunction and cognitive impairment, cognitive function in dentofacial deformity patients remains unexplored. This study represents the first comprehensive cognitive assessment in mandibular prognathism (MP) patients. Brain blood flow (BBF) during chewing was measured using functional near-infrared spectroscopy in MP patients versus normal occlusion controls (NORM, n = 17). Cognitive function using eye-tracking technology (Mirudake) in MP patients (n = 44) was compared to healthy controls (HC, n = 59), with assessing six cognitive domains. MP patients demonstrated significantly reduced chewing-induced BBF in bilateral inferior frontal gyrus compared to NORM. Global cognitive performance showed no significant difference between MP and HC groups (p = 0.48). However, positive correlations existed between BBF and cognitive performance domains including global performance and memory. ROC analysis using pooled bilateral BBF data (n = 88) revealed modest diagnostic potential for cognitive assessment (AUC = 0.657, 95% CI 0.431-0.865). The optimal threshold yielded 62.5% sensitivity, 73.8% specificity, and 72.7% overall accuracy for detecting cognitive impairment. This first systematic cognitive evaluation of MP patients revealed no significant global cognitive impairment despite confirmed reductions in masticatory-induced brain activation. The observed BBF-cognition correlations suggest BBF measurements may have potential utility as an adjunct measure in cognitive screening, warranting further investigation through longitudinal studies examining orthognathic surgery's potential cognitive effects.

Background/Objectives: Jaw deformities affect not only facial aesthetics but also various oral functions. While previous studies have demonstrated that mandibular prognathism (MP) alters masticatory-induced brain blood flow (BBF), the temporal characteristics of these hemodynamic changes have remained unclear. In this cross-sectional observational study, we investigated the following two specific objectives: (1) whether food hardness affects not only the magnitude but also the temporal patterns of BBF changes during mastication and (2) how malocclusion is associated with these temporal hemodynamic responses. Methods: Twenty-six participants with normal occlusion (NORM) and twenty patients with MP participated in this study. BBF was measured using functional near-infrared spectroscopy, while participants chewed soft paraffin or hard gummy candy. Maximum oxygenated hemoglobin (oxy-Hb) values and time-to-peak BBF were analyzed. Results: While food hardness did not significantly affect maximum oxy-Hb within groups, the MP group showed significantly lower responses during hard gummy candy mastication compared to the NORM group. The occlusal contact area exhibited significant positive correlation with maximum oxy-Hb values, while the ANB angle, an indicator of intermaxillary skeletal relationship, showed no significant correlation with BBF parameters. The hard gummy candy/paraffin ratio of maximum oxy-Hb was significantly higher in the NORM group compared to the MP group. Time-to-peak BBF was approximately twice as long for hard gummy candy compared to paraffin in both groups, with no significant differences between groups. Conclusions: These findings reveal that while MP attenuates the magnitude of masticatory-induced BBF, particularly during hard food mastication, the temporal adaptation to increased food hardness is preserved. This dissociation between magnitude and timing effects suggests that intact basic neurovascular coupling mechanisms would be maintained even in the condition of altered masticatory function in a MP subject, which is providing new insights for rehabilitation strategies in orthognathic surgery cases.

OBJECTIVES: This cross-sectional study aimed to investigate the age-related development of five major oral functions-tongue pressure, lip pressure, masticatory efficiency, bite force, and occlusal contact area-in subjects aged 5-20 years. MATERIAL AND METHODS: Ninety-two subjects were divided into four age groups: 5-8, 9-12, 13-16, and 17-20 years. Oral functions were evaluated using standardized methods, including Chew Check Gum for assessing masticatory efficiency, Dental Prescale II for measuring occlusal force and contact area, JMS tongue pressure measuring device for evaluating tongue pressure, and Ripple-Kun for measuring lip pressure. RESULTS: Occlusal contact area, maximum bite force, tongue pressure, and masticatory efficiency gradually increased with age, whereas lip pressure remained stable. The occlusal contact area temporarily decreased between 5 and 12 years of age, likely due to the replacement of erupting lateral teeth. Some differences were observed between males and females; however, overall trends in oral function parameters were similar. Spearman's rank correlation analyses revealed significant positive correlations between age and occlusal contact area, bite force, tongue pressure, and masticatory efficiency. CONCLUSIONS: Oral function parameters exhibited different age-related development patterns. Occlusal contact area, maximum bite force, tongue pressure, and masticatory efficiency gradually increased with age. Interestingly, the occlusal contact area tended to reduce temporarily between 5 and 12 years of age due to the replacement of erupting lateral teeth. Conversely, lip pressure was almost stable during this period. Our findings provide valuable insights into the developmental patterns of oral functions during the growing years.

Calcification plays a key role in biological processes, and breakdown of the regulatory mechanism results in a pathological state such as ectopic calcification. We hypothesized that ENPP1, the enzyme that produces the calcification inhibitor pyrophosphate, is transcriptionally regulated by Nrf2, and that Nrf2 activation augments ENPP1 expression to inhibit ectopic calcification. Cell culture experiments were performed using mouse osteoblastic cell line MC3T3-E1. Nrf2 was activated by 5-aminolevulinic acid and sodium ferrous citrate. Nrf2 overexpression was induced by the transient transfection of an Nrf2 expression plasmid. ENPP1 expression was monitored by real-time RT-PCR. Because the promoter region of ENPP1 contains several Nrf2-binding sites, chromatin immunoprecipitation using an anti-Nrf2 antibody followed by real-time PCR (ChIP-qPCR) was performed. The relationship between Nrf2 activation and osteoblastic differentiation was examined by alkaline phosphatase (ALP) and Alizarin red staining. We used mice with a hypomorphic mutation in ENPP1 (ttw mice) to analyze whether Nrf2 activation inhibits ectopic calcification. Nrf2 and Nrf2 overexpression augmented ENPP1 expression and inhibited osteoblastic differentiation, as indicated by ALP expression and calcium deposits. ChIP-qPCR showed that some putative Nrf2-binding sites in the ENPP1 promoter region were bound by Nrf2. Nrf2 activation inhibited ectopic calcification in mice. ENPP1 gene expression was transcriptionally regulated by Nrf2, and Nrf2 activation augmented ENPP1 expression, leading to the attenuation of osteoblastic differentiation and ectopic calcification in vitro and in vivo. Nrf2 activation has a therapeutic potential for preventing ectopic calcification.

The masticatory function of patients with skeletal anterior open bite (OPEN) is reported to be impaired compared with that of patients with normal occlusion (NORM). In this study, we compared brain blood flow (BBF) in patients with OPEN and NORM and investigated the factors related to BBF during mastication in patients with OPEN. The study included 17 individuals with NORM and 33 patients with OPEN. The following data were collected: number of occlusal contacts, jaw movement variables during mastication, and BBF measured with functional near-infrared spectroscopy during chewing. The number of occlusal contacts, maximum closing and opening speeds, closing angle, and vertical amplitude were smaller in the OPEN than in the NORM group. Interestingly, BBF increased less in the OPEN group. Correlation analysis revealed that several parameters, including number of occlusal contacts and closing angle, were correlated with changes in BBF during mastication. These results suggest that not only occlusion but also jaw movement variables and factors related to masticatory muscles contribute to the chewing-related increase in BBF. In conclusion, BBF increases less during mastication in patients with OPEN than in those with NORM. In addition, the higher increase in BBF is correlated with jaw movement. Together, we discovered that OPEN exhibits significant adverse effects not only on masticatory function but also on brain function.

Mandibular retrognathism occurs by insufficient mandibular growth and causes several issues, such as respiratory difficulty and diminished masticatory function. At present, functional orthodontic appliances are used for stimulating mandibular growth in pediatric cases. However, the effectiveness of functional appliances is not always stable in daily practices. A more effective, reliable, and safer therapeutic method for mandibular growth promotion would be helpful for growing mandibular retrognathism patients. As we previously discovered that nutritional supplementation of myo-inositol in growing mice specifically increases mandibular endochondral growth, we performed preclinical animal experiments in rabbits in this study. Briefly, six-week-old male Japanese white rabbits were fed with or without myo-inositol supplementation in laboratory chow until 25 weeks old, and 3D image analysis using micro CT data and histological examinations was done. Myo-inositol had no systemic effect, such as femur length, though myo-inositol specifically augmented the mandibular growth. Myo-inositol increased the thickness of mandibular condylar cartilage. We discovered that the nutritional supplementation of myo-inositol during the growth period specifically augmented mandibular growth without any systemic influence, even in rabbits. Our results suggest the possibility of clinical use of myo-inositol for augmentation of the mandibular growth in growing mandibular retrognathism patients in the future.

Vascular calcification, an ectopic calcification exacerbated by aging and renal dysfunction, is closely associated with cardiovascular disease. However, early detection indicators are limited. This study focused on dental pulp stones, ectopic calcifications found in oral tissues that are easily identifiable on dental radiographs. Our investigation explored the frequency and timing of these calcifications in different locations and their relationship to aortic calcification. In cadavers, we examined the association between the frequency of dental pulp stones and aortic calcification, revealing a significant association. Notably, dental pulp stones appeared prior to aortic calcification. Using a rat model of hyperphosphatemia, we confirmed that dental pulp stones formed earlier than calcification in the aortic arch. Interestingly, there were very few instances of aortic calcification without dental pulp stones. Additionally, we conducted cell culture experiments with vascular smooth muscle cells (SMCs) and dental pulp cells (DPCs) to explore the regulatory mechanism underlying high phosphate-mediated calcification. We found that DPCs produced calcification deposits more rapidly and exhibited a stronger augmentation of osteoblast differentiation markers compared with SMCs. In conclusion, the observation of dental pulp stones through X-ray examination during dental checkups could be a valuable method for early diagnosis of aortic calcification risk.

Bone remodelling is mediated by orchestrated communication between osteoclasts and osteoblasts which, in part, is regulated by coupling and anti-coupling factors. Amongst formally known anti-coupling factors, Semaphorin 4D (Sema4D), produced by osteoclasts, plays a key role in downmodulating osteoblastogenesis. Sema4D is produced in both membrane-bound and soluble forms; however, the mechanism responsible for producing sSema4D from osteoclasts is unknown. Sema4D, TACE and MT1-MMP are all expressed on the surface of RANKL-primed osteoclast precursors. However, only Sema4D and TACE were colocalized, not Sema4D and MT1-MMP. When TACE and MT1-MMP were either chemically inhibited or suppressed by siRNA, TACE was found to be more engaged in shedding Sema4D. Anti-TACE-mAb inhibited sSema4D release from osteoclast precursors by ~90%. Supernatant collected from osteoclast precursors (OC-sup) suppressed osteoblastogenesis from MC3T3-E1 cells, as measured by alkaline phosphatase activity, but OC-sup harvested from the osteoclast precursors treated with anti-TACE-mAb restored osteoblastogenesis activity in a manner that compensates for diminished sSema4D. Finally, systemic administration of anti-TACE-mAb downregulated the generation of sSema4D in the mouse model of critical-sized bone defect, whereas local injection of recombinant sSema4D to anti-TACE-mAb-treated defect upregulated local osteoblastogenesis. Therefore, a novel pathway is proposed whereby TACE-mediated shedding of Sema4D expressed on the osteoclast precursors generates functionally active sSema4D to suppress osteoblastogenesis.

Bone destructive diseases such as periodontitis are common worldwide and are caused by excessive osteoclast formation and activation. Receptor activator of nuclear factor-κB ligand (RANKL) is essential factor for osteoclastogenesis. This triggers reactive oxygen species (ROS), which has a key role in intracellular signaling as well exerting cytotoxicity. Cells have protective mechanisms against ROS, such as nuclear factor E2-related factor 2 (Nrf2), which controls the expression of many antioxidant enzyme genes. Conversely, BTB and CNC homology 1 (Bach1), a competitor for Nrf2, transcriptionally represses the expression of anti-oxidant enzymes. Previously, we demonstrated that RANKL induces Bach1 nuclear import and attenuates the expression of Nrf2-mediated antioxidant enzymes, thereby augmenting intracellular ROS signaling and osteoclastogenesis. However, it remains unknown if Bach1 inhibitors attenuate osteoclastogenesis. In this study, we hypothesized that Bach1 inhibition would exert an anti-osteoclastogenic effects via diminishing of intracellular ROS signaling through augmented antioxidation. We used RAW 264.7 cells as osteoclast progenitor cells. Using flow cytometry, we found that Bach1 inhibitors attenuated RANKL-mediated ROS generation, which resulted in the inhibition of osteoclastogenesis. Local injection of a Bach1 inhibitor into the calvaria of male BALB/c mice blocked bone destruction induced by lipopolysaccharide. In conclusion, we demonstrate that Bach1 inhibitor attenuates RANKL-mediated osteoclastogenesis and bone destruction in mice by inducing the expression of Nrf2-regulated antioxidant enzymes that consequently decrease intracellular ROS levels. Bach1 inhibitors have potential in inhibiting bone destructive diseases such as periodontitis, rheumatoid arthritis and osteoporosis.

Masticatory function is closely related to brain function, and normal occlusion is an essential factor for favorable brain function. Mandibular prognathism (MP) shows diminished occlusal function. Jaw deformity is presumed to adversely affect brain function due to decreased occlusal function. In this study, functional near-infrared spectroscopy (fNIRS) was used to examine brain blood flow to investigate the relationship between MP and brain function. <br>Seventeen subjects with normal occlusion and 69 patients with MP participated in this study. The number of occlusal contacts was counted using silicone materials. Electromyography (EMG) of the masseter muscles during clenching was also recorded. Brain blood flow was measured with fNIRS during a calculation task and chewing tasks of preferential chewing of paraffin wax and hard gummy. For parametric data, Student’s t-test was used for statistical significance. The Mann-Whitney U test was used for the statistical significance of nonparametric data. The case of P<0.05 was considered to show a statistically significant difference. <br>As a result, the number of occlusal contacts in MP was smaller than that in normal occlusion. Masseter muscle activity during the clenching task was weaker in MP than normal occlusion. The calculation task increased oxy-Hb in both groups. There was no significant difference in oxy-Hb between groups in the calculation task. The task of preferential chewing of paraffin wax increased oxy-Hb in both groups. This increase was significant in the bilateral inferior frontal gyrus, and interestingly, the increase was lower in MP patients than in normal occlusion. The task of chewing hard gummy also induced an increase in oxy-Hb in the bilateral inferior frontal gyrus in both groups, though the increase was lower in MP patients than in normal occlusion. Correlation analysis revealed quite a weak correlation between the number of occlusal contacts and oxy-Hb, though EMG exhibited a stronger correlation to oxy-Hb than the number of occlusal contacts. <br>In conclusion, compared with normal occlusion, MP patients had decreased brain blood flow during mastication in the bilateral inferior frontal gyrus, which is considered to be particularly involved in cognitive function in the prefrontal cortex. This suggests that MP may influence not only oral function but also systemic function such as brain blood flow.

The first molar fixed type Palatal Lever Arm System (PLAS) fixes the maxillary first molar via the PLAS from the mid-palatal miniscrews. It gives absolute anchorage to molars. In addition, it is possible to prevent side effects of the first molar and expand dental arch to unilateral side, which is considered to be difficult using conventional auxiliary devices. Case 1 : The patient was a 25-years-old Japanese woman. Her chief complaint was upper teeth protrusion. The degree of overjet was 11.3 mm. Mandibular left second premolar was positioned lingually due to crowding. Lateral cephalometric analysis showed a skeletal Class II jaw relationship. Maxillary both first premolars and mandibular left second premolar were extracted. Lingual orthodontic treatment was applied using mid-palatal miniscrews and the first molar fixed type PLAS. Absolute anchorage of the maxillary both first molars were obtained and proper overjet was achieved. Case 2 : The patient was a 20-years-old Japanese woman. Her chief complaint was crowding of upper teeth. The left molars showed crossbite, and the maxillary left lateral incisor was positioned lingually. The inter-canine Class II relationship was observed. Four first premolars were extracted. Lingual orthodontic treatment was applied using mid-palatal miniscrews and the first molar fixed type PLAS. By activating the left arm of the PLAS to the buccal side, the crossbite of the left molars were corrected.

Appropriate treatment of lymph node metastasis is essential to improve the prognosis for patients. Although lymph node dissection is a primary option, it is invasive and associated with adverse effects. The development of novel, non-invasive therapeutic methods in preclinical experiments using mice has been restricted by the small lymph node size (only a few millimeters in diameter) and the limited techniques available for non-invasive monitoring of the progression of lymph node metastasis. Here, we show a local drug delivery using nanobubbles and ultrasound via lymphatic vessels shows potential as a non-invasive treatment for tumors in the proper axillary lymph nodes (proper-ALNs) of MXHIO/Mo-Ipr/lpr mice, which develop systemic swelling of lymph nodes (up to 13 mm in diameter, similar in size to human lymph nodes). Tumor cells were inoculated into the proper-ALNs to develop a model of metastatic lesions, and any anti-tumor effects of therapy were assessed with an in vivo bioluminescence imaging system, a high-frequency ultrasound system, and immunohistochemical analysis. We confirmed the successful noninvasive deliveiy of small molecule (TOTO-3 iodide) into lymphocytes invaded by cancer. Our system has a potential for a novel local delivery system for the treatment of lymph node metastasis.

Basic data on periodontal ligament fibroblasts are essential for regenerative treatment of the periodontal ligament and for elucidating the cause of ankylosis. We focused on the specific environment of periodontal ligament tissue that maintains high phosphate levels by alkaline phosphatase activity. We then hypothesized that an increase in intracellular phosphate via the sodium-dependent phosphate co-transporters in periodontal ligament fibroblasts is the factor that leads to osteoblast differentiation. These results showed that an increase in intracellular phosphate concentration, rather than extracellular phosphate concentration, caused the expression of osteoblast markers and the production of calcification.

Articular cartilage is thought to exhibit tolerance against mechanical compression such as body weight loading on knee joint. On the other hand, mandibular condylar cartilage is sensitive to the mechanical loading. Orthodontic treatment uses this characteristics to modulate the growth at the mandibular condylar cartilage. In this research project, we hypothesized that epigenetic control of the anti-oxidative stress tolerance via the transcription factor, Nrf2, in the specificity of the condylar cartilage. To clarify the issue, we utilized in vivo and in vitro experiment, and revealed the possible epigenetic control of the anti-oxidative stress tolerance in the specificity of the condylar cartilage.

Epigenetical regulation of the phenotype of the periodontal ligament fibroblast was examined and compared with that of the gingival fibroblast using experimental rats. There are differences in the expression of monomethylated Histone H3K9 and dimethylated Histone H3K9, and PDL fibroblast exhibited intense expression of monomethylated Histone H3K9. Among known methyltransferases, Glp and G9a were shown difference in the expression between gingival and PDL fibroblasts. PDL fibroblasts expressed more intense Glp and G9a expression. These data obtained by animal experiments were confirmed in the cell culture experiments using human gingival and PDL fibroblast cell lines. Then we compared gene expression using microarray, and narrowed down the candidate genes to 10, including transcription factors.

We aimed to elucidate the regulatory mechanism of the induction of the different bone metabolisms, bone resorption and bone formation, at the compression and tension side during orthodontic tooth movement. We focused on the microRNA (miRNA), and analysed the expression of miRNA in PDL cells under compression and tension. We discovered that different miRNAs were upregulated in PDL cells under compression and tension. Then we examined the relationship between the change of OPG expression by different mechanical stresses and the difference of miRNA induction. miRNAs which can target OPG were firstly selected, and we found that miR-3198 was the miRNA which exhibited different change of the expression between compression and tension. miR-3198 and OPG inversely changed by compression and tension. Finally, we confirmed that miR-3198 play a role in the change of OPG expression under compression and tension.

ADAM family enzymes, which are highly expressed in cancer cells, were reported as the prognostic factor of tumor. In addition, some cancer cells favor to metastasize into bone marrow through the induction of osteoclastic bone resorption around cancer cells. In the present study, we investigated the degradation mechanism of interferon gamma, which shows anti-osteoclastogenic and anti-tumorigenic property, by cancer cells. We discovered ADAM 17 degrades interferon gamma for the first time.

The analysis of molecules induced by mechanical stress in the cells of the periodontal tissue (1) Both the osteoblasts differentiation-regulating factor and the osteoclast differentiation-regulating factor produced from the periodontal ligament (PDL) cells: Soluble factor produced from PDL cells stimulated by cyclic tension force inhibited osteoblast differentiation, suppressing the expression of transcription factor, osterix and runx-2. Osteoprotegerin (OPG) was also produced by PDL cells, and on the other hand, inhibited the osteoclast formation. (2) The differences of the response to continuous compression force between PDL cells and the osteoblasts: The PDL cells were resistance to the compression force, and promoted the production of the angiogenesis factor corresponding to the compression force. On the other hand, osteoblasts were caused apoptosis by activation of caspase-3 via caspase-8 signaling pathway. 2. Development of appropriate in vivo delivery system of in vivo gene transfer to periodontal tissue and functional analysis (1) Receptor activator of NF- r B ligand (RANKL) and the OPG gene transfer by the HVJ-envelop vector method: RANKL gene transfer promoted the experimental tooth movement (ETM). Moreover the OPG gene transfers inhibited ETM and relapse after ETM by regulating the osteoclast formation. (2) Bone morphogenetic protein (BMP) -4-gene transfer by the in vivo electroporation method: The alveolar bone density increased by the BMP-4 gene transfer, and the relapse after ETM inhibited. (3) The gene transfer by the ultrasound using nanobubbles together: Using the nanobubbles together, high efficiency and transient expression were observed in the experiment targeting to PDL cells in vitro, and to periodontal tissue in vivo.
