Abstract Jaw reconstruction using autogenous particulate cancellous bone and marrow (PCBM) is widely performed; however, objectively evaluating regenerated bone quality remains challenging because conventional assessments rely on density-based imaging. In this retrospective study, we characterized PCBM-regenerated jaw bone using micro-computed tomography (micro-CT) and Raman spectroscopy. Bone samples were obtained from nine patients during implant placement 6 months after PCBM transplantation. Micro-CT quantified trabecular microarchitecture, including bone volume fraction, trabecular bone thickness, trabecular number, and trabecular separation. Raman spectroscopy assessed the mineral- and matrix-related compositional parameters, including the mineral-to-matrix ratio, carbonate-to-phosphate ratio, and apatite crystallinity. No statistically significant differences in microstructural or Raman-derived parameters were observed between PCBM grafts covered with skin flaps and those covered with gingival mucosa. Similarly, no significant differences were observed between male and female patients. Histological analysis demonstrated newly formed lamellar bone in all specimens. These findings demonstrate the feasibility of using micro-CT, Raman spectroscopy, and histological evaluation for multimodal characterization of PCBM-regenerated jaw bone. The specimens showed interindividual structural and compositional variability. Further studies in larger, controlled cohorts are needed to determine the clinical significance of this variability.

Abstract This study was conducted to evaluate the effect of titanium mesh exposure on the augmented bone volume and implant stability in patients undergoing guided bone regeneration with autogenous bone grafts. In this retrospective cohort study, 30 patients underwent alveolar ridge augmentation between 2013 and 2023; 2 patients were excluded for early postoperative infection, leaving 28 patients for analysis (exposed, n = 13; unexposed, n = 15). Approximately 5 months after augmentation, computed tomography was used to assess the augmented bone volume and density. Volumetric and linear measurements were performed, and implant stability quotients were recorded at the time of implant placement. Between-group comparisons were performed using appropriate parametric and nonparametric tests; the significance level was set to p < 0.05, and 95% confidence intervals were calculated. The augmented bone volume was significantly smaller in the exposed group than in the unexposed group ( p = 0.01; 95% CI, 0.23 to 0.81). The vertical and horizontal augmentation rates at 3 mm from the alveolar apex did not differ between groups ( p = 0.21; 95% CI, −0.90 to 0.18 and p = 0.39; 95% CI, −0.77 to 0.30, respectively). The mean computed tomography attenuation values and implant stability quotients were 497 ± 91 and 523 ± 87 Hounsfield units ( p = 0.64; 95% CI, –1.22 were placed successfully at all intended sites. Thus, titanium mesh exposure was associated with a significant reduction in three-dimensional augmented bone volume but no change in computed tomography density, primary implant stability, or linear augmentation metrics. Careful soft-tissue management to minimize exposure may help to optimize guided bone regeneration outcomes. This study has several limitations, including its retrospective design, small sample size, and single-center setting. Clinically, minimizing TiME exposure may help optimize bone augmentation outcomes, and further prospective studies are needed to confirm these findings.

OBJECTIVE: Compressive mechanical stress was applied to the temporomandibular joints (TMJ) affected by antigen-induced arthritis to assess the etiological factors of idiopathic condylar resorption. METHODS: Following ovariectomy in 27 female rabbits, 12 received ovalbumin (OVA), 12 received phosphate-buffered saline, and three formed the control group. Each treated rabbit underwent an osteotomy, and a custom device was employed for one week postoperatively, with the length increased by 0.25 mm every 12 h to provide compressive mechanical stress to the TMJ. Thereafter, samples were obtained from the treated groups, subjected to histological staining and immunohistochemistry, and examined using micro-computed tomography. RESULTS: At each examination, the OVA group showed a greater area and depth of bone resorption, with bone resorption continuing for three weeks following distraction. Additionally, subcondylar bone resorption was noted significantly earlier and had a greater prevalence in the OVA group and greater numbers of tartrate-resistant acid phosphatase-positive cells. Immunostaining for metalloproteinase (MMP)-3 and MMP-13 of the anterior condylar head in the OVA group after two and three weeks revealed high levels of both proteins from the surface to the deep cartilage layer. CONCLUSION: Therefore, coexisting TMJ pathology factors, such as antigen-induced arthritis, promote a significantly greater amount of condylar head anterior surface bone resorption.

BACKGROUND: We aimed to construct an artificial intelligence-based model for detecting oral cancer and dysplastic leukoplakia using oral cavity images captured with a single-lens reflex camera. SUBJECTS AND METHODS: We used 1043 images of lesions from 424 patients with oral squamous cell carcinoma (OSCC), leukoplakia, and other oral mucosal diseases. An object detection model was constructed using a Single Shot Multibox Detector to detect oral diseases and their locations using images. The model was trained using 523 images of oral cancer, and its performance was evaluated using images of oral cancer (n = 66), leukoplakia (n = 49), and other oral diseases (n = 405). RESULTS: For the detection of only OSCC versus OSCC and leukoplakia, the model demonstrated a sensitivity of 93.9% versus 83.7%, a negative predictive value of 98.8% versus 94.5%, and a specificity of 81.2% versus 81.2%. CONCLUSIONS: Our proposed model is a potential diagnostic tool for oral diseases.

Fibrin glue from autologous plasma may prevent viral infection and allergic reaction. Moreover, this biomaterial contains growth factors such as TGF-β and VEGF that promote reconstruction of the mucous membrane by stimulating fibroblast proliferation and angiogenesis. Thus, autologous fibrin glue is predicted to improve healing better than commercial fibrin glue. Here, we evaluated the effects of autologous fibrin glue on the crucial early phase of wound healing. Epithelial defects were introduced in rats and covered with polyglycolic acid (PGA) sheets with or without commercial or autologous fibrin glue. Wound healing was assessed for six weeks by histology and immunohistochemistry. Our results demonstrate that wounds covered with PGA sheets and autologous fibrin glue achieved efficient wound healing without complications such as local infection or incomplete healing. The rate of recovery of the regenerating epithelium in this group was superior to that in wounds covered with PGA sheets and commercial fibrin glue. Immunohistochemistry of laminin, cytokeratin, and VEGF confirmed fine and rapid epithelial neogenesis. Collectively, our results indicate that covering surgical wounds with autologous fibrin glue promotes wound healing and epithelialization, improves safety, and reduces the risks of viral infection and allergic reaction associated with conventional techniques.

This study was designed to examine osteoconductive effects of Mg in rats tibia. The animals were sacrificed after 1, 2, and 8 weeks. The elemental analysis was performed using SEM/EDX at week 1. Following X-ray micrography at weeks 2 and 8, samples were embedded in paraffin. The expression of osteocalcin was observed by immunohistochemical staining. The element concentrations of fibrous capsules around the specimens were also measured by ICP-MS. The concentrations of Ca and P on the surface of the Mg specimen increased in SEM/EDX. The tissue specimen showed new bone formation on the bone surface near the implanted area. The concentrations of Mg, Ca, and P were high in the fibrous capsules surrounding Mg. Implantation induced differentiation of osteoblasts, and this process was considered to be associated with new bone formation. Induction of cell differentiation may be influenced by corrosion products in addition to corroding magnesium.

The CryoSeal® FS System has been recently introduced as an automated device for the production of complete fibrin glue from autologous plasma, rather than from pool allogenic or cattle blood, to prevent viral infection and allergic reaction. We evaluated the effectiveness of complete autologous fibrin glue and polyglycolic acid (PGA) sheet wound coverings in mucosa defect oral surgery. Postoperative pain, scar contracture, ingestion, tongue dyskinesia, and postoperative bleeding were evaluated in 12 patients who underwent oral (including the tongue) mucosa excision, and received a PGA sheet and an autologous fibrin glue covering. They were compared with 12 patients who received a PGA sheet and commercial allogenic fibrin glue. All cases in the complete autologous fibrin glue group demonstrated good wound healing without complications such as local infection or incomplete cure. All evaluated clinical measures in this group were similar or superior to the commercial allogenic fibrin glue group. Coagulation and adhesion quality achieved with this method was comparable to that with a PGA sheet and commercial fibrin glue. Covering oral surgery wounds with complete autologous fibrin glue produced by an automated device was convenient, safe, and reduced the risk of viral infection and allergic reaction associated with conventional techniques.
