Abstract This retrospective study aimed to kinematically classify mandibular movements collected during Temporomandibular Disorders (TMD) treatment, employing Fourier transformation (FT), Principal Component Analysis (PCA), and K-means clustering (k-means), and to investigate their correlation with symptoms of pain-related TMD. The study included five TMD participants diagnosed with myalgia (age: 39–86 years, with an SD of 18.96) and three healthy participants (age: 32–42 years, with an SD of 5.13) with no stomatognathic problems. TMD participants underwent tailored treatment for their symptoms, and their maximum unassisted mouth opening (MMO) was recorded randomly with a motion capture system (ARCUS digma II, Kavo, Biberach, Germany) at multiple time points. MMO for healthy participants served as a control. The dataset comprising 28 trials, was transferred to the AnyBody Modeling System (AnyBody Technology, Aalborg, Denmark) to extract joint angle time series, which were then transformed into Fourier series. Subsequently, PCA and k-means clustering were conducted. Two clusters were identified: Cluster 1, predominantly composed of symptomatic trials, and Cluster 2, mainly consisting of asymptomatic trials. Distinct transition pathways between the clusters were observed among participants, corresponding to the alleviation of pain-related symptoms during TMD treatment. These findings suggest that this approach has potential as an effective tool for diagnosing and assessing TMD by identifying symptomatic kinematic patterns and tracking temporal changes in mandibular movement. Despite the small dataset, these results suggest promise for a novel functional assessment method for TMD based on kinematic features.

    BACKGROUND: Research on resolving implant fracture is still gaining attention as it can be a serious treatment failure outcome. The implant fracture is likely to occur due to increased stress in implant body associated with peri-implant bone resorption. OBJECTIVE: This study aimed to investigate the relationship between degree of peri-implant bone resorption and stress distribution in implant body by using finite element analysis (FEA). METHODS: The magnitude and direction of loads on implants at the mandibular molar region were measured with 3D piezoelectric force transducers in a patient during maximal voluntary clenching, grinding, and tapping to obtain input for the FEA as the loading condition. Simplified finite element bone models were generated to simulate the six patterns of peri-implant bone resorption progression which the bone levels from implant platform were set at 0, 1, 2, 3, 4, and 5 mm. Three types of implants with different diameters (3.5, 4.0, and 5.0 mm), corresponding abutments and screws were created. FEA using the in vivo measured load was conducted to investigate how peri-implant bone loss affects stress distribution in the implant body. RESULTS: Loading condition affected stress distribution in the implant body. As bone resorption increased, von Mises stress in the implant body became larger, which was marked in smaller diameter implants, and this change was amply demonstrated when the bone resorption increased from 0 to 2 mm. CONCLUSION: The FEA demonstrated that as peri-implant bone loss increased, the magnitude of stress in the implant increased, especially in small diameter implants.

    Temporomandibular disorder (TMD) is a prevalent dental disease in common with dental caries and periodontitis. The major symptoms of TMD are masticatory muscle pain, temporomandibular joint (TMJ) pain and impairment of jaw movement due to the pain and pathologic derangement of TMJs. However, there are few studies using TMD patient-specific motion data to drive the musculoskeletal model that can elucidate kinematic and biomechanical characteristics of the patient. The purpose of this study is to develop the workflow of musculoskeletal modeling of the mandible with jaw motion data obtained from a TMD patient. This involves the establishment of patient-specific boundary conditions representing the characteristics of the TMJ. The jaw motion of a TMD patient was recorded and used as an input for driving the model.

    ABSTRACT: The patient was a man aged 57 at his first visit. His occlusal contacts in the intercuspal position were unbalanced because of a partially edentulous dental arch and a scissors-bite. The final prosthesis was placed after stabilizing the occlusal posture with a maxillary occlusal splint, which was a treatment denture covering the occlusal surfaces of the remaining teeth. Discussion: Using the face-bow transfer and cross-mounting technique, the elements of temporary occlusion with the treatment denture were reflected in the final denture, which allowed the patient to achieve improved mastication efficiency and mouth-related QOL. Conclusion: In cases with unbalanced posture of the mandible, prior to installing the final prosthesis, it is important to establish occlusal support and stabilize the occlusal posture by a treatment denture.<br /> Key words: Scissors bite, Treatment denture, Cross-mounting technique, Masticatory efficiency, OHIP-14

    The distribution of stress in the jawbone during mastication is highlighted in this paper as an important index of the mechanical performance of dental implants. The objective of this paper is twofold. The first objective was to demonstrate the importance of considering the heterogeneity (spatial distribution) of the mechanical properties of jawbone in stress analysis. Next, the influence of the alignments of the implants on the peri-implant bone stress was examined using a finite element method (FEM) and numerical analysis model which reproduced the heterogeneity of the jawbone based on the spatial distribution of the bone density measured as CT data. For the former objective, numerical analyses of a finite element model of the mandible of a patient were conducted. Using CT data of this patient and taking into account the non-uniformity (spatial distribution) of bone density, we reproduced a patient-specific geometry and mechanical properties of the jawbone. In-vivo three-dimensional loads measured by instruments attached to the implants were used in the numerical analyses. As for the material properties of jawbone, we employed two models: a multi-value model that considers the inhomogeneity and spatial variability of mechanical properties of the bone, and a binary model with two discrete mechanical properties for cortical bone and cancellous bone. In the numerical analyses, a large difference in the stress distributions was observed between these two models. This demonstrates the insufficiency of the over-simplified binary model and the validity of the multi-value model that is closer to reality. The effects of the alignments of the implants were analyzed against the maximal voluntary clenching (MVC) load measured in the patient. As a result, peri-implant bone stress was found to be greatly reduced when the alignments of the implants were changed to match the direction of MVC load. It is suggested that such alignments are optimal. As more realistic loads, the loads measured while chewing peanuts were used for implants aligned in such optimal directions. For those loads, stress reduction was observed, especially when the loads had large magnitudes. Thus, we demonstrated the importance of accurate modeling of bone material properties and the alignments of implants in terms of the distribution of peri-implant bone stress.

    This study aimed to clarify the mechanical stress distribution in the mandibular bone with implants supporting an overdenture using the finite element analysis (FEA) based on the morphological data and 3-dimensional (3-D) load measured in a subject. A couple of 3-D FEA models were created based on data from a 62-year-old edentulous woman, who had installed four implants in her mandible and wore an overdenture, using FEA software (Patran). One model furnished four implants aligned as in the patient. In the other model, each implant was aligned along the direction of the measured load vector in maximum clenching in the patient. The result showsthat the stress distribution was affected by the number of implants and the magnitude and the direction of occlusal load. In addition, changing the implants' alignment to the load direction reduced the von Mises stress to the result from the model as the patient.

    本研究は, 歯科インプラントにおける臨床的偶発症の予防, さらには歯科インプラントの埋入に係るメカノバイオロジカルな反応の把握した顎骨リモデリングシミュレーションの最適化に寄与する. すなわち, 従来の解剖学的な観点を考慮した手法に加えて本シミュレーションを埋入システムに包含することで, より安全で効率的な歯科インプラント治療を実現することが期待される.上記に示す展望の実現に向けて, これまで申請者らが従事してきた研究並びに臨床経験を基に新たな知見を得ることを目標として本課題に携わった. 特に申請書において, 研究目標を年度ごとに細分化してマイルストーンを設置している. 本年度は, 共同研究者の小川らが確立した実験モデルを踏襲して,Wistar系ラットの脛骨部について純チタンのカスタムメイドインプラントを埋入したモデルを用いて解析を行った. 骨強度評価ソフトウェア(MECHANICALFINDER, 計算力学研究センター, 日本製)を使用して, 撮像したラット脛骨CTのDICOMデータより骨形状を抽出し, 有限要素モデル(以下, FEモデル)を作成した. ラット脛骨の皮質骨および海綿骨の材料特性は, 部位特異的な骨梁構造及び材料非線形性を考慮した設定を行うことで骨内領域を従来手法に比べてより実現象の再現性に富んだモデル構築に成功している. 加えて, 本モデルは経時的な骨代謝の観察と対照することでインプラントの骨梁形態変化の評価を行う基盤となるFEモデルである.

    This study was aimed to elucidate the biomechanical effects of bone remodeling over a period of 12 years by means of morphological assessment and finite element analysis (FEA) studies from CT data. The subject with an implant-supported overdenture, was chosen to clarify the effects. Two kinds of FE models were constructed from CT data taken at two time points: pre-implantation (Model A) and 12 years post-implantation (Model B). The results show a homogenization of the stress and strain energy density between the implant regions from Model A to Model B. The results of the morphological assessments demonstrated that the bone mass and quality had significantly changed over 12 years. Area-specific bone resorption was also observed at the bone surrounding each implant. The combined findings indicate that the homogenization of mechanical factors was due to chronological changes in bone morphology, suggesting adaptation to mechanical loads by peri-implant bone remodeling.

    In this study, it was intended to construct the three dimensional simulation cord which could express the living bone remodeling and density of the jawbone which continued changing by bone metabolic disorders or osteoporosis or aging during the living life of human. In the results, comparing the simulation calculation with the bone density distribution that we measured from an actual CT data, the bone density in the simulation tended to be slightly lower than the bone density of CT measurement, but successfully reproduced the bone density changing through the life and gender-related tendency. The bone density simulation model may predict the mandibular bone density distribution and changing in the life.

    This time we would like to focus on how the superstructure design of implant-supported fixed prosthesis and the number of implant affects mechanical behavior. The patient is 67-year-old woman, and three implants were installed between second premolar to second molar region in the left side of mandible. 3 Types of superstructure design were modeld with biological loading data.In summary,1) Decrease of the number of implants supporting FPDs doesn’t always cause an increase of stress and displacement.2) Cantilever bridge design causes an increase of stress and displacement.3) The stress and displacement were increased remarkably during function. Finally in conclusion, from this study, the design of cantilever bridge may cause an increase of stress in peri-implant bone and displacement of superstructure in implant-supported FPDs.

    NaF Positron Emission Tomography (PET) can identify subtle functional variation prior to the major structural change detectable by X-ray. NaF-PET/CT scan was performed to examine the change of bone metabolism in the residual ridge underneath denture before and after the RPD treatment. The resultant mechanobiological response was analyzed using a 3D finite element (FE) model derived from the computerized tomography (CT) images of a corresponding patient under the occlusal loading. Mechanobiological stimulus determined from the FEA are quantified and compared with the standardized uptake values (SUV) of PET. The increased application of occlusal force after RPD insertion induced higher mechanical stimuli in the residual bone, and SUV increased in the region of residual ridge with higher mechanical stimuli. SUV had clear correlation with mechanical stimulus in the cancellous bone, especially after RPD insertion (R2 > 0.8, p < 0.01).

    The purpose of this study was to develop 3-demensional simulation code based on age-dependent bone density. During 3years of study periods, constructing of the simulation model, refining of individual analytical parameters and comparison between the measured CT data were carried out in consistently. Reviewing consistency between subjects CT data and the simulation model, correlation between the actual bone geometry and analytical bone geometry was observed. The results of this study is intended to the effectiveness of predictive simulation of bone remodeling in the mandible. Moreover, these results contribute to the diagnostic prediction in treatment with dental prosthesis.

    This study aimed to examine the influence of various loading on dynamic changes of bone-metabolism around the implant using a super-high resolution [18F]NaF-PET in rat tibiae. Adult male Wistar rats installed experimental implant on the tibia were used in this study. Increase of bone-metabolism was observed until 7 days after implantation surgery and then gradually decreased.     The purpose of this study was to investigate the stress in peri-implant bone and the effect of prosthetic designs by dynamic loading data based finite element analysis. Overall, the splinted superstructure model reduced stress in peri-implant bone in comparison with the non-splinted superstructure model.     The 3-D load-measuring device, comprised of a piezo-electric force transducer and the experimental superstructure, was developed. The devices were tightened with screws into implant-fixtures in the subjects. The 3-D loads during the maximum voluntary cenching (MVC)and biting a piece of paraffin wax (WAX)were registered. The magnitude and direction of the recorded loads were analyzed in the spatial coordinates based on her Frankfort and sagittal planes. The device enabled us to estimate the 3-D loads on t he implants. Load exerted on the implant supporting fixed superstructure during chewing function was larger than that during clenching. The amount of change in the load direction during chewing was larger than that during chewing. It was suggested that the species of teeth or occulusal contact was affect the load direction

    The present study aimed to development of implant treatment in clinical situation. In this study, we were concerned with a biomechanical relation between implant and peri-implant bone under the mastication. It is concluded that changing implant direction reduced the stress in the peri-implant bone. The result of this study is informative to the implant treatment for each case.

    The 3-dimensional (3-D) functional loads exerted on implants are essential factors in the biomechanical control of implant-supported overdentures. Since there have been few data in vivo, parameters which could affect the loads are still unclear. As for as the results of this study, the loads exerted on the implants during function are affected three dimensionally by the number of implants supporting the overdenture.
