緊急入院時のインテーク面接に向けた病棟看護師の状況認識の構造化の試み

関西の1000床規模の急性期病院1施設の看護師7名(平均年齢32.1±7.6歳、平均経験年数10.6±7.2年、平均部署配属年数3.7±1.8年)に、緊急入院時のインテーク面接に向けた病棟看護師の状況認識の構造について2018年7月から8月にインタビュー調査を実施した。対象者に質問紙に基本属性を記入してもらい「緊急入院に関するシナリオ」を配布し、内容を吟味してもらった後、もし自分が受け持ち看護師だとしたら、どのようにインテーク面接に向かうかをインタビューした。録音した面接内容から逐語録を作成し、Krippendorffのメッセージ分析の技法を参照して分析した。分析の結果、緊急入院した患者のインテーク面接に向けた看護師の状況認識について「事前情報に項目を立てて整理する」「事前情報に不足があることから、追加の情報収集の必要性を理解する」「患者に求められるケアを予測する」など五つのカテゴリーを生成した。

AIM: This cross-sectional study aims to explore the relationship between different types and sources of social support and psychological distress by age and sex among survivors living in temporary housing 10 months after the Great East Japan Earthquake. METHODS: Self-reported questionnaires/structured interviews administered from January to March 2012 recorded demographic characteristics, damage involving participants' families, social support, and psychological distress. RESULTS: Data on 296 participants aged 20 years or more from nine temporary housing complexes in Otsuchi were analyzed; K6 scores indicating psychological distress averaged 5.1 (standard deviation, 5.9; range, 0-24). Multiple logistic regression analyses indicated differences among types and sources of social support with regard to psychological distress by age and sex among disaster survivors. For men aged less than 65 years, social support by family was related to lower psychological distress. For women aged 65 years or more, emotional support from family, informational and instrumental support, and social companionship from friends in their own temporary housing complexes were related to less psychological distress. CONCLUSION: Differences in age and sex were related to different sources of social support in relation to psychological distress. It is necessary to pay more attention to those who lost family members in the disaster, especially men aged less than 65 years. It may also be necessary to support survivors in making friends when they relocate to temporary housing, especially women aged 65 years or more.

AIM: An earthquake of magnitude 9.0 and accompanying tsunami struck the Tohoku region in northeast Japan on 11 March 2011. The purpose of this study was to observe evacuees' health status such as general health, psychological distress, and feeling of isolation over time. METHODS: The authors distributed a questionnaire survey to residents of temporary housing in January to March (time 1) and again in October to December of 2012 (time 2). RESULTS: Two hundred respondents who filled out the questionnaire at both times were the subjects of this study. The self-reported general health in 39% and mental health status in 43.5% of the participants were poor at time 2. In addition, a higher percentage of participants felt a sense of isolation at time 2 even though more of the participants participated in community activities at that time. CONCLUSION: This result shows the evacuees' prolonged psychological distress and strengthening sense of isolation. It will be necessary to monitor them carefully and provide support continuously over the long term.

Purpose : This study aims to clarify cases of individual support measures for the elderly that were implemented by cooperation between neighbors and public health nurses (PHNs) based at comprehensive community support centers (CCSC) in Japan. Methods : Qualitative and inductive analysis was performed. Semi-structured interviews were conducted with nine PHNs having over two years of experience at CCSCs. The interviewees were asked about their intentions and actions in the cases wherein they sought the cooperation of the community in providing formal services. Results : Four categories were extracted as the supports of PHNs at CCSCs: "extending network in order to obtain information on the elderly from other community members," "understanding the present situation and prospecting possible directions of support for each case based on consultation and paying attention to information," "Establishing the foundations of a support system involving the neighbors," "increasing neighbors' supporting ability through motivation and empowering of individual neighbors." Conclusion : As seen in cases wherein individual support measures are implemented through the cooperation of neighbors, PHNs in CCSCs have created a system to acquire information about elderly residents in the community and empower the neighbors, which is an approach similar to the process of community empowerment
特定機能病院における病棟看護師の退院支援についての認識および実施状況退院支援部署の有無による比較に焦点を当てて

特定機能病院における退院支援部署の設置が,病棟看護師の意識に与える影響を検討することを目的に,国立大学病院で退院支援部署のある病院とない病院それぞれ1病院ずつを対象に,病棟看護師,看護師長にアンケートを実施した.退院支援部署のある病院では看護師199名(有効回答率37.1%),看護師長22名(同78.6%),また,退院支援部署のない病院では,看護師187名(同33.1%),看護師長13名(同59.1%)から,それぞれ有効回答を得た.その結果,退院支援部署がある病院の看護師の方が,病院外の資源に関する支援に取り組んでおり,看護保健について知識を有する割合も高かった.これらの結果から,退院支援部署の存在が,看護師の対外的なコーディネートや意識啓発を促進する可能性が示唆された

【背景・方法】東日本大震災で大きな被害を受けた自治体（以下、被災自治体とする）の地域見守り活動の活動内容とその結果を把握するため、被災自治体の災害公営住宅地域見守り活動を積極的に実施している地区でフィールドワークとインタビューを行った。【結果】地域見守り活動を行う前の状況について、災害公営住宅は、震災前に様々な地区に居住していた人が入居しているため、どこに誰が住んでいるのかも分からない状態であり、災害公営住宅への入居が始まってから2年間は、住民による組織的な活動は実施しておらず、近所のつながりがない状況であった。地域見守り活動として行っている活動は、月2回高齢者の単身世帯、高齢夫婦のみの世帯を中心に見回り隊が訪問活動を行っている。その他、災害公営住宅内の集会所でカフェを行ったり、スポーツサークルを立ち上げるなど、住民による活動が行われている。訪問活動などの活動が始まったきっかけは、秋祭りなどのイベントをやるようになり、自治会が機能しだしたことであった。訪問活動を担っているのは見守り隊であるが、活動計画時は老人クラブに担って貰う予定であったが、老人クラブのメンバーが受け入れなかったため、見守り隊を立ち上げるという経緯があった。見守り活動を担う見守り隊のリクルートは継続的な課題となっている。見守り活動を行う中での課題は、見守り訪問を行っても留守の事も多いが、耳が遠いなどで反応がない場合もあり、反応がない場合の対応に困ることが多いが、見守り隊はボランティアでもあるため、反応がない場合でも家の仲間では入って行かないようにする等、ルールを決めて対応している。このような見守り活動で、訪問を受けている人から「来てもらって安心だ」と言われている。しかし、見回り隊は自分達が必要性を感じてやっていることにやりがいは感じているが、やっていて良かったと感じることはなかった。

After disasters such as the Great East Japan Earthquake, a wide variety of health problems have been occurred. Therefore, in affected areas by the Great East Japan Earthquake, community watching activities have been implemented as one of health and welfare supports for affected people. But, the characteristics of the affected people who need to watch over have not been clear. Therefore, we conducted this research to extract the characteristics. As a result, the extracted features were similar to those in peacetime, but characteristics related to disasters were also extracted. Therefore, as a preparation for disasters, it is necessary to know what happens characteristically after a disaster, in addition to conduct community watching activities proactively from peacetime.

The nationwide survey for the hospitals with 100 or more beds showed that more than 50% of hospitals were "making efforts" to perceive needs of the needs of home care support for outpatient. The survey for the nurses working outpatient clinic at a special functioning hosital shows that less than half nurses were doing the support for home care needs among the outpatients. Nurses who have more knowledge about home care or make more communication with other professionals were likely to do the support to refer the home care support for outpatients. Obserbational points to grasp home care need of the outpatients are including their treatment and disease condition, how to visit to hospital, situation of attendant, self management and self care and so on. As to the survey, 90% or more nurses answered most of these points were important.
