  1. Combining Geriatric Nutritional Risk Index with Total Cholesterol to Predict Pneumonia Mortality Risks in a Cohort of General Older Adults. International-journal

    Background/Objectives: To examine the importance of a composite measure incorporating the Geriatric Nutritional Risk Index (GNRI) and total cholesterol (TC), termed TC-GNRI, in predicting pneumonia mortality in community-dwelling aged individuals. Methods: A longitudinal analysis of the Tsurugaya cohort in Japan, including 1124 participants aged 70 years or older, was used for tracking pneumonia-related deaths for an 11-year period. Total cholesterol thresholds were set at 171 mg/dL (males) and 192 mg/dL (females), classified into higher- and lower-groups. GNRI was divided into higher (≥100.64) and lower (<100.64) groups. A combined index (TC-GNRI) was then created by integrating these indices into three levels: high (both values at or above the thresholds), intermediate (one value above and one below), and low (both below). Cox proportional hazards models estimated hazard ratios (HRs) for pneumonia mortality, adjusting for age, gender, smoking, the Timed Up and Go test, %FVC, and tuberculosis, using high groups as references. Results: Kaplan-Meier curves showed that lower total cholesterol and lower GNRI were associated with higher pneumonia mortality than in the respective higher groups. The intermediate- and low-TC-GNRI groups had poorer survival rates than the high group. After adjustment, lower total cholesterol (HR = 3.03, 95% CI 1.41-6.52) and lower GNRI (HR = 2.69, 95% CI 1.21-5.99) were each associated with greater pneumonia mortality than the higher groups. The intermediate- (HR = 2.81, 95% CI 1.18-6.70) and low-TC-GNRI (HR = 6.17, 95% CI 2.15-17.74) groups showed greater pneumonia mortality than the high group. Conclusions: TC-GNRI may provide additional value in indicating pneumonia mortality risk than total cholesterol or GNRI alone. TC-GNRI may be a valuable tool for identifying older adults at particularly high risk of pneumonia mortality.

    BACKGROUND: Risk factors for aspiration pneumonia in aged adults include respiratory muscle weakness and ineffective coughing, which might be influenced by physical position and be associated with sarcopenia. OBJECTIVES: This investigation sought to elucidate the optimal physical position for maximising respiratory muscle force and cough effectiveness, to investigate correlations between respiratory muscle force and other sarcopenia-related parameters in older people, which remain largely unclear, and to search for a novel pneumonia prevention approach. METHODS: Twenty-six healthy older women were evaluated in six positions: supine (0°), 30° semi-recumbent, 60° semi-recumbent, 90° sitting, and 60° semi-recumbent with left/right trunk rotation. Maximum inspiratory and expiratory pressures, cough peak flow, respiratory function, tongue pressure, and self-assessed cough ease were measured. We analysed correlations between respiratory muscle force and other parameters. RESULTS: The 60° semi-recumbent position demonstrated significantly greater maximum inspiratory pressure, expiratory pressure, cough peak flow, and peak expiratory flow than supine and rotated positions. This position was rated subjectively easiest for coughing. Respiratory muscle strength showed moderate correlations with sarcopenia-related indices. Inspiratory muscle strength correlated with % predicted value forced vital capacity (%FVC; r = 0.509), quadriceps strength (r = 0.502), cough peak flow (r = 0.420), and handgrip strength (r = 0.371). CONCLUSIONS: The 60° semi-recumbent position optimised respiratory muscle force and cough effectiveness in older women, suggesting clinical implications for aspiration pneumonia prevention strategies. The link between respiratory and limb muscle indices possibly supported the concept of sarcopenia in respiratory muscles, suggesting its integration into approaches to pneumonia prevention and management.

    Aspiration pneumonia exacerbates swallowing and respiratory muscle atrophy. It induces respiratory muscle atrophy through three steps: proinflammatory cytokine production, caspase-3 and calpain, and then ubiquitin-proteasome activations. In addition, autophagy induces swallowing muscle atrophy. Nrf2 is the central detoxifying and antioxidant gene whose function in aspiration pneumonia is unclear. We explored the role of Nrf2 in aspiration pneumonia by examining swallowing and respiratory muscle mass and function using wild-type and Nrf2-knockout mice. Pepsin and lipopolysaccharide aspiration challenges caused aspiration pneumonia. The swallowing (digastric muscles) and respiratory (diaphragm) muscles were isolated. Quantitative RT-PCR and Western blotting were used to assess their proteolysis cascade. Pathological and videofluoroscopic examinations evaluated atrophy and swallowing function, respectively. Nrf2-knockouts showed exacerbated aspiration pneumonia compared with wild-types. Nrf2-knockouts exhibited more persistent and intense proinflammatory cytokine elevation than wild-types. In both mice, the challenge activated calpains and caspase-3 in the diaphragm but not in the digastric muscles. The digastric muscles showed extended autophagy activation in Nrf2-knockouts compared to wild-types. The diaphragms exhibited autophagy activation only in Nrf2-knockouts. Nrf2-knockouts showed worsened muscle atrophies and swallowing function compared with wild-types. Thus, activation of Nrf2 may alleviate inflammation, muscle atrophy, and function in aspiration pneumonia, a major health problem for the aging population, and may become a therapeutic target.

    Abstract <p>At the age of university students, it is important to accomplish the developmental tasks of adolescents. Furthermore, in addition to acquiring specialized knowledge and accomplishing developmental tasks, athletic colleges also require young athletes to be active. Therefore, we decided to investigate the use of a smartphone application, me-fullness® as an unprecedented method of support for female college students currently enrolled in athletic colleges. We conducted a psychological survey of female university students at the International Pacific University in Okayama, Japan, before and after one month of use of a me-fullness® app (N = 125) and a group of non-users (N = 85). The app-using group showed significant improvement in depressive symptoms, anxiety symptoms and stress scores as indicated by the Depression, Anxiety and Stress Scale − 21 Items (DASS-21), as well as insomnia, Premenstrual Syndrome (PMS) and resilience scores, while the non-app-using group showed improvement in anxiety and resilience (by Wilcoxon signed-rank test). This study suggests that use of the me-fullness app may have improved depression, stress, insomnia, and resilience. We hope that this study will lead to better college life and sports performance for female students in athletic universities and colleges.</p>

    Abstract Background Low‐intensity continuous inspiratory muscle training improves its strength. The abdominal muscles are the main expiratory muscles, and their training may improve expiratory muscle strength. Respiratory muscle strength regulates coughing effectiveness, which is critical for pneumonia management. In older people, risk factors for the development of pneumonia were respiratory muscle weakness and swallowing impairment. Currently, the impact of high‐intensity intermittent inspiratory and abdominal muscle combined training on the respiratory, swallowing, and systemic muscles is unclear. Objective We aimed to explore the effects of high‐intensity inspiratory muscle training combined with or without abdominal muscle training on respiratory muscle strength as well as the strength, mass, and performance of swallowing and systemic muscles. Methods Twenty‐eight healthy adults were divided into two groups. Participants performed high‐intensity intermittent inspiratory muscle single or its combination with abdominal muscle training for 4 weeks. Respiratory muscle strength, swallowing muscle strength and mass, systemic muscle strength, mass and performance were measured at baseline, Week 2 and Week 4. Results Both groups showed greater maximal respiratory pressures at Week 2 and Week 4 than baseline. Both groups showed improved tongue pressure and geniohyoid muscle thickness at Week 4. In addition, the combined training group improved body trunk muscle mass, handgrip strength and five‐time chair stand test, whereas the single training group did not. Conclusion This study revealed that high‐intensity inspiratory muscle training improved inspiratory muscle strength and swallowing muscle strength and mass. Moreover, inspiratory and abdominal muscle combined training showed an additional benefit of improving systemic muscle strength, mass and performance.     BACKGROUND: Streptococcal toxic shock syndrome (STSS) is a notifiable disease under Japan's Infectious Disease Control Law and has become a pandemic following COVID-19. STSS often leads to necrotizing fasciitis, with a mortality rate exceeding 30%. Even in surviving patients, limb amputations are common. CASE: A 39-year-old woman developed STSS. She underwent 18 surgeries and vacuum-assisted closure therapy, which resulted in ultra-short right transfemoral amputation. With a strong desire to walk again, she began fitting for a provisional prosthesis 3 months post-amputation. Given the preserved hip muscle strength, an ultra-short transfemoral prosthesis was selected over a hip disarticulation prosthesis. The key components included a plug-in quadrilateral socket, a belt used to suspend the transfemoral prosthesis, a hydraulic knee joint, and an energy-storing foot. She regained walking ability using crutches. To address pain and skin issues in the stump load-bearing area, compression and adhesion were improved using thick fabric spats. Muscle mass, including that of the paraspinal muscles, was maintained during follow-up evaluations using computed tomography, dual-energy X-ray absorptiometry, and bioelectrical impedance analysis. The phantom limb pain in the right leg diminished with medication and prosthetic training. Her quality-of-life scores measured using the 36-item Short Form Questionnaire and the Prosthesis Evaluation Questionnaire also showed improvement. She was discharged home 5 months post-amputation. DISCUSSION: This case highlights the importance of high motivation, multidisciplinary collaboration, preservation of the trunk muscle reserve from pre-illness exercise habits, and early trunk rehabilitation to achieve successful gait acquisition with a customized transfemoral prosthesis.

    BACKGROUND: Hospitalization often leads to a decline in activities of daily living (ADL) in older patients with heart failure. Although cardiac rehabilitation (CR) improves ADL, it can be difficult to perform CR due to the deconditioning of these patients. This study aimed to examine the factors associated with ADL at discharge in older patients with heart failure who underwent CR. METHODS: A total of 86 of 110 older heart failure patients aged ≥ 75 years (average age, 86.9 ± 5.7 years) transferred to our institution for CR were enrolled and classified into high ADL at discharge (n = 54) and low ADL at discharge (n = 32) groups. Physical characteristics, comorbidities, medications, blood test data, echocardiographic data, and nutritional status (Geriatric Nutritional Risk Index [GNRI]) were retrospectively examined from medical records. ADL were assessed using the Barthel Index (BI) at admission and discharge. Considering multicollinearity, the relationship between high ADL (BI ≥ 60) at discharge and these assessments at admission was analyzed using multiple logistic regression analysis. The receiver operating characteristic curve was analyzed to calculate the cutoff values for the parameters identified by the multiple logistic regression analysis. RESULTS: The GNRI was the only independent factor predicting high ADL at discharge (p = 0.041; odds ratio [OR], 1.125; 95% confidence interval [CI], 1.005-1.260). The area under the receiver operating characteristic curve for the GNRI was 0.770 (95% CI, 0.664-0.876). The cutoff value for the GNRI was 83.4 (sensitivity, 85.2%; specificity, 62.5%). CONCLUSION: These findings suggest that the GNRI score at admission predicts high ADL at discharge in older patients with heart failure who underwent CR.

    BACKGROUND: Body positions affect swallowing and gastroesophageal reflux. Swallowing impairment is one of the main causes of aspiration pneumonia. To prevent pneumonia, evaluation of body positions on gastroesophageal reflux recommended 30 degrees or higher semi-recumbent positions. The geniohypoid muscle and tongue play central roles in swallowing. However, the effects of body positions on contracting rates in the geniohyoid muscle and tongue pressure are unclear. Moreover, correlations between geniohyoid muscle contracting rates and subjective swallowing difficulties are unclear. AIMS: This study aimed to identify the proper body positions on contracting rates in the geniohyoid muscle, tongue pressure, and subjective swallowing difficulties. MATERIALS & METHODS: Twenty healthy adults swallowed 15- or 50 ml of water at 90 degrees sitting, 60- and 30 degrees semi-recumbent, and 0 degrees supine positions. We scored the subjective swallowing difficulties and measured the tongue pressure and the number of swallows. An ultrasound evaluated the geniohyoid muscle size and contracting rates. RESULTS: At sitting and 60 degrees semi-recumbent positions, the geniohyoid muscle showed greter contracting rates than at 30 degrees semi-recumbent and supine postions (P < 0.05), which resulted in easier swalloiwng. Greater tongue pressure was weakly correlated with fewer swallows (r = -0.339, P = 0.002), whereas the body positions did not affect. CONCLUSION: Considering swallowing and gastroesophageal reflux together, a trunk angle of 60 degrees or more might be beneficial for reducing the risk of aspiration.

    Inflammatory pleuritis often causes pleural effusions, which are drained through lymphatic vessels (lymphatics) in the parietal pleura. The distribution of button- and zipper-like endothelial junctions can identify the subtypes of lymphatics, the initial, pre-collecting, and collecting lymphatics. Vascular endothelial growth factor receptor (VEGFR)-3 and its ligands VEGF-C/D are crucial lymphangiogenic factors. Currently, in the pleura covering the chest walls, the anatomy of the lymphatics and connecting networks of blood vessels are incompletely understood. Moreover, their pathological and functional plasticity under inflammation and the effects of VEGFR inhibition are unclear. This study aimed to learn the above-unanswered questions and immunostained mouse chest walls as whole-mount specimens. Confocal microscopic images and their 3-dimensional reconstruction analyzed the vasculatures. Repeated intra-pleural cavity lipopolysaccharide challenge induced pleuritis, which was also treated with VEGFR inhibition. Levels of vascular-related factors were evaluated by quantitative real-time polymerase chain reaction. We observed the initial lymphatics in the intercostals, collecting lymphatics under the ribs, and pre-collecting lymphatics connecting both. Arteries branched into capillaries and gathered into veins from the cranial to the caudal side. Lymphatics and blood vessels were in different layers with an adjacent distribution of the lymphatic layer to the pleural cavity. Inflammatory pleuritis elevated expression levels of VEGF-C/D and angiopoietin-2, induced lymphangiogenesis and blood vessel remodeling, and disorganized the lymphatic structures and subtypes. The disorganized lymphatics showed large sheet-like structures with many branches and holes inside. Such lymphatics were abundant in zipper-like endothelial junctions with some button-like junctions. The blood vessels were tortuous and had various diameters and complex networks. Stratified layers of lymphatics and blood vessels were disorganized, with impaired drainage function. VEGFR inhibition partially maintained their structures and drainage function. These findings demonstrate anatomy and pathological changes of the vasculatures in the parietal pleura and their potential as a novel therapeutic target.

    Impaired % predicted value forced vital capacity (% FVC) is related to higher all-cause mortality in aged adults, and strong muscle force may improve this relationship. A muscle disease, sarcopenia, causes higher mortality. We aimed to identify the unknown disease that relates impaired % FVC with higher mortality in aged adults among the three major leading causes of death, and the effect of strong leg force on this relationship. Cox proportional hazard model analyzed the longitudinal Tsurugaya cohort that registered 1048 aged Japanese for 11 years. The primary outcome was the relationship between % FVC and mortality by cancer, cardiovascular disease, or pneumonia. Exposure variables were % FVC or leg force divided by 80% or median values, respectively. The secondary outcome was the effects of leg force on the relationship. Among the diseases, % FVC < 80% was related only to higher pneumonia mortality (hazard ratio [HR], 4.09; 95% CI, 1.90-8.83) relative to the % FVC ≥ 80% group before adjustment. Adding the leg force as an explanatory variable reduced the HR to 3.34 (1.54-7.25). Weak leg force might indicate sarcopenia, and its prevention may improve higher pneumonia mortality risk related to impaired % FVC, which we may advise people in clinical settings.

    Abstract After a natural disaster, it is important to maintain the health of the victims, but after the Turkey-Syria earthquake, this information has not yet been provided. Tohoku University has compiled information on maintaining health care after the Great East Japan Earthquake of March 2011 into a booklet, which is available to the public. The purpose of this study is to investigate user satisfaction with this booklet. A total of 505 organizations participated in this study by completing questionnaires and evaluating the booklet. Quantitative responses were analyzed and coded by theme. Open-ended responses indicated that it was difficult for the general public to understand the data in the booklet. This study shows the need to keep in mind the ease of understanding for the general public when providing information on post-disaster care. We hope that this study will lead to better health care information for the victims of the Turkey-Syria earthquake.

    The lymphatic vessels in the parietal pleura drain fluids. Impaired drainage function and excessive fluid entry in the pleural cavity accumulate effusion. The rat diaphragmatic lymphatics drain fluids from the pleura to the muscle layer. Lymphatic subtypes are characterized by the major distribution of discontinuous button-like endothelial junctions (buttons) in initial lymphatics and continuous zipper-like junctions (zippers) in the collecting lymphatics. Inflammation replaced buttons with zippers in tracheal lymphatics. In the mouse diaphragm, the structural relationship between the lymphatics and blood vessels, the presence of lymphatics in the muscle layer, and the distributions of initial and collecting lymphatics are unclear. Moreover, the endothelial junctional alterations and effects of vascular endothelial growth factor receptor (VEGFR) inhibition under pleural inflammation are unclear. We subjected the whole-mount mouse diaphragms to immunohistochemistry. The lymphatics and blood vessels were distributed in different layers of the pleural membrane. Major lymphatic subtypes were initial lymphatics in the pleura and collecting lymphatics in the muscle layer. Chronic pleural inflammation disorganized the stratified layers of the lymphatics and blood vessels and replaced buttons with zippers in the pleural lymphatics, which impaired drainage function. VEGFR inhibition under inflammation maintained the vascular structures and drainage function. In addition, VEGFR inhibition maintained the lymphatic endothelial junctions and reduced the blood vessel permeability under inflammation. These findings may provide new targets for managing pleural effusions caused by inflammation, such as pleuritis and empyema, which are common pneumonia comorbidities.

    Generally, weak muscle power is associated with high mortality. We aimed to evaluate the unknown association between % predicted value forced vital capacity (FVC% predicted) and mortality in asymptomatic older people, and the impact of muscle power on this association. We analyzed the Tsurugaya cohort that enrolled Japanese people aged ≥70 for 15 years with Cox proportional hazards model. Exposure variables were FVC% predicted and leg power. The outcome was all-cause mortality. The subjects were divided into quartiles by FVC% predicted or leg power, or into two groups by 80% for FVC% predicted or by the strongest 25% for leg power. Across 985 subjects, 262 died. The males with lower FVC% predicted exhibited higher mortality risks. The hazard ratio (HR) was 2.03 (95% CI 1.30-3.18) at the lowest relative to the highest groups. The addition of leg power reduced the HR to 1.78 (95% CI 1.12-2.80). In females, FVC% predicted under 80% was a risk factor and the HR was 1.67 (95% CI 1.05-2.64) without the effect of leg power. In FVC% predicted &lt;80% males HRs were 2.44 (95% CI 1.48-4.02) in weak and 1.38 (95% CI 0.52-3.64) in strong leg power males, relative to ≥80% and strong leg power males. Low FVC% predicted was associated with high mortality with potential unfavorable effects of weak leg power in males.

    Deafferentation induced by local anesthesia causes a larger perceived area than the real area of the mouth, which, in the perspective of body representation, belongs to implicit body representation. In this study, we applied repetitive peripheral magnetic stimulation (rPMS) on the motor branch of the radial nerve of participants’ non-dominant-side forearm to induce extension movements of wrist and fingers. This intervention was supposed to increase proprioception to the brain and had an enlargement effect on implicit body representation of the hand in our hypothesis. A total of 39 participants were randomly allocated to the real rPMS group (n = 19) or the sham rPMS group (n = 20). Implicit representation of the hand was measured by a simplified paradigm based on the proposal of Longo and Haggard that depicted perceived locations of fingertips and metacarpophalangeal joints of participants’ occluded hand, in which they showed that implicit body representation of the hand was smaller than the real hand. We compare the main effect of real rPMS vs. sham rPMS and its interaction effect with time by setting four timepoints—before stimulation, right after stimulation, 10 min after stimulation and 20 min after stimulation—to demonstrate the possible short-lasting effect. Results showed that real rPMS had a short-lasting enlargement effect on implicit representation of the hand in general, which was significant especially on the ulnar side of fingers. What is more, the enlarged implicit body representation of the hand was still within the boundary of a real hand, which might indicate the identification role of a real body part.

    Four patients with secondary lower limb lymphedema developed cellulitis at their lymphedema lesion following COVID-19 mRNA vaccinations. They did not develop adverse effects at their vaccination site. All the patients were Japanese females aged &lt;60 years. Three patients developed cellulitis following the first vaccination. The date of onset of cellulitis following the first vaccination varied from 0 to 21 days. Two received BNT162b2 mRNA vaccines and the others received mRNA-1273 vaccines. All the patients were treated with oral antibiotics and recovered. Two patients had repeated cellulitis. The patients with the repeated development of cellulitis could not perform good skincare. One patient had joint contractures in their lower limbs and could not reach her lymphedema lesions, and the other patient could not master the skincare. According to previous studies, the development of cellulitis following vaccination was rare. In this study, four patients aged &lt;60 years developed cellulitis among the eight patients that regularly visited our hospital for rehabilitation for their lower limb lymphedema. In patients with lymphedema, prolonged inflammation may impair lymphatic functions and worsen edema. Therefore, at the time of vaccination, we should keep in mind the prevention and immediate management of cellulitis using intensive skincare and antibiotic treatment.

    Introduction: The respiratory muscle strength regulates the effectiveness of coughing, which clears the airways and protects people from pneumonia. Sarcopenia is an aging-related loss of muscle mass and function, the worsening of which is associated with malnutrition. The loss of respiratory and swallowing muscle strength occurs with aging, but its effect on pneumonia is unclear. This study aimed to determine the risks of respiratory muscle weakness on the onset and relapse of pneumonia in older people in conjunction with other muscle-related factors such as malnutrition. Methods: We conducted a longitudinal study with 47 pneumonia inpatients and 35 non-pneumonia controls aged 70 years and older. We evaluated the strength of respiratory and swallowing muscles, muscle mass, and malnutrition (assessed by serum albumin levels and somatic fat) during admission and confirmed pneumonia relapse within 6 months. The maximal inspiratory and expiratory pressures determined the respiratory muscle strength. Swallowing muscle strength was evaluated by tongue pressure. Bioelectrical impedance analysis was used to evaluate the muscle and fat mass. Results: The respiratory muscle strength, body trunk muscle mass, serum albumin level, somatic fat mass, and tongue pressure were significantly lower in pneumonia patients than in controls. Risk factors for the onset of pneumonia were low inspiratory respiratory muscle strength (odds ratio [OR], 6.85; 95% confidence interval [CI], 1.56–30.11), low body trunk muscle mass divided by height2 (OR, 6.86; 95% CI, 1.49–31.65), and low serum albumin level (OR, 5.46; 95% CI, 1.51–19.79). For the relapse of pneumonia, low somatic fat mass divided by height2 was a risk factor (OR, 20.10; 95% CI, 2.10–192.42). Discussion/Conclusions: Respiratory muscle weakness, lower body trunk muscle mass, and malnutrition were risk factors for the onset of pneumonia in older people. For the relapse of pneumonia, malnutrition was a risk factor.

    BACKGROUND: Pneumonia is a common cause of illness and death of the elderly in Japan. Its prevalence is escalating globally with the aging of population. To describe the latest trends in pneumonia hospitalizations, especially aspiration pneumonia (AP) cases, we assessed the clinical records of pneumonia patients admitted to core acute care hospitals in Miyagi prefecture, Japan. METHODS: A retrospective multi-institutional joint research was conducted for hospitalized pneumonia patients aged ≥20 years from January 2019 to December 2019. Clinical data of patients were collected from the medical records of eight acute care hospitals. RESULTS: Out of the 1,800 patients included in this study, 79% of the hospitalized pneumonia patients were aged above 70 years. The most common age group was in the 80s. The ratio of AP to total pneumonia cases increased with age, and 692 out of 1,800 patients had AP. In univariate analysis, these patients had significantly older ages, lower body mass index (BMI), a lower ratio of normal diet intake and homestay before hospitalization, along with more AP recurrences and comorbidities. During hospitalization, AP patients had extended fasting periods, more swallowing assessments and interventions, longer hospitalization, and higher in-hospital mortality rate than non-AP patients. A total of 7% and 2% AP patients underwent video endoscopy and video fluorography respectively. In multivariate analysis, lower BMI, lower C-reactive protein, a lower ratio of homestay before hospitalization, a higher complication rate of cerebrovascular disease, dementia, and neuromuscular disease were noted as a characteristic of AP patients. Swallowing interventions were performed for 51% of the AP patients who had been hospitalized for more than two weeks. In univariate analysis, swallowing intervention improved in-hospital mortality. Lower AP recurrence before hospitalization and a lower ratio of homestay before hospitalization were indicated as characteristics of AP patients of the swallowing intervention group from multivariate analysis. Change in dietary pattern from normal to modified diet was observed more frequently in the swallowing intervention group. CONCLUSION: AP accounts for 38.4% of all pneumonia cases in acute care hospitals in Northern Japan. The use of swallowing evaluations and interventions, which may reduce the risk of dysphagia and may associate with lowering mortality in AP patients, is still not widespread.

    Pneumonia is a major cause of death in older people, and the number of such deaths is increasing. Present guidelines for pneumonia management are based on a pathogen-oriented strategy that relies on the optimal application of antibiotics. Older pneumonia inpatients show the high incidence of aspiration pneumonia. The main cause of aspiration pneumonia is an impairment in the swallowing and cough reflexes. These facts suggest a limitation of present management strategies and a requirement for new strategies for aspiration pneumonia. Sarcopenia is the loss of muscle strength and mass, and declining physical function with aging. Recently, a decrease in the mass or strength of the swallowing muscles was suggested to be associated with reduced swallowing function. Accordingly, dysphagia caused by sarcopenia of the systemic and swallowing-related muscles was named sarcopenic dysphagia. Presently, few studies have shown associations between aspiration pneumonia and sarcopenic dysphagia. As for the cough reflex, strong cough prevents aspiration pneumonia, and its strength is regulated by respiratory muscles. A few studies have reported a relationship between muscles and pneumonia in older people. Sarcopenia is a risk factor for pneumonia in older people, and aspiration pneumonia inpatients with low muscle mass show high mortality rates. Aspiration pneumonia induced muscle atrophy in respiratory, swallowing, and skeletal muscles in an animal model and humans. Associations between respiratory muscle strength and pneumonia are currently under investigation. Evaluation and management of sarcopenia could potentially become a new strategy to prevent and treat pneumonia in older patients, and research has only recently been launched.     Tumor-associated blood vessels and lymphatics are abnormal and dysfunctional. These are hallmarks of the tumor microenvironment, which has an immunosuppressive nature, such as through hypoxia. Treatment with anti-death receptor5 (DR5) monoclonal antibody MD5-1, which induces tumor cell death, is a potent anti-tumor immunotherapy. Generally, MD5-1 induces cell death mainly via antigen presenting cells (APCs) and generates tumor-specific effector T cells. To date, the effects of a simultaneous functional improvement of abnormal blood vessels and lymphatics on the immune microenvironment are largely unknown. A combination therapy using sunitinib, vascular endothelial growth factor (VEGF) and platelet-derived growth factor receptor inhibitor, and MD5-1 substantially inhibited tumor growth. Sunitinib improved pericyte coverage on endothelial cells and the expression levels of regulator of G-protein signaling 5, suggesting blood vessel normalization. Sunitinib also increased lymph flow from tumors to central lymph nodes, suggesting improved lymphatic function. In concordance with improved vasculature functions, sunitinib alleviated the tumor hypoxia, suggesting an improved tumor microenvironment. Indeed, the combination therapy induced strong activation of CD8+ T cells and dendritic cells in draining lymph nodes. The combination therapy reduced the ratio of immune-suppressive T regulatory cells in the tumors and draining lymph nodes. The combination therapy enhanced the numbers and activation of tumor-infiltrating CD8+ T cells. CD4 and/or CD8 depletion, or APC inhibiting experiments showed the contribution of CD8+ T cells and APCs to the combination therapy. These findings suggest that targeting blood vessels and lymphatics may have potential benefits for immunotherapy mediated by CD8+ T cells and APCs.

    NEW FINDINGS: What is the central question of this study? Does Toll-like receptor 7 (TLR7) have any direct effects on Ca2+ -dependent physiological function of tracheal submucosal gland cells? What is the main finding and its importance? TLR7 is co-localized with SERCA2 in tracheal submucosal gland cells and causes a rapid attenuation of acetylcholine (ACh)-induced, Ca2+ -dependent ionic currents through the activation of SERCA2-dependent Ca2+ clearance. TLR7 is abundantly expressed in the airways of both swine and healthy human subjects, but is significantly downregulated in chronic obstructive pulmonary disease (COPD) airways. These findings suggest that a dysfunction of TLR7 in COPD removes the brake on ACh-induced serous secretion during viral infections, resulting in prolonged airway hypersecretion, and that it is one of the triggers of COPD exacerbations. ABSTRACT: Airway surface fluids are mainly secreted from submucosal glands (SMGs) and play important roles in the defence of airways via the activation of mucociliary transport. Toll-like receptor 7 (TLR7) recognizes and eliminates single stranded RNA (ssRNA) viruses through the induction of innate immunity. However, there is no obvious connection between TLR7 and mucus secretion, aside from TLR7 recognizing ssRNA viruses, which are often associated with airway hypersecretion in chronic obstructive pulmonary disease (COPD). Here, we investigated whether TLR7 has any direct effects on the Ca2+ -dependent physiological function of tracheal SMG cells. Patch-clamp analyses revealed that TLR7 ligand inhibited the acetylcholine (ACh)-induced ionic currents in isolated tracheal SMG cells. Intracellular calcium assays and pharmacological analyses revealed that TLR7 attenuated the transient rises in the intracellular calcium concentration evoked by ACh by activating sarco/endoplasmic reticulum Ca2+ -ATPase 2 (SERCA2). Immunofluorescence staining and immunohistochemical staining revealed that TLR7 was co-localized with SERCA2. These findings suggest that the activation of TLR7 during viral infections contributes to the rapid attenuation of ACh-induced ionic currents through an increase in SERCA2-dependent Ca2+ clearance in healthy airway SMG cells. Our study also revealed that TLR7 expression was significantly downregulated in COPD airways. Based on these findings, we speculate that a dysfunction of TLR7 may not only have an adverse effect on the elimination of these viruses but also remove the brake on ACh-induced serous secretion, resulting in prolonged hypersecretion and acting as one of the triggers of COPD exacerbations.

    BACKGROUND: Repetition of the onset of aspiration pneumonia in aged patients is common and causes chronic inflammation. The inflammation induces proinflammatory cytokine production and atrophy in the muscles. The proinflammatory cytokines induce muscle proteolysis by activating calpains and caspase-3, followed by further degradation by the ubiquitin-proteasome system. Autophagy is another pathway of muscle atrophy. However, little is known about the relationship between aspiration pneumonia and muscle. For swallowing muscles, it is not clear whether they produce cytokines. The main objective of this study was to determine whether aspiration pneumonia induces muscle atrophy in the respiratory (the diaphragm), skeletal (the tibialis anterior, TA), and swallowing (the tongue) systems, and their possible mechanisms. METHODS: We employed a mouse aspiration pneumonia model and computed tomography (CT) scans of aged pneumonia patients. To induce aspiration pneumonia, mice were inoculated with low dose pepsin and lipopolysaccharide solution intra-nasally 5 days a week. The diaphragm, TA, and tongue were isolated, and total RNA, proteins, and frozen sections were stored. Quantitative real-time polymerase chain reaction determined the expression levels of proinflammatory cytokines, muscle E3 ubiquitin ligases, and autophagy related genes. Western blot analysis determined the activation of the muscle proteolysis pathway. Frozen sections determined the presence of muscle atrophy. CT scans were used to evaluate the muscle atrophy in aged aspiration pneumonia patients. RESULTS: The aspiration challenge enhanced the expression levels of proinflammatory cytokines in the diaphragm, TA, and tongue. Among muscle proteolysis pathways, the aspiration challenge activated caspase-3 in all the three muscles examined, whereas calpains were activated in the diaphragm and the TA but not in the tongue. Activation of the ubiquitin-proteasome system was detected in all the three muscles examined. The aspiration challenge activated autophagy in the TA and the tongue, whereas weak or little activation was detected in the diaphragm. The aspiration challenge resulted in a greater proportion of smaller myofibers than in controls in the diaphragm, TA, and tongue, suggesting muscle atrophy. CT scans clearly showed that aspiration pneumonia was followed by muscle atrophy in aged patients. CONCLUSIONS: Aspiration pneumonia induced muscle atrophy in the respiratory, skeletal, and swallowing systems in a preclinical animal model and in human patients. Diaphragmatic atrophy may weaken the force of cough to expectorate sputum or mis-swallowed contents. Skeletal muscle atrophy may cause secondary sarcopenia. The atrophy of swallowing muscles may weaken the swallowing function. Thus, muscle atrophy could become a new therapeutic target of aspiration pneumonia.

    Lung cancer accompanied by somatic activating mutations in the epidermal growth factor receptor (EGFR) gene, which is associated with a significant clinical response to the targeted therapy, is frequently found in never-smoking Asian women with adenocarcinoma. Although this implies genetic factors underlying the carcinogenesis, the etiology remains unclear. To gain insight into the pathogenic mechanisms, we sequenced the exomes in the peripheral-blood DNA from six siblings, four affected and two unaffected siblings, of a family with familial EGFR-mutant lung adenocarcinoma. We identified a heterozygous missense mutation in MET proto-oncogene, p.Asn375Lys, in all four affected siblings. Combined with somatic loss of heterozygosity for MET, the higher allele frequency in a Japanese sequencing database supports a causative role of the MET mutation in EGFR-mutant lung cancer. Functional assays showed that the mutation reduces the binding affinity of MET for its ligand, hepatocyte growth factor, and damages the subsequent cellular processes, including proliferation, clonogenicity, motility and tumorigenicity. The MET mutation was further observed to abrogate the ERBB3-mediated AKT signal transduction, which is shared downstream by EGFR. These findings provide an etiological view that the MET mutation is involved in the pathogenesis of EGFR-mutant lung cancer because it generates oncogenic stress that induces compensatory EGFR activation. The identification of MET in a family with familial EGFR-mutant lung cancer is insightful to explore the pathogenic mechanism of not only familial, but also sporadic EGFR-mutant lung cancer by underscoring MET-related signaling molecules.

    Erythropoietin (Epo), a known hematopoietic growth factor, has been reported to promote tumor growth and angiogenesis in Epo receptor (EpoR)-positive tumors, but its effects on EpoR-negative tumors have not been clearly shown. Here, we show that Epo accelerates the growth of EpoR-negative tumors by promoting tumor angiogenesis. Mice were inoculated with Lewis lung carcinoma cells and treated with Epo. Erythropoietin accelerated tumor growth and increased intratumoral microvessel density, although it did not accelerate Lewis lung carcinoma cell tumor proliferation in vitro. To observe the direct effect of Epo on endothelial cells, we examined human dermal microvascular endothelial cells (HMVECs) that expressed EpoR. Erythropoietin induced the proliferation of HMVECs and protected them from H2O2-induced cell death. Erythropoietin activated the extracellular signal-regulated kinase signaling pathway and up-regulated the expression of the downstream antiapoptotic protein Bcl-xL in HMVECs. Moreover, in both the absence and presence of tumors, in vivo treatment of mice with Epo increased circulating endothelial progenitor cells. To investigate the role of Epo in a primary tumor model, we inoculated the chemical carcinogen methylcholanthrene (MCA) subcutaneously into mice at two doses, a high or a low dose, which induced fibrosarcoma, and treated them with Epo. Erythropoietin promoted tumor growth after MCA inoculation at both doses and decreased the overall survival of the mice inoculated with the high-dose MCA. However, Epo did not increase the incidence of fibrosarcoma at either dose. Lewis lung carcinoma cells and MCA-induced fibrosarcomas did not express EpoR. These results suggest that Epo accelerates the growth of tumors that lack EpoR expression by promoting tumor angiogenesis.

    74歳男。発熱、呼吸困難を主訴とした。10日前に腹部大動脈破裂に対しY-graft置換術を施行されていた。胸部CTにて左上葉の浸潤影の悪化を認め、喀痰培養にてメチシリン耐性黄色ブドウ球菌を検出したため、院内肺炎を疑った。メロペネム・テイコプラニンの抗菌薬にて加療したが改善を認めず、気管支鏡検査を施行した結果、気管支肺胞洗浄液にて好酸球比率が82%と増加し、検鏡にて真菌(糸状菌)を認めた。真菌培養にて接合菌が培養陽性となり、遺伝子検査からCunninghamellla bertholletiaeと同定され、好酸球性肺炎と診断した。抗真菌薬とプレドニゾロンにて治療を開始したが敗血症性ショックの病像を呈し、第39病日に死亡した。病理解剖では両肺の上葉を主体として広く壊死巣が観察され、病巣内には糸状菌を多数認めた。また、心筋、甲状腺、腎臓、脾臓においても同様に糸状菌塊と炎症性細胞浸潤を認め、播種性浸潤性接合菌症と最終診断した。

    症例は51歳男性で、以前より左上葉に空洞性病変を指摘されていた。タイヤを持ち上げた際に腰痛を自覚し、その後、徐々に歩行困難となった。CT、MRIで脊椎への浸潤を伴う左上葉空洞影を認めた。両下肢に不全麻痺および深部感覚低下、腱反射低下を認めた。軽度閉塞性換気障害を認めた。両下肢の麻痺が進行し、排尿障害などの症状も出現したため、確定診断および除圧を目的として胸椎(Th2〜Th4)椎弓切除術を施行した。検体の病理組織学的検査により酵母様真菌を検出し、肺真菌症・脊椎浸潤(硬膜内髄外膿瘍)と診断した。術後よりアムホテリシンBを開始したが、腎機能障害のためホスフルコナゾールに変更した。治療開始後30日目に施行したCTでは脊椎へ浸潤する左上葉空洞影は縮小し、両下肢麻痺も徐々に改善した。フルコナゾールに変更し、杖歩行可能な状態まで改善し、排尿も自己導尿で対応可能となった。治療開始後56日目にリハビリ目的に転院した。

    1)入院数は1995年が185名,1996年が151名であった. 2)手術件数は1995年が130件,1996年が110件であった.麻酔別では全麻下の手術が両年とも95%以上で殆どを示した. 3)年齢別では例年と同じで多い順に幼児,乳児,学童,新生児となった.新生児は1995年が6例で9件の手術を行った.1996年は4例,7件の手術であった. 4)疾患では鼠径ヘルニアが最も多くついで虫垂炎であり,両疾患で全手術件数の約75%を占めていた.3位は1995年が鎖肛と肥厚性幽門狭窄症で各々2件,1996年はHirschsprung病で3件あった. 5)年齢別に疾患を多い順に示した

    当院で昭和61年より平成8年までに経験した25例の入院小児熱傷患児についてまとめた.年齢分布は7ヵ月から11歳で平均年齢は3.1歳だった.うち2歳以下が69%を占めた.重症度分類をBurn Indexに基づいて行うと0以上5未満が12例48%, 5以上10未満が4例16%, 10以上15未満が同じく4例16%, 15以上が5例20%であった.重症患者の受傷後24時間の輸液量をSchreinerの公式と比較すると,80%の患児でほぼ一致した値が得られたが,Burn Index 45の重症々例では2倍以上の輸液量を要した.投与コロイド量において同公式と比べ,平均で4.11倍と大きな解離を認めた

    1) 鶴ケ谷コホートで癌等の3大死因のうち低肺活量群の肺炎による死亡リスク増大を示した。本コホートは2002と2003年に呼吸機能や脚力等を調べ11年間死因を追跡した。癌、心血管疾患、肺炎、の3大死因のうち低肺活量群の肺炎による死亡リスク増大を示し強い脚力による死亡リスク低下を示した。そして論文として報告した. Journal of Clinical Medicine May12(9), 3272. 2023 <BR> 2) 呼吸筋力増強の肺炎再発予防効果を確認するため、高齢肺炎症例の前向き縦断介入研究を実施している。このため石巻市斉藤病院の入院肺炎症例(70歳以上)を対象としてエントリーしている。 <BR> 3) 胸膜脈管系の正常解剖や膿胸による変化を観察するためマウス胸壁胸膜可視化モデルで脈管の解剖やリンパ管の種類を各々の標識抗体群で免疫染色している。これまで共焦点顕微鏡で胸壁胸膜を撮影し3次元画像解析ソフトで立体構造を同定した。また胸膜炎を発症させリンパ管や血管の新生、リモデリングを確認した。これらを論文として報告した。 Ren Y, Okazaki T 他 Anatomy and function of the lymphatic vessels in the parietal pleura and their plasticity under inflammation in mice.     In this study, we identified a previously unknown normal anatomy of lymphatics and blood vessels in the pleura covering the mouse chest wall and diaphragm. We immuno-stained the chest wall and diaphragm with markers for lymphatics and blood vessels. We observed and identified and their anatomy using a microscope. Lymphatics include initial and collecting lymphatics. We identified their distribution in the pleura. We also confirmed the distribution of blood vessel networks of arteries, veins, and capillaries in the pleura. Next, we developed a disseminated metastatic mouse tumor model on the pleura of the chest wall and diaphragm. We immuno-stained this model with lymphatic and blood vessel markers. We were able to clearly observe the flowing out lymphatics and flowing in blood vessels of each tumor using a microscope. In this model, each tumor was small, and lymphangiogenesis and angiogenesis from the surrounding vasculatures to the tumor could be identified.

    Pleural effusion is produced by blood vessels in the pleura and drained from lymphatics, but when the balance is lost, it accumulates. Causes include inflammation and cancer, but current treatments are still poorly managed. There are a few reports of blood vessels such as vascular permeability for effusion, but there are few reports of lymphatics, which are the center of effusion drainage. In this study, we focused on the lymphatics of the pleura and investigated whether they could be therapeutic targets. We created a cancerous pleural effusion model in which lung cancer cells were administered into the thoracic cavity and an inflammatory pleural effusion mouse in which a bacterial toxin was administered. Evaluation of the lymphatics in the pleura of each model revealed dilation of the lymphatic vessel diameter. The cancerous pleural effusion model tended to delay pleural effusion drainage. Lymphangiogenesis inhibitors tended to reduce pleural effusion.

    歩行中に計算などの課題を同時に行う二重課題歩行は、認知及び運動機能のパフォーマンスの低下を引き起こすことが知られており、前頭前野脳血流を測定することで歩行中の注意機能を評価することができる。本研究では二重課題歩行中に近赤外線分光法装置(near-infrared spectroscopy : NIRS)を用い前頭前野脳血流測定と加速度計を用いた歩行解析を行い、脳血流変化と認知及び歩行変動との関連の検討を行うこととした。 2020年度には二重課題歩行中の運動機能と認知機能の研究に関するプロトコルを作成し、東北大学医学系研究科倫理委員会承認を受け、20名の被験者に対して二重課題歩行中の脳血流測定と加速度計の歩行データの取得を完了した。そこで本年度においては、まずこれらのデータ解析を行い、その結果に基づいて新たな計測プロトコールを検討することとしていた。脳血流についての解析では、前頭前野の位置因子とコンディション因子の2元配置分散分析を用い、分散分析の事後検定としてボンフェローニ法で多重比較検定を行い、加速度については、X軸、Y軸、Z軸の値を二乗平均平方根として算出した合計値を求め、各群における平地歩行条件とウォーキングマシーン歩行条件でt検定を行うことを予定していた。

    VEGFR-2 in angiogenesis and VEGFR-3 in lymphangiogenesis are the key players. An anti-DR-5 Ab, the apoptosis inducing antibody, has antitumor effects. Here, we combined anti-DR-5 Ab with VEGFR1-3 inhibitor sunitinib as anti-angiogenic and lymphangiogenic therapies. Mice were injected with 4T1 cells into the footpad and treated with sunitinib and/or anti-DR-5 Ab. The tumors and popliteal lymph nodes were isolated. The growth rate of 4T1 or CT26 tumors was measured. Sunitinib decreased the blood vessel and hypoxic area densities in the tumors, suggesting blood vessel normalization. Sunitinib decreased the amount of tumor injected evans blue and hyaluronic acid in tumors, suggesting better lymphatic flow. The combination increased the ratio of activated dendritic cells and CD8+ T cells in lymph nodes and the numbers of CD8+ and effector CD4+ T cells in tumors compared to controls. The combination decreased the tumor growth rate compared to each mono-therapy.

    Lymphangiogenesis occurs in binding to VEGFR3 mainly expressed in lymphatic vessels of their ligand,VEGF-C or VEGF-D. It has been reported that macrophages and skeletal muscle is source of VEGF-C,-D production. In the present study, we examined wheather M-CSF induce the VECF-C,-D production. VEGF-C, D production induced from mouse macrophages was not observed by administration M-CSF. But in skeletal muscle, diaphragm, and myocardium, we confirmed that the expression levels of VEGF-C,-D were significantly higher.

    Previous studies did not show chronic inflammation or vascular changes in aspiration pneumonia or its animal models. We established a mouse model of aspiration pneumonia, and showed chronic inflammation, blood vessel remodeling, and lymphangiogenesis in the model and in patients.
