    INTRODUCTION: Dialysis patients often develop advanced knee osteoarthritis, necessitating knee arthroplasty. Although total knee arthroplasty is commonly performed, limited data exist on perioperative complications among Japanese dialysis patients. This study uses a Japanese nationwide medical claims database to investigate the incidence of perioperative complications and mortality in dialysis versus non-dialysis patients undergoing knee arthroplasty. METHODS: A retrospective analysis was conducted using data from a Japanese nationwide database (April 2016 to March 2023). Propensity score matching was used to balance confounding variables, including age, sex, body mass index, surgical procedures, and comorbidities. A 1:1 propensity score-matched cohort of 1,923 dialysis and 1,923 non-dialysis patients was analyzed. Outcomes included in-hospital mortality, surgical site infections, and other complications. Multivariate logistic regression identified risk factors. RESULTS: Dialysis patients exhibited significantly higher in-hospital mortality and surgical site infection risks. The odds ratio for surgical site infection in dialysis patients was 2.17 (95% CI [confidence interval]: 1.31 to 3.61). The dialysis group demonstrated a significantly higher in-hospital mortality risk compared to the non-dialysis group (P < 0.0001). No in-hospital deaths were observed among non-dialysis patients post-matching, precluding the calculation of an odds ratio. Bilateral simultaneous surgery emerged as an additional independent risk factor, with an odds ratio of 9.94 (95% CI: 1.95 to 50.74) in dialysis patients. CONCLUSION: This propensity score-matched study highlights significantly elevated surgical site infection and in-hospital mortality risks for dialysis patients undergoing knee arthroplasty. These findings underscore the necessity for careful perioperative management and the avoidance of high-risk procedures, such as bilateral simultaneous surgeries, in dialysis patients.

    Dialysis patients who develop degenerative hip disease or femoral neck fractures may require total hip arthroplasty, and their comorbidities predispose them to complications. This study aimed to evaluate whether dialysis was associated with early postoperative complications using a large database of Japanese. In this cohort study, using the Japanese National Administrative Diagnosis Procedure Combination database on THA for patients on hemodialysis or not from December 2011 to March 2023, we assessed the surgical-related complications, medical complications, and mortality during hospitalization after propensity score matching by age, sex, BMI, and comorbidities. A total of 2,111 pairs of patients on hemodialysis and non-dialysis were included. In THA for patients on hemodialysis, the significant odds ratios for various complications were as follows: dislocation (2.616, 95% CI: 1.282 to 5.338, p < 0.01), reoperation (2.104, 95% CI: 1.222 to 3.623, p < 0.01), deep vein thrombosis (0.407, 95% CI: 0.286 to 0.579, p < 0.01), cerebrovascular events (4.426, 95% CI: 1.495 to 13.10, p < 0.01). These findings help identify postoperative THA risks for patients on dialysis, suggesting that more attention should be paid to preoperative planning and postoperative care.

    <jats:title>Abstract</jats:title> <jats:sec> <jats:title>Introduction</jats:title> <jats:p>Type 2 diabetes is associated with an increased risk of fragility fractures, even in individuals with normal or high bone mineral density. However, the impact of type 2 diabetes on postoperative outcomes after hip fracture surgery in elderly Japanese patients remains unclear. This study evaluated the association between type 2 diabetes and postoperative complications, including in-hospital mortality, using a nationwide database in Japan.</jats:p> </jats:sec> <jats:sec> <jats:title>Materials and methods</jats:title> <jats:p>A retrospective cohort study was conducted using the Diagnosis Procedure Combination (DPC) database from April 2016 to March 2022. Patients aged ≥ 65 years who underwent hip fracture surgery were included. Propensity score matching (1:1) was performed to adjust for confounders. Logistic regression analyses were used to assess associations between type 2 diabetes and outcomes.</jats:p> </jats:sec> <jats:sec> <jats:title>Results</jats:title> <jats:p>Of the 474,293 eligible patients included in this study, 18.5% were identified as having comorbid type 2 diabetes. Following 1:1 propensity score matching, the final analytic cohorts each comprised 83,283 patients. Although statistically significant, the presence of type 2 diabetes was associated with only modest increases in the risks of postoperative myocardial infarction (risk difference [RD]: 0.0007), cognitive dysfunction (RD: 0.0029), and in-hospital mortality (RD: 0.0045), with all comparisons yielding p-values of less than 0.0001. Additionally, the length of hospital stay was longer among patients with type 2 diabetes.</jats:p> </jats:sec> <jats:sec> <jats:title>Conclusions</jats:title> <jats:p>Although the absolute risk differences were small, type 2 diabetes remains an independent risk factor for adverse postoperative outcomes following hip fracture surgery in elderly Japanese patients. Tailored perioperative strategies may help optimize outcomes in this vulnerable population.</jats:p> </jats:sec>

    INTRODUCTION: Coronavirus disease 2019 (COVID-19) is a worldwide pandemic, and mortality increases in elderly patients with comorbidities. This study aims to examine the in-hospital complications and mortality for elderly patients with proximal femur fractures during the COVID-19 pandemic or countermeasure periods. MATERIALS AND METHODS: The proximal femur fractures undergoing surgery during the COVID-19 pandemic or countermeasure period were compared with the pre-pandemic in a Japanese national inpatient data. The assessed outcomes were the development of pneumonia, deep vein thrombosis (DVT), pulmonary embolism (PE), and mortality during hospitalization in two periods, and those for COVID-19-positive patients. RESULTS: A total of 284,922 proximal femur fractures aged over 65 years were included. In the COVID-19 pandemic period compared to the pre-pandemic, the odds of pneumonia, DVT, PE, and mortality decreased to 0.942 (95% confidence interval [CI]: 0.901 - 0.986, P = 0.0102) and 0.839 (95% CI: 0.745 - 0.946, P = 0.004), increased to 1.153 (95% CI: 1.112 - 1.195, P < 0.001), and 1.048 (95% CI: 0.982 - 1.118, P = 0.1554), respectively. For COVID-19 positivity at admission, the odds of PE increased significantly to 12.95 (95% CI: 8.795 - 19.06, P < 0.001). For COVID-19 positivity during hospitalization, the odds of pneumonia and mortality were increased to 2.896 (95% CI: 1.820 - 4.608, P < 0.001) and 6.303 (95% CI: 3.440 - 11.55, P < 0.001), respectively. CONCLUSION: These findings alert healthcare professionals and patients to the elevated complications, especially PE rate for proximal femoral fracture with COVID-19 positive.

    INTRODUCTION: Total knee arthroplasty (TKA) is a widely used treatment for advanced knee osteoarthritis. While cemented fixation is the standard technique, cementless fixation has demonstrated comparable long-term outcomes. However, the association between cement use and postoperative complications, including venous thromboembolism and surgical site infections, remains unclear, particularly in Japanese patients. This study aimed to investigate the incidence of postoperative complications in cemented and cementless TKA using a nationwide database. METHODS: A nationwide cohort study was conducted using Japan's DPC database from April 2016 to March 2023. Patients who underwent TKA were identified, and postoperative complications, including deep vein thrombosis, pulmonary embolism, pneumonia, cerebrovascular events, postoperative cognitive dysfunction, and surgical site infection, were analyzed. One-to-one propensity score (PS) matching was performed based on age, sex, body mass index, type of anesthesia, simultaneous bilateral surgery, Charlson comorbidity index, and comorbidities to ensure comparability. Statistical analyses included χ² tests, Student's t-tests, and multivariate logistic regression analysis. RESULTS: A total of 228,595 patients met the eligibility criteria, with 21,906 matched pairs in the cemented and cementless groups after PS matching. The incidence of deep vein thrombosis (OR: 1.231, 95% CI: 1.151-1.316, p < 0.0001) and surgical site infection (OR: 1.716, 95% CI: 1.420-2.073, p < 0.0001) was significantly higher in the cemented group. No significant differences were observed in pulmonary embolism or other complications. CONCLUSION: Cement application was associated with an increased risk of deep vein thrombosis and surgical site infection. These findings suggest that careful perioperative management may be warranted in patients undergoing cemented TKA.

    <jats:p> Titanium (Ti) and its alloys are widely used in orthopedic and dental implants owing to their high biocompatibility with tissues, low toxicity, and excellent mechanical properties, such as high strength, fatigue strength, and corrosion resistance. Total hip arthroplasty (THA) is predicted to rise from1.8 million in 2015 to 2.8 million in 2050, and the demand for Ti-based THA is also increasing. The biocompatibility of Ti originates from the several-nanometer-thick oxide layer present on its surface, which inhibits the redox reactions. The oxide forms spontaneously on the surface upon exposure to air and stays in thermodynamic equilibrium; however, it is easily disrupted by the interfacial shear stress owing to the low wear resistance of Ti. Ti exposed to corrosive body fluids elutes metal ions, generating wear debris in the biological fluids and tissues. This causes injury and disease, incites allergies, and promotes the formation of granulomas and even carcinomas. Furthermore, poor osseointegration due to poor adhesion with adjacent bone causes the loosening of the implant-bone interface and slows the healing process. To overcome these drawbacks of implant Ti materials, surface modifications using biocompatible TiO <jats:sub>2</jats:sub> are expected for imparting biofunctions such as osseointegration, antivirus activity, and tribocorrosion. Although various methods have been studied for the fabrication of TiO <jats:sub>2</jats:sub> on Ti alloys, anodic oxidation has attracted considerable attention owing to its advantages. This review aims to provide a comprehensive, evidence-based overview of current studies on the osseointegration, antimicrobial properties, and cytotoxicity of surface-modified implant Ti alloys, in addition to a brief introduction to different metallic biomaterials. </jats:p>

    INTRODUCTION: Advances in pharmacological treatments have reduced joint deformities in rheumatoid arthritis (RA), leading to a decline in total knee arthroplasty (TKA) among RA patients. However, RA remains associated with higher risks of postoperative complications. This study compares postoperative complications during hospitalization in patients with RA and osteoarthritis (OA) undergoing TKA in Japan. METHODS: This retrospective cohort study utilized data from the Japanese Diagnosis Procedure Combination database from April 2016 to March 2023. Patients who underwent TKA were identified, and propensity score (PS) matching was performed to balance age, sex, body mass index, simultaneous surgeries, and comorbidities, resulting in 9048 matched pairs. Outcomes included periprosthetic joint infections (PJI), cognitive dysfunction, deep vein thrombosis (DVT), pulmonary embolism (PE), and periprosthetic fractures. Statistical analyses were conducted using multivariate logistic regression with a significance threshold of p < 0.01. RESULTS: RA patients had higher risks of PJI (odds ratio [OR]: 1.473, 95% CI: 1.134-1.912, p = 0.004) and postoperative cognitive dysfunction (OR: 1.721, 95% CI: 1.190-2.488, p = 0.004) compared to OA patients. In contrast, no significant differences were observed in the incidence of DVT, PE, or periprosthetic fractures. CONCLUSION: RA patients undergoing TKA are at increased risk of PJI and cognitive dysfunction, highlighting the need for tailored perioperative management. These findings provide important insights into optimizing outcomes for RA patients.

    This study investigated whether octacalcium phosphate (OCP) enhances bone regeneration through its synergistic effect with stromal-derived factor-1 (SDF-1). Recombinant SDF-1 (0.5-5.0 μg) was combined with OCP granules through lyophilization. OCP/SDF-1 granules were implanted into a rat femoral standardized defect for 2 and 4 weeks and subjected to histomorphometry, C-X-C motif chemokine receptor 4 (CXCR4) and osteocalcin immunohistomorphometry, and tartrate-resistant acid phosphatase (TRAP) staining. Calcium-deficient hydroxyapatite (CDHA) was used as a control for in vitro analyses. Mesenchymal stem cell (MSC) migration was estimated using a Transwell system with OCP/SDF-1. SDF-1 release from OCP/SDF-1 into the supernatant was determined without cells. SDF-1 adsorption in 4-(2-hydroxyethyl)-1-piperazineethanesulfonic acid buffer onto OCP, the chemical structure of OCP immersed in the medium using Fourier transform infrared spectroscopy, and the degree of supersaturation of the medium were determined. Bone regeneration and OCP degradation were enhanced the most by 1.0 μg of OCP/SDF-1 at 2 weeks after implantation by CT analysis and increasing CXCR4-positive, osteocalcin-positive, and TRAP-positive cells accumulation around the OCP. MSC migration increased until 48 h in the following order: SDF-1 only, CDHA/SDF-1, and OCP/SDF-1, with the greatest effect with 1.0 μg of SDF-1 than from OCP. CDHA promoted a greater release than OCP at 48 h. The physicochemical analyses indicated that SDF-1 interacted with OCP through Freundlich-type adsorption and that the adsorption controlled SDF-1 release from OCP during the hydrolysis into CDHA. Therefore, leveraging its molecular affinity for the OCP surface, OCP/SDF-1 facilitates MSC migration and enhances bone formation by ensuring the controlled, sustained release of SDF-1 from OCP.

    AIM: The effectiveness of early surgery in preventing complications in elderly Japanese hip fracture patients and the impact of weekend hospitalization need further investigation. The purpose of this study was to determine whether weekend hospitalization affects the incidence of various sequelae and death during hospitalization in elderly hip fracture patients using a comprehensive Japanese hip fracture case database. METHODS: We retrospectively analyzed the Japanese National Administrative DPC (Diagnosis Procedure Combination) database from April 2016 to March 2022. During this period, approximately 1100 DPC-affiliated hospitals consistently provided medical records with consent for the study. The study focused on weekend hospitalizations and investigated the associations with postoperative pneumonia, pulmonary embolism, myocardial infarction, urinary tract infection, acute renal dysfunction, dementia, and in-hospital mortality after propensity score matching. Owing to the large population size of the study, significance levels were strictly enforced, and a P-value < 0.001 was considered statistically significant. RESULTS: After performing propensity score matching based on age, sex, and comorbidities, 111 035 patient pairs were identified, comparing those admitted on weekends versus weekdays. The analysis showed no heightened risk of sequelae for those admitted during the weekend compared with weekdays. Additionally, there was a slight trend toward higher mortality risk during weekend hospital stays; however, the increase was insignificant, with a hazard ratio of 1.071 (95% confidence interval: 1.005-1.140, P = 0.03). CONCLUSION: The results of this study indicate that weekend hospitalization for elderly patients with hip fractures is not definitively associated with an increase in various sequelae or in-hospital mortality and that the importance of early surgery for elderly patients with hip fractures may be recognized and promoted in Japan.     BACKGROUND: This study investigated the impact of total knee arthroplasty (TKA) on the alignment of the ankle joint, focusing on changes in coronal alignment of both ankles. METHODS: This retrospective study, conducted at our hospital, included 64 patients older than 70 years who underwent TKA for varus knee osteoarthritis (OA). Using full-length standing radiographs of both lower extremities taken preoperatively and 6 months postoperatively, we assessed ankle OA grading based on the Takakura-Tanaka classification and measured mechanical hip-knee-ankle (mHKA) and the tibiotalar angle (TTA) on both the surgical and contralateral side. Additionally, we examined the impact of ankle OA on postoperative clinical scores, the Knee injury and Osteoarthritis Outcome Score, following TKA. RESULTS: Based on the classification, ankle OA stage 2 or higher was present in 13 cases on the surgical side and 13 cases on the contralateral side. On the surgical side, mHKA changed from 12.2 to 1.8 degrees varus (P < .001) and TTA from 1.7 to 1.0 degrees varus (P = .002). On the contralateral side, mHKA changed from 4.0 to 5.4 degrees varus (P = .04), and TTA from 2.7 to 3.0 degrees varus (P < .001). In ankle OA stage 2 or higher patients, contralateral TTA changed from 5.2 to 6.5 degrees varus (P < .001) and unchanged in stage 1 patients. Ankle OA stage 2 or higher patients had worse ADL outcomes than stage 1 patients (score: 67.2 and 78.3, respectively, P = .03). CONCLUSION: This study suggests an apparent interrelationship between knee and ankle OA and the potential negative influence of TKA on ankle OA.

    OBJECTIVES: Rheumatoid arthritis (RA) is a common autoimmune disorder characterized by chronic inflammation and periarticular bone loss, leading to systemic osteoporosis and heightened fracture susceptibility, especially hip fractures among the elderly. This study aimed to evaluate the outcomes and complications associated with hip fractures in patients with RA relative to those without RA. METHODS: Using the Japanese National Administrative Diagnosis Procedure Combination (DPC) database, we examined cases of femoral neck fractures from April 2016 to March 2023. After one to three propensity score matching for age, sex, and complications, we examined the association between RA, complications, and mortality during hospitalization in elderly patients with hip fractures. RESULTS: The findings revealed that elderly Japanese RA patients with hip fractures had significantly higher complications of pneumonia than elderly hip fracture patients without RA, with a ratio of 1.232 (95% CI: 1.065-1.426, p=0.0056), and pulmonary embolism, with a ratio of 1.155 (95% CI: 1.036-1.287, p=0.009) in multivariate logistic regression analyses. Although not significantly elevated, it also found a trend toward higher mortality during hospitalization, with a ratio of 1.179 (95% CI: 0.973-1.429, p=0.096). CONCLUSIONS: A substantial study based on the Japanese DPC database revealed a significant association between RA and increased complications, including pneumonia and pulmonary embolism. On the other hand, there was a nonsignificant but higher trend for risk of mortality during hospitalization for hip fracture in elderly RA patients. Implementing preventive strategies is essential to minimizing complications in the treatment of hip fractures in patients with RA.

    Abstract Objective Spondyloarthritis encompasses conditions such as ankylosing spondylitis (AS) and psoriatic arthritis (PsA). Advanced axial spondyloarthritis causes significant spinal fusion, affecting daily activities. Recent therapeutic agents have enhanced the control of inflammation, yet they do not consistently stop the axial progression. This study aimed to identify factors influencing the progression of axial lesions over a two-year period in Japanese patients with axial spondyloarthritis. Methods This retrospective and cross-sectional study included 47 axial spondyloarthritis patients. Spinal lesions were assessed using the modified Stoke Ankylosing Spondylitis Spine Score (mSASSS). Patients were categorized into progressive and non-progressive groups based on changes in their mSASSS scores. Various clinical parameters were analyzed for correlation with ankylosis progression. Results The study found no significant correlation between ankylosis progression and traditional factors, including the level of inflammation or the use of biologics. However, modified Health Assessment Questionnaire (mHAQ) scores were associated with disease progression. The baseline mSASSS was another significant factor, underscoring the importance of early detection and management. Conclusion This study showed that patient-reported outcomes and baseline mSASSS scores are crucial for assessing axial spondyloarthritis progression. This underscores the need for a comprehensive treatment strategy that addresses both clinical indicators and patient-reported outcomes.

This study aims to use a comprehensive Japanese hip fracture case database to determine whether surgery within the day of admission and the following day reduces the incidence of complications and mortality during hospitalization in elderly hip fracture patients. MATERIALS AND METHODS: We retrospectively analyzed the Japanese National Administrative DPC (Diagnosis Procedure Combination) database from April 2016 to March 2022. Approximately 1100 DPC-affiliated hospitals consistently provided medical records with consent for research. The study investigated the association between postoperative pneumonia, deep vein thrombosis, pulmonary embolism, and mortality during hospitalization after propensity score matching, focusing on surgeries conducted on the day of admission and the following day. RESULTS: After one-to-one propensity score matching for age, gender, and comorbidity, we identified 146,441 pairs of patients who underwent surgery either within the day of admission and the following day or after the third day of admission. Surgery on the third day or later was independently associated with increased risks of pneumonia, deep vein thrombosis, pulmonary embolism, and mortality during hospitalization with risk ratios of 1.367 (95% CI 1.307-1.426), 1.328 (95% CI 1.169-1.508), 1.338 (95% CI 1.289-1.388), and 1.167 (95% CI 1.103-1.234), respectively. CONCLUSION: A comprehensive study of elderly Japanese patients with hip fractures in the DPC database showed that surgery on admission and the following day is crucial for preventing complications like pneumonia, deep vein thrombosis, pulmonary embolism, and mortality during hospitalization.

    Abstract Purpose The acetabular coverage in osteonecrosis of the femoral head (ONFH) affects the need for surgical intervention, and the collapse of the femoral head remains unclear. This study aimed to evaluate the relation between the acetabular coverage and the need for surgical treatment and femoral head collapse. Methods The study included 158 patients with 252 hips with glucocorticoid administration and idiopathic ONHF without osteoarthritis changes. The mean age at the first visit was 45.2 years, and the mean follow-up period was 92.2 months. All ONFH hips were subsequently divided into two groups: those needing surgical intervention and those without surgery. Additionally, it divided 167 initially non-collapsed hips into those that either later collapsed or not. Radiographic parameters with the centre-edge angle, acetabular roof obliquity, sharp angle, and necrotic location, following the guidelines of the Japanese Investigation Committee, were evaluated. Results There were no significant differences in radiographic parameters between the 106 hips that underwent surgery and the 146 hips without surgery. Among the 167 hips without initial collapse, 91 eventually collapsed while 76 did not; their radiographic findings have no significant differences. The necrotic locations were significantly larger in hips requiring surgical intervention or femoral head collapse. Furthermore, 21.8% (55 out of 252 hips) had acetabular dysplasia, which did not significantly correlate with the necessity for surgical treatment or the incidence of femoral head collapse. Conclusions Acetabular coverage has little effect on the necessity for surgical treatment and femoral head collapse in ONFH patients over a long-term follow-up.

    Pyoderma gangrenosum (PG) is a rare inflammatory skin disease characterized by skin ulcers that are associated with autoimmune diseases. Although the effectiveness of immunosuppression with glucocorticoids and tumor necrosis factor inhibitors in treating PG has been reported, the utility of negative-pressure wound therapy (NPWT) for severe ulcerative lesions in patients with PG remains controversial. Herein, we report the case of a 76-year-old woman with rheumatoid arthritis who developed PG after undergoing surgery for a forefoot deformity. The patient showed improvement in deep ulcer lesions through NPWT while receiving treatment with abatacept and systemic glucocorticoids. Subsequent topical glucocorticoid therapy led to the remission of the PG. This case suggests that NPWT, when used under immunosuppressive conditions, does not exacerbate the pathergy and may be beneficial for treating severe ulcerative PG.

    Abstract The photocatalytic properties of anodic oxides on a newly developed TiNbSn and commonly used Ti6Al4V alloys as biomaterials were investigated. The alloys were anodized in an electrolyte of sodium tartrate acid with H2O2 at a high voltage and the mechanism of the photocatalytic and antiviral activities was studied. The anodized TiNbSn and Ti6Al4V exhibited highly crystallized rutile TiO2 and poorly crystallized anatase TiO2, respectively. X-ray photoelectron spectroscopy analysis revealed the presence of oxides of the alloying elements in addition to TiO2. The anodized TiNbSn exhibited higher activities than Ti6Al4V, and electron spin resonance spectra indicated that the number of hydroxyl radicals (⋅OH) generated from the anodized TiNbSn was higher than that from the anodized Ti6Al4V. The results can be explained by two possible mechanisms: the higher crystallinity of TiO2 on TiNbSn than that on the Ti6Al4V reduces the number of charge recombination sites and generates abundant ⋅OH; charge separation in the anodic oxide on TiNbSn due to the electronic band structure between TiO2 and the oxides of alloying elements enhances photo activities. The excellent photoinduced characteristics of the anodized TiNbSn are expected to contribute to the safe and reliable implant treatment.

    Abstract Objective This study aimed to analyse the radiological characteristics and clinical diversity of Japanese patients with synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome, a heterogeneous disorder. Methods Radiographs and clinical information from 115 Japanese patients (female/male: 81/34, mean age at onset: 48.7 years) diagnosed with SAPHO syndrome between January 2007 and December 2020 were retrospectively reviewed. Additionally, the treatment for SAPHO syndrome was explored. Results Among the 115 patients, 70 patients had complications, including palmoplantar pustulosis, acne, or psoriasis. Imaging studies included bone scintigraphy, magnetic resonance imaging, computed tomography, and positron emission tomography in 71, 58, 70, and 23 patients, respectively. The most frequent lesions were arthritis and hyperostosis of the sternoclavicular joints in 96 patients; spinal lesions, including sacroiliac arthritis were observed in 85 patients. Peripheral aseptic osteitis was observed in 22 patients, and the tibia was involved in 12. The treatments consisted of analgesics, bisphosphonates, conventional synthetic disease-modifying anti-rheumatic drugs, and biologics (tumour necrosis factor inhibitors and interleukin-23p19 inhibitors) in 85, 15, 23, and 10 patients (8 and 2 patients), respectively. Conclusion Sternoclavicular hyperostosis and pustulosis are frequently observed in patients with SAPHO syndrome. Biological agents were more frequently used in patients with peripheral osteitis and arthritis.

    Abstract Background The difference between Young’s moduli of the femur and the stem causes stress shielding (SS). TiNbSn (TNS) stem has a low Young’s modulus and strength with gradient functional properties during the change in elastic modulus with heat treatment. The aim of this study was to investigate the inhibitory effect of TNS stems on SS and their clinical outcomes compared to conventional stems. Methods This study was a clinical trial. Primary THA was performed using a TNS stem from April 2016 to September 2017 for patients in the TNS group. Unilateral THA was performed using a Ti6Al4V alloy stem from January 2007 to February 2011 for patients in the control group. The TNS and Ti6Al4V stems were matched in shape. Radiographs were obtained at the 1- and 3-year follow-ups. Two surgeons independently checked the SS grade and appearance of cortical hypertrophy (CH). The Japanese Orthopaedic Association (JOA) scores before and 1 year after surgery were assessed as clinical scores. Results None of the patients in the TNS group had grade 3 or 4 SS. In contrast, in the control group, 24% and 40% of patients had grade 3 and 4 SS at the 1- and 3-year follow-ups, respectively. The SS grade was lower in the TNS group than in the control group at the 1- and 3-year follow-ups (p &lt; 0.001). The frequencies of CH in both groups were no significant difference at the 1- and 3-year follow-ups. The JOA scores of the TNS group significantly improved at 1 year after surgery and were comparable to control group. Conclusion The TNS stem reduced SS at 1 and 3 years after THA compared to the proximal-engaging cementless stem, although the shapes of the stems matched. The TNS stem could reduce SS, stem loosening, and periprosthetic fractures. Trial registration: Current Controlled Trials. ISRCTN21241251. https://www.isrctn.com/search?q=21241251.     This study examined the association between developmental dysplasia of the hip (DDH) and disease-associated loci in a Japanese cohort. A genome-wide association study (GWAS) of 238 Japanese patients with DDH and 2044 healthy individuals was performed. As a replicate, GWAS was also conducted on the UK Biobank data with 3315 cases and matched 74,038 controls. Gene set enrichment analyses (GSEAs) of both the genetics and transcriptome of DDH were performed. Transcriptome analysis of cartilage specimens from DDH-associated osteoarthritis and femoral neck fractures was performed as a control. Most of the lead variants were very low-frequency ones in the UK, and variants in the Japanese GWAS could not be replicated with the UK GWAS. We assigned DDH-related candidate variants to 42 and 81 genes from the Japanese and UK GWASs, respectively, using functional mapping and annotation. GSEA of gene ontology, disease ontology, and canonical pathways identified the most enriched pathway to be the ferroptosis signaling pathway, both in the Japanese gene set as well as the Japanese and UK merged set. Transcriptome GSEA also identified significant downregulation of genes in the ferroptosis signaling pathway. Thus, the ferroptosis signaling pathway may be associated with the pathogenic mechanism of DDH.

    Dysregulation of osteoclasts, the multinucleated cells responsible for bone resorption, contributes to several degenerative bone disorders. Previously, we showed that blocking the leukocyte immunoglobulin (Ig)-like receptor B4 (LILRB4), a kind of inhibitory receptor that plays an important role in immune regulation, promotes osteoclast differentiation in vitro. Here, we explored whether gp49B, the murine ortholog of LILRB4, regulates osteoclastogenesis in vivo, and whether fibronectin (FN), a ligand of LILRB4/gp49B, certainly contributes to LILRB4/gp49B-mediated osteoclastogenesis. In comparison with wild-type mice, gp49B deficiency mice exhibited a loss of trabecular bone number and an increase in osteoclast formation. Gp49B knockout improved the bone resorptive capacity of osteoclasts derived from murine Raw264.7 cells by increasing osteoclast formation. We further revealed that gp49B deficiency increased the receptor activator of nuclear factor (NF)-κB ligand (RANKL)-induced signaling transduction by increasing the phosphorylation of TGF-activated kinase 1 (TAK1), NF-κB and mitogen-activated protein kinases (MAPKs). Furthermore, the N-terminal 30 kDa proteolytic fragments of FN promoted gp49B-mediated inhibition of osteoclastogenesis by increasing Src homology-2-containing tyrosine phosphatase 1 (SHP-1) phosphorylation and tumor necrosis factor receptor-associated factor 6 (TRAF6)-SHP-1 association. In summary, the FN-LILRB4/gp49B interaction negatively regulates RANKL-induced TRAF6/TAK1/NF-𝜅B/MAPK signaling in osteoclastogenesis.

    Titanium alloys are useful for application in orthopedic implants. However, complications, such as prosthetic infections and aseptic loosening, often occur after orthopedic devices are implanted. Therefore, innovation in surface modification techniques is essential to develop orthopedic materials with optimal properties at the biomaterial–bone interface. In this review, we present recent research on the improvement in the osteoconductivity and antibacterial effect of the Ti-33.6% Nb-4% Sn (TiNbSn) alloy by anodic oxidation and other related studies. TiNbSn alloys are excellent new titanium alloys with a low Young’s modulus, high tensile strength, and with gradient functional properties such as a thermally adjustable Young’s modulus and strength. Titanium dioxide (TiO2), when obtained by the anodic oxidation of a TiNbSn alloy, improves bone affinity and provides antibacterial performance owing to its photocatalytic activity. The safety of TiO2 and its strong bonding with metal materials make its method of preparation a promising alternative to conventional methods for improving the surface quality of orthopedic implants. Implementing anodization technology for TiNbSn alloys may alleviate orthopedic surgery-related complications, such as loosening, stress shielding, and infection after arthroplasty.

    Background and objectives: The superior placement of the acetabular cup induced the delayed recovery of abductor muscle moment after total hip arthroplasty (THA) with a conventional posterior approach. The anterior-based muscle-sparing (ABMS) THA effectively reduces soft tissue damage, including muscles. The influence of hip center position on anterior-based muscle-sparing (ABMS) total hip arthroplasty (THA) for post-operative hip muscle strength was unclear. We evaluate whether the hip center position affects the recovery of hip muscle strength after ABMS THA. Materials and Methods: The study was performed as a retrospective cohort study, and included 38 hips in 38 patients that underwent primary ABMS THA. Muscle strength was measured using isokinetic dynamometry before the operation, and at 6 and 12 months after surgery. The horizontal and vertical centers of rotation (H-COR and V-COR), vertical shift (V-shift), leg length, and global femoral offset were determined radiographically in reference to a previous report. Results: A weak negative correlation was observed between abduction muscle strength at 6 months and V-shift; a V-shift more than 15 mm demonstrated significantly decreased abductor muscle strength at 6 months. Conclusions: The superior placement of the hip center caused delayed recovery of abductor muscle strength in hips with anterolateral minimally invasive THA. There seems to exist no biomechanical reason why the same should not also be the case for the muscle-sparing approach.

    <jats:p>In this study, we anodized a TiNbSn alloy with low Young’s modulus in an electrolyte of sodium tartrate with and without hydrogen peroxide (H<jats:sub>2</jats:sub>O<jats:sub>2</jats:sub>). The photo-induced characteristics of the anodized alloy were analyzed for crystallinity and electrochemical conditions with comparisons to the effect with the addition of H<jats:sub>2</jats:sub>O<jats:sub>2</jats:sub>. The antibacterial activity was evaluated using methicillin-resistant <jats:italic>Staphylococcus aureus</jats:italic> and other pathogenic bacteria according to ISO 27447, and time decay antibacterial tests were also conducted. The anodized oxide had a porous microstructure with anatase- and rutile-structured titanium dioxide (TiO<jats:sub>2</jats:sub>). In contrast, the peaks of rutile-structured TiO<jats:sub>2</jats:sub> were accelerated in the anodized TiNbSn alloy with H<jats:sub>2</jats:sub>O<jats:sub>2</jats:sub>. The formation of hydroxyl radicals and methylene blue breaching performance under ultraviolet irradiation was confirmed in the anodic oxide on TiNbSn alloy with and without H<jats:sub>2</jats:sub>O<jats:sub>2</jats:sub>. The anodic oxide on TiNbSn alloy had a robust antibacterial activity, and no significant difference was detected with or without H<jats:sub>2</jats:sub>O<jats:sub>2</jats:sub>. We conclude that anodized TiNbSn alloy with sodium tartrate electrolyte may be a functional biomaterial with a low Young’s modulus and an antibacterial function.</jats:p>

    <jats:p>The primary cilium is a hair-like immotile organelle with specific membrane receptors, including the receptor of Hedgehog signaling, smoothened. The cilium organized in preosteoblasts promotes differentiation of the cells into osteoblasts (osteoblast differentiation) by mediating Hedgehog signaling to achieve bone formation. Notably, 4.1G is a plasma membrane-associated cytoskeletal protein that plays essential roles in various tissues, including the peripheral nervous system, testis, and retina. However, its function in the bone remains unexplored. In this study, we identified 4.1G expression in the bone. We found that, in the 4.1G-knockout mice, calcium deposits and primary cilium formation were suppressed in the trabecular bone, which is preosteoblast-rich region of the newborn tibia, indicating that 4.1G is a prerequisite for osteoblast differentiation by organizing the primary cilia in preosteoblasts. Next, we found that the primary cilium was elongated in the differentiating mouse preosteoblast cell line MC3T3-E1, whereas the knockdown of 4.1G suppressed its elongation. Moreover, 4.1G-knockdown suppressed the induction of the cilia-mediated Hedgehog signaling and subsequent osteoblast differentiation. These results demonstrate a new regulatory mechanism of 4.1G in bone formation that promotes the primary ciliogenesis in the differentiating preosteoblasts and induction of cilia-mediated osteoblast differentiation, resulting in bone formation at the newborn stage.</jats:p>

    Rheumatoid arthritis is characterized by multiple chronic arthritis subsequently inducing joint destruction. Although subchondral geode is a well-known feature of high-disease activity, a large geode is rare. Moreover, the treatment effect of biologic agents in the repair of large geode has not been reported. The present report shows the significant effect of interleukin-6 receptor blocker, tocilizumab, in repairing the large geode in the left humeral lateral epicondyle. This case implies that tocilizumab might be an effective treatment in patients with rheumatoid arthritis even with large geode.

    発育性股関節形成不全(developmental dysplasia of the hip: DDH)は日本人に好発する家族集積性の高い疾患である。また本邦では変形性股関節症の約80%はDDHを原因としている。変形性股関節症に対する人工股関節置換術は年間約10万件となり、増加傾向である。変形性股関節症の予防のためにDDHの病態解明は必須であるが、DDHの原因遺伝子は未解明である。当施設では日本人ゲノム解析ツールであるジャポニカアレイを開発している。日本人に最適化されたジャポニカアレイを用いたDDHの原因遺伝子を解明することが目的である。 先行してDDH患者230症例についてジャポニカアレイでの解析を既に行っている。今後DDH 患者200症例の追加解析を目標とする。ジャポニカアレイを用いて日本人に特異的な約66万箇所のSNPの中で、DDH発症と相関するSNPを有する遺伝子群を解析する。また、約30億塩基の全ゲノム構造をインピュテーションすることでDDH疾患感受性遺伝子の網羅的な探索を行う。健常群は東北大学東北メディカル・メガバンクの2947人のデータを利用予定である。p値が1x10-7以下の疾患感受性が疑われるSNPに関して,ジャポニカアレイ検体とは異なるDDH患者200症例のゲノムDNAを対象にして、Taqman probeを用いたリアルタイムPCRでレプリケーションスタディを行い、ジャポニカアレイで同定された新規DDH関連SNPの発現について検証する。 DDHの網羅的ゲノム解析で探索されたDDH疾患感受性遺伝子について、家族歴・環境因子、画像での重症度評価、手術治療などの臨床情報と相関解析を行うことで重症化関連遺伝子を解明する。

    Osteogenesis imperfecta is one of the designated intractable diseases, and no effective therapeutic method has been developed. Discovering the mechanisms of osteogenesis connects to the development of a novel therapeutic method. In the present study, I found that a membrane scaffold protein 4.1G contributed to the formation of primary cilia in the osteoprogenitor cells, resulting in potentiation of the ciliary hedgehog signaling and differentiation to osteoblasts. By using isolated tibia from 4.1G-knockout mice, I further found that 4.1G regulates osteogenesis in both males and females from the embryonic to neonatal period and in the only females at puberty.

    The suppression effect of stress shielding was investigated in an experimental hip joint prosthesis model using a low elastic modulus titanium alloy TiNbSn. Finite element analysis confirmed the improvement of load-stress imbalance, and the suppression of bone atrophy in the canine low elastic modulus titanium alloy artificial hip joint model was clarified. We also investigated the effect of low modulus titanium alloy TiNbSn on promoting bone healing by making a metal plate and examining CT images of actual bone tissue, hard tissue specimens, and bone strength after healing in a rabbit tibial osteotomy model. The effect of TiNbSn alloy on early bone healing and improvement of bone strength after healing was confirmed in comparison with Ti6Al4V alloy.

    Developmental dysplasia of the hip (DDH) is common in Japanese with a frequency of approximately 0.3%. About 80% of patients with osteoarthritis of the hips (OA) are caused by DDH. The pain and deformity of the hip joint make walking difficult and the activities of daily living are significantly impaired. Prevention of OA is important. The purpose of this study is to elucidate the causative gene of DDH using Japonica array, which is a genome-wide association analysis tool optimized for Japanese. The data of healthy subjects and those of DDH patients were compared. As a result of Japonica array analysis, 157 SNPs with a significantly high frequency in DDH were identified. In particular, it was suggested that the ubiquinol-cytochrome c reductase complex chaperone (UQCC) 1 gene is involved in the development of DDH in Japanese.

    In order to impart osteoconductivity to a low Young's modulus TiNbSn alloy which suppresses stress shielding, anodic oxidation was applied in a sulfuric acid electrolyte under high voltage application. The crystallinity of the titanium oxide formed on the surface was controlled under the electrolytic conditions, and the osteoconductivity was confirmed in the TiNbSn alloy with high crystallinity TiO2. The pull-out strength of anodized TiNbSn alloy increased significantly as compared with the untreated material after in vivo test, and it was considered that the adhesion with the TiNbSn alloy was strengthened by the new bone tissue formed on the TiO2. TEM-EDX analysis after in vitro test confirmed the penetration of large amounts of Ca and P into porous TiO2, and proposed a model that osteoconductivity is attributed to the bioactive TiO2 loading and penetration of bone constituent elements into porous TiO2.

    Total hip arthroplasty stems of Ti6Al4V alloy and TiNbSn alloy were produced with the same design. It is confirmed that the Young’s modulus of the TiNbSn alloy stem is 55 GPa. The modulus of elasticity of the Ti6Al4V alloy stem was 110 GPa. Total hip arthroplasties were performed on 10 kg male Beagle dogs. We plan to observe the bone formation around the stem by the CT image and evaluate histological analyses of bone tissue around the stems at 1 year after surgery. We compare the results of stress analyses using FEM with CT images and histological findings. We also assessed the effects of anodizing treatment of TiNbSn alloy on improvement of bone compatibility and of the low elasticity on bone union acceleration. In the rabbit tibia model and the mouse tibia model, improved bone synthesis in the fracture site was indicated in TiNbSn group compared with the stainless steel group or the Ti6Al4V group.

    Fracture healing is a complex biological process involving the proliferation of mesenchymal progenitor cells, and chondrogenic, osteogenic and angiogenic differentiation. Here, we demonstrate Dickkopf-related protein 3 (Dkk3) expression in periosteal cells using Dkk3-green fluorescent protein (Dkk3-GFP) reporter mice. We found that proliferation of mesenchymal progenitor cells began in the periosteum, involving Dkk3-positive cell proliferation near the fracture site. In addition, Dkk3 was expressed in fibrocartilage cells with and smooth muscle alpha actin and Col3.6 in the early phase of fracture healing as a cell marker of fibrocartilage cells. Dkk3 was not expressed in mature chondrogenic cells or osteogenic cells. Transient expression of Dkk3 had disappeared in the late phase of fracture healing except in the superficial periosteal area of fracture callus. The Dkk3 expression pattern differed in newly formed type IV collagen-positive blood vessels and the related avascular tissue.
