Development of subgingival calculus detector utilizing optical fiber: Verification of its potential for clinical application

The removal of subgingival deposits, especially calculus, plays a crucial role in basic periodontal therapy. However, manual detection methods affect accuracy owing to the operator’s skill. To avoid this uncertainty, we have developed a calculus detection device named “Sensor probe” and evaluated its ability to detect calculus for future clinical applications. The Sensor probe consisted of a 635 nm-wavelength semiconductor laser and a 0.5 mm-diameter single-mode optical fiber. Initially, the performance of the device was evaluated using clinically obtained extracted teeth with calculus covered with a stainless-steel shielding plate with pinhole. Then, the effect of the optical fiber’s end shape on calculus detection performance was analyzed. Lastly, the performance of the Sensor probe was compared to that of a conventional periodontal probe in terms of accuracy, sensitivity, and specificity for calculus detection using calculus-covered extracted teeth. The results indicated that Sensor probe detected dental calculus through the pinhole with a diameter of 300 μm or more when applied from a distance of 100 μm. The results analyzing the effect of the optical fiber’s end shape on calculus detection performance showed that cutting the fiber end at an angle of 45° resulted in the most effective calculus detection. This may be because the laser light refracted on the cut surface and concentrated on the fiber side. Moreover, by comparing the performance of this device to a conventional periodontal probe revealed that the Sensor probe showed improved calculus detection accuracy in deeper periodontal pockets. This improvement was particularly significant in the apical third, where detection is typically difficult. In conclusion, a Sensor probe that uses an optical fiber with a 45° angled end may facilitate subgingival calculus detection. In future clinical applications, Sensor probes could lead to more accurate and efficient calculus removal, especially for deeper periodontal pockets.

頭頸部領域の腫瘍、先天性疾患ならびに外傷を有する患者の実態調査について報告した。頭頸部腫瘍・先天性疾患・外傷を有する288例を対象とし、頭頸部腫瘍250例、嚢胞6例、先天性疾患28例、外傷3例であった。初診時年齢は、29歳以下が全体の8.3%、30〜59歳が全体の27.4%で、60歳代と70歳代は全体の55.5%を占めた。紹介元は、口腔外科100例、予防歯科80例、頭頸部外科52例であった。悪性腫瘍が232例、86.8%と圧倒的に多かった。上顎癌は74例(27.8%)、下顎癌は103例(38.7%)で、歯肉癌58例、骨肉腫1例、舌癌32例、口底癌12例、頬粘膜癌14例、中咽頭癌16例などであった。また、良性腫瘍は30例、嚢胞6例であった。

宮城県立精神医療センターに入院中の患者79名(男性46名、女性33名、平均54歳)を対象に、歯科検診・機能検査を行った。入院期間は平均2808日、疾患は統合失調症が87%と最も多く、薬剤服用者は92%、院内歯科受診者は20名(25%)であった。平均値で口腔清掃状態(PI値)は前歯部1.7、臼歯1.3、う蝕・歯周疾患(CPI値)は最大値1.9、歯周ポケット4mm以上が50.6%であり、現在歯数は17.2歯、喪失歯は13.1歯、健全歯数は7.6歯、未処置歯数は4.1歯、処置歯数は4.1歯、欠損補綴歯数は5.3歯、DMFT指数は22.6であったが、義歯装着者は4名のみであった。口腔乾燥度は26.6、咀嚼機能検査は91.8mg/dl、嚥下機能検査(RSST)は5.4、RSST:3回未満の陽性率は42.9%であった。精神疾患患者の口腔環境は劣悪であることが判明し、積極的な口腔ケア指導、摂食・嚥下指導の必要性が示唆された。

当院では2015年10月から多職種による摂食嚥下治療センターの運用を開始した。その概要は、初診時に耳鼻咽喉・頭頸部外科で嚥下内視鏡検査(VE)と嚥下造影検査(VF)を行うとともに、歯科で口腔内診査と口腔機能検査を行い、これらの検査結果についてリハビリテーション科医や言語聴覚士、看護師などが加わったカンファレンスにより検討し、多角的に評価する。今回、2016年7月までに同センターを受診した287例を対象に、「性別」「年齢」「原疾患」「義歯使用の有無」「補綴処置を要する欠損の有無」「VE所見」「VF所見」などについて調査した。性別は男性178例、女性109例、年齢は平均65.4±15.3歳であった。原疾患は頭頸部腫瘍が最も多く62%、次いで神経筋疾患9%、脳血管障害6%の順であった。義歯を使用していたのは32.0%で、補綴処置を要する欠損は35.6%が有していた。VE所見は、兵頭スコアで概ね問題なしとされるスコア4以下の症例が68.1%、誤嚥のリスクがあるものが29.5%、経口摂取不可と評価されたものが2.4%であった。VF所見は、AsRスコアから経口摂取のみでの栄養摂取が可能と評価されたものが35.1%、補助栄養を要するが直接訓練は可能とされたものが6.5%、直接訓練は避けるべきとされたものが58.4%、早期咽頭流入など口腔期に障害を認めたものが44.2%であった。

広範囲顎骨支持型装置と広範囲顎骨支持型補綴について検討した。「広範囲顎骨支持型装置および広範囲顎骨支持型補綴」を適用した患者および補綴装置を調査した。適用患者数は12例(男性7名、女性5名、平均56.9歳)であり、適用したインプラント部位は上顎4例、下顎7例、1例は上下顎に適用であった。良性腫瘍5例、悪性腫瘍6例、先天欠損1例で、放射線療法の既往ある者はいなかった。骨造成は床義歯形態に1例、ブリッジ形態に5例に行われた。床義歯形態は6例、7個、ブリッジ形態は6例、6個に適用された。埋入インプラント平均数は可撤性装置が4個、固定性装置が2.3個であった。全顎補綴装置数91個に対する割合は13%であった。悪性腫瘍以外の6例中5例で、骨造成PCMB(チタンメッシュ+腸骨海綿骨)が行われていた。

It is reported that Ti-Nb-Sn has low Young's modulus possessing around 40 GPa close to human cortical bone (30 GPa) and low cytotoxicity as well as commercially pure Ti (cpTi). However, the availability for dental implant is unknown. The purpose of this study was to investigate the biomechanical behavior, biocompatibility and bioactivity of Ti-Nb-Sn. As a result, Ti-Nb-Sn is considered to have sufficient biological potential and capacity for osseointegration.

In this study we have prepared a series of new gelatin/OCP complexes using OCP synthesized under specific conditions. After considering optimal material condition of gelatin/OCP composite, we evaluated the bone regeneration ability and bone maturity of the regenerative bone by using a rat calvaria critical-sized bone defect model. As a result, the utility as a bone substitute material of gelatin/OCP has been confirmed.

再建舌モールドについて、これまでに検討した素材のうち、着脱時に最もモデルの変形を生じにくかった素材について、複数種類の厚みのモールドを作製し、強度等の評価の継続を予定していたが、新型コロナウイルス感染症の拡大とそれに伴う対策強化により、研究に使用する材料の輸入や研究機材の購入、搬入、および被験者、業者との接触に対して制限が設けられ、実施に支障が生じた。感染症対策については継続が予想されるため、対策下でも研究継続を可能にするため、オンライン上でのモールドの性能検討のための手法や、被験者対応の手法について検討中である。

Since oral functional movements such as chewing and swallowing are performed in the closed state, it has been difficult to evaluate the actual accurate behavior. Especially in the maxillary prosthesis, which has a maxillary defect and is required good marginal sealing ability even compared with ordinary denture, development of a method to evaluate the behavior and sealing property of marginal soft tissue has been demanded it was. In this study, we analyzed the behavior of the marginal soft tissue in patients with maxillary prosthesis attached using a capsule endoscope, which is a wireless and compact image capturing device used for image evaluation of the small intestine. And we examined methods to clarify the aspect of marginal sealing by maxillary prosthesis. By analyzing the photographed images, several findings were obtained on the behaviors of the marginal soft tissues. In the future, by increasing the number of subjects, we aim to acquire more detailed and superior knowledge.
